ABSTRACT

The "quality-oriented education" is closely integrated with the problem-based learning(PBL) teaching mode. In the teaching of Technologies for Microbial Detection course, the PBL teaching mode is the carrier, the student is the host, and the brainstorm guides the students to conduct "dialectical thinking", so as to cultivate the students' "innovation", "social morality" and "self-learning ability". We choose the interactive platform of "WeChat" and "MosoInk WebSite" to make up for the deficiency of PBL teaching in the large class, and comprehensively assess students' knowledge and skills, humanistic quality, and value judgment.

ABSTRACT

Objective:To evaluate the effect of remote lifestyle intervention on the management of weight and related indicators in the overweight and obese population. Methods:A total of 400 individuals with overweight or obesity who participated in remote lifestyle intervention in the Health Management Department of Shenzhen People′s Hospital from May 2015 to December 2018 were included as the remote intervention group, and 400 individuals with overweight and obesity who matched their age and gender were selected as the control group. Dietician established individual WeChat groups with the remote intervention group, and the WeChat platform was used to conduct remote lifestyle intervention for them, including setting weight control goals, giving timely feedback to the food log based on photos, providing exercise guidance and psychological support. The control group received personalized diet and exercise prescriptions, but did not receive remote intervention. After adjusting the data by propensity score matching method, univariate and multivariate logistic regression models were used to analyze the management effect of weight and related indicators in the two groups after one year. Results:After one year of intervention, effective data were obtained from a total of 755 cases (371 cases in the remote intervention group and 384 cases in the control group), and effective data were retained from 446 cases (223 cases in the remote intervention group and 223 cases in the control group) after bias matching. The body mass index (BMI), systolic blood pressure, fasting blood glucose, triglyceride, total cholesterol, low-density lipoprotein, and serum uric acid of the remote intervention group [(24.85±2.52) kg/m 2, (110.21±10.53) mmHg, (4.96±0.65) mmol/L, (1.25±0.82) mmol/L, (4.87±1.11) mmol/L, (2.88±0.74) mmol/L, and (306.01±95.66) mmol/L respectively] were significantly lower than that of the control group [(27.76±2.28) kg/m 2, (121.14±14.07) mmHg, (5.10±0.87) mmol/L, (1.54±0.83) mmol/L, (5.28±0.96) mmol/L, (3.13±0.80) mmol/L, (355.16±92.68) mmol/L respectively] (all P<0.05). After intervention, intervention was consistently being influencing factors when BMI was reduced by 4%―12%, ( P<0.05). The probability of a 12% reduction in BMI in the remote intervention group was 112.486 times higher than that in the control group (95% CI: 16.852-890.266). At the same time, the initial BMI was an influential factor for the restoration of normal BMI. For every 1 kg/m 2 decrease in the initial BMI, the probability of restoration of normal BMI was 4.76 times higher than that before the decrease (95% CI: 3.222-5.057). Conclusions:Remote lifestyle intervention has a certain effect on the management of weight and related indicators in the overweight and obese populations. It has significant effect on weight loss of overweight and mildly obese people, but has limited effect on moderate and severe obese people.

ABSTRACT

Objective:To understand the screening of cervical cancer in our county and provide epidemiological data for the prevention and control of cervical cancer. Methods:A total of 3 034 healthy women with 30~65 years old in Chun'an county were recruited in this prospective study from June 2015 to January 2016.All of the subjects received cervical cancer screening. The respondents were randomly divided into non-typeable HPV screening group (2018 cases) and the vision inspection with acetic acid and visual inspection with Lugol's iodine screen (VIA/VILI) group (1016 cases). Direct colposcopy or VIA/VILI visual examination was performed for those with positive HPV, and then colposcopy was performed for those with positive visual examination. The suspicious lesions tissues were applied for pathology detection. From September 2017 to February 2018, 2 363 patients who were initially negative received VIA/VILI combined with unclassified HPV examination, and those who were positive received colposcopy and pathological biopsy. The positive rate of primary screening and cervical squamous intraepithelial neoplasia disorders level 2 and above (CIN Ⅱ + ) lesion detection rate were analyzed in the two-stage screening. Results:From June 2015 to January 2016, the preliminary screening positive rate of unclassified HPV group was 11.60% (234/2 018). A total of 150 patients with positive results were recalled for colposcopy, with a recall rate of 64.10% (150/234). Among the 150 cases, 119 cases (79.33%) were positive in the unclassified HPV preliminary screening, and 31 cases (20.66%) were positive in VIA/VILI examination after HPV preliminary screening. The positive rate of VIA/VILI initial screening group was 20.28% (206/1 016), and the recall rate of colposcopy was 99.51% (205/206). A total of 108 cases underwent colposcopy with abnormal pathological biopsy examination in the two groups, and the pathological biopsy rate was 30.34% (108/356), and the pathologic biopsy CIN Ⅱ+ lesion detection rate was 297/100 000 (9/3 034). In the second round of screening from September 2017 to February 2018, 2 363 subjects were recruited back to the baseline, with a recall rate of 77.89% (2 363/3 034). In the 2 363 cases, the early screen VIA/VILI anomalies or HPV positive early screening positive rate was 17.01% (402/2 363), and the colposcopy examination the recall rate was 80.10% (322/402), and 80 cases received vaginal lens suspicious pathological biopsy, the pathological biopsy rate was 24.84% (80/322), the pathological results showed that CIN Ⅱ+ detection rate of 296/100 000 (7/2 363).Conclusion:The detection rate of precancerous suspicious lesions in cervical cancer screening in our county is lower than the national average level. On the one hand, we should strengthen the propaganda and education of active screening, improve the compliance and cooperation degree of screening objects, and improve the recall rate of suspicious cases. On the other hand, the government should invest more efforts to select the screening technology with high sensitivity, improve the screening technology of doctors at the basic screening point, and improve the detection rate of cervical cancer prelesions.

ABSTRACT

Objective To explore the dynamic characteristics of the thoracolumbar osteoporotic vertebral bodies under free state. Methods Based on CT data from the thoracic and lumbar vertebral body of a healthy female volunteer, model materialization and intervertebral disc tissue reconstruction were realized by using the computerized processing software. The finite element models of normal thoracolumbar vertebral body and thoracolumbar osteoporotic vertebral body were established in ABAQUS 6.14 to perform modal analysis. Results Compared with the normal model, the osteoporosis model had a lower natural frequency and a lager amplitude. As the vibration frequency increased, the model vibration type changed from uniaxial and unidirectional motion to multiaxial and multidirectional motion, and the responsible vertebral body for the maximum amplitude moved down gradually. Conclusions Modal analysis can better analyze dynamic characteristics of the thoracolumbar osteoporotic vertebral body. Patients with osteoporosis should try to avoid the specific vibration environment, so as to decrease the risk of intervertebral disc tissue degeneration, strain of thoracolumbar soft tissues and lesion in posterior structures of the vertebral body.

ABSTRACT

BACKGROUND@#Myocardial infarction occurs due to insufficient (ischemia) blood supply to heart for long time; plasmacytoma variant translocation 1 (PVT1) is a long non-coding RNAs (lncRNAs) involved in the pathogenesis of various diseases, including heart disease; However, few studies have explored its role. The present study evaluated the effects of lncRNA PVT1 on hypoxic rat H9c2 cells.@*METHODS@#Hypoxic injury was examined by measuring cell viability and apoptosis by using cell counting kit-8 activity and flow cytometry assays. Gene expressions after hypoxia were estimated by quantitative real time polymerase chain reaction and the signaling pathway were explored by Western blot analysis. RNA immunoprecipitation and luciferase reporter assays were applied to examine the interactions among genes. Data were analyzed using t-test with one-way or two-way analysis of variance.@*RESULTS@#The lncRNA PVT1 is up-regulated in hypoxia-stressed H9c2 cells and knockdown of PVT1 mitigates hypoxia-induced injury in H9c2 cells. PVT1 acts as a sponge for miR-135a-5p and knockdown of PVT1 attenuated the increased hypoxia-induced injury by up-regulating miR-135a-5p. Forkhead box O1 (FOXO1) was identified as a target of miR-135a-5p, and the expression was negatively regulated by miR-135a-5p. The exploration of the underlying mechanism demonstrated that knockdown of FOXO1 reversed PVT1/miR-135a-5p mediated hypoxia-induced injury in H9c2 cells.@*CONCLUSIONS@#PVT1 plays a crucial role in hypoxia-injured H9c2 cells through sponging miR-135a-5p and then positively regulating FOXO1.

ABSTRACT

Objective:To explore the effect of irinotecan combined with apatinib on the prognosis of patients with advanced gastric cancer.Methods:Sixty-two patients with advanced gastric cancer who were admitted to Shanghai Minhang District Cancer Hospital from January 2016 to October 2018 were selected. Among them, 30 patients were treated with apatinib (control group), and 32 cases were treated with irinotecan combined with apatinib(observation group). Before administration, the two groups were given routine symptomatic intervention, including prevention of vomiting, improvement of leukocyte, nutrition supplement, etc. During the treatment, routine blood examination, liver and kidney function examination were carried out regularly, and the drug dosage was adjusted according to the situation. The chemotherapy regimen was given once every 3 weeks, and both groups received 4 cycles of treatment. After 3 months of treatment, the short-term efficacy was evaluated, and the therapeutic effects of the two groups were compared. The incidence of side effects was analyzed. After 12 months of follow-up, the survival curves were drawn by Kaplan-Meier method, and the progression-free survival (PFS) and overall survival (OS) of the two groups were analyzed. Results:The recent overall remission rates of the observation group and the control group were 9.38% (3/32) and 3.33% (1/30), and the difference was not statistically significant ( χ2 = 0.203, P = 0.652). The disease progression rate of the observation group was lower than that of the control group, and the difference was statistically significant [28.13% (9/32) vs. 53.33% (16/30), χ2 = 4.089, P = 0.043]. There was no statistical difference in the incidence of platelet decline, hemoglobin decline, white blood cell decline, diarrhea, nausea and vomiting, liver damage, and bone marrow suppression between the two groups (both P > 0.05). The median PFS and OS time in the observation group were 5.8 months (5.0-6.3 months) and 8.1 months (7.2-9.3 months), respectively, which were longer than those in the control group [4.9 months (4.7-5.1 months) and 6.0 months (5.2-6.9 months), and the differences were statistically significant (both P < 0.01). Conclusion:The combination of irinotecan and apatinib can reduce the disease progression rate, prolong the survival time, and does not increase the adverse reactions.
