ABSTRACT
Objective To explore the levels of autoimmune antibodies and endocrine hormones in patients with endometriosis complicated with infertiilty.Methods 158 endometriosis and infertility patients were selected as study subjects,and they were divided into endometriosis complicated with infertility group(A group,52 cases),endometriosis group(B group,50 cases) and infertility group(C group,56 cases) according to the type of disease.At the same time,93 healthy women were selected as control group.The autoimmune antibodies such as anti-endometrial antibody(EMAb),anti-chorionic gonadotropin antibody(HCGAb),anti-sperm antibody(ASAb),anti-cardiolipin antibody(ACAb) and anti-ovarian antibody(AOAb),endocrine hormones such as luteinizing hormone(LH),follicle stimulating hormone(FSH),prolactin(PRL),estradiol(E2),insulin(INS) and testosterone(T) were compared in four groups.Results There was no significant difference in the total antibody positive rate between B group and C group (χ2=2.161,P>0.05).The total antibody positive rate of A group was significantly higher than that of B,C group and the control group,the difference was statistically significant(χ2=6.316,P<0.05).The PRL and INS levels of A group were much higher than those of B,C group and the control group,the differences were statistically significant(F=15.625,10.431,all P<0.05).The E2,INS and PRL levels of B group were much higher than those of C group and the control group(F=10.363,9.515,9.037,all P<0.05).The E2,T,INS and PRL levels of A group were much higher than those of B and C group,the differences were statistically significant (F=5.029,6.225,6.631,7.106,all P<0.05).The LH and FSH levels of the four groups had no significant differences(F=2.385,2.531,all P>0.05).There were no significant differences in all of the endocrine hormones between C group and the control group(t=1.535,0.931,0.825,1.363,1.005,1.259,all P>0.05).Conclusion The main factors that cause endometriosis patients complicated with infertility may be due to a endometriosis immune imbalance of T,E2 and PRL and endocrine abnormalities,through the analysis of the levels of autoimmune antibodies and endocrine hormones can provide a reliable basis for clinical.

Objective To describe and analyze the status quo of the doctor and nurse configuration in Heilongjiang Province,and to study their cultivation condition while predicting the number of medical staff.Methods Through the health workforce database of Heilongjiang Province in 2014,using Excel 2007 statistical software,the status quo of doctor and nurse configuration was analyzed.The grey prediction model was also used to analyze the number of medical staff in Heilongjiang province from 2004 to 2014,and the number of medical staff in Heilongjiang province from 2016 to 2018 was predicted.Results Up to 2015,the number of doctors and nurses in Heilongjiang Province accounted for 0.42% of the total population,composed of mainly young and middle-aged staff and mostly with bachelor's degree and junior college certificate.Doctor-to-nurse ratio was 1:0.96.The grey prediction model indicated that the number of medical staff in Heilongjiang Province would increase year by year,and the inversion of doctor-to-nurse ratio would be eased.Conclusion The reform and development of medical education in Heilongjiang Province has promoted the optimization of the professional title structure and educational structure.It is expected that by 2016 Heilongjiang medical care ratio inversion problem will be completely resolved.

Objective To investigate the expressions of Apaf-1 and Caspase-9 protein in ovarian serous carcinoma(OSC) and their clinicopathological significance.Methods The expressions of Apaf-1 and Caspase-9 protein in 45 cases of OSC,60 cases of ovarian serous cystadenomas and 32 cases of ovarian borderline serous cystadenomas were detected by immunohistochemical SP method.The relationship between the expressions of these two proteins and the clinicopathological features of OSC and the correlation between Apaf-1 and Caspase-9 expressions in OSC were analyzed.Results The positive rates of Apaf-1 in OSC,ovarian serous cystadenoma and ovarian borderline serous cystadenoma were 24.4%(11/45),75.0%(45/60) and 46.9%(15/32),the difference was statistically significant (χ2=26.734,P<0.01).Apaf-1 expression was correlated with pathological grade,clinical stage and lymph nodes metastasis (χ2=6.318,7.565,5.554,P<0.05).The expression rates of Caspase-9 in OSC,ovarian serous cystadenoma and ovarian borderline serous cystadenoma were 28.9%(13/45),83.3%(50/60) and 56.3%(18/32),the difference was statistically significant (χ2=31.682,P<0.01).The expression of Caspase-9 was correlated with pathological grade,clinical stage and lymph nodes metastasis (χ2=5.750,4.391,5.466,P<0.05).In OSC,the expressions of Apaf-1 and Capase-9 were positively correlated (k=0.433,P=0.003).Conclusion The low expressions of Apaf-1 and Caspase-9 in OSC may be related to the occurrence and development of OSC.

Rhubarb is a traditional Chinese medicines which possess laxative, lipid-lowering, and weight-loss activities, but the active compounds of lipid-lowering and underlying molecular mechanisms are not yet clear. This study aims to explore the effects of chrysophanol on the mRNA expressions of sterol regulatory element-binding proteins (SREBPs) and lipid metabolism in human liver carcinoma Huh-7 cells, which is one of the active compounds obtained from Rhubarb. A reporter gene assay was used to test the transcription of SREBP. The intracellular triglyceride and total cholesterol contents were measured by using commercially available test kits. The SREBPs target genes expressions were measured by Quantitative Real-Time PCR. Cell viability was evaluated by Cell Counting Kit-8. As the results shown, chrysophanol (40 μmol · L(-1), 16 h) could notably inhibited human SRE promoter activity in a dose-dependent manner and decrease intracellular cholesterol and triglyceride levels. Furthermore, the mRNA expressions of SREBPs target genes were significantly downregulated by chrysophanol treatment. However there are no significant differences on cell viability when compared with the control group. These results suggested that chrysophanol might improve lipid metabolism through suppressing the mRNA expressions of SREBPs target genes to attenuate intracellular lipid accumulation.

Purpose To study the expression of HIF-1αand Glut-1 in the molecular subtypes of breast carcinoma and their correlation with basal-like breast carcinoma. Methods 803 cases of invasive breast carcinoma from our database were identified. The clinicopath-ologic findings and the biologic markers including estrogen receptor ( ER) , progesterone receptor ( PR) , and human epidermal growth factor receptor-2 (HER-2) status were reviewed. Immunohistochemical MaxVision stains for cytokeratin 5/6 (CK5/6) and epidermal growth factor receptor ( EGFR) were performed. All breast carcinomas were subclassified into Luminal A, Lumincal B, HER-2 over-expression, normal-like, and basal-like subtypes according to Nielsen criteria. Immunohistochemical stain was also used to detect the expression of HIF-1αand Glut-1. Results Positive expression rates of HIF-1αprotein in basal-like, HER-2 over-expression, normal-like, Luminal A and Luminal B substypes were 77. 89% (74/95), 56. 06% (37/66), 55. 76% (92/165), 31. 97% (141/441), 25. 00% (9/36), respectively. The positive expression rates of Glut-1 protein were 80. 00% (76/95), 57. 58% (38/66), 58. 18%(96/165), 34. 01% (150/441), 25. 00% (9/36), respectively. The positive expression rates of HIF-1α and Glut-1 in the basal-like, HER-2 over-expressing and normal-like subtypes were remarkably higher than that in Luminal A and Luminal B subtypes ( P<0. 004 5) and the expression of HIF-1a and Glut-1 was negatively correlated with the expression of ER (P<0. 01). In the ER-negative breast cancers, the positive expression rates of HIF-1a and Glut-1 in basal-like substype were much higher than that in the other sub-types (P<0. 004 5), and the expression of HIF-1α was positively correlated with expression of Glut-1 in basal-like breast carcinoma (P<0. 01). Conclusion The overexpression of HIF-1αand Glut-1 may be closely related to the ER-negative breast cancer and HIF-1α and Glut-1 might play an important role in the development of basal-like breast carcinoma.

Objective To explore the risk factors of dyskinesias in Parkinson' s disease (PD) patients.Methods Patients with PD who had taken levodopa at least 1 month were recruited according to the United Kingdom Brain Bank (UKBB) Criteria..All patients were divided into dyskinesias group and non-dyskinesias group according to the clinical definition of dyskinesia.The parameters including gender,age,age onset,duration,body weight,UPDRS scale score,and treatment parameter,such as the maximum daily dose of levodopa,the cumulative time of medication of levodopa,levodopa dose per weight of patients with dyskinesias were recorded.Multivariate logistic regression analysis was used to analyze risk factors of dyskinesias.The patients were divided into dyskinesias and no dyskinesias groups based on presence of motor complications.Results The incidence rate of dyskinesias was 7.8% (11/142) in all 142 patients.Of which,9 cases were with peak-dose dyskinesia,and 2 cases with both wearing-off and dyskinesia.There were statistically significant difference between dyskinesias group and non-dyskinesias group in terms of sex,weight,the maximum daily dose of levodopa and levodopa dose per weight(P < 0.05).The same results appeared between wearing-off group and wearing-off with dyskinesia group (P < 0.05).Multivariate Logistic regression analysis showed that total dose of levodopa(OR =1.846,95% CI:1.234-2.762,P =0.003) and levodopa dose per weight were independentriskfactors(OR=0.991,95%CI:0.984-0.999,P=0.033).Conclusion The risk factors of dyskinesias in Parkinson's disease is closely linked to total dose of levodopa and levodopa dose per weight.

Objective Through investigation on hospital infection, to learn the actual status of hospital infection and improve hospital infection management work continuously. Methods Via transect investigation, all hospital-ized patients were investigated by bed-visiting and patients, medical records, checking at the same time. ResultsA total of 3032 cases were enrolled, and the prevalence from 2010 to 2012 were 6. 10%, 5.30% and 4.17%, re-spectively. ICU had the highest prevalence of 39.29%. The lower respiratory tract ranked the first place (45.16%) among infection sites. Invasive operation increased the risk of nosocomial infections. The utilization rates of antibiotics of the hospitalized patients from 2010 to 2012 were 67.7%, 64. 8%, and 54. 1%, respectively. The microbial submission rates for the therapeutic medication were 39 . 00%, 46.99%, and 55 . 01%, respective-ly. The gram-negative bacteria were dominant among the isolated bacterias, accounting for 61.39%. ConclusionThe prevalence survey in the nearly three years shows that the prevalence of nosocomial infections is decreasing,and the microbial submission rate for the therapeutic medication is also increasing by improved hospitalized conditions, strict management of antibiotics and enhanced surveillance.

Objective To explore the application value of detecting HPV E6/E7 mRNA in cervical cancer screening and prognosis.Methods bDNA technology was used to detect the HPV E6/E7 mRNA levels in 301 cases of paraffin-embedded cervical samples that were grouped based on pathological diagnosis.Statistic differece between multiple groups were assessed by chi-square segmentation method.Results The positive detection rate of HPV E6/E7 mRNA is higher in cervical cancer group than that in CIN1-3 groups and cervicitis group,which were 94.29 % (66/70),30.30 % (20/66),50.00 % (19/38),59.46 % (44/74),5.66 % (3/50),respectively (all P < 0.004 5).The HPV E6/E7-positive rates in all three CIN groups were higher than that in cervicitis group (P < 0.004 5).The positive rates in CIN2 and CIN3 group were higher than that in CIN1 group (P < 0.004 5).HPV E6/E7 mRNA levels in cervical cancer group were higher than those in CIN groups and cervicitis group (16 508.730±8 741.402,929:337±460.880,3 239.681±1 447.399,5 286.224±2 933.445,4.941±2.263,respectively,all P < 0.05).CIN groups had higher copy numbers of HPV E6/E7 mRNA than cervicitis group (P < 0.05).More copies of HPV E6/E7 mRNA were detected in CIN3 group than those in CIN1 group (P < 0.05).Conclusions The positive rate and expression levels of HPV E6/ E7 mRNA was related to the severity of cervical diseases.HPV E6/E7 mRNA detection in paraffin-embedded samples have a high clinical application value in predicting the risk of cervical cancer.It' s an effective measure for cervical cancer screening and follow-up.

In order to set up a base for stem cells to be widely used in clinical medicine, we tried to optimize, in this study, the technique that induces human mesenchymal stem cells (hMSCs) to differentiate into neural stem cells by using cerebrospinal fluid (CSF) from the different groups. After the induction, presence of neural stem cells was confirmed with microscope observation, flow cytometry analysis, immunohistochemistry and fluorescent immunohistochemistry. At the same time, we also compared and analysed the data of the number of stem cells when it totally met the requirements for clinical treatment and the days required. At last, we confirmed that hMSCs could be induced to differentiate into neural stem cells, and that the number of cells totally met the requirements for clinical treatment. But there were some differences both in the number of cells and the days required. Among the groups, the group that marrow mesenchymal stem cells from patients own induced by CSF from healthy volunteers used the shortest time and the quantity of the cells was significantly higher than those of the others.

@#Objective To study the related factors of leukoaraiosis (LA). Methods 480 elderly patients examined with cranial CT or MRI were investigated retrospectively. A single-factor and multi-Logistic regression analysis were performed. A rank correlation analysis was performed between the severity of LA and age as well as the course of hypertension. Results The prevalence of LA was 35%. The age (t=3.243,P=0.0013) and the incidence of hypertension (χ2=9.8941,P=0.0017) were higher in LA group than in non-LA group, the incidence of diabetic were lower (χ2=4.4193,P=0.0355). The age (OR=1.06,95%CI:1.027~1.103,P=0.0006), hypertension (OR=1.15,95% CI:1.065~1.236,P=0.0003), and diabetes (OR=0.85,95% CI:0.726~0.994,P=0.0421) were correlated with LA. Neither age (r=0.1553,χ2=4.9701,P=0.5477) nor the course of hypertension (r=-0.0758,χ2=1.9076,P=0.7527) were correlated with the severity of LA. Conclusion The age and hypertension was independent risk factors for LA, but the severity of LA was not correlated with age or course of hypertension.

Objective To describe the clinical features of convulsive status epileptieus (CSE) in our hospital and to provide a basis for future CSE prevention and treatment. Methods Patients with CSE hospitalized from January 1996 to October 2007 were prospectively observed. Logistic regression was used to identify predictors of prognosis. Results All 220 eases of CSE were prospeetively analyzed, a hundred and two patients(46.4%) originated from rural areas. The primary cause of CSE was central nervous system(CNS)infectian (72cases, 32.7%), followed by discontinuation or reduction of antiepileptie drugs (AEDs, 35 cases,15.5%). The median duration of CSE was 5 hours and median duration of seizures before treatment was 2 hours; both were longer in rural patients (7.0 and3.5 hours respectively) than in urban patients (3.0 and 2.0 hours, Z=-3.433,-1.558,both P<0.05). The fatality rate by the time of discharge was 15.9%. Logistic regression analysis showed that the duration of CSE (χ2=20.941), a history of epilepsy (χ2=4.910), and respiratory depression (χ2=16.086) were independent predictors of CSE prognosis (allP<0.05) . Comparisons between these data of USA and Europe were made. Conclusions CSE occurs mostly in rural population and epilepsy patients. CNS infection and withdrawal or reduction of AEDs in patients with epilepsy were important triggers of CSE. Antiepileptic therapy for status epilepticus in China falls behind that in the European countries.

OBJECTIVE To investigate the knowledge of Helicobacter pylori infection among outpatients,analyzing the data and providing useful information and measures for H.pylori infected and uninfected patients. METHODS Questionnaires about the knowledge of H.pylori were operated on 286 patients randomly selected from patients taking gastrointestinal endoscopy in October,2005. RESULTS Among 286 patients investigated,177 cases(61.9%) took the examination for diagnosis,while 157 cases(54.9%) had upper abdominal symptom more than 12 months.There were 60% patients investigated who knew only 2 items from total(7 items) about H.pylori in the questionnaires,while 46.7% patients wanted to obtain the knowledge of the disease and instructions on health care from medical professionals.In addition,it showed that there was no close relation between education level and knowledge of the disease. CONCLUSIONS Investigating the knowledge of special disease,which mastered by the patients,will help to identify patients blind to the disease they have,and patients who are in need of health-care instructions.

Objective To study the expression of heat shock 27 000 associated protein 1 ( HSPBAP1, GenBank: AK096705) in the brain tissues of patients with drug-refractory epilepsy and discuss its function in the pathogenesis. Methods Fluorescent quantitative polymerase chain reaction ( FQ-PCR) and immunohistochemistry were used to test the expression of HSPBAP1 in the surgically removed brain tissues of patients with drug-refractory epilepsy from the brain bank of our department ( n = 36) , and the results were compared with that of normal controls (n = 8 ). Objective:To study the expression of ULBP2 protein in the brain tissues of patients with drug-refractory epilepsy and its clinical significance.Methods:Gene-chip,immunofluorescence and Western blot were used to test expression of ULBP2 in the surgically removed brain tissue of patients with drug-refractory epilepsy from the brain bank of our department(n=42),and the results were compared with that of normal controls (n=12).Results:The relative increasing expression of ULBP2-gene in the brain of patients with drug-refractory epilepsy,and ULBP2 protein expression was significantly increased in temporal lobe cortex of patients with drug-refractory epilepsy as compared with the same regions of the controls specimens.Conclusion:The results indicate that the overexpression of ULBP2 may be involved in the pathophysiology of drug-refractory epilepsy.
