ABSTRACT
As a mature management method, the matrix management model has been embraced by various sectors at large. This article introduced the practice of a tertiary general hospital in combining hospital ethics with medical administration and code of conduct for medical service, leveraging the matrix management mode in hospital ethics construction. By sorting out the risk exposure in hospital ethics, the hospital identified specific responsibilities and departments involved. On such basis, eight stable matrix-based ethics working groups were established, namely hospital ethics training, hospital ethics publicity, hospital ethics interview, prevention against prescription statistics for drug rebate, patient rights protection, management of physician practice in other medical facilities, academic cooperation, and hospital ethics tour inspections. Matrix-based management meets the characteristics of hospital ethics development as it not only enables multi-department management needs, but also improves the efficiency. With the advancement and connotation enrichment of hospital ethics development, matrix management can become a regular management mode.

Objective To optimize the science research funding management,improve the quality of scientific research management,as well as scientific research.Methods The concept of lean management and full life cycle were brought in to analyze the problems and shortcomings under the traditional extensive management.A new administration model adopted in the full life cycle management was constructed.Results The management of hospital research funds plays a central part of the hospital scientific research management,which is also the premise to ensure the smooth implementation of hospital scientific research.Conclusions Through the exploration of full life cycle model of scientific research funds management,relevant departments,such as Department of Research,Finance Department,Equipment Department,Hospital Office and so on improved their internal functional communication,data and information can be shared,which helped streamlining the administration of research funding,as well as the plan and supervision of research funding use.

Objective The basic feature of translational medicine is collaborative innovation by multiple disciplines.The essence of translational medicine is the management innovation.Public hospitals have the advantages of disciplines and talents,which is one of the main forces for translational medicine research.Study on the current situation of intellectual property right (IPR) management department in public hospitals and the demand analysis of medical staff in the medical translation will improve the management system of innovational medical technology and support the government policymaking.Methods Questionnaire survey was carried out in the part of Shanghai secondary and tertiary public hospitals.Results A total of 150 questionnaires were surveyed with a responding rate of 98%.Nearly 65 % of inventors do not know the basic situation of the administrative staff,which is in charge of the IPR management.The main work focuses on the patent application (accounting for 70.9% of the responders).The medical staff want be supported in four areas including professional team(accounting for 62.5% of the responders),special funds(accounting for 80.3% of the responders),public research platform(accounting for 55.7% of the responders) and talent policy(accounting for 43.6% of the responders).Conclusions Full time management of IPR is proposed,active awareness of the administrative staff needs to be strengthened and government should support in these four areas.

In order to develop agents with superior chemopreventive and chemotherapeutic properties against hepatocellular carcinomas, mitochondria-targeted hydroxycinnamic acids (MitoHCAs) were synthesized by conjugation with a triphenylphosphonium cation. These synthetic compounds were evaluated for their antioxidant activities in hepatic mitochondria, including against OHand ROOinduced lipid peroxidation. HOproduction was decreased significantly by increasing glutathione peroxidase and catalase activities. In addition, cell proliferation data from three cell lines (HepG2, L02 and WI38) indicated that the MitoHCAs were selective for cancer cells. Interestingly, the MitoHCAs both with or without Catriggered mitochondrial dysfunction by inducing mitochondrial swelling, collapsing the mitochondrial membrane potential and causing cytochromerelease. In particular, an inhibitor of the mitochondrial permeability transition pore (mPTP), cyclosporin A, attenuated mitochondrial damage and cell apoptosis, indicating that mPTP may be involved in the antiproliferative activity of MitoHCAs. Further studies focused on structural optimization of these compounds are onging.

Objective The article analyzed the output and influence of papers published on SCI and statistical journals in our hospital from 2008 to 2013.The results could be provided to the scientific research management as reference.Methods Bibliometrics methodology was adopted to analyze the publication number of papers,citation conditions and impact factors of publishing journals to conduct retrospective analysis of the paper output and influence of our hospital.Results According to the longitudinal research,the output of sci tech papers published on SCI and statistical journals have increased significantly in 2008-2013,but the influence needs to be further improved.On the other hand,the result also reflects the improvement of scientific research capacity of hospital.Conclusions High level sci-tech Paper is an important embodiment of the scientific research output and level in re search institutions.The analysis shows continuous improvement of discipline construction as well as evaluation mechanism has played a positive role to enhance sci tech output and influence of the hospital.

Objective To evaluate the effects of meglumine cyclic adenylate (MCA) pretreatment on acute lung injury (ALI) induced by lipopolysaccharide (LPS) in rats.Methods Fifty-four adult male Sprague Dawley rats,aged 2-3 months,weighing 200-250 g,were randomly divided into 3 groups (n =18 each) using a random number table:control group (group C),ALI group and MCA pretreatment group (group MCA).ALI was induced with LPS 10 mg/kg injected via the femoral vein in ALI and MCA groups.In group MCA,MCA 2 mg/kg was injected via the femoral vein at 20 min before LPS injection,while the equal volume of normal saline was given in C and LPS groups.Immediately before LPS injection and at 2 and 4 h after LPS injection,the blood samples were taken from the abdominal aorta for determination of PaO2,PaCO2 and plasma concentrations of tumor necrosis factor-alpha (TNF-α) and interleukin-10 (IL-10).Six rats were sacrificed at 4 h after LPS injection and pulmonary specimens were obtained for microscopic examination of the pathological changes and for determination of cyclic adenosine monophosphate (cAMP) activity (by ELISA) and NF-κB p65 expression in nucleus (by Western blot).Results Compared with group C,PaO2 and cAMP activity in lung tissues were significantly decreased,and PaCO2,plasma concentrations of IL-10 and TNF-α and NF-κB p65 expression in nucleus in lung tissues were increased in group LPS.Compared with group LPS,PaO2,cAMP activity in lung tissues and plasma concentrations of IL-10 were significantly increased,and PaCO2,plasma concentration of TNF-α and NF-κB p65 expression in nucleus in lung tissues were decreased and the pathologic changes of lungs were attenuated in group MCA.Conclusion MCA pretreatment can attenuate ALI induced by LPS,and the mechanism is related to increased level of cAMP,activated cAMP signaling pathway,inhibited NF-κB activity and reduced inflammatory responses in lung tissues of rats.

Objective To evaluate the effect of trimetazidine pretreatment on myocardial injury induced by propofol in rats.Methods Forty healthy Sprague-Dawley rats,weighing 240-250 g,aged 2.5 months,were randomly divided into 4 groups (n =10 each):fat emulsion group (group L),propofol group (group P),and different doses of trimetazidine groups (TL and TH groups).In group L,10% fat emulsion was infused intravenously for 12 h at a rate of 30 mg·kg-1 ·h-1.In group P,1% propofol was infused intravenously for 12 h at a rate of 30 mg·kg-1 ·h-1.In TL and TH groups,trimetazidine 2 and 5 mg/kg were injected intravenously,respectively,and 1.5 h later propofol was infused intravenously for 12 h at a rate of 30 mg·kg-1 ·h-1.At 6 and 12 h after beginning of administration,blood samples from the internal jugular vein were taken for determination of the levels of serum creatine kinase (CK),creatine kinase isoenzyme-MB (CK-MB),lactate dehydrogenase (LDH),α-hydroxybutyric acid dehydrogenase (α-HBDH),cardiac troponin Ⅰ (cTnⅠ),triglycerides (TG),low-density lipoprotein (LDL) and high-density lipoprotein (HDL).After blood sampling at 12 h after beginning of administration,myocardial specimens were obtained for microscopic examination of the pathological changes with light and electron microscope.Results Compared with group L,the serum CK,CK-MB,LDH,α-HBDH,cTnⅠ and TG levels were significantly increased,HDL was decreased,and no significant changes were found in LDL concentrations in P and TL groups,and the serum TG,HDL and LDL were increased,and no significant changes were found in the other parameters in group TH.Compared with group P,the serum CK,CK-MB,LDH,α-HBDH,and cTnⅠ levels were significantly decreased in TL and TH groups,the serum TG level was increased in TL group,and the serum TG,HDL and LDL levels were increased in TH group.Compared with group TL,the serum CK,CK-MB,LDH,α-HBDH and cTnⅠ levels were significantly decreased,and the serum TG,HDL and LDL levels were increased in group TH.Conclusion Trimetazidine pretreatment can attenuate myocardial injury induced by propofol,but it induces increase in blood lipids.

ObjectiveTo investigate the effects of HO-1 on mast cells degranulation during liver ischemia/reperfusion injury.MethodsSD rats ( n =20) were randomly divided into 4 groups : ( 1 ) sham operation group, (2) ischemia/reperfusion injury group (I/RI group), (3) CoPP group, CoPP was given before 24 h(5 mg/kg), (4) ZnPP group, ZnPP was given before 24 h (20 mg/kg). The rat model of liver ischemia/reperfusion was built. Samples were collected after 2 hours reperfusion. HO-1 mRNA expression was assessed by RT-PCR. The expression of HO-1 protein was assessed by Western blot. Serum ALT, AST levels were determined. The amount of degranulated mast cells were detected by toluidine blue staining. Liver injuries were evaluated by HE staining.ResultsCompared to sham group, liver HO-1mRNA and protein expression, ALT, AST levels, the number of degranulated mast cells significantly increased in I/RI group, CoPP group and ZnPP group. Moreover, liver injuries were more serious. Compared to I/RI group, there were increased HO-1mRNA and protein expression, reduced ALT, AST levels and the number of degranulated mast cells, also reduced liver injuries were found in CoPP group. Compared to I/RI group, the expression of HO-ImRNA and protein was down-regulated, ALT, AST levels elevated and the number of degranulatedmastcellsincreasedaccompanyingmoreseriousliverinjuriesinZnPPgroup.ConclusionsHO-1 overexpression reduces liver ischemia/reperfusion injury, possibly by inhibiting mast cells degranulation in liver tissues.

Heme oxygenase(HO) is a rate-limiting enzyme that catalyzes heme degradation.It catabolizes heme into 3 products: carbon monoxide(CO),free iron and bilirubin,which play an important role in keeping the steady state of the organism.In recent years,some studies show that the promotion of HO-1 expression can reduce ischemia-reperfusion injury of an organ transplant and regulate the transplantation immunological rejection.It has an important application prospect in liver transplantation.
