ABSTRACT

Purpose@#This study explored the performance of prenatal ultrasonography in the differential diagnosis of cystic biliary atresia (CBA) and choledochal cyst (CC). @*Methods@#Fetuses diagnosed with hepatic hilar cyst in the second trimester were included in this study. A series of prenatal ultrasound examinations were performed in the second and third trimesters. The diameter of the gallbladder (GB) and hepatic cyst were measured, as well as the wall thickness of the GB. The GB-cyst connection, visibility of the right hepatic artery (RHA), and other concomitant abnormalities were carefully evaluated. A neonatal transabdominal ultrasound examination was performed within 1 week after birth, and clinical data were followed up to 6 months after birth. @*Results@#Between January 1, 2016 and January 31, 2020, 53 fetuses diagnosed with hepatic hilar cyst were recruited. Eight were excluded because they were lost to follow-up. Among the 45 cases included in this study, 10 were diagnosed with CBA and 35 with CC after birth. Statistically significant differences were found in GB width, wall thickness, change in GB width, change in cyst length, GB-cyst connection, and RHA visibility between the CBA and CC groups. GB width showed the best diagnostic performance with an area under the curve (AUC) of 0.899. The combination of GB width, GB wall thickness, and GB-cyst connection yielded a comparable AUC of 0.971. @*Conclusion@#The GB should be carefully evaluated in fetuses with hepatic hilar cyst. Prenatal ultrasound findings could provide suggestive parameters for the differential diagnosis of CBA from CC.

ABSTRACT

Four new prenylflavonoid glycosides, namely koreanoside H-K (1-4), together with eleven known ones (5-15) were isolated from the leaves of Epimedium koreanum Nakai. Their structures were elucidated by 1D NMR, 2D NMR, HR-ESI-MS, IR and UV. The identification of the sugar moieties was carried out by means of acid hydrolysis and HPLC analysis of their derivatives. It is worth noting that compound 3 and compound 4 were elucidated to contain fucose and quinovose moieties, which were two extremely rare sugar units from the genus Epimedium. The anti-pulmonary fibrosis activity of the new compounds was evaluated using A549 cell line. Compounds 1, 2 and 4 showed significant anti-pulmonary fibrosis activities.

ABSTRACT

The significant clinical feature of bisphosphonate-related osteonecrosis of the jaw (BRONJ) is the exposure of the necrotic jaw. Other clinical manifestations include jaw pain, swelling, abscess, and skin fistula, which seriously affect the patients' life, and there is no radical cure. Thus, new methods need to be found to prevent the occurrence of BRONJ. Here, a novel nanoparticle, tFNA-KLT, was successfully synthesized by us, in which the nanoparticle tetrahedral framework nucleic acid (tFNA) was used for carrying angiogenic peptide, KLT, and then further enhanced angiogenesis. TFNA-KLT possessed the same characteristics as tFNA, such as simple synthesis, stable structure, and good biocompatibility. Meanwhile, tFNA enhanced the stability of KLT and carried more KLT to interact with endothelial cells. First, it was confirmed that tFNA-KLT had the superior angiogenic ability to tFNA and KLT both in vitro and in vivo. Then we apply tFNA-KLT to the prevention of BRONJ. The results showed that tFNA-KLT can effectively prevent the occurrence of BRONJ by accelerating angiogenesis. In summary, the prepared novel nanoparticle, tFNA-KLT, was firstly synthesized by us. It was also firstly confirmed by us that tFNA-KLT significantly enhanced angiogenesis and can effectively prevent the occurrence of BRONJ by accelerating angiogenesis, thus providing a new avenue for the prevention of BRONJ and a new choice for therapeutic angiogenesis.

ABSTRACT

Objective:To assess the efficacy and toxicity of chemoradiotherapy in the treatment of early stage extranodal nasal-type NK/T-cell lymphoma (ENKTCL).Methods:Retrospective review was conducted for 174 patients with pathological proved early stage ENKTCL who were treated in the Department of Radiation Oncology, Peking University Cancer Hospital & Institute. The Kaplan-Meier survival analysis was adopted to calculate the local-regional control (LRC), overall survival (OS), and progression free survival (PFS), and the Log-rank test COX regression model were applied to univariate and multivariate analyses. Results:The patients in this study included 102 and 72 patients diagnosed with Ann Arbor stage-Ⅰ and stage-Ⅱ, respectively. Among them, two patients received radiotherapy alone and 172 patients were treated with combined chemoradiotherapy. The overall response rate of all the patients was 94.2%, with a complete response (CR) rate of 87.9% (153). Furthermore, the rates of 5-year OS, PFS, and LRC were 87.3%, 83.1%, and 91.9%, respectively. The most common toxicities during the chemotherapy and radiotherapy included myelosuppression and oral mucositis, with grade ≥ 3 myelosuppression and grade ≥ 3 oral mucositis accounting for 62.1% and 10.9% of all patients, respectively. As shown by multivariate analysis, the adverse prognostic factors for OS included age > 60, B symptoms, and stage Ⅱ, while the adverse prognostic factors for PFS included age > 60 and stage Ⅱ. Meanwhile, the PFS rate was significantly improved by increasing the radiation dose (≥ 50 Gy vs.<50 Gy), and the 5-year PFS rates of the two groups were 83.5% and 76.5%, respectively [hazard ratio ( HR) 0.374; 95% CI, 0.169-0.826; P=0.015]. Conclusions:A good therapeutic effect can be achieved for early stage NK/T-cell lymphoma and the toxicities after combined chemoradiotherapy can be tolerated.

ABSTRACT

The lumbar intervertebral disc is a disc-shaped fibrocartilaginous structure located between the vertebral bodies and acts as an important anatomical structure in the spine for stability. The spinal stability system consists of three subsystems: passive subsystem, active subsystem, and neural control subsystem. Lumbar intervertebral disc degeneration (IVDD), the leading cause of low back pain, induces pathological changes in other tissues of the three subsystems and interacts to degrade spinal stability. IVDD is commonly accompanied by degeneration of facet joint and ligament, Modic change, decreased vertebral blood flow, increased paraspinal muscle fat infiltration, reduced axial tension injury of the nerve, et al. Magnetic resonance imaging (MRI) is the preferred imaging modality for assessing spinal stability. Conventional MRI could completely demonstrate the morphological changes of the three subsystems in IVDD patients, and functional MRI could quantitatively evaluate the degree of pathophysiological change. We summarized the morphological and functional changes of MRI in lumbar intervertebral disc, facet joint, ligament, vertebral body, paraspinal muscle, and nerve of patients with IVDD, and then analysed the changes in spinal stability caused by IVDD, aiming to provide more imaging information for improving diagnosis accuracy and developing appropriate management of patients with low back pain.

ABSTRACT

Objective:To correlate N-terminal pro-B-type natriuretic peptide (NT-proBNP) and heart type fatty acid binding protein (H-FABP) levels with the prognosis of patients with heart failure.Methods:Forty-nine patients with severe heart failure who received treatment in Tongde Hospital from December 2017 to June 2019 were included in the observation group. Forty-nine healthy subjects who concurrently received health examination were included in the control group. The heart function of the 49 patients was graded by the New York Heat Association (NYHA) classification. The levels of NT-proBNP and H-FABP were measured in different NYHA grades of patients and the healthy controls. The value of NT-proBNP and H-FABP levels in the diagnosis of heart failure was investigated and NT-proBNP and H-FABP levels were correlated with prognosis of heart failure. Results:In the observation group, NT-proBNP and H-FABP levels were (341.52 ± 12.74) pg/mL and (69.01 ± 11.45) ng/mL, respectively, which were significantly higher than (189.60 ± 12.73) pg/mL and (37.25 ± 11.44) ng/mL, respectively, in the control group ( t = 59.047 and 13.735, both P < 0.001). In patients with NYHA III grade severe heart failure, NT-proBNP and H-FABP levels were (295.62 ± 42.38) pg/mL and (75.09 ± 19.15) ng/mL, respectively, which were significantly higher than (120.43 ± 21.69) pg/mL and (21.76 ± 5.22) ng/mL, respectively in patients with NYHA I grade severe heart failure, (203.51 ± 42.77) pg/mL and (44.92 ± 10.85) ng/mL in patients with NYHA Ⅱ grade severe heart failure ( F = 55.100, 46.040, both P < 0.001). NT-proBNP and H-FABP levels in patients with NYHA I grade severe heart failure were significantly lower than those in patients with NYHA Ⅱ grade severe heart failure ( t = 7.231, 8.130, both P < 0.001). The sensitivity and accuracy of detecting NT-proBNP and H-FABP levels alone in the diagnosis of heart failure (76.92%, 78.85%, 84.69%, 86.73%) were significantly lower than those of detecting NT-proBNP and H-FABP levels in combination (98.08%, 97.96%, χ2 = 75.760, 10.913, both P < 0.001). NT-proBNP and H-FABP levels were positively correlated with the prognosis of heart failure ( r = 0.362, 0.611, both P < 0.05). Conclusion:NT-proBNP and H-FABP levels are positively correlated with the prognosis of severe heart failure in older adult patients. Detecting NT-proBNP and H-FABP levels in combination can increase the sensitivity and accuracy in the diagnosis of severe heart failure.

ABSTRACT

The compatibility of kanamycin with sodium citrate for the formulation of kanamycin sulfate injection was determined, including optimization of the amount of sodium citrate in the injection and the sterilization process. An HPLC coupled with an evaporative light scattering detector (ELSD) was used to measure the amount of sodium citrate and the impurity profiles. A validated post-column derivatization HPLC coupled with a fluorescence detector (FLD) was used to determine the correlation between specific impurities in a domestic factory and sodium citrate, and then the formulation was evaluated by HPLC coupled with mass detector (MS) characterization of degradation products. The results show that the amount of sodium citrate in kanamycin sulfate injection from a domestic factory is about 40 times higher than that of the Meiji formulation. Several specific impurities can be detected in solutions heated under simulated sterilization conditions (121 ℃), which were correlated with the amount of sodium citrate. Impurities were characterized by HPLC-MS/MS, and data showed that the identified impurities were interaction products of kanamycin and sodium citrate. These results indicate that greater attention should be directed at formula optimization in domestic factories, as it is crucial to the safety and efficacy of the preparations. Drug-excipient chemical compatibility should also be evaluated in the development of pharmaceutical dosages forms especially when the active pharmaceutical ingredients have a primary amine group.

ABSTRACT

Objective:To observe the changes of nutrition impact symptoms (NIS) and fat-free mass and analyze the relationship between them in patients with head and neck cancer (HNC) during radiotherapy. Methods:A convenient sampling method was adopted to select HNC patients who received radiotherapy in the outpatient clinic of department of radiotherapy for head and neck cancer in a cancer hospital in Beijing from March 2017 to January 2020. The nutrition impact symptoms (NIS) were assessed by a nutrition impact symptoms checklist and the fat free mass was assessed by bioelectrical impedance analysis at three time points: before radiotherapy (T1), during radiotherapy (T2) and at the end of radiotherapy (T3). The Generalized Estimating Equations (GEE) was used to analyze the relationship between them. Results:A total of 542 HNC patients were included in the analysis. During radiotherapy, the patients' NIS number and score presented an increasing trend, reaching the highest level and peaked at the end of radiotherapy. The change of fat-free mass showed a decreasing trend and reached the minimum at the end of radiotherapy. Patients with higher NIS scores had more percentage loss of fat-free mass. Conclusions:During radiotherapy, patients with HNC showed an increase in NIS score and lost fat-free mass. Patients with higher NIS score lost more percentage of fat-free mass, which suggested that clinical medical staff should pay attention to NIS management and take comprehensive intervention measures in time to reduce the loss of fat-free mass.

ABSTRACT

Purpose@#This study aimed to investigate the factors associated with chemoresistance to neoadjuvant chemotherapy (NACT) followed by radical hysterectomy (RH) and construct a nomogram to predict the chemoresistance in patients with locally advanced cervical squamous carcinoma (LACSC). @*Materials and Methods@#This retrospective study included 516 patients with International Federation of Gynecology and Obstetrics (2003) stage IB2 and IIA2 cervical cancer treated with NACT and RH between 2007 and 2017. Clinicopathologic data were collected, and patients were assigned to training (n=381) and validation (n=135) sets. Univariate and multivariate analyses were performed to analyze factors associated with chemoresistance to NACT. A nomogram was built using the multivariate logistic regression analysis results. We evaluated the discriminative ability and accuracy of the model using a concordance index and a calibration curve. The predictive probability of chemoresistance to NACT was defined as > 34%. @*Results@#Multivariate analysis confirmed menopausal status, clinical tumor diameter, serum squamous cell carcinoma antigen level, and parametrial invasion on magnetic resonance imaging before treatment as independent prognostic factors associated with chemoresistance to NACT. The concordance indices of the nomogram for training and validation sets were 0.861 (95% confidence interval [CI], 0.822 to 0.900) and 0.807 (95% CI, 0.807 to 0.888), respectively. Calibration plots revealed a good fit between the modelpredicted probabilities and actual probabilities (Hosmer-Lemeshow test, p=0.597). Furthermore, grouping based on the nomogram was associated with progression-free survival. @*Conclusion@#We developed a nomogram for predicting chemoresistance in LACSC patients treated with RH. This nomogram can help physicians make clinical decisions regarding primary management and postoperative follow-up of the patients.

ABSTRACT

Objective:Using bioinformatics analysis methods, the differential genes in situ hepatocellular carcinoma and metastatic foci and their involved metabolic pathways were screened to find new metabolic targets for liver cancer metastasis treatment. Methods:The data numbered GSE40367 of primary and metastasis liver cancer were downloaded from the gene expression omnibus (GEO) database. After the differential expressed genes were screened out, the differential expressed genes were enriched with gene ontology (GO) functions and the Kyoto encyclopedia of genes and genomes (KEGG) pathway analysis. The STRING database and cytoscape software were used to analyze protein interactions. The changed metabolites in liver cancer metastasis were obtained from the research. Then the differential genes and metabolites were analyzed by MetaboAnalyst 4.0. Kaplan-Meier survival curve was used for survival analysis of the key genes on line. Results:A total of 564 differential expressed genes were selected from GSE40367, compared with tumors in situ, there were 287 up-regulated and 277 down-regulated in metastasis tumors. GO functions were enriched in processes such as monocarboxylic acid metabolism, fatty acid metabolism, and lipid transport. KEGG enrichment analysis showed that these differential genes were mainly concentrated in bile secretion, ABC transporters, cysteine and methionine metabolism, and gluconeogenesis. The combined analysis showed that the mainly relevant metabolic pathways were nicotinate and nicotinamide metabolism, phenylalanine metabolism, arginine biosynthesis and glycolysis and gluconeogenesis. Survival analysis of key genes found that CTH downregulation may be associated with a poor prognosis of liver cancer, while the high expression of PCK1 and AOX1 may be associated with a poor prognosis of liver cancer. Conclusion:The bioinformatics analysis reveals metabolic pathways related to liver cancer metastasis and the changed genes and metabolites in the pathway, which helps to understand the molecular mechanism of metastasis, and provides new targets for the treatment of metastasis liver cancer.

Objective:To investigate the effects and mechanism of Tuina on denervation-induced atrophy. Methods:A total of 42 Sprague-Dawley rats were randomly divided into sham group (n = 6), model group (n = 18) and Tuina group (n = 18). The model group and Tuina group freed and excised right tibia nerve about one centimeter, while the sham group freed the right tibia nerve only. From the second day after operation, Tuina group accepted Tuina on the injured area, while the sham group and the model group were only fixed without any intervention. Six rats were sacrificed on the 14th, 21st and 28th day after operation in the model and Tuina groups, and the sham group was sacrificed on the 28th day after operation. The gastrocnemius muscles were measured wet weight ratio. The diameter and area of muscle cells were measured under HE staining. The expression of Pax7, MyoD, MyoG, microRNA-1, microRNA-133a and microRNA-206 in the gastrocnemius muscles were detected with reverse transcription real-time quantitative polymerase chain reaction. Results:Compared with the sham group, the wet weight ratio, the area of muscle cells (except the 14-day-Tuina group) and the diameter of muscle cells decreased at each time point in the model group and Tuina group (P < 0.05); compared with the model group, the wet weight ratio, muscle cell diameter and muscle cell area increased at each time point in Tuina group (P < 0.05). Compared with the sham group, the expression of Pax7 increased in the 14-day-model group (P < 0.05) and decreased in the 28-day-model group (P < 0.05), and it increased at each time point (except 28-day) in Tuina group (P < 0.05); compared with the model group, the expression of Pax7 increased at each time point in Tuina group (P < 0.05). Compared with the sham group, the expression of MyoD and MyoG increased at each time point in the model group and Tuina group (P < 0.05); compared with the model group, the expression of MyoD and MyoG increased at each time point (except 14-day) in Tuina group (P < 0.05). Compared with the sham group, the expression of microRNA-1 and microRNA-133a decreased, and microRNA-206 increased in the model group and Tuina group at 21-day (P < 0.05); compared with the model group, the expression of microRNA-1, microRNA-133a and microRNA-206 increased in Tuina group (P < 0.05). Conclusion:Tuina may activate the Pax7/MyoD/MyoG pathway by increasing the expression of muscle-specific microRNA, to promote the proliferation and differentiation of muscle satellite cells, and delay denervation-induced atrophy.
