ABSTRACT
ObjectiveTo observe the effect of polysaccharides from root, stem, leaf and fruit of Schisandra chinensis on exercise endurance in the aging mice induced by D-galactose. MethodMale ICR mice were randomly assigned into six groups: blank control group, model group, root polysaccharide group, stem polysaccharide group, leaf polysaccharide group and fruit polysaccharide group. The mice were administrated with distilled water or root, stem, leaf and fruit polysaccharide (total sugar content of 35 mg·kg-1) by gavage. Thirty minutes after the administration, the blank control group was subcutaneously injected with normal saline, and the other groups with D-galactose (300 mg·kg-1), once daily for 6 weeks. The anti-fatigue effects were evaluated by rotarod test, forelimb grip strength test, and weight-loaded swimming test. The fatigue and oxidation indicators such as blood urea nitrogen (BUN), serum lactic acid (LD), lactic dehydrogenase (LDH), creatine kinase (CK), superoxide dismutase (SOD), malondialdehyde (MDA), glutathione peroxidase (GSH-Px), and reactive oxygen species (ROS) were measured by chemical colorimetry. The protein levels of pro-apoptotic protein B cell lymphoma-2 (Bcl-2)-associated X protein (Bax), anti-apoptotic Bcl-2 and cleaved cysteinyl aspartate-specific protease-3 (cleaved Caspase-3) in mouse skeletal muscle were detected by Western blot. ResultIn the rotarod test, the time on rod was shorter in the model group than in the blank control group (P<0.01) and the root, stem and fruit polysaccharide groups (P<0.01). In the forelimb grip strength test, the forelimb grip strength in the model group was lower than that in the blank control group (P<0.01) and the root, stem, leaf and fruit polysaccharide groups (P<0.01). In the weight-loaded swimming test, the weight-loaded swimming time in the model group was shorter than that in the blank control group (P<0.01) and the root, stem, leaf and fruit polysaccharide groups (P<0.01). Compared with those in the blank control group, the BUN, LD, LDH and CK levels significantly increased in the model group (P<0.05, P<0.01). The increases in BUN and LDH levels were decreased by the root, stem and fruit polysaccharides (P<0.05, P<0.01) and those in LD and CK by the root, stem, leaf and fruit polysaccharides (P<0.05, P<0.01). Compared with the blank control group, the model group showed decreased SOD and GSH-Px activities (P<0.01) and increased MDA and ROS content (P<0.01). Compared with the model group, the root, stem, and fruit polysaccharide increased the SOD activity (P<0.05, P<0.01) and decreased ROS content (P<0.01). The root and stem polysaccharides decreased the MDA content (P<0.01) and increased the GSH-Px activity (P<0.05, P<0.01). Compared with the blank control group, the model group showed up-regulated protein levels of Bax and cleaved Caspase-3 and down-regulated protein level of Bcl-2 (P<0.01). Compared with the model group, the root, and stem polysaccharides down-regulated the protein levels of cleaved Caspase-3 (P<0.05) and up-regulated protein level of Bcl-2 (P<0.01). ConclusionThe polysaccharides from the root, stem, leaf, and fruit of S. chinensis have anti-fatigue effect in D-galactose-induced aging mice. The polysaccharides may exert such effect by improving the antioxidant capacity and inhibiting the apoptosis of skeletal muscle cells.

Impaired immunomodulatory capacity and oxidative stress are the key factors limiting the effectiveness of mesenchymal stem cell transplantation therapy. The present study was aimed to investigate the effects of jujuboside A (JuA) on the protective effect and immunomodulatory capacity of human umbilical cord mesenchymal stem cells (hUC-MSCs). Hydrogen peroxide was used to establish an oxidative damage model of hUC-MSCs, while PBMCs isolated from rats were used to evaluate the effect of JuA pre-treatment on the immunomodulatory capacity of hUC-MSCs. Furthermore, Hoechst 33258 staining, lactate dehydrogenase test, measurement of malondialdehyde, Western blot, high-performance liquid chromatography; and flow cytometry were performed. Our results indicated that JuA (25 μmol·L-1) promoted the proliferation of hUC-MSCs, but did not affect the differentiating capability of these cells. JuA pre-treatment inhibited apoptosis, prevented oxidative damage, and up-regulated the protein expression of nuclear factor-erythroid factor 2-related factor 2 and heme oxygenase 1 in hUC-MSCs in which oxidative stress was induced with H2O2. In addition, JuA pre-treatment enhanced the inhibitory effect of hUC-MSCs against abnormally activated PBMCs, which was related to stimulation of the expression and activity of indoleamine 2,3-dioxygenase. In conclusion, our results demonstrate that JuA pre-treatment can enhance the survival and immunomodulatory ability through pathways related to oxidative stress, providing a new option for the improvement of hUC-MSCs in the clinical setting.

Aging-induced changes in the immune system are associated with a higher incidence of infection and vaccination failure. Lymph nodes, which filter the lymph to identify and fight infections, play a central role in this process. However, careful characterization of the impact of aging on lymph nodes and associated autoimmune diseases is lacking. We combined single-cell RNA sequencing (scRNA-seq) with flow cytometry to delineate the immune cell atlas of cervical draining lymph nodes (CDLNs) of both young and old mice with or without experimental autoimmune uveitis (EAU). We found extensive and complicated changes in the cellular constituents of CDLNs during aging. When confronted with autoimmune challenges, old mice developed milder EAU compared to young mice. Within this EAU process, we highlighted that the pathogenicity of T helper 17 cells (Th17) was dampened, as shown by reduced GM-CSF secretion in old mice. The mitigated secretion of GM-CSF contributed to alleviation of IL-23 secretion by antigen-presenting cells (APCs) and may, in turn, weaken APCs' effects on facilitating the pathogenicity of Th17 cells. Meanwhile, our study further unveiled that aging downregulated GM-CSF secretion through reducing both the transcript and protein levels of IL-23R in Th17 cells from CDLNs. Overall, aging altered immune cell responses, especially through toning down Th17 cells, counteracting EAU challenge in old mice.

Objective:To explore the postoperative biliary leakage and severe complication rate and its related risk factors of hepaticojejunostomy (HJ) for biliary disease.Methods:The clinical data of patients undergoing HJ for benign biliary diseases at the First Affiliated Hospital of Anhui Medical University from Jan 2003 to Dec 2017 were retrospectively analyzed. Multi-factor analysis was used to find a risk factor for postoperative bile leakage and severe complications.Results:Two hundred and eighty-three patients received HJ. The median age was 39 years.The short-term complication rate after surgery was 19.1% ( n=54), and the biliary leakage rate was 11.7% ( n=33), and the severe complication rate was 15.2% ( n=43). By multi-factor analysis: preoperative sepsis[ OR=3.875, 95% CI (1.583, 9.485), P=0.003], liver cirrhosis [ OR=3.212, 95% CI (1.001, 10.307), P=0.050], intraoperative blood loss≥400 ml [ OR=6.054, 95% CI (1.231, 29.781), P=0.027],postoperative hospitalization ≥9 days [ OR=6.738, 95% CI (2.287, 19.855), P=0.001] are the independent risk factors for postoperative bile leakage. Main bile duct stone[ OR=2.764, 95% CI (1.174, 6.510), P=0.020], preoperative sepsis [ OR=4.310, 95% CI (1.666, 11.149), P=0.003], intraoperative bleeding ≥400 ml [ OR=5.944, 95% CI (1.231, 29.781), P=0.022] and postoperative hospitalization ≥ 9 days [ OR=11.422, 95% CI (1.317, 49.859), P=0.001] are the independent risk factors for serious complications. Conclusions:HJ should be conducted when the sepsis was under control and sufficient bile drainage.The patients' condition should be comprehensively and accurately assessed before operation. The indications for partial hepatectomy need to be strictly defined.

Objective:To study on performance comparison between clinic digital memory assessment and previously used assessments in dementia risk screening.To compare the performance comparison between Beijing Aging Brain Rejuvenation Initiative(BABRI)Brain Health System's Clinic Digital Memory Detection as study and Alzheimer Disease-8(AD8)and the Brief Community Screening Instrument for Dementia(BCSID)as controls, We calculated and evaluated the accuracy, sensitivity and specificity of screening Mild cognitive impairment(MCI)among these tests.Furthermore, BABRI Brain Health System was used to conduct a large sample brain health examination and early dementia screening to test the validity, adaptability and stability of the evaluation results by BABRI Brain Health System'Clinic Digital Memory Detection.Methods:Dataset 1 contained 669 elderly subjects from five communities in Beijing were recruited according to inclusion and exclusion standard.The diagnosis of MCI was based on the full set of neuropsychological scale and Petersen standard.Dataset 1 was used to compare the discriminant effect of BABRI Brain Health System'Clinic Digital Memory Detection as study versus AD8 and BCSID as controls.The sensitivity, specificity, positive predictive value, negative predictive value and Youden index of each measurement tool were calculated.Then, the receiver operator characteristic(ROC)curve was prepared to compare the discrimination ability of MCI between each measurement tool.While the area under the curve(AUC)of different tools was compared by Wald χ2 test.Dataset 2 contained 284 103 subjects from 16 communities in Beijing, which were used to test the applicability of large sample screening in BABRI Brain Health System. Results:77 patients with MCI were found among 666 people, and incidence rate was 11.56% using the full set of neuropsychological scales in dataset 1.Compared with the results of other tests, the sensitivity of BABRI Brain Health System to correctly distinguish MCI was 0.753, which was close to BCSID, and better than AD8.In addition, BABRI Brain Health System's Youden's index was 0.741 and AUC was 0.905, which suggested that the specificity, positive predictive value, negative predictive value and cognitive domain coverage of MCI screening were generally better in BABRI Brain Health System than in AD8 and BCSID.Finally, the Brain Health Examination results of 284, 000 people in dataset 2 showed that the high-risk detection rate of MCI(8.65%)of the tool for people over 50 years old under a large sample was quite close to the results of dataset 1(8.67%), indicating that the BABRI Brain Health System had high stability.Conclusions:BABRI Brain Health System has not only high sensitivity and specificity, but also wide cognitive field coverage and high stability.BABRI Brain Health System is suitable for large-scale brain health examination and dementia risk screening in grass-roots communities, and is worthy of popularization.

Objective:To evaluate the efficacy of the domestic D-Shant device for the treatment of patients with chronic heart failure (CHF) using echocardiography.Methods:Twenty-four CHF patients who were treated with domestic D-Shant device in Union Hospital, Tongji Medical College, Huazhong University of Science and Technology from September 2020 to December 2021 were enrolled in the study. Pulmonary capillary wedge pressure (PCWP)/ left atrial pressure (LAP), right atrial pressure (RAP), pulmonary artery pressure, interatrial septal gradient pressure, cardiac index and pulmonary/systemic blood flow ratio (Qp/Qs) were measured before and after implantation using right heart catheterization.Left atrial end-diastolic area index (LAEDAI), left atrial end-diastolic volume index (LAEDVI), left ventricular end-diastolic volume index (LVEDVI), left ventricular end-systolic volume index (LVESVI), left ventricular ejection fraction (LVEF), right atrial end-diastolic diameter, right ventricular end-diastolic diameter, tricuspid annular plane systolic excursion (TAPSE), right ventricular tractional area change (RVFAC), device shunt aperture, velocity and pressure, together with mitral and tricuspid regurgitation severity were measured using echocardiography before, and 1 month as well as 3 months after D-Shant device implantation. Clinical data were collected and analyzed including 6-minute walking test (6MWT), New York Heart Association (NYHA) classification and the Kansas City Cardiomyopathy Questionnaire (KCCQ). Spearman correlation analysis was used to determine the relation between the changes in PCWP/LAP as well as echocardiographic parameters before and 3 months after implantation and NYHA classification. Binary Logistic regression analysis was performed to determine the predictive factors of NYHA classification improvement at 3-month follow-up after D-Shant device implantation.Results:①D-Shant devices were successfully implanted in all patients. ②Compared with preoperative values, invasive PCWP/LAP systolic, diastolic and mean pressures, transatrial septal gradient, and pulmonary systolic, diastolic and mean pressures decreased significantly after implantation(all P<0.001); Qp/Qs increased significantly after implantation( P<0.001). ③Compared with preoperative values, TAPSE, RVFAC and pulmonary artery flow velocity increased at 1 month after implantation(all P<0.05), whereas a significant reduction in mitral regurgitation grade, and an increase in LVEF and pulmonary artery flow velocity at 3 months after implantation(all P<0.05). Right atrial end-diastolic diameter, right ventricular end-diastolic diameter, LAEDAI, LAEDVI, LVEDVI, LVESVI, ratio of early to late diastolic peak velocities of mitral inflow(E/A), systolic peak velocity of mitral annulus at septal site(S′), ratio of early diastolic peak velocity of mitral inflow to diastolic peak velocity of mitral annulus(E/e′), pulmonary artery diameter, inferior vena cava diameter and degree of tricuspid regurgitation did not change among before, and 1 month as well as 3 months after implantation. There were no significant changes in the device shunt aperture, velocity and pressure between 1 month and 3 months after implantation(all P>0.05). ④The significant improvements in NYHA classification, KCCQ scores and 6MWT were observed at 1 and 3 months after implantation compared with preoperative values (all P<0.01). ⑤NYHA classification at 3 months after implantation was correlated with LVEF pre-post, PCWP/LAP pre-post, TAPSE pre-post and RVFAC pre-post ( rs=0.738, -0.730, 0.738, 0.723; all P<0.001). Logistic regression analysis showed that LVEF pre-post was an independent predictor for NYHA classification improvement at 3 months after implantation ( OR=0.687, 95% CI=0.475-0.992, P=0.045) . Conclusions:Domestic D-Shant device can effectively improve the cardiac function and clinical symptoms in patients with CHF. Echocardiography is a feasible and effective method to evaluate the benefits of domestic D-Shant device for the treatment of CHF.

Objective:To analyze the changing trend of the incidence and mortality rates of liver cancer in the Chinese population from 1990 to 2019, and predict the future development trend of liver cancer.Methods:The descriptive epidemiologic method was conducted. Based on the Global Burden of Disease data from the Institute for Health Metrics and Evaluation at the University of Washington, the crude incidence rate and total number, crude mortality rate and total number, age- and sex-specific incidence rate and number, age- and sex-specific mortality rate and number of liver cancer in the Chinese population from 1990 to 2019 were collected. The age-standardized rate was calculated using the year 2000 China's standard population. Observation indicators: (1) the incidence and mortality of liver cancer in the Chinese population from 1990 to 2019; (2) changing trend of the age-standardized incidence and mortality rates of liver cancer in the Chinese population from 1990 to 2019; (3) prediction of the incidence and mortality of liver cancer in the Chinese population during 2020-2044. Count data were described as absolute numbers, percentages and ratios. The Joinpoint V.4.9.0.0 software was used to calculate the annual percent change (APC), average annual percent change (AAPC) and its 95% confidence interval ( CI) of age-standardized incidence and mortality rates of liver cancer in different time periods. The age-period-cohort model in the Nordpred package of R software (V.4.1.1) was used to predict the incidence and mortality of liver cancer in the Chinese population during 2020-2044. Results:(1) The incidence and mortality of liver cancer in the Chinese population from 1990 to 2019: the crude incidence rate and the age-standardized incidence rate of liver cancer in the Chinese population decreased from 20.01/100,000 and 24.31/100,000 in 1990 to 14.80/100,000 and 9.71/100,000 in 2019, respectively. The crude incidence rate and the age-standardized incidence rate of liver cancer in the Chinese male popula-tion decreased from 27.88/100,000 and 34.76/100,000 in 1990 to 22.05/100,000 and 15.22/100,000 in 2019, respectively. The crude incidence rate and the age-standardized incidence rate of liver cancer in the Chinese female population decreased from 11.63/100,000 and 13.51/100,000 in 1990 to 7.26/100,000 and 4.29/100,000 in 2019, respectively. The crude mortality rate and the age-standardized mortality rate of liver cancer in the Chinese population decreased from 19.64/100,000 and 23.97/100,000 in 1990 to 13.20/100,000 and 8.44/100,000 in 2019, respectively. The crude mortality rate and the age-standardized mortality rate of liver cancer in the Chinese male population decreased from 27.03/100,000 and 34.10/100,000 in 1990 to 19.18/100,000 and 13.03/100,000 in 2019, respectively. The crude mortality rate and the age-standardized mortality rate of liver cancer in the Chinese female population decreased from 11.78/100,000 and 13.64/100,000 in 1990 to 6.98/100,000 and 3.97/100,000 in 2019, respectively. (2) Changing trend of the age-standardized incidence and mortality rates of liver cancer in the Chinese population from 1990 to 2019: the trend of age-standardized incidence rate of liver cancer in the Chinese population could be divided into 5 periods, namely year 1990 to 1996, year 1996 to 2001, year 2001 to 2005, year 2005 to 2010 and year 2010 to 2019. In these 5 periods, the APC of age-standardized incidence rate changed from 1.27%(95% CI as 0.81% to 1.73%, P<0.001) to 1.12%(95% CI as 0.91% to 1.33%, P<0.001) of the total Chinese population, from 1.68%(95% CI as 1.19% to 2.17%, P<0.001) to 1.65%(95% CI as 1.42% to 1.87%, P<0.001) of the Chinese male population and from 0.21%(95% CI as -0.32 % to 0.75%, P=0.406) to -0.14%(95% CI as -0.40% to 0.11%, P=0.241) of the Chinese female population, respectively. The trend of age-standardized mortality rate of liver cancer in the Chinese population could be divided into 5 periods, namely year 1990 to 1996, year 1996 to 2000, year 2000 to 2005, year 2005 to 2012 and year 2012 to 2019. In these 5 periods, the APC of age-standardized mortality rate changed from 1.47%(95% CI as 0.74% to 2.20%, P=0.001) to 1.34%(95% CI as 0.78% to 1.90%, P<0.001) of the total Chinese population, from 1.96%(95% CI as 1.18% to 2.75%, P<0.001) to 1.79%(95% CI as 1.18% to 2.41%, P<0.001) of the Chinese male population and from 0.14%(95% CI as -0.54% to 0.82%, P=0.670 ) to 0.48%(95% CI as 0.02% to 0.93%, P=0.041) of the Chinese female population, respectively. From 1990 to 2019, the AAPC of age-standardized incidence and age-standardized mortality rates of liver cancer were -3.22%(95% CI as -3.41% to -3.03%) and -3.51%(95% CI as -3.82% to -3.19%) in the Chinese population, -2.90%(95% CI as -3.10% to -2.71%) and -3.22%(95% CI as -3.57% to -2.88%) in the Chinese male population, -3.96%(95% CI as -4.17% to -3.76%) and -4.13%(95% CI as -4.43% to -3.82%) in the Chinese female population, respectively. (3) Prediction of the incidence and mortality of liver cancer in the Chinese population during 2020-2044: the age-standardized incidence rate of liver cancer would decrease from 9.51/100,000 in 2015-2019 to 5.78/100,000 in 2040-2044 in the Chinese population, from 14.84/100,000 in 2015-2019 to 9.75/100,000 in 2040-2044 in the Chinese male population and from 4.28/100,000 in 2015-2019 to 1.88/100,000 in 2040-2044 in the Chinese female population, respectively. The age-standardized mortality rate of liver cancer would decrease from 8.40/100,000 in 2015-2019 to 4.62/100,000 in 2040-2044 in the Chinese population, from 12.91/100,000 in 2015-2019 to 7.59/100,000 in 2040-2044 in the Chinese male population and from 4.01/100,000 in 2015-2019 to 1.70/100,000 in 2040-2044 in the Chinese female population, respectively. The incidence number and mortality number of liver cancer from 2020 to 2044 would remain stable at around 160,000 per year and 140,000 per year in the Chinese population, 128,500 per year and 109,000 per year in the Chinese male population, 36,000 per year and 34,900 per year in the Chinese female population, respectively. Conclusion:The incidence and mortality rates of liver cancer in the Chinese popula-tion show a significant downward trend from 1990 to 2019, and the incidence number and mortality number of liver cancer in the Chinese population will remain stable above 100,000 during 2020-2044.

Objective:To investigate the early evaluation potential of serum levels of apolipoprotein B/apolipoprotein A1 (Apo B/A1), microtubule-associated protein 1-light chain 3 (MAP1-LC3) and intercellular adhesion molecule-1 (ICAM-1) in acute pancreatitis (AP) patients.Methods:A total of 413 AP patients who were treated at the Second Affiliated Hospital of Anhui Medical University between January 2019 and August 2020 were enrolled. Serum samples were collected from AP patients within 24 h of admission. Patients were divided into the non-severe acute pancreatitis (Non-SAP, n=315) and severe acute pancreatitis (SAP, n=98) groups according to the severity of the disease. Sixty healthy controls were recruited. The differences of serum Apo B/A1, MAP1-LC3 and ICAM-1 among the three groups were compared by one-way analysis of variance, and the correlation between Apo B/A1, MAP1-LC3 and ICAM-1 and the severity of AP was analyzed by Pearson correlation analysis. Sensitivity and specificity in assessing AP severity were predicted by receiver operating characteristic curve (ROC). Results:The early levels of Apo B/A1, MAP1-LC3 and ICAM-1 were all significantly higher for AP patients than for healthy controls ( P<0.05), and the levels of Apo B/A1, MAP1-LC3 and ICAM-1 in SAP patients were significantly higher than those in non-SAP patients[Apo B/A1: 2.21±1.40 vs. (0.96±0.34); MAP1-LC3: 0.92±0.29 vs. (0.48±0.24) ng/mL and ICAM-1: (235.57±54.50 ) vs. (120.28±61.69)ng/mL; P<0.05]. Pearson correlation analysis showed that levels of Apo B/A1, MAP1-LC3 and ICAM-1 were positively correlated with the first Ranson score after admission ( P<0.05), and ICAM-1 showed the highest degree of correlation with AP severity ( r=0.519). Areas under the receiver operating characteristic curve (AUROC) were 0.769 for Apo B/A1, 0.811 for MAP1-LC3, 0.828 for ICAM-1, and 0.938 for combined detection. Conclusions:Serum levels of Apo B/A1, MAP1-LC3 and ICAM-1 within 24 h after admission are significantly correlated with the severity of AP, which has clinical significance for early prediction of the severity of AP.

Artificial intelligence (AI) refers to the use of computer programs to simulate and extend human intelligence, and has application prospects in the diagnosis and treatment of diseases. This review focuses on the research status of the screening and diagnosis of NAFLD and nonalcoholic steatohepatitis using artificial intelligence technology, electronic health record data, multi-omics prediction models, image recognition technology based on liver imaging and pathological biopsy, and new drugs research and development, with a view to provide new ideas for the diagnosis and treatment.

BACKGROUND@#The cancer burden in the United States of America (USA) has decreased gradually. However, China is experiencing a transition in its cancer profiles, with greater incidence of cancers that were previously more common in the USA. This study compared the latest cancer profiles, trends, and determinants between China and USA.@*METHODS@#This was a comparative study using open-source data. Cancer cases and deaths in 2022 were calculated using cancer estimates from GLOBOCAN 2020 and population estimates from the United Nations. Trends in cancer incidence and mortality rates in the USA used data from the Surveillance, Epidemiology, and End Results program and National Center for Health Statistics. Chinese data were obtained from cancer registry reports. Data from the Global Burden of Disease 2019 and a decomposition method were used to express cancer deaths as the product of four determinant factors.@*RESULTS@#In 2022, there will be approximately 4,820,000 and 2,370,000 new cancer cases, and 3,210,000 and 640,000 cancer deaths in China and the USA, respectively. The most common cancers are lung cancer in China and breast cancer in the USA, and lung cancer is the leading cause of cancer death in both. Age-standardized incidence and mortality rates for lung cancer and colorectal cancer in the USA have decreased significantly recently, but rates of liver cancer have increased slightly. Rates of stomach, liver, and esophageal cancer decreased gradually in China, but rates have increased for colorectal cancer in the whole population, prostate cancer in men, and other seven cancer types in women. Increases in adult population size and population aging were major determinants for incremental cancer deaths, and case-fatality rates contributed to reduced cancer deaths in both countries.@*CONCLUSIONS@#The decreasing cancer burden in liver, stomach, and esophagus, and increasing burden in lung, colorectum, breast, and prostate, mean that cancer profiles in China and the USA are converging. Population aging is a growing determinant of incremental cancer burden. Progress in cancer prevention and care in the USA, and measures to actively respond to population aging, may help China to reduce the cancer burden.

Chronic kidney disease (CKD) is a progressive disease with many complications (eg, cardiovascular disease and acidosis and anemia) and high morbidity and mortality occurs in the population. There is no cure for this disease, current treatments including renin-angiotensin-aldosterone pathway inhibitors and sodium-glucose co-transporter 2 inhibitors can only delay the progression to end-stage renal disease. With the identification of more key factors and mechanisms in CKD development, new potential therapeutic approaches for CKD can be developed. This review summarizes the mainstays of therapy and strategies for CKD and related comorbidities to support the development of novel treatments.
ABSTRACT
Objective: To analyze and compare the distribution of the high-risk population of upper gastrointestinal (UGI) cancer and the factors influencing the compliance rate of endoscopic screening in urban China and rural China. Methods: From 2015 to 2017, an epidemiological survey was conducted on residents aged 40-69 in two rural areas (Luoshan county of Henan province, Sheyang county of Jiangsu province) and two urban areas (Changsha city of Hunan province, Harbin city of Heilongjiang province). As a result, high-risk individuals were recommended for endoscopic screening. Chi-square χ(2) test was used to compare the high-risk rate of UGI cancer between urban and rural residents. In addition, the multivariate logistic regression model was used to analyze the factors influencing the compliance rate of endoscopic screening. Results: A total of 48, 310 residents aged 40-69 were enrolled in this study, including 22 870 (47.34%) residents from rural areas and 25 440 (52.66%) residents from urban areas. A total of 23 532 individuals were assessed with a high risk of UGI cancer, with an overall risk rate of 48.71%. A higher proportion of participants with high risk was observed in rural China (56.17%, 12 845/22 870) than in urban China (42.01%, 10 687/25 440). A total of 10 971 high-risk individuals with UGI cancer participated in endoscopic screening, with an overall compliance rate of 46.62% (10 971/23 532), 45.15% (5 799/12 845) in rural China, and 48.40% (5 172/10 687) in urban China. In rural population, the compliance rate of endoscopic screening was higher in those of females, aged 50-69 years, primary school education or above, high income, a family history of UGI cancer, history of gastric and duodenal ulcer, history of reflux esophagitis, and history of superficial gastritis, but lower in smokers (P<0.05). Among the urban population, the compliance rate of endoscopic screening was higher in those aged 40-49 years, uneducated, low income, family history of UGI cancer, history of reflux esophagitis, history of superficial gastritis, but lower in smokers (P<0.05). Conclusions: The proportion of participants with high risk of UGI cancer in rural areas is higher than that of urban areas. The compliance rates of endoscopic screening in urban and rural areas are low, and influencing factors of endoscopic screening exhibit some differences in rural China and urban China.

Objective:To investigate the clinical significance of the primary tumor size in patients with endometrial carcinoma (EC).Methods:A total of 385 patients with EC admitted to Peking University People's Hospital from January 2006 to December 2016 with complete follow up data were selected, whose tumor size data before biopsy were retrospectively studied.Results:(1) The mean diameter of the primary tumor was (3.6±1.8) cm (range: 1-15 cm). And 48 cases were 0-<2 cm, 78 cases were 2-<3 cm, 92 cases were 3-<4 cm, 73 cases were 4-<5 cm, 94 cases were ≥5 cm. The diameter of the tumor was associated with age <60 years old, premenopause, CA 125≥35 kU/L, non-parturition, poor differentiation, stage Ⅲ-Ⅳ, depth of myometrial infiltration ≥1/2, cervical interstitial involvement, adnexal metastasis and lymph node metastasis (all P<0.05), but not associated with body mass index, hypertension, diabetes mellitus, pathology, lymph-vascular space invasion (all P>0.05). (2) Among the 334 patients underwent lymphadenectomy, 45 (13.5%, 45/334) cases with lymph node metastasis were observed. Stratified analysis showed that lymph node metastasis and recurrence rate of patients with EC gradually increased with the increase of tumor size ( P<0.05). Adopting 2, 3, 4 and 5 cm as cut-off values of tumor size, there were significant differences in the rate of lymph node metastasis and recurrence among them observed ( P<0.05), except for lymph node metastasis rate and recurrence rate when the cut-off value was 2 cm ( P>0.05). (3) An receiver operating characteristic (ROC) curve analysis showed that a tumor diameter of 4.25 cm was the cut-off prognostic value to predict lymph node metastasis and recurrence of EC. Conclusions:Tumor diameter is significantly correlated with lymph node metastasis and recurrence in patients with EC. Tumor size should be considered in determining the scope of surgery and adjuvant therapy.

Objective@#To investigate the effects of parental feeding practices on anemia in children aged 0-6 years in the minority area of the plateau, and to provide evidence for anemia intervention.@*Methods@#A total of 1 726 children aged 0-6 years in 4 counties of Gannan Tibetan Autonomous Prefecture were selected by stratified random cluster sampling to measure hemoglobin level. Parental feeding practices were evaluated.@*Results@#The anemia prevalence rate among children aged 0-6 years was 50.3%. The prevalence of anemia was higher in those with younger age, Tibetan ethnic, rural residence, low parents education level and family income(χ2=156.95,899.51,148.17,8.18,16.36,11.03,P<0.05). Parentl awareness rates on feeding knowledge were 10.0%-53.7%. The report rates of distraction and force-feeding were 51.4% and 36.1%, respectively. Logistic regression analysis showed that parents incorrect feeding knowledge and compulsive feeding behaviors were positively associated with anemia in children(P<0.05).@*Conclusion@#Improvement of parents health literacy and feeding behavior is an important intervention to reduce anemia in children aged 0-6 years.

OBJECTIVE@#To verify the superiority of motor imagery acupuncture in improving muscle tension for patients with upper limb hemiplegia in early stroke.@*METHODS@#A total of 64 patients of stroke hemiplegia with upper limb flaccid paralysis were randomly divided into an observation group (32 cases, 1 case dropped off ) and a control group (32 cases, 4 cases dropped off ). The observation group was treated with motor imagery acupuncture (both acupuncture and motor imagery therapy at affected upper limb were performed).The control group was treated with acupuncture plus motor imagery therapy at affected lower limb, 2 h later after acupuncture, motor imagery therapy was applied to upper limb. Baihui (GV 20) to Taiyang (EX-HN 5) of healthy side, Fengchi (GB 20) and Jianyu (LI 15), Jianjing (GB 21), Quchi (LI 11), Waiguan (TE 5) on the affected side, ect. were selected in both groups, once a day, 5 times a week for 4 weeks. Before and after treatment, 4, 8 weeks after treatment, the modified Ashworth scale (MAS) grade and Brunnstrom stage were compared in the two groups.@*RESULTS@#Compared before treatment, the muscle tension of shoulder, elbow and wrist each time point after treatment was increased in the two groups (@*CONCLUSION@#Motor imagery acupuncture could promote hemiplegia upper limb muscle tension recovery in patients of stroke hemiplegia with upper limb flaccid paralysis, make the patients gradually shift to the separate fine movement mode, inhibit and relieve the appearance and development of spasm.

BACKGROUND: Traumatic spinopelvic dissociation severely damages the stability of the lumbosacral region, with many treatment complications, poor prognosis and many influencing factors. OBJECTIVE: To analyze the influencing factors of clinical efficacy of lumbopelvic internal fixation with nail-rod system treatment of traumatic spinopelvic dissociation. METHODS: Complete data of 16 patients with traumatic spinopelvic dissociation from Tianjin Ninghe Hospital between September 2009 and September 2018 were analyzed retrospectively. All patients received lumbopelvic internal fixation with nail-rod system. There were 12 cases with nerve injury, of which 10 cases were treated with sacral nerve decompression. Two cases had no responsible fracture block on imaging and were not treated. Postoperative effect of treatment was evaluated according to the evaluation criterion of Japanese Orthopaedic Association and radiographic measurements of pelvic incidence and British Medical Research Council neurological function. Univariate statistical analysis was performed in the correlation between age, gender, timing of surgery, reduction quality, comorbid injury, degree of nerve injury and postoperative functional score. Among them, linear correlation analysis was conducted between age and Japanese Orthopaedic Association score. The correlation of gender, timing of surgery, reduction quality, comorbid injury, and degree of nerve injury with Japanese Orthopaedic Association score was analyzed with the Chi-square test. RESULTS AND CONCLUSION: (1) All patients were followed up for 17-38 months. (2) Postoperative evaluation results of Japanese Orthopaedic Association score showed that 4 cases were rated excellent, 5 good, 4 fair and 3 poor. (3) Imaging measurements showed that the pelvic incidence improvement rate of flexion sacral fracture was 78%, and the neutral type and extension type did not improve. (4) British Medical Research Council results demonstrated that neurological injuries were recovered of “excellent” in 5 cases, “good” in 4 and “no changes” in 3 patients. Clinical recovery rate of neurological injury was 75%. (5) Statistical analysis showed that the degree of nerve injury and accompanying anterior pelvic ring injury affected the surgical outcome. (6) It is indicated that the efficacy of surgical treatment of traumatic spinopelvic dissociation is affected by many factors. Based on the treatment of combined injury, the traumatic spinopelvic dissociation should be treated operatively with lumboiliac fixation early to maintenance the stability of spin-pelvic and pelvic ring, and exact decompression for neurological injuries is the key to achieve a satisfactory curative effect.

BACKGROUND@#Early detection of gastric cancer (GC) has been the topic of major efforts in China. This study aimed to explore the risk factors associated with GC and to provide evidence for the selection of a high-risk population of GC.@*METHODS@#Based on the cancer screening cohort of the National Cancer Screening Program in Urban China, GC patients diagnosed by endoscopy and pathological examinations constituted the case group, and controls were 1:3 matched by sex and age (±5 years) individually. The variables were selected by univariable analysis of factors such as body mass index (BMI), dietary habits, lifestyle, stomach disease history, and family history of GC; and multivariable logistic regression was used to analyze the influencing factors of GC and to calculate the odds ratio (OR) of related factors and its 95% confidence interval (CI).@*RESULTS@#A total of 215 GC cases and 645 matched healthy controls were included in the final analysis, with a median age of 61 years for the case and control groups. Overall analysis showed that high educational level (above primary school) (OR = 0.362, 95% CI = 0.219-0.599, P < 0.001), overweight/obesity (BMI ≥24 kg/m2; OR = 0.489, 95% CI = 0.329-0.726, P < 0.001), cigarette smoking (OR = 3.069, 95% CI = 1.700-5.540, P < 0.001), alcohol consumption (OR = 1.661, 95% CI = 1.028-2.683, P = 0.038), history of stomach disease (OR = 6.917, 95% CI = 4.594-10.416, P < 0.001), and family history of GC in first-degree relatives (OR = 4.291, 95% CI = 1.661-11.084, P = 0.003) were significantly correlated with the occurrence of GC. Subgroup analyses by age and gender indicated that GC risk was still increased in the presence of a history of stomach disease. A history of chronic gastritis, gastric ulcer, or gastric polyposis was positively associated with GC, with adjusted ORs of 4.155 (95% CI = 2.711-6.368), 1.839 (95% CI = 1.028-3.288), and 2.752 (95% CI = 1.197-6.326).@*CONCLUSIONS@#Subjects who smoke, drink, with history of stomach disease and family history of GC in first-degree relatives are the high-risk populations for GC. Therefore, attention should be paid to these subjects for GC screening.

Excessive exercise makes the body consume more oxygen and produce excessive free radicals. The increased free radicals lead to oxidative stress injury and dysfunctions in liver tissue. Our previous study showed that Anwulignan, an active monomer in Schisandra sphenanthera Rehd. et Wils. (Schisandra), had anti-fatigue effects in mice. However, whether Anwulignan has a protective effect on liver damage in exhausted mice and the mechanism underlying remain elusive. An exhaustive swimming mice model was used to study the protective effects of Anwulignan on liver damage. The involvement of the nuclear factor (erythroid-derived 2)-like 2 (NRF2)/antioxidant responsive element (ARE) antioxidative pathway in Anwulignan-mediated anti-fatigue was analyzed using NRF2 inhibitor ML385 in HepG2 cells treated with H2O2. Animal welfare and experimental process follow the regulations of the Animal Ethics Committee of Beihua University. Anwulignan significantly lowered serum alanine aminotransferase (ALT) and aspartate aminotransferase (AST) levels, reduced liver tissue damages, increased superoxide dismutase (SOD), glutathione peroxidase (GSH-Px), and catalase (CAT), and decreased malondialdehyde (MDA) and 8-hydroxy-2 deoxyguanosine (8-OHdG) contents in the livers of exhausted mice, demonstrating a strong antioxidant effect. Furthermore, Anwulignan up-regulated the NRF2/ARE antioxidant pathway in liver tissue, increased B-cell lymphoma 2 (Bcl-2) expression, and decreased Bcl-2-like protein 4 (Bax) and caspase3 expression. In HepG2 cells, Anwulignan improved the cell viability and SOD activity, reduced reactive oxygen species (ROS) and MDA contents, up-regulated the expression of the NRF2/ARE signaling pathway and Bcl-2, and decreased Bax and caspase3 expression in the cells. Furthermore, pretreated ML385 partly abolished all these effects of Anwulignan. Anwulignan protects the liver from damage in the exhausted mice by its antioxidant effects and related to its activation of the NRF2 pathway.

Objective To investigate the genetic polymorphisms of Plasmodium falciparum multidrug resistance protein 1 (PfMDR1), chloroquine resistance transporter (PfCRT) and Kelch 13 (PfK13) genes in Bioko Island, Equatorial Guinea, so as to provide insights into the development of the malaria control strategy in local areas. Methods A total of 85 peripheral blood samples were collected from patients with Plasmodium falciparum infections in Bioko Island, Equatorial Guinea in 2018 and 2019, and genomic DNA was extracted. The PfMDR1, PfCRT and PfK13 genes were amplified using a nested PCR assay. The amplification products were sequenced, and the gene sequences were aligned. Results There were no mutations associated with artemisinin resistance in PfK13 gene in Bioko Island, Equatorial Guinea, while drug-resistant mutations were detected in PfMDR1 and PfCRT genes, and the proportions of PfMDR1_N86Y, PfMDR1_Y184F and PfCRT_K76T mutations were 35.29% (30/85), 72.94% (62/85) and 24.71% (21/85), respectively. Conclusion There are mutations in PfMDR1, PfCRT and PfK13 genes in P. falciparum isolates from Bioko Island, Equatorial Guinea.

Objective@#To understand changes of physical fitness, among college freshmen in one college in Xi an from 2005 to 2018.@*Methods@#Body mass index (BMI), vital lung capacity and cardiorespiratory endurance test data from 47 047 freshmen from 2005 to 2018 were included and analyzed. BMI, vital lung capacity, and cardiorespiratory endurance were classified according to the National Student Physical Fitness and Health Standard. Descriptive statistics, t test was used in the data analysis.@*Results@#Mean BMI, vital lung capacity and cardiorespiratory endurance for male and female were (21.69±3.26) (20.88±2.65) kg/m 2, (4 189.87±788.57) (2 815.06±572.07) mL, (246.17±24.09) (239.97±20.23) s, respectively, all of which had significant statistical differences (t=29.48, 218.87, 28.70, P<0.05). Physical fitness indicators showed significant differences over the years (F=104.82, 185.43, 40 891.99, P<0.05). Mean BMI and vital lung capacity in male and female freshmen showed nonlinear increase trend across the past 14 years, while endurance showed a "V"shaped change with a significant inflection point in the year of 2009.@*Conclusion@#From 2005 to 2018, BMI and vital lung capacity showed an upward trend, and cardiorespiratory endurance quality continued to decline since 2009 among college freshmen.
