ABSTRACT

OBJECTIVE To investigate the mechanism of catalpol affecting the differentiation of helper T cell 17 (Th17) by interfering the expressions of pyruvate kinase M2 (PKM2) and lactate dehydrogenase A (LDHA). METHODS The naive CD4+ T cells were selected from the spleen of C57BL/6 mice, and were differentiated into Th17 cells by adding directional differentiation stimulants for 72 hours. At the same time, the cells were treated with 0 (directed control), 20, 40 and 80 μg/mL catalpol. The flow cytometry was used to detect the proportion of Th17 cell differentiation in cells; the colorimetric method was adopted to detect the levels of pyruvate and lactate in cell culture supernatant; mRNA expressions of retinoid-related orphan nuclear receptor gamma t (RORγt), PKM2 and LDHA were detected by qRT-PCR method; Western blot was used to detect the expression levels of PKM2, LDHA, signal transducer and activator of transcription 3 (STAT3), and phosphorylated STAT3 (p-STAT3) proteins in cells. RESULTS Compared with the directed control group, after 72 hours of treatment with 20, 40, 80 μg/mL catalpol, the differentiation ratio of Th17 cells were decreased by 6.74%, 8.41%, 9.24%, and the levels of pyruvate and lactate in the cell culture supernatant, the mRNA expressions of PKM2, LDHA and RORγt as well as the protein expressions of PKM2 and LDHA and the phosphorylation of STAT3 were significantly reduced (P<0.05). CONCLUSIONS Catalpol can reduce the glycolysis level by down-regulating the expressions of PKM2 and LDHA, thereby inhibiting the differentiation of Th17 cells.

ABSTRACT

Objective:To explore the impact of different six-character qigong training modes on respiratory muscle function early after a stroke.Methods:Sixty-six stroke survivors in the early stage of recovery were randomly divided into a control group, a modified training group, and an ancient training group, each of 22. In addition to routine rehabilitation training, the control group received conventional respiratory training. The modified training and ancient training groups were trained in modified six-character qigong or ancient six-character qigong, respectively, for two weeks. Before the treatment, after the two weeks and one month later, maximum inspiratory pressure (MIP) and maximum expiratory pressure (MEP), forced expiratory volume in one second, forced vital capacity, peak expiratory flow, maximum mid-expiratory flow, and peak inspiratory flow were measured. Diaphragm mobility during quiet inspiration and maximum inspiration were also quantified.Results:After 2 weeks of treatment and at 1 and 3 months after the end of the treatment, all three groups showed significant improvement in MIP, MEP and the pulmonary ventilation indicators, but the average improvement in the modified training group was significantly greater than in the other two groups. Their average diaphragm mobility was also significantly greater.Conclusion:Modified six-character qigong respiratory training is more effective than its ancient counterpart in improving respiratory muscle function, pulmonary ventilation, and diaphragm mobility early after a stroke, with effects which persist for at least one month.

ABSTRACT

Cryptococcal meningitis is a central nervous system infection caused by cryptococcal invasion of meninges and (or) brain parenchyma. It is characterized by severe intracranial hypertension and brain parenchyma damage. This opportunistic infection commonly occurs among HIV infected individuals, organ transplant recipients, and other immunosuppressive situations, and otherwise immunologically normal hosts. The global burden of human immunodeficiency virus/acquired immunodeficiency syndrome has declined over the years, but cryptococcal meningitis still has a high morbidity and mortality. The risk groups, clinical presentation, imaging characteristics, diagnostic criteria and therapeutic issues were reviewed in this article.

ABSTRACT

Objective:To observe the effect of customized orthotic insoles on the gait and balance of hemiplegic stroke survivors.Methods:Sixty stroke survivors with gait abnormalities were randomly divided into a group fitted with ankle foot orthoses (AFO) ( n=30) and a group who received customized orthotic insoles ( n=30). All received conventional rehabilitation training for 4 weeks. Before the fitting, as well as 8 hours and 4 weeks afterward, both groups were evaluated using the Tinetti gait scale (TGS), the plantar pressure balance index, the difference in length between their right and left step, step width, the Timed Up and Go test (TUGT), the Fugl-Meyer lower extremity assessment (FMA-LE), the 6-minute walk test (6MWT), a trunk impairment scale (TIS), the Berg Balance Scale (BBS) and the Barthel Index (BI). Results:At 8 hours after the fitting all of the insole group′s measurements were better than those of the AFO group, on average, but the differences were not statistically significant. After 4 weeks the average TGS, balance index barefoot and wearing the orthosis, step length difference, BBS and BI of the insoles group were significantly better than the AFO group′s averages. The other indicators were not significantly different.Conclusions:Customized orthotic insoles are more effective than an AFO in relieving the biomechanical abnormalities in hemiplegic patients′ feet and ankles, and enhancing their balance and gait.

ABSTRACT

ObjectiveTo form a questionnaire of high reliability and validity on mate preferences of postgraduates in military academy. MethodsIn December 2020, full-time postgraduates of a military academy were randomly selected as research objects, and their mate preferences were investigated via a self-compiled open-ended questionnaire. The item screening and expert consultation were carried out to compile an initial questionnaire on mate preferences of the postgraduates in military academy, and the questionnaire survey was conducted on 89 postgraduates in the military academy. Then the item analysis and exploratory factor analysis were performed to test the questionnaire, and the reliability and validity of the questionnaire were tested by calculating Pearson correlation coefficient, Cronbach's α coefficients and split-half coefficient. ResultsThe questionnaire on mate preferences of postgraduates in military academy has been formed, which consisting of 4 factors (intrinsic quality, personal ability, external conditions and quality of life) and 25 items was formed. The correlation coefficient between the total score of the questionnaire and the score of each factor ranged from 0.739 to 0.864, Cronbach's α coefficient of total score and each factor ranged from 0.723 to 0.926, and the split-half coefficient was between 0.682 and 0.899. ConclusionThe compiled questionnaire on mate preferences of postgraduates in military academy has good credibility and validity, and can be used as a tool to collect the data of psychological marriage and love on the mate preferences of postgraduates in military academy.

ABSTRACT

Yunüjian (Jing Yue's Collected Works), composed of Gypsum Fibrosum, Rehmanniae Radix Praeparata, Anemarrhenae Rhizoma, Ophiopogonis Radix, and Achyranthis Bidentatae Radix, is used for the treatment of "headache, toothache, consumptive thirst, and other diseases caused by heat and Yin deficiency in the stomach". In modern clinical settings, it has definite hypoglycemic efficacy, as it improves insulin resistance, inhibits oxidative stress and inflammatory response, and down-regulates estrogen level. It is often directly used, modified before use, combined with other formulae (Liuwei Dihuangtang, Shashen Maidongtang, Shenqi Sanjingtang, Huangqi Guizhi Wuwutang, Zengyetang, Zengye Baihutang, etc.), or used together with western medicine (insulin, metformin, pioglitazone, glurenorm, etc.). Modified Yunüjian can be applied for the treatment of type 2 diabetes mellitus, gestational diabetes, polycystic ovary syndrome with insulin resistance, pruritus induced by diabetes mellitus, diabetic peripheral neuropathy, diabetic nephropathy, diabetic ketoacidosis, diabetic retinopathy, and diabetic periodontitis. According to previous experiment, the protection for pancreatic islet β-cells is an important way of Yunüjian to improve diabetes mellitus. The formula can remove damaged proteins and organelles by increasing the expression of autophagy-related 2B (Atg2B), ubiquitin-like-conjugating enzyme (Atg3), γ-aminobutyric acid type A receptor-associated protein (GABARAP), selective autophagy receptor (p62/SQSTM1), etc. Moreover, it plays an active role in islet cell proliferation and apoptosis by regulating the expression of mammalian target of rapamycin (mTOR) and reducing the expression of islet β-cell autophagy gene (Beclin). These are pivotal for maintaining the quantitative structure and functions of islet cells. The modulatory effect of Yunüjian on growth hormone-releasing peptide (ghrelin), gastrin, and growth hormone secretagogue receptor (GHSR) mRNA expression is also the underlying mechanism for the formula to protect pancreatic β-cells. In this study, we summarized the clinical and experimental studies on the therapeutic effect of this formula on diabetes mellitus, discussed the mechanisms, and proposed suggestions on the problems, hoping to provide a reference for the clinical application and further development of Yunüjian. This study is of practical significance for the scientific interpretation of the modern connotation of Yunüjian and the expansion of its clinical application.

ABSTRACT

Hypervirulent Klebsiella pneumoniae (HvKP) is a new variant of Klebsiella pneumoniae. It is characterized by strong virulence and easy dissemination. It mainly causes liver abscess with multiple invasive infections, including eye, lung and central nervous system, with a high fatality rate. A case of severe intracranial infection caused by HvKP was reported. The patient was a 44-year-old formerly healthy man. He had acute onset of fever, headache, and disturbance of consciousness, which rapidly progressed to intracranial hypertension and respiratory failure. Cerebrospinal fluid examination suggested purulent infection, and bacterial culture suggested Klebsiella pneumoniae, which was sensitive to other commonly used antibiotics except ampicillin. Brain magnetic resonance imaging showed multiple abnormal signals in bilateral frontal, parietal and temporal lobes, right centrum semiovale, bilateral corona radiata, basal ganglia, thalamus and insula, as well as enhancement of meningeal and ependymal membrane, and swelling of brain tissue. During hospitalization, the patient developed a blood stream infection of pan-drug-resistant Klebsiella pneumoniae and was in critical condition. After aggressive treatment, the patient was cured and discharged from the hospital. After half a year follow-up, his prognosis was good and his social function was restored. The clinical data, diagnosis and treatment of the patient were reported and the literature was reviewed to provide clinical reference for the disease.

ABSTRACT

Objective@#To understand cognition and willingness of EV71 vaccination among parents of children, so as to provide scientific basis for vaccination.@*Methods@#A cross-sectional survey (census) was adopted. A questionnaire survey was conducted among parents of 2 783 preschool children from 12 kindergartens to analyze the parents’ cognition towards EV71 vaccine and willingness of vaccination of EV71 vaccine.@*Results@#Among the 2 783 parents surveyed, 1 843 heard of EV71 vaccine. The parents’ awareness rate of EV71 vaccine was 51.9%. The main source of EV71 vaccinerelated information was obtained through hospitals(n=887), followed by schools(n=752). 1 014 received EV71 vaccine. The main reason of EV71 vaccination was that vaccine was believed to be effective in preventing hand,foot and mouth disease(HFMD) (76.1%), while the main reason for unwillingness for vaccination was lack of knowledge(31.7%). And 2 478 considered that EV71 vaccine should be included in planned immunization. Multivariate Logistic regression analysis showed that children in public schools(OR=0.72, 95%CI=0.59-0.89), mothers from urban residents(OR=0.76, 95%CI=0.64-0.90), parents lack of EV71 vaccine awareness(OR=0.42,95%CI=0.35-0.49), and perceived high price of EV71 vaccine (OR=0.66, 95%CI=0.47-0.92) had a low vaccination rate. Higher EV71 vaccination rate was related to children’ status as baby class (OR=2.55,95%CI=1.18-5.52),bottom class (OR=2.24,95%CI=1.54-3.24) and middle class(OR=1.51, 95%CI=1.05-2.15)(P<0.05).@*Conclusion@#EV71 vaccination in preschool children in our jurisdiction was relatively high. School type, mother’s household registration, children’s grade and the parental percption of EV71 vaccine price are the main factors influencing EV71 vaccine vaccination in preschool children.

ABSTRACT

Objective@#To explore the association of the glycosylated hemoglobin (HbA1c) level at admission with 90 days functional outcome in patients with spontaneous intracerebral hemorrhage (ICH).@*Methods@#Patients admitted to the Department of Neurology, Tongji Hospital from January to December 2017 were prospectively and continuously enrolled in this study. Clinical data were collected at admission and functional outcomes 90 days after ICH were assessed by using the modified RANKIN scale. Univariate and multivariate conditional logistic regression models were constructed. Patients were divided into four groups according to the quartile of HbA1c values. The median value of HbA1c in each group was taken as the substitute value and P for trend was calculated. The logistic regression model was fitted by restricted cubic splines to investigate the association between HbA1c level and outcome of ICH.@*Results@#A total of 345 patients with ICH were enrolled, including 214 with favorable outcomes and 131 with poor outcomes (99 severe disability cases and 32 deaths). The risk of poor 90 days outcomes was significantly associated with HbA1c level at admission indicated by multivariate logistic regression analysis, and the P for trend test was <0.001 (middle-level group vs. low-level group: OR=2.33, 95%CI: 1.07-5.07; high-level group vs. low-level group: OR=2.52, 95%CI: 1.12-5.64; extremely high-level group vs. low-level group: OR=6.80, 95%CI: 3.01-15.34). Results from the restricted cubic spline showed that there was a linear correlation between HbA1c level at admission and poor 90 days outcomes of ICH (χ2=14.81, P<0.001; non- linear test: P=0.118). Compared with patients with HbA1c level of 6.5%, the risk of poor outcomes in patients with HbA1c level of <6.5% decreased linearly with the decrease in HbA1c level at admission, and the risk in patients with HbA1c level >6.5% was higher but not significantly.@*Conclusion@#There was correlation between high HbA1c level at admission and 90 days poor outcome of ICH. High HbA1c level is an independent prediction indicator for ICH.

Postoperative complications of ERCP include pancreatitis,cholangitis,hemorrhage,mechanical damage (such as perforation) of the digestive tract and drug reaction.Since the the complications definition,surgical indications,operation technique,data collection and processing method were inconsistent,there were controversies for the incidences of complications after ERCP and its risk factors for different institutions.This paper discussed the evolution of ERCP risk factors,current situation and prophylaxis of major complications after ERCP and summarized the current research progress in order to provide theoretical help for prevention and prophylaxis of postoperative complications of ERCP.

Rapid progression or aggravation of jaundice, coagulation abnormalities and liver-related complications in patients with alcoholic hepatitis indicates that they may develop severe alcoholic hepatitis. Some of these patients can progress acute-on-chronic liver failure with acute insults, such as infection and binge drinking and binge, showing as acute decompensation, organ failure and high 28-day mortality rate. Early identification and effective intervention can improve the prognosis of acute-on-chronic liver failure. Intestinal barrier dysfunction and liver-gut axis imbalance play an important role in the development of severe alcoholic hepatitis and acute-on-chronic liver failure. It is important to improve the basic nutritional status of patients. Effective drug therapies are limited in improving the condition and prognosis of acute-on-chronic liver failure. Early liver transplantation can bring great benefits to these patients.

ABSTRACT

Objective To explore the protective effect and mechanism of hydrogen-rich water (HRW) on ethanol-induced acute gastric injury.Methods The experimental study was conducted.Forty kunming mice were divided into the 4 groups by random number table method:normal control group [0.01 mL/g normal saline (NS)+ 0.03 mL/g NS],HRW group (0.01 mL/g NS +0.03 mL/g HRW),ethanol model group (0.01 mL/g 56°alcoholic drinks +0.03 mL/g NS),HRW treated group (0.01 mL/g 56°alcoholic drinks +0.03 mL/g HRW).Ten mice in each group were administrated twice a day for 7 days.Testing indicators:(1) gastric ulcer index was measured,(2) pathological examination of gastric tissues,(3) activity of serum superoxide dismutase (SOD) and expressions of malondialdehyde (MDA),interleukin 6 (IL-6) and tumor necrosis factor α (TNF-α) were measured,(4) expressions of SOD and MDA in gastric tissues were measured,(5) protein expressions of apoptosis related factors (caspase-3,bax,caspase-9,fas and caspase-8) in gastric tissues were measured by immunohistochemistry,(6) relative expressions of mRNA of apoptosis related factors (caspase-3,bax,caspase-9,fas and caspase-8) in gastric tissues were measured by realtime polymerase chain reaction (RT-PCR).Measurement data with normal distriburion were presented as (x)±s.Comparisons among groups were done using the one-way ANOVA and comparison between groups was done using the LSD-t test.Results (1) Gastric ulcer index was measured:gastric ulcer index of mice in the normal control group,HRW group,ethanol model group and HRW treated group were respectively 0,0,10.40± 1.64 and 3.92 ± 0.23,with statistically significant differences (F=175.050,P<0.05).There was a statistically significant difference between the ethanol model group and normal control group or HRW treated group (t =19.835,12.352,P< 0.05).(2) Pathological examination pathological examination of gastric tissues:① Macropathology of gastric tissues:the surface of the gastric mucosa was normal and smooth in the normal control group and the HRW group,without ulcer,erosion and inflammation.The partial gastric mucosal erosion and ulcer in the ethanol model group was large and very severe.Compared with the ethanol model group,the area of gastric mucosal lesion was reduced in the HRW treated group.② Results of pathological examination of gastric tissues:gastric mucosa in the normal control group and HRW group were integrity.Compared with the normal control group,the partial gastric surface epithelium was degenerate and impaired in the ethanol model group.Compared with the ethanol model group,the gastric mucosal erosion and inflammatory cell infiltration were alleviated in the HRW treated group.(3) Expressions of serum SOD,MDA,IL-6 and TNF-α:expressions of serum SOD,MDA,SOD/MDA and IL-6 were respectively (70±6)U/mL,(7.52±0.23) μmol/L,9.40 ± 1.07,(6.3 ± 1.8) ng/L in the normal control group and (74 ± 4) U/mL,(7.61 ±0.91) μmol/L,9.91 ± 1.55,(5.1 ± 1.6)ng/ L in the HRW group and (101 ± 4) U/mL,(16.95 ± 0.66) μmol/L,5.99±0.17,(19.2±4.9) ng/L in the ethanol model group and (115±5) U/mL,(14.02±0.58) μmol/L,8.23±0.32,(7.1±1.8)ng/L in the HRW treated group,with statistically significant differences among the 4 groups (F=97.405,269.950,16.486,25.663,P<0.05).The serum TNF-α levels were respectively (53± 14) ng/L,(67± 17) ng/L,(52± 13) ng/L,(58±21) ng/L in the above 4 groups,with no significant difference (F=0.862,P>0.05).(4) Expressions of SOD and MDA in gastric tissues were measured:expressions of SOD and MDA and SOD/MDA were respectively (93 ± 18) U/mL,(7.90± 1.72) μmol/L,12.48±4.54 in the normal control group and (93±13) U/mL,(6.96± 1.49) μmol/L,13.83±3.40 in the HRW group and (121±31) U/mL,(17.10±4.88) μmoL/L,7.88± 3.70 in the ethanol model group and (143 ± 26) U/mL,(7.31 ± 1.58) μmoL/L,20.00±4.68 in the HRW treated group,with statistically significant differences among the 4 groups (F=5.463,15.051,7.388,P< 0.05).(5) The expressions of apoptosis related factors in gastric tissues:the results of immunohistochemistry showed that the levels of caspase-3,bax and fas were repectively 0.065 5± 0.003 7,0.065 7±0.003 0,0.225 4±0.024 3 in the normal control group and 0.065 7±0.002 7,0.064 9±0.003 0,0.246 0±0.022 3 in the HRW group and 0.330 7±0.017 3,0.335 4±0.033 3,0.397 0±0.028 5 in the ethanol model group and 0.096 7±0.003 0,0.084 0.72±0.43,0.66±0.26,1.57±0.31,0.50±0.19 in the HRW group and 3.19±0.87,1.58±0.76,3.04± 1.15,2.84±0.98 in the ethanol model group and 0.49±0.16,0.69±0.25,2.98±0.85,0.53±0.24 in the HRW treated group,showing statistically significant differences among the 4 groups (F=32.106,5.038,9.706,23.387,P<0.05).The mRNA levels of caspase-8 were respectively 1.00±0.00,1.50±0.60,1.36±0.34,1.32±0.43 in normal control group,HRW group,ethanol model group and HRW treated group,with no significant difference among the 4 groups (F=1.337,P>0.05).Conclusions Hydrogen-rich water could alleviate ethanolinduced acute gastric injury by antioxidant,anti-inflammatory and anti-apoptosis.Hydrogen-rich water is safe and reliable,without toxic and side effects on the body.

ABSTRACT

Objective To understand patients with gestational diabetes mellitus (GDM)in postpartum dia-betes incidence,and analyze risk factors of postpartum diabetes.Methods 144 cases with GDMwere selected as the research objects,according to the 5 year follow -up results,they were divided into the abnormal glucose tolerance group,diabetes group and normal blood glucose group,blood glucose during pregnancy,HbA1c,before the number of pregnancy,the changes of BMI and age were compared,the risk factors of postpartum diabetes was observed.Results Gestational diabetes mellitus patients 5 years postpartum diabetes morbidity rate was 25%,Postpartum diabetes gesta-tional fasting blood glucose(5.6 ±1.2)mmol/L and HbA1c(4.3 ±1.6)% were higher than those of the abnormal glucose tolerance group(6.6 ±1.3)mmol/L,(6.7 ±2.4)% and normal blood glucose group (7.3 ±1.2)mmol/L, (8.2 ±2.9)%.The difference among the three groups had statistical significance(P <0.001);HOMA -beta index (82.4 ±39.5)were lower than the other two groups(P =0.001),Pregnancy HbA1c(OR =6.37,P =0.001)and fasting blood glucose(OR =4.32,P =0.003)as well as previous pregnancies (OR =5.51,P =0.006)were inde-pendent predictors of postpartum diabetesindications.Conclusion Pregnancy HbA1c and fasting blood glucose level can determine the risk of gestational diabetes mellitus patients with postpartum diabetes;Have a family history of dia-betes and previous history of pregnancy patients.The risk of postpartum diabetes mellitus increase.

Objective To explore the clinical effect of laparoscopic splenectomy caused by liver cirrhosis portal hypertension .Methods 38 patients treated with splenectomy as a result of hypersplenism induced by liver cir-rhosis portal hypertensions were selected .Laparotomy splenectomy was used in 18 cases ( OS group ) , laparoscopic splenectomy was used in 20 cases(LS group).Operation time , amount of bleeding during operation ,duration of hos-pital stays,liver and kidney function,blood routine,AYM were retrospectively analyzed.Results Compared with the preoperative group,LS ALT 2D after operation,postoperative 3~6d were significantly decreased(t=6.75,9.32,all P<0.05);3~6d AST levels after operation in LS group significantly decreased (t=8.87,P<0.05);LS group 2D after operation,postoperative 3~6d GGT levels were significantly lower than that of the preoperative (t=6.54,10.76, all P<0.05);group LS patients urea nitrogen levels falling postoperation ,there was statistically significant difference (t=3.43,P<0.05),leukocyte levels,PLT and HBG in the two groups increased postoperation ,these were statistical-ly significant difference(t=19.32,10.21,8.42,6.76,9.32,13.32,all P<0.05),OS group increased more obvious-ly.Conclusion LS has a better treatment effect on hypersplenism caused by liver cirrhosis ,which is worthy of popu-larization and application .

ABSTRACT

Objective To investigate the effect and safety of the recombinant tissue plasminogen activator ( rt-PA) combined with reduced glutathione ( RGSH ) in the treatment of acute ST-elevation myocardial infarction ( ASTEMI) .Methods 110 cases of ASTEMI were randomly divided into the control group and the observation group,55 cases in each group.The control group was intravenously injected 50mg rt-PA once a day,while the observa-tion group was injected 1200 mg RGSH freeze-dried powder once a day based on the control group .The vascular reca-nalization rate,the changes of myocardial enzymes ,cardiac structure and function parameters before and after treat-ment and adverse reactions in two groups were compared .Results The recanalization rate of the observation group and the control group reached 92.73%and 89.09% respectively,the difference between two groups was not signifi-cant (P>0.05).After treatment,the levels of CK,AST,cTnT,cTnI and CK-MB in the observation group were signifi-cantly lower than those of the control group (P<0.05),while the level of SOD was significantly higher than the con-trol group(P<0.05).The LVEDd and LVESd in the observation group were (52.24 ±5.83)mm and (39.95 ± 7.06)mm respectively,which were significantly lower than those in the control group (P <0.05),while the EF [(63.25 ±9.38)%] was significantly higher than that of the control group (P<0.05).The total adverse reaction rate in the observation group was 10.91%,which was significantly lower than 21.82% in the control group ( P<0.05).The reinfarction rate in the observation group was 3.63%,which was significantly lower than 12.73% in the control group(P<0.05).Conclusion The rt-PA routine thrombolysis therapy combined with RGSH could improve the recanalization rate and left ventricular structure and function of the ASTEMI patients and has less adverse reac -tion.The curative effect is better than single rt-PA.

ABSTRACT

Objective To investigate the effect of Niaoduqing Particles on the microinflammatory state in patients with maintenance hemodialysis. Methods Totally 240 patients with hemodialysis for more than 6 months were randomly divided into conventional hemodialysis group, Chinese medicine group (treated with Niaoduqing Particles) and western medicine group (treated with Vitamin E), 80 cases in each group, with 30 healthy people as control group. The levels of hs-CRP, IL-6 and HCY were detected before and after 1, 3, 6 months of treatment. Results Before treatment, the levels of inflammatory factors in hemodialysis patients were higher than control group. And after 3 months of treatment, the levels of hs-CRP, IL-6 and HCY of Chinese medicine group were decreased. And after 6 months of treatment, the levels of inflammatory factors were decreased obviously compared with pretherapy (P<0.01), and the difference with the conventional hemodialysis group was significant (P<0.01). Compared with pretherapy, the changes of inflammatory factors of western medicine group were statistically insignificant. Conclusion Niaoduqing Particles can significantly improve the microinflammatory state in maintenance hemodialysis patients, and the effect is superior to Vitamin E.

Objective To study the therapeutic effects and its possible mechanism ofBushen Paidu Mixture on renal interstitial fibrosis. Methods A total of 60 mice were randomly divided into a normal group,UUO group, Bushen Paidu Mixture group and Lotensin group. Mice models were induced by unilateral ureteral occlusion (UUO). Execute the mice at 7, 14, 21, and 28 days after UUO. The involved kidney were separated and had HE staining. Determine integrinβ1 in kidney tissue through immunohistochemical methods. Results Integrinβ1 tissues had a weak expression in normal kidney. Integrinβ1 expressed higher in UUO group than normal group. The expression of integrinβ1 in Bushen Paidu Mixture group and Lotensin group was lower than the UUO group at every timing point. There is no significant difference between the Bushen Mixture group and Lotensin group in terms of integrinβ1 expression. Conclusion Bushen Paidu Mixture can slow down the process of renal interstitial fibrosis through regulating integrinβ1 expression.

Chronic and heavy alcohol consumption is one of the causes of heart diseases. However, the effects of ethanol on insulin sensitivity in myocardium has been unclear. To investigate the effects of ethanol on the expression of AMP-activated protein kinase (AMPK), myocyte enhancer factor 2 (MEF2) and glucose transporter 4 (GLUT4), all of which are involved in the regulation of insulin sensitivity, in the myocardium, we performed three parts of experiments in vivo and in vitro. I: Rats were injected with 5-amino-4-imidazolecarboxamide ribonucleotide (AICAR, 0.8 mg.kg(-1)) for 2 h. II: Rats received different dose (0.5, 2.5 or 5 g.kg(-1).d(-1)) of ethanol for 22-week. III: Primary neonatal rat cardiomyocytes were isolated and treated with or without 100 mM ethanol or 1 mM AICAR for 4 h. The cardiac protein and mRNA expression of AMPKalpha subunits, MEF2 and GLUT4 were observed by western-blotting and RT-PCR, respectively. Serum TNFalpha levels were assessed by ELISA. The results showed chronic ethanol exposure induced insulin resistance. Ethanol decreased the mRNA levels of AMPKalpha1 and alpha2, the protein levels of total- and phospho-AMPKalpha in cardiomyocytes. Similarly, ethanol showed inhibitory effects on both the mRNA and protein levels of MEF2A and 2D, and GLUT4 in a dose-response-like fashion. Correlation analysis implied an association between phospho-AMPKalpha and MEF2A or MEF2D, and between the levels of MEF2 protein and GLUT4 transcription. In addition, ethanol elevated serum TNFalpha level. Taken together, chronic ethanol exposure decreases the expression of AMPKalpha and MEF2, and is associated with GLUT4 decline in rat myocardium.
