ABSTRACT

The concept of spread through air spaces (STAS) was first proposed in the World Health Organization (WHO) Classification of Tumors of the Lung, Pleura, Thymus and Heart (version 2015). STAS is defined as the micropapillary clusters, solid nests or single cells of tumor that exist in the air spaces of the surrounding lung parenchyma beyond the edge of the main tumor. Meanwhile, apart from the traditional invasion modes of lung adenocarcinoma (interstitial, visceral pleura and lym-phovascular invasion), STAS has been identified as the fourth invasion mode of lung adenocarcinoma. In recent years, the research on STAS has been a hot spot in the field of lung adenocarcinoma. The existence of STAS is related to lung cancer histopathology, gene mutation and other factors, and many studies have also confirmed that it can be used as an independent factor for tumor recurrence and prognosis. However, according to some studies, human factors can cause morphological artifacts of STAS, which still needs to be distinguished in clinical work. This paper reviews the research progress of STAS classification, related pathological features, genetic status changes, and human factors that may cause STAS artifacts.

ABSTRACT

Objective:To explore the effect of cognitive-behavioral therapy on fear of hypoglycemia and self-management behavior of patients with steroid diabetes mellitus.Methods:A total of 76 patients with steroid diabetes mellitus from August 2018 to October 2019 in the First Affilated Hospital of Nanjing Medical University were selected and divided into experimental group and control group according to the admission time, and each group contained 38 cases. The control group recieved routine therapy and nursing care, on the basis of this, the experimental group was given cognitive-behavioral therapy for 8 weeks. Before and after 8 weeks of intervention, the effects were assessed by Chinese version Hypoglycemia Fear Surgey-Worry Scale(CHFSⅡ-WS) and Sumary of Diabetes Self Care Activities (SDSCA), respectively.Results:There was no significant difference in the score of CHFS Ⅱ-WS, SDSCA before intervention between the two groups( P>0.05). After 8 weeks of intervention, the scores of worry & fear and total CHFS Ⅱ-WS scores were (6.61±1.66), (8.75±1.73) points in the experimental group and (7.60±1.44), (9.69±1.77) points in the control group, the differences were statistically significant ( t values were 2.678, 2.247, P<0.05); in addition, the scores of diet, exercise, blood sugar testing, medications in SDSCA were (20.36±1.91), (11.86±2.27), (7.19±1.56), (6.86±0.99) points in the experimental group, and (18.51±1.50), (9.89±2.29), (5.71±1.45), (5.31±1.18) points in the control group, the differences were statistically significant ( t values were 3.644-5.983, P<0.05). Conclusions:Cognitive-behavioral therapy can effectively alliviate fear of hypoglycemia and promote self-management behavior of steroid diabetes mellitus patients.

ABSTRACT

Chronic lymphocytic leukemia (CLL) is an oncologic disease with clonal proliferation of lymphocytes. In recent years, great advances have been made in the basic and clinical research of CLL. The 62nd American Society of Hematology Annual Meeting in 2020 presented a detailed report on the latest advance in basic research on these diseases, providing a deeper understanding of how CLL occurs and develops, and playing a guidance role in the clinical treatment.

ABSTRACT

Peripheral T-cell lymphoma (PTCL) is a relatively common subtype of non-Hodgkin lymphoma in China. PTCL is clinically highly aggressive, and it progresses rapidly. The current treatment methods are ineffective and the overall prognosis is poor. The 62nd American Society of Hematology Annual Meeting reported on the progress of PTCL molecular targeted therapy and immunotherapy, including programmed death receptor 1/programmed death receptor ligand 1 antibodies, JAK inhibitors, brentuximab vedotin, etc. These novel drugs bring a better prospect for patients.

ABSTRACT

OBJECTIVE@#To observe the clinical and laboratory characteristics of adult Langerhans cell histiocytosis(LCH) patients and to analyze the influencing factors of its prognosis.@*METHODS@#The clinical and laboratory charac-teristics of 38 adult LCH patients treated in our hospital from January 2010 to August 2019 were retrospective analyzed, and the clinical prognosis of the patients was analyzed.@*RESULTS@#The median age of 38 patients was 41 (21-65) years old, and the ratio of male and female was about 2∶1. Among 38 patients, 44.7% (17/38) were involved in multiple systems, and 31.6% (12/38) were involved in high-risk organs (including liver, lung, hematopoietic system or spleen). The bone involvement was the most common (21/38, 55.3%), and the most common clinical symptom was pain (19/38, 50.0%). The result of laboratory showed that anemia (4/38,10.5%), thrombocytopenia (1/38,2.6%), neutropenia (2/38,5.3%), lymphopenia (6/38,15.8%), monocytosis (11/38,28.9%), C-reactive protein increasing (6/21,28.6%), erythrocyte sedimentation rate increasing (10/18, 55.3%), and ferritin protein increasing (9/17, 55.3%). The median follow-up time was 53 months, and a total of 5 patients were died. The 10-year overall survival rate of patients with single-system involvement was 100%, which was significantly higher than that of patients with multiple-system involvement (70.1%) (P=0.0078). The prognosis of patients without risk-organ involvement was better than that of patients with risk-organ involvement (10-year overall survival rate: 100% vs 60.6%) (P=0.0007). Further analysis showed that in addition to multiple-system involvement and risk-organ involvement, the increase of peripheral blood monocyte cells and the increase of ferritin protein were also associated with poorer prognosis of the patients.@*CONCLUSION@#The multiple system involve-ment and risk-organ involvement, the increasing of monocyte cells and the increasing of ferritin protein were the independent risk factors of adult LCH patients.

ABSTRACT

Objective::To investigate the effect of bergapten on the apoptosis of HepG2 and Hep3B cells through phosphatidylinositol 3 kinase (PI3K)/protein kinase B(Akt) pathway. Method::Bergamot (5, 50, 200 μmol·L-1) groups and blank group were set up. The effect of bergapten at different concentrations on proliferation of HepG2 and Hep3B cells for 24, 48 h were detected by thiazolyl blue(MTT) assay. Apoptosis was detected by Annexin V-FITC/propidium iodide double staining. Quantitative real-time fluorescence reverse transcriptional polymerase chain reaction (Real-time PCR) and Western blot assay were used to detect relevant mRNA and proteins expressions. The clone formation rate and the effect of HepG2 and Hep3B cells in each group were evaluated by plate cell clone formation. Result::MTT assay showed that bergapten could significantly inhibit the proliferation activity of HepG2 and Hep3B cells in a time-dependent manner. Flow cytometry analysis showed that bergapten in 200 μmol·L-1 concentration groups had significant pro-apoptotic effect on HepG2 and Hep3B cells after 48 h (P<0.05). Western blot results showed that bergamolactone could up-regulate the protein expressions of Caspase-3, Caspase-8 (P<0.05), and down-regulate protein expressions of B-lymphocytoma-2 (Bcl-2), PI3K (P<0.05). Real-time PCR results showed that mRNA expressions of PI3K and Akt were decreased(P<0.05). The results of plate cell clone formation experiment showed that with the increase of the concentration of bergamolide, the cell clone formation rate of each group showed a decreasing trend, particularly in 200 μmol·L-1 concentration group (P<0.05). Conclusion::Bergapten can inhibit the proliferation of HepG2 and Hep3B cells, which may be induced through the PI3K/Akt signaling pathway.

ABSTRACT

Chronic lymphocytic leukemia (CLL) is a malignant disease of the lymphatic and hematopoietic systems with monoclonal and mature small lymphocytes. Although CLL is an indolent leukemia, its clinical outcome is highly heterogeneous, the high-risk patients have a rapid clinical progression, and are prone to relapse and drug resistance, and have a short survival time. Therefore, it is particularly important to accurately judge the prognosis of patients and carry out the stratified treatment. In recent years, although the combination of immunochemotherapy and ibrutinib has a good effect on the overall CLL patients, the clinical efficacy for high-risk CLL patients is still not satisfactory. This article mainly reviews the changes in the diagnosis of high-risk CLL and the treatment progress in the era of novel agents.

ABSTRACT

Objective To investigate the characteristics of primary mediastinal large B-cell lymphoma (PMBL) in 18F-fluorodeoxyglucose (FDG) PET/CT imaging.Methods From July 2010 to April 2019,18F-FDG PET/CT images of 27 patients (10 males,17 females,median age 31 (19-57) years)with pathologically confirmed PMBL from the First Affiliated Hospital of Nanjing Medical University were retrospectively analyzed.The location,shape,density,presence of necrosis and calcification,and invasion around or beyond the lesions were observed.The maximum standardized uptake value (SUVmax),metabolictumor volume (MTV) and total lesion glycolysis (TLG) were measured by automatic segmentation algorithm method.Spearman correlation analysis was used to evaluate the correlation between SUVmax or MTV or TLG and the maximum diameter or Ann Arbor staging.Results The lesions appeared as anterior mediastinal huge masses in 27 patients,and grew in the anterior mediastinal cross-regionally in 25 patients,lobulated at the edge in 24 patients.Low-density necrosis lesions were found in 18 patients.The lesions were surrounded by large blood vessels in 15 patients and tracheae were compressed in 12 patients.Lung tissues were invaded in 3 patients,abdominal lymph nodes and bone marrow were invaded in 1 patient,and no splenomegaly was found in 27 patients.The maximum diameter,SUVmax,MTV and TLG were (11.6±3.7) cm,21.07(15.78,25.09),190.43 (130.14,350.75) cm3 and 2165.54 (1465.86,4185.21) g,respectively.There was no correlation between SUVmax and the maximum diameter of lesions (rs =-0.305,P =0.122),while MTV and TLG were positively correlated with the maximum diameter (rs values:0.741,0.532,both P<0.05).The maximum diameter,MTV and TLG were positively correlated with Ann Arbor staging (rs values:0.394,0.413,0.422,all P<0.05),while SUVmax was not (rs=0.031,P>0.05).Conclusions PMBL mostly presents as large anterior mediastinal mass with the high 18F-FDG uptake in 18F-FDG PET/CT imaging,and the focal necrosis is common,while abdominal lymph nodes,spleen and bone marrow invasion are rare.MTV and TLG of lesions positively correlate with Ann Arbor staging.

ABSTRACT

Objective:To investigate the effect of Roy adaptation model-based intervention on fatigue and posttraumatic growth in patient with malignant lymphoma during chemotherapy.Methods:A total of 110 malignant lymphoma patients who undergoing chemotherapy were randomly divided into study group and control group, with 59 cases in each group. The control group received routine nursing method, the study group carried out Roy adaptation model-based intervention. The post-traumatic growth inventorynventory (PTGI) and the revised piper fatigue scale (PFS-R) was evaluated the effect of intervention, respectively.Results:Before and after chemotherapy, the new possibilities, relating to others, personal strength, appreciation of life, spiritual change and total PTGI scores were (8.90±1.00), (17.27±3.66), (15.11±2.63), (15.23±4.19), (4.16±0.87), (60.45±6.49) and (9.64±1.38), (21.49±4.43), (17.78±4.17), (19.31±5.82), (4.95±0.89), (73.16±8.85), in addition, those index scores in the control group were [(8.74±1.40), (17.13±3.86), (15.46±3.55), (15.80±2.81), (4.26±1.15), (61.43±6.93) and (9.14±1.86), (18.44±4.31), (16.34±2.77), (17.97±3.18), (4.74±1.07), (66.63±6.68)]. There was no significant difference in PTGI scores between study group and control group ( P>0.05), however, relating to others, personal strength and total PTGI scores were significantly increased in the study group compared to the control group ( P<0.05). Before and after chemotherapy, the sensory, emotional, knowledge, behavioral and total PFS-R scores were (5.68±0.82), (4.80±1.06), (4.27±0.81), (4.16±0.47), (5.05±0.74) and (2.43±0.73), (4.27±0.61), (2.44±0.45), (4.07±0.73), (3.89±0.77), in addition, those index scores in the control group were [(5.73±1.23), (4.85±0.64), (4.37±0.74), (4.17±0.38), (4.98±0.82) and (4.48±0.72), (4.90±0.75), (3.45±0.53), (4.23±0.81), (4.60±0.57)]. There was no significant difference in PFS-R scores between study group and control group ( P>0.05), however, sensory, emotional, knowledge and total PFS-R scores were significantly decreased in the study group compared to the control group ( P<0.05). Conclusion:Roy adaptation model-based intervention can effectively alleviate fatigue and promote post-traumatic growth in patient with malignant lymphoma during chemotherapy.

ABSTRACT

Objective:Analyze the epidemiological data and the clinical data of local foodborne botulism, summarize the influencing factors and clinical characteristics of the foodborne botulism, mitigate future misdiagnosis.Methods:An epidemiological investigation was conducted to a local outbreak with botulism, which involved 24 individuals consuming homemade bean curd and 14 cases with foodborne botulism through analyzing the influencing factors of the poisoning. For 14 patients with botulism, their clinical manifestations, first laboratory test and imaging data, treatment and prognosis, were documented and explored.Results:All 14 cases were diagnosed as type A foodborne botulism, of which 2 were misdiagnosed. The incidence of the foodborne botulism was reduced by 81.7% in alcoholic drinkers ( P=0.009). The median incubation period of the 14 patients with botulism was 2.0 (1.0~3.0) days. The initial clinical symptom of 10 patients was diplopia, while that of the other 4 patients was blurred vision. Nine patients subsequently suffered from paralysis of oropharyngeal muscles, leading to dysphagia, speech impairment, etc. Four patients progressed to dyspnea and chest tightness due to respiratory muscle involvement. All of the patients had clear consciousness without sensory abnormalities, no fever and abdominal symptoms such as diarrhea and abdominal pain. There were no obvious abnormal findings in 14 patients with laboratory tests and cranial CT/MRI assessment. 14 patients with the poisoning were relieved after injection of botulinum antitoxin for 8.0 (7.0~8.5) days. Follow-up of all the patients 6 months later found that 9 patients still had slight blurred vision. Conclusions:The typical clinical manifestation of foodborne botulism was symmetric descending flaccid paralysis. Drinking liquor could reduce the incidence of foodborne botulism. Botulinum antitoxin was effective in the treatment of botulism and should be used as early as possible.

ABSTRACT

Objective@#To evaluate the relationship between the bisphenol A (BPA) exposure and obesity in children and adolescents with Meta-analysis, and to provide a new idea for prevention and control of obesity.@*Methods@#Reviewer independently searched and screened potentially relevant studies that reported the association between BPA and obesity in children and adolescents through Medline, Web of Science, CBM, CNKI, WANFANG DATA and CQVIP to March 7, 2019. Chi-square based Q test and I 2 statistic were used to assess the heterogeneity. Forest plot was used to display results graphically. Potential publication bias was assessed by the Egger and Begg tests and funnel plot.@*Results@#In total, 13 studies with 3 932 participants met the eligibility criteria, 13 articles were indentified involving 1 046 obesity children and 2 886 normal control children. Overall summary estimates indicated significant positive correlation between urinary BPA and obesity ( SMD =0.26,95% CI =0.01-0.51), and a nonsignificant association between serum BPA and obesity ( SMD =0.07,95% CI =-0.44-0.58). Significant heterogeneity was observed in overall estimates ( I 2=87.3%, P <0.01). In additon, a significant positive association was estimated in American ( SMD =0.27, 95% CI =0.16-0.39) and Europe ( SMD =3.76, 95 CI =2.77-4.74). The association between BPA and obesity was not changed in each individual (sensitivity analysis by leaving one out approach). The funnel plots did not reveal severe asymmetry, and both Egger and Begg tests were non-significant ( P >0.05); therefore, publication bias was assessed as “undetected”.@*Conclusion@#The finding from our systematic review and Meta-analysis suggest that children and adolescents exposure to BPA may result in obesity.

ABSTRACT

Purpose@#Diabetes mellitus (DM) is associated with elevated cancer risk and poor survival outcome in malignancies. The objective of this study was to evaluate the prognostic value of preexisting DM in chronic lymphocytic leukemia (CLL). @*Materials and Methods@#Six hundred and thirty-three subjects with newly-diagnosed CLL between 2007 and 2016 were recruited. Propensity score-matched method was performed to balance baseline characteristics and eliminate possible bias. Univariate and multivariate Cox regression analyses screened the independent risk indicators for time-to-first-treatment (TTFT) and cancer-specific survival (CSS) of CLL. Receiver operator characteristic curves and the corresponding areas under the curve assessed the predictive accuracy of CLL–International Prognostic Index (IPI) together with DM. @*Results@#The results showed that 111 patients had pre-existing DM. In the propensity-matched cohort, DM was correlated with inferior TTFT and CSS in CLL patients, and it was an independent prognostic factor for both CSS and TTFT. Pre-diabetics also shared undesirable prognostic outcome compared with patients with no diabetic tendency, and a positive association between longer diabetic duration and poorer prognosis of CLL was identified. DM as one additional point to CLL-IPI had larger area under the curve compared with CLL-IPI alone in CSS prediction and could improve the prognostic capacity of CLL-IPI. @*Conclusion@#Pre-existing DM was found to be a valuable prognostic predictor and could help predict life expectancy and build refined prognostication models for CLL.

ABSTRACT

Purpose@#The purpose of this study was to investigate the prognostic impact of Epstein-Barr virus(EBV)–microRNA (miRNA, miR)-BHRF1-1 with chronic lymphocytic leukemia (CLL) as wellas role of EBV-miR-BHRF1-1 in p53 gene. @*Materials and Methods@#Quantitative reverse transcription–polymerase chain reaction and western blotting wereused to quantify EBV-miR-BHRF1-1 and p53 expression in cultured CLL. @*Results@#p53 aberration was associated with the higher expression level of EBV-miR-BHRF1-1 (p <0.001) which was also an independent prognostic marker for overall survival (p=0.028;hazard ratio, 5.335; 95% confidence interval, 1.193 to 23.846) in 97 newly-diagnosed CLLpatients after adjusted with International Prognostic Index for patients with CLL. We identifiedEBV-miR-BHRF1-1 as a viral miRNA regulator of p53. EBV-miR-BHRF1-1 repressedluciferase reporter activity by specific interaction with the seed region within the p53 3-untranslated region. Discordance of p53 messenger RNA and protein expression wasassociated with high EBV-miR-BHRF1-1 levels in CLL patients and cell lines. EBV-miR-BHRF1-1 inhibition upregulated p53 protein expression, induced cell cycle arrest and apoptosisand decreased cell proliferation in cell lines. EBV-miR-BHRF1-1 mimics downregulated p53protein expression, decreased cell cycle arrest and apoptosis, and induced cell proliferationin cell lines. @*Conclusion@#This study supported the role of EBV-miR-BHRF1-1 in p53 regulation in vitro. Our resultssupport the potential of EBV-miR-BHRF1-1 as a therapeutic target in EBV-associated CLLwith p53 gene aberration.

ABSTRACT

OBJECTIVE@#To investigate the prognosis prediction value of PET/CT in DLBCL patients treated with CAR-T therapy.@*METHODS@#The effects of PET/CT were retrospectively explored on 13 R/R DLBCL patients who were treated with CAR-T therapy. Parameters reflecting tumor metabolic burden, such as metabolic tumor volume (MTV) and total lesion glycolysis (TLG) were measured before and after CAR-T treatment.@*RESULTS@#Patients with larger baseline MTV or longer sum of longest diameters showed shorter overall survival (OS) time than those with low tumor burden. Patients achieved complete remission (CR), partial remission (PR) and minor remission (MR) determined by response evaluation criteria in lymphoma (RECIL) in 12 weeks showed progression-free survival and OS time superior to those of patients with no remission. In addition, it was found that 2 patients with residual masses classified as PR by contrast-enhanced CT of patients were evaluated as complete metabolic response by PET/CT imaging.@*CONCLUSION@#PET/CT shows a great value in the evaluation of prognosis and response in CAR-T-treated R/R DLBCL patients.

Objective@#To understand literacy development of school-aged children of grade 2-5 and its influencing factors in Guangzhou,and to provide a reference for the reform of the teaching of Chinese subject.@*Methods@#By using cluster sampling method, 1 661 school-age children from grade 2 to grade 5 from 5 primary schools in Guangzhou were selected. The self-designed questionnaire was used to obtain demographic data of the participants. The Primary School Literacy Assessment Scale and the Pupil Rating Scale Revised Screening for Learning Disabilities was used to evaluate the literacy and the cognitive characteristics of participants.@*Results@#The average literacy of children of grade 2 to grade 5 in Guangzhou was as follows: grade 2 was (1 159±295), grade 3 was (1 919±394), grade 4 was (2 599±365), grade 5 was (2 947±303), higher than the norm(P<0.01). The average literacy of grade 2 students was lower than the national curriculum requirements while students of grade 3 to grade 5 met the requirements. Univariate analysis found differences in literacy among school-age children based on gender, reading experience before age 6, and parental education background (P<0.01). Multiple linear regression analysis revealed that grade (β=607.04), preschool reading experience (β=109.89), father’s education (β=27.14), language factor (β=27.21), social behavior (β=16.03) was positively correlated with literacy in boys (P<0.05). Grade (β=603.53), auditory comprehension and memory (β=29.39), language factor (β=16.74) was positively correlated with the literacy of girls (P<0.05), while time and orientation judgments were negatively correlated with the literacy of both boys (β=-18.95) and girls (β=-21.93) (P<0.05).@*Conclusion@#The literacy level of school-age children in grade 2 to 5 in Guangzhou has reached the national literacy requirements with students in grade 2 being relatively lower. Literacy is related to grade, preschool reading experience, father’s education, and child’s cognitive characteristics. Factors affecting literacy in boys and girls are different. Literacy education should vary according to gender.

ABSTRACT

BACKGROUND@#Serum antinuclear antibodies (ANAs) are positive in some patients with chronic lymphocytic leukemia (CLL), but the prognostic value of ANAs remains unknown. The aim of this study was to evaluate the role of ANAs as a prognostic factor in CLL.@*METHODS@#This study retrospectively analyzed clinical data from 216 newly diagnosed CLL subjects with ANAs test from 2007 to 2017. Multivariate Cox regression analyses were used to screen the independent prognostic factors related to time to first treatment (TTFT), progression free survival (PFS) and overall survival (OS). Receiver operator characteristic curves and area under the curve (AUC) were utilized to assess the predictive accuracy of ANAs together with other independent factors for OS.@*RESULTS@#The incidence of ANAs abnormality at diagnosis was 13.9%. ANAs positivity and TP53 disruption were independent prognostic indicators for OS. The AUC of positive ANAs together with TP53 disruption was 0.766 (95% confidence interval [CI]: 0.697-0.826), which was significantly larger than that of either TP53 disruption (AUC: 0.706, 95% CI: 0.634-0.772, P = 0.034) or positive ANAs (AUC: 0.595, 95% CI: 0.520-0.668, P < 0.001) in OS prediction. Besides, serum positive ANAs as one additional parameter to CLL-international prognostic index (IPI) obtained superior AUCs in predicting CLL OS than CLL-IPI alone.@*CONCLUSION@#This study identified ANAs as an independent prognostic factor for CLL, and further investigations are needed to validate this finding.

ABSTRACT

OBJECTIVE@#To investigate the clinical manifestation, immunophenotypes and prognostic factors of patients with primary tonsil afftive large B cell lymphoma ( PT-DLBCL ).@*METHODS@#The clinical data including clinical characterstics, typing, staging, treatment efficacy and prognostic factors of PT-DLBCL patients were collected and analyzed restrospectively.@*RESULTS@#Out of 36 cases with the detinite cell origin, 24 cases (66.7%) were detecmined as the type of germinal center B-cell (GCB) and 12 cases (33.3%) was non-germinal center B-cell (non-GCB), 15 (40.5%) out of 37 cases were in Ann Arbor stage Ⅰ, and 22 (59.5%) in stage Ⅱ. With the median follow-up of 44 (10-101) months, 2 cases (5.4%) failed to be followed-up, after treatment for 6 (3-8) cycles 35 patients were evaluated. Among them 26 cases (74.3%) reached to complete remission (CR), 8 cases (22.9%) to partial remission ( PR ), and 1 (2.8%) to stable disease (SD). Both the 3 years and 5 years progression-free survival ( PFS ) were 82.5%, and both 3 and 5 years overall survival (OS) were 95.5%. 5 cases (13.5%) received radiotherapy. The patients aged>60 ( P<0.05 ) or aged>70 (P<0.05) had shorter PFS than younger patients. The patients with increased lactic dehydrogenase ( LDH ) level (P<0.01) and without rituximab (R) (P<0.05) in the treatment regimen had relatively short OS.@*CONCLUSION@#The patients sensitive to chemotherapy and/or radiotherapy have a good prognosis. Most of the patients can obtain long-term survival after treatment. The effect of combined immunotherapy are better than that of the simple chemotherapy.

ABSTRACT

PURPOSE: The purpose of this study was to investigate the prognostic significance of total metabolic tumor volume (TMTV) and total lesion glycolysis (TLG) in patients with follicular lymphoma (FL) at baseline and mid-treatment with ¹⁸F-fluorodeoxyglucose positron emission tomography-computed tomography (PET-CT) scans. MATERIALS AND METHODS: The study analyzed data from 48 patients with FL who were treated in Jiangsu Province Hospital and reviewed their baseline PET-CT scans. TMTV and TLG were computed by using the absolute value of 2.0, 2.5, and 3.0 thresholding method, respectively. RESULTS: Median age was 53 years, 75.0% of patients had stage III to IV disease, 43.8% had a Follicular Lymphoma International Prognostic Index 1 (FLIPI1) score of 3 to 5 and 20.8% had a FLIPI2 score of 3 to 5. Receiver operating characteristic (ROC) curve analysis showed the optimal cut-off values for TMTV3.0 and TLG3.0 were 476.4 (sensitivity, 85.7%; specificity, 78.0%; area under the curve [AUC], 0.760; p=0.003) and 2,676.9 (sensitivity, 71.4%; specificity, 78.0%; AUC, 0.760; p=0.003). On multivariable analysis, TMTV3.0 and TLG3.0 were independent predictors of both progression-free survival (PFS) (hazard ratio [HR], 5.406; 95% confidence interval [CI], 1.326 to 22.040; p=0.019 and HR, 6.502; 95% CI, 1.079 to 39.182; p=0.042) and overall survival (OS) (HR, 4.111; 95% CI, 1.125 to 15.027; p=0.033 and HR, 5.885; 95% CI, 1.014 to 34.148; p=0.049). ROC curve analysis showed the optimal cut-off values for ΔTMTV3.0 and ΔTLG3.0 were 66.3% (sensitivity, 85.7%; specificity, 63.4%; AUC, 0.774; p 66.3%) and TLG (ΔTLG > 64.5%) reduction are valuable tools for early treatment response assessment in FL patients.

ABSTRACT

PURPOSE: Chronic lymphocytic leukemia (CLL) is one of the most frequent type of B-cell chronic lymphoproliferative disorders and chronic inflammation takes part in the development of CLL. However, there has been no valid immune biomarker to predict the prognosis of untreated CLL patients. MATERIALS AND METHODS: In this retrospective study, we analyzed the clinical correlations and prognostic value of albumin-to-fibrinogen ratio (AFR) detected at diagnosis in 191 CLL patients. RESULTS: The cut-off value of AFR was 9.7 calculated by X-tile. Patients who were more than 65 years old were often accompanied by low level of AFR (p < 0.001). Survival analysis showed that patients with low level of AFR had shorter overall survival (OS) than patients with high level of AFR (p < 0.001). Multivariate analysis illustrated that AFR had a negative impact on OS (p=0.003) and was independent of parameters involved in CLL international prognostic index and other prognostic markers such as CD38 and ZAP-70. CONCLUSION: These data provide a comprehensive view of AFR and shows that AFR at diagnosis is an adverse prognostic factor in untreated CLL patients.
