ABSTRACT

Abstract@#Allostatic load (AL) is related to stress. Adverse childhood experiences(ACEs), as a common stress in childhood, can make a serious and lasting impact on it. Allostatic load can reflect the wear and tear of an individual s physiological system. This article mainly reviews the functional changes of several systems of AL who have experienced ACEs, including neuroendocrine, metabolism, immune, and cardiovascular systems, as well as the different effects of the occurrence time and subtypes of ACES on AL, providing some theoretical basis for the development of early intervention plans in the future and reducing the occurrence and development of deleterious outcomes.

ABSTRACT

Objective:To evaluate the short-term precision and accuracy of bone mineral density (BMD) measured with quantitative CT (QCT) and dual-energy X-ray absorptiometry (DXA) in multi-centre clinical research with a European spine phantom (ESP).Methods:From January 2016 to April 2020, totally 40 CT scanners (12 Siemens from Germany, 12 Philips from Netherlands, 9 GE from US, 5 Toshiba from Japan, 2 United Imaging from China) and 53 DXA instruments (34 GE Lunar from US, 14 Hologic from US, 5 Medlink from France) used in multi-centre in China were enrolled. The CT equipment came from 31 centers and DXA equipment from 32 centers.Using Mindways QCT software, the ESP was scanned 10 times on each instrument with repositioning using standardized spine protocols with QCT and DXA. The BMD value of the three lumbar vertebrae with low, medium, high density and the mean BMD value were measured. Accuracy was assessed by comparing BMD values measured on each device with the actual value of the phantom. Short-term precision was calculated as the root-mean-square standard deviation (RMS-SD) and root-mean-square standard deviation coefficient of variation (RMS-%CV) for the repeated measurements. The repeated measures variance analysis was used to compare the differences in BMD between different devices.Results:The differences in BMD values were statistically significant among different CT and DXA devices.The ranges of the accuracy measured by different QCT devices were 1.20% to 7.60% for Siemens, -1.83% to 0.20% for Philips, 1.18% to 13.20% for GE, -0.12% to 3.55% for Toshiba, -1.65% to 6.32% for United Imaging, 6.59% to 21.34% for GE Lunar, -6.65% to 5.45% for Hologic, and -6.97% to -0.68% for Medlink, respectively. The RMS-%CV of all vertebral BMD values measured by QCT and DXA ranged from 0.38% to 3.85%. The RMS-SD of QCT was 0.54 to 2.45 mg/cm 3, of DXA was 0.009 to 0.037 g/cm 2. The RMS-%CV values of each vertebral body measured by different QCT and DXA devices decreased with the increase of BMD, while the RMS-SD values showed the opposite tendency. Conclusions:Based on ESP, the QCT and DXA devices have significant differences in lumbar spine BMD measurement. Comparing the measurement results among different devices requires cross-calibration. Overall, the accuracy and short-term precision are within a reasonable range, which can be used for clinical follow-up observation. The short-term precision error and accuracy error range of QCT in evaluating the density of ESP were slightly smaller than those of DXA.

ABSTRACT

Objective:To explore the effects of Yuanzhisan (YZS) containing Ginseng Radix et Rhizoma (YZSR) or Codonopsis Radix (YZSD) on memory disorder based on network and experimental pharmacology. Method:The active components and targets of YZS were retrieved from the component database and literature, and the targets of memory disorder from the disease databases. The intersection targets revealed by Veen diagram were subjected to pathway analysis. The common active components of YZSR and YZSD were molecularly docked onto the core targets. Scopolamine hydrobromide was used to establish the memory disorder model, which was employed in the behavioral experiments for evaluating the effect of YZSR and YZSD on memory disorder. Result:There existed 33 active components for Ginseng Radix et Rhizoma and 31 for Codonopsis Radix, with four common active components and 380 common targets. YZSR contained 85 active components and 790 drug targets, and YZSD 81 active components and 781 drug targets. The mapping of 425 memory disorder targets with those of YZSD and YZSD yielded 133 and 130 intersection targets, respectively. The metabolic pathways involved calcium ion signaling pathway, hypoxia-inducible factor-1 (HIF-1) signaling pathway, mitogen-activated protein kinase (MAPK) signaling pathway, etc. As revealed by molecular docking, the binding energy of common active components to the targets was negative, and the binding effect of frutinone A was the best. Behavioral experiment results showed that both YZSR and YZSD alleviated the memory disorder. In the step-down test, the number of errors in the YZSD group was significant lower than that in the model group (<italic>P</italic><0.01). In Morris water maze test, the movement distance of the YZSD group was remarkably shortened in comparison with that of the model group (<italic>P</italic><0.05). In the open field test, the movement distances of both the YZSR and YZSD group were shortened in contrast to that in the normal group (<italic>P</italic><0.05). Conclusion:YZS had a certain effect on memory disorder. There are similarities and differences between YZSR and YZSD in the treatment of memory disorder.

ABSTRACT

Objective:To investigate the effect of Huoxue Jiedu Runzao prescription on the morphology, apoptosis, and function of submandibular gland in the mouse model of Sjögren's syndrome (SS) and its functioning mechanism, we analyzed the expression of the apoptosis inhibitor Survivin in the submandibular gland cells of SS mice. Method:Female BALB/c57 mice were selected as the normal group. The naive non-obese diabetic (NOD/Ltj) female mice were selected as the SS model, which were randomly assigned into the model group, Paeoniae Radix Alba total glucosides capsule (0.234 g·kg<sup>-1</sup>) group, and low-, medium-, and high-dose (15.6, 31.2, 62.4 g·kg<sup>-1</sup>, respectively) Huoxue Jiedu Runzao prescription groups. Each group had 15 mice. The morphological and functional changes of submandibular gland and the Survivin expression were observed and measured after 8 weeks of drug intervention. Survivin expression was determined by immunohistochemistry (IHC), Western blot, and reverse transcription-polymerase chain reaction (RT-PCR). Result:Compared with normal group, salivary flow and submandibular gland index in model group were significantly decreased (<italic>P</italic><0.01), and histopathological score of submandibular gland was significantly increased (<italic>P</italic><0.01). Western blot showed that Survivin protein expression was significantly decreased (<italic>P</italic><0.05). IHC showed that, Survivin mRNA expression was significantly decreased (<italic>P</italic><0.01), and RT-PCR results showed that Survivin mRNA expression was significantly decreased (<italic>P</italic><0.01). Compared with model group, salivary flow and submandibular gland index of mice in Huoxue Jiudu Runzao prescription groups and Paeoniae Radix Alba total glucosides capsules groups were significantly increased (<italic>P</italic><0.05), histopathological score of submandibular gland was significantly decreased (<italic>P</italic><0.05), IHC results showed that Survivin expression was significantly increased (<italic>P</italic><0.01). Western blot and RT-PCR results showed that Survivin protein and mRNA expression of Huoxue Jiudu Runzao prescription high-dose group and Paeoniae Radix Alba total glucosides capsule group were significantly increased (<italic>P</italic><0.05). Conclusion:Huoxue Jiedu Runzao prescription can improve the secretion function of submandibular acinus, increase the submandibular gland index, and saliva secretion of SS mice by up-regulating survivin in submandibular gland cells of SS mice.

BACKGROUND: Domestic and foreign scholars have done a lot of researches on the stress of articular cartilage under different mechanical environments and cyclic compressive loads, but they mainly studied the effect of cartilage under the cyclic compressive load. Studies on the effects of age factor on the mechanical properties of cartilage and studies regarding the properties of cartilage in complex stress environments are not exhaustive. OBJECTIVE: To investigate the effects of different rolling load conditions on the ratcheting behavior of adult and juvenile articular cartilage. METHODS: Adult cartilage and juvenile cartilage were used as experimental objects, and the load was applied by a rolling load device under different experimental conditions (compression: 10%, 20%, 30%; rolling rate: 1.66, 3.44, 6.68 mm/s; defect width: 1, 2,4 mm). At the same time, non-contact digital technology was used to collect the sample during the loading process, and the cyclical pressure was studied by analyzing and processing the image. The ratcheting behavior of adult and juvenile articular cartilage was studied under rolling load. RESULTS AND CONCLUSION: Under rolling load, the ratcheting strain of adult cartilage and juvenile cartilage showed a rapid increase followed by a slow increase tendency with the rolling load. With the increase of compression, the ratcheting strain of adult cartilage and juvenile cartilage increased. At the same amount of compression, the ratcheting strain of juvenile cartilage was greater than that of adult cartilage, and their ratcheting strain gradually decreased from the surface layer to the deep layer along the depth of cartilage. As the rolling rate increased, the ratcheting strain of adult cartilage and juvenile cartilage decreased. The ratcheting strain values and trends of 1 mm microdefect articular cartilage were similar to those of intact articular cartilage. Under the condition of 2,4 mm defect, the ratcheting strain value of the defected cartilage was higher than that of the intact cartilage.

ABSTRACT

ObjectiveTo investigate the changes in gut microbiota after transjugular intrahepatic portosystemic shunt (TIPS) in cirrhotic patients with mild hepatic encephalopathy (MHE) in different prognosis groups. MethodsA total of 28 MHE cirrhotic patients who were hospitalized and underwent TIPS in Xijing Hospital of Digestive Diseases from July 2016 to July 2017 were enrolled. Fecal samples and related clinical data were collected on days 1-3 before surgery and at 1 month after surgery. According to the prognosis after surgery, the patients were divided into none-hepatic encephalopathy (HE) group with 8 patients, MHE group with 12 patients, and overt hepatic encephalopathy (OHE) group with 8 patients. Fecal samples were analyzed by 16S rRNA sequencing to obtain the relative abundance of gut microbiota, and SPSS and R packages were used to analyze the biodiversity, postoperative changes, and differences in such changes of gut microbiota at the genus level between groups. The chi-square test was used for comparison of categorical data between groups; the Kruskal-Wallis H test was used for comparison of continuous data between three groups; the Bonferroni method was used for multiple comparisons of multiple samples; the Wilcoxon signed-rank test was used for comparison before and after surgery within each group. For microbiome beta-diversity analyses, a principal coordinate analysis (PCoA) was performed based on Bray-Curtis distance matrix, and the Adonis method (PerMANOVA) was used for comparison between groups. ResultsPCoA based on Bray-Curtis distance matrix showed that only the MHE group had a significant change in beta diversity after surgery (F=2.71, P=0.049). After surgery, the non-HE group had significant increases in the abundance of the native flora Dialister, Coprococcus, Ruminococcaceae_uncultured, Flavonifractor, and Clostridium_sensu_stricto_1 (Z=2.521, 2.1, 2.1, 2.1, and 1.96, all P<0.05); the MHE group had significant reductions in the abundance of the harmful flora Granulicatella(Z=2.521,P=0.012), Enterococcus(Z=2.51,P=0.012), Streptococcus(Z=2.432,P=0.015), and Rothia(Z=2.001,P=0.045) and significant increases in the abundance of Veillonella(Z=2.353,P=0.019) and Megasphaera(Z=1.955,P=0.05); the OHE group only had a significant increase in the abundance of Veillonella after surgery (Z=2.38, P=0.017). There was a significant difference in the change in gut microbiota (postoperative abundance/preoperative abundance) between the non-HE group, the MHE group, and the OHE group [2.00 (1.11-91.61) vs 1.21 (0.26-679) vs 0.09 (0.01-0.92), χ2=6.249, P=0.043]. ConclusionThere is a significant difference in the change in gut microbiota after TIPS between patients with different prognoses, and the increase in the abundance of native flora may have a certain influence on the remission of MHE.

@#AIM:To evaluate the repeatability and comparison of corneal visualization scheimpflug technology(Corvis ST)parameters in keratoconus eyes before and after accelerated transepithelial corneal cross-linking(ATE-CXL).<p>METHODS: Thirty eyes of 30 progressive keratoconus patients were included in the prospective study. Three repeated corneal biomechanical measurements were performed preoperatively and one month postoperatively by Corvis ST. The interclass correlation coefficient(ICC)and 95% confidence interval(<i>CI</i>), Cronbach'α, repeatability coefficient(RC), and coefficient of variation(CV)were used to evaluate the repeatability of Corvis ST parameters. Paired <i>t</i>-test or Wilcoxon rank test was used to evaluate the differences between preoperative and postoperative data.<p>RESULTS: At preoperative, 26 of 39(66.67%)parameters showed good to excellent repeatability, 6(15.38%)showed moderate, and 7(17.95%)showed poor repeatability. Similarly, 34(87.18%)parameters showed good to excellent repeatability, 3(7.69%)showed moderate, and 2(5.13%)showed poor repeatability after ATE-CXL. After ATE-CXL 1mo, the intraocular pressure(IOP), biomechanical corrected IOP(bIOP), first applanation time(A1T), Radius, deformation amplitude at the first applanation(A1DA), deflection length at the maximum deformation(HCDLL)and stiffness parameter at first applanation(SP A1)parameters increased, while the steep keratometry(Ks), flat keratometry(Kf), mean keratometry(Kmean), second applanation time(A2T), DA Ratio Max(2 mm)and integrated radius parameters decreased(all <i>P</i><0.05). <p>CONCLUSION:The repeatability of the Corvis ST parameters before and 1mo ATE-CXL follow up were both acceptable, and the corneal stiffness was improved after 1mo ATE-CXL.

ABSTRACT

Objective:To investigate the accurrence and related factors of depression in patients with parkinson's disease (PD).Methods:One hundred PD patients with Parkinson′s disease (PD) were enrolled in Department of Neurology in Beijing Hospital from March to June 2019. The depressive mood, sleep status and health-related quality of life of PD patients were evaluated by Beck Depression Scale (BDI), Parkinson′s Disease Sleep Scale 2 (PDSS-2), and 8 items of Parkinson′s Disease Questionnaire (PDQ-8). Spearman correlation analysis and multivariate regression analysis were used to analyze the related factors of PD depression. Nonparametric test was used to analyze the differences between different sleep disorder domains in PD patients with depression.Results:The total score of BDI in 100 PD patients ranged from 0 to 44, with an average of 9.9±8.6 points; the total score of PDSS-2 ranged from 0 to 43, with an average of 12.0±9.2; the total scores of PDQ-8 ranged from 0 to 24, with an average of 6.4±6.0 points. The detection rate of depression in 100 PD patients was 42.0% (42/100), and the total BDI score of 22 patients with mild depression ranged from 10 to 15 with an average of 13.0±1.9; the total BDI score of 14 patients with moderate depression ranged from 16 to 25, with an average of 19.1±3.0; the total BDI score of 6 patients with major depression ranged from 28 to 44 points, with an average of 33.3±6.4. The depression in PD patients was positively correlated with PDQ-8 score ( t=8.908, P<0.01) and PDSS-2 score ( t=2.286, P=0.02); and 85.7% (36/42) of PD patients with depression had sleep disturbance. The score of sleep disturbance domain of PD patients with depression (9.8±3.9) was higher than that of nocturnal motor symptom domain (4.8±4.5) and nocturnal PD non-motor symptom domain (4.1±3.3) ( P<0.01). The scores of emotional health, relationship with loved ones, attention, communication ability dimensions of PDQ-8 were positively correlated with PD depression ( P<0.05). Conclusion:Depression is a common non-motor symptom of PD patients; and PD patients with depression suffer from reduced quality of life and are more prone to sleep disorders.

ABSTRACT

Objective:To investigate clinical and histopathological characteristics of nevoid melanoma.Methods:A retrospective analysis was carried out on clinical and histopathological data collected from 3 patients with nevoid melanoma diagnosed in Department of Dermatology, Xijing Hospital from 2000 to 2020.Results:Among the 3 patients with nevoid melanoma, 2 were females and 1 was male. Skin lesions initially manifested as black macules and papules. After surgical resection, 2 patients developed new nodular lesions or recurrent skin lesions which enlarged into plaques. Histopathological examination showed proliferation of epithelioid melanocytes in the epidermis and dermis, with cytological atypia and some deeply stained nuclei. Immunohistochemical study revealed positive staining for Melan-A and S100 in tumor cells in the skin lesions, diffusely positive staining for HMB45 in dermal tumor cells, locally negative staining for HMB45 in the deep dermis, increased Ki67 proliferation index, and active expression of cyclin D1.Conclusion:Nevoid melanoma is easy to be misdiagnosed as pigmented nevi or seborrheic keratosis; when recurrence or metastasis occurs in patients with histologically diagnosed pigmented nevi, attention should be paid to the possibility of nevoid melanoma.

ABSTRACT

Objective:To investigate the value of computed tomography (CT) in the diagnosis of lung adenocarcinoma in different stages of focal ground-glass nodules (GGN).Methods:153 cases of lung adenocarcinoma with focal ground-glass nodule confirmed by surgery and pathology in Xiangya Hospital of Central South University from 2017 to 2020 were retrospectively analyzed. The location, size, shape, boundary, density, blood vessels and bronchus of the lesions were analyzed by statistical analysis to determine the correlation between CT features and lung adenocarcinoma in different periods.Results:153 focal ground-glass nodules showed pure ground-glass nodules (pGGN) in 21 cases, mixed ground-glass nodules (mGGN) in 132 cases. 32 cases of pre-invasive lesions in lung adenocarcinoma at different stages, including 13 cases of type adenomatous hyperplasia (AAH), 19 cases of carcinoma in situ (AIS); 121 cases of invasive disease, including 35 cases of microinvasive adenocarcinoma (MIA) and 86 cases of invasive adenocarcinoma (IA); There were statistically significant differences in the CT image features of four different stages of pulmonary ground-glass nodule adenocarcinoma patients in the edge lobulation, thick and short burr, bronchial passage and occlusion, vascular thickening and convergence ( P<0.05). There were no statistically significant difference in the clear boundary, shape, slender burr, mild bronchiectasis, irregular vascular passing signs among the groups ( P>0.05). There were significant differences in the maximum length diameter and CT value of ground-glass nodules before infiltration and infiltrating lesions in 153 cases ( P<0.05). Receiver operating characteristic (ROC) curve analysis found that the combined evaluation of the maximum layer length and CT value of lesions had a good value for the invasion of lung adenocarcinoma, and the cut-off points were 13.04 mm, -447.48 Hu, respectively. There were significant differences in the maximum layer length and CT value between pGGN and mGGN ( P<0.05). Conclusions:Through the analysis of CT findings of lung adenocarcinoma in different stages of focal ground-glass nodule, it was found that different signs had different value for the pathological types of lung adenocarcinoma in different stages. mGGN was more inclined to invasive lesions, and GGN appeared lobulation, thick and short burr, vascular convergence, bronchial occlusion, and when the lesion diameter was more than 13.04 mm, average CT value >-447.48 Hu, highly suggestive of invasive lung adenocarcinoma.

ABSTRACT

OBJECTIVE Although the underlying mechanism is largely unknown, gut dysbiosis has emerged as a central initiator of obesity-related diseases including nonalcoholic fatty liver disease (NAFLD), type 2 diabetes and meta?bolic syndrome. The emerging evidence support the use of prebiotics like herb-derived polysaccharides for treating NAFLD by modulating gut microbiome. So, our study focused on the microbiota-dependent anti-NAFLD effect and the exact mechanisms of Astragalus polysaccharides (APS) extracted from Astragalus mongholicus Bunge in high-fat diet (HFD) fed mice. METHODS Co-housing experiment was used to assess the microbiota dependent anti-NAFLD effect of APS. Then, targeted metabolomics and metagenomics were adopted for determining short-chain fatty acids (SCFAs) and bacteria that were specifically enriched by APS. Further in vitro experiment was carried out to test the capacity of SCFAs-producing of identified bacterium. Finally, the anti-NAFLD efficacy of identified bacterium was tested in HFD fed mice. RESULTS Our results first demonstrated the anti-NAFLD effect of APS in HFD fed mice and the contribution of gut microbiota. Moreover, our results indicated that SCFAs, predominantly acetic acid were elevated in APS-supplemented mice and ex vivo experiment. Metagenomics revealed that D. vulgaris from Desulfovibrio genus was not only enriched by APS, but also a potent generator of acetic acid, which showed significant anti-NAFLD effects in HFD fed mice. In addition, D. vulgaris modulated the hepatic gene expression pattern of lipids metabolism, particularly suppressed hepatic fatty acid synthase (FASN) and CD36 protein expression. CONCLUSION APS enriched D. vulgaris is effective on attenuating hepatic steatosis possibly through producing acetic acid, and modulation on hepatic lipids metabolism in mice. Further studies are warranted to explore the long-term impacts of D. vulgaris on host metabolism and the underly?ing mechanism.

ABSTRACT

Objective:To investigate the application value of Laennec approach in laparoscopic anatomical right hemihepatectomy (LARH).Methods:The retrospective and descriptive study was conducted. The clinicopathological data of 2 female patients who underwent LARH via Laennec approach in the First Affiliated Hospital of Kangda College of Nanjing Medical University from May to July 2020 were collected. The two patients were 51 and 57 years old, respectively. Observation indicators: (1) surgical situations; (2) postoperative situations and follow-up. Follow-up using outpatient examination and telephone interview was conducted to detect post-operative survival and tumor recurrence of patients up to December 2020. Count data were repre-sented as absolute numbers.Results:(1) Surgical situations: 2 patients successfully underwent LARH via Laennec approach, without conversion to open surgery. The operation time was 180 minutes and 185 minutes, and the volume of intraoperative blood loss was 200 mL and 400 mL, respectively. No blood transfusion or gastrointestinal decompression was performed in either patient. (2) Postoperative situations and follow-up: 2 patients began to take liquid diet on the first day and out-of-bed activities on the postoperative second to third day. There was no postoperative bile fistula or bleeding, but different degrees of peritoneal and pleural effusion occurred to the 2 patients after operation. One case was improved after right-sided thoracentesis and chest tube drainage due to dyspnea, and the other case was cured after conservative therapy. There was no perioperative death. The duration of postoperative hospital stay of 2 patients was 13 days and 11 days, respectively. Results of pathological examination showed 1 case of hepatic hemangioma and 1 case of primary liver cancer, respectively. The Laennec capsule was observed on the hepatic vein branches of segment Ⅴ, Ⅵ, Ⅶ, Ⅷ, and the gap existed between the Laennec capsule and the hepatic vein. Two patients were followed up for 7 months and 5 months,respectively. They survived during the follow-up,without tumor recurrence.Conclusion:It is safe and feasible to perform LARH by Laennec approach.

ABSTRACT

Borneol (Bo) and Arg-Gly-Asp (RGD) co-modified docetaxel (DTX) loaded MPEG-PLGA nanoparticles (DTX-Bo-RGD-NPs) were prepared to improve the therapeutic effect of DTX against glioma after intranasal administration. DTX-Bo-RGD-NPs were prepared by emulsification-solvent evaporation method, and their morphology, particle size, zeta potential, drug loading capacity (DLC), stability, and in vitro release properties were investigated. The fluorescence probe coumarin-6 loaded NPs were prepared for investigating the NPs' uptake property on C6 and 16HBE cell models to evaluate in vitro targeting ability. The DiR loaded NPs were prepared for observing the fluorescence intensity at the brain tumor site after intranasal administration through in vivo imaging system in a C6 rat orthotropic model, evaluating the targeting ability in vivo. The anti-tumor effects of DTX-Bo-RGD-NPs were also investigated in such C6 rat orthotropic model in vivo. Animal welfare and experimental procedures are in compliance with the regulations of the Animal Ethics Committee of Shanghai University of Traditional Chinese Medicine. The results showed that DTX-Bo-RGD-NPs were spherical and uniformly distributed, with a particle size of about 140 nm and a zeta potential of -20 to -30 mV. The drug delivery system showed good stability and sustained release property in vitro, and favorable brain tumor targeting effect in vitro and in vivo. Such novel drug delivery system significantly improved the accumulation of DTX-Bo-RGD-NPs in tumor sites and displayed a higher brain tumor targeting efficiency, providing promising therapeutics of DTX for the treatment of glioma after intranasal administration.

ABSTRACT

Objective:To study the effect and related mechanism of Fuyou granule on danazol-induced precocious puberty model in rats. Method:Totally 21 cages of SD female rats were randomly divided into normal group, model group, Leuprorelin(0.1 g·kg<sup>-1</sup>) and Fuyou mixture group(37.9 g·kg<sup>-1</sup>), and high-dose, mid-dose and low dose Fuyou granule<italic> </italic>groups(17.0,8.5,4.3 g·kg<sup>-1</sup>). Rats at 5 days of age were given a single subcutaneous injection of 300 μg danazol to establish the precocious puberty model. After 10 days of modeling, drug intervention was started. Vaginal opening was examined at the age of 20 days, and the gonadal development was observed by hematoxylin-eosin (HE) staining. The levels of serum luteinizing hormone (LH), follicle-stimulating hormone (FSH) and estradiol (E<sub>2</sub>) were determined by radioimmunoassay. The mRNA expressions of hypothalamic gonadotropin releasing hormone (GnRH), Kiss-1, G protein-coupled receptor 54 (GPR54) were detected by Real-time fluorescent quantitative polymerase chain reaction (Real-time PCR), and the expression of GnRH cells in the hypothalamus was detected by immunohistochemistry. Result:Compared with the normal group, the vaginal opening of the model group was significantly earlier, and the uterus and ovarian coefficients were significantly increased (<italic>P</italic><0.05), indicating that the danazol-induced precocious puberty model was successfully established. The expression levels of GnRH, Kiss-1, and GPR54 also increased significantly (<italic>P</italic><0.05), indicating that the danazol model can activate the HPG axis in advance, thereby inducing precocious puberty<bold>. </bold>Compared with the model group, the mid-dose Fuyou granule group significantly delayed the time of vaginal opening (<italic>P</italic><0.01), high-dose Fuyou granule group<italic> </italic>significantly reduced uterine wall thickness and uterine coefficient (<italic>P</italic><0.05,<italic>P</italic><0.01), mid-dose group reduced ovarian coefficient and uterine wall thickness (<italic>P</italic><0.05). All the three dosage groups of Fuyou granule significantly reduced the content of serum hormones E<sub>2</sub>, LH and FSH (<italic>P</italic><0.05,<italic>P</italic><0.01), reduced the expression levels of hypothalamic GnRH, Kiss-1 and GPR54 mRNA (<italic>P</italic><0.05), and decreased the expression of GnRH cells (<italic>P</italic><0.05). Conclusion:Fuyou granule can achieve therapeutic precocity by regulating the Kiss-1/GPR54 system and down-regulating the expression of GnRH to inhibit the activation of the HPG axis.

There have been many recent clinical trials on post-TKA peripheral nerve block; however, they have reported inconsistent findings. In this meta-analysis, we aimed to comprehensively analyze studies on post-TKA analgesia to provide evidence-based clinical suggestions. @*Methods@#We performed a computer-based query of PubMed, Embase, the Cochrane Library, and the Web of Science to retrieve related articles using neurothe following search terms: nerve block, nerve blockade, chemodenervation, chemical neurolysis, peridural block, epidural anesthesia, extradural anesthesia, total knee arthroplasty, total knee replacement, partial knee replacement, and others. After quality evaluation and data extraction, we analyzed the complications, visual analogue scale (VAS) score, patient satisfaction, perioperative opioid dosage, and rehabilitation indices. Evidence was rated using the Grading of Recommendations Assessment, Development, and Evaluation approach. @*Results@#We included 16 randomized controlled trials involving 981 patients (511 receiving peripheral nerve block and 470 receiving epidural block) in the final analysis. Compared with an epidural block, a peripheral nerve block significantly reduced complications. There were no significant between-group differences in the postoperative VAS score, patient satisfaction, perioperative opioid dosage, and rehabilitation indices. @*Conclusions@#Our findings demonstrate that the peripheral nerve block is superior to the epidural block in reducing complications without compromising the analgesic effect and patient satisfaction. Therefore, a peripheral nerve block is a safe and effective postoperative analgesic method with encouraging clinical prospects.

ABSTRACT

OBJECTIVE@#To investigate the characteristics of gene mutations in patients with myelodysplastic syndromes (MDS) and its prognostic significance.@*METHODS@#High-throughput sequencing was used to detect 34 blood tumor-related genes in 210 patients with MDS, and the relationship with the revised International Prognostic Scoring System (IPSS-R) and the impact on prognosis of the patients were analyzed.@*RESULTS@#Among the 210 MDS patients, 142 cases (67.6%) showed mutations, and the first six genes with the highest mutation detection rate were ASXL1(20.5%), TET2(17.1%), U2AF1(14.3%), DNMT3A (11.9%), TP53(10.5%) and RUNX1(10.0%). The gene mutation rate of the patients in IPSS-R relatively high-risk group was higher than those in relatively low-risk group (P=0.001). Both TP53 and BCOR genes showed higher mutation rates in the higher risk group than in the lower risk group (P<0.05). Survival time of the patients in TP53 mutant group was lower than those in non-mutant group (P<0.001), survival time of patients in SF3B1 mutant group was higher than those in non-mutant group (P=0.018). According to the number of gene mutations, the patients could be divided into groups with 0-1, 2 and ≥3 gene mutations, and the median OS of the three groups were not reached, 43 and 27 months, respectively (P=0.004). The Multivariate analysis showed that the increasing number of gene mutations and TP53 mutation was the independent risk factors affecting prognosis of the patients, while SF3B1 mutation was the independent protective factor for the prognosis of the patients.@*CONCLUSION@#The gene mutation rate was higher in MDS patients. And the increasing numbers of gene mutation, TP53 and SF3B1 were the influence factors of prognosis in the patients.

ABSTRACT

OBJECTIVE@#To study the antidepressant-like effect and action mechanism of geniposide and eleutheroside B combination treatment on the lipopolysaccharide (LPS)-induced depression mice model.@*METHODS@#Depression mice model was established by lipopolysaccharide (LPS) injection. Totally 48 mice were randomly divided into 6 groups (8 rats per group) according to a random number table, including normal, model, fluoxetine (20 mg/kg), geniposide (100 mg/kg) + eleutheroside B (100 mg/kg), geniposide + eleutheroside B + WAY 100635 (0.03 mg/kg), geniposide + eleutheroside B+ N-methyl-D-aspartic acid receptor (NMDA, 75 mg/kg) groups, respectively. After continuous administration for 10 days, autonomic activity tests after 30 min of administration were performed on the 10th day. On the 11th day, except for the normal group, the mice in the other groups were intraperitoneally injected with LPS (1 mg/kg), and the behavioral tests were performed 4 h later. Enzyme linked immunosorbent assay was used to detect tumor necrosis factor alpha (TNF- α) and interleukin-1 β (IL-1 β) levels in mice serum. The mRNA expression of indoleamine 2,3-dioxygenase (IDO) and nuclear transcription factor (NF- κB) were detected by real-time quantitative polymerase chain reaction. Western-blot analysis was used to detect IDO and NF- κB protein expressions in hippocampus tissue.@*RESULTS@#Compared with the normal group, a single administration of LPS increased the immobility time in the forced swimming test (FST) and tail suspension test (TST, P<0.01), without affecting autonomous activity. Compared with the model group, fluoxetine and geniposide + eleutheroside B administration significantly improved the immobility time of depressed mice in the FST and TST, decreased serum IL-1 β content, inhibited the expression levels of NF- κ B gene and protein in hippocampus tissues (P<0.05 or P<0.01). Compared with the model group, geniposide + eleutheroside B treatment significantly reduced serum TNF-α content and inhibited IDO mRNA and protein expressions in hippocampus (P<0.05 or P<0.01). In addition, NMDA partly prevented the inhibition of IDO mRNA expression by geniposide + eleutheroside B; NMDA and WAY-100635 also partly prevented the reduction of IL-1 ß content induced by geniposide + eleutheroside B treatment (P<0.05 or P<0.01).@*CONCLUSIONS@#The combination of geniposide and eleutheroside B showed a certain antidepression-like effect. Its main mechanism of action may be contributed to inhibiting the activation of NF- κB, decreasing the proinflammatory cytokines such as TNF-α, IL-1 β, and inhibiting in the neuroinflammatory reaction. Additionally, it also affects tryptophan metabolism, reduces the expression of a key enzyme of tryptophan metabolism, IDO. And this antidepressant-like effect may be mediated by 5-hydroxytryptamine and glutamate systems.

ABSTRACT

To systematically evaluate the efficacy and safety of Gongxuening Capsules in the treatment of abnormal vaginal bleeding after medical abortion. CNKI, Wanfang, SinoMed, VIP, PubMed, Cochrane Library and EMbase databases were retrieved to comprehensively collect the clinical randomized controlled trials(RCTs) of Gongxuening Capsules for treatment of abnormal vaginal bleeding after medical abortion from the establishment of the databases to October 10, 2020. Literature screening, data extraction and quality evaluation were conducted independently by two system reviewers according to the inclusion and exclusion criteria. Cochrane Handbook bias risk assessment tool was used for the literature methodology quality evaluation, RevMan 5.3 software was used for Meta-analysis, and the evidence quality of outcomes was evaluated by the evidence quality grading system(GRADE). A total of 16 RCTs were inclu-ded. The results of Meta-analysis showed that as compared with the western medicine treatment alone, the addition of Gongxuening Capsules to the western medicine treatment can reduce the amount of vaginal bleeding(RR=1.23, 95%CI[1.19, 1.27], P<0.000 01), shorten vaginal bleeding time(RR_(≤15 d number of people)=1.39, 95%CI[1.31, 1.48], P<0.000 01; MD_(number ofdays)=-1.20, 95%CI[-1.66,-0.74],P<0.000 01). However, there was no obvious advantage in abortion effect(RR=1.02, 95%CI[0.99, 1.06], P=0.14) and menstrual recovery(MD=-0.35, 95%CI[-0.96, 0.25], P=0.25). The results of GRADE showed that the grading level was low for vaginal bleeding volume and vaginal bleeding time, and extremely low for abortion effect and mens-trual recovery. In terms of safety, 16 studies reported adverse events. Only one study showed no adverse events and the rest showed transient nausea, vomiting, stomach burning, upper abdominal discomfort and other gastrointestinal symptoms. The results show that the addition of Gongxuening Capsules to the application of western medicine in treatment of drug abortion can reduce the amount of vaginal bleeding and shorten vaginal bleeding time, but the abortion effect and menstrual recovery have no obvious advantages. The use of Gongxuening Capsules helps to achieve less adverse reactions and higher safety. Due to the small sample size of the included studies and many methodological quality problems, no conclusions with clinical guidance value can be obtained. Large sample-zise, high-qua-lity randomized controlled trials are still needed for further verification.
