ABSTRACT

Objective:To investigate the influence of DTAS gene mutations (DNMT3A, TET2, ASXL1, SRSF2) on survival of newly diagnosed acute myeloid leukemia (AML) patients.Methods:A retrospective analysis of 163 patients with next-generation sequencing(NGS)data in the hematology Department of Affiliated Hospital of Xuzhou Medical University from January 1, 2018 to October 31, 2021 was performed. Clinical data of patients were collected and analyzed including patient′s height, weight, gender, age, bone marrowblast ratio, induction chemotherapy regimen, transplantation or not, complete blood count, etc. There were 88 males and 75 females with a median age of 48 (4-81) years. According to the next-generation sequencing data, patients with any mutation of the above four genes were divided into the DTAS gene mutation group, and vice versa, they were divides into non-DTAS gene mutation group.The Kaplan-Meier method and Cox proportional risk model were used to analyze survival and prognosis.Results:Among 163 patients, DTAS gene mutation was detected in 50 patients (30.7%), and not in 113patients(69.3%). Among the 50 patients with DTAS genemutations, 8 cases(16%) had≥2 mutations and 42 cases(84%) had any gene mutation in DTAS. In the DTAS gene mutation group, the patients were older, the stratification of the European leukemia network(ELN) was poor, the duration of remission was shorter, the proportion of sever myelosuppression degree was higher (61.8% vs 34.8%) at day28 after induction chemotherapy, and the lymphocyte-monocyte ratio was lower than that of the healthy control group when CR was reached after treatment.The results of K-M survival analysis showed that the overall survival(OS)time ( P=0.003) and event-free survival(EFS) ( P=0.008) time of the DTAS gene mutant were significantly shorter than those of the non-DTAS gene mutated group.The medianOS timein theh DTAS gene mutations was significantly shorter than that in the non-DTAS gene mutated group ( P=0.003, HR=2.041) [21(95% CI 16.63-25.37) months vs 43 (95% CI 33.01-52.99) months].The results of multivariate COX analysis revealed that DTAS gene mutations was an independent risk facror forOS time(HR=2.041, 95% CI: 1.285-3.244, P=0.003) in AML patients. Conclusion:DTAS gene mutation does not affect the hematopoietic recovery time after induction chemotherapy, but the duration of remission is shorter in the DTAS gene mutations group. DTAS gene mutations indicate a poor prognosis, which is an independent risk factor for OS.

ABSTRACT

Objective:To investigate the first aid management, doctors′ command of first aid knowledge of private dental clinics in Zhangjiakou city and the influencing factors, for references in strengthening the emergency management level of these clinics and the emergency capacity of doctors.Methods:Customized questionnaire was used in a survey of 192 doctors in 64 private dental clinics in the city from June to September in 2019, and 172 valid responses were recovered. The questionnaire covered such aspects as basics of these doctors, first-aid training received and first-aid knowledge command, as well as first-aid management of these clinics. Enumeration data were indicated with frequency number and percentage, while measurement data were indicated with ± s. Univariate analysis of first-aid knowledge scoring was made with Mann-Whitney U test. Results:The first-aid knowledge of dental doctors in private dental clinics in Zhangjiakou was poor, scoring 51.8±14.6 points in general, and those scoring above 60 points accounted for merely 19.3%. The proportion of dental doctors participating in first-aid knowledge training and emergency drill was low. Only 7.8% of the private dental clinics in Zhangjiakou had complete rescue equipment. Scoring of the dental doctors participating in emergency drill and emergency knowledge training was higher than those without such experiences, with the difference being statistically significant( P<0.001). Conclusions:Private dental clinics should regularly arrange their staff to participate in first-aid training and emergency drills; prepare an emergency plan, equip rescue facilities and first-aid medication; dental doctors should strengthen their study of first-aid knowledge.

ABSTRACT

<p><b>Objective</b>To explore the practicability and safety of the F4.8 visual miniature nephroscope in the diagnosis and treatment of hematospermia.</p><p><b>METHODS</b>This study included 12 cases of refractory hematospermia accompanied by perineal or lower abdominal pain and discomfort. All the patients failed to respond to two months of systemic anti-inflammatory medication and local physiotherapy. Seminal vesicle tumor and tuberculosis were excluded preoperatively by rectal seminal vesicle ultrasonography, MRI or CT. Under epidural anesthesia, microscopic examination was performed with the F4.8 miniature nephroscope through the urethra and ejaculatory duct orifice into the seminal vesicle cavity, the blood clots washed out with normal saline, the seminal vesicle stones extracted by holmium laser lithotripsy and with the reticular basket, the seminal vesicle polyps removed by holmium laser ablation and vaporization, and the seminal vesicle cavity rinsed with diluted iodophor after operation.</p><p><b>RESULTS</b>Of the 10 patients subjected to bilateral seminal vesiculoscopy, 3 with unilateral and 2 with bilateral seminal vesicle stones were treated by holmium laser lithotripsy, saline flushing and reticular-basket removal, 2 with seminal vesicle polyps by holmium laser ablation and vaporization, and the other 3 with blood clots in the seminal vesicle cavity by saline flushing for complete clearance. The 2 patients subjected to unilateral seminal vesiculoscopy both received flushing of the seminal vesicle cavity for clearance of the blood clots. The operations lasted 10-55 (25 ± 6) minutes. There were no such intra- or post-operative complications as rectal injury, peripheral organ injury, and external urethral sphincter injury. The urethral catheter was removed at 24 hours, anti-infection medication withdrawn at 72 hours, and regular sex achieved at 2 weeks postoperatively. The patients were followed up for 6-20 (7 ± 2.3) months, during which hematospermia and related symptoms disappeared in 10 cases at 3 months and recurrence was observed in the other 2 at 4 months after surgery but improved after antibiotic medication.</p><p><b>CONCLUSIONS</b>The F4.8 visual miniature nephroscope can be applied to the examination of the seminal vesicle cavity and treatment of seminal vesicle stones and polyps, with the advantages of minimal invasiveness, safety and reliability.</p>

ABSTRACT

Objective To investigate the therapeutic effects of penetrating resin and fluoride on early enamel caries. Methods Sixty intact bovine incisors were immersed in demineralized solution for 24 hours to make bovine incisor enamel caries.The specimens were divided into four groups(n=15 for each group)according to the treatment methods:control group (CON)-immersion in artificial saliva, DF group-immersion in 0.05% fluoride solution daily, WF group-2% fluoride gel weekly and IC group-resin infiltration.After processing for four and eighe weeks,the microhardness of the surface of each group was measured.After the treatment for four weeks,the depth of penetration and the microhardness of the samples were measured. After 8-week treatment, all samples were reintroduced into the demineralized solution for 24 hours and the microhardness of the samples was measured again.Results Results of microhardness assessment showed that there were no significant differences in baseline values(after white spots)between four groups(P>0.05).After treatment for four weeks the microhardness value reached the peak in IC group. After treatment for eight weeks the microhardness values reached the peak in DF group and WF group.The values of microhardness were significantly higher at different time points in IC group than those of other groups(P<0.05).After 4-week treatment,the percentages of penetration depth were significantly higher in DF,WF and IC groups than those of control group(P<0.01).The penetration depth was significantly higher in IC group than that of DF group and WF group (P<0.05). There was no significant difference in the penetration depth between DF group and WF group(P>0.05).Conclusion For the early enamel caries penetration resin,the penetration percentage is significantly higher than the fluoride treatment.

ABSTRACT

Objective To evaluate the safety and effectiveness of modular flexible ureteroscope combined with holmium laser lithotripsy in treatment of bilateral renal calculi smaller than 1.5 cm. Methods Clinical data of 24 patients from August 2013 to March 2016 using a modular flexible ureteroscope in treatment of bilateral renal calculi smaller than 1.5 cm was retrospectively analyzed. The clinical data included operation time, stone clearance rate and occurrence of complications. Results All the 24 patients were successfully placed in bilateral ureteral sheath soft lenses, and enter lithotripsy smoothly. The operation time was 40~105 (71.0 ± 21.5) min. Lithotripsy success rate was 100.00% and 1 month stone clearance rate was 89.50% (43/48), 5 sides with residual stones diameter 7~10 mm, were given extracorporeal shock wave lithotripsy, 3 months stone clearance rate was 93.75% (45/48), 3 sides with residual stones 5~7 mm located lower calyx accepted regular review. 4 cases with postoperative fever were cured after anti-inflammatory treatment. There was no bleeding, ureteral perforation, postoperative avulsion, renal dysfunction, septic shock and other complications. Conclusion Modular flexible ureteroscope lithotripsy in treatment of bilateral renal calculi smaller than 1.5 cm is safe and effective.

Objective To introduce a new appointment registration way in the outpatient department.Methods A WeChat appointment registration platform based on HIS was established with smart phone and WeChat public platform,which was gifted with the functions of online registration and payment and etc.The processes including preliminary diagnosis and return visit,cancelling appointment,treatment of appointment breaking,and the limitations during the appointment could be accomplished by the appointment registration.Results The platform realized appointment registration,information feedback and etc.Conclusion The appointment registration based on WeChat public platform contributes to satisfying the patient,enhancing medical service and distributing medical resources rationally.

ABSTRACT

Objective To observe the clinical effect of oblique supine lithotomy position rigid single-access percutaneous nephrolithotomy combined with FURS in treatment of complex urinary tract stones. Methods From May 2013 to January 2016, 40 cases of multiple kidney stones and staghorn calculi patients underwent oblique supine lithotomy position balloon establish standard single renal access, using percutaneous percutaneous combind with FURS, clinical data of patients, including operative time, blood loss, stone stone clearance rate, hospitalstay, incidence of complications and other indicators. Results The patients were successfully implemented Phase I single-access surgery, the median time 52.5 min, stone stone clearance rate of 100%, before and after surgery no difference in the amount of bleeding, no blood transfusion patients, followed up for 3 months to six months, no significant concurrent operation disease. Conclusion Oblique supine lithotomy bit single-access percutaneous percutaneous combined with FURS treatment of complex renal calculi can improve stone clearance rate, increased patient comfort during surgery to avoid damage to the kidneys multi-channel, shorter operative time, treatment obvious.

ABSTRACT

Objective To observe ransurethral flexible ureteroscopey for two-step precise method treatment of parapelvic cyst. Methods From May 2014 to November 2015, 23 patients of parapelvic cyst underwent a ransurethral flexible ureteroscopey for two-step precise method treatment, then analyze the clinical data of patients, observe the therapeutic effect. Results All patients underwent primary surgery successfully while 4 cases among them were confirmed as urine level puncture calyceal diverticulum treated with flexible ureteroscopy diverticulum expand drainage, the other 19 cases were confirmed as renal cyst and treated with cyst incision drainage successfully. The hemoglobin before and after surgery showed no changes. With no blood transfusion, the average postoperative hospitalization time was 2 d. 1 day after surgery, KUB was to locate the DJ tube position; 1 month later, ultrasound or CT check show that the cyst volume was significantly reduced, postoperative follow-up range from 6 months to 1 year. All the patients had no long-term complications. Conclusion Ransurethral flexible ureteroscopey for two-step precise method treatment of parapelvic cyst and renal pelvis lamp beside diverticulum provide a clear direction for the next operation. It's simple operation with no significant complications and recovered quickly, which is an ideal method for the treatment of renal pelvic.

ABSTRACT

Objective To evaluate the efficacy and safety of tubeless of modular flexible ureteroscope combined with ultramini nephrostomy tract for the treatment of upper ureteral calculi in paediatric patients. Methods 20 cases of children with upper ureteral calculi (age≤7 years old), in our hospital underline of modular flexible ureteroscope combined with ultramini nephrostomy tract lithotripsy, the stones located on the left side in 12 cases, 8 cases on the right. The average age of children in the 4 years (2 to 7 years), the average diameter of stones 1.0 cm (0.7 ~1.5 cm), multiple 9 (including 3 cases of multiple bilateral, 6 cases of unilateral multiple) single stones 11 cases. Ultrasound-guided percutaneous nephrolithotomy establish supermicro channel (F11), application modular and flexible uretero﹣scope holmium laser lithotripsy systems. Results Tubeless of modular flexible ureteroscope combined with ultramini nephrostomy tract calculi Clear Phase 91%(21/23), 2 cases of bilateral stones with phased surgical treatment, surgi﹣cal staging stone clearance rate was 100 % (23/23). Postoperative hemoglobin decline was no significant change. Children do not need a blood transfusion, creatinine, blood urea nitrogen and CRP were no significant differences in change, not placed before and after surgery and renal fistula DJ tube (no tube-based). The mean postoperative hospi﹣tal stay was two days, followed up for 6 months to 1 year, all patients had no long-term complications. Conclusion Tubeless of modular flexible ureteroscope combined with ultramini nephrostomy tract for the treatment of Upper ureteral calculi in paediatric patients is an effective and safety method, can shorten hospital stay.

ABSTRACT

Objective Comparing and analyzing the advantages and disadvantages of rigid and flexible ureteroscopy in treatment of renal multiple stones or staghorn calculi. Methods 80 patients of renal multiple stones and staghorn calculi from May 2012 to March 2015 were randomly divided into two groups, group A: flexible ureteroscopy as adjuvant technique of balloon dilatation to establish standards channel of rigid nephrolithotripsy, group B: rigid ureteroscopy as auxiliary to flexible ureteroscopy nephrolithotomy. Clinical data of the two groups including operative time, blood loss, stones clearance rate, hospital stay and other indicators of surgical complications were comparatively analyzed. Results TAll the patients received successful surgical procedures. The operation time in group A was 50.4 min, gravel time was 25.6 min, while operation time in group B was 90.3 min, gravel time was 70.3 min. The differences between the two groups was statistically significant. Other indicators such as blood loss, stones clearance rate, complication rate showed no statistical significance. Conclusion Combined techniques of rigid and flexible ureteroscopy was proceeded widely to improve stone clearance rate when dealing with multiple stones and staghorn calculi, while rigid-ureterscopy-based combination has advantages of shorter operation time and this combination should be promoted in top units.

ABSTRACT

Objective To investigate the relationship of RhoA and Snail expressions, and the invasion and metastasis in salivary adenoid cystic carcinoma (SACC). Methods The expressions of RhoA protein and Snail protein in 55 samples of SACC (SACC group ) and 20 samples of para-carcinoma normal tissues(control group) were detected using immunohisto?chemical method. The relationship between RhoA protein and Snail protein expressions and clinical and pathological charac?teristics were analyzed. Results The positive expressions of RhoA protein (69.1% vs 5.0%) and Snail protein (72.7% vs 10.0%) were significantly higher in SACC group than those in control group (P < 0.05). The positive expression rates of RhoA protein and Snail protein were significantly higher in patients with lymph node metastasis than those in patients with?out lymph node metastasis. The positive expression rates of RhoA protein and Snail protein were significantly higher in pa?tients atⅢ+Ⅳstage than those in patients atⅠ+Ⅱstage. The positive expression rates of RhoA protein and Snail protein were significantly higher in substantive carcinal tissues than those in screen roller type and tubular carcinal tissues. The posi?tive expression of Snail protein was significantly higher in substantive and tubular carcinal tissues than that in screen roller type carcinal tissues (P<0.05). There were no significant differences in positive expression rates of RhoA and Snail between different gender, age and different carcinal tissues. There was a positive correlation beween expression rates of RhoA and Snail protein in SACC (r=0.414, P<0.001). Conclusion RhoA and Snail may both facilitate the infiltration and metastasis of SACC through RhoA/ROCK/PKD1/NF-kappa B/Snail signaling pathways.

ABSTRACT

90 extracted single rooted human teeth were inoculated with E.faecalis.The cannals of the teeth were irrigated with MTAD, 5.25% NaClO,compound chlorhexidine gargle,tinidazole mouthwash,distilled H2 O(infection control)and autoclave followed by distilled H2 O(autoclaving control),respectively(n =1 5).Then E.faecalis infection of the samples were examined.The results showed that MTAD had significantly greater antibacterial effects than 5.25% NaClO,tinidazole mouthwashes,compound chlorhexidine gargle and tinidazole mouthwash(P <0.05).There was no significant difference between each 2 of the other 3 irrigants.

ABSTRACT

PURPOSE: To assess the safety and efficacy of an ultramini nephrostomy tract, which we were using for the first time, combined with flexible ureterorenoscopy (URS) in the treatment of pediatric patients with multiple renal calculi. MATERIALS AND METHODS: Twenty pediatric patients (age, < or =6 years) underwent ultramini percutaneous nephrolithotomy (PCNL) combined with flexible URS. The group had multiple renal calculi, which were bilateral in 3 cases and were located in a total of 23 sites. The calculi were located in 2 calyces in 10 cases, scattered in more than 2 calyces in 7 cases, and limited to 1 calyx in 3 cases. The average patient age was 37.35 months (range, 14-68 months). The average stone diameter was 2.0 cm (range, 1-3.0 cm). In all patients, an ultramini nephrostomy tract was established under ultrasound guidance (dilated to F10) with simultaneous sheath placement. The flexible URS was placed into the collecting system during holmium laser lithotripsy. RESULTS: When ultramini PCNL was combined with flexible ureterorenoscopic holmium laser lithotripsy, the complete stone-free rate was 87% (20/23). The average level of hemoglobin decreased to 1.0 g/dL after the operation. No blood transfusions were needed. Levels of blood urea nitrogen, creatinine, and C-reactive protein were not significantly different before and after the operation. The average duration of hospitalization was approximately 4.85 days, and all cases were followed up for 6 to 12 months. No complications were found. CONCLUSIONS: Ultramini PCNL combined with flexible ureterorenoscopic holmium laser lithotripsy is a safe and effective treatment for children with multiple renal calculi.

ABSTRACT

Objective To analyze the clinical treatment effect of non-gland-derived mass on the neck,and discuss the distribution types and the best treatment options.Methods A total of 76 cases with non-gland-derived mass on the neck were analyzed retrospectively,and the treatment effect was analyzed.Results In the 76 cases,25 cases were benign tumors,19 cases were malignant tumors,32 cases were non-rumors.Surgical treatment was preferred to tumors.According to the nature of the lesions,non-tumors mass shoud choose surgical treatment or medication.Conclusion Non-gland-derived mass on the neck with multiple presentations,it is necessary to choose personal therapies according to the characteristics of masses.

ABSTRACT

<p><b>OBJECTIVE</b>To study the radiobiological characteristic of human nasopharyngeal carcinoma cell lines CNE1 and CNE2 and the changes in expression MRN (Mre11-Rad50-Nbs1) complex in the cell lines exposed to irradiation.</p><p><b>METHODS</b>CNE1 and CNE2 were irradiated by a linear accelerator. Radiobiological characteristics were detected by colony assay and MTT assay. MRN complex expression were examined by Western blot.</p><p><b>RESULTS</b>Surviving fraction at 2 Gy (SF2), quasi-threshold Dose (Dq), and mean lethal dose (Do) of CNE1 were 0.56, 1.449 Gy and 1.480 Gy; SF2, Dq, and Do of CNE2 were 0.44, 0.776 Gy and 1.685 Gy, respectively. Survival fraction of CNE1 at the day 6 after 4 Gy irradiation was 0.59 and that of CNE2 was 0.79 when compared with control, with the up-regulated expressions of Rad50 in CNE1 and Mre11, Rad50 and Nbs1 in CNE2 (P < 0.05).</p><p><b>CONCLUSIONS</b>CNE1 and CNE2 were sensitive to radiation, but there were radioresistance cells in CNE2. The expressions of some components of MRN complex were up-regulated to repair DNA lesions induced by radiation.</p>

ABSTRACT

U.S. Renewable Fuel Standard (RFS) is a mandatory policy for promoting the utilization of biofuels in road transpiration sector in order to reduce the country's dependency on foreign oil and greenhouse gas emissions. U.S. Environmental Protection Agency (EPA) defines the proportion of renewable fuels according to RFS annual target, and requests obligated parties such like fossil fuel refiner, blenders and importer in the U.S. to complete Renewable Volume Obligation (RVO) every year. Obligated parties prove they have achieved their RVO through a renewable fuels certification system, which generates Renewable Identification Numbers (RINs) for every gallon of qualified renewable fuels produced or imported into U.S., RINs is a key for tracking renewable fuel consumption, which in turn is a key for implementing the RFS in the U.S., separated RINs can be freely traded in market and obligated parties could fulfill their RVO through buying RINs from other stakeholders. This briefing paper highlights RFS policy implementing mechanism and marketing tracking, mainly describes importance of RINs, and the method for generating and tracking RINs by both government and fuels industry participants.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effects of pre- and post-natal exposure to soy isoflavones (SIF) on the related indicators of the reproductive system development and ER-beta expression in male rat offspring.</p><p><b>METHODS</b>Pregnant SD rats were randomly divided into a corn oil group (control), 3 SIF groups (50, 200 and 400 mg/kg body weight), and a diethylstilbestrol (DES, 0.1 mg/ kg body weight) group to be treated respectively by intragastric gavage from pregnancy day 0. On postnatal day (PND) 21, the male offspring were treated in the same manner till sexual maturity, and on PND 49 and 90, 6 male rats from each group were killed for observation of the related indicators of the reproductive system development and the detection of the expression of ER-beta in the testis.</p><p><b>RESULTS</b>The mean daily food intake showed no evident differences among the SIF and DES groups, but the food efficiency ratio (FER) was significantly decreased in the 200 and 400 mg/kg SIF and 0.1 mg/kg DES groups (P < 0.05) at 49 and 90 PND. At the dose of > or = 50 mg/kg, SIF markedly reduced the body weight of the rats (P < 0.05), even more so at 90 PND than at 49 PND. The increased dose of SIF was correlated with the reduction of testis weight, sperm head count and daily sperm production, and so was it with the elevation of the ER-beta expression, both more obviously at 90 PND than at 49 PND.</p><p><b>CONCLUSION</b>Pre- and post-natal exposure to SIF affects the reproductive system development of male rat offspring, and the elevation of ER-beta expression may be one of its mechanisms.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the effect of transurethral resection of the prostate (TURP) in the treatment of urinary retention following seed implantation for prostate cancer.</p><p><b>METHODS</b>We treated 2 cases of urinary retention following seed implantation for prostate cancer by TURP, and analyzed their clinical data.</p><p><b>RESULTS</b>The operations were successful in both of the cases, with mean operation time of 60 min. Postoperative nocturnal urination of the patients averaged 2 times per night as compared with 5 - 6 times per night preoperatively, and their scores on IPSS and quality of life were significantly improved after surgery.</p><p><b>CONCLUSION</b>TURP affords satisfactory results in the treatment of urinary retention following seed implantation for prostate cancer patients.</p>

ABSTRACT

BACKGROUND: The hypospadias is usually repaired by the autologous tissues by surgery, but there is few local tissues available in patients with urinary leakage after I stage surgical repair, while other tissues require complex operations and induces large injury.OBJECTIVE: To observe the tissue engineered patches in the treatment of hypospadias, and to evaluate the biocompatibility and clinical efficacy.METHODS: Eight patients with urinary leakage after surgery for hypospadias repair underwent tissue patch I stage urethroplasty from January 2009 to October 2010. The curative effect and perioperative histocompatibility were determined.RESULTS AND CONCLUSION: Eight patients were uniformly successful after I stage surgery. One out of the 8 patients appeared mild urinary leakage and well cured by topical fibrin glue. There was no fever, local edema, skin tube and flap necrosis occurred during the postoperative hospital stay. All patients were followed up for 4-6 months, patients were observed to urinate standing, without urinary leakage, refractory prepuce edema and other complications.
