ABSTRACT

Protein O-GlcNAcylation is a post-translational modification that links environmental stimuli with changes in intracellular signal pathways, and its disturbance has been found in neurodegenerative diseases and metabolic disorders. However, its role in the mesolimbic dopamine (DA) system, especially in the ventral tegmental area (VTA), needs to be elucidated. Here, we found that injection of Thiamet G, an O-GlcNAcase (OGA) inhibitor, in the VTA and nucleus accumbens (NAc) of mice, facilitated neuronal O-GlcNAcylation and decreased the operant response to sucrose as well as the latency to fall in rotarod test. Mice with DAergic neuron-specific knockout of O-GlcNAc transferase (OGT) displayed severe metabolic abnormalities and died within 4-8 weeks after birth. Furthermore, mice specifically overexpressing OGT in DAergic neurons in the VTA had learning defects in the operant response to sucrose, and impaired motor learning in the rotarod test. Instead, overexpression of OGT in GABAergic neurons in the VTA had no effect on these behaviors. These results suggest that protein O-GlcNAcylation of DAergic neurons in the VTA plays an important role in regulating the response to natural reward and motor learning in mice.

ABSTRACT

Selective activation or inhibition of the angiotensin II type 2 (AT2) receptor can cause vasodilation, decrease cell proliferation, promote neurite outgrowth and block neuronal excitability. The AT2 receptor can be used as a potential drug target for the treatment of cardiovascular diseases, fibrosis, inflammation, neuronal diseases and tumors. Research and development of new drugs that selectively act on the AT2 receptor and the mechanism of intracellular signal transduction involving the AT2 receptor is a challenging but worthy endeavor. We review research progress on the AT2 receptor and compounds that act on the AT2 receptor, along with structure-activity relationship analysis, to provide reference and guidance for further research in this field.

ABSTRACT

Objective@#To explore associated factors of non-suicidal self-injury in adolescents with depression, and to provide a reference for clinical intervention and treatment.@*Methods@#A cross-sectional survey was conducted among 80 adolescents aged 14-18 years with depressive disorder who visited an outpatient clinic in the PLA 904nd Hospital from January 2015 to January 2017. Eysenck Personality Questionnaire(EPQ), Egma Minnenav Bsrdndosnauppforstran (EMBU), non-suicidal self-injury behavior rating scale were used to investigate them.@*Results@#Male patients with depression had more non-suicidal self-injury than female patients(76.74% vs. 54.05%), and patients with highly educated mothers had more non-suicidal self-injury(77.78%)(χ 2=4.58, 6.11, P<0.05). Self-injury behavior in patients with internal and external (E) the orientation factor score (5.22±2.82) was lower than the score of the patients without self-injury behavior(8.85±2.93)(t=-5.37, P<0.05), neuroticism (N), while psychoticism (P) and hide(L) score [(14.53±3.94) (5.60±1.58) (12.94±0.54)] were higher than patients without self-injury behavior [(11.55±5.71) (3.13±1.43) (12.09±0.55)](t=2.74, 6.82, 6.31, P<0.05); Factors such as "severe paternal punishment" "paternal excessive interference" "paternal refusal to deny" "maternal refusal to deny" and other factors in patients with non-suicidal self-injury [(21.57±4.65) (21.73±4.38) (14.25±5.08) (20.34±4.64)] were all higher than those without nonsuicidal self-injury [(18.50±7.53) (18.77±3.55) (11.58±2.97) (16.55±6.32)](t=2.25, 3.04, 2.96, 3.05, P<0.05); Logistic regression analysis showed that males, mothers with high educational level, and E, N, P (<38.5 and >61.5) were associated with non-suicidal self-injury behavior in adolescents with depression(OR=1.99, 2.31, 1.71, 2.52, 1.98, P<0.05).@*Conclusion@#Gender, maternal education and symptom score were the important factors influencing non-suicidal self-injury.It is necessary to arouse the attention of the family, society and school.

ABSTRACT

Quantitative evaluation of analgesic efficacy improves understanding of the antinociceptive mechanisms of new analgesics and provides important guidance for their development. Lappaconitine (LA), a potent analgesic drug extracted from the root of natural Aconitum species, has been clinically used for years because of its effective analgesic and non-addictive properties. However, being limited to ethological experiments, previous studies have mainly investigated the analgesic effect of LA at the behavioral level, and the associated antinociceptive mechanisms are still unclear. In this study, electrocorticogram (ECoG) technology was used to investigate the analgesic effects of two homologous derivatives of LA, Lappaconitine hydrobromide (LAH) and Lappaconitine trifluoroacetate (LAF), on Sprague-Dawley rats subjected to nociceptive laser stimuli, and to further explore their antinociceptive mechanisms. We found that both LAH and LAF were effective in reducing pain, as manifested in the remarkable reduction of nocifensive behaviors and laser-evoked potentials (LEPs) amplitudes (N2 and P2 waves, and gamma-band oscillations), and significantly prolonged latencies of the LEP-N2/P2. These changes in LEPs reflect the similar antinociceptive mechanism of LAF and LAH, i.e., inhibition of the fast signaling pathways. In addition, there were no changes in the auditory-evoked potential (AEP-N1 component) before and after LAF or LAH treatment, suggesting that neither drug had a central anesthetic effect. Importantly, compared with LAH, LAF was superior in its effects on the magnitudes of gamma-band oscillations and the resting-state spectra, which may be associated with their differences in the octanol/water partition coefficient, degree of dissociation, toxicity, and glycine receptor regulation. Altogether, jointly applying nociceptive laser stimuli and ECoG recordings in rats, we provide solid neural evidence for the analgesic efficacy and antinociceptive mechanisms of derivatives of LA.

OBJECTIVE@#To explore the predictive value of newly diagnosed IgG levels in the recurrence of IgG-type multiple myeloma (MM) patients after initial treatment.@*METHODS@#The clinical and pathological data of 91 patients newly diagnosed IgG-type MM who were hospitalized in the Department of Hematology of the Second People's Hospital of Yichang and Department of Oncology of The Affiliated Hospital of Jianghan University from April 2010 to March 2019 were collected. According to the median IgG level at the time of initial diagnosis, patients were divided into high IgG group and low IgG group. The recurrence time after initial treatment was followed up, and the correlation between newly diagnosed IgG level and recurrence was analyzed by univariate and multivariate analysis, as well as the influencing factors of IgG levels in order to predict furtherly the potential mechanism of recurrence.@*RESULTS@#Univariate survival analysis showed that high revised international staging system (R-ISS) staging, high level of bone marrow plasma cell (BMPC), lactate dehydrogenase (LDH), creatinine, β@*CONCLUSION@#The higher the serum IgG concentration of IgG-type MM patients at first diagnosis, the earlier the recurrence, which is related to the low level of serum albumin, and can be used as a potential recurrence predictor after complete remission of IgG-type MM patients.

ABSTRACT

Non-image forming functions of eyes include the regulation of biological circadian rhythm and biological magnetoreception.Biological magnetoreception means that various organisms including human obtain the direction and position information through the geomagnetic field.Creatures with retina realize magnetoreception regarding retinal cryptochrome as magnetoreceptor.Hypotheses of magnetoreception contain the radical-pair theory and the biological compass theory.The two theories both reckon retinal elements as possible receptor protein of magnetoreception, and eyes as receptor organ.The radical-pair theory suggests that change of radical spin influences the structure of retinal cryptochrome, leading to different downstream chemical reaction products, which makes the variable magnetic field information perceivable.And the biological compass theory proposes a rod-like complex composed of polymerized cryptochromes and magnetoreceptor proteins, which can point to different directions due to light and magnetic signals.These changes in retina transmit geomagnetic field signal to the brain, and then sense of direction is formed.Researching biological magnetoreception promotes a novel perspective in the diagnosis and treatment of eye and brain diseases, and brings innovation in magnetic material field.In this article, non-image forming functions of eyes, hypotheses of magnetoreception and possible mechanism of non-image forming functions of eyes in magnetoreception were reviewed.

ABSTRACT

Objective:To investigate the diagnosis and treatment of hepatic artery thrombosis (HAT) after adult orthotopic liver transplantation.Methods:The retrospective and descriptive study was conducted. The clinicopathological data of 411 patients who underwent adult orthotopic liver transplantation in the First Affiliated Hospital of Xi ′an Jiaotong University from December 2011 to July 2018 were collected. There were 328 males and 83 females, aged from 21 to 66 years, with a median age of 46 years. Observation indicators: (1) incidence of HAT and its clinical characteristics; (2) diagnosis of HAT; (3) treatment of HAT; (4) follow-up. Follow-up using outpatient service, telephone interview or WeChat group communication was conducted to detect the incidence of biliary stricture and survival of patients up to August 2018. Measurement data with normal distribution were represented as Mean± SD, measurement data with skewed distribution were represented as M(range). Count data were described as absolute numbers or percentages. Survival rate was estimated using the Kaplan-Meier method. Results:(1) Incidence of HAT and its clinical characteristics: 11 of 411 patients had HAT after orthotopic liver transplantation with the incidence of 2.68%(11/411), including 10 males and 1 female, aged 44 years(range, 22-63 years). The time to occurrence of postoperative HAT was 4 days(range, 1-15 days). The etiologies of 11 patients included 6 cases of hepatitis B virus-related cirrhosis, 1 case of hapatitis related cirrhosis, 1 case of hepato-cellular carcinoma, 1 case of liver cirrhosis, 1 case of alcoholic hepatitis related cirrhosis, 1 case of wilson disease. All the 11 patients were ABO compatible. The cold ischemic time and warm ischemic time of donor liver were (316±89)minutes and (13±4)minutes, respectively. Type Ⅰ arterial anasto-mosis was conducted in 11 patients. The clinical manifestations included asymptomatic type in 10 patients and sepsis type in 1 patient. (2) Diagnosis of HAT: all the 11 patients were confirmed with HAT by endovascular angiography, including 7 cases showed no arterial flow under Color Doppler ultrasound, and contrast-enhanced ultrasound indicated HAT. Two patients showed increased hepatic artery resistance index under Color Doppler ultrasound, and contrast-enhanced ultrasound indicated 1 case of HAT and 1 case of anastomotic stenosis. One patient showed slow velocity of hepatic artery blood flow and low resistance index under color Doppler ultrasound, and contrast-enhanced ultrasound indicated HAT. One patient showed slight blood flow signals under Color Doppler ultrasound, and contrast-enhanced ultrasound indicated HAT. (3) Treatment of HAT: 11 patients received endovascular therapy. Six patients had HAT completely disappeared after thrombolytic therapy, 5 patients with residual thrombosis continued thrombolytic therapy with microcatheter urokinase. Six patients with complications were improved after symptomatic treatment. HAT completely disappeared after (6.7±2.6)days of treatment and the clinical success rate was 11/11. (4) Follow-up: 11 patients were followed up for 19-1 722 days, with a median follow-up time of 46 days. During the follow-up, 4 patients had biliary stricture and underwent stent implantation. Nine patients survived with 1-, 3-, 5-year overall survival rates of 75%, 75%, 75%, and 2 patients died.Conclusions:The incidence of HAT after adult orthotopic liver transplantation is low and clinical manifestations are atypical. Contrast enhanced ultrasound can improve diagnosis of suspected thrombosis. Endovascular therapy is safe and effective, which can significantly improve the blood flow of hepatic artery.

ABSTRACT

This study aims to investigate the etiology, pathogenic properties and pathogenic characteristics of corona virus disease-2019 (COVID-19) in traditional Chinese medicine(TCM), so as to provide ideas for clinical treatment based on syndrome differentiation. Efforts were made to retrive relevant literature concerning clinical studies, theoretical discussions and TCM diagnosis and treatment schemes issued by the state and various provinces, municipalities, autonomous regions and municipalities directly under the central government in relation to TCM from China Knowledge Network(CNKI) and Wanfang Database, and to analyze and summarize the etiology, pathology, theoretical viewpoints, clinical symptoms and signs, syndrome differentiation and medication rules. Currently, the common understanding of the etiology of COVID-19 in the field of TCM is the infection of "pestilential pathogen". However, there is a dispute over cold and heat or mixed understanding of cold and heat in terms of pathogenic attributes. The pathogenic factors are different from each other in dampness, toxin, dryness, fire (heat), wind, filth, depression, etc. There are various understandings on the pathogenesis including dampness, cold, heat, toxin, stasis, phlegm, stagnation, knot, dryness, filth, deficiency, blocking, collapse and asthma, etc. The etiology and pathogenesis are often mixed up. Integration of cold and heat, dryness and dampness, and other contradictory pathogens or pathogenesis is widely seen, which lacks the logicality of theoretical systems, and does not in line with the thinking characteristics of TCM on the etiology, pathogenesis, and syndrome differentiation of exogenous diseases. The main idea of medication in treatment is to diffuse the lung, clear away heat, eliminate dampness, resolve phlegm and repel foulness with aromatics. Maxing Shigantang is used as the core prescription. Chosen warm acrid drugs are mainly the ones with the effect of fragrance, removing dampness, resolving phlegm, and invigorating spleen. They are not the ones with the effects of warming yang and dissipating cold, but the combination of cold and heat, suggesting the complexity of etiology and pathogenesis. COVID-19 is categorized as plaque in TCM, and its etiology is "pestilential pathogen". This pestilential pathogen possesses not only the basic properties of toxin and filth, but also the characteristics of dampness, heat and wind. Throughout the course of the disease, phlegm, stasis, stagnation and other secondary pathogenic factors also occur. The evolution of pathogenesis is characterized by depression, blocking, and deficiency. There are more evidences that the pestilential pathogen of COVID-19 belongs to heat property no matter in the aspects of clinical manifestation, transmission law (syndrome differentiation at different stages), or in compatibility of medication.

Objective:As the amount of scientific reagents/consumables procurement in tertiary medical institutions increases year by year, it is necessary to seek for a more optimized operation mode for the current management needs to improve the procurement service and management of scientific reagents/consumables.Methods:Focused on the bottleneck problem of scientific research reagents/consumables procurement management, combined with the investigation of management mode in the same field and the review of relevant literature, a scientific research reagents/consumables procurement platform suitable for hospital was established, which was operated in the whole hospital for 2 years and continuingly improved.Results:The application of the scientific reagents/consumables procurement platform is expected to save 7%~8% of the scientific reagents/consumables related expenditure, which can make the scientific research procurement work standardized and simple, the procurement management precise and fast, and also provide favorable support for the prevention and control of epidemic diseases in hospitals.Conclusions:This platform is conducive to improving the service and management level of scientific reagents/consumables procurement in our hospital, and has conditions to expand the scope and increase the depth of management and service.

ABSTRACT

Objective:This paper aim to analyze the influencing factors of individual academic performance based on the construction of a research-oriented hospital, to provide possible reference for improving the service of science and technology management.Methods:Selecting 45 experts determine the constituent index and assign value of the academic performance evaluation system by using Delphi method. The index setting emphasized clinical application orientation, and weighted total score was used as the academic performance evaluation standard. Conduct a-350-person anonymous questionnaire survey and use descriptive analysis and multiple logistic regression methods to analyze the influencing factors.Results:The system includes a total number of 6 first-level indicators and 43 second-level indicators. The authoritative coefficients of the consulting experts are 8.5±0.6, and the Kendall of the two rounds of consultation are 0.78 and 0.83( P<0.05). The questionnaire results show that respondents with Ph. D degree, senior professional titles, mentor qualification, from large team and higher scientific research service support have better academic performance. personal background, team support and scientific research services have significant impact on academic performance. Conclusions:In order to improve the scientific research strength of the hospital, it is significant to focus on systematic scientific research training, strengthening discipline teams and research platforms, and improving the professional level of scientific research management.

ABSTRACT

Objective:To investigate the clinical effects and influence factors of intravitreal injection of anti-vascular endothelial growth factor (VEGF) drugs in the treatment of idiopathic choroidal neovascularization (ICNV).Methods:This retrospective study involved 27 patients (27 eyes) with ICNV from July 2012 to July 2015. Patients received intravitreal bevacizumab (1.25 mg), ranibizumab (0.05 mg), additional injection was provided if it was needed. The average follow-up time was 168 weeks. The recovery of best corrected visual acuity (BCVA) and central foveal retinal thickness (CRT) of the affected eye was observed. Follow up once a month after the initial treatment until the lesion was completely absorbed or scarred (the first follow-up period). Follow up every 12 weeks was performed to observe the recurrence of the lesions (the second stage of long-term follow-up). One month after the last injection of the first follow-up period, according to the regression of choroidal neovascularization (CNV), the affected eyes were divided into a significant improvement group (significant improvement group) and an insignificant improvement group (non-significant improvement group)), to analyze the effects of age, course of disease, type of drugs, number of injections, baseline BCVA and CRT on the regression of CNV lesions. According to the results of long-term follow-up, the eyes were divided into recurrence group and non-recurrence group, and the factors affecting the recurrence of CNV lesions were analyzed. Measurement data between groups was compared by using independent sample t test or non-parametric test; count data was compared by using χ2 test. Logistic regression analysis was used to analyze the factors affecting the regression and recurrence of the lesion. Results:At baseline and 1 month after the last injection in the first stage, the average BCVA of the eyes were 55.70±15.21 and 73.59±12.08 letters; CRT was 338.3±89.32 and 264.5±47.47 μm, respectively. The BCVA and CRT of the affected eyes were compared at the two time points, and the differences were statistically significant ( Z= -3.886, -4.061; P<0.001). The BCVA of the eyes in the significant improvement group and the insignificant improvement group were 65.38±17.27 and 51.63±12.61 letters, respectively; the difference between the two groups of BCVA was statistically significant ( t=-2.316, P=0.029). The results of long-term follow-up showed that of the 27 eyes, 6 eyes had recurrence; the average recurrence time was 90.83±49.02 weeks. After another intravitreal injection of anti-VEGF drugs, the CNV lesions was resolved. The average injection times of the relapsed group and the non-relapsed group were 3.67±0.816 and 2.24±0.768, respectively. The average injection times of the relapsed group was significantly higher than that of the non-relapsed group, and the difference was statistically significant ( Z=-3.253, P<0.001). There was no statistically significant difference between the two groups of eyes at baseline and CRT at the last follow-up ( Z=-1.342,-1.313; P=0.195, 0.195). Conclusion:Intravitreal injection of anti-VEGF drugs can effectively increase the regression rate of BCVA and CNV lesions in ICNV eyes; high baseline visual acuity indicates better CNV lesion regression after treatment. Relapsed patients can be effectively improved after re-treatment with anti-VEGF drugs, and CNV recurrence has no significant effect on the final prognosis.

ABSTRACT

Objective:To investigate the clinical efficiency of gastrocnemius muscle flap combined with antibiotics loaded calcium sulfate in the treatment of postoperative infection and plate exposure of tibial plateau fracture in elderly patients.Methods:From January, 2015 to May, 2019, 21 elderly patients with postoperative infection and plate exposure of tibial plateau fracture were treated, including 14 males and 7 females with an average age of 72.6 years, ranging from 61 to 82 years. The average course of disease was 22.7 days, ranging from 6 to 91 days. The site of wound infection was at medial in 8 cases, lateral in 9 cases and bilateral in 4 cases. The size of wound ranged from 2.0 cm×3.5 cm to 5.0 cm×12.0 cm. All wounds were implanted with antibiotics loaded calcium sulfate and repaired by gastrocnemius muscle flap combined with skin graft after debridement. Muscle flap survival, wound healing, inflammatory index, fracture healing and knee joint function were recorded. The curative effect was evaluated by McKee infection treatment criteria and the knee joint function was evaluated by HSS scoring criteria.Results:All 21 muscle flaps survived. In 1 case, skin graft necrosis occurred in a small area, and the wound healed well after dressing change. One case had exudation which was clear and the bacterial culturing was negative, and the wound healed after 2 weeks of dressing change. The other incisions were healed in stage I, and the healing rate was 90%. All patients were followed-up for an average of 28.7(16-39) months. The redness and swelling occurred in 1 case without exudation after 2 months which disappeared after anti-infection treatment and didn't recur again. The infection recurred in 1 case after 5 months, and it which was controlled after debridement and plate removal. According to McKee criteria, 19 cases were cured, 1 improved and 1 recurred with an effective rate of 95.2%. The fracture healing time was from 3 to 7 months, with an average of 4.6 months. According to HSS scoring criteria, the knee joint function was excellent in 12 cases, good in 7 cases and moderate in 2 cases.Conclusion:After thorough debridement, gastrocnemius muscle flap combined with antibiotic loaded calcium sulfate can effectively control the infection, repair the wound, promote fracture union and restore limb function in the treatment of postoperative infection and plate exposure of tibial plateau fracture in elderly patients.

OBJECTIVE@#To summarize and discuss the clinical efficacy and application value of intravenous drip of linezolid combined with local targeted sustained-release of vancomycin in the treatment of traumatic osteomyelitis of extremities infected with MRSA.@*METHODS@#Thirty patients with traumatic osteomyelitis of extremities infected by MRSA from March 2015 to March 2017 were analyzed retrospectively, including 21 males and 9 females; aged 25 to 64 years old, with an average age of(47.94± 6.23) years old;the course of disease ranged from 9 to 23 months, with an average of (15.68±6.23) months. The lesions were located in tibia in 18 cases and calcaneus in 12 cases. The causes of injury were fall injury in 12 cases, trafficaccident injury in 9 cases and fall injury in 9 cases. There were 22 patients with closed fractures and 8 patients with open fractures. There were 13 cases of internal fixation. Twenty-two patients had sinustract, 8 patients had soft tissue defect with bone and internal fixation exposure, soft tissue defect area ranged from 2.0 cm × 3.0 cm to 8.2 cm × 12.3 cm;10 patients had bone defect, defect area ranged from 0.5 to 3.4 cm;bacterial culture of sinus tract or wound secretion in all patients was MRSA. On the basis of thorough debridement, calcium sulfate artificial bone loaded with vancomycin was implanted in the lesion, and linezolid and glucose injection was given intravenously during the perioperative period. The patients were followed up regularly according to the time of antibiotic use, blood routine, erythrocyte sedimentation rate, high-sensitivity C-reactive protein, liver and kidney function and other related laboratory indexes, X-ray, CT and other imaging examinations, bone healing, flap survival, joint function and McKee's osteomyelitis cure criteria.@*RESULTS@#All the patients were followed up, and the duration ranged from 3 to 6 years, with a mean of (4.23±0.76) years. No recurrence of osteomyelitis occurred. Fracture healing, infection control, wound healing and functional recovery were achieved.@*CONCLUSION@#Intravenous drip of linezolid combined with local targeted sustained-release of vancomycin for the treatment of MRSA infected traumatic osteomyelitis in limbs have significant effects and low recurrence rates.

ABSTRACT

OBJECTIVE:To establish a metho d for simultaneous determinat ion of α-pinene,β-pinene,myrcene,limonene, γ-terpinene and α-terpineol in volatile oil from Citrus medica . METHODS :GC-QAMS method was adopted. The determination was performed on Agilent DB- 17 capillary column. Flame ionization detector was adopted with nitrogen as carrier gas (purity 99.999%) at the flow rate of 0.5 mL/min. The flow rate of nitrogen (make-up gas )was 25 mL/min,the flow rate of hydrogen was 30 mL/min, and the flow rate of air was 400 mL/min. The inlet temperature was 250 ℃,and the detector temperature was 280 ℃(programmed temperature),and injection volume was 0.5 μL with split ratio of 35 ∶ 1. Using limonene as internal reference ,relative correction factors of α-pinene,β-pinene,myrcene and γ-terpinene were calculated. The contents of 4 components in the samples were calculated according to the relative correction factors. The results were compared between QAMS and internal standard method (ISM,using dodecane as internal substance ,6 components to be determined ). RESULTS :The linear range of α-pinene,β-pinene, myrcene,limonene,γ-terpinene and α-terpineol were 0.030 5-0.213 8,0.066 6-0.466 0,0.021 8-0.152 3,0.765 2-5.356 4,0.387 3- 2.710 8,0.034 2-0.239 6 mg/mL(all r>0.999). The limits of detection were 4.82,7.89,4.01,4.54,5.53,2.47 µg/mL,and the limits of quantification were 15.34,25.91,13.69,15.70,18.68,8.36 µg/mL,respectively. RSDs of precision ,repeatability and stability tests (24 h)were all less than 2%. The average recovery rates were 96.08%,97.48%,100.90%,101.22%,100.54%, 95.84%(all RSD <3%,n=6). The average relative correction factors of α-pinene,β-pinene,myrcene and γ-terpinene were 0.997 8, 1.530 7,0.952 4 and 1.025 5 (all RSD <1%). The contents of α-pinene,β-pinene,myrcene,limonene,γ-terpinene and α-terpineol by ISM were 3.296 9-20.994 1,11.300 6-39.440 9,3.684 2-11.291 4,174.857 8-511.611 8,0-285.127 3,0-48.858 6 mg/g, respectively. The contents of α-pinene,β-pinene,myrcene and γ-terpinene determined by QAMS were 3.296 8-20.994 0,11.300 3- 39.439 7,3.684 1-11.291 1,0-285.126 6 mg/g,respectively. Relative content errors of 4 components as α-pinene determined by 2 methods were all less than 1% . ABSTRACT

Objective:To investigate the mechanism of Nardostachyos Radix et Rhizoma and Corydalis Rhizoma in treatment of atrial fibrillation by predicting targets and signaling pathways based on network pharmacology.Method:The traditional Chinese medicine system platform (TCMSP) database was used to screen out active components of Nardostachyos Radix et Rhizoma and Corydalis Rhizoma,predict targets,and construct the active component-predicted target network.Through the Online Mendelian Inheritance in Man (OMIM),Therapeutic Target Database (TTD),and Genecards databases,potential target information of atrial fibrillation was retrieved.STRING 11.0 database was used to obtain the protein-protein interaction data of relevant targets,and the results were visualized by Cytoscape 3.7.1 software to construct protein-protein interaction network relating to atrial fibrillation.The predicted targets of Nardostachyos Radix et Rhizoma and Corydalis Rhizoma were mapped to the potential targets of atrial fibrillation.The intersection targets were the potential targets for the treatment of atrial fibrillation with Nardostachyos Radix et Rhizoma and Corydalis Rhizoma.Then,Visualization and Integrated Discovery (DAVID),a database for annotation,was used to analyze biological functions and pathways of the potential targets of Nardostachyos Radix et Rhizoma and Corydalis Rhizoma in the treatment of atrial fibrillation.Finally,Cytoscape3.7.1 software was utilized to construct active component-potential target-signal pathway network of Nardostachyos Radix et Rhizoma and Corydalis Rhizoma in treatment of atrial fibrillation.Result:Totally 51 active components of Nardostachyos Radix et Rhizoma and Corydalis Rhizoma were screened out,and 18 potential targets for the treatment of atrial fibrillation with Nardostachyos Radix et Rhizoma and Corydalis Rhizoma were predicted.The effect was mainly correlated with the regulation of interleukin-6 (IL-6),sodium channel protein type 5 subunit alpha (SCN5A),tumor necrosis factor (TNF),nitric-oxide synthase,endothelial (NOS3),potassium voltage-gated channel subfamily hmember 2 (KCNH2),collagen alpha-1(I) chain (COL1A1),retinoic acid receptor RXR-alpha (RXRA),tissue factor (F3),alpha-1B adrenergic receptor (ADRA1B) and other target proteins,cyclic guanosine monophosphate (cGMP)/cGMP-dependent protein kinase G (PKG) signaling pathway,phosphatidylinositol-3-kinases (PI3K)/protein kinase B (Akt) signaling pathway,transcriptional disorders in cancer,calcium signaling pathways,and adrenergic signals in cardiomyocytes.Conclusion:Nardostachyos Radix et Rhizoma and Corydalis Rhizoma treat atrial fibrillation based on multiple components,multiple targets and multiple channels,and provide a scientific basis for subsequent experimental studies for further explainning its mechanism of action.

ABSTRACT

Objective:To observe the clinical efficacy of Duhuo Jishengtang combined with minimally invasive decompression of lumber spine in the treatment of degenerative lumbar spinal stenosis with kidney deficiency syndrome for more than 3 years. Method:Through a single-center parallel randomized controlled clinical trial, 63 patients with degenerative lumbar spinal stenosis who were hospitalized in Jiangsu Hospital of Traditional Chinese Medicine from October 2015 to April 2017 were observed for more than three years. Among them, there were 31 cases in the operation combined with traditional Chinese medicine group (observation group) and 32 cases in the operation combined with Celebrex and Mecobalamin group (control group). Both groups were given routine treatment. The observation group was treated with Duhuo Jishengtang for 3 weeks, while the control group was treated with Celebrex and Mecobalamin for 3 weeks. Result:In the aspect of pain improvement, only 3 months after operation, the observation group was superior to the control group (P<0.05), with no significant advantage at other time nodes during follow-up. During the 3-month follow-up and a longer period, it was confirmed that observation group had a significant advantage in improving lumbar function (P<0.01). At the last follow-up, the good efficacy rate of observation group was 83.87% (26/31), while the good efficacy rate of the control group was 62.5% (20/32). There was no unstable performance in the last follow-up of the two groups. There was no statistically significant difference between two groups. Conclusion:The middle and long-term follow-up for minimally invasive decompression of lumber spine showed a satisfactory efficacy in the treatment of degenerative lumbar spinal stenosis, with no impact on the stability of the operative segments. For patients of lumbar spinal stenosis with kidney deficiency syndrome, Duhuo Jishengtang can promote the recovery of postoperative neurological function, and improve the good efficacy rate of clinical treatment, but with no obvious advantage in pain relieve after operation.

ABSTRACT

Objective:To collect and analyze the non-compliance/protocol violation or deviation reported to the hospital Ethics Committee from 2017 to 2019, to explore the pattern of such events, as well as the causes and related logic. Empirical data evidence, lessons learned and preliminary advice were proposed to improve the understandings, management and implementation of clinical trials regarding to the non-compliance/protocol violation or deviations.Methods:The chi-square test and non-parameter test were conducted using SPSS 22.0.A total number of 754 reports received were analyzed, compared and summarized to provide the preliminary advice.Results:From 2017 to 2019, 754 reports on non-compliance/protocol violation or deviation, which covered 219 projects from 29 clinical departments, were received. 88% of these projects were drug/medical device clinical trials, while other 12% were investigator initiated trials. The reportrd events could be divided into 7 categories. The distribution of project resources, trial types and clinical specialties are significantly different. Meanwhile, the reports submitted by investigators have a lot of space for improvement.Conclusions:There were significant differences in the type and number of reported events on non-compliance/protocol violation or deviation between different trial types, as well as clinical trial specialties. The application report of protocol violation is generally poor. Combined with the working practice, the common reasons for the protocol violation are " the low overall compliance of the subjects" and " the insufficient compliance of the researchers" . Preliminary suggestions for management reform, such as increasing the training for researchers, improving the reporting mode, and regularly issuing follow-up and review research reports are recommended.

ABSTRACT

Objective:To compare the tropism of different adeno-associated virus (AAV) serotypes in retinal cells.Methods:The plasmids pFastBacDual-inCap and pFastBacDual-ITR-CMV-EGFP were constructed for AAV packaging with the baculovirus expression system.Recombinant adeno-associated virus type 2(rAAV2), 6, 8 and 9 serotypes were packaged, and the infectivity of rAAV was evaluated by infecting HEK293T cells at multiplicity of infection(MOI)2000.Twenty-five C57BL/6 mice were divided into five groups, with five mice per group.In the three experimental groups, both eyes of each mouse were injected 1 μl rAAV intravitreally, and 1 μl phosphate buffered saline (PBS) for the eyes of the control group.Two weeks after injection, the retinal tissues were collected for preparing flat mounts and cryosections.Enhanced green fluorescent protein (EGFP) gene expression was observed via fluorescence microscopy and laser scanning confocal microscopy.The study protocol was approved by the Ethics Committee of Suzhou Institute of Biomedical Engineering and Technology.Results:The infection efficiency of the recombinant virus to HEK293T cells was rAAV2>rAAV6>rAAV8>rAAV9, and the transduction efficiency was 39.5%, 18.4%, 8.7% and 4.6%, respectively.In mouse retinal transduction, rAAV2 and rAAV6 were highly expressed in the ganglion cells, and rAAV8 and rAAV9 were highly expressed in the retinal pigment epithelium (RPE) and photoreceptor cells.rAAV2-mediated EGFP expression in retinas was stable within three months after injection.Conclusions:Different rAAV serotypes have varying tropism and transduction efficiencies in retinal cells through intravitreal injection, rAAV2 has a high transduction efficiency and it can be stably expressed in retinas within three months after injection.

ABSTRACT

Objective To evaluate the clinical effects of vitrectomy with or without fovea-sparing internal limiting membrane peeling on macular foveoschisis (MF) secondary to pathologic myopia.Methods A non-randomized controlled study was adopted.Twenty-three patients (25 eyes) with refractive error ≥-8.00 DS and MF either combined with foveal retinal detachment or epi-macualr membrane or lamellar macular hole.The subjects were divided into non-internal limiting membrane peeling group (11 patients / 11 eyes) who underwent triamcinolone (TA) assisted vitrectomy and fovea-sparing internal limiting membrane peeling group (12 patients/14 eyes) who underwent TA assisted vitrectomy with fovea-sparing internal limiting membrane peeling.The baseline data such as age,best corrected visual acuity (BCVA),refractive error,axial length were not significant difference between the two group.Main outcomes were BCVA,remission of MF defined by optical coherence tomographyc OCT as well as complications.This study followed the Declaration of Helsinki and this protocol was approved by Ethic Committee of Xiamen Eye Center of Xiamen University (NO.XMYKZX-2016-YWS-007).Results All patients completed follow-up for more 6 months.BCVA (LogMAR) was 0.47±0.30 in non-internal limiting membrane peeling group and 0.40-±0.33 in fovea-sparing internal limiting membrane peeling group,showing no significant difference between the two groups (t =0.66,P=0.52).Complete remission of MF was achieved in 22 eyes.The remission time in non-internal limiting membrane peeling group was 2.5 (1.8,9.3) months,and 1.0 (1.0,3.8) months in fovea-sparing internal limiting membrane peeling group,no statistical significance was obtained between the two groups (U =35.00,P =0.09).One eye had post-operative macular hole in non-internal limiting membrane peeling group,accounting for 9%,while in fovea-sparing internal limiting membrane peeling group,one eye had macular hole,accounting for 7%,and one eye had rhegmatogenous retinal detachment post-operatively,accounting for 7%.Conclusions MF can be resolved by vitrectomy while complete remission can achieved more quickly when combined with fovea-sparing internal limiting membrane peeling.

ABSTRACT

Source control is the core of intra-abdominal infections (IAIs) treatment, in which precise positioning and effective drainage is the key and aporia. On the basis of membrane anatomy theory and understanding of anatomic structural abnormality under IAIs, the authors propose the concept of 'abdominal partition under IAIs’ in order to locate the source of infection and design of safe drainage path precisely, which could improve the diagnosis and treatment of IAIs and the prognosis.
