ABSTRACT
Objective:To investigate the prevalence of Helicobacter pylori infection among family members, and analyze associated risk factors. Methods:The current investigation was a cross-sectional study. The Qinghai region was stratified into urban areas, agricultural areas, and pastoral areas. The urban areas of Xining City, the agricultural areas of Haidong City, and the pastoral areas of Haibei Tibetan Autonomous Prefecture were selected. A total of 396 resident families (1 131 people) who underwent health checkups from 2021 to 2022 in the above areas were included in the survey study. Questionnaires were administered and H. pylori infection was detected using the 13C-urea breath test. Numerical data were expressed as cases and percentages, and the Chi-square test was used to compare differences in H. pylori infection rates in the populations and families in each group. Multifactorial logistic regression was used to analyze risk factors for H. pylori infection, and P<0.05 was considered statistically significant. Results:The prevalence of H. pylori infection in Qinghai province was 52.8% (597/1 131) and the prevalence of H. pylori infection in households was 80.6% (319/396). In H. pylori-positive households with at least 1 infected spouse, 40.4% (36/89) had only 1 infected spouse, and in 59.6% (53/89) both spouses were infected. In analysis of children infected by parents with H. pylori, 20.0% (9/45) of households had fathers and children infected, 48.9% (22/45) had mothers and children infected, and 31.1% (14/45) had both parents and children infected. In univariate analysis there was a statistically significant difference in the overall comparison of H. pylori infection rates among families with different numbers of people living together ( χ2=11.12, P=0.004), and between-group comparisons suggested that H. pylori infection rates were higher in families with 4 or 5 people and more than 5 people living together than in families with 2 or 3 people living together. The H. pylori infection rate was higher in families that did not use serving chopsticks and spoons during family meals than in families that did use serving chopsticks and spoons ( χ2=6.12, P=0.013). In multifactorial logistic regression analyses the number of people living together in a family and whether or not serving chopsticks and spoons were used at family meals were associated with H. pylori infection ( P<0.05). Conclusion:The H. pylori infection rate in families in Qinghai Province is high, and there is a clear association with family aggregation. It is more common for both members of a couple to be infected, and H. pylori infection of a mother has a greater effect on the children′s infection status than H. pylori infection of a father. The infection rate of H. pylori was lower in families that used serving chopsticks and spoons during dinner gatherings, and the fewer the number of people living together in the family, the lower the H. pylori infection rate.
ABSTRACT
ObjectiveThe mechanisms underlying therapeutic efficacies of Detumescence Analgesic Plaster was analyzed based on "effect-target" associations. MethodBased on CNKI and PubMed databases, the chemical components of Artemisia seed, bastard speedwell, and menthol in Detumescence Analgesic Plaster were collected. The capacity of transdermal absorption was predicted based on the Encyclopedia of Traditional Chinese Medicine (ETCM 2.0). Golden Triangle of compounds with Accepted used for candidate target prediction based on the Integrative Pharmacology-based Research Platform of Traditional Chinese Medicine (TCMIP v2.0)according to the similarity of chemical structures. At the same time, the SoFDA data platform was employed to collect the symptoms related to the efficacy of Detumescence Analgesic Plaster and its related genes information. In addition, based on the interaction between the above-mentioned candidate targets and their efficacy-related genes, the "effect-target" interaction network of Detumescence Analgesic Plaster was constructed. The key targets by topological features calculation, and functional mining was carried out to explain the efficacy mechanism of Detumescence Analgesic Plaster. ResultA total of 165 candidate targets were obtained based on ETCM 2.0 and TCMIP v2.0 databases, and symptoms related to the efficacy of clearing heat, detumescence, and relieving pain, as well as 1 744 related genes were collected based on the SoFDA database. Network construction and analysis showed that the core effect targets of Detumescence Analgesic Plaster were mainly involved in regulating the "immune-inflammation" balance of the body and maintaining the homeostasis of material and energy metabolism, blood circulation, and nervous system functions, and they were closely related to the efficacy of this prescription in clearing heat, reducing detumescence, and relieving pain. Among them, the heat clearing group of Detumescence Analgesic Plaster had the functions of heat clearing, detoxifying, antibacteria, and anti-inflammation. The biological function of its key effect target group was related to correcting the imbalance of "immune-inflammation" induced by pathogens. The detumescence group of Detumescence Analgesic Plaster had the functions of reducing water and swelling and resolving hard lumps, and the biological function of its core effect target group was related to improving microcirculation disturbance. The pain relieving group of Detumescence Analgesic Plaster had the functions of removing stasis, promoting blood circulation, and relieving pain, and its core effect target group was related to correcting the nervous system and the disorder of material and energy metabolism. ConclusionThe heat clearing, swelling reducing, and pain relieving effects of Detumescence Analgesic Plaster may be closely related to its act on related candidate targets, so as to correct the imbalance of "nerve-immunity-vascular-axis", regulate neuronal excitability and inflammatory response, and intervene in material and energy metabolism. The relevant research results lay a theoretical foundation for clarifying the advantages of Detumescence Analgesic Plaster and assisting its clinical precise positioning.
ABSTRACT
Estrogen impacts neural development; meanwhile, it has a protective effect on the brain. Bisphenols, primarily bisphenol A (BPA), can exert estrogen-like or estrogen-interfering effects by binding with estrogen receptors. Extensive studies have suggested that neurobehavioral problems, such as anxiety and depression, can be caused by exposure to BPA during neural development. Increasing attention has been paid to the effects on learning and memory of BPA exposure at different developmental stages and in adulthood. Further research is required to elucidate whether BPA increases the risk of neurodegenerative diseases and the underlying mechanisms, as well as to assess whether BPA analogs, such as bisphenol S and bisphenol F, influence the nervous system.

ABSTRACT
Bronchial asthma is the most common chronic disease in childhood. In clinical diagnosis and treatment, it is found that some children with asthma often have neuropsychiatric disorders of different severity, such as autism spectrum disorders, attention deficit-hyperactivity disorders, panic disorders and anxiety, which render the prognosis and treatment of asthma difficult. Some reports suggest that the " lung-brain axis" of bronchial asthma is related to the outbreak of inflammation-related mechanisms.A new idea to improve bronchial asthma with neuropsychiatric diseases may be inhibiting the release of inflammatory factors and blocking lung-brain inflammation communication. As one of the more thoroughly studied inflammasome family members, NOD-like receptor pyrin domain-containing protein 3 (NLRP3) is widely involved in the mechanisms of neuroinflammation and bronchial asthma attacks. In this article, the role of NLRP3 in the " lung-brain axis" immune inflammation mechanism of bronchial asthma is reported, which may provide new ideas for the diagnosis, treatment and research of bronchial asthma and neuropsychiatric comorbidities.
ABSTRACT
Diabetic retinopathy is one of the serious microvascular diseases caused by diabetes. It is the leading cause of visual impairment among workers over 40 years old in developed countries. At present, western medicine methods for treating diabetic retinopathy include pan-retinal photocoagulation, vitrectomy, and intravitreal injection of anti-vascular endothelial growth factor and other methods. Traditional Chinese medicine treatment of diabetic retinopathy is mainly to treat patients using oral Chinese herb preparation based on syndrome differentiation and using certain external traditional Chinese medicine methods, such as auricular point therapy. This paper investigates the research progress of diabetic retinopathy treatment with traditional Chinese and western medicine and provides novel ideas for treating diabetic retinopathy with traditional Chinese combined with western medicine.

ABSTRACT
Objective To investigate the noninvasive indicators of indications for antiviral therapy in HBeAg-negative chronic hepatitis B virus (HBV) infection patients with alanine aminotransferase (ALT) ≤40 U/L under the guidance of liver pathology. MethodsA retrospective analysis was performed for the clinical data of 377 HBeAg-negative chronic HBV infection patients with ALT ≤40 U/L who were hospitalized in Affiliated Hospital of Yan’an University, from October 2013 to August 2018 and underwent liver biopsy, among whom the patients with inflammatory activity <A2 and fibrosis stage <F2 were enrolled as non-antiviral therapy group(n=266), and the patients with inflammatory activity ≥A2 or fibrosis stage ≥F2 were enrolled as antiviral therapy group(n=111). The chi-square test was used for comparison of categorical data between two groups; the t-test was used for comparison of normally distributed continuous data between two groups, and the Mann-Whitney U test was used for comparison of non-normally distributed continuous data between two groups; univariate and multivariate binary logistic regression analyses were used to screen out the influencing factors for the initiation of antiviral therapy; the receiver operating characteristic (ROC) curve was plotted to evaluate the diagnostic efficiency of each indicator in determining the need for antiviral therapy in HBeAg-negative chronic HBV infection patients with ALT ≤40 U/L. ResultsOf all 377 patients, 266 (70.6%) did not need antiviral therapy for the time being, and 111 (29.4%) had marked liver damage and thus needed active antiviral therapy. The multivariate analysis showed that liver stiffness measurement (LSM) (odds ratio [HR]=2.003, 95% confidence interval [CI]: 1.647-2.437, P<005), HBsAg (HR=1.563, 95% CI: 1.110-2.200, P<0.05), HBV DNA (HR=1.519, 95% CI: 1173-1.966, P<0.05), and albumin (HR=0.939, 95% CI: 0.884-0.998, P<0.05) were independent influencing factors for the initiation of antiviral therapy. The ROC curve analysis showed that the area under the ROC curve (AUC) was 0.749 (95% CI: 0.699-0799) for LSM, 0642 (95% CI: 0.586-0.699) for HBV DNA, and 0.565 (95% CI: 0.507-0.623) for HBsAg, and the combination of LSM, HBV DNA, and HBsAg had a larger AUC of 0.779 (95% CI: 0.732-0.827). ConclusionThe levels of LSM, HBV DNA, and HBsAg have a reference value in determining the initiation of antiviral therapy in HBeAg-negative chronic HBV infection patients with ALT≤40 U/L.
ABSTRACT
Hepatocellular carcinoma (HCC) is one of the most common malignant tumors in the world. At present, the treatment methods for HCC mainly include surgical treatment, local regional treatment, radiotherapy, systematic treatment, and traditional Chinese medicine treatment. This article introduces the indications for surgical treatment, surgical procedures, and the advantages of postoperative adjuvant therapy, analyzes the indications for liver transplantation and its future application in clinical practice, and describes the basic principles, optimal indications, and technical advantages of mature and new ablation techniques at present. It is pointed out that for patients with early-stage HCC, the advantages and shortcomings of each treatment regimen should be fully understood to select a precise treatment regimen.
ABSTRACT
At present, hepatitis B virus (HBV) infection is recognized as an important risk factor for hepatocellular carcinoma (HCC) in the world; however, during the development and progression of hepatitis B, liver cirrhosis, and liver cancer, other factors may promote the development of HCC independently or synergistically with HBV, such as sex, age, family history, alanine aminotransferase/aspartate aminotransferase, smoking and drinking history, metabolic syndrome, and HCV or HIV infection. This article reviews the research advances in the risk factors associated with HCC.
ABSTRACT
OBJECTIVE: To study release behavior in vitro of Risperidone sustained-release tablets and its pharmacokinetics in rabbits. METHODS: Risperidone sustained-release tablets were prepared by using mesoporous silica as matrix. Release rates in vitro within 12 h (Q12 h) of commercially available Risperidone tablets, Risperidone sustained-release tablets and its physical mixture in 0.1 mol/L HCl fluid were investigated with basket method. The release model of Risperidone sustained-release tablets were fitted. Using clozapine as an internal standard, HPLC method was used to determine blood concentration of risperidone and 9-hydroxyrisperidone in rabbits 48 h (n=6) after intragastric administration of commercially available Risperidone tablets and Risperidone sustained-release tablets 2 mg. Pharmacokinetic parameters were calculated by using non-compartmental model of Kinetica 4.4 software. RESULTS: Compared with commercially available Risperidone tablets (Q12 h=97%) and physical mixtures (Q12 h=95%), release rate of Risperidone sustained-release tablets (Q12 h=83.7%) slowed down significantly, and the release of Risperidone sustained-release tablets in 0.1 mol/L HCl fluid was closed to first-order release (R2=0.998 9), with diffusion as the main factor and dissolution as the supplement. By risperidone, the pharmacokinetic parameters of commercially available Risperidone tablets and Risperidone sustained-release tablets included that t1/2 were (4.64±0.93),(6.65±0.92) h; cmax were (34.46±7.75) and (8.57±6.91) ng/mL; MRT were (11.48±1.23), (17.46±2.10) h; AUC0-48 h were (314.39±10.33),(192.98±49.14) ng·h/mL, respectively. By 9-hydroxyrisperidone, the pharmacokinetic parameters of them included that t1/2 were(7.08±0.93),(10.45±0.78) h; cmax were (98.08±5.43),(54.55±4.88) ng/mL; MRT were (11.48±1.23), (17.46±2.10) h; AUC0-48 h were (894.71±131.15), (1 227.99±112.12) ng·h/mL (n=6), respectively. Compared with commercially available Risperidone tablets, t1/2 and MRT of Risperidone sustained-release tablets prolonged significantly, while cmax decreased significantly (P<0.05). CONCLUSIONS: Risperidone loaded in mesoporous silica has sustained release effect and prolong the time of drug efficacy.
ABSTRACT
To investigate the value of liver stiffness measurement (LSM) in the diagnosis of liver fibrosis degree in HBeAg-positive patients with chronic HBV infection. Methods A retrospective analysis was performed for the clinical data of 330 HBeAg-positive patients with chronic HBV infection who were hospitalized in Affiliated Hospital of Yan’an University from October 2013 to August 2018 and underwent liver biopsy, and according to liver pathological results, these patients were divided into mild liver fibrosis group (F0-F1) and significant liver fibrosis group (F2-F4). The t-test or the Mann-Whitney U test was used for comparison of continuous data between two groups, and the chi-square test was used for comparison of categorical data between two groups. Univariate and multivariate analyses were performed to screen out the indices for the diagnosis of significant liver fibrosis, and the receiver operating characteristic (ROC) curve was plotted for these indices to evaluate their value in the diagnosis of significant liver fibrosis. Results Of all 330 patients, 245 had mild liver fibrosis and 85 had significant liver fibrosis. There were significant differences between the two groups in HBV DNA, HBsAg, HBeAg, HBcAb, alanine aminotransferase, aspartate aminotransferase, total bilirubin, and LSM (all P<0.05). The multivariate analysis showed that HBsAg and LSM were independent risk factors for significant liver fibrosis (both P<0.05). The ROC curve analysis showed that only LSM had a diagnostic value, with an area under the ROC curve of 0.744 (95% confidence interval: 0.680-0.808). At the optimal cut-off value of 6.15 kPa, LSM had a sensitivity of 62.4%, a specificity of 76.3%, an accuracy of 72.1%, a positive predictive value of 72.5%, and a negative predictive value of 67.0% in predicting significant liver fibrosis. ConclusionLSM has a good value in predicting significant liver fibrosis in HBeAg-positive patients with chronic HBV infection.
ABSTRACT
Objective: To estimate the prevalence of HIV/HCV co-infection and explore the influence factors and their interaction on HIV/HCV co-infection of patient's access to methadone maintenance treatment (MMT). Methods: A face to face interviews were conducted among 750 patients at two MMT clinics in Guangxi Zhuang Autonomous Region. The questionnaires information included demographic characteristics, HIV and HCV infection status, history of drug abuse, urine test for morphine, high risk sex behaviors, needle sharing, dropped out etc. Methods of χ(2) test one-way, multivariate logistic regression and interactions were used to analyze the related factors of HIV/HCV co-infection. Results: The study subjects included 750 participants, 18.31% (127/691) of patients were co-infected with HIV and HCV. The HIV/HCV co-infection rate in patients who shared needles with others or dropped out of treatment was 35.84% (81/226) and 19.88% (64/322) respectively, which were higher than those who have never shared needles or dropped out (9.89%, 46/465 and 17.07%, 63/369). Logistic regression analysis results showed that after adjusted for confounding factors, patients who shared needles (OR=4.50, 95%CI: 2.72-7.43) and dropped out of treatment (OR=1.71, 95%CI: 1.04-2.80) were more likely to be infected with HIV/HCV. Interaction analysis showed that sharing needles and dropping out of treatment exist additive effect on co-infection of HIV and HCV (RERI=4.21, AP=0.44, SI=1.95). Conclusions: Needle sharing and dropping out of treatment are associated with HIV/HCV co-infection. Health education, psychological counseling and other measures should be taken to reduce needle sharing and dropping out of MMT.

BACKGROUND: A large number of cytokines produce in the periodontium on the tension side during orthodontic tooth movement, participate in periodontal remodeling, and make the tooth move to its normal position, thus achieving alveolar remodeling. Heat shock protein 27 (HSP27) is reported to be involved in the periodontal remodeling. OBJECTIVE: To investigate the expression of HSP27 in the rat periodontium on the tension side during the orthodontic tooth movement.METHODS: The rat models of orthodontic tooth movement were constructed using NiTi coil spring with the force value of 50 g. The periodontal samples were collected at 1, 3, 5, 7, 10 and 14 days after exerting force to detect the expression level of HSP27 in the periodontium on the tension side.RESULTS AND CONCLUSION: Immunofluorescence staining results revealed that the expression level of HSP27 in the periodontal tissues began to increase at the 1st day after exerting force, reached the highest level at the 3rd day, and then fell to the initial level at the 5th day. To conclude, HSP27 is evoked by orthodontic force early, and it may play a role in periodontal remodeling.
ABSTRACT
Objective To explore the Hobiletin improving cognitive function and the possible mechanism to protect the hippocampal neurons. Methods Eight of Wistar rats,SPF,12 weeks of age, male, were set to the normal group;Eight of GK male rats,SPF,12 weeks of age, only for diabetes group; Both of the two groups were given the normal feed.6 of GK male rats,12 weeks of age,to feed Hobiletin of orange peel 10 mg/kg and lavage for 8 weeks, were set to the NOB group. After 22 weeks the rats were tested by Morris water maze behavior experiment for test the ability of learning and memory;the expression of Nrf-2 was detected by immunohistochemistry and Western Blot. Results Compared with the normal group, the incubation period of diabetes group was longer (P<0.05),and theescape latency of NOB group was shorter than that of diabetes group (P<0.05).Compared with the normal group, the residence time in the target quadrant was shorter in the diabetes group (P<0.05),and prolonged in the NOB group than in the diabetes model group (P<0.05).The expression of Nrf-2 in diabetes group was significantly higher than that in diabetes group (P<0.05).The expression of NH-2 in NOB group was significantly higher than that in diabetes group (P<0.05).Conclusion NOB can increase the expression of Nrf-2 in hippocampus and improve the cognitive function of diabetic rats, which may be related to the enhancement of Nrf-2 activity and the activation of anti-oxidative stress pathway.
ABSTRACT
BACKGROUND:Extensive research has been concentrated on the reconstruction of periodontium under orthodontic force. However, there are no reports regarding the expression of heat shock protein 70 (HSP70) in rat periodontium during orthodontic tooth movement. OBJECTIVE:To investigate the expression of HSP70 in rat periodontium during orthodontic tooth movement. METHODS:Rat orthodontic tooth movement models were established using NiTi extension spring with the force of 50 g. The orthodontic periodontium was col ected at 1, 3, 5, 7, 10 and 14 days after exerting force to detect the expression of HSP70. RESULTS AND CONCLUSION:According to immunofluorescence staining, the expression of HSP70 in the periodontal tissues was increased on day 1 after exerting force, and reached the highest level on day 5, then decreased to the initial level on day 14. These results suggest that the expression of HSP70 is evoked by orthodontic force, and it may play a role in periodontal remodeling.
ABSTRACT
Metabolic memory means if the hyperglycemia can't be controlled at early stage of diabetes, chronic complications such as diabetic retinopathy (DR) will continue to develop even if the blood glucose level maintains normal level at later stage.Oxidative stress plays an important role in the metabolic memory of DR,which interacts with the nitrative stress, advanced glycation end products, genetic modification and endoplasmic reticulum stress in the pathogenesis of DR.Further elucidation of the relationship between oxidative stress and metabolic memory of DR can open the way for the discovery of novel therapeutic targets to prevent DR progression.
