ABSTRACT

Objective To compare vein graft quality and patency between the segmental incision and the open saphenous vein harvesting in coronary artery bypass grafting. Methods The data of patients underwent CABG in the First Affiliated Hospital of Wenzhou Medical University from January 2013 to December 2015 were retrospectively analyzed. The patients were devided into 2 groups, which is 99 open and 114 sagmental incision,by the way of harvesting. The operative risk factors, harvesting time and length of venous grafts,as well as the inci-sion complications were compared between the above two groups. One year after operation, the vein graft patency was detected by the coronary artery CTA. Results No significant differences in the risk factors of incision compli-cations were observed between the two groups, but the incidence of various incision complications was significantly reduced in the segmental incision group(P < 0.05). No significant difference in patency rate of venous grafts was observed between two groups at 1 years post?operation. Conclusion Segmental incision saphenous vein harvesting was safe and feasible, which can decrease the incision complications without obvious effect on postoperative patency.

ABSTRACT

AIM:Toobservetheeffectsofmicroparticlesderivedfrombonemarrowmesenchymalstemcells ( MSC-MPs) on angiogenesis and cardiac function in a rat myocardial infarction model .METHODS:MSCs were obtained from Sprague-Dawley rats.MSCs were treated under serum-free condition in hypoxia for 72 h, and the microparticles were isolated from the supernatants .The phenotypic profile of MSC-MPs was determined by bead-based flow cytometry and the morphology was observed under a transmission electron microscope .The rat myocardial infarction model was established . The cardiac function was evaluated by echocardiography after the intramyocardial injection of MSC -MPs.The myocardial in-farct size was observed by Masson staining .The blood vessel density in the peri-infarcted area was measured using immuno-histochemical staining for von Willebrand factor and α-smooth muscle actin. The expression of vascular endothelial growth factor ( VEGF) was analyzed by real-time PCR.RESULTS: Apoptotic MSCs released a large quantity of microparticles which were phenotypically similar to the parent MSCs and 100~1 000 nm in diameter. The cardiac functions of myocardial infarction rat model were improved at 7 d and 28 d after intramyocardial injection of MSC-MPs compared with control group . The myocardial infarct size was reduced and angiogenesis was promoted significantly in the infarcted heart injected with MSC-MPs 28 d after treatment .MSC-MPs treatment also increased the expression level of VEGF within 7 d.CONCLU-SION:MSC-MPs protect cardiac tissue from ischemic injury and improve cardiac function by promoting angiogenesis after myocardial infarction .

ABSTRACT

AIM:To investigate the effects of microRNA-193 (miR-193) on the proliferation and apoptosis of rat bone marrow mesenchymal stem cells (MSCs).METHODS:Cultured rat MSCs were transfected with pre-miR-193 or anti-miR-193 to regulate the expression of miR-193.The proliferation of the MSCs after transfection was evaluated by MTS assay, colorimetric BrdU cell proliferation assay and Ki-67 immunostaining.Cell apoptosis was analyzed by flow cytometry with Annexin V/PI staining. The effect of miR-193 on the expression of cell cycle-related proteins was evaluated by qRT-PCR.RESULTS:Transfection of pre-miR-193 or anti-miR-193 regulated the expression of miR-193 in MSCs effectively . Over-expression of miR-193 significantly promoted the proliferation of MSCs ( P0.05).The result of qRT-PCR indicated miR-193 promoted the expression of cyclin-dependent kinase 2 ( CDK2 ) significantly ( P<0.01).CONCLUSION:miR-193 promotes the proliferation of MSCs possibly through the CDK 2 pathway.

ABSTRACT

Objective To detect the expression of Per2 and VEGF in NSCLC tissue, and analyze its clinical significance. Methods The expression of Per2 and VEGF was measured in 60 NSCLC and 20 normal lung tissue by immunohistochemistry assay, then the relation with clinicopathological parameters was analyzed. Results The positive expression rate of Per2 in NSCLC and in normal lung tissue were 71.7% and 95.0% ( x2 =4.683,P <0.05 ),while the positive expression rate of VEGF were 58.3% and 15% (x2 =11.295,P < 0.01 ).The negative expression of Per2 in NSCLC was correlated with pathological differentiation or TNM stage( x2 =4.413,6.179,all P <0.05),while the positive expression of VEGF in NSCLC was correlated with pathological differentiation or lymphatic metastasis (x2 =6.524,P < 0.05 ).The expression of Per2 was negatively correlated with the expression of VEGF in NSCLC (r =-0.381,P < 0.01 ).Conclusion The overexpression of VEGF in NSCLC may contribute to invasion and metastasis, while Per2 may be a beneficial prognosis factor in patients with NSCLC.

ABSTRACT

Objective To investigate the application data of 6-minute walk test to evaluate the surgical effect of congenital heart disease with pulmonary hypertension. Methods 83 cases of congenital heart disease with pulmonary hypertension were selected and divided into mild(28 patients) ,moderate(30 patients) and severe(25 patients)three groups according to the ultrasound preoperative pulmonary artery systolic pressure estimation (SPAP), all patients underwent surgery correction of heart defects, and before and after surgery the patients were detected SPAP and 6MWT,6MWT observed for evaluating the effect of surgery. Results After surgery, the SPAP of, mild, moderate and severe group were ( 28.7 ± 5. 6) mm Hg, ( 28.7 ± 5.6 ) mm Hg, ( 28.7 ± 5.6 )mm Hg, significantly decreased, 6 MWT were (436. 5 ± 37. 0) m, ( 411.0 ± 39. 8 ) m, ( 338.7 ± 32. 4 ) m, significantly increased, more significant changes in moderate group; The mechanical ventilation and ICU stay of severe group were ( 12. 6 ± 3.2) h, ( 5.5 ± 1.0) d, significandy longer than the mild and moderate groups ( t = 11. 770,6. 118,18. 721,9. 970, all P < 0. 05 ). Conclusion 6-minute walk test to evaluate the surgical effect of congenital heart disease with pulmonary hypertension had clinical significance, and in moderate to severe pulmonary hypertension in patients with clinical assessment more meaningful, and it was worthy of attention.

ABSTRACT

<p><b>BACKGROUND</b>Boonchoplasty can not only remove tumor but also reserve lung tissue maximally, and it becomes an alternative choice for patient with poor pulmonary function who could not accept pneumonectomy. The aim of this study is to summarize the experience of carinal resection and reconstruction, bronchoplasty and pulmonary arterioplasty in the treatment of central-type lung cancer.</p><p><b>METHODS</b>From March, 1987 to March, 2005, A total of 79 patients with central-type lung cancer underwent operation. The operations included: left bronchoplasty (34 cases) combined with pulmonary arterioplasty in 10 cases and partial resection of left atrium in 3 cases; right bronchoplasty (45 cases) combined with carinal resection in 14 cases and segmentplasty in 5 cases, pulmonary arterioplasty in 5 cases, partial resection of superior vena cava wall in 5 cases.</p><p><b>RESULTS</b>There were no perioperative deaths. Twenty-eight cases (35.4%) had postoperative complication. The 1-, 3-and 5-year survival rate were 86.1%, 55.2% and 32.1% respectively.</p><p><b>CONCLUSIONS</b>Proper selection of carinal resection, bronchoplasty and pulmonary arterioplasty can expand the indications. They can reduce the ratio of pneumonectomy and improve the postoperative quality of life and the prognosis of lung cancer patients.</p>
