ABSTRACT
Objective@#To explore the role of D-dimer level in patients with acute exacerbation of chronic obstructive disease (AECOPD) in predicting the re-admission of patients.@*Methods@#One hundred and twenty chronic obstructive pulmonary disease (COPD) patients in the Shanghai General Hospital of Shanghai Jiao Tong University form January 2016 to December 2018 were divided into AECOPD group (62 cases) and stable COPD group (58 cases).The level of serum D-dimer was analyzed and Pearson correlation analysis was performed with the patient′s blood gas analysis and COPD assessment test (CAT) score. The area under the receiver operating characteristic (ROC) curve was used to evaluate the predictive value of serum D-dimer level for readmission.@*Results@#Serum D-dimer level was significantly higher in AECOPD group than that in stable group: (1.24 ± 0.56) mg/L vs. (0.39 ± 0.22) mg/L, and there was statistical difference (P<0.01). Serum D-dimer level was negatively correlated with PaO2 in COPD patients (r = 0.712, P = 0.000), but positively correlated with PaCO2 (r = 0.683, P = 0.000) and CAT score (r = 0.652, P = 0.000). The area under the ROC curve of D-dimer level to re-admission of COPD patients within one year was 0.848. The cutoff value was 1.015 mg/L.@*Conclusions@#The level of D-dimer in COPD patients is of great value in predicting the re-admission of patients, which can be used as an independent indicator of disease progression.

<p><b>OBJECTIVE</b>To compare the therapeutic effects on dysphagia in post-stroke pseudobulbar palsy between the combined treatment with acupuncture and rehabilitation training and the combined treatment with low-frequency electrical therapy and rehabilitation training and to explore its more effective therapy, using the ultrasonographic examination.</p><p><b>METHODS</b>A total of 60 patients of dysphagia in post-stroke pseudobulbar palsy were randomly assigned into an observation group and a control group, 30 cases in each one. The rehabilitation treatment was provided in the two groups. Additionally, in the observation group, acupuncture was used at the three-tongue points [Shanglianquan (Extra), located in the depression between the lingual bone and the border of lower jaw, 1 inferior to the midline of the jaw; and the other two points, located at 0.8 bilateral to Shanglianquan (Extra)], bilateral Fengchi (GB 20) and Fengfu (GV 16). After arrival of , the last two tongue-points were stimulated with electricity, with disperse-dense wave, periodically at 2 Hz, 10 Hz and 100 Hz, at the interval of 3 s, lasting for 30 min. Bilateral Fengchi (GB 20) was stimulated with electricity, with disperse-dense wave and the same frequency as above, lasting for 30 min. In the control group, the low-frequency electrical stimulation was adopted. The electrodes were placed on the bilateral sides of the midline of the throat, 30 min each time. The treatment was for 1 month in the two groups, once a day. The scores of the standardized swallowing assessment (SSA) and the videofluoroscopy swallowing study (VFSS) were observed and the reducing rate of the hyoid-thyroid cartilage distance was determined with ultrasound quantitative measure.</p><p><b>RESULTS</b>After treatment, SSA scores of the two groups were all lower than those before treatment and VFSS scores were higher than those before treatment (all <0.05). The above scores in the observation group were better than those in the control group (both <0.05). The reducing rates of the hyoid-thyroid cartilage distance were higher than those before treatment in the two groups (both <0.05). The result in the observation group was higher than that in the control group (<0.05).</p><p><b>CONCLUSION</b>The treatments with acupuncture and rehabilitation training or with the low-frequency electrical therapy and rehabilitation obtain the effects on dysphagia in post-stroke pseudobulbar palsy in the patients. The effects of the combined treatment with acupuncture and rehabilitation training are better. The ultrasonographic technology effectively and quantitatively analyzes the changes in the glossopharyngeal complex in the patients of dysphagia.</p>

Objective To evaluate the effect of repetitive transcranial magnetic stimulation (rTMS) on unilateral spatial neglect in stroke patients. Methods Published articles from the earliest date available to July, 2016 were recalled from PubMed, OVID, Embase, Co-chrane Library, CNKI, VIP and Wanfang Database. Two reviewers selected independently the randomized controlled trials (RCTs) about rT-MS for stroke patients with unilateral spatial neglect, and extracted data independently and analyzed with RevMan 5.3 and GRADE profiler 3.6. Results Twelve trials that represented 353 participants were evaluated. For improving Line Bisection Test, Star Cancellation Test, Albert Test, rTMS groups were significantly more effective than the controls. rTMS groups with different frequencies all showed significantly effec-tive:for low frequency rTMS, SMD=-1.21, 95%CI=-2.17 to-0.25;for high frequency rTMS, SMD=-2.56, 95%CI=-3.54 to-1.58;for continuous theta burst stimulation (cTBS), SMD=-2.51, 95%CI=-3.66 to-1.36. Conclusion rTMS is effective on unilateral spatial neglec-tin in stroke patients.
ABSTRACT
Objective To explore the difference in short-term efficacy between peroral endoscopic myotomy (POEM) and pneumatic dilatation (PD) in achalasia patients.Methods A retrospective analysis was applied.From September 2010 to March 2015,patients with POEM or PD were enrolled and divided into POEM group (n=26) and PD group (n=40).High-resolution manometry (HRM) before and one month after treatment were compared between POEM group and PD group.Before and three months after treatment,Eckardt score and gastroesophageal reflux symptom was compared between groups (Eckardt score ≤ 3 as the standard for successful treatment).Wilcoxon signed rank test was performed for before and after treatment comparison in the same group.Mann-Whitney U test was used to compare between two groups and Fisher's exact test was used for rate comparison.Results The successful treatment rates at one and three month after POEM group were 92.3 % (24/26) and 96.2% (25/26),respectively.The successful treatment rates at one and three month after PD were 87.5% (35/40) and 75.0% (30/40).At three month after treatment,the successful treatment rate of POEM group was higher than that of PD group (Fisher's exact test,P =0.02).At three months after treatment,the Eckardt score of POEM group was lower than that of PD group (1.35,0 to 4.00,vs2.73,0 to 6.00;U=-3.921,P>0.01).By the end of three months after treatment,the rate of gastroesophageal reflux symptom of POEM group was higher than that of PD group (7/26,26.9 % vs 2/40,5.0%;Fisher's exact test,P=0.01).The postoperative 4 second integrated relaxation pressure (4s-IRP) and lower esophageal sphincter pressure (LESP) of POEM group were both lower than those of PD group (7.01 mmHg,3.48 to 10.40 mmHg vs 10.11 mmHg,5.75 to 12.91 mmHg,U=-4.541,P<0.01;11.61 mmHg,4.21 to 14.64 mmHg vs 17.85 mmHg,8.39 to 24.57 mmHg,U=-6.142,P<0.01).The analysis of achalasia subtypes indicated that the efficacy of POEM was better than that of PD both in type Ⅰ and type Ⅱ.Conclusion During short-term follow-up,the efficacy of POEM was better than that of PD in achalasia patients,however there was a higher incidence of post-operative gastroesophageal reflux after POEM.

Objective To investigate expression characteristics in regenerating hair follicles induced by Wnt10b, and to explore mechanisms underlying Wnt10b-induced regeneration of hair follicles. Methods Both adenovirus containing the Wnt10b gene(AdWnt10b)and that containing the green fluorescent protein-encoding gene(AdGFP)were amplified in HEK-293 cells and purified by caesium chloride density gradient centrifugation. A total of 36 C57BL/6J mice were randomly and equally divided into the AdWnt10b group and AdGFP group to be intracutaneously injected with AdWnt10b and AdGFP on the back respectively. Three mice were sacrificed on day 2.5, 5, 7, 9, 14 and 28 after the injection separately, and skin samples were resected from the injected sites subsequently. Hematoxylin and eosin(HE) staining and immunohistochemical staining were performed to observe hair follicle structure, analyze expression characteristics of the Wnt signaling pathway, and to estimate proliferative activity of regenerating hair follicles induced by Wnt10b. Results As HE staining showed, new hair follicles appeared as early as day 5 after the injection, then grew normally, and entered into catagen phase on day 28 in the AdWnt10b group. Immunohistochemical staining showed that AE15 expression was observed in new hair follicles as early as day 5 after the injection in the AdWnt10b group, then increased along with the growth of hair follicles, but decreased on day 28. On day 5 after AdWnt10b injection, both β-catenin and Lef1 expressions were seen in the cell nucleus. Lef1 was expressed specifically in hair germs and hair matrix, and its expression began to decrease on day 28. In addition, Ki67 expression was observed in the epidermis and outer root sheath of hair follicles as early as day 2.5 after the injection, in the bulge region of hair follicles on day 2.5, 7, 9 and 14, and in hair matrix cells as early as day 7. Conclusion Wnt10b could induce regeneration of hair follicles with normal structure, likely by activating the canonical Wnt signaling pathway in hair follicle stem cells and their daughter cells.

@#Objective To investigate the effects of electroacupuncture (EA) on expression of phosphatase and tensin homology deleted on chromosome ten (PTEN) protein and growth associated protein-43 (GAP-43) in rats with Barrel cortex focal ischemia. Methods 24 adult male Sprague-Dawley rats were divided into sham group (n=8), model group (n=8) and EA group (n=8). The EA group accepted EA at Baihui (DU20) and Fengfu (DU16) acupoints 3 days after modeling, 30 min a time, once a day. They were assessed with Corner Test 1 day before modeling, and 3 days, 7 days and 14 days after medication. The expression of GAP-43 and PTEN around infarct zone were detect with Western blotting 14 days after medication. Results The frequence of turn-right decreased in the EA group compared with that in the model group (P<0.001) 14 days after modeling. The expression of GAP-43 increased in the model group compared with that in the sham group (P<0.05), and increased more in the EA group than in the model group (P<0.05). There was no significantly difference in PTEN expression between the model group and the sham group (P=0.460), and significantly decreased in the EA group (P<0.001). Conclusion EA may inhibit expression of PTEN protein and increase expression of GAP-43, which may be involved in nerve regeneration and functional recovery.

@# Objective To explore the effects of a single bout of aerobic exercise on oxidative stress and total peripheral vascular resistance. Methods 36 prehypertensive patients were divided into aerobic exercise group (n=18) and control group (n=18). The aerobic exercise group performed cycle ergometry at 70% maximal oxygen consumption for 30 min. Total peripheral vascular resistance (TPVR), malondialdehyde (MDA), mean arterial pressure (MAP), vitamin C (VC) and vitamin E (VE) were measured before, immediately, 1 hour and 2 hours after the aerobic exercise. Results There was no significant difference in TPVR, MDA, MAP, VC and VE between two groups before exercise (P>0.05). TPVR was all significantly lower in the aerobic exercise group than in the control group immediately, 1 hour and 2 hours after exercise (P<0.001), and showed a rising trend of recovery. MDA was significantly higher in the aerobic exercise group than in the control group 1 hour and 2 hours after exercise (P<0.05), however there was no significant difference in MDA between two groups immediately after exercise (P>0.05). MAP was significantly lower immediately after exercise in the aerobic exercise group than in the control group (P< 0.001), but there was no significant 1 hour and 2 hours after exercise (P>0.05). There was no significant difference in VC and VE between two groups immediately, 1 hour and 2 hours after exercise (P>0.05). Conclusion The level of oxidative stress increases after a single bout of aerobic exercise, and the TPVR decreases.

Objective To explore the mechanisms of mesenchymal stem cells (MSC) in the treatment of concanavalin A (ConA)-induced acute immune liver injury in mice.Methods MSC were isolated and cultured from bone of the four limbs of three-week-old C57BL/6 mice.The specific surface markers were identified and osteogenic,adipogenic differentiation ability were tested.A total of 15 six to seven-week old C57BL/6 mice were divided into control group,MSC treatment group and phosphate buffer saline (PBS) treatment group,five mice in each group.The mice of MSC treatment group was injected through tail firstly with ConA and then MSC,PBS treatment group was injected through tail firstly with ConA and then PBS,control group was injected through tail with PBS twice.The mice were sacrificed in 14 to 16 hours after injection.The level of alanine aminotransferase (ALT),aspartate transaminase (AST) in peripheral blood were detected and the pathological change in liver tissue was scored by Knodell score system.Activation rate of splenic CD4+ T cells and the proportion changes of T hepler cell (Th)1,Th2,Th17 and regulatory T cells (Treg) were detected by flow cytometry and the ratio of Th17/Treg was calculated.The levels of tumor necrosis factor α (TNF-α),interferon (IFN)-γ and interleukin (IL)-4 in peripheral blood were detected by enzyme linked immunosorbent assay (ELISA).Independent-sample t test was used for comparison between groups of measurement data.Results ALT,AST and Knodell score of MSC treatment group was (174.2± 46.9) U/L,(185.6± 71.6) U/L and 3.4±1.3,respectively,which were better than those of PBS treatment group ((647.0± 118.0) U/L,(749.0± 104.0) U/L and 5.2 ±0.8,respectively),and the differences were statically significant (t =8.33,9.98 and 2.55,all P<0.05).The activation rate of splenic CD4+ T cell of PBS treatment group was (26.10±2.17) %,the proportion of Th1 and Th2 in CD4+ T cell was (5.81±0.79) % and (5.98± 1.22)%,the ratio of Th17/Treg was 0.29±0.03,the levels of TNF-α,IFN-γ and IL4 in peripheral blood were (1 281.95±88.61) U/L,(1 838.66±196.91) U/L and (1 192.36±163.94) U/L,which were higher than those of control group ((13.74±1.59)%,(1.35±0.17)%,(2.13±0.17)%,0.15± 0.05,(21.71±2.50) U/L,(11.84±1.28) U/L and (24.46±3.96) U/L),and the differences were statistically significant (t=10.26,12.37,7.02,5.30,31.79,15.93 and 20.75,all P<0.01).There was no statistically significant difference in the activation rate of splenic CD4+T cell between MSC treatment group and PBS treatment group ((26.20±3.09)% vs (26.10±2.17)%,P>0.05).However,in MSC treatment group,the proportion of Th1 and Th2 in CD4+ T cell ((1.83±0.52) % and (2.75±1.06%)),the ratio of Th17/Treg (0.18±0.02) and the levels of TNF-α,IFN-γ and IL-4 in peripheral blood ((760.71± 73.19) U/L,(742.49±76.46) U/L and (825.76±101.74) U/L) significantly decreased compared with those of PBS treatment group,and the differences were statistically significant (t=9.45,4.48,6.41,10.14,5.56 and 10.22,all P<0.01).There was no significant difference in the ratio of Th17/Treg between MSC treatment group and control group (P>0.05).Conclusions The therapeutic effects of MSC on ConA induced acute immune liver injury were through influence splenic CD4+ T cell subsets by decreasing the proportion of Th1 and Th2 and then declining the levels of secreted cytokines such as TNF,IFN-γ and IL-4 in peripheral blood,increasing the proportion of Treg and decreasing the proportion of Th17 and keeping the balance of Th17/Treg.

Objective To investigate the effects of peroral endoscopic myotomy (POEM) on esophageal dynamics in patients with achalasia (AC) and appraise the role of high resolution manometry (HRM)in assessment of POEM therapy.Methods From July 2011 to September 2012,20 patients with achalasia underwent POEM in the department of gastroenterology,Tianjin Medical University General Hospital.Preoperation esophageal dynamics of all the patients were evaluated by high resolution manometry (HRM) system and one month after POEM operation the test was repeated.Data of lower esophageal sphincter (LES)and esophageal body were analyzed,with 15 healthy volunteers as a contrast study.Results Mter POEM,the LES pressure (LESP) and 4-second integrated relaxation pressure (4sIRP) significantly decreased:LESP [pre-operation (24.5 ± 13.1) mm Hg vs.post-operation (8.5 ± 3.1) mm Hg,P < 0.05] ; 4sIRP [pre-operation (20.7 ± 6.8) mm Hg vs.post-operation (5.0 ± 3.4) mm Hg,P < 0.05].The LES relax rate (LESRR) significantly elevated [pre-operation (12.7 ± 9.8) % vs.post-operation (39.6 ± 18.1) %,P < 0.05].However,the esophageal aperistalsis remained after POEM.The total of 20 patiems were all type Ⅰ achalasia and responded well to POEM therapy.Post-operation data on the symptom scores markedly decreased and a significant correlation was found between the decreasing level of symptom scores and LESP,as well as the scores 4sIRP in the 20 patients (LESP:r =0.751,P < 0.05 ; 4sIRP:r =0.500,P < 0.05).Conclusion POEM can significantly improve esophageal dynamics in patients with achalasia and the treatment outcome is definite.HRM plays an important role in evaluation of POEM therapy on achalasia.

ObjectiveTo explore the early diagnosis and effective treatment of redundant colon,and to reduce the misdiagnosis and shorten the medical treatment time before the diagnosis.MethodsClinical data of twentyseven patients with redundant colon from February 2005 to December 2011 were retrospectively analyzed in General Surgery Department of 117th and 322th People's Liberation Army Hospital.ResultsThe clinical symptoms of 27 patients nainly as early recurrent intractable constipation,bloating,abdominal pain,weight loss and other symptoms,were likely to be in a misdiagnosis.In addition to three patients with redundant sigmoid colon concurrent reverse came to hospital emergency with surgery,twenty-four cases' symptoms persisted and came to many hospitals with medical treatment up to 32 years,diagnosed by the out-patient barium enema.After surgical resection disease bowel,7-11 months follow-up,patients abdominal distension,abdominal pain,constipation,weight loss and other systemic unwell symptoms disappeared.ConclusionThis disease is rare,we must raise the medical staff's awareness of this disease.X-ray examination with barium enema is the best way to diagnose this disease.After diagnosis,surgery is the most effective treatment.

<p><b>OBJECTIVE</b>To improve the detection sensitivity of coagulative necrosis caused by high intensity focused ultrasound (HIFU) exposure.</p><p><b>METHODS</b>Sixty rabbits bearing VX2 breast neoplasms at the second bilateral breast were divided randomly into 3 groups (n=20) for targeted HIFU exposure at 90, 120, or 150 W for 3 s. With the right breast neoplasms as the control, the left ones were irradiated following nanoscale bubble injection. The ultrasonograms generated before and immediately after the exposure were analyzed for changes in the gray scale values and for computation of the related functions in the target area. The accuracy and sensitivity of gray evaluation and analysis of the related functions were compared in the detection of coagulative necrosis.</p><p><b>RESULTS</b>Analysis of the related functions of the 120 samples showed an significantly higher accuracy (83.3%) than gray scale-based evaluation (68.3%) in detecting the coagulative necrosis (P<0.05). Under the same irradiation conditions, the mean values of the related function was significantly lower in microbubble group than in the control group (P<0.05).</p><p><b>CONCLUSION</b>Analysis of the related functions of the ultrasonographic data can be used to evaluate the coagulative necrosis in the target exposure area following HIFU with a greater sensitivity and accuracy than gray-scale evaluation.</p>

Objective To investigate the risk factors for osteoporosis (OP) in aged people at Beicai Town in Shanghai, to provide proofs for preventing OP. Methods Questionnaire survey was carried out on bone density in 608 aged people (aged from 60 to 79 years) who were randomly chosen from 4 resident committees of Beicai Town in Shanghai. Everyone underwent bone mineral density (BMD) assay of lumbar spine and total hip by DEXA machine. Results The rate of OP was 23 % in male group, and 61% in female group. It was 40% in 60-64 years old group, 44% in 65-69 years old group, 53% in 70-74 years old group and 66% in 75-79 years old group. The rate of OP was 52% in manual work group, and 42 % in non-manual work group. In males, it was 54 % in current weight less than 60 kg group, 19% in 60-70 kg group, 15% in 70-80 kg group and 23% in over 80 kg group. In females, it was 76% in current weight less than 50 kg group, 67% in 50-60 kg group, 63% in 60-70kg group, 30% in over 70 kg group. It was 56% in low body weight group, 41% in normal weight group and 58% in over weight group in their 25-year-old. It was 61% in normal body weight group, 43% in overweight group. It was 41% in non-fracture history group, 67% in once fracture group,74% in fracture history more than twice group. It was 60% in mother humpback history group, and 47% in no history of mother humpback group. Logistic regression analysis showed that gender, age,history of fracture, mother's humpbacked history, disease of internal secretion and metabolism system were the risk factors for OP. Conclusions The occurrence of OP is related with many factors in aged people at Beicai Town in Shanghai, especially for the aged women, who suffer from diseases of internal secretion and metabolism, and has the history of fracture and mother's humpbacked, the attention should be payed to the prevention and treatment of OP.

Senile dementia is a common degenerative disease of center nervous system in old age.The pathogeny and pathomechanism haven't been made clear.And the precise therapy is lack.It is thought that the Heart and Spleen has physiological and pathological relevance with brain by analyzing the relation between the Heart and Spleen and senile dementia in the article.Using method of benefiting the Heart and Spleen in treating senile dementia not only improves the somatic symptoms,also delays the degeneration of intelligence of the patients.The point puts forward the new thinking and method to cure senile dementia.
ABSTRACT
BACKGROUND: What deserves exploration is the relationship between menstrual cycle phase and athletic ability of female athletes, especially the mechanism of athletic performance influenced by different menstrual cycle phases.OBJECTIVE: To compare the hormone level of different menstrual cycle phases and the changes of athletic performance as well as its effect on athletic performance.DESIGN: Single sample and single factor analysis.SETTING: Zhejiang Insftute of Sport Science; College of Education,Zhejiang University; Department of Obstetrics and Gynecology, Zhejiang Provincial People's Hospital.PARTICIPANTS: Specially designed menses card were distributed to all the female athletes in the gig team and track & field team in Zhejiang Province in 2000. Consecutive 3-month recording was made to investigate the status of menstrual cycle phases of female athletes. Twenty-five athletes with regular menses were chosen as subjects, among whom 12 were specialized in gig racing with the average age of 19 years and 13 were specialized in track & field events with the average age of 18 years. They all had more than 3-year training experience, 6 days a week and 4-6 hours a day. They were healthy, had regular menstrual cycle, and had not taken any drug which influences the menstrual cycle.METHODS: Blood samples of the testees were collected at hour 8-9 AM at follicular phase (days 6-10) and luteal phase (days 19-23). Full-automatic chemiluminescence immunoassay equipment was used to measure the level of estradiol (E2), follicle-stimulating hormone (FSH), luteinizing hormone (LH), progesterone, and testosterone in serum. coefficient of variability in batch of reagent was <5%. Testees accomplished test of their athletic performance and the test of lactic acid 1 to 2 days after blood sample collection. Gig athletes performed 2000 m and 500 m full-strength pull with Concett Ⅱ boating ergometer while track & field athletes performed 100 m and 200 m full-speed running; the time needed was recorded. Gig athletes conducted 2000 m movements on the ergometer with the frequency of 26 times per minute. Track & field athletes carried out three times of 300-meter running with 90% of intensity with 3-minute intervals. Immediately after movements, the lactic acid value was measured following blood collection.Paired t test of the small sample study was used for significance detection.MAIN OUTCOME MEASURES: The level of E2, FSH, LH, proges-terone and blood lactic acid in serum; 2000 m and 500 m performance of gig athletes as well as 100 m and 200 m performance of track & field athletes were measured by the ergometer.RESULTS: Twenty-five athletes accomplished detection of all the indexes and entered the result analysis. The level of FSL, progesterone and testosterone of gig athletes was higher at luteal phase than at follicular phase;however, E2 and LH level did not significantly differ. The level of progesterone and testosterone of track & field athletes was higher at luteal phase than at follicular phase; however, E2, FSH and LH level did not significantly differ. 500 m performance of gig athletes detected by the ergometer, and 100 m and 200 m full-speed running performance of track & field athletes at luteal phase were superior to those at follicular phase, but no significant difference was found in 2000 m performance of gig athletes. When the same loading was accomplished at follicular phase and luteal phase, the lactic acid value at luteal phase was significantly lower than that at follicular phase.CONCLUSION: Tested athletes at luteal phase have good functional status, which may be related to the changes of menstrual cycle hormone level and synergistic effect of various hormones under thes state of athletic movements. It suggests that sufficient attention should be paid to the menstrual cycle of female athletes.
