ABSTRACT
This article reported a male patient with neonatal onset mental retardation autosomal dominant 35 (MRD35). The boy presented with repeated convulsions, hypotonia, enlarged head circumference, congenital muscular torticollis and feeding difficulties in the neonatal period. Dynamic electroencephalogram showed paroxysmal epileptic discharges in the left central-temporal region. High-throughput whole-exome sequencing revealed a heterozygous mutation of c.139G>A (p.Glu47Lys) in the PPP2R5D gene, which was a de novo mutation not inherited from his parents. The child had significant developmental delay at the age of one year. MRD35 lacks typical clinical manifestations and requires whole-exome sequencing for definitive diagnosis. Currently, there is no specific treatment for MRD35 and symptomatic treatments, including rehabilitation training, language training and seizure control, are mostly adopted.
ABSTRACT
This article reported a case of neonatal-onset autoinflammation with infantile enterocolitis (AIFEC) caused by NLRC4 gene mutation. The boy developed the disease in the neonatal period, presenting with recurrent fever, rash, hepatosplenomegaly and enterocolitis. Laboratory tests showed some indicators including ferritin and C-reactive protein were elevated. His condition was complicated by macrophage activation syndrome and anti-infective treatment was ineffective. High-throughput whole exome sequencing revealed a de novo heterozygous mutation of c.1021G>C (p.Val341Leu) in the NLRC4 gene and AIFEC was confirmed. AIFEC is a rare disease with no effective treatment at present, which can be developed in the neonatal period and diagnosed by whole exome sequencing.
ABSTRACT

ABSTRACT
Objective To observe the change ofbrain water content, levels of cAMP response element binding protein(CREB)and phosphorylated CREB(p-CREB) in the early stage of traumatic brain injury(TBI) and to investigate the effect of TBI and p-CREB on learning and memory. Methods Fifty-four male adult Wistar rats were randomized into the normal group(18 cases),control group(18 cases) and TBI group(18) according to the random number table method. The TBI model was built according to the modified Feeney method and previous experimental parameters. At 12 h after TBI,Western blot analysis were performed to measure the expressions of hippocampal tissue CREB and p-CREB,the Morris water maze test was used to detect the behavior of rats in each group and the wet-dry method was applied to test brain water content. Results The brain water content at 12 h after TBI in the TBI group was remarkably risen compared with the normal group and control group;the expression levels of hippocampal CREB and p-CREB at 12 h after TBI in the TBI group were significantly decreased compared with the normal group and control group, the latent stage was increased and the frequency searching the accuracy within 2 min was decreased. Conclusion Brain edema is obvious after TBI and the levels of CREB and p-CREB are decreased, which maybe one of the reasons for the impairment of learning and memory function after TBI.
ABSTRACT
Objective To investigate the dynamic images of traumatic penumbra(TP)in a rat model of traumatic brain injury (TBI)treated with AQP4-RNAi using multimodal MRI(MM-MRI)at 7.0 Tesla. Methods A rat model of TBI was established by the improved Feeney's method.MRI scans were performed including T2WI,DWI,ADC and SWI.The pathological changes in penumbra area were observed. All statistical analyses were performed using SPSS 21.0 software package. Results Over time rs-T2WI,rs-DWI and rs-SWI in TBI group were gradually increased. The r-ADC began to increase at one hour after trauma and reached the peak at 6 h. Then it began to fall and reached the bottom at 12 h.At each time point, the difference was statistically significant(P<0.05).Compared with RNA interference group, the rs-T2WI and rs-DWI in TBI group were decreased significantly at 6 h and 12 h(P<0.05),the r-ADC was not decreased significantly at 1 h(P>0.05)and decreased significantly at 6 h and 12 h(P<0.05).At each time point, there was no significant difference in rs-SWI(P>0.05).The area of mismatch between rs-SWI and rs-DWI was the most obvious at 6 h and 12 h,and the area of mismatch was decreased in size after treatment with AQP4-RNAi.There was no significant difference between TBI group and placebo group(P>0.05).In two groups, similar pathological changes were observed, which was depicted as vasogenic edema predominantly at 1 h and mixed edema at 6 h and 12 h. In RNA interference group, intracellular edema was markedly reduced at 6 h and 12 h,and the vasogenic edema was relieved to some extent at 12 h.Conclusion The treatment with APQ4-RNAi markedly alleviates cerebral edema.MM-MRI can reflect its pathological changes. The area of mismatch between SWI and DWI can prompt early detection of traumatic penumbra, which may provide useful information for clinical treatment.

ABSTRACT
BACKGROUND: Previous studies have shown that the complex of polyethylene glycol diacrylate and chitosan is biodegradable, has the anti-inflammation effect and can promote wound healing, with good biocompatibility. However, the composite material is brittle, and thus, it is necessary to develop a mechanical support material due to the anatomy of the nasal cavity.OBJECTIVE: To study the cytocompatibility of a novel degradable absorbable nasal packing material made of polyethylene glycol diacrylate composite hydrogel.METHODS: In our experiment, the composite hydrogel was formed by the combination of different ratios of polyethyleneglycol diacrylate, chitosan and polyvinyl alcohol using photo-crosslinking method. According to the international standard of medical devices, we used direct and indirect international standard experimental methods to evaluate the cytotoxicity of the materials. (1) Direct contact method: the polyethylene glycol acrylate composite hydrogel and mouse fibroblasts L929 were co-cultured for 24 hours to observe the morphological changes of cells. (2) Indirect extraction: mouse fibroblasts L929 were cultured in polyethylene glycol diacrylate composite hydrogel extracts (100, 50 μL) and RPMI1640 medium (100 μL), respectively, for 3 days to observe the proliferation and cytotoxicity by cell counting kit-8 assay.RESULTS AND CONCLUSION: (1) Direct contact method: the cells around the material formed a flat structure with fusiform or stellated protrudes. The material was confirmed to have good biocompatibility depending upon the presence of adherent cell growth, full and strong refraction, and the overall growth trend. (2) Indirect extraction method: with the increase of time, the number of cells in the hydrogel extract group increased, the relative proliferation rate of cells increased gradually, and the cytotoxicity was 0 to 1. To conclude, these experimental findings indicate that the polyethylene glycol acrylate composite hydrogel material has good cytocompatibility.
ABSTRACT
Objective@#To evaluate the value of combined detection of negative costimulatory molecule B7-H4 and carcinoembryonic antigen (CEA) in diagnosing malignant and benign pleural effusion.@*Methods@#Ninety-seven pleural effusion specimen were collected, 55 of which were diagnosed as malignant pleural effusion and 42 were benign pleural effusion. Enzyme-linked immunosorbent assay(ELISA) was used to examine the concentration of B7-H4 and CEA in pleural effusion. Electro-chemiluminescence immunoassay was used to detect the CEA level in pleural effusion. Receiver operating characteristic (ROC) curve was established to analyze and evaluate the single or combined detection of B7-H4 and CEA in diagnosing malignant and benign pleural effusion.@*Results@#The concentrations of B7-H4 and CEA in malignant pleural effusion (MPE) group were (60.08±35.04) ng/ml and (41.49±37.16) ng/ml, respectively, obviously higher than (27.26±9.55) ng/ml and (2.41±0.94) ng/ml of benign pleural effusion (BPE) group (both P<0.01). Area under curve (AUC) of B7-H4 was 0.884 in MPE groupand the diagnostic sensitivity and specificity were 81.8% and 90.5%, respectively, at the optimized cut off value of 37.25 ng/ml. Likewise, area under curve (AUC) of CEA was 0.954 and the sensitivity and specificity were 87.3% and 95.2%, respectively, at the cut off value of 4.18 ng/ml. When B7-H4 >37.25 ng/ml or CEA>4.18 ng/ml, the sensitivity of diagnosis as MPE was down-regulated to 90.9% and the specificity was elevated to 88.1%. When B7-H4 >37.25 ng/ml and CEA>4.18 ng/ml, the sensitivity of diagnosis as MPE was down-regulated to 78.2% and the specificity was elevated to 97.6%. The sensitivity and specificity of combined detection of B7-H4 and CEA to diagnose MPE were elevated to 90.9% and 97.6%, respectively. The level of B7-H4 in MPE and BPE were both positively correlated with CEA (r=0.670, P=0.001 in MPE and r=0.002, P=0.001 in BEP).@*Conclusions@#B7-H4 is a potential tumor marker in diagnosing the benign and malignant pleural effusion. Although the diagnostic value of B7-H4 may not precede to CEA, the combined detection of B7-H4 and CEA can improve the diagnostic sensitivity and specificity of MPE.

Objective To investigate factors associated with prevalence, awareness, treatment and control of hypertension in Dagang Oil Field, and to provide the scientific evidence for the intervention measurements. Methods A cross-sectional, population-based survey was conducted in Dagang Oil Field from 2008 to 2010. Totally, 16 553 adults, 8 174 men (49.38%) and 8 379 women (50.62%), completed all of the questionnaires, measurement examination and blood specimen collection. Results The prevalence of hypertension was 27.30%. Multivariate logistic regression analysis showed that male, old age, overweight or obesity, family history of hypertension, marriage, education attainment, snoring, high heart rate, fasting blood glucose, serum total protein, and hyperuricemia were associated with hypertension. The awareness, treatment, control rates among all hypertensive subjects, and control rate among treated hypertensive participants were 52.65%, 40.07%, 15.51%, and 38.78%, respectively. Awareness was more common in those who were 45-64 years old , high medical costs, family history of hypertension, and smoking currently. Odds ratio (OR) (95% CI) of awareness was 5.24 (2.78 to 9.88), 24.11 (11.91 to 48.84) and 27.46(13.92 to 54.18) for medical costs 1-59.9 yuan/month, 60-199.9 yuan/month, and≥200 yuan/month, respectively. In contrast, exercising 2-4 times/week and longer sleep duration were associated with lower awareness. Medication treatment was more common in those who had old age, high medical costs, hypertension family history, coronary heart disease, prefer vegetable, while less common in those who exercise occasionally, sleep longer and drink alcohol currently. OR (95%CI) of treatment was 10.89 (3.76 to 31.56), 54.07 (19.20-152.24), and 87.31 (31.54 to 241.75) for medical costs 1-59.9 yuan/month, 60-199.9 yuan/month, and ≥200 yuan/month, respectively. Controlled hypertension was more common in those who had old age, high medical costs, exercise 2-4 times/week, coronary heart disease, prefer crude fibre diet, but less common in those who drink alcohol currently, have higher heart rate, and higher education attainment. OR (95%CI) of controlled hypertension was 13.59 (6.50-28.44), 14.67 (7.01-30.68), and 17.43 (8.34-36.39) for medical costs 1-59.9 yuan/m, 60-199.9 yuan/m, and≥200 yuan/m, respectively. Conclusion Despite high rate of awareness, treatment and control of hypertension still need to be strengthened.
ABSTRACT
[ABSTRACT]OBJECTIVETo compare the differences of aerodynamic characteristics of pharyngeal cavity between normal subjects and OSAHS patients, and to study the nasal obstruction in the pathogenesis of the OSAHS.METHODSA total of 60 normal subjects and 60 OSAHS patients were enrolled in this study. Numerical simulation was performed to calculate the airflow dynamic indexes of three sections of pharyngeal cavity. Correlation analysis of the nasal resistance and negative pressure were studied.RESULTSThe average pharyngeal negative pressure and airflow velocity in OSAHS patients were significantly increased than that in normal subjects. The total nasal airway resistance significantly correlated with the average negative pressure of pharyngeal cavity. CONCLUSIONAirflow dynamic indexes of OSAHS patients had significant different pharyngeal aerodynamic characteristics from normal subjects. The increased average negative pressure in pharynx might contribute to the severity of pharyngeal collapse for OSAHS patients. Higher total nasal resistance might play a role in the pathophysiological mechanisms of OSAHS.
ABSTRACT
[ABSTRACT]OBJECTIVETo compare the aerodynamic differences before and after nasal cavity ventilation expansion techniques in patients with OSAHS. METHODSA total of 30 adult patients with OSAHS were included in this study. Pre-operative upper airway CT of each subject was acquired. Each subject underwent surgery. Postoperative upper airway CT was obtained at least 3 months later. By means of CFD simulation method, numerical simulation was performed to calculate the airflow dynamic indexes of the upper airway. The pre-and post-operative aerodynamic characteristics were compared. RESULTSAfter operation, post-operative negative pressure nephogram of the nasal cavity indicated more smooth variation of pressure gradient, post-operative velocity nephogram of nasal cavity indicated slower airflow velocity. Proper values of flow field indicated the volume of nasal cavity increased significantly (t=4.025,P<0.01), the total nasal airway resistance decreased significantly (t=-2.065,P<0.01). The total negative pressure of the upper airway decreased significantly (t=-2.659,P<0.01) after operation. However, the proper values of flow field increased in 5 patients. CONCLUSIONNasal cavity ventilation expansion effectively increase the volumes of nasal cavity, improve the nasal ventilation, and reduce the air flow velocity of both nasal cavity and pharynx in OSAHS patients, those reduce the pharyngeal negative pressure and the pharyngeal collapse. However, for patients with its narrowest segment at the velopharyngeal level, operation cannot do anything or even aggravate the pharyngeal collapse. Operation should be performed individually.
ABSTRACT
[ABSTRACT]OBJECTIVETo evaluate the curative effect of endoscopic sinus surgery from the viewpoint of biomechanics analysis.METHODSWe select one case undergoing nasal endoscopic surgery for chronic sinusitis patients, after half year follow-up and collect preoperative and postoperative spiral CT data to build a numerical model of nasal cavity and paranasal sinuses with computer numerical simulation (CFD) research methods, to explore nasal airflow distribution and flow field characteristics of value changes before and after the operation. Patients undergo subjective assessments: nasal obstruction of VAS score, olfactory VAS score, SNOT-20, objective assessment: Lund Kennedy score, Lund Mackay score, saccharin test time and acquisition clinical efficacy data to verify the credibility of the analysis method is feasible and the results. RESULTSThe nasal resistance after endoscopic sinus surgery in nasal cavity and paranasal sinuses numerical simulation decreased, middle meatus of nose and nasal air flow velocity decreased, the pressure change slightly;nasal threshold region, middle turbinate head end section in maxillary sinus ostium section, after nearly sieve horizontal cross-section average velocity decreased, nasal airflow for the mucosa of the shear stress reduced. The volume of the nasal cavity and the area of the mucosa was increased, the volume ratio was decreased, the subjective and subjective assessment of the patients was improved, and the objective assessment was lighten.CONCLUSIONAerodynamic rules are presented through the calculation of numerical simulation of fluid mechanics which is invisible before. Better understanding of changes in the nasal airflow distribution and the changes of flow field characteristics can be useful to establish nasal endoscopic surgery in numerical analysis of offshore platform, which provides a new research tool for the understanding of the pathophysiology of chronic sinusitis by endoscopic sinus surgery.
ABSTRACT
<p><b>BACKGROUND</b>Aberrant epithelial repair has been observed in chronic rhinosinusitis (CRS) patients; however, the mechanism of epithelial cell repair regulation is unclear. Epidermal growth factor (EGF) plays an important role in regulating epithelial cell repair in lower airway and may be a critical factor in the remodeling processes of CRS. The objective of our research is to evaluate the differences between CRS and normal subjects and between chronic rhinosinusitis without nasal polys (CRSsNP) and chronic rhinosinusitis with nasal polys (CRSwNP) in the regulation of EGF pathways and the regulating proliferative position of classic Ras/Raf/MEK/ERK pathways.</p><p><b>METHODS</b>We evaluated the proliferation rates of ethmoidal mucosal cells before and after stimulation with EGF, epidermal growth factor receptor (EGFR) kinase inhibitor AG1478, and extracellular signal-regulated kinase 1/2 (ERK1/2) inhibitor PD98059 using MTT assays. We also analyzed the sinonasal epithelial cells collected from control subjects and patients with CRS subtypes CRSsNP and CRSwNP for the expression of ERK1/2, phosphorylated ERK1/2, P21, P15, and P27 using western blotting analyses.</p><p><b>RESULTS</b>The proliferation rates of sinonasal epithelial cells before and after EGF stimulation were lower in CRS patients than in the controls. AG1478 or PD98059 inhibitor treatment of control epithelial cells did not result in a significant difference in proliferation. Although, AG1478 and PD98059 inhibited the proliferation of CRS cells, the degree of proliferation inhibition was markedly different in CRSsNP. AG1478 suppressed the proliferation of CRSwNP epithelial cells, whereas PD98059 had no effect. The ratio of ERK1/2 phosphorylation in CRS cells was lower than that of the control cells. Cyclin-dependent kinase inhibitors were highly expressed in CRS cells compared with that of control cells. ERK1/2 and P27 showed differential expression in CRSsNP and CRSwNP.</p><p><b>CONCLUSIONS</b>Differences existed in EGF pathways in CRS patients and normal subjects as well as in CRSsNP and CRSwNP. Classical Ras/Raf/MEK/ERK pathway may assume absolute superiority in control cells. Ras/Raf/MEK/ERK classical pathway and other pathways might be active at the same time to stimulate epithelial cell proliferation in CRSsNP. The function of Ras/Raf/MEK/ERK classical pathway was weaker in CRSwNP than in CRSsNP and when the classical pathway was blocked in CRSwNP, some other pathway could have completely compensated the proliferation induced by the Ras/Raf/MEK/ERK pathway.</p>

This paper proposed a mobile vital-signs monitoring system based on ZigBee localization and wireless transmission technology for the elderly in nursing home. The system can monitor the vital-signs (pulse, ECG and blood oxygen), localize human body and warn in emergency. The validity and accuracy of this system were testified by the experiments of mobile acquisition and storage of pulse. In these experiments, the measurement of pulse ranged from 50 to 170 time a minute, the mean error of which was less than 3%. The mean error of localizing was less than 4 m. And the data transmission rate was 250 kbps. The system, which effectively conducts the real-time monitoring of the health and safety situation for the elderly, has a great significance for protecting the elderly's life safety.
