ABSTRACT
Under the background of performance evaluation in nationwide tertiary public hospitals and the DRGs reform, how to improve the efficiency of daytime surgery management is becoming a hotspot for hospital managers. Since July 2020, a tertiary general hospital applied the theory of incentive compatibility to guide the medical workers to consciously strive to achieve such management goals as improving the quantity and quality of daytime surgeries by constructing an organizational management system, increasing performance rewards for daytime surgical teams, rewarding advanced collectives and individuals, convening work coordination and promotion meetings, formulating penalty terms, and strengthening supervision of surgical quality and safety indicators. The implementation rate of daytime surgery in hospitals, the number of departments conducting daytime surgery, the number of covered diseases, and the satisfaction rate of inpatients had increased from 6.94%, 6 departments, 64 diseases, and 90.5% in 2019 to 24.08%, 21 departments, 125 diseases, and 95.0% in 2022, respectively. The incidence of daytime surgical bleeding, and readmissions within 15 days decreased from 0.6% and 0.5% in 2019 to 0.5% and 0.2% in 2022, respectively. The theory of incentive compatibility was in line with the development goals of daytime surgery in China, providing an optimal strategy for improving the management efficiency of daytime surgery according to local conditions.
ABSTRACT
OBJECTIVES@#To compare the root length of maxillary and mandibular anterior teeth between open bite patients and normal overbite patients via cone-beam computed tomography (CBCT) as well as the root length of anterior teeth in different degree of open bite patients, and to analyze the correlation between the degree of open bite and root length.@*METHODS@#A total of 106 untreated patients were enrolled retrospectively (53 anterior open bite patients and 53 normal overbite patients).Three-dimensional position of the CBCT image for the patient's teeth was performed using Dolphin software. The median sagittal plane of the tooth was used as the measurement plane, and the line connecting the apical point and the midpoint between the labial and lingual cementoenamel junctions was defined as the root length. Independent @*RESULTS@#Significant differences were found in the root length of maxillary and mandibular anterior teeth between the open bite group and the normal overbite group (@*CONCLUSIONS@#The root length of maxillary and mandibular anterior teeth in the open bite patients is shorter than that in the normal overbite patients, and the severer the degree of open bite, the shorter the root length of the mandibular central incisor tends to be. There is a certain relationship between maxillary anterior teeth and the root length of anterior teeth.

Objective:To analyze the infection results of The immunofluorescence detection of respiratory syncytial virus (RSV) in huazhou region from 2009 to 2010.Methods: The N gene sequence of RSV was taken as a reference to design specific primers and TaqMan fluorescent probe and to establish real-time The immunofluorescence method to detect sensitivity and specificity.21548 swab samples of children with acute respiratory infections in huazhou region from 2009 to 2010 were tested with real-time fluorescence. The immunofluorescence method. Results: The specificity and sensitivity of real-time fluorescence. The immunofluorescence method for detection of RSV infection were higher, without cross-reactivity to other respiratory viruses.10,477 cases of clinical specimens in 2009 were detected, with positive RSV infection rate of 9.73%, by real-time The immunofluorescence method detection. Positive detection rate of RSV infection had two peaks, which lied in February and December; the positive rate was 11% and 11% respectively. RSV infection rate was the highest for children under 6 years old. Conclusion: Real-time fluorescence features convenient detection, high specificity, and high sensitivity, which can be used for clinical detection of RSV infection. From 2009 to 2010, RSV infection remains a major pathogen of acute respiratory infections in children in huazhou region.

Objective To quantitatively assess severity of movement disability by analyzing physical activities recorded by an actigraph monitor in patients with neurology disorders. Methods Eighty-one patients with Parkinson' s disease(PD)and 61 patients with acute cerebral infarction(ACI)accompanying impaired upper limb motor function were included in the study.PD patients and ACI patients were treated using the international PD and ACI treatment guidelines, respectively. The patients were asked to wear an Actigraph monitor for 6 days before the treatment in both PD and ACI patient groups and at 24-38 days post-treatment in PD patients or at 28 days post-treatment in ACI patients. The recorded data was analyzed by power-law exponent(PLE)and detrended fluctuation analysis(DFA).Clinically, before and after the treatments,PD patients were evaluated using the conventional Unified Parkinson Disease Rating Scale (UPDRS),and ACI patients were evaluated by assessing upper limb motor function using Fugl-Meyer Assessment(FMA)and Functional Independence Measure(FIM).The correlation of the UPDRS scores with PLE was analyzed in PD patients, and the correlation of FMA or FIM with DFA in ACI patients. Results Both the UPDRS scores and the PLE values in PD patients were improved after the drug administration(UPDRS total:32.8 ± 16.2 and 28.8 ± 14.7,Z =2.080,P =0.038; UPDRS Ⅲ:18.6 ± 8.2 and 15.7±6.8,Z=2.155,P=0.031; PLE:0.98 ±0.25 and 0.82 ±0.21,Z=2.212,P=0.027,before and after the treatment, respectively). There were a linear correlation coefficient of 0.699 between the improvements of total UPDRS scores and the PLE values, and of 0.823 between the UPDRS Ⅲ and the PLE values.FMA,FIM scores and DFA were improved significantly than before treatment(FMA:12.39 ± 8.21 and 30.28 ±7.29,Z=3.016,P =0.004; FIM:8.98 ±7.29 and 13.21 ±7.6,Z =2.282,P=0.038; DFA:0.86 ±0.31 and 0.98 ±0.27,Z =2.360,P =0.036,before and after the treatment, respectively). It also showed linear correlations between the improvements of FMA scores and DFA(r =0.638),and between FIM scores and DFA(r =0.712,both P <0.05).There was no correlation between UPDRS scores and DFA values in PD patients, nor between FIM scores or FMA scores and PLE values in ACI patients. Conclusions Actigraph device can be used to monitor patients activity in movement disorders.Analysis of its PLE can provide a quantitative evaluation in PD while its DFA may provide useful specific assessment of impaired upper limb motor function in ACI patients. It can also be used in quantitatively assessing new drug efficacy.
