ABSTRACT

Acute intermittent porphyria(AIP) is a rare inherited metabolic disease that can cause severe and fatal acute attacks. This article shares the treatment and management of a severe AIP patient. It is proposed that (1) avoiding incentives is essential; (2) emotional problems easily overlooked should be paid attention; (3) long-term follow-up and patient education can improve the prognosis. The patient underwent renal biopsy during the remission period. We found a red-brown-yellow-white refractive index crystal under a polarized light microscope that had not been reported in the previous literature, which was speculated to be a porphyrin crystal.

ABSTRACT

Objective To analyze the imaging characteristics of 18 F-fluorodeoxyglucose ( FDG) PET/ CT in patients with multiple myeloma (MM). Methods Sixty-eight patients (41 males, 27 females, age (61.5±17.2) years) with pathologically proven MM were retrospectively reviewed between January 2011 and August 2016. Imaging characteristics of 18 F-FDG PET/ CT in patients were analyzed. Maximum stand-ardized uptake value (SUVmax ), score of the bone involvement (bones of whole body were classified into 10 groups, and when 1 group was involved, the score was 1), the number of bone lesions and soft tissue swell-ing around bone lesions were investigated. Results A total of 1310 lesions were detected in 68 MM pa-tients. The SUVmax varied widely and mild metabolic activity (2.5≤SUVmax<5.0) was observed in the high-est proportion of MM patients (49.8%, 652/ 1310). The SUVmax of MM patients was 6.63±3.02. The score of bone involvement was 4.49±3.01. The number of bone lesions was 10.50(3.00, 33.00), and soft tissue swelling around bone lesions was observed in 22 patients. Conclusions There are special imaging charac-teristics of 18 F-FDG PET/ CT in MM patients, such as multiple osteolytic lesions with mild to moderate met-abolic activity, clear boundary and extensive bone involvement, and lesions usually limit to the skeletal sys-tem. The characteristics are of certain value in the diagnosis and differential diagnosis of MM.

ABSTRACT

Objective:To investigate the effects of Exendin-4 (Ex-4) on the expressions of lipid metabolism related genes in the human liver cancer HepG2 cells with insulin resistance (IR),and to elucidate the effect of Ex-4in improvement of IR.Methods:The HepG2 cells in logarithmic growth phase were induced into IR model with high concentration of insulin,then divided into control group (HepG2 cells),IR group (HepG2 cells were treated with insulin,HepG2-IR cells),and Ex-4 group (HepG2-IR cells were treated with Ex-4).Glucose oxidase (GOD-POD)kit was used to detect the consumption of glucose.The cell morphology and intracellular lipid drip formation were observed by Oil red O staining.The triglyceride (TG) level in cells was detected by kit;qRT-PCR was used to detect the mRNA expression levels of acetyl-CoA carboxylase (ACC),fatty acid synthase (FAS),sterol regulatory element-binding protein-1c (SREBP-1c) and apolipoprotein B100 (apoB100).Results:Compared with control group (HepG2 cells),the glucose consumption in the HepG2-IR cells in IR group was significantly decreased (P<0.01).Compared with IR group,the glucose consumption in the HepG2-IR cells in Ex-4 group was increased (P<0.05).The Oil O red staining results showed that compared with control group,the fat percentage in the HepG2-IR cells in IR group was increased (P<0.05);compared with IR group,the fat percentage in Ex-4 group was decreased (P<0.05).Compared with control group,the level of TG in the cells in IR group was significantly increased (P<0.01);compared with IR group,the level of TG in the cells in Ex-4 group was significantly decreased (P<0.05).The qT-PCR results showed that compared with control group,the expression levels of ACC FAS and SREBP-1cmRNA in the cells in IR group were increased (P<0.01),and the expression level of apoB100 mRNA was decreased (P<0.05);compared with IR group,the expression levels of ACC,FAS and SREBP-1c mRNA in the cells in Ex-4 group were decreased (P<0.05),and the expression level of apoB100 mRNA was increased (P<0.01).Conclusion:Ex-4 can regulate the expressions of lipid metabolism related genes in the HepG2 cells and improve IR.

OBJECTIVE@#To observe the morphological alterations in pharyngeal ostium of Eustachian tube and its surrounding tissues and its correlation with secretory otitis media.@*METHOD@#72 volunteers (144 ears) as control group, 89 patients (151 ears) as SOM group, all were observed for the shape of pharyngeal opening of auditory tube and the morphological changes of its surrounding structures.@*RESULT@#Statistic difference was found between the shape of pharyngeal ostium of eustachian tube of adult patients with SOM and that of control group, but for children, there was no difference. It indicated that there was a difference between adult and children in morphological changes of pharyngeal ostium of eustachian tube when suffered from SOM.@*CONCLUSION@#Morphological changes of pharyngeal ostium of eustachian tube play an important role in the genesis and development of adult SOM . The primary change in children patients is the inflammation of nasopharynx, then is the hypertrophy of adenoid and tubal tonsil. Nasopharyngeal-fibroscope is a very useful tool in the diagnosis and treatment of SOM.

ABSTRACT

The level of serum lipid peroxide in rat is increased as the age of rat increased gradually. When the animals were injected with sodium selenite (0.1mg/kg) and vitamin E (4mg/kg) for five weeks the serum lipid peroxide level was decreased, the combined use of sodium selenite and vitamin E could decrease the level of SGPT apparently.
