ABSTRACT

Bladder wall cysts are clinically rare, and even rarer when occurring simultaneously with bladder cancer. In this paper, we reported a patient with bladder cancer combined with a bladder wall cyst, who was admitted due to the urinary frequency and gross hematuria. Preoperative ultrasound and CT examination revealed a tumor in the left and right anterior wall of the bladder respectively. The cystoscopy showed low-grade uroepithelial carcinoma of the left wall of the bladder and occupancy of the right anterior wall of the bladder. Transurethral plasma enucleation of bladder tumor and suprapubic small incision partial cystectomy was performed. Postoperative pathology confirmed high-grade uroepithelial carcinoma of the left wall of the bladder and cyst of the right wall. There was no recurrence or metastasis 15 months after surgery.

Objective:To analyze the epidemiological characteristics and clinical features of the patients with 2019-nCoV infection, so as to provide basis for clinical diagnosis.Methods:The epidemiology, clinical symptoms, laboratory and radiologic data of 23 patients with 2019-nCoV infection admitted to the Fifth People's Hospital of Xinyang City from January 22,2020 to January 29, 2020 were retrospectively analyzed.Results:The 23 patients with 2019 nCov infection consisted of 15 men and 8 women, and the median age was 46.0 (40.5, 52.0) years (27-80 years); 9 of them had basic disease (39%), including hypertension (17%), cardiovascular diseases (17%), diabetes (9%), hypothyroidism (4%) and old tuberculosis (4%). All the 23 patients had contact history in Wuhan area or with confirmed infections. Clinical symptoms included: fever (100%), cough (70%), expectoration (43%), myalgia (26%), headache (17%) and dyspnea (17%), and the less common symptoms were diarrhea (4.3%). Blood routine test: white blood cells (WBC) < 4×10 9/L in 11 cases (48%), (4-10)×10 9/L in 10 cases (43%), >10 × 109/L in 2 cases (9%); lymphocytopenia in 13 cases (56%). All 23 patients had different degrees of infective lesions in chest CT examination, with 9 cases (39%) on one side and 14 cases (61%) on both sides. Classification: 19 mild cases, 4 severe cases, no critical or death case. Complications included acute respiratory distress syndrome [4 (17%)]. No case was reported with the damage of liver or kidney function and with secondary infection. Conclusions:Epidemic history of contact, fever, pneumonia signs of chest CT, normal or decreased count of WBC and lymphocytopenia are the clinical basis for diagnosis of the disease. However, at present, the treatment of patients has not been completed, the effective treatment strategy and final prognosis are not clear.

ABSTRACT

Objective:To analyze the epidemiological characteristics and clinical features of the patients with coronavirus disease 2019 (COVID-19), so as to provide basis for clinical diagnosis.Methods:The epidemiology, clinical symptoms, laboratory and radiologic data of 23 patients with COVID-19 admitted to the Fifth People's Hospital of Xinyang City from January 22nd to January 29th, 2020 were retrospectively analyzed.Results:There was 23 patients with COVID-19, with 15 men and 8 women, and the median age was 46.0 (40.5, 52.0) years old (ranged from 27 years old to 80 years old). Nine patients had basic disease (39.1%), including hypertension (17.4%), cardiovascular diseases (17.4%), diabetes (8.7%), hypothyroidism (4.3%) and past history of tuberculosis (4.3%). All the 23 patients had contact history in Wuhan area or with confirmed cases. Clinical symptoms included fever (100%), cough (69.6%), expectoration (43.5%), myalgia (26.1%), headache (17.4%) and dyspnea (17.4%), and the less common symptom was diarrhea (4.3%). Blood routine tests showed leukocytopenia in 11 patients (47.8%), normal leukocyte counts in 10 patients (43.5%), and leukocytosis in 2 patients (8.7%); lymphopenia was found in 13 patients (56.5%). All 23 patients had different degrees of infective lesions in chest CT, with 7 patients (30.4%) on one side and 16 patients (69.6%) on both sides. There were 19 mild patients, 4 severe patients, and no critical or death case. Complications included acute respiratory distress syndrome (17.4%). No patient was reported with liver, kidney or heart dysfunction or secondary infection.Conclusions:Epidemic history of contact, fever, pneumonia signs of chest CT, normal or decreased count of leukocyte and lymphopenia are the clinical basis for diagnosis of COVID-19. However, at present, the treatment of patients has not been completed, and the effective treatment strategy and final prognosis are unclear.

Objective@#The successfully diagnosis and treatment of a woman with 38 weeks singleton pregnancy complicated with novel coronavirus pneumonia (critical type), and a case of neonatal pneumonia with 2019-nCoV infection were retrospectively analyzed.@*Methods@#The successfully diagnosis and treatment of a woman with 38 weeks singleton pregnancy complicated with novel coronavirus pneumonia (critical type), and a case of neonatal pneumonia with 2019-nCoV infection were retrospectively analyzed.@*Results@#A single male was successfully delivered at 38-week gestation of his mother by cesarean section under third level protection in operation room. The delivery woman was diagnosed with 2019-nCoV infection at day 2 of delivery. Dyspnea and severe hypoxemia soon developed, and invasive mechanical ventilation was given. After active rescue and treatment, the delivery woman had been taken off line successfully and the condition was stable. Pharyngeal swab specimen of the neonate was sent for examination 3 days after birth, and was positive for novel coronavirus nucleic acid by fluorescence reverse transcript polymerase chain reaction.@*Conclusion@#2019-nCoV may be transmitted vertically from mother to child.

ABSTRACT

Objective To explore the clinical effect of traditional Chinese medicine on lipid metabolism dis-orders in hemodialysis patients.Methods 70 patients with hemodialysis received in the hospital from Novem-ber 2015 to April 2017 were enrolled in the study.They were randomly divided into control group and treat-ment group with 35 cases in each group.The control group was treated with simvastatin and and the treatment group was treated with self-made fat recipe combined with simvastatin.The levels of serum lipid ,serum in-flammatory cytokines ,renal function and liver function ,the total effective rate of treatment and the incidence of adverse reactions were compared between the two groups before and after treatment.Results There were no statistical significances in serum lipid indexes such as triglyceride (TG) ,total cholesterol (TC) ,high densi-ty lipoprotein cholesterol (HDL-C) ,low density lipoprotein cholesterol (LDL-C) ,C reactive protein (CRP) , interleukin 6 (IL-6) ,tumor necrosis factor α(TNF-α) and other serum inflammatory factors ,and renal func-tion indexes ,including urea nitrogen (BUN) ,creatinine (Scr) ,liver function indexes ,including alanine amin-otransferase (ALT) ,aspartate aminotransferase (AST) between two groups before treatment (P>0.05) ;af-ter treatment ,the TG ,TC ,LDL-C ,CRP ,IL-6 ,TNF-α ,BUN ,Scr ,ALT and AST levels of the patients in the treatment group were significantly lower than those in the control group ;the HDL-C level of the patients in the treatment group was significantly higher than that in the control group ;the total effective rate of the treat-ment group was significantly higher than that in the control group ;the incidence of adverse reactions was sig-nificantly lower than that of the control group ,and the differences were statistically significant (P>0.05). Conclusion The lipid-lowering effect of treatment of hemodialysis patients with traditional Chinese medicine lipid-lowering treatment is remarkable ,and worthy of promotion.

ABSTRACT

Objective To evaluate the effects of laparoscopic filling with pedicled greater retina for hepatic cysts. Methods Clinical data were reviewed on 36 patients with hepatic cysts undergoing laparoscopic filling with pedicled greater retina from January 2010 to May 2014.The diameters of cysts ranged from 10 to 26 cm, with a diameter smaller than 20 cm in 29 patients and bigger than 20 cm in 7 patients.Laparoscopic fenestration of hepatic cysts was performed to destroy cyst cells with iodine and absolute alcohol. Pedicled greater retina was used for filling the cavity of cysts. Results The procedures were completed smoothly.The operative time was 45-115 min, with an average of (80.4 ±13.1) min.No abdominal cavity infection happened.Primary healing of abdominal incision was achieved in all the patients.The postoperative hospital stay was 5-10 d, with an average of (6.4 ±1.2) d.All the 36 patients were followed up for 6-12 months with a mean of (9.3 ±1.5) months.None of the patients showed recurrence under liver B-ultrasonography or CT scanning. Conclusion Laparoscopic filling with pedicled greater retina for hepatic cysts has advantages of minimal invasion, quick postoperative recovery, short hospital stay, and good results.

ABSTRACT

Objective To observe the effect of triple therapy to the levels of serum interleukin 10(IL)-10 ,transforming growth factor beta 1(TGF-β1) and IL-17 in eldly patients with helicobater pylori associated gastritis .Methods 90 eldly patients with heli-cobater pylori associated gastritis were randomly divided into observation group and control group ,45 cases in each group ,the ob-servation group was given standard triple therapy ,the control group was separately given omeprazole treatment ,after 14 days , ELISA method was used to detect the levels of serum IL-10、TGF-β1 and IL-17 .Results Compared with before treatment ,the lev-els of IL-10、TGF-β1 and IL-17 in of the two groups had significantly reduced after treatment (P<0 .05);compared with the control group after treatment ,the levels of IL-10、TGF-β1 and IL-17 in observation group after treatment had significantly reduced (P<0 .05) .Compared with helicobater pylori negative and helicobater pylori positive patients of the experimental group after treatment , the levels of IL-10、TGF-β1 and IL-17 had significantly reduced(P<0 .05) .The levels of IL-10、TGF-β1 and IL-17 in serum of pa-tients of two groups before treatment showed significantly positive correlation (P<0 .05) .Conclusion The triple therapy could sig-nificant decrease the levels of serum IL-10、TGF-β1 and IL-17 ,it has good treatment effect .

ABSTRACT

Objective To investigate the effects of different surface treatments on shear bond strength between diatomite-based dental ceramic and IPS e.max veneering ceramic.Methods Diatomite-based dental ceramic was randomly divided into acid-etched group,sandblasting group and untreated group,and then sintered with IPS e.max veneering ceramic,respectively.Shear bond strength were detected,and the interface microstructures and elemental distribution were observed by scanning electron microscope-energy dispersive spectrometer (SEM-EDS).Results Shear bond strength of acid-etched group was significantly higher than that of untreated group (P<0.01).SEM observation results showed that the interface of acid-etched group bonded closely.There was no statistical difference of the shear bond strength between the sandblasting group and untreated group (P>0.05).Conclusions Acid-etched treatment can significantly enhance the bond strength of diatomite-based dental ceramic and IPS e.max veneering ceramic.

ABSTRACT

Objective To evaluate the association between stress hyperglycemia and clinical out-comes among patients with and without diabetes mellitns admitted to medical intensive care unit (MICU).Methods We reviewed the medical ICU records of 1016 consecutive patients admitted to Wu Han Univer-sity Ren Min Hospital. According to admission glucose level, 126 patients with debates and 890 patients without known debates were divided into group 1 ( <6 mmol/L), group 2(6 ~ 8mmol/L), group 3(8 ~10mmol/L), and group 4( > 10mmol/L). The primary end point of the study was in-hospital mortality. Secondary end points included length of hospital stay, length of ICU stay, the ratio of mechanical ventila-tion, blood transfusions, antibiotics and blood glucose treatment. Results The hospital mortality among patients without a history of diabetes mellitus was 4. 7%, 12. 1%, 20. 5%, 30. 3% in group 1, 2, 3, 4,respectively. The ratio of mechanical ventilation, blood transfusions, and antibiotics increased progressively as glucose values increased. Conclusions Hyperglycemia was a common disease, and it may be an impor-tant marker of poor clinical outcome and mortality in patients without history of diabetes.

ABSTRACT

Objective To make a rapid determination of ferulic acid and identification of its structure in rhizome of Phragmites communis (RPC). Methods The chemical constituents were extracted by solvent and the relative content and structure of ferulic acid in RPC were determined and identified using GC-TOFMS technique. Results By GC-TOFMS, the ferulic acid was successfully isolated from the extracts of RPC in a relative content of 5%. According to the exact masses and corresponding elemental compositions of molecular ion and eight characteristic ions, a possible fragmentation pathway of the ferulic acid molecule was proposed, by which the molecular structure was confirmed. Conclusion GC-TOFMS technique is a rapid, accurate, sensitive, and reliable method for determination of the relative content of ferulic acid in RPC and identificatiion of its structure.
