ABSTRACT

Objective:To investigate the consistency of indirect calorimetry, Harris-Benedict (H-B) formula method and rule of thumb in calculating the daily total energy expenditure (TDEE) of patients with acute pancreatitis (AP), and provide the basis for the formulation of reasonable nutritional support program.Methods:A total of 66 AP patients admitted to Henan Provincial People's Hospital from June 2020 to December 2020 were included and retrospectively analyzed, including 35 males and 31 females with an average age of 41.8 years. They were divided into mild acute pancreatitis (MAP) group ( n=20) and moderate to severe AP group ( n=46) according to severity. TDEE was calculated by indirect calorimetry, H-B formula and rule of thumb, and Pearson correlation analysis and Bland-Altman diagram were used to analyze the correlation and consistency of the three methods, respectively. Results:There were no statistical significances in the TDEE of MAP group calculated by indirect calorimetry, H-B formula method and rule of thumb (both P>0.05). TDEE calculated by indirect calorimetry were higher than that of calculated by H-B formula and thumb method both in moderate and severe AP groups, and the difference was statistically significant (both P<0.05). Pearson correlation analysis showed that H-B formula method was positively correlated with the TDEE calculated by indirect calorimetry, and the correlation was 0.61 ( P<0.05). Bland-Altman plot analysis showed the consistence of the two methods. TDEE calculated by rule of thumb was positively correlated with that calculated by indirect calorimetry, and the correlation was 0.75 ( P<0.05). The bland-Altman plot analysis showed a good consistency between the two methods. Conclusion:For MAP patients, TDEE can be calculated by the H-B formula method and rule of thumb. In moderate and severe AP patients, TDEE should be calculated by indirect calorimetry as the basis for the formulation of nutritional support plan.

ABSTRACT

Objective To explore the disease recognition and life demands in the elderly patients with chronic heart failure in order to help construction mode for the chronic patients in the community. Methods A qualitative, pilot study was conducted using semi structured interviews of 15 elderly chronic heart failure patients. Data were analyzed and verified using qualitative research techniques. Results Four themes were extracted including the difference about the diseases recognition;cognitive bias of the cardiac rehabilitation;want to improve the quality of life and want to have continue nursing. Conclusion Elderly chronic heart failure patients in the community have specially thought, strong relationships should build between hospital and community, strengthen the continue nursing, perfect the 7C intervene treatment system in order to improve the quality of life.

ABSTRACT

Objective To explore the Padua risk assessment model for the application of the prevention of VTE in medical inpatients. Methods A cross-sectional surgery research method was used. A total of 3 990 patients were interviewed of a hospital between June and August in 2017. The Padua assessment model was used to screen patients for VTE risk assessment. Results 3990 patients were interviewed, in which,a high VTE risk was found in 757 cases, accounting for 18.97%. The number and ratio of patients with high risk of VTE in Respiration department (158/32.18%), Endocrinology department (48/7.00%), Neurology department (91/20.31%), Nephrology department (102/18.58%), Gastroenterology epartment (41/8.63% ), Cardiology department (139/29.45% ), Hematology department (69/ 17.12% ) and Medical oncology department (109/23.40% ). Patients with high risk VTE in 11 risk factors were (246/32.5% ), (58/7.66% ), (531/70.15% ), (4/0.53% ), (92/12.15% ), (493/64.99% ), (74/9.78% ), (89/11.76% ), (131/17.31% ), (25/3.30% ), (72/9.51% ), two groups compare between age≥70 and <70 was statistically significant (χ2=952.20, P=0.00), two group compare between mobility and reduced mobility was statistically significant (χ2=1832.28, P=0.00). Conclusions Medical inpatients have a high risk of VTE, it is suggested that include VTE in clinical quality evaluation standard, establish an effective system of VTE risk management. The Padua risk assessment model has been easy to use. Some medical department should be special attention, and has been great significance to early screening and prevention.

ABSTRACT

Objective To describe the psychological experience of the patients with end-stage cardiopathy in China. Methods Phenomenology method was used in the study. Fifteen end stage cardiopathy patients received a non-structured interview. Results Four themes were extracted including heavy psychological burden about the disease and the selection of treatment options; complex psychological responses to families and relatives; wishing of social support;lose heart to personal future and prospects. Conclusions The patients on the end stage cardiopathy have complicated and intensive psychological responses. Medical staff should focus on the emotional of these patients, and provide effective strategies to promote patients'physical and psychological rehabilitation.

ABSTRACT

Objective:To explore the levels of serum adiponectin,insulin growth factor Ⅰ (IGF-Ⅰ) and prealbumin in preterm infants,and analyze their correlation with preterm EUGR infant.Methods:321 cases of preterm infants who were born in our hospital and transferred to neonatal department in 24 hours from January,2014 to January,2016 were enrolled in the present study.According to the weight at discharge,they were divided into the EUGR group and the non EUGR group,the growth and development as well as the levels of serum adiponectin,IGF-Ⅰ and PA in the two groups were compared.Results:The occurrence rate of EUGR was 55.76% (179/321) in the 321 cases ofpreterm infants;the weight of EUGR group was significantly lower than that of the non EUGR group at 42 th days and three months after birth(p<0.05).The SDS of both groups were all negative,and that of EUGR group was significantly lower than non EUGR group (p<0.05).The levels of serum adiponectin,IGF-Ⅰ and PA of both groups were significantly lower than the control group at seventh days of birth,and the PA level of EUGR group was significantly lower than non EUGR group (p<0.05).There was no significant difference in the serum adiponectin and PA levels between 7 days and 14 days in EUGR group (P > 0.05),however,the serum adiponectin and PA levels in the non EUGR group was obviously increased and significantly higher than that of the EUGR group (p < 0.05).The level of IGF-Ⅰ in both groups keep unchanged during two weeks (P>0.05).Conclusion:The lower levels of serum adiponectin,IGF-Ⅰ and PA were closely related to the EUGR of preterm infants,which could be used as biochemical indexes to early diagnosis of EUGR.

ABSTRACT

Objective To evaluate the quality of life and influencing factors in patients with breast cancer after breast-conserving surgery, so as to provide theoretical support for preoperative health education and postoperative rehabilitative intervention for these patients. Methods 120 breast conserving surgery (BSC) and 140 modified radical mastectomy (MRM) patients were assessed with Functional Assessment of Cancer Therapy-Breast (FACT-B V4.0) one week after operation. Life quality was evaluated between the two groups, and the influencing factors were also analyzed. Results Postoperative breast cancer patients after breast conserving surgery were with good breast complementary attention, lower emotion state. The other scores of quality of life had no statistical difference. Influencing factors on the quality of life of breast cancer patients were anxiety state, relationship of family members, position of surgery and character type. Conclusions It suggests that nursing specialists should make out corresponding, scientific and reasonable nursing intervention schemes based on the characteristics of different groups of people to ease psychological burden, and elevate the quality of life effectively for postoperative breast conserving surgery patients.

ABSTRACT

Objective To explore systematic intervention methods on life quality of heroin addicts during convalescence stage of drug detoxification period in order to supply evidence for effective nursing intervention. Methods 156 patients accorded with standard were selected by subjective sampling method and were grouped randomly. 78 patients in the control group were given routine nursing,78 patients in the intervention group received systematic intervention including psychological and behavioral intervention and the therapy of physical symptoms. QOLDA was used to evaluate the life quality of all patients. Results The scores of the intervention group were significantly higher than those of the control group, and the intervention group had a higher body mass index (BMI). Conclusions Systematic nursing intervention can perfect the physiological function, mental function and social function, reduce the withdrawal syndrome and adverse effect of heroin addicts, and improve their quality of life.

cases of acute promyelocytic leukemia (APL) were treated with drugs of all trans retinoic acid (ATRA) and Proplylene glucol mannurate sulfate (PGMS).Complete Remission rate was 90%. Because of using Proplylene glucol mannurate sulfate (PGMS), the rate of DIC and the rate of death were decreased.
