ABSTRACT
In order to effectively manage the atrial fibrillation patients, West China Hospital has established an integrated general-specialty management model within medical consortium. This model takes the atrial fibrillation-stroke integrated management outpatient clinic as the platform, combines general practice and specialty to provide standardized care for atrial fibrillation patients. The model was characterized by primary diagnosis, two-way referral, up and down linkage, and differential management for acute and chronic conditions. This article, taking West China Hospital as an example, introduces the integrated team of cardiologists and general practitioners in the tertiary hospital with general practitioners in the community, and the preliminary accomplishment in the management of atrial fibrillation within the medical consortium. It would provide a reference for the long-range management of atrial fibrillation in other provinces and cities.
ABSTRACT
<p><b>INTRODUCTION</b>This study aims to assess whether cryosurgery is a feasible local therapy for bone metastatic prostate cancer (bmPCa).</p><p><b>METHODS</b>A total of 23 patients with bmPCa who received cryosurgery and adjuvant androgen deprivation therapy (ADT) were included in the cryosurgery group (Group 1). Another 23 matched patients who received only ADT served as the control (Group 2). Prostate-specific antigen (PSA) nadir level, time to PSA nadir, time to castration-resistant prostate cancer (CRPC), progression-free survival and therapy response of bone metastases were compared between the groups.</p><p><b>RESULTS</b>The median follow-up time in Group 1 and Group 2 patients was 37 (range 19-53) months and 42 (range 24-56) months, respectively. Patients in Group 1 had fewer local complications, lower PSA nadir level (0.23 ng/mL vs. 4.01 ng/mL; p = 0.024), shorter median time to PSA nadir (3 months vs. 7 months; p < 0.001), longer median time to CRPC (36 months vs. 27 months; p = 0.002) and longer progression-free survival (35 months vs. 26 months; p = 0.003) compared to those in Group 2. Therapy responses of bone metastases were similar in the two treatment groups (p = 0.689).</p><p><b>CONCLUSION</b>Cryosurgery is a feasible local therapy for bmPCa patients with prostate volume less than 50 mL and without bulk tumours outside the prostate capsula. Cryosurgery may decrease PSA nadir level, local complications and time to PSA nadir, delay time to CRPC and improve progression-free survival.</p>
ABSTRACT
In order to investigate the effect of S-adenosylmethionine decarboxylase (SAMDC) gene in melon resistance to powdery mildew, according to the previous results of EST sequences from cDNA-AFLP library and the melon genome sequence data, the SAMDC gene was isolated from Chinese wild melon clone 'Yuntian930' by RT-PCR and designated as CmSAMDC (GenBank Accession No. KF151861). The mORF (main open reading frame) was 1 095 bp encoding 364 amino acids with a molecular mass of 40 kDa. The predicted protein sequence showed high similarity with Cucumis sativus and Citrofortunella microcarpa. The SDS-PAGE showed that the expression protein was a fusion protein with the molecular weight of 40 kDa, which perfectly matched the mass calculated from the amino acid sequence. Quantitative real-time PCR analysis indicated that the expression level of CmSAMDC reached a maximum at 48 hpi (hours post inoculation) that over 7-fold to 0 hpi, and the expression of CmSAMDC was also detected in tendril, root, leaf and stem. These results indicate that CmSAMDC gene may play protective roles in melon resistance to powdery mildew infection.

Objective To analysis the effect of conbercept ophthalmic injection on peripheral blood vascular endothelial growth factor,intraocular pressure and visual acuity in patients with age related macular degeneration.Methods 60 patients who were diagnosed with age related macular degeneration were collected.All patients were randomly divided into experimental group and control group,30 cases in each group, lucentis intravitreal injection was given into the vitreous cavity in control group, experimental group give conbercept injection into the vitreous cavity,once a month.All patients were treated for 3 months,after the end of treatment,all patients serum VEGF and CRP,CMT,CNV and fluorescence leakage rate,intraocular pressure and visual acuity were detected.ResuIts After treatment,compared with control group,the content of serum VEGF was significantly lower in the experimental group(P<0.05).The content of serum CRP was significantly lower in experimental group(P<0.05).The CMT,CNV and fluorescence leakage rate were significantly lower in the experimental group (P<0.05).The level of IOP and visual acuity(logMAR)were significantly lower in the experimental group (P<0.05).ConcIusions Conbercept ophthalmic injection can reduce age-related macular degeneration patients week blood VEGF and CRP levels and decrease the CMT, CNV and fluorescence leakage rate, reduce intraocular pressure, visual acuity improvement and have guiding significance for clinic.

In this study, the effect of cyclosporin A (CsA) eye drop on keratoconjunctivitis Sicca (KCS) and its mechanism were studied. The KCS models were established by injecting Pertussis vaccine, complete freunds adjuvant (CFA) and antigen of conjunctiva from isotype mice. Then the KCS models were treated with cyclosporin A eye drop. Changes in breaking-up time (BUT), lacrimal secretion in 30 min and diversion in 24 h were measured. The percentage of beaker cells, the lymphocytic infiltration in conjunctiva were observed. The expression levels of Aquaporin-3(AQP3) in conjunctiva epithelial cells, beaker cells and accessory lacrimal gland were immunohistochemically detected. The results showed that there were significant differences in BUT, the percentage of beaker cells, lacrimal secretion in 30 min, the lymphocytic infiltration and the expression of AQP3 between the experimental group and an control group. It was concludedthat CsA eye drop exerts marked therapeutic effect on KCS by inhibiting T lymph cells, increasing the goblet cells and AQP3 expression in conjunctiva.

Objective To investigate the polymorphism of the vitamin D receptor gene (VDR)TaqⅠin relation to diabetic retinopathy. Method Fragment length discrepant allele specific PCR(FLDAS-PCR) were used to determine VDR genetypes in 158 patients with diabetic retinopathy and in 198 normal subjects. Results The frequency distribution of VDR genotypes in diabetic retinopathy patients was 106 (67.1%) in TT, 33(20.9%) in Tt, 19(12.0%) in tt; and in normal persons was 165 (83.3%) in TT, 23(11.6%) in Tt, 10 (5.1%) in tt.
