ABSTRACT

Objective:To investigate the quality regionalization and environmental impact factors of Stephaniae Tetrandrae Radix based on main active ingredients,and provide a reference for the determination of high-quality production areas and the dominant environmental factors affecting the content of Stephaniae Tetrandrae Radix. Method:Partial least squares regression analysis (PLS) and principal component analysis (PCA) were used to study the quality regionalization and environmental impact factors based on the main active ingredients of tetrandrine and fangchinoline, and investigate the environmental factors of the producing areas. Result:The content of fangchinoline was positively correlated with soil pH and annual average temperature,negatively correlated with latitude. The content of tetrandrine was positively correlated with soil pH,negatively correlated with annual rainfall and longitude. The total content was positively correlated with soil pH and annual average temperature,but negatively correlated with annual rainfall,latitude and longitude. Principal component analysis showed that the 50 production areas could be divided into four groups of quality formation. The groups with the highest scores were Shixing county in Guangdong,Shexian county in Anhui,Songxi county in Fujian,Nanxiong city in Guangdong and Xiangxiang city in Hunan,all of which were best areas for accumulation of the two main active ingredients. Conclusion:Soil pH,annual average temperature,annual rainfall,latitude and longitude are the main environmental factors affecting the main active ingredients of Stephaniae Tetrandrae Radix. The best areas for accumulation of tetrandrine and fangchinoline are Shixing county in Guangdong,Shexian county in Anhui,Songxi county in Fujian,Nanxiong city in Guangdong and Xiangxiang city in Hunan.

ABSTRACT

Objective::Sixty-nine germplasm samples of Picria felterrae collected from the main producing areas in Guangxi were subject to genetic diversity and genetic relationship analyses using the simple seguence repeat(SSR) molecular marker technology and good germplasm genes associated with the content of picfeltarraenins were screened so as to provide references for germplasm resource evaluation, phylogenetic analysis, and molecular mark assisted breeding of that species. Method::20 pairs of randomly selected primers were amplified based on the transcriptome sequencing technology. The genetic diversity of and genetic relationship between the 69 samples were analyzed using the genetic polymorphic information for each marker locus, and one-variable linear regression and multiple stepwise regression analyses were performed to screen molecular markers associated with the content of picfeltarraenins. Result::The amplification using the 20 pairs of SSR primers produced 76 alleles, 3.8 alleles for each locus on average, higher than effective alleles (1.969 2), and the rare allele rate was 38.2%, suggesting that the alleles distributed unequally. The polymorphism rates of alleles varied between 0-59%, with an average of 38.24%, showing a great difference among loci. The polymorphic information content (PIC) varied between 0-0.621 1, with an average of 0.378 0.Shannon polymorphic information index varied between 0-1.240 1, with an average of 0.759.Nei's gene diversity index (Nei) varied between 0-0.682 3, with an average of 0.440 9.P21 had the highest level accompanied with the lowest P7 for the above three indexes, and significant genetic diversity differences were identified among the loci. For all loci, the mean observed heterozygosity was 0.382 4, lower than the average expected heterozygosity of 0.442 5, suggesting the loss of heterozygosity, the average genetic differentiation coefficient (Fst) was 0.365 9 and the average gene flow Nm was 0.433 2, suggesting a high genetic divergence and a low gene flow. The results of one-variable linear regression and multiple stepwise regression showed that there were 5 loci related to each of the picfeltarraenin IA and IB, and only 1 loci was associated with the content of both. Conclusion::There were significant differences in the genetic diversity of 20 SSR marker sites, and the 69 germplasm samples were greatly genetically differentiated and had low gene flow. From the selected 20 SSR markers 9 marker loci associated with the content of picfeltarraenin IA and IB were selected. The results can be used as a reference for phylogenetic analysis and selective breeding of Picria felterrae.

ABSTRACT

Objective@#To explore the CT imaging features of artificial stone-agglomerated quartz associated silicosis.@*Methods@#A total of 37 cases confirmed with artificial stone-agglomerated quartz associated silicosis from December 2016 to August 2018 and 38 cases confirmed with traditional classical silicosis at the same period were retrospectively reviewed.The clinical characteristics (including gender, age and working age)and the imaging features (including the nodule features, ground glass opacity, merging features, consolidation, secondary changes, etc.) of the two groups were recorded. The variables including the imaging findings between the two groups were analyzed by Fisher exact test.@*Results@#Of all the cases, there were bilateral diffuse small nodules which distributed with upper-zone predominance. Small nodules can merge together and form mass shadow. The complications such as lymphadenopathy, calcification, emphysema/pneumatocele, pulmonary interstitial fibrosis could also be found. Among the various imaging features, the presence of ground glass opacity, small nodules merging together in the subpleural zones, consolidation, pulmonary interstitial fibrosis, pneumothorax were found in 28,16,18,17,5 cases, respectively in artificial stone-agglomerated quartz associated silicosis group, and 2,4,4,4,0 cases, respectively in the traditional classical silicosis group. The percentages of these above signs were higher in the artificial stone-agglomerated quartz associated silicosis group than the traditional classical silicosis group, and the differences were statistically significant (P<0.01). The artificial stone-agglomerated quartz associated silicosis group had 3 cases with mediastinal lymph node enlargement with calcification, while the traditional classical silicosis group had 12 cases. This sign showed a lower significantly incidence in the artificial stone-agglomerated quartz associated silicosis group than the traditional classical silicosis group (P<0.01).@*Conclusion@#The characteristic changes of CT imaging features in artificial stone-agglomerated quartz associated silicosis are small nodules with the background of ground glass opacity, the nodules merging together under subpleural zones, consolidation, mediastinal lymph node enlargement with less calcification and complicated with pulmonary interstitial fibrosis and pneumothorax.

ABSTRACT

OBJECTIVE: To report the clinical manifestations,chest imaging,pathological changes,diagnosis and treatment on one case of hard metal lung disease( HMLD).METHODS: The occupational history,clinical manifestations,chest computed tomography and lung histopathological examinations of a case with pathologically confirmed HMLD were summarized.RESULTS: The patient began to cough and have dyspnea on exertion after 7 years of hard metal dust exposure of cobalt and tungsten, and pulmonary function test showed a restrictive ventilation dysfunction.Chest computed tomography showed bilateral areas of diffuse lesions,including ground-glass attenuation,areas of consolidation,diffuse small nodules, extensive reticular opacities and traction bronchiectasis.The lung histopathological changes include macrophage interstitial pneumonia,which is macrophages and a large number of multinucleated giant cells in the alveolar space.CONCLUSION: A clear history of hard metal dust exposure and the pathological examination of lung tissue are helpful for the diagnosis of HMLD.

ABSTRACT

·AIM: To research the morphological and fuctional change of human retinal pigment epithelium ( RPE ) induced by mobile phone screen light in vitro and the mRNA express of apoptotic inhibitor B-cell lymphoma (Bcl-2), Bcl2-Associated X(Bax) and apoptotic actuators Caspase-3 in different groups. ·METHODS: Cultured human RPE cells were divided into different group in random. According to the time of exposing, the four groups were 3h group, 6h group, 12h group and no light-exposed group. Cells of different groups were exposed to the opening mobile phone that was kept high-light brightness and playing on the colorful picture on silent mode. The morphological and functional change of RPE was quantified after exposed by HE staining, TUNEL staining, MTT, and use the technology of polymerase chain reaction ( PCR) to detect the mRNA express of apoptotic inhibitor Bcl-2, Bax and apoptotic actuators Caspase-3 in different groups. ·RESULTS: The different was not statistically significant (P>0. 05) in the way of HE staining, TUNEL staining and MTT. The expression of Bcl - 2, Bax and Capase - 3 intracellular was no difference statistically significant in 3h, 6h group to no exposure group on the way of PCR (P>0. 05). But as the time of exposed increased to 12h, the expression of apoptotic inhibitor Bcl-2 decreased, at the same time the expression of apoptotic actuators Bax and Caspase - 3 increased, and the different was statistically significant (P<0. 05). ·CONCLUSION: The human RPE would be injured under the constantly high- light brightness of mobile phone screen as the man-made light source.

ABSTRACT

OBJECTIVE:To study risk factors for carbapenem-resistant Acinetobacter baumannii(CRAB)infection,and to provide reference for its clinical prevention. METHODS:In retrospective study,302 A. baumannii(AB)infection patients were collected from our hospital during Dec. 2012 to Jun. 2017. According to the results of drug sensitivity test,those patients were divided into CRAB group(116 cases)and non-CRAB group(186 cases). Risk factors for CRAB infection were analyzed by using univariate analysis. Multivariate Logistic regression analysis was performed for variables with significant difference between 2 groups. RESULTS:Univariate analysis showed that the factors of significant difference in 2 groups including patients suffering from septic shock(P=0.003),sepsis(P=0.000),combined with other infection(P=0.006),diabetes(P=0.029),malignant tumors(P=0.036),patients suffering from infection of other site except for pulmonary infection,intraabdominal infection and skin infection(P=0.009)before AB isolation,patients given carbapenems(P=0.002)and antifungal drugs 28 d before AB isolation(P=0.002). Multivariate Logistic regression analysis showed that the factors of significant difference in 2 groups including patients suffering from sepsis(P=0.033)or diabetes(P=0.011)before AB isolation. CONCLUSIONS:Independent risk factors for CRAB infection include patients suffer from sepsis or diabetes before AB isolation.

ABSTRACT

<p><b>OBJECTIVE</b>To systematically evaluate the efficacy and safety of acupuncture on diarrhea-predominant irritable bowel syndrome (IBS) in the past five years.</p><p><b>METHODS</b>Online databases, including CNKI, VIP, WANFANG, PubMed, CBM, ScienceDirect OnSite, Cochrane Library, etc. were searched for randomized controlled trials (RCTs) of acupuncture for diarrhea-predominant IBS. Retrieval time was from January of 2011 to January of 2016. According to modified Jadad standard, the bias risk and quality assessment of each RCT included were evaluated by two researchers. RevMan 5.3 software was adopted for the meta-analysis.</p><p><b>RESULTS</b>Totally 17 RCTs were included with 1 333 patients. The result of meta-analysis indicated the total effective rate of clinical symptoms improvement in the acupuncture group or acupuncture combined with western medicine group was superior to that in the western medicine group (=3.92, 95%:2.83~5.43,<0.01), and the funnel plot was basically symmetry. The result of meta-analysis showed 3-month recurrence rate:=0.22, 95%:0.12~0.41 (<0.01), indicating the recurrence rate in the acupuncture group was lower than that in the western medicine group in three month.</p><p><b>CONCLUSIONS</b>Acupuncture for diarrhea-predominant irritable bowel syndrome is superior to conventional treatment of western medication, which can improve the clinical symptoms and reduce the recurrence rate of patients.</p>

ABSTRACT

@#AIM:To investigate the effect of small incision non-phacoemulsification cataract surgery on corneal endothelial cell.METHODS:Totally 140 cataract patients (152 eyes) treated in our hospital from August 2013 to March 2016 were selected.All of the patients underwent small incision non-phacoemulsification cataract surgery.Before surgery and at 1wk, 1mo and 3mo after surgery, we measured the corneal endothelium of upper cornea, the central cornea and the lower part of cornea.The proportion and density of hexagonal cells, loss rate and cell area in corneal endothelial cells were calculated and compared.RESULTS:Postoperatively 36 eyes had posterior elastic layer folds, aqueous flare, which disappeared after treatment.After surgery, corneal incision was found to have mild corneal edema.No other complications were found.A total of 128 patients (137 eyes) were followed up for a total of up to 3mo, with a loss of follow-up in 12 patients (15 eyes).After surgery, corneal endothelial cell density of the upper part of the cornea, the central cornea and the lower part of cornea, was statistically significantly different with each other (F=3.526, P=0.031), the difference in different time points was statistically significant (F=4.111, P=0.018).The postoperative corneal endothelial cell loss rate was statistically significant different between the three groups (F=3.526, P=0.031), at different time pionts (F=4.111, P=0.018).There was statistical significance of percentage of hexagonal cells between the three groups (F=4.348, P=0.014), the difference was significantly different between different time points (F=4.964, P=0.008).There was statistical significance of corneal endothelial cell area between the three groups (F=3.180, P=0.044), the difference was significantly different between the time points (F=3.511, P=0.032).CONCLUSION:After small incision non-phacoemulsification cataract surgery, corneal endothelial cell density decreased, hexagonal cell proportion decreased, corneal cell area expanded.

ABSTRACT

Objective To observe the clinical efficacy of balance acupuncture predominantly by puncturing Jiaji points (EX-B2) from C4 to T1and from T12 to L1in treating hemiplegic balance disturbance after cerebral stroke. Method A total of 180 hemiplegia patients were randomized into 3 groups, 60 cases in balance acupuncture group, 60 cases in ordinary acupuncture group, and 60 cases in basic control group. After 1-month treatment and 3 months after the treatment, the motor function (Fugl-Meyer Assessment, FMA), balance function (Berg Balance Scale, BBS; Timed Up and Go Test, TUGT), and activities of daily living (ADL) (Barthel Index, BI) were observed.Result After 1-month treatment and 3 months after the treatment, limb function, balance ability and ADL were significantly different from those before the treatment in balance acupuncture group (P<0.01); after 1-month treatment, limb function in balance acupuncture group was significantly different from that in basic control group (P<0.05), and the differences were more statistically significant in comparing the rest indexes between the two groups (P<0.01); there were significant differences between ordinary acupuncture group and basic control group (P<0.05). Three months after the treatment, there was a significant difference in comparing balance function between balance acupuncture group and basic control group (P<0.05), and the differences were more statistically significant in comparing the rest indexes between the two groups (P<0.01), there was no significant difference in comparing balance function between ordinary acupuncture group and basic control group (P>0.05).Conclusion In combination with basic treatment, balance acupuncture works better than ordinary acupuncture and basic control in improving limb function, ADL and balance function of hemiplegia patients.

ABSTRACT

Objective To evaluate the effects of a self-designed multi-function anti-reflux drainage connector on preventing catheter-associated urinary infection in patients with long-term indwelling catheters.Methods A total of 100 elderly males with indwelling catheters from 10 nursing homes in our city were selected and randomly divided into the control group (n=50) and the experimental group (n=50) from January 2013 to December 2015.The patients in the control group were indwelled with Foley catheters and connected with an ordinary disposable drainage bags;the patients in the experimental group were indwelled the same catheters and connected to disposable drainage bags with an multi-function anti-reflux drainage connector.Patients' urine in catheters and drainage bags from two groups were collected for urine culture on 7th,14th,21st,28th days.The cases of catheter plugging on the 7th,14th,21st,28th days and the cases of catheter encrustation on 28th day in two groups were recorded.Results The cases of bacteriuria on the 7th,14th,21st,28th days in the experimental group were significantly less than those in the control group (P<0.05),and were also significantly less than those in the drainage bags in the same group (P<0.05).The cases of catheter plugging on the 7th,14th,21st,28th days were not significantly different between two groups(P>0.05).The cases of catheter encrustation on the 28th day in the experimental group were significantly less than those in the control group (P<0.05).Conclusion Multi-function anti-reflux drainage connector can safely and effectively prevent catheter-associated urinary infection,reducing bacteria ascending with reflux of urine as well as catheter encrustation.

ABSTRACT

<p><b>OBJECTIVE</b>Some research has shown that ambroxol can alleviate lung injury induced by cardiopulmonary bypass (CPB). However, whether ambroxol has protective effects against CPB-induced renal injury remains unknown. This study investigated the effect of ambroxol on renal function in children undergoing CPB.</p><p><b>METHODS</b>Forty children at ages of 3-8 years with cardiac function class I or II and weighing 12-25 kg, underwent repair of ventricular septal defect (VSD) under CPB. They were randomly divided into two groups (n=20 each):control and ambroxol-treated. The children in the ambroxol-treated group received ambroxol of 4.5 mg/kg in 10 mL normal saline by intravenous injection after skin incision. The control group received 10 mL of normal saline instead. Serum concentrations of urea nitrogen (BUN), beta(2)-microglobulin (beta(2)-MG) and creatinine (Cr) and urinary beta(2)-MG, retinol-binding-protein (RBP) and N-acetyl-beta-D-glucosaminidase (NAG) were measured before operation, and 2, 12, 24 and 48 hrs after operation.</p><p><b>RESULTS</b>Serum Cr and urinary beta(2)-MG concentrations 2 hrs after operation, serum beta(2)-MG concentration 2 and 12 hrs after operation, urinary RBP concentration 2, 12, 24 and 48 hrs after operation, and urinary NAG concentration 2, 12 and 24 hrs after operation in the control and the ambroxol-treated groups increased significantly as compared with their baseline values (before operation) (P<0.05). Serum Cr concentration 2 hrs after operation, serum beta(2)-MG and urinary beta(2)-MG concentrations 2 and 12 hrs after operation, urinary RBP concentration 2, 12, 24 and 48 hrs after operation, and urinary NAG concentration 12 and 24 hrs after operation in the ambroxol-treated group were significantly lower than those in the control group (P<0.05).</p><p><b>CONCLUSIONS</b>Ambroxol administration before CPB is effective in reducing CPB-induced renal injury in children undergoing repair of VSD. Further research is required to understand the mechanism.</p>

ABSTRACT

OBJECTIVE:To discuss the role of clinical pharmacists participating in successful treatment of critically ill children with Influenza A (H1N1). METHODS:Considering patients' conditions,clinical pharmacists participated in drug treatment in respect of pharmacokinetics,pharmacodynamics,drug interactions and adverse drug reactions and so on. RESULTS:Combining with advantages of grade three class A comprehensive hospital,medical treatments had been achieved by multidisciplinary corporation. The symptoms of children under critically care disappeared completely and all test indicators returned normal so that critically ill children were cured. CONCLUSION:Clinical pharmacist is an important component of the medical team. Therefore,clinical pharmacists participating in clinical drug treatment can improve clinical efficacy and safety of drug treatment.
