ABSTRACT

Acute pancreatitis-associated ascites fluid (PAAF) is a common complication in patients with acute pancreatitis (AP) and is closely associated with the severity of AP, the development of local and systemic complications, and prognosis. PAAF may originate from the leakage of abdominal blood vessels, lymphatic vessels, and pancreatic duct. Recent studies have found that early removal of PAAF by abdominal paracentesis drainage can help to reduce systemic inflammation and alleviate pancreatitis-associated organ injury, thereby improving the conditions of patients with severe AP and reducing mortality. However, it is still not completely clear how PAAF aggravates systemic inflammatory response, participates in pancreatic injury and damage of distal organs, and leads to the aggravation of disease conditions in patients with AP. Therefore, this article gives an overview of PAAF and summarizes related studies in recent years, so as to provide directions for exploring the pathophysiological process and treatment of AP.

ABSTRACT

Surgical operation is the main treatment method for pancreatic cancer, and in clinical practice, radical surgery for pancreatic cancer is often combined with superior mesenteric-portal vein confluence pancreaticoduodenectomy to achieve R0 resection. However, severe left-sided portal hypertension (LSPH) may occur after splenic vein dissection, resulting in a series of pathological changes such as congestive splenomegaly, thrombocytopenia, backflow obstruction of splenic vein, and gastrointestinal varices, and in some cases, it can lead to fatal gastrointestinal hemorrhage and hemorrhagic shock. Therefore, in order to better manage LSPH in clinical practice, this article systematically analyzes and reviews the pathogenesis, treatment regimens, and control strategies of LSPH after combined superior mesenteric-portal vein confluence pancreaticoduodenectomy and put forward corresponding suggestions based on current studies.

ABSTRACT

Tissue regeneration is an important engineering method for the treatment of oral soft and hard tissue defects.Growth factors,as one of the three elements of tissue regeneration, are a necessary condition for tissue regeneration.Concentrated growth factor(CGF)is a new generation of blood extract prepared by changing the centrifugal speed on the basis of the preparation of platelet-rich plasma(PRP)and platelet-rich fibrin(PRF).It contains abundant growth factors and a fibrin matrix with a three-dimensional network structure, being capable of activating angiogenesis and promoting tissue regeneration and healing.CGF has been widely used in the repair and regeneration of oral soft and hard tissues. This paper introduces the preparation and composition of CGF and reviews the application of CGF in oral implantation and the regeneration of oral bone tissue, periodontal tissue, and dental pulp tissue.

ABSTRACT

OBJECTIVES@#This study aimed to investigate the common types and directions of root fractures of the maxillary first molar and the influence of root canal treatment on the prevalent sites of root fractures.@*METHODS@#A total of 274 maxillary first molars with root fractures diagnosed via cone beam computed tomography were included. The root fractures of nonendodontically and endodontically treated teeth were identified to be spontaneous and secondary root fractures, respectively. The sites, types, and directions of spontaneous and secondary root fractures were determined.@*RESULTS@#Among the spontaneous root fractures, the proportion of palatal root fractures (56.1%) was higher than those of mesial buccal root fractures (36.1%) and distal buccal root fractures (7.8%). Among the secondary root fractures, the proportion of mesial buccal root fractures (52.7%) was higher than those of palatal root fractures (36.5%) and distal buccal root fractures (10.8%). The distribution of predominant fracture sites was statistically significant (@*CONCLUSIONS@#This study provided an epidemiological basis for the clinical features of root fractures of the maxillary first molar. During the dia-gnosis and treatment of the maxillary first molar, the possibility of palatal root fractures should be considered. The occurrence of mesial buccal root fractures may be related to root canal treatment. Therefore, the risk of mesial buccal root fractures caused by iatrogenic factors should be minimized.

ABSTRACT

Objective: To investigate the clinical characteristics of patients with hypertrophic cardiomyopathy (HCM) and restrictive cardiomyopathy (RCM) complicating with intracardiac thrombosis. Methods: This is a retrospective observational study. Consecutive patients diagnosed with HCM or RCM and complicated with intracardiac thrombosis (including left and right atrium or ventricular thrombosis), who were admitted to the Heart Failure Care Unit of Fuwai Hospital, Chinese Academy of Medical Sciences, from September 2008 to September 2018, were enrolled in this study. Patients with myocardial infarction were excluded. The general clinical data of the enrolled patients, including demographic data, major complications, laboratory indicators, echocardiographic indicators, drug application and distribution of intracardiac thrombosis, were collected from electronic medical record system and analyzed. Results: A total of 98 patients were enrolled in this study, including 52 patients (53.1%) with HCM and 46 patients (46.9%) with RCM. The most common comorbidity was atrial fibrillation/flutter: 40 patients (76.9%) in HCM group and 36 patients (78.3%) in RCM group. Majority of patients received oral anticoagulants treatment: 43 patients (82.7%) in HCM group and 35 patients (76.1%) in RCM group. Intracardiac thrombosis was mainly located in the left atrium in both HCM group (39 cases (75.0%)) and RCM group (32 cases (69.6%)). Thrombosis was found in ≥ 2 chambers in 7 patients (7.1%). Rate of left atrial thrombosis was the highest (81.6% (62/76)) in HCM and RCM patients complicating with atrial fibrillation/flutter. Intra-aneurysmal thrombosis occurred in 4 out of 5 patients complicated with apical left ventricular aneurysm. The rate of left ventricular thrombosis in patients with left ventricular ejection fraction≥50% was 7.4% (4/54), which was significantly lower than that in patients with left ventricular ejection fraction<50% (34.5%(10/29)) (P<0.01). Conclusion: There are certain distribution characteristics of HCM and RCM patients with intracardiac thrombosis, and the left atrium is the most common site of thrombosis, more attention should be paid in HCM and RCM patients on the diagnosis and treatment of intracardiac thrombosis.

ABSTRACT

Objective To study the influence of tarsal gland massage on postoperative curative effect of femtosecond laser combined with excimer laser keratomileusis. Methods A prospective study was conducted on patients who underwent the same surgical approach by the same surgeon. 78 eligible patients were randomly divided into two groups:39 patients in experimental group, equal to 78 eyes, were given tarsal gland massage before the operation;and the other 39 patients in control group, equal to 78 eyes, were not given tarsal gland massage before the operation. The presence condition of postoperative interlayer eyelid fat of the two groups of patients was investigated. After that, the patients were divided into two groups according to above eyelid fat condition:with eyelid fat group and without eyelid fat group, and the visual acuity recovery status were compared between the two groups. Results In the control group after the operation, 3 patients could not be contacted in the follow-up study, but a total of 36 patients, equal to 72 eyes were observed by slit lamp, and there were totally 10 eyes with interlayer eyelid fat, accounting for 13.89%;Whereas, 2 patients could not be contacted in the experimental group, but a total of 37 patients, equal to 74 eyes were observed, and there were only 2 eyes with interlayer eyelid fat, accounting for 2.7%. The P value of comparison between these two groups was less than 0.01, which had statistical differences. There was significant difference between the two groups ( <0.05) in the recovery of visual acuity within one month after the operation. Conclusion Tarsal gland massage can effectively reduce the residual of interlayer eyelid fat after the operation of femtosecond laser combined with excimer laser keratomileusis, thereby it can avoid the delay of vision recovery and effectively improve the postoperative curative effect.

ABSTRACT

Purpose To investigate the expression of mitogen-activated protein kinase kinase kinase 3 (MAP3K3) mRNA in ovarian carcinoma patients and to explore the correlation among its expression, clinicopathological features and prognosis. Methods The expression of MAP3K3 mRNA in ovarian carcinoma and fallopian tube tissues were detected by qRT-PCR, and the correlation between MAP3K3 mRNA expression and clinicopathological features was also analyzed. Whether MAP3K3 mRNA expression could be used as an independent predictor of prognosis for patients with ovarian carcinoma was further determined. Results The expression of MAP3K3 mRNA in ovarian carcinoma was significantly higher than that in fallopian tube tissues (P<0.05). High expression of MAP3K3 mRNA was significantly correlated with FIGO stage and Challenge model of ovarian carcinoma (P<0.05). Kaplan-Meier survival analysis showed that the disease-free survival time and overall survival time of patients with high MAP3K3 mRNA expression were shorter than those with low expression (34 months vs 52.2 months, P<0.05.38.6 months vs 52.5 months, P<0.05). Univariate analysis showed that high expression of MAP3K3 mRNA was a risk factor for poor prognosis of ovarian carcinoma patients (HR=4.198, 95%C/: 1.711 ~10.302, P<0.05). Conclusion MAP3K3 mRNA is highly expressed in ovarian carcinoma tissues. Its high expression is associated with FIGO stage, Challenge model and poor prognosis of ovarian carcinoma, which may involve in the malignant transformation of ovarian carcinoma.

ABSTRACT

Serpiginous choroiditis (SC) is infrequent, chronic and posterior uveitis displaying a geographic pattern of choroiditis easy to recur. Studies reveal that the active lesions of inflammatory processes are mainly localized to the choriocapillaris and retinal pigment epithelium cells. SC may manifest with variable features, although a creeping pattern of choroiditis, extending from the juxtapapillary area, with grayish yellow discoloration. Fundus fluorescein angiography, indocyanine green angiography, fundus auto-fluorescence and optical coherence tomography are helpful to diagnose atypical SC. In addition, these image examinations can evaluate the activity and progression of lesion, and detect any complication that might occur. SC is mainly distinguished from multifocal SC related with tuberculosis or virus and etc. Pathogenesis is unclear, an organ-specific autoimmune inflammation or infection seems likely to be the underlying process. It is mainly using glucocorticoid with immunosuppressant therapy at present. Timely and effectively control inflammation can effectively prevent vision loss, choroidal neovascularization and choroidal scar in SC patients.

ABSTRACT

Purpose To explore the association between single nucleotide polymorphisms (SNPs) of TNF-αand IL-10 genes as well as their plasma levels and pathogenesis of diffuse large B-cell lymphoma (DLBCL).Methods Snapshot SNP genotype technique was used to assess genetic variation in 6 SNPs for TNF-α and IL-10 in 38 DLBCL cases and 77 healthy controls, and enzyme-linked immunosorbent assay (ELISA) was used to measure the plasma concentrations of TNF-α and IL-10 in the above population. Results Plasma IL-10 level in DLBCL cases was higher than that in controls (P < 0.05),but there was no significant difference in plasma TNF-αlevel between the two groups (P > 0.05).Of all the candidate SNPs,the geneotype distribution for TNF-α-863C/A showed significant differences between the case and control groups (P < 0.05),and carriers with CA/AA genotypes had more two-fold risks of DLBCL than those with the CC genotype (95% CI =1.091-4.765,P =0.028).However several other candidate SNPs showed no significant difference in the geneotype distributions between the cases and controls (P > 0.05).After excluding the effects of disease status on plasma TNF-alpha and IL-10 levels, the polymorphism of each gene had no significant effect on the corresponding plasma concentration (P > 0.05).Conclusion TNF-α-863C/A polymorphism may be associated with the risk of DLBCL,and IL-10 play an important role in occurrence and development of DLBCL.

ABSTRACT

<p><b>OBJECTIVE</b>To understand the perception for the use of cataract surgical services in a population of acceptors and non-acceptors of cataract surgery in urban Beijing.</p><p><b>METHODS</b>From a community-based screening program a total of 158 patients with presenting visual acuity of less than 6/18 on either eye due to age-related cataract were informed about the possibility of surgical treatment. These patients were interviewed and re-examined 36 to 46 months after initial screening. The main reasons for not accepting surgery were obtained using a questionnaire. Vision function and vision-related quality of life scores were assessed in those who received and did not receive surgery.</p><p><b>RESULTS</b>At the follow-up examination 116 of the 158 patients were available and 36 (31.0%) had undergone cataract surgery. Cases who chose surgery had higher education level than those who did not seek surgery (OR=2.64, 95% CI: 1.08-6.63, P=0.02). There were no significant differences in vision function (P=0.11) or quality of life scores (P=0.16) between the surgery group and the non-surgery group. Main reasons for not having surgery included no perceived need (50.0%), feeling of being "too old" (19.2%), and worry about the quality of surgery (9.6%). Cost was cited by 1 (1.9%) subject as the main reason for not seeking surgery.</p><p><b>CONCLUSIONS</b>The data suggest that in China's capital urban center for patients with moderate visual impairment there is a relative low acceptance rate of cataract surgery, mainly due to people's perception of marginal benefits of surgery. Cost is not a determining factor as barrier to undergo surgery and patients with poorer education are less likely to undertake surgery.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate efficacies of three commonly used oral drugs including Berbamine Hydrochloride Tablet (B), Qijiao Shengbai Capsule (Q), and Leucogen Tablet (L) (by single drug, two drugs or three drugs) combined with granulocyte colony-stimulating factor (G-CSF) for treat ment of chemotherapy related leukocytopenia in mice.</p><p><b>METHODS</b>Totally 156 Kunming male mice were divided into the normal control group (A, n=24), the model group (B, n=24), the G-CSF group (C, n =24), the G-CSF+Q group (D, n=12), G-CSF+ B (E, n=12), the G-CSF+L group (F, n=12), the G-CSF + Q + B group (G, n=12), the G-CSF + Q + L group (H, n=12), the G-CSF + L + B group (I, n=12), and the G-CSF + L + Q + B (J, n=12). Mouse models of chemotherapy related leukocytopenia were established by intraperitoneal injection of cyclophosphamide (CTX). A G-CSF group was set up as a positive control. Mice were treated by a single oral drug, a single oral drug combined with G-CSF, and two or three drugs combined with G-CSF respectively, and the death rate calculated. Hemocytes [such as white blood cells (WBC) and its classification, red blood cells (RBC), platelet (PLT), hemoglobin (Hb)] were calculated by hematology analyzer. Mice were anatomized and important organs weighed. Organ indices were calculated.</p><p><b>RESULTS</b>There was no statistical difference in the mortality rate among all groups (P > 0.05). Compared with Group B, WBC was elevated in all other groups (P < 0.01). WBC and PLT were elevated most in Group J, Hb and RBC were also increased at the same time (P < 0.05, P < 0. 01). Compared with Group B, RBC increased in Group E, F, G, I, and J (P < 0.01); Hb obviously increased in Group C, E, F, H, I, and J (P<0.01). Compared with Group B and D, the promotion of erythroid hematopoiesis by G-CSF could be elevated in any group contained drug B and L (P < 0.05, P < 0.01). The spleen index of model mice could be significantly improved in Group C, D, and G (P < 0.01). The thymus index of model mice could be significantly improved in Group H (P < 0.05).</p><p><b>CONCLUSIONS</b>The best scheme to treat mice with chemotherapy related leukopenia or decreased three blood series was to administrate three commonly oral drugs combined with G-CSF. Authors speculated that G-CSF and Q might have a certain effect on CTX induced immune inhibition.</p>
