ABSTRACT

AIM:To explore the changes and influencing factors of macular retinal blood flow density in patients with proliferative diabetic retinopathy(PDR)after vitrectomy, and establish a predictive model.METHODS: The clinical data of 173 patients with 173 eyes in PDR who underwent vitrectomy in our hospital from June 2019 to June 2021 were analyzed retrospectively. According to the changes of macular retinal blood flow density after operation, the patients were divided into normal group(118 cases with 118 eyes)and descending group(55 cases with 55 eyes). The general data, preoperative laboratory examination index and intraoperative condition of the two groups were compared. Multivariate Logistic regression was used to analyze the factors affecting the changes of retinal blood flow density in macular area after operation. Construct the line chart prediction model and evaluate its prediction efficiency.RESULTS: The course of diabetes, proportion of patients with complicated maculopathy, serum TC, TG, LDL-C, HbA1c, vascular occlusion and hyperplastic retinal traction in the decreasing group were significantly higher than those in the normal group, while HDL-C was significantly lower than that in the normal group(P<0.05). Multivariate Logistic regression analysis showed that course of diabetes > 17a(OR=4.526), complicated maculopathy(OR=4.983), HbA1c > 6.25%(OR=4.283), vascular occlusion(OR=5.216)and hyperplastic retinal traction(OR=4.765)were all risk factors of the decrease of macular retinal blood flow density after operation(P<0.05). The line chart prediction model has good differentiation and accuracy, and has high prediction value. CONCLUSION: Course of diabetes, complicated maculopathy, HbA1c, vascular occlusion and proliferative retinal traction are all related to the decrease of macular retinal blood flow density after vitrectomy with PDR. Understanding the risk factors is beneficial to surgical decision-making.

ABSTRACT

ObjectiveTo investigate the effects of exogenous Fusarium oxysporum and Trichoderma viride on the diversity of soil fungal community and carbon metabolic function of cultivated Panax ginseng. MethodIllumina HiSeq 2500 high-throughput sequencing combined with Biolog-ECO was used to analyze the species diversity and functional diversity of soil fungal communities in P. ginseng soil under different exogenous treatments. ResultThe results of high-throughput sequencing showed that the number and species of microorganisms in the soil were significantly changed after exogenous microorganisms were added. The soil fungi with relative abundance greater than 1% included Mortierella sp.,Fusarium sp.,Humicola sp.,and Simplicillium sp. Mortierella sp. in each treatment group significantly increased. Humicola sp. and Simplicillium sp. could be induced to increase by exogenous addition of F. oxysporum, while T. viride at a high concentration could significantly inhibit the growth of F. oxysporum. As revealed by Biolog and principal component analysis (PCA),the average well color development (AWCD) in the high-dose T. viride group (MG) was significantly higher than that in the control group (QS)and the low-dose F. oxysporum group(LD). The utilization abilities for amino acids, carboxylic acids, polymers, and amines were enhanced in the MG group, but the microbial metabolic activity was reduced in the high-dose F. oxysporum group (LG). There was no significant increase in the utilization of phenolic acids by soil microorganisms in both groups. ConclusionExogenous addition of F. oxysporum can lead to the growth and reproduction of other pathogenic fungi. Exogenous addition of T. viride can enhance the soil fungal community structure and metabolic diversity, inhibit the proliferation of F. oxysporum, and improve the soil microbial environment of cultivated P. ginseng.

ABSTRACT

Objective@#To explore the relationship and potential mechanism between peer victimization and early onset of breast development in girls, and to provide new strategies for precocious puberty prevention.@*Methods@#A total of 950 girls aged (9.0±1.2) years participated in this survey in Ma anshan City in China. Revised Bully/Victim Questionnaire (BVQ) was used. Each girl was assessed on breast Tanner stage by one female pediatric endocrinologist. All the girls collected morning awakening saliva sample with Salivette at home. Body fat percentage (%BF) was derived from Bioelectrical Impedance Analyzer (BIA) measurements. The association between peer bullying and early breast development was analyzed by stepwise Logistic regression. The mediation coefficient was estimated using the Logistic regression.@*Results@#Totally 23.2% (220/950) of all the girls reported being victimized at school. More girls reported peer victimization had early breast onset (24.1%,53) compared with girls with no victimization experience ( 13.7 %,100). Logistic regression analyses indicated that peer victimization was associated with early onset of breast development ( OR=1.74, 95%CI =1.16-2.59) after body fat and awakening cortisol adjusted. Mediation analysis showed that high cortisol and high fat both had indirect effects in the association between peer victimization and early onset of breast development.Mediating analysis showed that both high cortisol level and high body fat rate had indirect effects between peer bullying and early breast development, and the mediating effect values were 0.37 and 0.45,respectively.@*Conclusion@#Peer victimization significantly correlated with early onset of breast development among girls, which might be mediated through HPA axis hyperactivity and high adiposity.

ABSTRACT

According to human carboxylesterase 2(hCE2) inhibitors reported in the literature, the pharmacophore model of hCE2 inhibitors was developed using HipHop module in Discovery Studio 2016. The optimized pharmacophore model, which was validated by test set, contained two hydrophobic, one hydrogen bond acceptor, and one aromatic ring features. Using the pharmacophore model established, 5 potential hCE2 inhibitors(CS-1,CS-2,CS-3,CS-6 and CS-8) were screened from 20 compounds isolated from the roots of Paeonia lactiflora, which were further confirmed in vitro, with the IC_(50) values of 5.04, 5.21, 5.95, 6.64 and 7.94 μmol·L~(-1), respectively. The results demonstrated that the pharmacophore model exerted excellent forecasting ability with high precision, which could be applied to screen novel hCE2 inhibitors from Chinese medicinal materials.

ABSTRACT

Median arcuate ligament syndrome(MALS)is compression of the celiac trunk by the median arcuate ligament.Median arcuate ligament release is the corner stone for the surgical treatment of MALS.Open surgery, laparoscopic surgery, and robot-assisted surgery have been developed, among which laparoscopic surgery has been proposed as the preferred approach in view of its minimal trauma and short hospital stay.Auxiliary celiac plexus neurolysis could further alleviate the patient's discomfort. Moreover, vascular reconstitution is of vital importance in the case of persistent stenosis in the celiac artery despite of median arcuate ligament decompression.Vascular reconstruction has satisfactory long-term patency rate, while endovascular treatment is less invasive. This article aims to summarize the consensuses and advances and shed light on the surgical treatment of MALS.

ABSTRACT

Guided by cell-based anti-anaphylactic assay, eighteen cage-like monoterpenoid glycosides (1-18) were obtained from the bioactive fraction of P. lactiflora extract. Among these, compounds 1, 5, 6, 11, 12, 15, and 17 significantly reduced the release rate of β-HEX and HIS without or with less cytotoxicity. Furthermore, the most potent inhibitor benzoylpaeoniflorin (5) was selected as the prioritized compound for the study of action of mechanism, and its anti-anaphylactic activity was medicated by dual-inhibiting HDC and MAPK signal pathway. Moreover, molecular docking simulation explained that benzoylpaeoniflorin (5) blocked the conversion of L-histidine to HIS by occupying the HDC active site. Finally, in vivo on PCA using BALB/c mice, benzoylpaeoniflorin (5) suppressed the IgE-mediated PCA reaction in antigen-challenged mice. These findings indicated that cage-like monoterpenoid glycosides, especially benzoylpaeoniflorin (5), mainly contribute to the anti-anaphylactic activity of P. lactiflora by dual-inhibiting HDC and MAPK signal pathway. Therefore, benzoylpaeoniflorin (5) may be considered as a novel drug candidate for the treatment of anaphylactic diseases.

ABSTRACT

In recent years, with the rapid development of traditional Chinese medicine industry in China, the efficacy of Chinese medicinals in treating disease and maintaining health has been increasingly recognized. Tripterygium wilfordii, a Chinese medicinal for expelling wind, dredging collaterals, removing dampness, and relieving pain, is commonly used for treating acute and chronic glomerulonephritis, rheumatoid arthritis, and other diseases. However, the frequent occurrence of adverse reactions has limited its wide application in clinical practice. The existing studies have gradually confirmed that T. wilfordii and its active ingredients exert the bidirectional effects on kidney function. This paper reviewed the related clinical applications and articles published in the past decades and summarized the material basis for its bidirectional effects and the specific action mechanisms in renal protection and renal damage. It was found that the main active ingredients in T. wilfordii were tripterygium glycosides and triptolide, which exerted the protective or toxic and side effects on kidney by regulating immunity, influencing mitogen-activated protein kinases (MAPK) pathway, and changing the expression and function of renal transporters. Besides, the roles of administration time, dosage, and body status in the exertion of protective or toxic and side effects by T. wilfordii were also discussed. The review aimed to provide new ideas for the research on the treatment of kidney diseases with T. wilfordii and its safety application.

ABSTRACT

Objective:To investigate the renoprotective effects that Sanjiao Qushi prescription ameliorates cationic bovine serum albumin (C-BSA) induced membranous nephropathy(MN) in mice model and its influence on nuclear factor erythroid-2-related factor-2(Nrf2)/heme oxygenase-1(HO-1) signaling pathway. Method:Sixty female BALB/c mice were randomly divided into the normal group(n=10) and the model group(n=50). The mice in the model group received C-BSA injection via tail vein (6.5 mg·kg-1). The mice that were successfully modeled were randomized into the model group, the low dose Sanjiao Qushi prescription group(3.71 g·kg-1), the high dose Sanjiao Qushi prescription group(7.42 g·kg-1) and benazepril hydrochloride group(1.3 mg·kg-1). And they were administered with the corresponding medicine by gavage once a day for four consecutive weeks. 24 hour-urine protein quantitation were performed before C-BSA injection and after C-BSA injection as well as the medicine gavage. When the treatment was finished, all of the mice were sacrificed and the biochemical indicators such as serum creatinine(SCr), blood urea nitrogen(BUN), triglyceride(TG), total cholesterol(TC), total protein(TP) and albumin(Alb) were measured. And the renal pathological morphology changes were observed by light microscope with hematoxylineosin(HE), Masson and periodic acid-silver metheramine(PASM) staining. The deposition of immunoglobulin G(IgG) in the glomerulus was detected by fluorescence microscope. The expression of reactive oxygen species (ROS) of kidney was detected by fluorescence immunoassay. The protein expression levels of Nrf2 in cell nucleus and cytoplasm and the downstream protein factors HO-1 and NADH quinone acceptor oxidoreductase 1(NQO1) were detected by Western blot. Result:Compare to normal group, the levels of 24 hour-urine protein quantitation, TG and TC significantly increased in model group(P<0.01), while TP and Alb levels significantly decreased(P<0.01). The model group exhibited enlarged volume of glomerular, significantly thickened glomerular basement membrane(GBM), fuchsinophilic protein deposition and spike formation through light microscope. Immunofluorescence staining for the model group exhibited granular deposition of IgG along the capillary wall. The expression of ROS in kidney significantly increased(P<0.01). The protein expression levels of Nrf2 in cell nucleus significantly increased(P<0.01), while Nrf2 in cytoplasm significantly decreased(P<0.01).The protein expression levels of HO-1 and NQO1 significantly increased(P<0.01). Compared to model group, the levels of 24 hour-urine protein quantitation, TG and TC significantly decreased in each treated group(P<0.01), TP and Alb levels significantly increased(P<0.05,P<0.01). The pathological damages alleviated obviously. The expression of ROS in kidney significantly decreased(P<0.01). The protein expression levels of Nrf2 in cell nucleus significantly decreased(P<0.01), while Nrf2 in cytoplasm significantly increased(P<0.05,P<0.01). The protein expression levels of HO-1 and NQO1 significantly increased(P<0.01). Conclusion:Sanjiao Qushi prescription worked on MN mice possibly by regulating related proteins in the Nrf2/HO-1 signaling pathway and relieving oxidative stress, thus decreasing 24 hour-urine protein and blood lipid, increasing serum protein, and alleviating the pathological damages to protect renal function and delay progress of the disease.

ABSTRACT

Objective To analyze the data of road traffic accidents, deaths and injuries in 31 provinces and municipalities in China from 2010 to 2017, and to predict that the number of deaths and injuries caused by road accidents set in the sustainable development goals (SDGs) will be halved by 2020 compared with the target set in 2015. Methods Describing and analyzing the current situation of road traffic accidents in 31 provinces and municipalities in China, and using the trend extrapolation method to predict whether the SDGs target can be achieved by 2020. Results The numbers of traffic accidents, deaths and injuries in Guangdong, Jiangsu, Zhejiang and Shandong Provinces showed a downward trend from 2010 to 2017, but still ranked the front in China. In 2017, Beijing had the highest death rate followed by Guizhou as the second, and Guizhou had the highest injury rate followed by Tianjin as the second. The projected results showed that the numbers of deaths and injuries caused by road traffic accidents in China would be increased by 17% and 1.3% respectively in 2020 indicating that SDGs target could not be met. Among the 31 provinces/municipalities, it was predicted that the numbers of deaths and injuries in Hunan would be reduced to 50.7% and 65.3% in 2015, respectively by 2020, and the target could be achieved; the number of injuries in Shanghai was decreased by 83.3%, but the deaths was only decreased by 34.5%, and there was still a gap with the target; the rest 29 provinces/municipalities could not meet SDGs target. It was expected that the deaths in 11 provinces and municipalities would increase by 2020, with Beijing, Jilin, Jiangxi, Hubei and Guizhou Provinces showing the most significant growth. The number of injured people in 8 provinces and municipalities shows an increasing trend, with Jilin, Jiangxi, Hubei and Guizhou provinces showing significant growth. Conclusions Except for Hunan Province, it was difficult for the whole country and the rest provinces and municipalities to reach the SDGs target. According to the forecast results and the actual situation, a targeted scientific prevention and control strategy can be formulated. The measures taken by Hunan and Shanghai in traffic accident prevention and control were effective and worth learning.

ABSTRACT

Purpurolides D-F (1-3), three new polyoxygenated bergamotanes bearing a 6/4/5/5 tetracyclic ring system, were isolated from the endophytic fungus Penicillium purpurogenum IMM 003. Their structures were unambiguously elucidated based on extensive spectroscopic data analyses, C NMR chemical shifts calculations coupled with the DP4+ probability method, and the calculated and experimental electronic circular dichroism (ECD) spectra. Compounds 1-3 showed significant inhibitory activity against pancreatic lipase (PL). The result highlights that the presence of 3-hydroxylated decanoic acid moiety at C-14 is important for increasing the inhibition potency against PL.

ABSTRACT

Objective@#This study aimed to explore the protective effect of procyanidin B2 (PCB2) on acute liver injury induced by aflatoxin B (AFB ) in rats.@*Methods@#Forty Sprague Dawley rats were randomly divided into control, AFB , AFB + PCB2, and PCB2 groups. The latter two groups were administrated PCB2 intragastrically (30 mg/kg body weight) for 7 d, whereas the control and AFB groups were given the same dose of double distilled water intragastrically. On the sixth day of treatment, the AFB and AFB + PCB2 groups were intraperitoneally injected with AFB (2 mg/kg). The control and PCB2 groups were intraperitoneally administered the same dose of dimethyl sulfoxide (DMSO). On the eighth day, all rats were euthanized: serum and liver tissue were isolated for further examination. Hepatic histological features were assessed by hematoxylin and eosin-stained sections. Weight, organ coefficient (liver, spleen, and kidney), liver function (serum alanine aminotransferase, aspartate aminotransferase, alkaline phosphatase, total bilirubin, and direct bilirubin), oxidative index (catalase, glutathione, superoxide dismutase, malondialdehyde, and 8-hydroxy-2'-deoxyguanosine), inflammation factor [hepatic interleukin-6 (IL-6) mRNA expression and serum IL-6], and bcl-2/bax ratio were measured.@*Results@#AFB significantly caused hepatic histopathological damage, abnormal liver function, oxidative stress, inflammation, and bcl-2/bax ratio reduction compared with DMSO-treated controls. Our results indicate that PCB2 treatment can partially reverse the adverse liver conditions induced by AFB .@*Conclusion@#Our findings indicate that PCB2 exhibits a protective effect on acute liver injury induced by AFB .

ABSTRACT

Objective:To compare the efficacy and safety of picosecond alexandrite laser and ultrapulsed fractional CO 2 laser for the treatment of facial atrophic acne scars in a prospective, split-face clinical trial. Methods:From October 2015 to October 2017, patients with facial symmetrical atrophic acne scars were enrolled from Department of Cosmetic Laser Surgery, Hospital for Skin Diseases, Chinese Academy of Medical Sciences and Peking Union Medical College. The left side of the face of the patients was treated with ultrapulsed fractional CO 2 laser, and the right side of the face was treated with a 755-nm picosecond alexandrite laser with a diffractive lens array. All patients received 3 sessions of laser treatment at a 2-month interval. The severity of scars was evaluated by 2 dermatologists before and after the treatment according to the ECCA grading scale (échelle d′évaluation clinique des cicatrices d′acné) , and clinical efficacy was self-assessed by the patients according to a 4-point scale. Pain scores and adverse reactions were recorded. Paired t test was used to compare ECCA scores and pain scores between the 2 sides of the face, as well as between pre- and post-treatment values, and Wilcoxon rank sum test was used to compare the 4-point scores between the 2 groups. Results:There was no significant difference in the ECCA score between the picosecond laser side and the fractional laser side before the treatment ( t = 1.06, P = 0.300) , while the ECCA score was significantly higher in the picosecond laser side than in the fractional laser side after 3 sessions of treatment (70.98 ± 21.48 vs. 58.04 ± 17.63, t = 3.76, P = 0.001) . Compared with the pre-treatment severity score of scars, the improvement in severity score was significantly less in the picosecond laser side than in the fractional laser side (2.21 ± 1.09 vs. 2.83 ± 1.11, z = 2.70, P = 0.007) . Compared with the fractional laser side, the picosecond laser side showed significantly lower pain scores (3.71 ± 0.62 vs. 6.23 ± 1.06, t = 11.93, P < 0.001) and less adverse reactions, which mainly manifested as transient erythema and edema. Conclusions:Both picosecond alexandrite laser and fractional CO 2 laser can effectively improve atrophic acne scars. Fractional CO 2 laser is more effective, but picosecond alexandrite laser has less adverse effects.

ABSTRACT

Objective:To evaluate the feasibility of the gasless transoral endoscopic thyroidectomy using an innovative space maintaining method. Methods:From Dec. 2018 to Apr. 2019, patients from Anhui Tumor Hospital treated with gasless transoral endoscopic thyroidectomy were retrospectively analyzed. Index such as intraoperative blood loss, operative time, postoperative hospital stay, and complications were recorded. Results:A total of 24 patients were included into this analysis. All operations were successfully completed. The mean blood loss was (41.3±26.4) ml, operation time was (191.8±40.8) min, postoperative hospital stay was (6.3±1.0) d. Nobody had drinking cough, recurrent laryngeal nerve palsy, massive bleeding, hypoparathyroidism or infection with use of preventive antibacteria. Transient lower jaw numbness was found in 8 patients and almost recovered within one month. Conclusions:Gasless transoral endoscopic thyroidectomy with the suspension device seems to be feasible and safe. Further study is needed to verify its validity.

ABSTRACT

A new lignan glucoside,(+)-fragransin A_2-4-O-β-D-glucopyranoside(1), has been isolated from the dry root of Paeonia lactiflora by column chromatography on silica gel, Sephadex LH-20, and MCI-gel resin, as well as preparative RP-HPLC. The structure of the new compound was elucidated by spectroscopic data analysis(MS, UV, IR, CD, 1 D and 2 D NMR) and chemical method. Compound 1 showed moderate inhibition against lipopolysaccharide induced nitric oxide production in RAW264.7 macrophages, with an IC_(50) value of 21.3 μmol·L~(-1).
