ABSTRACT

Bupleuri Radix, serving as the sovereign medicinal in many antidepressant compound preparations, has been proved effective in treating depression in mice, but its effect on the intestinal flora remains unclear. The present study aimed to investigate the effects of Bupleurum chinense(one of the original materials of Bupleuri Radix) on the behaviors and the diversity of intestinal flora of depressed mice. A depression mouse model was induced by repeated social defeat stress. Specifically, C57 BL/6 J male mice were exposed to the attack from the CD-1 mice. Then, C57 BL/6 J male mice were divided into a depression group and a B. chinense group, with normal saline and B. chinense administered(ig) respectively. Sucrose preference test and tail suspension test were conducted during and after the experiment respectively, to analyze the effects of B. chinense on the behaviors of the depressed mice. The feces were collected after the experiment. The V3-V4 16 S rDNA regions of intestinal flora of mice in each group were sequenced by Ion S5 TMXL for the analysis of the number of operational taxonomic units(OTUs), richness, alpha and beta diversity indexes, and differential phyla and genera. The results indicated that B. chinense could decrease depressive-like behaviors of mice, increase sucrose preference, and shorten the time of immobility in tail suspension test. After B. chinense intervention, the relative abundance of Firmicutes was significantly decreased, while that of Bacteroidetes was increased at the phylum level. At the genus level, the relative abundance of Lactobacillus and Lachnoclostridium decreased(P<0.05), while that of Bacteroides, Alistopes, etc. was elevated(P<0.05). The findings demonstrate that B. chinense can regulate the intestinal flora and improve the depressive-like behaviors of mice with depression.

ABSTRACT

According to Chinese medicine, the atlantoaxial joint is a composite joint composed of tendons and bones, and the stability of the joint depends on the 'tendon-bone balance' involving tendons, ligaments, atlas and axis. Multiple causes of 'tendon off-position, joint subluxation' will lead to joint 'tendon-bone imbalance', which will evolve into atlantoaxial subluxation (AAS), endangering human health. Chinese therapeutic massage (tuina) is a very effective treatment for AAS in adults, but conventional manipulations are prone to ineffectiveness or accidents due to neglect of the causal relationship of the 'tendon-bone imbalance' and inappropriate manipulations. Compared with conventional manipulations, the rational choice of modified manipulations under the guidance of 'tendon-bone balance' theory is more effective and less risky, and more worthy of clinical promotion. From the 'tendon-bone balance' theory, we considered the shortcomings of conventional manipulations, and introduced several modified manipulations that have their own strengths in 'tendon smoothing' and 'bone setting', in order to provide new ideas for treatment of AAS in adults.

ABSTRACT

Objective:To observe the short-term efficacy of sub-Tenon’s triamcinolone acetonide (TA) in the treatment of cystoid macular edema (CME) after cataract surgery. Methods:A retrospective study. From October 1, 2013 to October 1, 2018, 21 patients (21 eyes) with CME after cataract surgery diagnosed at The Affiliated Eye Hospital of Wenzhou Medical University were included in this study. All eyes underwent BCVA, intraocular pressure, frequency domain OCT examination. The standard logarithmic visual acuity chart was used for BCVA examination, which was converted into logMAR visual acuity when recorded. A frequency domain OCT instrument was used to measure the foveal retinal thickness (CRT). Among 21 eyes of 21 patients, 9 eyes were male and 12 eyes were female; the average age was 66.38±10.88 years. The average logMAR BCVA was 0.46±0.23; the average CRT was 519.90±131.59 μm; the average intraocular pressure was 11.01±3.97 mmHg (1 mmHg=0.133 kPa). All eyes underwent a single treatment of 100 mg/ml TA suspension 0.4 ml (containing TA 40 mg) under Tenon's capsule. The average follow-up time after treatment was 32.86±20.2 days. The changes of BCVA, CRT and intraocular pressure were observed 1 month after treatment. The BCVA, CRT and intraocular pressure before and after treatment were compared by paired t test. Results:One month after treatment, the average logMAR BCVA, CRT, and intraocular pressure of the CME eyes were 0.29±0.22, 307.71±35.82 μm and 14.19±6.30 mmHg, respectively. Compared with those before treatment, the differences were statistically significant ( t=5.252, 8.166, 4.128; P=0.000, 0.000, 0.001). Among 21 eyes, 13 eyes (61.90%) of the retinal cysts in the macular area completely disappeared; 8 eyes (38.1%) were still visible between the retinal layers. Three eyes with intraocular pressure over than 21 mmHg were treated with lowering intraocular pressure and returned to normal. Conclusion:Sub-Tenon’s capsular injection of TA to treat CME after cataract surgery can reduce CRT and improve vision in a short time.

ABSTRACT

BACKGROUND: Acupuncture is a common treatment for urinary retention after spinal cord injury. It has been widely used in the clinical treatment of urinary retention after spinal cord injury due to its small side effects and simple operation. However, there is a lack of evidence-based medicine. OBJECTIVE: To examine the effectiveness of acupuncture for urinary retention in patients with spinal cord injury by a systematic review and Meta-analysis of randomized controlled trials (RCTs). METHODS: A computer-based online retrieval of CBM, CNKI, WanFang, Cochrane Library, PubMed, EMBASE, and Web of Science databases was performed. The keywords were "spinal cord injury, neurogenic bladder, acupuncture" in English and Chinese, respectively. RCTs concerning acupuncture or acupuncture combined with conventional therapy were identified and a Meta-analysis was conducted on RevMan 5.3 software. RESULTS AND CONCLUSION: Nine RCTs involving 632 participants were included. In terms of urodynamic changes, compared with the control group, acupuncture was more effective in improving the residual urine volume[mean difference(MD)=-144.26,95% confidence interval(CI)(-252.15,-36.37),P=0.009]and maximum cystometric capacity[MD=35.69,95%CI(3.15,68.22),P=0.03].However,there were no significant differences between two groups for average daily number of urination[MD=-1.66,95% CI(-3.40,0.07),P=0.06].Concerning total effective rate, acupuncture was more effective than control group[RR=1.24,95%CI(1.11,1.37),P<0.0001].To conclude, compared with the conventional therapy, acupuncture is more effective in improving residual urine volume, maximum cystometric capacity and total effective rate for urinary retention in patients with spinal cord injury.

ABSTRACT

Objective To summarize the clinicopathological features of monomorphic epitheliotropic intestinal T cell lymphoma (MEITL),and provide the experiences and lessons for diagnosis and treatment. Methods The clinical manifestations, histopathological and immunohistochemical characteristics, diagnosis, treatment and prognosis of 3 cases of MEITL were analyzed retrospectively.Meanwhile, the other cases published in the literatures were reviewed and summarized. Results Two cases of middle aged and aged men and one young woman were reported in present paper. These lesions of all the 3 patients were at the small intestine,of which the female patients had skin and mammary gland involvement. The clinical manifestations were abdominal distention, mulligrubs and intestinal perforation, and the main pathological feature was the diffuse infiltration of the homogeneous tumor cells in the whole layer intestinal wall.Immunohistochemical examination showed that tumor cells expressed all T cell markers CD3,CD8,CD56,T cell cytoplasmic antigen (TIA-1),CD4 and CD5,and Ki-67 proliferation index was 50%-70%.As to the treatment, case 1 received CHOP and GDP regimens successively, and died 2 years after final diagnosis.Case 2 received CHOP regimen for 1 course of chemotherapy and died of giving up treatment.Case 3 gave up treatment, voluntary discharged and then lost.It is clear according to the previous literatures and our experiences that the MEITL possesses high invasiveness and low incidence, and makes the diagnosis difficult;the patient's response to the current treatment is poor. Conclusions MEITL is prone to be in the small intestine. The diagnosis of MEITL should be based on clinical manifestations, pathological features and immunohistochemical detection results.MEITL is rare, the course of disease progresses rapidly, and the patients have the tendency to have intestinal perforation and poor prognosis.Early diagnosis and positive chemotherapy may lead to a better prognosis.

Objective: To establish the common model of four kinds of rattan traditional Chinese medicinal materials (TCMM) (Paederiae Scandens Caulis, Lonicerae Japonicae Caulis, Polygoni Multiflori Caulis, and Ipomoea batatas Caulis) determinate the drying kinetic parameters, and study the stability of active ingredients in the drying process. Methods: The experimental data of four rattan TCMM were fitted to the nine thin layer models (Page, Wang and Singh, Two-term exponential, Newton, Logarithmic, Aghbashlo, Two-term, Midilli, Diffusion approach) at the temperature of 40, 50, 60, and 70 ℃. R2, χ2, and RMSE were used as the criterion of fit. Fick diffusion law was used to calculate the drying dynamical parameters. Based on the principle of chemical kinetics, the stability of active ingredients in the drying process was studied. Results: Aghbashlo model was the best one to describe drying process of four rattan TCMM. The effective moisture diffusion coefficients D and activation energy Ea of four rattan TCMM were between 1 × 10−10 and 1 × 10−9 m2/s and 40—50 kJ/mol respectively. The degradation reactions of ursolic acid, THSG, and polysaccharide conformed to the first-order kinetic equation. The content of chlorogenic acid increased with the drying temperature and drying time increased. Conclusion: The drying dynamical parameters values of TCMM with different parts have different ranges, their drying process should be described by a common model. The quality of TCMM was affected greatly by drying process.

ABSTRACT

Objective To study the chemical structure and the conformation in the aqueous solution for a novel neutral polysaccharide (DMP2-A) purified from Dendrobium moniliforme. Methods The chemical structural characterization was determined with sugar analyses, methylation analyses, and IR spectroscopic methods. Moreover, the molecular weight and conformation in the aqueous solution were analyzed by size-exclusion chromatography (SEC) combined with laser light scattering (LLS). Results The polysaccharide DMP2-A consisted of 1-linked terminal xylosyl residue, 1,3-, 1,4-, and 1,3,6-linked glucosyl residues, 1,3-, 1,4-, and 1,3,6-linked galactosyl residues, 1,3,5-linked arabinosyl residue and 1,3,6-linked mannosyl group. Moreover, the weight-average molecular (Mw) of polysaccharide DMP2-A was determined to be (1.07 × 104). A small amount of aggregates were formed with the aggregation number of approximately 38 in the 0.15 mol/L NaNO3 aqueous solution. Conclusion DMP2-A is a kind of polysaccharide with multi-branch and complicated structure. It’s the first time that the polysaccharide DMP2-A was isolated and the chemical structure and conformation in aqueous solution were reported.

ABSTRACT

Objective@#To study the effect of nutrition intervention on the body composition and blood glucose in newly diagnosed patients with type 2 diabetes mellitus complicated with overweight and obesity.@*Methods@#A total of 84 newly diagnosed patients with type 2 diabetes mellitus complicated with overweight and obesity were selected from the department of endocrinology in China-Japan Friendship Hospital from April 2015 to December 2016. Basic information and body composition of these patients were collected and measured. Also the food frequency questionnaire (FFQ) was used to investigate the dietary status in the past month, and calculate the intake of energy, three major nutrients and dietary fiber. All patients received nutrition intervention by nutritionist for three months as requested by Diabetes guidelines. After 3-month intervention, blood glucose, body composition and dietary status were examined again. Relevant indicators of patients were compared before and after the intervention. All patients were divided into three groups according to the degree of body mass index (BMI)′s change: <5%, 5%-10% and >10%. The differences of body weight, muscle, body fat rate, visceral fat index and blood glucose level among three groups were quantified.@*Results@#There were 53 male patients with age (41.5±8.9) years and HbA1c (7.5±0.4)%, and 31 female patients with age (40.1±8.5) years with HbA1c (7.6±0.5)%. The intake of energy, carbohydrate and fat of patients were significantly decreased (P<0.05) and dietary fiber was significantly increased (P<0.05) after nutrition intervention. The body weight, muscle mass, fat mass and visceral fat index were also significantly decreased (P<0.05). The average decrease of fat was about 2.8 kg accounting for 73.7% (2.8 kg/3.8 kg) of the total weight loss. The body fat rate decreased from (31.2±3.1)% to (28.8±3.2)% (P<0.05). The fasting blood glucose and postprandial blood glucose were both decreased significantly after intervention (P<0.05). Among three BMI decreased groups, there was no significant difference in body weight and muscle mass (P>0.05), but a significant difference in body fat rate and visceral fat index (P<0.05). The body fat rate, visceral fat index, fasting blood glucose and postprandial blood glucose had more decreases with the greater amount of weight loss (P<0.05).@*Conclusion@#The three-month nutrition intervention could change the dietary habit and components of patients with type 2 diabetes mellitus by reducing the blood glucose, body fat rate and visceral fat index significantly. The degree of reduced BMI is positively related to the decrease of fasting blood glucose and postprandial blood glucose.

ABSTRACT

The mechanism of metabolism of bone and cartilage are one of focus of orthopedic knowledge, and its clinical and basic study plays an important role in preventing and treating osteoporosis, degeneration of bone and joint, bone tumor. At present, the mechanism of metabolism of bone and cartilage studied from protein, signal pathway to mRNA transcription factor regulation. As a new regulatory factor for non-coding proteins, long non-coding RNA(LncRNA) participates in physiological and pathological process. In recent years, a large number of researche showed that LncRNA plays an important role in metabolism of bone and cartilage, and proved LncRNA participates in development and conversion of bone metabolism. With the exposure of function chennel and target of LncRNA, the key role of LncRNA in bone metabolism were proved constantly. The paper concluded biological characteristics of LncRNA, function and target spot of LncRNA in bone and cratilage metabolism, and in further state gene regulatory mechanism of bone and cartilage metabolism, explore diagnosis and treatment, provide new thought and treatment target for bone metabolism.
