ABSTRACT

Background@#Local hyperthermia is recommended for the treatment of patients with fixed cutaneous sporotrichosis, though the effectiveness and mechanisms of action remain elusive. While neutrophils represent the main inflammatory cells associated with sporotrichosis lesions, the issue of whether hyperthermia is involved with interactions between neutrophils and Sporothrix globosa remains unclear. @*Objective@#To evaluate the effect of local hyperthermia on sporotrichosis and determine whether local hyperthermia involves effects of neutrophils against Sporothrix. @*Methods@#For the in vivo study, mice were infected with yeast cells of S. globosa followed by treatment with local hyperthermia. In vitro, an isolated S. globosa strain was co-cultured with or without neutrophils and subjected under different temperatures. Immunofluorescence was used to assess the formation of neutrophil extracellular trap (NETs) were formed under these different culture conditions and the number of fungi colony forming units were compared. Results: Hyperthermia was significantly more effective in clearing the lesions in the mouse model, as compared to sham treatment. Neutrophils failed to exert any fungicidal effects against S. globosa in response to hyperthermia. Moreover, NETs were formed after interactionwith S. globosa, and the percentage of NETs formed was not significantly different at 41o C or 37o C. @*Conclusion@#While hyperthermia could serve as an effective therapy for fixed cutaneous sporotrichosis, this ability does not involve the formation of NETs.

ABSTRACT

The bioactivity of a working reference standard was determined by replicate bioassays with calibration against a primary reference standard. In this study the number of bioassay replicates needed for calibration first was calculated theoretically, and if the mean value of the experimental bioassay replicates fell within the predefined bioactivity level the bioactivity of the working reference was defined as 100%. Our results showed that when the total intermediate precision of the bioassay method was at 11.66% and the predefined bioactivity level was set at 95%-105% with a confidence level of 95%, 21 bioassay replicates should be carried out for calibration. The average value of the 22 experimental bioassay replicates was 101.96%, so the bioactivity of the working reference standard was consistent with that of the primary reference standard at 100%. The results suggest that a strategy of first calculating the number of bioassay replicates needed for calibration and then determining whether the resulting experimental mean value is within the predefined bioactivity level will be of value to the biopharmaceutical industry.

ABSTRACT

The high performance liquid chromatography-fluorescence micelle assay (HPLC-FMA) method for the content determination of polysorbate 80 in monoclonal antibody drugs was validated to study its applicability and transferability between various laboratories, and the feasibility to be included in the Chinese Pharmacopoeia. Both J.T. Baker and Nanjing Well-sourced polysorbate 80 was used in the collaborative validation of polysorbate 80 content analysis in seven different laboratories. The results show that when the protein concentration was no more than 20 mg·mL-1 and the concentration of polysorbate 80 ranged from 0.05 to 0.5 mg·mL-1, the method had good specificity. The recovery rates of the spiked samples ranged from 92.20% to 117.70% for J.T.Baker and from 93.90% to 117.20% for Nanjing Well. The intra-laboratory precision (%RSD) was less than 4.30% for J.T. Baker, and less than 2.60% for Nanjing Well, while the overall precision was less than 5.45% for J.T. Baker, and less than 6.70% for Nanjing Well. The linear correlation coefficient was more than 0.98 for J.T. Baker and more than 0.99 for Nanjing Well. The results of the collaborative validation prove that the HPLC-FMA method has good accuracy, precision, linearity, and specificity, and could be used for release control analysis of polysorbate 80 content in monoclonal antibodies across different laboratories.

ABSTRACT

Objective:This study aimed to investigate the time duration of norovirus shedding among day-care children and students during norovirus outbreaks, as well as influencing factors affecting the viral shedding. Methods:Suspected cases of norovirus infection and their close contacts were collected from child care and school settings during norovirus outbreaks in Xuhui District, Shanghai, from 2017 through 2019. Specimens were detected using real-time RT-PCR to determine whether children had been infected with norovirus. Subsequently, further specimens were collected every 3-7 days from infected children until specimens tested negative for norovirus. Results:A total of 76 outbreaks were reported involving 1 014 suspected cases. In the 421 suspected cases, 311 confirmed cases were diagnosed after examination. Furthermore, a total of 58 confirmed cases participated in this study with informed consent, with a participation rate of 18.65%. The average time duration of norovirus shedding was (16.24±13.80) days, in which 79.31% had viral shedding more than 7 days, 37.93% more than 14 days and 17.24% more than 21 days. A Cox proportional-hazards model showed that children with more severe symptoms (HR=2.06,P=0.040), day-care children (HR=4.13,P=0.012), and confirmed cases in 2019 (HR=0.11,P<0.001) had longer duration of viral shedding. Conclusion:Children may remain shedding norovirus after their recovery and back to class. Improvement in sanitation for these recovered children in child care and schools is especially necessary, which may avert secondary transmission.

ABSTRACT

Objective:To explore the clinical significance of combined detection of the promoter methylation of plasma free Septin9, SDC2 and BCAT1 genes in peripheral blood for the diagnosis of colorectal cancer. Methods The data of patients admitted to the Department of Gastroenterology, Shanghai East Hospital Affiliated to Tongji University from January to September 2019 were retrospectively analyzed. They were divided into colorectal cancer group (62 cases of colon cancer, 59 cases of rectal cancer), precancerous lesions group (77 cases of colorectal adenoma, 5 cases of high-grade intraepithelial neoplasia), interference group (61 cases of colorectal cancer and advanced adenoma negative but suffered other intestinal lesions, 17 cases of non-colorectal cancer) and healthy group (94 cases). The methylation status of three genes (Septin9, SDC2 and BCAT1) in peripheral blood plasma was detected simultaneously by fluorescence PCR. The relationship between the positive rate of three genes detected jointly and the clinic pathological characteristics of colorectal cancer was analyzed and compared with serum carcinoembryonic antigen (CEA) positive rate. The colorectal cancer group was divided into stage Ⅰ, Ⅱ, Ⅲ and Ⅳ according to TNM stage, and the colorectal cancer group was analyzed and counted by grade. The diagnostic efficiency of detection methods was analyzed by receiver operating characteristic (ROC) curve, and the area under ROC curve (AUC) was compared. Results:The positive rate of combined detection of SDC2 and BCAT1 gene methylation was higher than other three groups (χ 2 =237.246, P<0.001). The positive rate of combined detection of plasma Septin9, SDC2 and BCAT1 gene methylation was higher than CEA in colorectal cancer group ( P<0.001). The positive rates of the combined detection of plasma Septin9, SDC2 and BCAT1 gene methylation in stage Ⅰ-Ⅳ of colorectal cancer group were 73%(16/22), 87%(34/39), 86%(30/35) and 96%(24/25), respectively. Compared with CEA group, the positive rate of combined detection of plasma Septin9, SDC2 and BCAT1 gene methylation in stage Ⅰ-Ⅲ of colorectal cancer group was higher than serum CEA ( P<0.001), but the positive rate of stage Ⅳ was not statistically significant compared with CEA group ( P>0.05). ROC curve analysis showed that the AUC of Septin9, SDC2 and BCAT1 was 0.857(95% CI 0.810-0.903),0.819(95% CI 0.768-0.871)and 0.862(95% CI 0.816-0.909), respectively. The AUC of combined detection of three gene methylations was 0.889 (95% CI 0.846-0.933), and the AUC of combined detection with serum CEA was 0.913 (95% CI 0.874-0.951). There was no significant difference in the positive rate of combined detection of Septin9, SDC2 and BCAT1 gene methylation among different gender, age and cancerous site of colon cancer patients (all P>0.05). Conclusion:The combined detection of the promoter methylation of plasma free Septin9, SDC2 and BCAT1 genes in peripheral blood plasma is helpful for the early diagnosis of colorectal cancer. The positive rate in stage Ⅰ-Ⅲ of colorectal cancer group is higher than serum CEA. The combined diagnosis of the three genes can improve the diagnostic efficiency.

ABSTRACT

Objective:To investigate the effect of pelvic motion in late pregnancy on the outcome of first trimester delivery. Methods:A total of 127 first-borns admitted to the midwifical clinic of Nanjing Maternal and Child Health Hospital from November 1, 2019 to March 10, 2020 were selected as the research objects, and they were divided into the walking group and the pelvic motion group according to the random number table method. In the end, 62 cases were included in the walking group and 65 cases in the pelvic motion group. The walking group did brisk walking in the third trimester. In the pelvic motion group, the motion in the third trimester consisted of the combined pelvic motion involving the two planes of the upright spine and the horizontal spine, and multiple peripelvic joints. The changes of fertility, birth canal and fetus before and after exercise were observed, and the outcomes of childbirth were compared between the two groups. Results:Indicators reflecting productivity: persistence time of "air bike" on the day before delivery was (27.12±7.73) seconds in the pelvic motion group and (20.56±6.60) seconds in the walking group, the difference was statistically significant ( t value was 5.12, P<0.01). The time of contractions after exercise before delivery was (269.38±123.70) seconds in the pelvic motion group and (591.29±201.82) seconds in the walking group, the difference was statistically significant ( t value was 10.89, P<0.01). The indicators reflecting the situation of the maternal birth canal were as follows: "sitting forward flexion level", pelvic motion group was (33.64±5.91) cm, walking group was (29.50±5.28) cm, the difference was statistically significant ( t value was 4.15, P<0.01). The cervical score during regular contractions was 8.01±1.69 in the pelvic motion group and 7.30±1.40 in the walking group, the difference was statistically significant( t value was 2.56, P<0.05). At the beginning of regular contractions, the proportion of fetal "occipital position" was 93.85% (61/65) in the pelvic motion group and 72.58%(45/62) in the walking group, the difference was statistically significant ( χ2 value was 10.41, P<0.01). The "no pain rate after exercise the day before delivery" was 72.31% (47/65) in the pelvic motion group and 29.03% (18/62) in the walking group, the difference was statistically significant ( χ2 value was 25.30, P<0.01). Time of the first labor was (395.84±165.24) min in the pelvic motion group and (574.35±152.23) min in the walking group, the difference was statistically significant ( t value was 6.32, P<0.01). The second stage of labor was (25.98±9.88) min in the pelvic motion group and (33.62±13.94) min in the walking group, the difference was statistically significant ( t value was 3.57, P<0.01). Conclusions:Pelvic motion in middle and late pregnancy can affect labor force, birth canal, fetus and other delivery factors, reduce movement pain and shorten the time of labor.

ABSTRACT

Polysaccharides are macromolecular compounds formed by more than 10 monosaccharide molecules linked by glycosidic bonds. Polysaccharides have a wide range of sources, high safety and low toxicity, with a variety of biological activities, such as anti-tumor, anti-virus, immune regulation, lowering blood glucose, and lowering blood lipids. Type 2 diabetes mellitus(T2 DM) is a chronic metabolic disorder characterized by hyperglycemia, insulin resistance and low inflammation. In recent years, the treatment of T2 DM with polysaccharide has become a research hotspot. Polysaccharides can not only make up for the side effects such as hypoglycemia, weight gain, gastrointestinal injury caused by long-term treatment of acarbose, biguanidine and sulfonylurea, but also play an effective role in reducing glucose by regulating glucose metabolism, oxidative stress, inflammatory response, intestinal flora, etc. In this paper, the research progress of polysaccharides in the treatment of T2 DM was reviewed. In addition, the hot spots such as the hypoglycemic activity of polysaccharides with structural modifications were summarized, providing theoretical guidance for the development of active polysaccharide hypoglycemic medicines and the further study of action mechanism.

ABSTRACT

In recent years, unconventional fractionated radiotherapy has shown increasing advantages in the treatment of multiple system tumors. Simultaneous integrated boost intensity-modulated radiotherapy (SIB-IMRT) involves the delivery of standard-fraction doses of radiotherapy to different areas, achieving the delivery of higher doses of radiotherapy per fraction to the high-risk gross tumor volume (GTV) without sacrificing the irradiation dose to the normal tissues. The dosimetric advantages of SIB-IMRT have been widely recognized. At present, the local control, survival advantage, indication population and the optimal upper limit of single fraction of SIB-IMRT for esophageal carcinoma are still unclear. This article reviews the application of SIB-IMRT in esophageal carcinoma.

ABSTRACT

Objective:To investigate the clinical efficacy of superior hypogastric nerve block (SHNB) in relieving pelvic pain after uterine artery embolization (UAE).Methods:Totally 50 patients of UAE before the curettage of uterine incision pregnancy were selected in Affiliated Zhenjiang Fourth People′s Hospital,Jiangsu University from February 2019 to December 2020 and were randomly divided into SHNB group( n=22) and control group( n=28) using random number table. The SHNB group underwent fluoroscopy-guided SHNB before UAE, and the control group received dizocine 10 mg intramuscularly before embolization. Both groups were treated with curettage 24 h after UAE. The pain scores were evaluated by using a numerical rating scale (NRS) to compare the pain scores between the two groups atthe time period A1 (from the beginning of UAE to immediate postoperative period) and at the time period A2 (from leaving the interventional operating room to the time before curettage). Data was recorded and compared between the two groups at the time period from the beginning of UAE surgery to the time before curettage for the doses of opioids used. The differences inage and weight between the two groups were compared by independent sample t test, and the NRS score and morphine dose were compared by Mann-whitney U test. Results:All patients completed SHNB and UAE without serious complications. There was no significant difference in age and weight between the two groups ( P>0.05).The maximum pain scores in the SHNB group were lower than those in the control group at both the A1 and A2 time periods[0(0, 0.25) vs. 3.00 (2.00, 4.00), and 2.50 (0.75, 5.50) vs. 4.00 (3.25, 7.00); Z=-4.932, -2.351, P<0.05]. The equivalent dose of morphine required in the SHNB group was lower than that of the control group [0(0, 10.00) vs. 10.00 (5.00, 15.00)mg, Z=-2.247, P=0.025]. Conclusion:Fluoroscopic-guided SHNB is a safe, effective, and minimally invasive way to reduce pain and the opiate dose after UAE.

The patients in the control group were treated with conventional dietary nursing measures after thyroidectomy. The patients in the intervention group were treated with dietary guidance according to the results of quantifying food thickness and conventional dietary nursing. The incidence of aspiration was compared during hospitalization, the swallowing function were evaluated in the two groups pre intervention and on discharge, and relationship was evaluated between Kubota drinking test level and food consistency grade of safe eating in intervention group. Results:The incidence of aspiration in the intervention group (0) was lower than that in the control group 28.57% (10/35) and the difference was statistically significant ( χ2 value was 11.667, P<0.01). Before intervention the difference in the degrees of Kubota drinking test had no statistical significance between two groups before intervention ( Z value was -0.283, P>0.05). After interventions the degrees of Kubota drinking test was graded as gradeⅠ(1 example), gradeⅡ(4 examples), gradeⅢ(20 examples), grade Ⅳ(9 examples), gradeⅤ(1 example) respectively, meanwhile the degrees of control group was graded as gradeⅠ(0 example), gradeⅡ(1 example), gradeⅢ(16 examples), grade Ⅳ(17 examples), gradeⅤ(1 example) respectively. The difference between two groups was statistically significant ( Z value was -2.170, P<0.05). Moreover, the improvement of swallowing function in the intervention group was larger than that of the control group and the difference in the degree of the swallowing function was statistically significant between two groups ( Z value was -2.029, P<0.05). Before intervention the difference of the score of Eating Assessment Tool-10 had no statistical significance between two groups ( t value was 0.168, P>0.05). After intervention the score of Eating Assessment Tool-10 in the intervention group was 8.43±2.21, the score of Eating Assessment Tool-10 in the control group was 10.06±2.45, the difference had statistical significance between two groups( t value was -2.919, P<0.01). Conclusion:Quantification of fluid food thickness can more accurately and objectively judge the grade of food consistency that patients can eat safely, and it can effectively reduce the incidence of aspiration and contribute to improving the swallowing function in patients with swallowing difficulty after thyroidectomy and can also help them to recovery.

ABSTRACT

ObjectiveTo describe the epidemiological characteristics of confirmed COVID-19 cases in Shangrao City, and provide scientific evidence for the development of prevention and control strategies. MethodsEpidemiological characteristics of 123 confirmed cases, such as time, sex, age, occupations, area, and source of infection, were analyzed by combining epidemiological investigation with laboratory testing. ResultsOf the 123 confirmed cases, 34 were imported cases and 89 were local cases. The sex ratio was 1.32∶1. Average time interval of the COVID-19 was (18.4±4.4) days, with a median time interval between the onset and final diagnosis of 5 days. There were 19 clustering cases, of which 89.5% were family clusters and 77.3% occurred in families. Majority of the cases were common type. The main source of infection was travelers from the epidemic areas. ConclusionHealth monitoring of travelers from epidemic areas and strict quarantine of close contacts are the main measures to control the spread of the COVID-19 epidemic.
