ABSTRACT
Objective:To investigate the correlation between hyponatremia and the severity of coronavirus disease 2019 (COVID-19).Methods:Clinical data of 12 patients with COVID-19 admitted to Shantou Central Hospital from January 23 to February 5 in 2020 were retrospectively analyzed, including gender, age, symptoms, lab test and clinical outcomes, to analyze the change trend of blood Na + level in the patients with COVID-19. Results:Among the 12 patients with COVID-19, there were 8 males and 4 females with the mean age of (38.0±16.3) years old, most of them were admitted to the hospital with cough and/or fever. All patients had a positive nucleic acid test for 2019 novel coronavirus (2019-nCoV), and were discharged after clinical treatment with oxygen therapy, antiviral, antibacterial, anti-inflammatory, and nutritional support. All patients were of ordinary type when they were admitted to the hospital. Among them, 1 patient turned into a severe case during the course of the disease, and 1 patient showed a tendency to become severe case. It was found that 10 patients without severe conversion had an average blood Na + of (138.3±1.3) mmol/L at admission, and the lowest blood Na + during the course of disease was (135.9±3.1) mmol/L. However, 2 patients who became severe and had a tendency to become severe disease (Na + levels at admission were 140.0 mmol/L and 138.0 mmol/L, respectively) experienced hyponatremia during the course of the disease (the lowest blood Na + levels were 129.0 mmol/L and 122.0 mmol/L). Further analysis showed that the lower serum Na + level, the higher level of white blood cell count (WBC) and C-reactive protein (CRP), but serum Na + level was consistent with the change trend of lymphocytes, suggesting that hyponatremia was closely correlated with severe inflammation reaction. Conclusions:Serum Na + showed decreasing tendency during the development of COVID-19, and hyponatremia was closely related to the severity of COVID-19. It was necessary to pay great attention to the change trend of blood Na + level. However, further research was needed to obtain more reliable conclusions and explorer the pathophysiological mechanisms.
ABSTRACT
[Objective]To discuss the intension and extension of five excesses & five deficiency in Canon of Medicine, and to investigate its guiding value in clinical practice.[Method]By consulting document, and to know how these ancient Chinese medicine doctors to understand and analyse five excesses & five deficiency in Canon of Medicine, with deep demonstration, and finally to prove by clinical use.[Result] Most ancient Chinese medicine doctors agreed that five excesses & five deficiency was clinical critical disease, usually signified an ominous prognosis, and finally led to die.On the one hand it was excessive pathogenic qi rampant, disturbing five organs; on the other hand it was asthenia of health qi, and fall of five organs. In summary, the pathogenesis of five excesses was five organs being attacked, pathogenic qi impatented, excessive heat, riddlling in both internal and external, main pathogenesis was internal stagnancy of pathogenic heat, leading exhaustion of yin or yang, while the pathogenesis of five deficiency was real yang exhausted, real yin exhausted. Therefore treatment principle of five excesses & five deficiency has guiding significance for clinician, especially to protect stomach-qi in clinical practice.[Conclusion]The Canon of Medicine first proposed the theory of five excesses & five deficiency, including the conception, prognosis, and treatment principle. Five excesses & five deficiency were the earliest classification of excesses & deficiency of five-viscera, and it established the foundation of excesses & deficiency syndrome differentiation in TCM, and It had guiding significance to identify, judge and treatment of TCM clinical critical disease, and it needed paying more attention and further research.

ABSTRACT
2013 witnessed the full coverage of the medical insurance lump-sum prepayment management at all hospitals above secondary level in Beijing. As a result, medical insurance management of the hospital turned its focus from“ expense settlement” to“ expense management”,with a multidimensional,multi-method,multi-sectoral and multi-index lump-sum prepayment of medical insurance management system set up.Methods and concepts introduced included diagnosis related groups (DRGs) management tools in hospitalization service management; equivalent hospital departments comparison in outpatient service for post management; the concept of total budget management concept, and the “market-operations-finance-performance” linkage mechanism for priority control of drug expenses; and focused management on key patient groups and drug/consumable suppliers.
ABSTRACT
Objective To invcstigate the effects of Maixiansan on insulin-like growth factor binding protein 7 (IGFBP7) and apoptosis in rats with ulcerative colitis related colorectal cancer. Methods The rat model of ulcerative colitis-related coloreetal cancer was induced by dextran sulfate sodium (DSS) and azoxymethane(AOM). 40 male SD rats [weight (160 ± 10) g] were randomly divided into 4 groups: model, Maixiansan and Meisalazine treatment as well as normal group peritoneally irjected with saline. The expression of IGFBP7 and apoptosis in coloreetal tissue were detected by real-time PCR and TUNEL after 16 weeks. Results The numbers of colorectal cancer in model group( 1.2 ± 0.4 ),in Maixiansan group ( 0.70 ± 0.15 ),in Meisalazine group ( 0.60 ± 0.16 )were higher than in normal control (P < 0.05), but no differences were found among model,Maixiansan and Meisalazine groups(P>0.05).The apoptosis in colonic mucosa for Meixiansan(8.70±3.47) group and Mesalazine group were enhanced as compared with that in model group( 1.20 ±0.26 vs.0.38±0.11,P<0.05).The mRNA expression of IGFBP7 in colon for Meixiansan group were higher than those in model group,Meisalazine group, and normal control(50.5 ± 14.0 vs.18.0 ±3.9 and 39.3±11.4,46.4±6.0,P<0.05). Conclusions Maixiansan may resist the occurrence and development of ulcerative colitis-related colorectal cancer through upregulating IGFBP7 expression of colorectal tissue and promoting apoptosis of tumor cell.
ABSTRACT
[Objective] To observe the curative effects and the influence of activity of Superoxide dismutase(SOD) and contents of Maleic dialdehyde(MDA) of Qingchangyukui Gel in Rats with Experimental Ulcerative Colitis. [Method]Ulcerative Colitis model was established in Wistar rats by injecting trinitro-benzene-sulfonic acid (TNBS)into anus, and then treated with Qingchangyukui Gel and salisy-lazosulfa-pyridine(SASP) respectively. Isolating the colon and measuring the SOD and MDA after treating for two weeks. [Results]Colon mucous membrane in the model group had serious erosion, ulcer, damaging of glandular organ, high inflammation grade; activity of SOD in model group was significantly lower than in normal group and contents of MDA increased. The other groups, especially in Qingchangyukui Gel group, had different levels of inflammation repairing, ulcer healing, increasing of SOD and reducing of MDA. [Conclusion]TNBS can cause Colon mucous membrane inflammation and ulcer, reducing of SOD and increasing of MDA , which are the typical pathological change of Ulcerative Colitis; the Qingchangyukui Gel has the effects of increasing the activity of SOD and reducing contents of MDA, therefore it can effectively treat the UC rat.
ABSTRACT
Observe and study the morphological changes and effects on yoleperoxidase (MPO) of traditional Chinese medical complex Intestine-clearing Suppository (ICS) in preventing and treating ulcerative colitis (UC).Method.. Divide the experimental rats randomly into 6 groups, i.e., blank, salicylazosulfapyridine (SASP), model, high, middle and low dosage of ICS, except the blank group, makeother rats experimental UC model with albumen immunologic method of rabbit colon mucosa, further divide them randomly into 2 experimental groups, i. e. , prevention and treatment, afterwards at 2nd~21st day, give prevention group the medicine, 22nd~35th day, treatment group, then decapitate them and take the colon tissue, and observe it morphologically under optical microscope, measure active MPO, analyse statastic results. Result: Through general morphological observation ,we found there were serious congestion ,edema, mucosa erosion, many large ulcers in the rats of model group; less serious mucosa erosion and quantity of ulcers in SASP and low-dosage ICS groups: small ulcers, not serious mucosa erosion, low degree of congestion and edema in high-and middle-dosage ICS groups; the scores of inflammatory injury of ICS groups were markedly lower than that of model (P<0. 01), meanwhile, the preventive and reating effectiveness of high-and middle-dosage groups was better than that of SASP (P<0.05). Under the optical microscope, there were large area of mucosa coloboma in the rats of model group, the ulcer penetrated to muscular layer and serous coat; in SASP group and low-dosage ICS group, there were erosive mucosa and inflammatory cell infiltration, granulation on the base; in middle-and high-dosage groups, large part of colon mucosa recovered, granulation formed on the base, ulcers were obviously less than that in model group (P <<0.01); there had marked difference compared with that in SASP group (P<0.05). The MPO active values of high-and middle-dosage groups were markedly lower than that of model group (P < 0.01). Conclusion: Traditional Chinese complex ICS had good prevention and treatment to rats UC, itseffectiveness was positively correlated with dosage and better than that of SASP suppository.
