ABSTRACT

Chronic refractory wound (CRW) is one of the most challengeable issues in clinic due to complex pathogenesis, long course of disease and poor prognosis. Experts need to conduct systematic summary for the diagnosis and treatment of CRW due to complex pathogenesis and poor prognosis, and standard guidelines for the diagnosis and treatment of CRW should be created. The Guideline forthe diagnosis and treatment of chronic refractory wounds in orthopedic trauma patients ( version 2023) was created by the expert group organized by the Chinese Association of Orthopedic Surgeons, Chinese Orthopedic Association, Chinese Society of Traumatology, and Trauma Orthopedics and Multiple Traumatology Group of Emergency Resuscitation Committee of Chinese Medical Doctor Association after the clinical problems were chosen based on demand-driven principles and principles of evidence-based medicine. The guideline systematically elaborated CRW from aspects of the epidemiology, diagnosis, treatment, postoperative management, complication prevention and comorbidity management, and rehabilitation and health education, and 9 recommendations were finally proposed to provide a reliable clinical reference for the diagnosis and treatment of CRW.

ABSTRACT

Bardet-Biedl syndrome (BBS) is an autosomal recessive hereditary rare disease with high heterogeneity in clinical phenotypes, which can affect multiple systems, such as six fingers/digits deformity, mental impairment, retinopathy, polycystic kidney, etc, and is easily diagnosed according to a single system disease, resulting in misdiagnosis. The paper reports a case of BBS, diagnosed with "renal cyst, developmental delay, chondrosis, and left foot hexadactylism" at 5 years old, "anemia, renal insufficiency, distal renal tubule poisoning, renal osteodystrophy" at 7 years old. At 10 years old, the patient received hemodialysis treatment for uremia. Cloudy vision appeared in both eyes at 14 years old. Because of the prominent manifestation of kidney damage, the patient was misdiagnosed as "polycystic kidney" for a long time, and other systemic damages were ignored. The result of gene sequencing showed that chromosome 16 NM_031885.5 exon17 had one homozygous nonsense mutation. Therefore, the patient was accurately diagnosed as BBS. This paper is the first report of BBS in Li nationality in China. The relevant literature of BBS were reviewed to strengthen clinicians' understanding of the disease and improve patients' prognosis.

ABSTRACT

Objective:To explore the relation of plasma Elabela with 3-month prognoses in large vessel occlusion-acute ischemic stroke (LVO-AIS) patients accepted endovascular thrombectomy (EVT).Methods:A prospective study was performed; 94 LVO-AIS patients aceepted EVT in Department of Neurology, Anhui Provincal Hospital, Anhui Medical University from August 2020 to August 2022 were selected. Plasma Elabela was detected before EVT, and 24 and 72 h after EVT. Modified Rankin scale (mRS) was used to evaluate the prognoses of the patients 3 months after EVT; differences in clinical data and plasma Elabela level between the good prognosis group and poor prognosis group were compared. Independent influencing factors for prognoses of LVO-AIS patients 3 months after EVT were analyzed by multivariate Logistic regression. Receiver operating characteristic (ROC) curve was used to analyze the efficacy of Elabela in predicting prognoses of patients with LVO-AIS 3 months after EVT.Results:Compared with the poor prognosis group, the good prognosis group had significantly lower percentages of patients with stroke history and diabetes, and lower NIHSS scores at admission ( P<0.05). Elabela level in the good prognosis group was significantly higher than that in the poor prognosis group 72 h after EVT ( P<0.05). Multivariate Logistic regression analysis showed that stroke history ( OR=0.148, P=0.037, 95% CI: 0.025-0.889), diabetes mellitus ( OR=0.148, P=0.037, 95% CI: 0.025-0.889), hypertension history ( OR=3.488, P=0.024, 95% CI: 1.177-10.339), and Elabela level 72 h after EVT ( OR=1.064, P=0.005, 95% CI: 1.019-1.111) were independent influencing factors for prognoses of LVO-AIS patients 3 months after EVT. ROC curve showed that area under ROC curve of plasma Elabela level 72 h after EVT in predicting prognosies of LVO-AIS patients 3 months after surgery was 0.718 ( P<0.001, 95% CI: 0.614-0.822). Conclusion:Plasma Elabela level 72 h after EVT may be a potential prognostic biomarker for LVO-AIS patients after EVT.

ABSTRACT

LIN28 is an RNA binding protein with important roles in early embryo development, stem cell differentiation/reprogramming, tumorigenesis and metabolism. Previous studies have focused mainly on its role in the cytosol where it interacts with Let-7 microRNA precursors or mRNAs, and few have addressed LIN28's role within the nucleus. Here, we show that LIN28 displays dynamic temporal and spatial expression during murine embryo development. Maternal LIN28 expression drops upon exit from the 2-cell stage, and zygotic LIN28 protein is induced at the forming nucleolus during 4-cell to blastocyst stage development, to become dominantly expressed in the cytosol after implantation. In cultured pluripotent stem cells (PSCs), loss of LIN28 led to nucleolar stress and activation of a 2-cell/4-cell-like transcriptional program characterized by the expression of endogenous retrovirus genes. Mechanistically, LIN28 binds to small nucleolar RNAs and rRNA to maintain nucleolar integrity, and its loss leads to nucleolar phase separation defects, ribosomal stress and activation of P53 which in turn binds to and activates 2C transcription factor Dux. LIN28 also resides in a complex containing the nucleolar factor Nucleolin (NCL) and the transcriptional repressor TRIM28, and LIN28 loss leads to reduced occupancy of the NCL/TRIM28 complex on the Dux and rDNA loci, and thus de-repressed Dux and reduced rRNA expression. Lin28 knockout cells with nucleolar stress are more likely to assume a slowly cycling, translationally inert and anabolically inactive state, which is a part of previously unappreciated 2C-like transcriptional program. These findings elucidate novel roles for nucleolar LIN28 in PSCs, and a new mechanism linking 2C program and nucleolar functions in PSCs and early embryo development.

ABSTRACT

Ultra-high performance liquid chromatography coupled with quadrupole/time-of-flight tandem mass spectrometry (UPLC-Q/TOF-MS/MS) has been used to detect the metabolites of schaftoside in plasma, bile, urine and feces of mice after oral administration. The study was approved by the Experimental Animal Ethics Committee from Xuzhou Medical University (No. XZMULL201612024). Compounds were identified by analyzing their high-resolution mass spectrometry data, mass spectra, and comparison with reference substances and the literatures. The parent compound and 29 metabolites were detected in the plasma, bile, urine and feces samples of mice. The main metabolic pathways of schaftoside in mice include deglycosylation/glycosylation, hydroxylation/dehydroxylation, hydrogenation, methylation, acetylation, sulfation, and glucuronidation. This study provides references for the material basis of schaftoside in vivo.

ABSTRACT

Objective:To investigate the influencing factors for prognoses of patients with acute basilar artery occlusion (BAO) receiving bridging therapy (intravenous thrombolysis [IVT] combined with endovascular treatment [EVT]).Methods:The clinical and imaging data of 404 patients with acute BAO accepted bridging therapy were collected from Endovascular Treatment for acute Basilar Artery Occlusion (ATTENTION) database. The prognoses of these patients were evaluated by modified Rankin Scale (mRS) 90 d after treatment. The differences of clinical and imaging data between patients with good prognosis and poor prognosis were compared, and the independent influencing factors for prognoses were analyzed by multivariate Logistic regression. Results:Among 404 patients with acute BAO, 156 (39%) had good prognosis and 248 (61%) had poor prognosis. Compared with the good prognosis group, the poor prognosis group had significantly higher proportions of patients older than 65 years, patients with NIHSS scores>20 on admission, and patients with thrombectomy frequency>1, significantly lower proportion of patients with successful vascular recanalization, and significantly different distribution of occlusive sites ( P<0.05). Multivariate Logistic regression analysis showed that NIHSS scores on admission>20 ( OR=0.510, 95%CI: 0.331-0.786, P=0.002), and occlusion site at the distal basilar artery ( OR=2.241, 95%CI: 1.357-3.702, P=0.002), thrombectomy frequency>1 ( OR=0.523, 95%CI: 0.339-0.807, P=0.003) and successful vascular recanalization ( OR=2.022, 95%CI: 1.032-3.961, P=0.040) were independent influencing factors for good prognosis of these patients. Conclusion:Acute BAO patients with NIHSS scores>20 on admission, thrombectomy frequency>1, non-distal BAO occlusion, and unsuccessful recanalization have poor prognosis after bridging therapy.

ABSTRACT

In China, peritoneal metastasis of gastric cancer has main characteristics of high incidence, late staging and poor prognosis. However, the proposal of conversion therapy has brought hope to patients.Conversion therapy of peritoneal metastasis in gastric cancer is a novel concept, which aims at down-staging of the gastric cancer's primary lesion and effectively controlling the peritoneal metastases at the same time through valid chemotherapy and other means. Then the surgeons strive for performing radical gastrectomy and lymph node dissection (D2) to prolong survival time of the patients with advanced gastriccancer and improve their life quality.Systemic chemotherapy is the core of the methods of conversion therapy, while the local intraperitoneal chemotherapy can be used as a supplement.Neoadjuvant intraperitoneal-systemic chemotherapy (NIPS) is the most promising technique as conversion therapy due to the comprehensive advantages of the systemic chemotherapy and local intraperitoneal chemotherapy. In recent years, there were many clinical studies reporting NIPS for conversion therapy of peritoneal metastasis in gastric cancer. Therefore, this paper systematically reviews experiences of clinical application in order to provide references for clinical practice of conversion therapy in gastric cancer.

ABSTRACT

BACKGROUND:At present, most of the experiments on the treatment of skin wound by mesenchymal stem cells have been performed in rats, mice and rabbits, while the research on skin wound treatment by canine bone marrow mesenchymal stem calls is less reported.OBJECTIVE:To observe the effect of canine bone marrow mesenchymal stem calls on skin wound healing.METHODS:A 3 cmx3 cm wound was made on the both sides of the scapula and buttocks of the dog, with the right side as experimental group and the left side as control group. After the wound was made, allogeneic canine bone marrow mesenchymal stem calls suspension was injected subcutaneously around the wound in the experimental group on the 1st and 3rd days. The control group was injected subcutaneously around the wound with mesenchymal stem call culture medium on the 1st and 3rd days after the wound was made.Wound healing was observed dynamically in both groups.RESULTS AND CONCLUSION:In the 1st week, there were pale yellow inflammatory substances in the wound of two groups indicating obvious inflammations.Compared with the control group, the inflammatory substances were fewer and the growth rate of the granulation tissue was faster in the experimental group. From the 2nd week until the wound healing, epithelialization on the wound became obvious following the formation of the granulation tissue, which was mainly displayed by the formation of fresh epithelial tissues from the surrounding to the wound center. The epithelialization time of the experimental group was earlier than that of the control group, and the wound area of the experimental group was smaller than that of the control group. In the 3rd week, the wound in the experimental group healed completely, and became smoother than that in the control group. The wound area of the experimental group was slightly smaller than that of the control group on the 8th and 12th days after cell transplantation, and the healing speed of the experimental group was slightly faster than that of the control group, but there was no significant difference between the two groups. Our findings indicate that the transplantation of canine bone marrow mesenchymal stem cells has the possibility or trend to promote skin wound healing.
