ABSTRACT

Objective:To investigate the effect of cardiopulmonary rehabilitation on motor function recovery in stroke patients. Methods:From February, 2018 to February, 2019, 100 stroke patients in convalescence stage were enrolled in this study. They were divided into control group (n = 50) and observation group (n = 50). The control group received routine rehabilitation, while the observation group received cardiopulmonary rehabilitation in addition. They were assessed with Fugl-Meyer Assessment (FMA), Self-rating Anxiety Scale (SAS), Self-rating Depression Scale (SAS) and pulmonary function test before and after treatment. Results:After treatment, the scores of FMA increased in both groups (t > 2.459, P < 0.05), which were higher in the observation group than in the control group (t = 2.678, P = 0.005); the scores of SAS and SDS decreased in both groups (t > 2.009, P < 0.05), which were lower in the observation group than in the control group (t > 3.261, P < 0.01); most of the forced vital capacity (FVC), the forced expiratory volume in 1 second (FEV1) and FEV1/FVC increased in both groups (t > 2.249, P < 0.05), which were higher in the observation group than in the control group (t > 2.147, P < 0.05). Conclusion:Cardiopulmonary rehabilitation could further promote the recovery of motor function of stroke patients, and improve the emotion and pulmonary function.

ABSTRACT

Objective:To compare the clinical efficacy of rehabilitation training at different stages on patients with anterior talofibular ligament injury after conservative treatment. Methods:From October 2017 to October 2018, 58 patients with anterior talofibular ligament injury were divided into six-week rehabilitation group (n = 29) and twelve-week rehabilitation group (n = 29), according to the time they began rehabilitation after injury. Visual Analogue Score (VAS) and American Orthopaedic Foot & Ankle Society (AOFAS) ankle-hindfoot function scoring system were used to evaluate ankle function before rehabilitation, and two weeks, four weeks and eight weeks after rehabilitation, respectively. Results:Two weeks, four weeks and eight weeks after rehabilitation, the scores of VAS and AOFAS improved in both groups (|t| > 4.137, P < 0.001), the scores of VAS were lower (|t| > 3.110, P < 0.01), and the scores of AOFAS were higher (|t| > 3.016, P < 0.01) in the six-week rehabilitation group than in the twelve-week rehabilitation group. Conclusion:It is suggested to begin rehabilitation within six weeks for patients with anterior talofibular ligament injury after conservative treatment.

ABSTRACT

Objective:To investigate the protective effect and mechanism of Baihe Gujin Tang on lipopolysaccharide induced acute lung injury (LPS-ALI). Method:KM mice were randomly divided into 5 groups:blank control group, model group, dexamethasone 0.002 g·kg-1 group, Baihe Gujin Tang (0.417, 1.25 g·kg-1) group. Except for the blank control group, the other groups were given LPS to induce the mouse ALI model. Except for the blank control group and the model group, the other groups were continuously given intragastric administration for 7 days on the 1st to 7th days before modeling. The lung tissue of the mice was taken 6 h after modeling, and the wet/dry mass ratio (W/D) of the left lung was measured. The serum levels of superoxide dismutase(SOD), malondialdehyde (MDA),reactive oxygen species (ROS)and nitric oxide (NO) were detected in the mice. Thepathological changes of the lung tissues were observed by hematoxylin-eosin(HE) staining. The expression levels of nuclear factor E2 related factor 2(Nrf2), Kelch-likeECH-associated protein 1 (Keap1), p62 and autophagy associated proteinsLC3Ⅱ proteins in the lung tissues were detected by Western blot. Result:Compared with the blank control group, the W/D of the model group was significantly increased (PPPP-1 group was significantly lower (P-1 group and dexamethasone group were able to significantly inhibited MDA levels in serum (PPPPPPConclusion:Baihe Gujin Tang has obvious protective effect on LPS-ALI mice, and its mechanism may be related to the regulation of Nrf2/Keap1/autophagy feedback loop.

ABSTRACT

Objective To evaluate the efficacy of endovascular interventional minimally invasive techniques in treatment of acute brachial artery injury.Methods A retrospective case series study was designed to analyzed data of 16 cases (11 males and 5 females) sustaining acute brachial artery injury treated from March 2011 to August 2016.Mean age was 47.9 years (range,27-66 years).Left upper extremity vascular injury occurred in 7 cases and right upper extremity vascular injury occurred in 9.There were 13 cases of arterial intima injury and 3 of arterial pseudoaneurysm.All patients underwent endovascular therapy including stent implantation and balloon dilatation.Angiography was used to evaluate the repair of brachial artery and the operation time was recorded.Improvements of distal limb arterial pulse and limb ischemic symptoms were observed,and the patients who were followed up with stent lumen stenosis more than 50% received endovascular treatment again.Results The procedures was successful in all patients,with operation time of 30-75 minutes (mean,45 minutes).After the insertion of a stent,recovered brachial artery blood flow and disappeared pseudoaneurysm were observed on angiography,without occurrence of stent stenosis or contrast extravasation.Symptoms of local pain and numbness were alleviated,and blood supply of the distal brachial artery was improved after operation.All patients were followed up for 3-36 months(mean,16.7 months).The false aneurysm did not recur.Two cases of stent stenosis more than 50% had a second balloon dilation procedure and the blood flow returned to normal.Stent patency was found in 14 cases with stent stenosis of less than 50%.During the follow-up period,no ischemic necrosis,displacement of the stent or stent fracture were found.Conclusion For acute brachial artery injury,endovascular management exhibits higher technical success rate and good clinical outcome with a low incidence of complications.

ABSTRACT

Objective To discuss the expression of RUNX2 and MDM21 in rats with periodontitis under the chronic intermittent hypoxia. Methods A total of 32 SD healthy rats were randomly divided into four groups, with 8 rats in each group. The molecular biological techniques of immunohistochemistry, RT-PCR and Western blotting were employed to detect the effect of different hypoxia time (0, 6, 12, 24 and 48 h) and different concentrations of hypoxia (0.000, 0.001, 0.010, 0.060 and 0.100 ppm) on the expression of RUNX2 and MDM21 in rats of four groups. Results The expression of RUNX2 and MDM21 in each group was significantly higher than the one at other concentrations when the concentration was 0.010 ppm, with the statistical difference (P normoxic periodontitis group > hypoxia control group > hypoxia periodontitis group under the action with the concentration of 0.010 ppm for 12 h, but there was no significant difference for the comparison among groups (P > 0.05). Conclusions The condition of chronic intermittent hypoxia can reduce the expression of RUNX2 and MDM21 in rats with periodontitis and aggravate the damage of periodontal bone.

ABSTRACT

Objective To investigate the correlation between antiphospholipid antibody syndrome and the early onset of preeclampsia. Methods From May 2010 to July 2013, one hundred and threecases in-patient treatment of the early onset preeclampsia were enrolled in this study. The maternal serum anticardiolipin antibodies(ACA)and anti-β2-glycoproteinⅠantibody (Aβ2-GPⅠ) were detected by ELISA method. 58 cases of pregnant women were randomly divided into the routine treatment group (30 cases) and the anticoagulant therapy group (28 cases). Results ACA positive predictive value of the early onset preeclampsia value was 3.9%. No significant difference was found in the prolonged anticoagulation of early onset preeclampsia time between the control group and the treatment group. Conclusion ACA may not be used to predict the early onset preeclampsia. Anticoagulation therapy can′t extend the early onset preeclampsia time and improve the outcome of pregnancy.

ABSTRACT

<p><b>OBJECTIVE</b>To analyze the reasons of intraoperative complications of microendoscopic disectomy (MED) and corresponding preventive measures.</p><p><b>METHODS</b>From October 2001 to January 2012, the data of 851 patients with lumbar disc herniation underwent MED were retrospectively analyzed. There were 469 males and 382 females with an average age of 42.5 years ranging 16 to 75. Course of disease was from 1 to 18 months with an average of 3 months. The segments of herniated disc including L3,4 of 24 cases, L4,5 of 418 cases and L5S1 of 409 cases . Main symptoms included low back pain with lower extremity radial pain and numbness. Of them,unilateral lower extremity symptom was in 729 cases and bilateral symptom was in 122 cases. There were at least 2 abnormal signs in the four signs which including feeling anormaly, muscle strength anormaly,dysreflexia and muscle atrophy. Distraction test of nerve was positive. CT or MRI findings must coincide with the clinical symptoms and signs. No lumbar instability,spinal stenosis,the upper lumbar disc herniation or combined with cauda equina nerve syndrome were found in 851 patients. The intraoperative complications were recorded and analyzed for the reasons of the intraoperative complication and related prevention measures.</p><p><b>RESULTS</b>According to the Macnab standard,424 cases obstained excellent results, 321 good,106 fair,with excellent and good rate of 87.5%. The result was similar to the traditional open operation. One cases transferred to open operation due to equipment breakdown, case died for myocardial infarction at 11 days after the operation, 2 cases occurred acute epidural hematoma in 1 hour after operation. Injury of dura mate of spinal cord occurred in 28 cases and incidence rate was 3.29%(28/851); traction injury of nerve root occurred in 38 cases and incidence rate was 4.46% (38/851). One case occurred in retroperitoneal hematoma, 2 cases in incomplete cauda equina injury and 2 cases in incomplete nerve root breakage.</p><p><b>CONCLUSION</b>Skilled endoscopic hemostasis techniques,careful and meticulous operation is very important for the prevention of intraoperative complications. Moreover,timely finding and treating the complications was effective measures to prevent the coniplications.</p>

ABSTRACT

Objective To estimate the clinical effect and application value of percutaneous transluminal angioplasty and stenting (PTAS) on patients with lower extremity atherosclerotic occlusive disease (LEAOD). Methods Percutaneous transluminal angioplasty (PAT) and PATS were performed on 127 patients with diabetic lower limb arteriosclerosis. Results Total achievement ratio was 97.64% (124/127). PTAS was failed in 3 patients, clinical symptoms disappeared or improved in 124 patients. The average ankle brachial index (ABI) before and after operation was 0.35 (0.11-0.58) and 0.87 (0.67-1.06), respectively. The difference in ABI before and after operation was significant (P<0.05). During 3-60 months following-up, arteria iliaca remained open in 53 patients, and no recurrence of clinical symptom was found. At 6, 12, 36 and 60 months after operation, the encumbered superficial femoral artery kept smooth bloodstream with a proportion of 97.78%, 91.11%, 84.44%, 75.56% in 45 patients in upper 2/3 segment and 89.66%, 79.31%, 72.41%, 65.52% in 29 patients in infer 1/3 segment, respectively. Conclusion Long-term vessel recanalization can be obtained effectively and haemodynamics index be remarkably improved with PTA and PTAS in LEAOD patients.

ABSTRACT

Objective To evaluate the clinical effect and feasibility in treating urethral stricture caused by benign prostatic hyperplasia in elder with transurethral implanted stent(Memotherm) under X-ray guided.Methods 47 patients included in this study,they ranged in age from 72 to 93 years,with an average age of 78.3 years,the symptom of dysuria ranged from 7 to 21 years.After 1% Lidocaine urethra mucous membrane anaesthesia,the processes of transurethral implanted stents at the most narrowing area of the urethra under X-ray guided were performed.Results The treating processes were successful in all cases(100%) one time,of them,the micturition in 43 cases was free and could act on one's own immediately after stent implantation.None having urethral restricture and lithiasis occured.Conclusion The clinical therapeutic effect was obvious using the memory alloy net-like stent for the treatment of urethral stricture caused by benign prostatic hyperplasia,it is worth promoting application in clinic.

Objective To evaluate the efficacy of the treatment of hilar biliary malignant obstruc tion with multiple stent drainage.Methods One hundred and twenty-seven patients with malignant biliary hilar obstruction were enrolled.The obstructions at the common hepatic duct within less than 1 cm to the junction of the left and the right hepatic duct were found in 66 cases,at the proximal common hepatic duct and the left and the right hepatic ducts in 45 cases,at the right hepatic duct in 5 cases and at the both left and right hepatic duets in 11 cases.Sixty-six patients received stent placement through the right biliary ducts and the common bile duct by puncturing the right mid-axillary line.The other 37 patients received 2 stents placement (disposed"Y"style) through the left and the right hepatic duct punc turing routway.Seven patients received 2 stents placement (disposed"┌"style) through the right hepatic duct and the common bile duct with a stent placed between the left and the right hepatic duct.Three patients had right hepatic duct stent placed first,followed by right hepatic duct and common hepatic duct stent. Twelve patients had stents placed in the right hepatic duct with external drainage from the left hepatic duct. Two patients had multiple strictures at the right hepatic duct,who got multiple external drainages.The total serum bilirubin levels were measured pre-and post-operatively.Results One hundred and twenty-seven patients with bi[iary obstraction had internal stents placed for drainage.The average total bilirubin levels among 121 patients were (283.4?175.4 )?mol/L pre-operation and (63.2?11.8)?mol/L post-operation (P

ABSTRACT

Objective To evaluate the effect of percutaneous transhepatic embolization of gastroesophageal varices on acute upper gastrointestinal bleeding in cirrhotic portal hypertensive patients. Methods Percutaneous transhepatic embolization was performed in 37 cases of acute upper gastrointestinal bleeding until varix thrombosed. Results Bleeding stopped in all patients. No technical complications occurred. During 1~38 months follow-up, variceal bleeding recurred in two cases in 3 months. One died of heart failure, five died of hepatic carcinoma in 6 months . Conclusion Percutaneous transhepatic embolization of gastroesophageal varices is a safe and effective treatment for acute upper gastrointestinal bleeding in portal hypertensive patients.

ABSTRACT

Objective To evaluate the interventional therapeatic efficacy of ureteral stricture or obstruction with two ways of percutaneous renal puncture and via urethra simultaneously.Methods 131 patients with ureteral stricture or obstruction were treated with both percutaneous renal puncture and via urethra.Results 149 ureters in 125 patients with benign ureteral stricture were treated with stents in this way,123 patients(149 ureters) were successful and 2 cases failed.9 ureters in 6 patients with malignant ureteral stricture or obstruction were treated with stents successfully.All patients were followed-up for 6 to 120 months,the clinical symptoms of patients were relieved or disappeared,and the infection was controlled,and the uretral obstruction wasn't noticed.Conclusion The treatment of ureteral strictures with percutaneous renal puncture in combination with via urethra is effective,easy and safe.

ABSTRACT

Objective To study elimination of the Micro CT projective image noise and improve the quality of the recon-structed image for the Micro CT system.Methods The forming factors of the Micro CT projective image noise were analyzed,and proposed the Micro CT projective image correction method to correct the random noise,dark field,light field,response nonuniformity and defect picture element and eliminate the random noise and the ring artifact respectively.Results Through the comparative study,it proposes a simple and practical correction method for the actual response nonuniformity which combines the corrections of light field,dark current and gain inconsistency and settles the three corrections with one circu-lation.The method can enhance the algorithm efficiency greatly and the validity of the correction protocol can be proved through experiments.Conclusion The Micro CT projective image noise is eliminated effectively and the good basis for the following work are established such as three-dimensional reconstruction.
