ABSTRACT

Objective To evaluate the influence of stress distributions on bone-anchored maxillary protraction at different protraction sites, so as to guide patients to choose an optimal protraction site in clinic. Methods A three-dimensional (3D) finite element model of child head with implant anchorages was establised. Four protraction sites were set according to the position of implant installation. Working condition 1: the alveolar bone at the intersection of distal 2 mm of primary lateral incisor crown distal surface and gingival cervical margin to 5 mm. Working condition 2: the alveolar bone at the intersection of mesial 2 mm of maxillary first primary molar crown mesial surface and gingival cervical margin to 5 mm. Working condition 3: the alveolar bone at the intersection of mesial 2 mm of maxillary first molar crown mesial surface and gingival cervical margin to 5 mm. Working condition 4: the alveolar bone at the intersection of distal 2 mm of maxillary first molar crown distal surface and gingival cervical margin to 5 mm. The finite element models were loaded with 500 g protraction force at each side with 30° forward direction to the occlusal plane. Stress distributions on each suture were analysed. Results The maximum stress of frontomaxillary suture was in working condition 2 (1 477-28 190 Pa). The maximum stress of nasomaxillary suture was in working condition 1 (5.296-924 Pa). The maximum stress of zygomaticomaxillary suture was in working condition 4(394.7-13 130 Pa). The maximum stress of zygomaticofrontalis suture was in working condition 4 (495.2-31 690 Pa). The maximum stress of zygomaticotemporal suture was in working condition 3 (1 148-15 870 Pa). The maximum stress of medianpalatine suture was in working condition I (6.479-730 Pa). Conclusions When the protraction sites are set in distal maxillary primary lateral incisor and mesial maxillary first primary molar, it is of positive significance to improve the concave profile, especially in nose root. When the protraction sites are set in mesial or distal maxillary first molar, it is of positive significance to improve the concave profile, especially in maxillary basal bone of the midface.

ABSTRACT

Objective:To study the efficacy and safety of Methazolamide(MTZ)for the treatment of normal pressure hydrocephalus(NPH)patients.Methods:A randomized, double-blind, placebo-controlled, prospective clinical study was conducted in Aviation General Hospital.A total of 35 NPH patients including 29 idiopathic normal pressure hydrocephalus(iNPH)and 6 secondary normal pressure hydrocephalus(sNPH)received drug treatment in our hospital from September 2019 to March 2021.All patients were unsuitable for or refused surgical treatment for some reasons.The patients were divided into drug group(n=18)and control group(n=10), taking oral MTZ or placebo 25 mg twice daily, increasing to 50 mg twice daily after 1 week if there was no discomfort.The 10 m gait score, cognitive function score, brain MRI check were completed before and 1 month after oral administration.The assessment of idiopathic normal pressure hydrocephalus scale(iNPHGS)score were performed 1 month and 3 months after oral administration.The primary efficacy endpoint was iNPHGS score for 3 months treatment and the secondary efficacy endpoint was the assessment of above scales for 1 month treatment.Results:As compared with baseline, the effect of 1 month treatment showed that MOCA scores[(16.2±8.8)and(14.8±8.7)scores, t=-2.68, P=0.02], 10 m gait scores[(22.3±11.2)and(25.6±12.9), t=2.76, P=0.02], iNPHGS scores[(7.3±3.2)and(8.1±3.5), t=4.08, P<0.01]were improved.The effect of 3 month treatment showed that the iNPHGS score(6.1±2.4)was improved compared with baseline( t=5.07, P<0.01)and 1 month( t=4.11, P<0.01). But the above scores of the control group were not significantly improved compared with the baseline(all P>0.05). After 1 month treatment, the 10 m gait score and iNPHGS score in the drug group were improved compared with those in the control group(all P<0.05). After 3 months treatment, the iNPHGS score was improved compared with the baseline level in the control group( t=-4.41, P<0.05). The above 35 patients had no serious adverse reactions such as hypokalemia and acidosis.There was no significant difference in adverse events between the two groups( χ2=0.01, P=1.00). Conclusions:The treatment of MTZ could effectively improve the clinical symptoms of NPH patients with good safety.

ABSTRACT

Objective:To investigate the features of gait disorders in patients with idiopathic normal pressure hydrocephalus(iNPH).Methods:Clinical data of 42 patients diagnosed with iNPH admitted to the neurological department of Aviation General Hospital from March 2017 to July 2020 were consecutively collected.The features of gait disturbance and activity of daily living were retrospectively studied.Results:Of the 42 patients, 29 cases(69%)were males, aged(73.0±8.3)years, and 13 cases were females, aged(73.0±8.3)years old.The mean course of disease was(3.5±3.0)years.There were 90.5% of patients(38/42)complained of gait disorders, and 61.9%(26/42)had fall histories.All patients had gait abnormalities.The time for 3m-Timed Up and Go(TUG)test were(27.8±17.0)seconds, and the proportion of patients with the time for 3m-TUG less than 10 seconds, 10~20 seconds and more than 20 seconds were 5.9%(2/34), 35.3%(12/34)and 58.8%(20/34), respectively.In the 10-meter walking test, 8 patients needed assistance.Of the 34 patients who could walk independently, 94.1%(32/34)had tandem walking disturbed and tendency toward falling, 76.5%(26/34)had turning disturbed, 44.1%(15/34)had wide based stride and small steps, 29.4%(10/34)had trunk balance disturbed, 23.5%(8/34)had reduced foot-floor clearance, and 8.8%(3/34)had start hesitation, 97.6%(41/42)had abnormal step counting and 90.5%(38/42)had abnormal time evaluation.The proportion of patients with activity of daily living(Barthel Index)scores of(mild dependence), moderate dependence and severe dependence were 71.4%(30/42), 28.6%(12/42)and 0%(0/42). Of the 30 patients with mild dependence, 56.7%(17/30)had fall histories.Conclusions:In our study, all patients have gait dysfunction, which causes some impact on activity of daily living and a higher risk of falls.Gait slowness, tandem walking disturbed, turning disturbed, tendency toward falling, wide based stride and small steps are the most involved gait characteristics.Early assessment of gait disorders is of great significance in early diagnosis and the prevention of falls and living ability decline.

ABSTRACT

Objective:To investigate the expression of serum microRNA (miR)-92 in patients with liver cancer and its relationship with poor prognosis.Methods:The clinical data of 70 patients with liver cancer in the First People′s Hospital of Huzhou City, Zhejiang Province and the Central Hospital of Huzhou City, Zhejiang Province from May 2015 to May 2018 were retrospectively analyzed. The miR-92 expression level was detected by real-time quantitative polymerase chain reaction method and compared with that of 80 healthy subjects. Receiver operating characteristic (ROC) curve was drawn to evaluate the efficacy of serum miR-92 in diagnosing liver cancer and early stage (stage Ⅰ to Ⅱ) liver cancer. The relationship between serum miR-92 expression level and clinicopathological characteristic was analyzed. Independent risk factors affecting the prognosis in patients with liver cancer were analyzed by multivariate Cox regression model.Results:The expression level of serum miR-92 in patients with liver cancer was significantly higher than that in healthy subjects (4.10 ± 1.74 vs 1.88 ± 0.78), and there was statistical difference ( P<0.05). ROC curve analysis result showed that the area under curve (AUC) of serum miR-92 for the diagnosis liver cancer was 0.874, the best cut-off value was 2.43, the sensitivity was82.86%, and the specificity was 86.25%; the AUC of serum miR-92 for the diagnosis early liver cancer was 0.755, the best cutoff value was 2.38, the sensitivity was 68.57%, and the specificity was 84.42%. The serum miR-92 expression level was related to TNM stage and lymph node metastasis ( P<0.01), but not related to gender, age, tumor diameter, history of hepatitis, liver cirrhosis, degree of differentiation and portal vein tumor thrombus ( P>0.05). The 5-year overall survival rate in liver cancer patients with high serum miR-92 expression was significantly lower than that in liver cancer patients with low serum miR-92 expression (17.1% vs. 31.5%), and there was statistical difference ( χ2 = 5.561, P<0.05). Multivariate Cox regression analysis result showed that miR-92 was an independent risk factor affecting the prognosis in patients with liver cancer ( HR = 0.282, 95% CI 1.179 to 3.562, P<0.05). Conclusions:The expression level of serum miR-92 in patients with liver cancer is significantly increased, which plays an important role in the progression of the disease. Serum miR-92 can be used as an indicator for early diagnosis and prognosis evaluation of liver cancer.

ABSTRACT

Objective:To explore the clinical characteristics and appropriate treatment of occult pneumococcal bacteremia (OPB) in children.Methods:The clinical characteristics, drug sensitivity and antibiotic use of 39 children with OPB who met the inclusion criteria in the Pediatric Internal Medicine Ward of Shenzhen Children′s Hospital from January 2013 to December 2018 were retrospectively analyzed.Results:The median age of onset in OPB in children was 2 years and 4 months, and 74.4% of children(29/39 cases) were between 6 months and 3 years.The average total fever duration was 3.69 days (1-14 days), and the average hospital stay was 6.74 days.Peripheral blood white blood cell count was >15×10 9/L in 89.7% (35/39 cases) of the children, C-reactive protein was increased in 76.9% (30/39 cases) of the children, and procalcitonin > 2 mg/L in 38.9% (14/36 cases) of the children.During hospita-lization, all the children received the treatment of intravenous antibiotics.The antibiotics used initially included Cefuroxime in 11 cases (28.2%), Amoxicillin sulbactam sodium in 10 cases (25.6%), Ceftriaxone sodium in 7 cases (17.9%) and Meloxicillin sulbactam in 6 cases (15.4%). The average fever clearance time after the antibiotic therapy was 1.4 days, and the average intravenous antibiotics treatment time was 6.2 days.The results indicated that the insensitivity rate of Penicillin was 46.2%, the insensitivity rate of Amoxicillin, Ceftriaxone and Cefotaxime were 22.2%, 10.3% and 17.9%, respectively, Erythromycin resistant rate was 100.0%, and no patient was resistant to Vancomycin and Linezolid. Conclusions:OPB occurs most frequently in infants aged from 6 months to 3 years old, with low drug resistance to Amoxicillin, Ceftriaxone and Cefotaxime.Children with complete immunity have shorter fever duration and good prognosis after antibiotic treatment.

ABSTRACT

OBJECTIVE:To optimi ze and improve the quality standard for Keqing capsules. METHODS :According to general rule 0502 method stated in 2015 edition of Chinese Pharmacopeia (part Ⅳ),TLC method was used to identify Reineckia carnea and Morus alba in Keqing capsules [the developing solvents were dichloromethane-ethyl acetate-formic acid (10 ∶ 4 ∶ 0.2,V/V/V) and ethyl acetate-carbinol-ammonia (12 ∶ 2 ∶ 1,V/V/V),respectively]. The contents of morphine and codeine phosphate in Keqing capsules were determined by HPLC. The determination was performed on XBridge C 18 column with mobile phase consisted of acetonitrile-0.01 mol/L potassium dihydrogen phosphate aqueous solution (pH value adjusted to 2.7 with 5% phosphoric acid solution)(5 ∶ 95,V/V)at the flow rate of 1.0 mL/min. The detection wavelength was set at 210 nm,and the column temperature was 35 ℃. The sample size was 10 µL. RESULTS :In TLC of R. carnea and M. alba in samples ,same color spots were shown in the correspon ding positions of reference substance chromatogram without interference from negative control. The linear range of morphine and codeine phosphate were batches of Keqing capsules were 0.97-1.37,0.16-0.37 mg/g,respectively. CONCLUSIONS :TLC identification method for R. carnea and M. alba ,as well as HPLC content determination method for morphine and codeine phosphate in Keqing capsules are established;the method is simple ,accurate and reliable with strong specificity ,which improves the quality standard of Keqing capsules.

ABSTRACT

Diffuse neurofibrillary tangles with calcification (DNTC) is a type of presenile dementia. The clinical features are temporal and/or frontal and/or extrapyramidal symptoms. Memory impairment which might be less prominent in early stages of disease becomes evident with progression; The imagings are featured with bilateral basal ganglia and/or cerebellar dentate calcification (Fahr?type calcification) and/or localized atrophy of temporal or temporofrontal lobes; The pathological features are diffuse neurofibrillary tangles without large number of A?amyloid plaques (characteristic of Alzheimer′s disease) and without Pick body (characteristic of Pick disease), and calcium deposition is similar to Fahr′s disease in sites but minor in degree. Till now, most case reports are from Japan. It is speculated that there are a large number of misdiagnosis in other countries. Criteria for clinical diagnosis of DNTC was proposed by Iwai K(1995), Kosaka H(2001), Ukai K and Kosaka K (2016, 2017) successively, which can facilitate the diagnosis.

Objective@#To study the relationship between STAT3 gene (rs2293152, rs957970) polymorphism and susceptibility to bronchial asthma in children, and to lay the foundation for search of the candidate pathogenic gene of asthma and early prevention of the disease.@*Methods@#One hundred and thirty hospitalized asthmatic children or out-patients with asthma at Affiliated Hospital of Southwest Medical University from September 2013 to September 2017 were selected as asthmatic group, and at the same time 145 healthy children were selected from the Affiliated Hospital of Southwest Medical University as the healthy control group.Apply sequence specific primers-polymerase chain reaction (SSP-PCR) was applied to detect susceptibility single nucleotide polymorphism (SNP) of STAT3 gene (rs2293152, rs9579700). The statistical differences in distribution between genotype and allele were analyzed between the asthma group and the healthy control group.@*Results@#The distribution frequency of CC genotype of rs2293152 in STAT3 gene was significantly higher in the asthmatic group (19.2%)than that in the healthy control group (9.0%) (χ2=6.065, P=0.014). C allele was significantly higher in asthmatic group (41.9% vs.33.8%) (χ2=3.861, P=0.049). There was no significant difference in the distribution of rs957970 in STAT3 gene between the asthma group and the healthy control group.@*Conclusions@#The SNP of rs2293152 in STAT3 gene is associated with susceptibility to asthma in children in South Sichuan.C allele may be the susceptible gene for childhood asthma, while the SNP of rs957970 in STAT3 gene has no significant correlation with childhood asthma in Southern Sichuan.

ABSTRACT

Objective To explore the strategies of constructing clinical biobank based on the real world study.Methods Biospecimens have been collected since November 2014,as well as continuingly construction of the biobank resources to match the implenmentation of real world study.Related measurements adopted including standardized ethical management,quality control,automated data management,and follow-up management.Results Biospecimens and clinical data information were collected with high quality according to standard SOPs.Sophisticated equipment and data automation management mode were used to make sure the protection of patient privacy and data security.At the same time,biospecimens and data information management system were improved gradually to fulfill the integration and sharing of biospecimen data resources.Conclusions Biorepositories are the key to translational medical research,biospecimens collected in our biobank will link with clinical data information simultaneously,which increase the resource pool that can be applied in real world study.

ABSTRACT

Objective To study the protective effects of curcumin on oxidative stress injury of H9C2 cardiac myocytes induced by H2O2.Methods The oxidative stress injury model in H9C2 cardiac myocytes was established in vitro, and cells were divided into the control group, H2O2 group (200μmol/L), low- (10μmol/L), medium- (20μmol/L), high- (40μmol/L) dose curcumin, and each group set six holes. After H2O2 stimulation for 24 h, the morphology changes were observed by microscope, and the survival rates were measured using CCK8 method. The activity of LDH, CK, and AST, and the content of MDA in culture medium were detected, and the activities of SOD, CAT, and GSH-Px in cardiac myocytes were also determined.Results Compared with the H2O2 group, the activity of LDH (247.36 ± 28.04 U/L, 195.14 ± 21.37 U/L, 132.27 ± 16.33 U/Lvs. 247.58± 28.34 U/L), AST (67.27 ± 10.58 U/ml, 54.81 ± 8.60 U/ml, 42.14 ± 5.95 U/mlvs.84.34 ± 12.36 U/ml), CK (1.34 ± 0.66 U/ml, 0.95 ± 0.42 U/ml, 0.71 ± 0.39 U/ml vs.1.56 ± 0.74 U/ml) and the content of MDA (227.39 ± 32.05 nmol/ml, 185.11 ± 24.37 nmol/ml, 143.50 ± 16.37 nmol/mlvs. 274.19 ± 36.51 nmol/ml) in culture medium of curcumin were significantly decreased (P<0.05). The morphology of H9C2 cardiac myocytes in curcumin group was improved, the survival rate was significantly increased (P<0.05). The activity of SOD (17.67± 1.36 U/mg, 21.54 ± 1.72 U/mg, 28.37 ± 2.36 U/mgvs. 14.37 ± 1.22 U/mg), CAT (19.58 ± 3.87 U/mg, 25.34 ± 4.06 U/mg, 30.21 ± 4.83 U/mgvs. 14.77 ± 3.25 U/mg) and GSH-Px (1.99 ± 1.17 U/mg, 2.56 ± 1.29 U/mg, 3.04 ± 0.45 U/mgvs. 1.67 ± 0.87 U/mg) in cardiac myocytes were significantly increased (P<0.05). Conclusions Curcumin can effectively improve oxidative stress injury of H9C2 cardiac myocytes induced by H2O2, and curcumin has dose-dependent protective effects against the oxidative stress of H9C2 cardiac myocytes induced by H2O2.

Objective@#To explore the expression of SLAMF6 on CD8+ T cells in patients with severe aplastic anemia (SAA) and its correlation with disease immune status.@*Methods@#By flow cytometry (FCM), SLAMF6 expression level in peripheral blood CD8+ T cells was detected in 21 patients with SAA and 15 normal controls respectively from February 2017 to April 2018. The correlation between SLAMF6 expression level and hematopoietic functions, including HGB, PLT, the neutrophil granulocyte and reticulocyte absolute value in peripheral blood, hyperplasia degree (percentage of granulocytes, erythrocytes, lymphocytes and megakaryocytes in bone marrow) and perforin, granzyme B, IFN-γ expression level in CD8+ T cells were evaluated. To further confirm the effect of SLAMF6 on CD8+ T cells, anti-SLAMF6 Ab was used to block SLAMF6 pathway (IgG as control), and FCM was used to detect the perforin, granzyme B, and IFN-γ production of CD8+ T cells.@*Results@#The expression of SLAMF6 on CD8+ T cells in untreated SAA patients[(56.29±12.97)%]was significantly lower than that of normal controls[(80.96±7.36)%](t=-7.672, P<0.001). The expression of SLAMF6 on CD8+ T cells in SAA patients were positively correlated with the HGB, PLT, the neutrophil granulocyte and reticulocyte absolute value in peripheral blood, percentage of granulocytes, erythrocytes in bone marrow (all P<0.05), but they were negatively correlated with the percentage of lymphocytes in bone marrow, and the expression of perforin, granzyme B, and IFN-γ of CD8+ T cells (all P<0.05). After blocking SLAMF6 pathway by anti-SLAMF6 Ab, the expression levels of perforin, granzyme B and IFN-γ in SAA patients were significantly higher than those in the untreated group, and the differences were statistically significant (all P<0.05).@*Conclusions@#SLAMF6 is significantly down-regulated on CD8+ T cells in SAA patients, which may act as a negative immunoregulatory molecule participating in the mechanism of SAA by affecting the functional molecules secretion on CD8+ T cells.

ABSTRACT

Patients with HCV infection can develop decompensated cirrhosis, hepatocellular carcinoma (HCC), even liver failure. As a result, efficient antiviral treatment is very essential to prevent HCV-related disease progression. Newly developed direct-acting antiviral agents (DAAs) have shown safety profile, favorable tolerability, and relatively short duration, which provide an opportunity to expand the number of patients who can be treated for HCV infection. There is a need for further clinical observation and summaries for DAAs in a real world. In the era of DAAs, special patients with HCV infection still get lots of attention from doctors. This review aims at the application of DAAs in patients with HCV infection, combined with chronic kidney diseases, hepatocellular carcinoma, HBV/HCV co-infection, HIV/HCV co-infection, post liver transplantation, pregnancy, children, lymphoma and retreatment.

ABSTRACT

Objective To investigate the diagnostic value of the partial least square discriminant analysis (PLS-DA) model based on seven serum cytokines for children patients with mycoplasma pneumonia,the 7 cytokines includ interleukin-2 (IL-2),IL-4,IL-6,IL-8,IL-10,tumor necrosis factor alpha (TNF-α) and interferonγ(IFN-γ).Methods Serum levels of cytokines were measured by the double antibody sandwich method of ELI-SA in 140 patients with pneumoniae infection and 135 normal healthy controls,and data was analyzed with the receiver operation characteristic (ROC) curve and the PLS-DA.Results The area under the ROC curve (AUC) of serum IL-10 and IFN-γfor the diagnosis of MPP was 0.84 (95% CL:0.79 ~ 0.89).The sensitivity of IL-10 and IFN-γ for the diagnosis of MPP was 91.4% and 82.1%,with the specificity was 77.0% and 82.2%,respectively.The diagnostic accuracy of the PLS-DA model based on seven serum cytokines for children MPP and the controls was 90.0% and 88.15%,and the prediction accuracy was 86.4% and 87.4%,respectively.Conclusion Seven serum cytokines based on PLS-DA model was helpful for the diagnosis of children patients with MPP.

ABSTRACT

Objective To explore the relationship of two loci (rs2293152, rs9579700) gene polymorphisms of signal transduction and transcription factor-3 (STAT3) with susceptibility to Henoch-Scho. .nlein purpura (HSP) and HSP nephritis (HSPN). Methods From September 2013 to September 2015, 180 children with HSP (group HSP) and 205 healthy children (control group) were enrolled in this study. Single nucleotide polymorphism (SNP) of intron 11 rs2293152C/G and intron 23 rs957970C/T of STAT3 gene was detected by sequence specific primer polymerase chain reaction (SSP-PCR). Results The frequency of CC genotype in STAT3 gene intron 11 rs2293152 in HSP group (26.1%) was significantly higher than that in control group (8.8%), and the frequency of allele gene of rs2293152C in HSP group (48.6%) was significantly higher than that in control group (32.7%) (P=0.013, 0.025). There were no differences in distribution of genotype and allele in rs957970C/T loci of intron 23 of STAT3 gene between two groups (P>0.05). The frequencies of genotype and allele of the two loci of STAT3 gene were no difference between HSPN and non HSPN groups (P>0.05). Conclusions The allele gene C of intron 11 rs2293152C/G of STAT3 gene may be a susceptible gene of HSP, while there was no association of 23 rs957970C/T polymorphism to HSP and there was no association of the two loci polymorphisms to HSPN.

ABSTRACT

Objective:To investigate the clinical characteristics of patient with giant ovarian mature cystic teratoma with contralateral ovarian endometriosis cysts,and to analyze the diagnostic experience and treatment process.Methods:The chinical materials of one case of giant hepatic mature cystic teratoma with contralateral ovarian endometriosis cyst was analyzed retrospectively,and the related literatures were reviewed.Results:The patient was admitted to the hospital for peritoneal giant mass in a physical examination.A large abdominal cysts was seen by gynecologic ultrasound.In the left ovary,a 61 mm× 48 mm cystic light mass was seen,and the viscous liquid was found in it.The abdominal CT showed a huge lump of mixed density in the right abdomen.The areas of density of fat,calcification density and density of soft tissue were found in the abdomen.The liver was pressured by the mass.The right ovarian tumor size was about 40 cm×35 cm×30 cm,up to the diaphragm,down to the pelvic tumor;the posterior wall of tumor adhered the left lobe of liver and the liver was squeezed to the upper right.The size of the left ovarian endometriosis cyst was about 7 cm × 6 cm× 6 cm.The operation was to remove the right ovarian tumor and left ovarian endometriosis cyst,and to reconstruct bilateral ovaries.The pathological diagnosis was right ovarian mature cystic teratoma and left ovarian endometriosis cyst.After 2 years of follow-up,norecurrence of the tumor was found.The patient was conceived with in vitro fertilization frozen embryo transfer (IVF-ET).Conclusion:The huge mature teratoma combined with contralateral ovarian endometrial cyst,which adhereds to the liver,is rare.Surgical treatment will gain good prognosis.IVF-ET should be early done when the patient is going to be infertile after surgery.

ABSTRACT

Objective To investigate the infectivity of serum and vaginal fluid in early cases and late latent cases of syphilis .Methods From January 2015 to December 2015 ,sixty-eight female patients with latent syphilis were enrolled in the outpatient clinic of Ningbo Women and Children′s Hospital ,including 38 early cases and 30 late cases .Tolulized red unheated serum test (TRUST ) was detected by using kits and Treponema Pallidum DNA (TP-DNA) was detected by polymerase chain reaction .TP-DNA levels in serum and vaginal fluid were compared before and after benzathine penicillin treatment in patients with early and late latent syphilis .Chi square test was performed for count data analysis .Results TRUST negative conversion cases in early latent syphilis after 3 ,6 and 12 months of treatment were 11 (28 .9% ) , 16 (42 .1% ) and 27 (71 .0% ) ,respectively .TRUST negative conversion cases in late latent syphilis after 3 ,6 and 12 months of treatment were 6 (20 .0% ) ,12 (40 .0% ) and 14 (46 .7% ) ,respectively .There were no statistical differences in TRUST negative conversion rates after 3 months and 6 months after treatment between early cases and late cases (χ2= 0 .716 and 0 .031 ,respectively ,both P> 0 .05) ,while those after 12 months of treatment between early cases and late cases were statistical different (χ2=4 .164 ,P < 0 .05) .The vaginal fluid TP-DNA positive cases in early syphilis before and after 3 ,6 ,and 12 months of treatment were 4 (10 .5% ) ,0 ,1 (2 .63% ) and 1 (2 .63% ) ,respectively ,with no statistical difference (χ= 6 .247 ,P> 0 .05) .The serum TP-DNA positive cases in early syphilis before and after 3 , 6 ,and 12 months of treatment were 18 (47 .3% ) ,4 (10 .5% ) ,5 (13 .2% ) and 5 (13 .2% ) ,respectively , with statistical difference (χ= 21 .217 , P < 0 .05 ) . The vaginal fluid TP-DNA positive cases in late syphilis before and after 3 ,6 , and 12 months of treatment were 2 (6 .7% ) ,0 ,0 and 1 (3 .3% ) , respectively ,with no statistical difference (χ2= 3 .761 , P> 0 .05) .The serum TP-DNA positive cases in late syphilis before and after 3 ,6 ,and 12 months of treatment were 8 (26 .7% ) ,1 (3 .3% ) ,1 (3 .3% ) and 1 (3 .3% ) ,respectively ,with statistical difference (χ2= 14 .712 , P < 0 .05) .Conclusions Standard treatment can reduce the level of TP in serum and vaginal fluid of latent syphilis women and improve the negative conversion rate of TRUST .Late latent syphilis is easier to create sero-resistance than early cases . The early cases have stronger infectivity than late cases before standard treatment .

ABSTRACT

Quorum sensing (QS) is a cell-cell communication mechanism that allows bacterial populations to coordinate gene expression in response to cell density. N-acylhomoserine lactones (AHL) are used as quorum-sensing signal molecules by many Gram negative bacteria. Acinetobacter sp. 77, an AHL-degrading bacterium, was isolated in our previous work. The gene aidE for AHL inactivation was cloned in this study by screening a genomic DNA library. The deduced protein AidE is 268 amino acids in length and shares a high identity (95%) with the beta-lactamase family protein in Acinetobacter gyllenbergii CIP110306, but low identities with known AHL-degrading enzymes. HPLC analysis of the AidE-degraded C6-HSL products revealed that AidE functioned as an AHL lactonase. Sequences alignment suggested that the aidE gene is not conserved in Acinetobacter species, flanking sequences of aidE and their arrangement are specific in Acinetobacter sp. 77 genome, and some IS insertion sequences were found downstream of the aidE gene. These evidences indicated that the aidE gene might be foreign DNA taken up via horizontal gene transferring or had changed its relative location due to the genome rear-arrangement. Expression of the aidE gene in Pectobacterium carotovorum subsp. carotovorum Z3-3 significantly reduced its AHL production as well as the pathogenicity on host plants, indicating that AidE was able to effectively quench quorum sensing-dependent functions in bacteria. In conclusion, aidE is a newfound AHL-lactonase with a potential for suppression of bacterial infections.

ABSTRACT

Objective To investigate the antibiotic resistance trend of commonly used antibiotics of Helicobacter pylori (H.pylori) in Huzhou district,and to summarize the efficacy of eradication in related digestive diseases.Methods In year 2009,2013,2014 and 2015,8 139 gastric mucosa samples of patients undergoing gastroendoscopy examination were collected and H.pylori strains were isolated and cultured.The situation of resistance to levofloxacin,clarithromycin,metronidazole,amoxicillin,tetracycline and furazolidone was analyzed.The infection and antibiotics resistance of H.pylori were analyzed in 11 digestive diseases including functional dyspepsia,chronic gastritis,acute gastritis,duodenitis,gastric ulcer,duodenal ulcer,gastrointestinal dysfunction,gastric cancer,residual gastritis,reflux esophagitis and gastric lymphoma.The eradication schemes and eradication rate of H.pylori was reviewed in six digestive diseases including functional dyspepsia,chronic gastritis,duodenitis,gastric ulcer,duodenal ulcer and reflux esophagitis.Fisher's exact test was performed to compare the differences among the groups.Results A total of 3 263 H.pylori strains were obtained and the infection rate of H.pylori was 40.09% (3 263/8 139).The H.pylori infection rates of 11 digestive diseases were from 0 to 57.89%,and which was high in duodenal ulcer,gastric ulcer,duodenitis,chronic gastritis and functional dyspepsia (57.89%,726/1 254;49.83%,301/604;42.91%,118/275;37.45%,1 518/4 053 and 36.78%,146/397;respectively).The results of single antibiotic resistance analysis in six digestive diseases needed or planed for H.pylori eradication indicated that resistance rate of H.pylori to levofloxacin and clarithromycin reached 23.09% (663/2 871) and 17.87% (513/2 871),respectively.The resistance rate tolevofloxacin dramatically increased from 5.03% (8/159) in 2009 to 28.24% (586/2 075) in 2015;the resistance rate to clarithromycin increased from 12.58% (20/159) in 2009 to 21.78% (452/2 075) in 2015;meanwhile,the resistance rate of H.pylori to metronidazole was nearly 100.00%.However,the resistance rates to amoxicillin,tetracycline and tetracycline were all zero.The results of double antibiotic resistance analysis in six digestive diseases needed or planed for H.pylori eradication indicated that the rate of both levofloxacin and clarithromycin resistance was 7.73 % (222/2 871).The double antibiotic resistance rate of levofloxacin and clarithromycin in different diseases fluctuated between 4.82 % and 10.96 %.Totally 1 479patients of six digestive diseases were treated with quadruple therapy,and 1 363 patients were followed up after eradication therapy,with the eradication rate of 85.99% (1 172/1 363).Conclusions In Huzhou district,for six common digestive diseases needed or planed for H.pylori eradication,any combination of two drugs in all three drugs including amoxicillin,tetracycline and furazolidone is the first choice for treatment.Only when patients are allergic to penicillin or furazolidone and tetracycline can not be obtained,will levofloxacin and clarithromycin be chosen.A high eradication rate can be achieved by choosing eradication schemes according to the results of H.pylori drug sensitivity test in local region.
