ABSTRACT

Objective To study and estimate the public dose constraint and the atmospheric radiation capacity of an isotope industrial park, and to provide a basis for management of radioactive effluent discharge in isotope industrial parks. Methods We collected the natural environment data, meteorological data, and population distribution and dietary data as well as the source terms of radioactive airborne effluents of an isotope industrial park. The public dose constraint value of the isotope industrial park was determined based on the natural environmental radiation hazard level. The atmospheric environmental radiation capacity of the industrial park was estimated by the all-pathways method. Results The public dose constraint value of the industrial park was 0.2 mSv/a. At the end of industrial park planning, the maximum individual effective dose to the public caused by airborne effluent discharge was 1.07×10−6 Sv/a, which accounted for 0.54% of the public dose constraint value. The proportions of discharges of airborne radionuclides to respective environmental radiation capacities ranged from 0.04% to 4.34%. Conclusion The determined public dose constraint value of the isotope industrial park can be used as the total radiation capacity of the regional atmospheric environment to constrain the discharge of radioactive effluents. The environmental radiation capacity of each radionuclide can be estimated by the all-pathways method considering the comprehensive impact of radionuclide discharges, which is suitable for the control and management of isotope production enterprises in industrial parks.

ABSTRACT

@#<b>Objective</b> To investigate and analyze the levels of γ dose rate and radon concentration at all sites in a large open-pit coal mine with radioactivity that had been mined for many years in Xinjiang, China, and to estimate the effective dose exposed to the personnel. <b>Methods</b> A portable γ dose-rate instrument FH40G was used for fixed-point monitoring of the mining area, and a continuous radon detector was used for 24 h continuous monitoring of radon concentration level in the site. The personnel exposure dose was estimated according to the measured γ dose rate and radon concentration. <b>Results</b> In this open-pit coal mine, the range of γ dose rate was 51.4-435.8 nGy/h; the mean 24 h radon concentration was 15-25 Bq/m<sup>3</sup>; the range of annual effective dose to the personnel was 0.29-1.29 mSv/a. <b>Conclusion</b> According to the results of the survey, radon concentration levels at all sites are low, and no remedial action is required. The personnel exposure dose at most of the sites is far below the standard requirements, and some protective measures need to be taken in some areas of the dump.

ABSTRACT

Because of the complex buildings, the wind flow and the pollution diffusion will be affected in the microscale such as urban neighborhood or the site of facility, the straight Gaussian plume model will not be available. Based on the Reynolds-Averaged Naiver-Stokes(RANS) method which was used to calculate the wind field with the Lagrange particle random walking model, the radioactive pollution diffusion model and microscale radiation environmental impact assessment software system were developed. This system can be used in the factory or city streets and other similar conditions for simulating radioactive pollutants diffusion behavior. The development of the system will provide technical tools for the microscale radiation environmental impact assessment in the future.

ABSTRACT

Objective To understand the distribution of associated radioactivity, though investigating and analyzing the level of environmental radioactivity in certain coal mines in Xinjiang, and estimate the effective dose received by the workers. Methods Taking coal mining activities in a certain area of Xinjiang Province as the object of the study, the environmental radioactivity level in the mining process was obtained through on-site measurement and sampling analysis, and the exposure dose of the workers in the coal mining process was evaluated accordingly. Results The radiation dose rate and radon concentration in the underground environment of most coal mines in this area are high. The content of radionuclides in gangue of each coal mine is less than 1 Bq/g, the content of radionuclides in most coal samples is less than 1 Bq/g, and the total α and β in the gusher water of each coal mine are lower than the discharge limits of the sewage discharge standards. The radon concentration level in coal mines is an important factor affecting the radiation exposure of workers. Conclusion Through the investigation of this project, the radioactive level of coal resources stored underground in a certain area is understood,which provides an objective basis for the management and planning of existing coal mines in this area, and provides technical support for the management of exploitation and utilization of mineral resources in China.

ABSTRACT

Objective To evaluate the changes of left ventricular structure and cardiac function in patients after video-assisted thoracic pneumonectomy by ultrasonic vector flow mapping (VFM) and quantitatively evaluate the diagnostic value of left ventricular flow energy loss(EL) for pneumonectomy patients with impaired cardiac function.Methods Thirty-six pneumonectomy patients were selected as case group and 30 health cases as control group.The echocardiographic parameters were acquired and left ventricular volume,left atrial volume and parameters of left diastolic founction were routinely measured.The EL of the left ventricularat early diastole and late diastole under the VFM mode were acquired.Results ①Left ventricular volume and left atrial volume comparsion:there was no significant difference in left ventricular end diastolic volume(LVEDV),left ventricular end systolic volume (LVESV),left ventricular ejective fraction(LVEF),and left atrial end diastolic volume(LAVd) between the two groups (all P > 0.05);left atrial end systolic volume(LAVs) and left atrial emptying volume(LAVe) in case group were mild lower than control group(P <0.05).②Comparsion of diastolic function parameters:E,A,E/e and HR of case group were significant higher than those in control group (all P <0.05),EDT and E/A ratio in case group were significant lower than those in control group (all P < 0.05),there was no significant difference of IVRT between two groups (P >0.05).③Comparison of EL at early diastole:only ELTB and ELAM had mild difference between case group and control group (all P <0.05).④Comparison of EL at late diastolic:EL of entirety and each segment of left ventricle of case group increased (all P < 0.05).Conclusions VFM can quantitatively evaluate the left ventricular flow characteristics after video assisted thoracic pneumonectomy,and the increase in EL may reflect the left ventricular diastolic dysfunction in patients after pneumonectomy.

ABSTRACT

BACKGROUND:Early rehabilitation after total knee arthroplasty (TKA) can help the rapid functional recovery of knee,but some patients are fear of moving and refuse to do exercising because of kinesiophobia,thereby delaying functional recovery.Foreign study has confirmed that patients with cognitive behavioral therapy (CBT) can effectively alleviate kinesiophobia,but no related studies have been reported in China.OBJECTIVE:To explore the effectiveness of CBT on patients with kinesiophobia after TKA.METHODS:108 patients with kinesiophobia after TKA were recruited from Department of Orthopedics,the First Affiliated Hospital of Zhengzhou University,China between March 2014 and January 2015,and were then equivalently randomized into experimental and control groups using random number table.Both groups were subjected to conventional treatment,while the experimental group received extra CBT.The scores of Tampa Scale of Kinesiophobia and American Knee Society were evaluated immediately,1,3,and 6 months after hospital discharge;the hospitalization time and total hospitalization expenditure were compared between two groups immediately after discharge.RESULTS AND CONCLUSION:The scores of Tampa Scale of Kinesiophobia in the experimental group were significantly lower than those in the control group at different time points postoperatively (P < 0.05).The scores of American Knee Society in the experimental group were significantly higher than those in the control group at 3 and 6 months after discharge and kept on rising (P < 0.01),but showed no significant difference at discharge and 1 month after discharge (P > 0.05).(2) The hospitalization time and total hospitalization expenditure in the experimental group were significantly lower than those in the control group (P < 0.01).(3) To conclude,CBT cannot only shorten the hospitalization time and total hospitalization expenditure,but also relieve kinesiophobia,which is conductive for functional recovery of the knee.

ABSTRACT

Objective Currently, there are few articles about kinesiophobia in patients with total knee arthroplasty (TKA) in China.This study aims to investigate the incidence of kinesiophobia and its influencing factors in TKA patient, and provide evidence for the intervention of kinesiophobia.Methods A total of 298 TKA patients from our hospital were investigated by general information questionnaire, Tampa Scale of Kinesiophobia (TSK), Knee Self-Efficacy Scales, Simplified Coping Style Questionnaire and Social Support Rating Scale.Single-factor analysis and logistic regression analysis were conducted to explore influencing factors.Results The score of TSK was 38.50±13.52, and 31.88% of TKA patients reported kinesiophobia.Logistic regression analysis showed that duration of pain (OR=5.546, 95%CI: 2.143-14.353), education level (OR=0.145, 95%CI: 0.067-0.314), self efficacy(OR=0.606, 95%CI: 0.470-0.780), positive response (OR=0.784, 95%CI: 0.683-0.900), objective support (OR=0.807, 95%CI: 0.691-0.943) and utilization of social support (OR=0.507, 95%CI: 0.461-0.705) were factors influencing kinesiophobia in TKA patients.Conclusion Attention should be paid to the kinesiophobia in TKA patients, especially those influencing factors including duration of pain, education level, and objective support.Health care providers should encourage early stage rehabilitation exercise to improve the postoperative knee function of TKA patients.

ABSTRACT

Objective To explore the effect of the combination of video feedback and telephone intervention on postdischarge rehabilitation in patients receiving artificial total hip replacement.Methods A total of 60 patients receiving artificial total hip replacement were divided into intervention group and control group with 30 cases each by random digits table method.The combination of video feedback and telephone intervention was conducted in intervention group,and conventional discharge guidance and telephone follow-up on a regular basis in control group.In intervention group,the patients or their family member were asked to record the video of the exercises and daily living when patients were at home according instructions and researchers assessed the condition of patient's joint and muscle function and provided individual education and instructions to the patients on the basis of the feedback video through telephone.Harris hip function rating scale and Functional Independence Measure (FIM) were investigated at discharge and 1,3,6 months after discharge.Results There were no significant differences in Harris hip function rating scale and FIM score at discharge between 2 groups,P>0.05.Harris hip function rating scale score were (72.70± 11.65),(83.11±5.12),(94.83±3.08) scores at 1,3,6 months after discharge in intervention group,and (63.20±14.96),(77.00±11.62),(87.50±7.94) scores in control group,there were significant differences,P<0.05 or <0.01.FIM score were (96.30±4.63),(108.76±3.28),(125.79±1.19) scores at 1,3,6 months after discharge in intervention group,and (78.23±5.09),(86.43±6.30),(96.54±5.35) scores in control group,there were significant differences,P<0.01.Conclusion The combination of video feedback and telephone intervention could help to promote the postdischarge hip function for patients receiving artificial total hip replacement.

ABSTRACT

[Objective] To observe the clinical therapeutic effect of Yanyan Decoction on treating yin deficiency of lung and kidney chronic pharyngitis . [Methods] 120 cases of patients were randomly divided into treatment group(60 cases) which treated with Yanyan Decoction and control group(60 cases) which treated with Jinguo drink.[Results] The total effective power was 80.00% in the treatment group and 63.33% in the control group .Two groups of patients total effective rates had difference of statistical meaning .[Conclusion] The effect of Yanyan Decoction in the treatment of yin deficiency of lung and kidney chronic pharyngitis is significant,it is worthy of clinical promotion.

ABSTRACT

Objective To investigate the value of dynamic contrast-enhanced magnetic resonance imaging ( DCE-MRI) in differentiating serological negative early stage rheumatoid arthritis (RA) from osteoarthritis (OA)in hand. Methods Plain MRI, enhanced MRI of whole hand and DCE-MRI of single slice were performed in 53 patients suspected of RA or OA and 18 healthy volunteers. Twenty-three of them were diagnosed as RA,including 18 cases of serological negative RA and 18 of them were diagnosed as OA after 3 to 6 months follow-up. The contrast rate,slope, thickening of synovial membrane were measured on DCE-MRI in three groups and the MRI findings were also were detected in both RA and OA groups. The contrast rate and slope of synovial membrane were compared among three groups using rank sum test. The thickening of synovial membrane were compared among three groups using variance analysis. MRI signs of RA and OA group were evaluated with rank sum test Results The dynamic contrast rate of synovial membrane in RA group,OA group and control group was(100.78±61.96)%,(40.44±15.43)% and (23.56±9.14)%,respectively.Individually,RA group to OA group,u=3.101,P=0.002;RA group to control group,u=4.669,P=0.000;OA group to control group,u=3.482,P=0.000.The slope of contrast curve of synodal membrane of RA group,OA group and control group was 72.50°±13.34°,45.39°±9.94°,14.56°±5.75°,respectively.Individually,RA group to OA group,u=8.002,P=0.000;RA group to control group,u=17.102,P=0.000;OA group to control group,u=9.100,P=0.000.The synovial membrane thickening of RA group,OA group and control group was(3.3±0.5),(2.8±0.7)and (1.4±0.6)mm,respectively.Individually,RA group to OA group,q=2.622,P=0.011;RA group to control group,q=9.583,P=0.000;OA group to control group,q=6.961,P=0.000.Conclusion The quantitative index of DCE-MRI,including contrast rate and synovial membrane thickening may provide useful information for differentiating OA from semlogical negative early stage RA.The contrast rate and thickening of synovial membrane in RA group are higher than those in OA group.Many signs of MRI Can help differentiate OA from RA.

ABSTRACT

Objective To evaluate the clinical effects of interfered therapy of peptic ulcer with lip. Methods 184 patients were randomly divided into two groups,only durgs were given for conventional treatment group:pan- toprazole and teprenone; the same drugs, the type of life and individual diets were given for interfered treatment group. Therapy time lasted for 6 weeks in two groups. Radical eradication of Hp: pantoprazole plus clarithromycinand amoxycilLin. Results The time. of complete alleviation of clinical symptoms: most of patients in interfered treat- ment group alleviated after one week, half past patients in conventional treatment group alleviated after one week; there were significant difference in alleviation rate of clinical symptoms between two groups( P < 0.01 ). Healing of ulcer:after the end of treatment, all patients underwent endoscopy, most of ulcers healed in interfered treatment group, but 90.22% of ulcers healed in conventional treatment group, there were significant difference in he.sling rate of ulcer between two groups(P< 0.05). Eradicating of Hp: the eradicating of Hp was similar in two groups,there were no significant difference between the interfered treatment group and the conventional treatment group (P > 0.05). Conclusion Through the interfered treatment(drugs combining the type of life and the habit of diet), we can alleviate clinical symptoms and improve the effects of healing of ulcer.

Results Many students by PBL teaching accept PBL teaching Method,and improve their self-study ability,understanding ability,analyzing ability and so on. Conclusion PBL teaching method is much better than LBL teaching method,and can improve the quality of pathogen and immunology teaching.
