ABSTRACT

Objective To analyze factors that affect axial mechanical performance of the posterior pedicle fixation system. Methods The mechanical model for axial mechanical properties of the posterior pedicle fixation system was established to perform mechanical analysis. By referring to the Standard YY/T 0961-2014, tests on axial ejection force and axial clamping torque of JHL-I multi-axis components, JHL-I single-axis components, JHL-V multi-axis components and JHL-V single-axis components were performed by using Instron E3000 and Instron E10000 mechanical testing machines. Results The ejection rigidity of the single-axis component of vertebral nail system was significantly higher than that of the multi-axis component of vertebral nail system; the torsion rigidity of JHL-V was higher than that of JHL-I. The type and size of the locking bolt as well as its locking torque were the main factors affecting axial ejection force of the posterior spinal pedicle fixation system; the size of the locking bolt and the diameter of the connection rod were the main factors affecting axial clamping torque of the system; axial mechanical properties of the system could be improved by increasing the diameter of the connecting rod, the coefficient of friction between each connecting element, and the pitch diameter of the locking bolt. Conclusions The research findings provide references for optimizing and improving axial mechanical properties of the posterior pedicle fixation system.

ABSTRACT

Mitotic catastrophe (MC) is a form of programmed cell death induced by mitotic process disorders, which is very important in tumor prevention, development, and drug resistance. Because rapidly increased data for MC is vigorously promoting the tumor-related biomedical and clinical study, it is urgent for us to develop a professional and comprehensive database to curate MC-related data. Mitotic Catastrophe Database (MCDB) consists of 1214 genes/proteins and 5014 compounds collected and organized from more than 8000 research articles. Also, MCDB defines the confidence level, classification criteria, and uniform naming rules for MC-related data, which greatly improves data reliability and retrieval convenience. Moreover, MCDB develops protein sequence alignment and target prediction functions. The former can be used to predict new potential MC-related genes and proteins, and the latter can facilitate the identification of potential target proteins of unknown MC-related compounds. In short, MCDB is such a proprietary, standard, and comprehensive database for MC-relate data that will facilitate the exploration of MC from chemists to biologists in the fields of medicinal chemistry, molecular biology, bioinformatics, oncology and so on. Parkinson's disease (PD), known as one of the most universal neurodegenerative diseases, is a serious threat to the health of the elderly. The current treatment has been demonstrated to relieve symptoms, and the discovery of new small-molecule compounds has been regarded as a promising strategy. Of note, the homeostasis of the autolysosome pathway (ALP) is closely associated with PD, and impaired autophagy may cause the death of neurons and thereby accelerating the progress of PD. Thus, pharmacological targeting autophagy with small-molecule compounds has been drawn a rising attention so far. ABSTRACT

Objective:To analyze clinicopathological characteristics of trauma-related melanoma and their relationship with the prognosis of patients.Methods:Clinical data were collected from 87 cases of trauma-related melanomas in Department of Dermatology, Xijing Hospital, the Fourth Military Medical University from 2009 to 2020, and their clinicopathological characteristics were retrospectively analyzed. Mann-Whitney test was used to analyze the difference in Breslow thickness of tumors between patients of different ages or genders; Spearman rank correlation analysis was used to analyze the correlation between the time from trauma to the notice of melanoma and Breslow thickness, Kaplan-Meier survival analysis and Log-Rank test were employed to analyze the relationship between clinicopathological characteristics of melanoma and the prognosis of patients; Cox regression model was used to analyze risk factors for survival duration of patients with trauma-related melanoma.Results:Among the 87 patients with trauma-related melanoma, 47 (54.02%) were males and 40 (45.98%) were females. Among them, melanoma occurred in 50 (57.47%) cases after sharp injuries, and in 37 (42.53%) after blunt injuries. In addition, 31 (35.63%) cases presented with primary lesions on the hands, and 48 (55.17%) on the feet. The Breslow thickness of the primary tumors was significantly higher in the group aged > 55 years than in the group aged ≤ 55 years ( U= 623.500, P= 0.010) , but there was no significant difference between patients of different genders ( P= 0.138) . The time from trauma to the notice of melanoma was negatively correlated with the Breslow thickness of tumors ( r=-0.203, P= 0.037) . The age of patients, Breslow thickness of tumors, Ki67 proliferation index and genetic background of tumor significantly affect the survival duration of patients with trauma-related melanoma ( P= 0.011, 0.031, 0.002 and 0.031, respectively) ; the gender, type of trauma and ulceration of tumor mass did not significantly affect the survival duration of patients ( P= 0.618, 0.114 and 0.379, respectively) . Cox regression model analysis showed that the Ki67 proliferation index and Breslow thickness were independent risk factors affecting the prognosis of trauma-related melanoma (risk ratio [ RR] and 95% confidence interval [ CI] were 1.946 (1.234, 4.217) and 1.839 (1.014, 3.332) , P= 0.039 and 0.045, respectively) . Conclusion:The Breslow thickness of trauma-related melanoma is related to the age of patients and time from trauma to the notice of melanoma; the age, Breslow thickness of tumors, Ki67 proliferation index and genetic background of tumor all affect the survival duration of patients with melanoma, and Ki67 proliferation index and Breslow thickness are independent risk factors affecting prognosis.

ABSTRACT

@#This study was aimed to design the first dual-target small-molecule inhibitor co-targeting poly (ADP-ribose) polymerase-1 (PARP1) and bromodomain containing protein 4 (BRD4), which had important cross relation in the global network of breast cancer, reflecting the synthetic lethal effect. A series of new BRD4 and PARP1 dual-target inhibitors were discovered and synthesized by fragment-based combinatorial screening and activity assays that together led to the chemical optimization. Among these compounds, 19d was selected and exhibited micromole enzymatic potencies against BRD4 and PARP1, respectively. Compound 19d was further shown to efficiently modulate the expression of BRD4 and PARP1. Subsequently, compound 19d was found to induce breast cancer cell apoptosis and stimulate cell cycle arrest at G1 phase. Following pharmacokinetic studies, compound 19d showed its antitumor activity in breast cancer susceptibility gene 1/2 (BRCA1/2) wild-type MDA-MB-468 and MCF-7 xenograft models without apparent toxicity and loss of body weight. These results together demonstrated that a highly potent dual-targeted inhibitor was successfully synthesized and indicated that co-targeting of BRD4 and PARP1 based on the concept of synthetic lethality would be a promising therapeutic strategy for breast cancer.

ABSTRACT

Currently, biomanufacturing technology and industry are receiving worldwide attention. However, there are still great challenges on bioprocess optimization and scale-up, including: lacing the process detection methods, which makes it difficult to meet the requirement of monitoring of key indicators and parameters; poor understanding of cell metabolism, which arouses problems to rationally achieve process optimization and regulation; the reactor environment is very different across the scales, resulting in low efficiency of stepwise scale-up. Considering the above key issues that need to be resolved, here we summarize the key technological innovations of the whole chain of fermentation process, i.e., real-time detection-dynamic regulation-rational scale-up, through case analysis. In the future, bioprocess design will be guided by a full lifecycle in-silico model integrating cellular physiology (spatiotemporal multiscale metabolic models) and fluid dynamics (CFD models). This will promote computer-aided design and development, accelerate the realization of large-scale intelligent production and serve to open a new era of green biomanufacturing.

ABSTRACT

Objective:To evaluate the effect of anesthesia factor on postoperative outcome in patients undergoing thoracoscopy through a comparison between different analgesic methods.Methods:One hundred patients of both sexes, aged 18-64 yr, with body mass index of 18.5-24.0 kg/m 2, of American Society of Anesthesiologists physical statusⅠ or Ⅱ, scheduled for elective single-hole thoracoscopic surgery, were divided into 4 groups ( n=25 each) using a random number table method: control group (group C), erector spinae plane block (ESPB) group (group E), thoracic paravertebral nerve block (TPVB) group (group T), and ESPB combined with TPVB group (ET group). Before induction of anesthesia, ultrasound-guided nerve block was performed, and ESPB, TPVB, and ESPB combined with TPVB were performed on the affected side in E, T and ET groups, respectively.The mixture of 0.5% ropivacaine 20 ml and dexamethasone 5 mg was injected in T and E groups, and 0.5% ropivacaine 5 ml was injected into the paravertebral space, the needle tip was retracted to touch the transverse bone, and then another 15 ml was injected in group ET.The block plane was detected by acupuncture at 15 min after completion of the nerve block, and successful nerve block was defined as the block plane reaching T 4-6 in the areas innervated by the spinal nerve.Both groups received patient-controlled intravenous analgesia(PCIA) and were admitted to postanesthesia care unit after surgery.Flurbiprofen axetil 50 mg was intravenously injected as rescue analgesic when the visual analog scale(VAS)score was more than 3.Venous blood samples were collected immediately before nerve block operation (T 0), 30 min after operation (T 1), 30 min after tracheal extubation (T 2), and 24 h after operation (T 3) for determination of the concentrations of plasma cortisol and adrenocorticotropic hormone (ACTH) by enzyme-linked immunosorbent assay.The VAS score for cough was recorded at 15 min after tracheal extubation (T 4) and 6, 12, 24 and 48 h after surgery (T 5-8). The patients were followed up at 48 h after surgery.The pressing times of PCIA, consumption of flurbiprofen, and development of nausea and vomiting were recorded. Results:Compared with group C, VAS scores for cough were significantly decreased at T 4-6, the pressing times of PCIA was reduced, and the consumption of flurbiprofen, and incidence of nausea and vomiting and plasma Cor and ACTH concentrations at T 1-2 were decreased in the other three groups ( P<0.05). Compared with group E, VAS scores for cough at T 4, pressing times of PCIA and consumption of flurbiprofen were significantly decreased in T and ET groups ( P<0.05). There was no significant difference in each index between ET group and T group ( P>0.05). Conclusion:TPVB performs better than ESPB and TPVB combined with ESPB in improving the postoperative outcome of patients undergoing thoracoscopy.

Vascular aging refers to degenerative changes in vascular structure and function.Based on speckle-tracking echocardiography, velocity vector imaging can be used to comprehensively evaluate the mechanical characteristics and wall functions in the process of vascular aging and plays an important role in the early detection and treatment of vascular age-related diseases by accurately locating the vessels and three-dimensionally evaluating the velocity, strain and strain rate in different directions.In this article, the application of this technique in the evaluation of vascular aging and related diseases including hypertension, diabetes and atherosclerosis is described.

ABSTRACT

The national outbreak of coronavirus disease 2019 (COVID-19) has brought a severe challenge to the management of standardized residency training (SRT). To protect SRT residents from being infected by 2019 novel coronavirus (2019-nCoV), to guarantee the training program well carried out, and to prevent psychological health problems are conundrums to the management of SRT. In this article, the specific countermeasures are introduced from the following aspects: perfecting the management system, implementing quarantine and life support, conducting epidemic prevention and control training, turning training patterns suitable to epidemic prevention, and maintaining the psychological health of the residents. And we expect to provide references for SRT management under the public health emergency in the future.

Furthermore, intelligent control of industrial bioprocess is further addressed, in terms of new sensing technology for intracellular physiological metabolism, big database establishment and data depth calculation, intelligent decision.

Objective On whole-genome scale,we tried to explore the correlation between obesity-related traits and DNA methylation sites,based on discordant monozygotic twin pairs.Methods A total of 90 pairs of 6-17 year-old twins were recruited in Chaoyang district,Yanqing district and Fangshan district in Beijing in 2016.Information on twins was gathered through a self-designed questionnaire and results from physical examination,including height,weight and waist circumference of the subjects under study.DNA methylation detection was chosen on the Illumina Human Methylation EPIC BeadChip.R 3.3.1 language was used to read the DNA methylation signal under quality control on samples and probes.Ebayes function of empirical Bayes paired moderated t-test was used to identify the differential methylated CpG sites (DMCs).VarFit function of emp irical Bayes paired moderated Levene test was used to identify the differentially variables CpG sits (DVCs) in obese and normal groups.Results According to the obesity discordance criteria,we collected 23 pairs of twins (age range 7 to 16 years),including 12 male pairs.A total of 817 471 qualified CpG loci were included in the genome-wide correlation analysis.According to the significance level of FDR set as <0.05,no positive sites would meet this standard.When DMC CpG site cg05684382,with the smallest P value (1.26E-06) as on chromosome 12,the DVC CpG site cg26188191 with the smallest P value (6.44E-06) appeared in CMIP gene on chromosome 16.Conclusions In this study,we analyzed the genome-wide DNA methylation and its correlation with obesity traits.After multiple testing corrections,no positive sites were found to have associated with obesity.However,results from the correlation analysis demonstrated sites cg05684382 (chr:12) and cg26188191 (chr:16) might have played a role in the development of obesity.This study provides a methodologic reference for the studies on discordance twins related problems.

Objective:To explore clinical and fistula origin characteristics of congenital coronary artery fistula (CAF).Methods:Clinical and coronary angiographic (CAG)data of 37 congenital CAF patients,who were treated in our hospital from Apr 2011 to Aug 2016 and diagnosed by CAG,were retrospectively analyzed.Chief complaint symptoms were analyzed.According to fistula origin location,patients were divided into left anterior descending (LAD)group (n=23),right coronary artery (RCA)group (n=6),LAD & RCA group (n=2),left circumflex (LCX)group (n=5)and left main (LM)group (n=1).Characteristics of fistula origin and drainage location dis-tribution were analyzed.Results:Among the chief complaints of 37 congenital CAF patients,there were 24 cases (64.9%)of chest tightness and short of breath,six cases (16.2%)of chest pain,five cases (13.5%)of palpitations and two cases (5.4%)of heart murmur in physical examination.Compared with RCA group,LAD & RCA group, LCX group and LM group,there was significant rise in percentage of LAD fistula origin (16.2%,5.4%,13.5%, 2.7% vs.62.2%),P <0.01 all,but there were no significant difference among other groups,P >0.05 all.The 23 cases of LAD fistula origin included 22 cases of pulmonary artery fistulas (PAF)and one case of bronchial artery fis-tulas (BAF);the six cases of RCA fistula origin included three cases of PAF,two cases of right atrial fistulas (RAF) and one case of left ventricular fistulas (LVF);the five cases of LCX fistula origin included two cases of RAF,and one case of PAF,LVF and RVF respectively.Conclusion:Coronary angiography is the gold standard to diagnose congenital CAF.Percentage of congenital CAF originated from left coronary artery is significantly more than that of right coronary artery.

ABSTRACT

Objective To analyze the effect of mechanical press and manual press in cardiopulmonary resuscitation.Methods A prospective randomized controlled study was performed.A total of 95 cases who needed cardiopulmonary resuscitation were collected,according to the inclusion criteria,which including mechanical compression group (n =48),and the manual compression group (n =47).Demographic,clinical course of two groups were collected.Arterial and venous blood gas analysis in the recovery after the start of 5 minutes,15 minutes,30 minutes,the end of recovery were also collected and the pH value,partial pressure of oxygen,extracellular base remaining,actual bicarbonate,lactic acid,calcium and central venous oxygen saturation (ScvO2) and other data were mainly analyzed.All data collation and statistical analysis were used Microsoft excel,SAS 9.4.The enumeration data were expressed by the frequency and percentage,and chi square test was used.The measurement data were expressed by mean and standard deviation,and t test was used.Multiple Logistic regression analysis was performed to analysis of the effect of various factors.Results There were no statistically differences in age and pathogenic factors between the mechanical compression group and manual compression group (P > 0.05).The press time,success rate of resuscitation and hospitalization days were respectively (22 ±14) minutes,81%,(37 ± 16) days in the mechanical compression group and (27 ± 19) minutes,54%,(41 ±20) days in the manual compression group,with no statistically significant differences (all P > 0.05).Mechanical compression group and manual compression group were cured in 17 cases,10 cases,survived in 21 cases,9 cases,deaded in 10 cases,28 cases,with statistically significant differences between 2 groups (P <0.05).The troponin Ⅰ and creatine kinase MB were respectively (4.1 ± 2.0) ng/ml,(31 ± 15) U/L in the manual compression group and (5.6 ± 1.9) ng/ml,(43 ± 22) U/L in the mechanical compression group,with statistically significant differences between 2 groups (P < 0.05).The defibrillation energy and defibrillation times in manual group were significantly higher than that of mechanical compression group (P < 0.05).Multivariate logistic regression analysis showed that the central venous oxygen saturation at the first 5 minutes after the start of compression (x2 =65.0538,P < 0.0001),the arterial blood pH at the end of compression (x2 =21.5779,P < 0.0001),and the 5% sodium bicarbonate was significantly correlated with the prognosis of survival after resuscitation,mechanical compression group was better than the manual compression group (x2 =3.1421,P =0.0463).Conclusions The effect of cardiopulmonary resuscitation machine is better than that of manual compression,which can effectively improve the clinical treatment effect.The result recommends the use of cardiopulmonary resuscitation machine for patients with cardiac arrest.

ABSTRACT

In the article, the development of medical treatment for eye injuries in the mainland of China was reviewed. According to the data provided in Eye Injury Vitrectomy Study (EIVS), 27% of 72 eyes with no light perception (NLP) gained recovery in term of antomy and visual function. Vitrectomy initiated at more than 4 weeks after open eye injury is an independent risk factor for developing PVR. Prognosis of anatomy and visual function of the injured eye with PVR is markedly worse than that without PVR. Serious injuries of ciliary body, choroid and retina are three key parts of the eye with NLP. The concept that the treatment of the eye injury gradually focus on the whole globe is embodied. The data from 13575 in patients with traumatic eyes in 14 hospitals revealed that the rate of immediate enucleation was remarkable reduced with comparison of 20 years ago.

Objective To discuss the application of Germany Cooperation for Transparency and Quality in Health Care (KTQ) quality certification system in the field of nursing management. Methods The Plan-Do-Check-Act (PDCA) from KTQ was used in the management of ward nursing quality. The post-interventional quality of basic nursing, critical nursing, health education, ward management, nursing safety and disinfection and isolation were compared and contrasted with the pre-interventional one. Result The quality of basic nursing, critical nursing, health education, ward management, nursing safety and disinfection and isolation were significantly improved after intervention with PDCA from KTQ (P<0.01). Conclusion The KTQ quality certification system can be applied to improve the nursing quality in the process of nursing management.

ABSTRACT

Objective To evaluate the effects of interferon therapy after curative surgical intervention on improving prognosis of patients with hepatitis C-related hepatocellular carcinoma (HCC).Methods We searched randomized clinical trials from 1990 to 2015 on interferon therapy after curative surgical intervention in patients with hepatitis C-related HCC from the Cochrane Library,the Cochrane Database of Systematic Reviews,MEDLINE and Embase.A Meta-analysis was carried out using Revman 5.Results There were 7 studies included in this research.The results showed that interferon therapy after curative surgical intervention in patients with hepatitis C-related HCC reduced the recurrence rate of HCC at 3 years (RR =0.84,95% CI 0.73 ~0.97,P <0.05).The therapy could not improve the 3-year survival rate in these patients (RR =1.04,95% CI 0.90 ~ 1.21,P > 0.05).Stratified subgroup analyses showed interferon therapy after liver resection reduced the recurrence rate (RR =0.62,95 % CI 0.39 ~ 1.00,P =0.05).For patients with tumors less than 3 cm,interferon therapy reduced the recurrence rate (RR =0.82,95% CI 0.69 ~ 0.98,P < 0.05).Conclusion Interferon therapy after curative surgical intervention improved prognosis in patients with hepatitis C-related HCC.
