ABSTRACT

Vanin-1 is an amidohydrolase that catalyses the conversion of pantetheine into the amino-thiol cysteamine and pantothenic acid (coenzyme A precursor), which plays a vital role in multiple physiological and pathological processes. In this study, an enzyme-activated near-infrared (NIR) fluorescent probe (DDAV) has been constructed for sensitively detecting Vanin-1 activity in complicated biosamples on the basis of its catalytic characteristics. DDAV exhibited a high selectivity and sensitivity toward Vanin-1 and was successfully applied to the early diagnosis of kidney injury in cisplatin-induced kidney injury model. In addition, DDAV could serve as a visual tool for in situ imaging endogenous Vanin-1 in vivo. More importantly, Enterococcus faecalis 20247 which possessed high expression of Vanin-1 was screened out from intestinal bacteria using DDAV, provided useful guidance for the rational use of NSAIDs in clinic. Finally, oleuropein as a potent natural inhibitor for Vanin-1 was discovered from herbal medicines library using a high-throughput screening method using DDAV, which held great promise for clinical therapy of inflammatory bowel disease.

ABSTRACT

In this study, artificial intelligence aided diagnosis system of fundus images was applied to clinical teaching of ophthalmology to explore its role in improving the teaching quality. Taking the clinical teaching of diabetic retinopathy as an example, we analyzed the specific steps, effects and improvement strategies. The results revealed that the course could be more interesting and impressing by using the typical cases provided by the artificial intelligence aided diagnosis system of fundus images. In the meanwhile, this system provided the excellent chances for ophthalmology postgraduates to exercise after class, which deepened their understanding of knowledge and enhanced their learning enthusiasm. The application effect has been affirmed and recognized by students.

ABSTRACT

OBJECTIVE:To study the reversal effect of quercetin on human cervical squamous carcinoma cisplatin-resistant cell line SiHa/DDP. METHODS :The drug resistance index of cisplatin to SiHa/DDP cells ,and the reversal resistance multiple of quercetin to SiHa/DDP cells were determined. The effects of quercetin (0.005 μg/mL),cisplatin(2.5 μg/mL),cisplatin combined with quercetin (2.5 μg/mL cisplatin+0.005 μg/mL quercetin),quercetin combined with pathway inhibitor(0.005 μg/mL quercetin+ 20 nmol/L rapamycin )on the apoptotic rate of SiHa/DDP cells were investigated ,as well as its effects on the expression of phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt)/mammalian rapamycin target protein (mTOR) signaling pathway related proteins (PI3K,Akt,mTOR,P-gp,p70S6K). RESULTS :The resistance index of cisplatin to SiHa/DDP cells was 5.19, and reversal resistance multiple of quercetin to SiHa/DDP cells was 4.00. Compared with cisplatin alone and quercetin alone , cisplatin combined with quercetin ,quercetin combined with rapamycin could significantly increase the apoptotic rate of SiHa/DDP cells(P<0.05),while decreased the phosphorylation of Akt ,mTOR and p 70S6K protein as well as the expression of P-gp protein (P<0.05). CONCLUSIONS :Quercetin can effectively reverse drug resistance of SiHa/DDP cells to cisplatin ,which may be associated with inhibiting the expression of the protein related to PI 3K/Akt/mTOR signaling pathway.

ABSTRACT

Objective:To determine the median effective dose (ED 50) and the 95% effective dose (ED 95) of remifentanil inhibiting responses to endotracheal intubation without neuromuscular relaxant when combined with dexmedetomidine in patients undergoing thyroid surgery. Methods:American Society of Anesthesiologists physical status Ⅰ or Ⅱ patients of either sex, aged 18-64 yr, with body mass index of 18-28 kg/m 2, scheduled for elective thyroid surgery under intraoperative neuromonitoring, were enrolled in this study.Dexmedetomidine was intravenously injected in a loading dose of 0.8 μg/kg at 10 min before anesthesia induction.Anesthesia was induced by intravenously injecting midazolam 0.1 mg/kg, etomidate 0.4 mg/kg and the preset dose of remifentanil.The dose of remifentanil was determined using up-and-down sequential method.The initial dose was set at 3.7 μg/kg.The dose of remifentanil in the next case was determined according to whether responses to endotracheal intubation occurred, and the ratio between the two successive doses was 1.1.The ED 50, ED 95 and 95% confidence interval (CI) were calculated by Probit analysis. Results:when combined with dexmedetomidine for anesthesia induction, the ED 50 (95% CI) of remifentanil inhibiting responses to endotracheal intubation without neuromuscular relaxant was 3.39 (3.29-3.50) μg/kg, and the ED 95 (95% CI) was 3.52 (3.48-3.64) μg/kg. Conclusion:when combined with dexmedetomidine, the ED 50 of remifentanil inhibiting responses to endotracheal intubation without neuromuscular relaxant is 3.39 μg/kg, and the ED 95 is 3.52 μg/kg.

ABSTRACT

Objective:To investigate the relationship between the mechanism of exogenous hydrogen sulfide (H 2S)-induced reduction of apoptosis in neurons during focal cerebral ischemia-reperfusion (I/R) and PINK1/Parkin pathway-mediated mitochondrial autophagy in rats. Methods:Two hundred and sixteen healthy male Sprague-Dawley rats, aged 6-8 weeks, weighing 250-270 g, were divided into 4 groups ( n=54 each) using a random number table method: control group (group C), I/R group, H 2S group and H 2S plus 3-methyladenine (3-MA) group (H 2S+ 3-MA group). Focal cerebral ischemia was induced by middle cerebral artery occlusion in anesthetized rats.In group H 2S+ 3-MA, 3-MA 10 mg/kg was intraperitoneally injected at 15 min before the onset of reperfusion, while the equal volume of normal saline was given instead in the other groups.In H 2S and H 2S+ 3-MA groups, 0.25% NaSH (a donor of exogenous H 2S) 10 mg/kg was intraperitoneally injected at the onset of reperfusion, while the equal volume of normal saline was given instead in the other groups.At 1, 3 and 7 days of reperfusion, neural function was scored, and corner test (the percentage of left turn was calculated) was performed.Brains were removed and brain tissues were obtained for determination of the cerebral infarct size, Bax, Bcl-2 and caspase-3 positive cells, cell apoptosis, and expression of mitophagy-related protein microtubule-associated protein 1 light chain 3 (LC3), PINK1 and Parkin (by Western blot). The percentage of cerebral infarct size, rate of Bax, Bcl-2 and caspase-3 positive cells and apoptosis rate were calculated.The ratio of LC3-Ⅱexpression to LC3-Ⅰexpression (LC3-Ⅱ/LC3-Ⅰ) was also calculated. Results:Compared with group C, the neural function score was significantly decreased, the percentage of left turn, percentage of cerebral infarct size, rate of Bax, Bcl-2 and caspase-3 positive cells, apoptosis rate of neurons, and LC3-Ⅱ/LC3-Ⅰ were increased, and the expression of PINK1 and Parkin was up-regulated at each time point of reperfusion in group I/R ( P<0.05). Compared with group I/R, the neural function score and rate of Bcl-2 positive cells were significantly increased, the percentage of left turn, percentage of cerebral infarct size, rate of Bax and caspase-3 positive cells, and apoptosis rate of neurons were decreased, the expression of PINK1 and Parkin was up-regulated, and LC3-Ⅱ/LC3-Ⅰ were increased at each time point of reperfusion in group H 2S ( P<0.05), and no significant change was found in the parameters mentioned above in group H 2S+ 3-MA ( P>0.05). Compared with group H 2S, the neural function score and rate of Bcl-2 positive cells were significantly decreased, the percentage of left turn, percentage of cerebral infarct size, rate of Bax and caspase-3 positive cells, and apoptosis rate of neurons were increased, the expression of PINK1 and Parkin was down-regulated, and LC3-Ⅱ/LC3-Ⅰ was decreased at each time point of reperfusion in H 2S+ 3-MA group ( P<0.05). Conclusion:The mechanism by which exogenous H 2S inhibits apoptosis in neurons during focal cerebral I/R is related to enhancing mitochondrial autophagy mediated by the PINK1/Parkin pathway in rats.

ABSTRACT

Objective:To explore the characteristics of the sleep structures of patients with both chronic insomnia and obstructive sleep apnea (OSA) in plateau area.Methods:Polysomography Alice 5 was applied to 23 patients with primary chronic insomnia [insomnia group, age (48.2±9.9) years], 22 patients with both chronic insomnia and OSA [comorbidity group, age (46.8±8.9) years], who both came from Qinghai Red Cross Hospital between January, 2014 to June, 2015 and 20 subjects with normal sleep [healthy group, age (46.2±7.1) years] in plateau area (mainly in Xining, altitude 2 250 meters or above) to compare and explore their sleep structures by the whole night sleep monitoring in the sleep monitoring room. The sleep structures were compared according to the American Academy of Sleep Medicine (AASM) Manual for the Scoring of Sleep and Associated Events.Results:Compared to healthy group, insomnia group and comorbidity group both had significantly lower sleep efficiency [(62.4%±16.7%), (59.8%±16.0%) vs (80.9%±8.8%)], non-rapid eye movement (NREM) phase 2 sleep ratio [(37.9%±12.2%), (36.2%±12.5%) vs (49.7%±6.2%)] and rapid eye movement (REM) sleep ratio [(7.7%±4.0%), (6.5%±4.0%) vs (12.5%±4.6%)] (all P<0.05); comorbidity group had a significantly higher oxygen desaturation index than insomnia group and healthy group [(30.8±29.2) vs (7.9±7.5), (5.9±2.7) times/h] ( P<0.05); insomnia group′s sleep latency of NREM3 stage was significantly longer than comorbidity group and healthy group [(148.9±113.6) vs (89.3±51.8), (59.1±40.3) min] (both P<0.05). Conclusion:Patients with both chronic insomnia and OSA and patients with chronic insomnia only in plateau area have different sleep structures, and both of their sleep quality are lower than the people with normal sleep; patients with both chronic insomnia and OSA could enter deep sleep more quickly after sleep onset.

Objective:To evaluate the feasibility and accuracy of a multi-dimensional peripheral quick contrast sensitivity function (pqCSF) measurement established based on Bayesian probability estimation algorithm.Methods:A cross-sectional study was conducted.Nineteen eyes of 12 healthy emmetropic subjects in Zhongshan Ophthalmic Center of Sun Yat-sen University from September 2017 to March 2018 were included, with an average age of (22.92±2.91) years.The average spherical power and cylindrical power were (-0.34±0.52)D and (-0.30±0.42)D, respectively, and the average uncorrected vision acuity was≥1.0.Based on the Bayesian probability algorithm, the peak contrast sensitivity γ max, the peak spatial frequency ? max, the bandwidth β and the low contrast intercept δ were used to quickly describe the contrast sensitivity function (CSF) curve of the full spatial frequency through multi-dimensional pqCSF method.The 16 peripheral visual field positions of all subjects were tested at 6°, 12°, 18° and 24° eccentricity of the superior, inferior, the temporal and nasal visual field by the pqCSF method, but the 18° eccentricity of temporal field, which was near the physiological blind spot, was excluded.The area under Log CSF (AULCSF) of different peripheral visual fields and the Log CSF of 19 spatial frequencies (distributed at equal intervals in logarithmic units) were compared.This study followed the Declaration of Helsinki, and the study protocol was approved by an Ethics Committee of Zhongshan Ophthalmic Center of Sun Yat-sen University (No.2018KYPJ017). Written informed consent was obtained from each subject prior to any examination. Results:With the increase of eccentricity in different visual fields, the AULCSF decreased gradually, and there were significant differences in AULCSF between different eccentricities (all at P<0.05). The AULCSF of the nasal and temporal visual field at 6°, 12° and 24° eccentricity was significantly larger than that of the superior and inferior visual field (all at P<0.05). As the distance from the fovea was increased, the pqCSF, the AULCSF, and the high-frequency cutoff were all decreased, and the standard deviation of AULCSF was increased gradually. Conclusions:The pqCSF method can depict a relatively complete peripheral CSF curve of a wide peripheral visual field, and reflect the function quality of the peripheral vision comprehensively and accurately.

ABSTRACT

Objective:To explore the clinical effect of lenalidomide combined with anti-B-cell maturation antigen chimeric antigen receptor T cell (anti-BCMA CAR-T) in the treatment of relapsed/refractory multiple myeloma (RRMM).Methods:The clinical data of a patient with RRMM who underwent lenalidomide combined with anti-BCMA CAR-T therapy in Henan Province Hospital of Traditional Chinese Medicine in January 2020 were analyzed. Clinical manifestations, diagnosis and treatment were also analyzed, and the related literature was reviewed.Results:The patient was a 51-year-old man who was diagnosed as IgD-λ multiple myeloma (MM) in October 2015. The patient achieved remission after 10 courses of chemotherapy regimens including immunomodulators and proteasome inhibitors, followed by autologous hematopoietic stem cell transplantation. MM relapsed after 14 months of transplantation. His disease continued to progress after multiple chemotherapy regimens and mouse or human-derived anti-BCMA CAR-T therapy. After a conditioning chemotherapy regimen of fludarabine and cyclophosphamide, the patient took lenalidomide on day 1 and was infused human-derived anti-BCMA CAR-T cells on the next day. Grade 3 cytokine releasing syndrome (CRS) appeared after infusion, and was resolved after symptomatic treatment. Very good partial response (VGPR) was achieved on day 14 after anti-BCMA CAR-T treatment. VGPR had been maintained for more than 3 months by press time.Conclusion:Lenalidomide combined with anti-BCMA CAR-T therapy is feasible and effective in the treatment of RRMM.

ABSTRACT

The prompt detection and proper evaluation of necrotic retinal region are especially important for the diagnosis and treatment of acute retinal necrosis (ARN). The potential application of artificial intelligence (AI) algorithms in these areas of clinical research has not been reported previously. The present study aims to create a computational algorithm for the automated detection and evaluation of retinal necrosis from retinal fundus photographs. A total of 149 wide-angle fundus photographs from 40 eyes of 32 ARN patients were collected, and the U-Net method was used to construct the AI algorithm. Thereby, a novel algorithm based on deep machine learning in detection and evaluation of retinal necrosis was constructed for the first time. This algorithm had an area under the receiver operating curve of 0.92, with 86% sensitivity and 88% specificity in the detection of retinal necrosis. For the purpose of retinal necrosis evaluation, necrotic areas calculated by the AI algorithm were significantly positively correlated with viral load in aqueous humor samples (

ABSTRACT

To evaluate whether midlife consumption of sugar-sweetened beverages (SSBs), including juices and soft drinks, were associated with late-life cognitive impairment in Chinese adults. Follow up was conducted for participants from Singapore Chinese Health Study, a population-based prospective cohort, for 16-23 (mean 20) years. The information about their SSBs consumption were collected at baseline survey from 1993 to 1998 by using a validated food frequency questionnaire and their cognition status were evaluated by using a Singapore-modified Mini-Mental State Examination Scale in the 3(rd) follow-up visit during 2014- 2016. Multivariable logistic regression models were used to estimate the and 95s. A total of 16 948 participants were included in the analysis and 2 443 of them were identified as cognitive impairment using education-specific cutoffs. Sex, age, ancestral home, education level, physical activity level, total diet fiber intake level, BMI, alcohol drinking were significantly associated with cognitive impairment (0.05). Compared with those who never or hardly ever drank soft drinks, no significant differences in cognitive impairment risk were observed for those who drank soft drinks 2 or more times a week (=0.91, 95: 0.77-1.08), those who drank 1 time a week (=1.00, 95:0.82-1.23) and those who drank 1-3 times a month (=0.94, 95: 0.80-1.09) (trend =0.306). Compared with those who never or hardly ever drank juices, no significant differences in cognitive impairment risk were observed for those who drank juices 2 or more times a week (=1.03, 95:0.88-1.20), those who drank 1 time a week (=0.96, 95: 0.82-1.12) and those who drank 1-3 times a month (=0.94, 95: 0.82-1.08) (trend =0.930). No significant interactions were found with age, sex, and BMI status. SSBs consumption in midlife was not significantly associated with risk of late-life cognitive impairment in Singaporean Chinese adults with relatively low consumption levels. Further researches are needed to verify the results.

Objective To forecast the trend of mosquito density index in Pudong New Area, Shanghai so as to provide evidence for disease control and risk-control measures for vector-borne diseases. Methods Mosquito monitoring data was collected in Pudong New Area between 2011 and 2015 at the city-level monitoring sites for analysis on the trend of the mosquito density index in Pudong New Area of Shanghai by using the Autoregressive Integrated Moving Average Model (ARIMA). Results From 2011 to 2015, a total of 135 times labor-hour monitoring were carried out at the city-level monitoring points in Pudong New Area.The mosquito density index averaged 6.17/labor-hour with a standard deviation at 4.93, S=[0, 18]/labor-hour.Using ARIMA to analyze the change trend of mosquito density index in Pudong New Area, ARIMA(2, 0, 1)became the final fitting model, with R2=0.808.In the model, the Ljung-Box Q test value was 19.632(AR1=1.866, AR2=-0.907), and MA parameter was 0.999. Conclusion ARIMA model can be used to predict mosquito density monitoring data, but low monitoring frequency and irregular cycle length will affect the prediction results.

Objective To learn the density, distribution, population composition, and seasonal fluctuation of mosquitoes in Shanghai Pudong New Area, and to provide evidence for the mosquito-borne diseases control and prevention. Methods From 2017 to 2019, light trap method was used to monitor the mosquito density in farmyard, parks, hospitals, residential areas and the barns, three times a month from April to November. Results A total of 24 times/set light traps were set up and 7 104 mosquitoes were captured.The average density was 3.07 insects per lamp.Culex pipienspallens (81.52%) and Aedes albopictus (12.10%) were the mostly captured, followed by Armigeres subalbatus (3.24%), Culex tritaeniorhynchus (2.78%) and Anopheles sinensis (0.36%).The proportion of Culex pipienspallens was increasing year by year, and that of Aedes albopictus was decreasing year by year.The seasonal variation curve of mosquito density showed a single peak distribution, with the peak from June to July.Among the five habitat types, the mosquito density from high to low was hospital (4.68/set), farmyard (4.38/set), livestock shed (3.25/set), residential household (2.81/set) and park (1.38/set).The mosquito density in urban area, urban-rural junction and rural area was 2.69, 3.32 and 3.36 respectively (i.e., rural area > urban-rural integration > urban area). Conclusion Culex pipienspallens and Aedes albopictus are the dominant species in Pudong New Area.Parks, hospitals and residential areas are the most important mosquitoes infestation areas, therefore more attention must be paid to mosquito control in these areas.

OBJECTIVE@#To study the difference in age estimation based on quantitative analysis of DNA methylation by MassARRAY and pyrosequencing techniques.@*METHODS@#The methylation levels of 9 CpG sites from two independent whole blood sample sets (containing 65 and 62 samples) were detected using MassARRAY and pyrosequencing techniques. Z-score transformation was used to remove the batch effects of different techniques, and a linear regression model was used for age prediction.@*RESULTS@#For age prediction using the MassARRAY system, the 65 samples showed a mean absolute difference (MAD) of 2.49 years before Z-score transformation of the data and 2.44 years after the transformation, similar to the results in the 62 samples (MAD of 3.36 years before and 3.42 years after Z-score transformation). For data typed from pyrosequencing, the 65 samples showed a MAD of 4.20 years before and 2.76 years after data Z-score transformation, also similar to the results in the 62 samples (MAD of 3.92 years before and 3.63 years after data transformation).@*CONCLUSIONS@#Z-score transformation can effectively reduce the system batch effect between MassARRAY and pyrosequencing. Data from the MassARRAY system allows direct age estimation without further data processing, while the pyrosequencing data may increase the error in age estimation, which can be corrected by Z-score transformation of the data.

Objective:To assess the association between visual-related quality of life (QOL) and the severity of quick contrast sensitivity function (qCSF) defects in patients with early cataract.Methods:A cross section design was performed.Ninety-two eyes of 46 patients with early cataract were enrolled in Zhongshan Ophthalmic Center from March to June in 2017.All subjects completed the Chinese version of the National Eye Institute Visual Function Questionnaire (NEI VFQ-25). The qCSF measurements were performed monocularly and binocularly while the patients wearing full spectacle correction spectacles.Rasch analysis was performed to obtain final NEI VFQ-25 scores.Best corrected visual acuity (BCVA)(LogMAR) was measured and the eye with the better or the worse visual acuity was defined accordingly.The correlation between the Rasch calibrated QOL composite scores and the qCSF measurements of the better eye, the worse eye and binoculus, including the area under Log contrast sensitivity function (AULCSF), qCSF under cut-off frequency and 1.0, 1.5, 3.0, 6.0, 12.0, 18.0 c/d were analyzed.This study protocol was approved by the Ethic Committee of Zhongshan Ophthalmic Center.This study complied with Declaration of Helsinki.Results:The Rasch calibrated QOL composite score was 64.13±4.76.Significant negative correlations were found between composite scores of QOL and the BCVA of both the better or worse eye(the better eye: r=-0.386, P=0.008; the worse eye: r=-0.413, P=0.004). Significant positive correlations were found between the composite scores of QOL and the AULCSF of binoculus, the better and worse eye (binoculus: r=0.531, P<0.001; the better eye: r=0.524, P<0.001; the worse eye: r=0.711, P<0.001); Significant positive correlations were found between the composite scores of QOL and the qCSF under cut-off of binoculus, the better and worse eye (binoculus: r=0.504; the better eye: r=0.419; the worse eye: r=0.694, P<0.01). The composite scores of QOL was positively correlated with the qCSF under 1.5, 3.0, 6.0, 12.0 c/d of binoculus, the better and worse eye (binoculus: r=0.444, 0.544, 0.545, 0.493; all at P<0.05; the better eye: r=0.374, 0.506, 0.530, 0.431; all at P<0.05; the worse eye: r=0.513, 0.679, 0.710, 0.558; all at P<0.05). The composite scores of QOL was positively correlated with the qCSF under 1.0, 18.0 c/d of binoculus and the worse eye (1.0 c/d: r=0.296, 0.292; both at P<0.05; 18.0 c/d: r=0.386, 0.321; both at P<0.05). Conclusions:The qCSF measurements can reflect visual function of patients with early cataract at different spatial frequencies.The qCSF measurements of binoculus, the better eye and the worse eye show significant correlations with the composite scores of QOL.The QOL of early cataract patients shows a strong dependency on the worse eye.

ABSTRACT

Objective@#To evaluate whether midlife consumption of sugar-sweetened beverages (SSBs), including juices and soft drinks, were associated with late-life cognitive impairment in Chinese adults.@*Methods@#Follow up was conducted for participants from Singapore Chinese Health Study, a population-based prospective cohort, for 16-23 (mean 20) years. The information about their SSBs consumption were collected at baseline survey from 1993 to 1998 by using a validated food frequency questionnaire and their cognition status were evaluated by using a Singapore-modified Mini-Mental State Examination Scale in the 3rd follow-up visit during 2014- 2016. Multivariable logistic regression models were used to estimate the ORs and 95%CIs.@*Results@#A total of 16 948 participants were included in the analysis and 2 443 of them were identified as cognitive impairment using education-specific cutoffs. Sex, age, ancestral home, education level, physical activity level, total diet fiber intake level, BMI, alcohol drinking were significantly associated with cognitive impairment (P<0.05). After adjusted the above variables, potential dietary variables and disease status, no significant association was observed between SSBs consumption and cognitive impairment (P>0.05). Compared with those who never or hardly ever drank soft drinks, no significant differences in cognitive impairment risk were observed for those who drank soft drinks 2 or more times a week (OR=0.91, 95%CI: 0.77-1.08), those who drank 1 time a week (OR=1.00, 95%CI:0.82-1.23) and those who drank 1-3 times a month (OR=0.94, 95%CI: 0.80-1.09) (trend P=0.306). Compared with those who never or hardly ever drank juices, no significant differences in cognitive impairment risk were observed for those who drank juices 2 or more times a week (OR=1.03, 95%CI:0.88-1.20), those who drank 1 time a week (OR=0.96, 95%CI: 0.82-1.12) and those who drank 1-3 times a month (OR=0.94, 95%CI: 0.82-1.08) (trend P=0.930). No significant interactions were found with age, sex, and BMI status.@*Conclusion@#SSBs consumption in midlife was not significantly associated with risk of late-life cognitive impairment in Singaporean Chinese adults with relatively low consumption levels. Further researches are needed to verify the results.

ABSTRACT

Objective To provide anatomic evidence for chock-block technique used for acromioplasty.Methods Thirty cadaveric specimens of intact shoulder fixed by formaldehyde solution were used for this anatomic study.They were chosen from 15 males and 15 females and from 17 left and 13 right sides.First the specimens were dissected for observation of general morphology of the acromion.According to the Bigliani's classification,the acromions were sorted in morphology as type Ⅰ (flat),type Ⅱ (curved) and type Ⅲ (hooked).The shortest distance from the acromion to the humeral head (A-H) was measured.Then the location of subacromial impingement was observed and the thickness of the anterolateral angle was measured preoperatively.At last,acromioplasty using the chock-block technique was performed in the acromions of types Ⅱ and Ⅲ.Postoperatively,the distance A-H and the thickness of the anterolateral angle were measured again for comparison with the preoperative values.Results According to the Bigliani's classification,7 acromions were type Ⅰ,13 type Ⅱ and 10 type Ⅲ.Subacromial impact occurred only in the anterior acromions of types Ⅱ and Ⅲ.The distance A-H was 0.670 ± 0.035 cm in type Ⅰ acromions,significantly longer than the preoperative values of type Ⅱ (0.506 ±0.029 cm) and type Ⅲ (0.334 ±0.023 cm) (P < 0.05),but showed no significant differences from the postoperative values of type Ⅱ (0.692 ±0.025 cm) and type Ⅲ (0.699 ±0.024 cm) (P > 0.05).The anterolateral angular thickness of type Ⅰ acromions was O.413 ± 0.015 cm,significantly thinner than the preoperative values of type Ⅱ (0.607 ±0.014 cm) and type Ⅲ (0.623 ±0.025 cm) (P < 0.05),but showed no significant differences from the postoperative values of type Ⅱ (0.414±0.035 cm) and type Ⅲ (0.412±0.033 cm) (P> 0.05).Conclusion The chock-block technique used for acromioplasty is effective because it can enlarge the supraspinatus outlet and deter impingement.

ABSTRACT

In the discussion on the establishment of the system of living will in China, the major controversy is whether the living will made by patients in medical treatment is valid. Different countries and regions have different decision-making modes for the validity of living will, which are mainly divided into patient-independent decision-making mode, physician based decision-making mode and family-based decision-making mode. On the basis of drawing lessons from international experience and combining with national conditions, this paper made a comparative analysis of the decision-making modes of the validity of living will. It is believed that the legislation of the decision-making of living will, which suits China's national conditions, should give priority to appointing agents from family members, consolidate the validity of living will through pre-consultation procedure, and confirming their rationality and legitimacy through legal custody and periodic review. In terms of legislative steps, it is suggested that non-legislative forms can be promoted first, and then incorporated it into legislation when conditions are ripe.

ABSTRACT

Objective To investigate the prevalence of nutritional risk (NRS2002) and malnutrition inhospitalized stroke patient and their nutritional intervention. Methods The stroke patients admitted to three de-partments of vascular neurology ward including cerebral hemorrhage, cerebral infarction and subarachnoid hemor-rhage in Beijing Tiantan Hospital from January 2018 to January 2019 were recruited using cluster sampling. Nutri-tional risk screening 2002 ( NRS 2002) was used to screen the nutritional risk of inpatients Malnutrition was as-sessed by criteria:(1) body mass index (BMI) <18. 5 kg/m2 with poorer general condition from January 2018 to January 2019;(2) Global leadership initiative on malnutrition ( GLIM) criteria were used except whole body muscle mass measurement from October 2018 to January 2019. The nutritional intervention for patients were closely observed during hospitalization. Results A total of 1532 patients were registered and1036 patients were included in the final analysis considering the inclusion and exclusion criteria. The prevalence of nutritional risk was 33. 0% ( 342/1036) . The prevalence of malnutrition based on BMI and GLIM criteria was 0. 9%( 9/1036) and 2. 5% (10/393) respectively. Among the 342 patients with nutritional risk, 112 patients received nutritional support therapy by tube feeding, but only 29 patients received nutritional support that met guideline standards. 81 patients received not standard nutritional support, and 2 patients received highly unregulated nutritional sup-port. No patients received sugar and electrolyte infusion, oral nutritional supplements ( ONS) , oral nutritional a-gents and compound nutrition intervention. The other 230 patients took hospital diet. Conclusion The prevalence of nutritional risk in hospitalized patients with cerebral hemorrhage, cerebral infarction or subarachnoid hemorrhage was high, and the prevalence of malnutrition was extremely low. There was a low proportion of nutri-tional support. High quality of large sample cohort studies will be conducted to show whether reasonable applica-tion of nutritional support therapy in patients with nutritional risk can improve patient outcome.

ABSTRACT

Objective To investigate the influence of the length of time of supine ultrasound examination on the parameters of umbilical artery blood flow in the late pregnant fetus. Methods From February 2019 to June 2019,the ultrasound data of 93 pregnant women with the third-trimester pregnant were analyzed retrospectively. Fetal umbilical artery systolic/diastolic ( S/D ) ratio, resistance index ( RI ), pulsatility index ( PI ) and fetal heart rate ( HR ) were acquired at the beginning of the ultrasound examination and those after the routine ultrasound examination ( the examination of fetal, placenta and amniotic fluid). Forty-seven pregnant women from 29 to 31 weeks of gestation were assigned to around 30-week gestation group,while 46 pregnant women from 35 to 37 weeks of gestation were assigned to around 36-week gestation group. Sixty-five pregnant women with normal S/D ratio at the beginning were assigned to the initial normal group,while 28 pregnant women with abnormal S/D ratio at the beginning were assigned to the initial abnormal group. Fetal umbilical artery S/D ratio,RI,PI and HR at the beginning of the ultrasound and after the routine ultrasound examination were compared using paired sample test within around 30-week gestation group, around 36-week gestation group, initial normal and initial abnormal group, respectively. Results There was no significant difference in fetal umbilical artery S/D,RI PI and HR in the pregnant women at around 30-week gestation(all P>0. 05). RI before the 36 week pregnant group was 0. 56 ±0. 05,and increased to 0. 58±0. 05 after the routine examination (t=-2. 190,P=0. 034). The S/D,RI and Pi of umbilical artery increased from 2. 36±0. 31,0. 57±0. 06,and 0. 84±0. 11 to 2. 50±0. 42,0. 59±0. 06, and 0. 88±0. 14 in the initial normal group ( t=-3. 087,-3. 002,-2. 287,respectively; P=0. 003,0. 004, 0. 025,respectively),of which 16 S/D increased to abnormal; the RI of umbilical artery increased from 0. 67 in the initial abnormal group ( 28 women ) after routine examination ) . The difference was statistically significant (t=2. 616,P=0. 014) . The ratio of S/D decreased to normal after routine examination in the initial abnormal group ( 14/28), which was higher than that in the initial normal group ( 16 / 65) . The difference was statistically significant ( χ2 =5. 771, P<0. 05) . Conclusion Umbilical artery blood flow parameters should be measured at the beginning of the ultrasound examination in the third-trimester pregnant women. Umbilical artery Doppler ultrasound should be performed repeatedly if the pregnant woman has abnormal S/D ratio at the beginning of the examination.
