ABSTRACT

Objective:To investigate the effect of Ranae Oviductus (RO) on ovarian follicular development, phosphatidylinositol 3-kinase (PI3K)/protein kinase B (Akt) signaling pathway, and pregnancy function in rats, and the estrogen-like mechanism of OR. Method:Seventy female Wistar rats were randomly divided into a normal group, a progynova+ luteohormone group (1 mg·kg<sup>-1</sup>+40 mg·kg<sup>-1</sup>), a clomiphene group (10 mg·kg<sup>-1</sup>), and high-dose(400 mg·kg<sup>-1</sup>) and low-dose(200 mg·kg<sup>-1</sup>) RO groups. Rats were administered correspondingly by gavage for eight weeks. After seven weeks of intragastric administration, the estrus cycle of all rats was measured. After eight weeks of intragastric administration, four rats from each group were selected to give birth. For other rats, blood was collected on the day of estrus, and the serum levels of estradiol (E<sub>2</sub>),progesterone (P), testosterone (T), follicle-stimulating hormone (FSH), and luteotropic hormone (LH) were detected. Uterus and ovaries were extracted and weighed to calculate organ index. One ovary was made into pathological sections, and the follicles at different developmental stages and corpus luteum were counted. Real-time fluorescence-based quantitative polymerase chain reaction(Real-time PCR)and Western blot were performed on the other ovary to detect mRNA and protein changes in the PI3K/Akt signaling pathway. Forty female Kunming mice were randomly divided into a normal group and RO groups (400 mg·kg<sup>-1</sup>) with 14 days, 28 days, and 56 days of intervention. Mice in the RO groups were raised with male mice in cages after intragastric administration of OR for 14, 28, and 56 days, respectively. After 18 days, the number of intrauterine fetuses on both sides and the number of stunted fetuses were counted. Result:After eight weeks of intragastric administration of OR, the rats showed decreased uterine index (<italic>P</italic><0.05), declining serum LH (<italic>P</italic><0.05), reduced luteum (<italic>P</italic><0.01), dwindled primary follicles (<italic>P</italic><0.05), and increased rate of follicle atresia (<italic>P</italic><0.01). Additionally, more luteal or interstitial glands degenerated into interstitial structures in the ovarian cortex in a short time. The mRNA expression levels of PI3K and Akt in the ovary were elevated (<italic>P</italic><0.01), while the mRNA expression levels of mTOR and PTEN were reduced (<italic>P</italic><0.01). The phosphorylation level of Akt protein showed a downward trend without significant difference. For the rats, the number of fetuses was decreased (<italic>P</italic><0.05). The pregnancy rate of mice was decreased to varying degrees after administration of RO for different durations, with the lowest in the 14 day RO group, as low as 30%. After 28 days of intragastric administration of RO, the difference in left and right uterine pregnancy increased (<italic>P</italic><0.05). Conclusion:Long-term administration of RO can lead to premature ovarian failure by over-stimulating the ovary, which is similar to clomiphene. Short-term administration can result in decreased pregnancy rate, excessive ovulation on one side, and inhibition of ovulation on the other side. The influence on follicles needs further exploration.

ABSTRACT

Objective:To explore the clinical efficacy and mechanism of Quyu Qingjintang in the treatment of stroke complicated with lung infection (SCLI) with phlegm-heat accumulation lung syndrome and blood stasis syndrome. Method:The 60 patients with SCLI with phlegm-heat accumulation lung syndrome and blood stasis syndrome were selected and divided into control group (30 cases) and observation group (30 cases). The patients in both groups received basic treatments such as thrombolysis and anticoagulation, and were injected with imipenem cilastatin sodium and ambroxol hydrochloride. The patients in control group additionally received Tanreqing capsule on the basis of the conventional treatment while those in observation group were additionally treated with Quyu Qingjintang on the basis of the conventional treatment. The clinical efficacy, lung function, inflammatory factor levels, time to symptoms disappearance, haptoglobin (HPT) and amyloid A (SAA) levels, T lymphocyte subset level and safety index were compared between two groups. Result:The total effective rate was 93.33% (28/30) in observation group, significantly higher than 70.00% (21/30) in control group (<italic>χ<sup>2</sup>=</italic>5.450, <italic>P</italic><0.05). After treatment, the symptoms of both groups were improved (<italic>P</italic><0.05). The scores of fever, cough, wheezing, and expectoration in observation group were lower than those in control group (<italic>P</italic><0.05). The levels of interleukin-6(IL-6), white blood cell(WBC), procalcitonin(PCT), C-reactive protein(CRP), HPT, and SAA in observation group were significantly lower than those in control group (<italic>P</italic><0.05). The forced expiratory volume in one second (FEV<sub>1</sub>), forced vital capacity (FVC) and peak expiratory flow rate (PEF) in observation group were better than those in control group (<italic>P</italic><0.05). The levels of CD3<sup>+</sup>, CD4<sup>+</sup>, CD4<sup>+</sup>/CD8<sup>+</sup> in observation group were significantly higher than those in control group (<italic>P</italic><0.05). The recovery time of WBC count, hospitalization time, lung rales, the time to fever and cough disappearance in observation group were lower than those in control group (<italic>P</italic><0.05), no serious adverse reactions occurred in two groups. Conclusion:Quyu Qingjintang in the treatment of stroke complicated with lung infection with phlegm-heat accumulation lung syndrome and blood stasis syndrome can significantly improve the treatment efficiency, improve the symptoms of pulmonary infection, reduce the level of inflammatory factors, and improve lung function, with less adverse reactions and high safety, so it is worthy of clinical application.

ABSTRACT

<b>Objective::To observe the clinical efficacy of modified Qingjin Huatan Tang on bronchiectasis with syndrome of phlegm-heat accumulating lung at acute exacerbation and its inhibitory effect on pro-inflammatory factors and proteolytic activity. <b>Method::One hundred and thirty patients were randomly divided into control group and observation group by random number table. Patients in control group got tazobactam sodium and piperacillin sodium for injection, 3.375 g/time, 1 time/6 hours, and the types of antibiotics were regulated according to the bacterial culture results. And patients in control group also got Ambroxol Hydrochloride injection, 30 mg/time, 2 time/days, and postural drainage. In addition to the therapy of control group, patients in observation group were also given modified Qingjin Huatan Tang, 1 dose/day. Before and after treatment, symptoms and signs were scored. And levels of white blood cell count (WBC), neutrophile granulocyte (GRAN), C-reactive protein (CRP), procalcitonin (PCT) were detected. And scores of forced expiratory volume in one second (FEV<sub>1</sub>), forced vital capacity (FVC), peak expiratory flow rate (PEFR) and BODE were graded. And levels of tumor necrosis factor-<italic>α</italic> (TNF-<italic>α</italic>), interleukin-4 (IL-4), IL-6 and IL-8 in sputum, peripheral neutrophil elastase (NE) and cathepsin G were detected. <b>Result::By rank sum test, the clinical efficacy in observation group was better than that in control group (Z=2.086, <italic>P</italic><0.05), while scores of symptoms and signs in observation group were lower than those in control group (<italic>P</italic><0.01). WBC, GRAN, CRP, PCT, airflow limitation (O), dyspnea (D), motor ability (E) score, BODE index, TNF-<italic>α</italic>, IL-4, IL-6, IL-8, plasma NE and cathepsin G were all lower than those in control group (<italic>P</italic><0.01). And levels of FEV<sub>1</sub>, FVC, PEF and FEV<sub>1</sub>/FVC were higher than those in control group (<italic>P</italic><0.01). <b>Conclusion::In addition to routine anti-infection and expectoration western medicine therapy, modified Qingjin Huatan Tang can be added to control symptoms and signs, alleviate the degree of illness, improve pulmonary function and the quality of life of patients, and inhibit expression of airway pro-inflammatory factor and proteolysis, with a better clinical efficacy than pure western medicine.

ABSTRACT

Objective:To observe the efficacy of Yuebi Jia Banxiatang on old patients with community acquired pneumonia (CAP) combined with syndrome of phlegm heat damming lung, and observe effect on inflammatory markers and prognosis. Method:A total of 120 patients with CAP were randomly divided into control group (60 cases) and observation group (60 cases) by random number table. In control group, 55 patients finished the therapy (3 patients fell off or were lost to follow-up, 2 were eliminated), 55 patients in observation group completed the therapy (5 patients fell off or were lost to follow-up). Both groups' patients got anti-infection and other comprehensive therapies. Patients in control group got Feilike mixture, 20 mL/time, 3 times/day. Patients in observation group obtained Yuebi Jia Banxiatang, 1 dose/day. The course of treatment for the two groups continued for 10 days. Antipyretic time and antipyretic rate at the 5th day after treatment, relief time of cough, expectoration and lung rale were recorded. And clinical pulmonary infection score (CPSI), time (CPSI<6) and time to stop antibiotics were recorded. Before and after treatment, syndrome of phlegm heat damming lung was scored, and failure of initial treatment was also recorded. Levels of procalcitonin (PCT), red blood cell volume distribution wid (RDW), D-dimer (D-D) and synpeptin, high sensitivity C-reactive protein (hs-CRP), tumor necrosis factor-α (TNF-α), interleukin (IL-1β) and IL-6 were measured. Recurrence rate of chest radiograph was recorded, and safety was evaluated. Result:Relief time of antipyretic, cough, expectoration and lung rale in observation group were less than those in control group (P<0.01). Antipyretic rate at the 5th day after treatment, antibiotic discontinuation rate, recurrence rate of chest radiograph were 85.45% (47/55), 94.55% (52/55) and 90.91% (50/55), which were higher than 65.45% (36/55), 81.82% (45/55) and 74.55% (45/55) in control group. And initial treatment failure rate was 5.54% (3/55), which was lower than 20.00% (11/55) in control group (P<0.05). Scores of CPSI and syndrome of phlegm heat damming lung were less than those in control group (P<0.01). And levels of PCT, RDW, D-D, peptide, hs-CRP, TNF-α, IL-1β and IL-6 were all below the level in control group (P<0.01). Total effective rate of traditional Chinese medicine (TCM) syndrome was 96.36% (53/55), which was higher than 83.64% (46/55) in control group (χ2=4.949, P<0.05). And there was no adverse reactions relating to traditional Chinese medicine. Conclusion:In addition to anti-infection and other comprehensive therapies, Yuebi Jia Banxiatang can control the clinical symptoms, reduce the degree of pulmonary infection and disease, control the inflammatory reaction, shorten the course of disease and improve the prognosis, with a significant effect and safety in clinical use.

ABSTRACT

BACKGROUND@#Takayasu arteritis-induced renal arteritis (TARA), commonly seen in Takayasu arteritis (TA), has become one of the main causes of poor prognosis and early mortality in patients with TA. TARA progressing into Takayasu arteritis-induced renal artery stenosis (TARAS), could lead to severe complications including malignant hypertension, cardiac-cerebral vascular disease, and ischemic nephropathy. Since there existed no guidelines on treatments, this study aimed to review the comprehensive treatments for TARA.@*METHODS@#We searched systematically in databases including PubMed, Ovid-Medline, EMBASE, Web of Science, China National Knowledge Infrastructure, Wanfang, and SinoMed, from inception to May 2018. Literature selection, data extraction, and statistical analysis were performed.@*RESULTS@#Eighty-two literatures were recruited focusing on medical treatments (n = 34) and surgical treatments (n = 48). We found that combined medical treatments of glucocorticoids and conventional synthetic disease-modifying anti-rheumatic drugs could reach high rates of remission in patients with TARA, and biological disease-modifying anti-rheumatic drugs were preferred for refractory patients. After remission induction, surgical treatment could help reconstruct renal artery and recover renal function partly. Percutaneous transluminal angioplasty was the first choice for patients with TARAS, while open surgery showed a good long-term survival.@*CONCLUSIONS@#Patients with TARA should benefit both from medical treatments and from surgical treatments comprehensively and sequentially. Multidisciplinary team coordination is recommended especially in patients with severe complications.

ABSTRACT

OBJECTIVE@#To evaluate short-term clinical results of fixed-bearing unicompartmental knee arthroplasty (UKA) for patients with medial compartmental knee osteoarthritis.@*METHODS@#From January 2015 to December 2017, 62 patients with medial compartmental knee osteoarthritis were treated by fixed-bearing UKA. Among them, including 19 males and 43 females, aged from 47 to 83 years old with an average of (65.3±8.2) years old. The courses of disease ranged from 5 to 72 months with an average of(19.4±14.3) months. Postoperative complications were observed, VAS score was used to observe degree of pain relief, Hospital for Special Surgery(HSS) score was used to evaluate clinical effects.@*RESULTS@#All patients were followed up from 4 to 40 months with an average of(19.9 ±10.2) months. The incision healed well without blood transfusion, hospital stays ranged from 2 to 10 days with an average of (3.5±1.6) days. No deep infection, dislocation, loosing or per prosthetic fracture occurred after operation, while 1 patient suffered from posterior tibial vein thrombosis. HSS score improved from 69.9±7.2 before operation to 90.1±7.4 at final following-up, and 51 patients obtained excellent results, 9 patients good and 2 moderate. VAS score decreased from 3.8±0.9 before operation to 1.1±0.9 at final following-up.@*CONCLUSIONS@#Fixed-bearing UKA for medial compartmental knee osteoarthritis could obtain short-term clinical results for shorter hospital stay, less complications. with fewer complication and fast recovery.

OBJECTIVE@#To analyze and evaluate the clinical outcomes of enhanced recovery after surgery(ERAS) for hemiarthroplasty for the treatment of femoral neck fractures in elderly patients.@*METHODS@#From January 2015 to December 2016, 466 patients with femoral neck fractures were admitted in our department. Among them, 154 patients were more than 80 years old, with an average age of (83.9±3.2) years old (ranged, 80 to 96 years old), including 27 males and 127 females. According to Garden classification system, 68 cases were type III and others were type IV. ERAS for hemiarthroplasty were performed for all the patients. The time before operation, blood transfusion rate, complications rate, lenth of stay and Harris score system were evaluated.@*RESULTS@#Among 154 patients, 142 patients were followed up, with an average duration of 24.6 months(ranged, 14 to 38 months). Thirty-two patients (20.8%) completed the operation within 48 hours after admission, 67 patients(43.5%) completed the operation within 72 hours after admission, 76 patients(49.4%) discharged within 48 hours after operation, and the blood transfusion rate was 15.9%. One patient was re-hospitalized because of prosthetic dislocation. The re-hospitalization rate was 0.6%. No other patients were re-hospitalized because of superficial or deep infection or periprosthetic fracture. At the latest follow-up, the Harris hip score was 90.2±7.1, 106 patients got an excellent result, 23 good and 9 fair.@*CONCLUSIONS@#ERAS for hemiarthroplasty can effectively shorten hospitalization time, relieve pain, reduce blood transfusion rate, reduce postoperative complications, and do not increase the readmission rate of elderly patients with femoral neck fracture, which can achieve good clinical efficacy.

ABSTRACT

<p><b>BACKGROUND</b>Approximately 15-20% cases of systemic lupus erythematosus (SLE) are diagnosed in children. There have been a few studies reporting the epidemiological data of pediatric-onset SLE (cSLE) in China, neither comparing the differences between cSLE and adult-onset SLE (aSLE). The aim of this study was to describe the impact of age of onset on clinical features and survival in cSLE patients in China based on the Chinese SLE Treatment and Research group (CSTAR) database.</p><p><b>METHODS</b>We made a prospective study of 225 cSLE patients (aged Results: The mean age of cSLE patients was 12.16 ± 2.92 years, with 187 (83.1%) females. Fever (P < 0.001) as well as mucocutaneous (P < 0.001) and renal (P = 0.006) disorders were found to be significantly more frequent in cSLE patients as initial symptoms, while muscle and joint lesions were significantly less common compared to aSLE subjects (P < 0.001). The cSLE patients were found to present more frequently with malar rash (P = 0.001; odds ratio [OR], 0.624; 95% confidence interval [CI], 0.470-0.829) but less frequently with arthritis (P < 0.001; OR, 2.013; 95% CI, 1.512-2.679) and serositis (P = 0.030; OR, 1.629; 95% CI, 1.053-2.520). There was no significant difference in SLE disease activity index scores between cSLE and aSLE groups (P = 0.478). Cox regression indicated that childhood onset was the risk factor for organ damage in lupus patients (hazard ratio 0.335 [0.170-0.658], P = 0.001). The survival curves between the cSLE and aSLE groups had no significant difference as determined by the log-rank test (0.557, P = 0.455).</p><p><b>CONCLUSIONS</b>cSLE in China has different clinical features and more inflammation than aSLE patients. Damage may be less in children and there is no difference in 5- year survival between cSLE and aSLE groups.</p>

ABSTRACT

Objective:To investigate the therapeutic effects of Gengnian Anshu granule on the endocrine system of menopausal rats. Methods:The rats aged 13-15 months received intragastrical treatment with Gengnian Anshu granule at the doses of 1. 9,0. 95 and 0. 48 g·kg - 1 ·d - 1 for 6 weeks. At the end of treatment,serum and hypothalamic hormone levels were detected,and the organ indices including uterus and ovaries were determined. Results:In the menopausal rats,Gengnian Anshu granule could significantly in-crease the serum estradiol(E2 )(P < 0. 05 or P < 0. 01),decrease serum follicle-stimulating hormone(FSH)and luteotrophic hor-mone(LH)(P < 0. 05 or P < 0. 01),increase norepinephrine(NE),dopamine(DA),5-hydroxytryptamine(5-HT)and β-endor-phine(β-EP)(P < 0. 05),increase the number of cells positive for estrogen receptor(ER)protein expression in hypothalamus(P <0. 05 or P < 0. 01),and decrease the number of cells positive for gonadotropin-releasing hormone(GnRH)protein expression(P <0. 01). Conclusion:With a multi-target action mechanism and an integral regulatory role,Gengnian Anshu granule can provide favor-able regulation against endocrine disorder in menopausal rats.

ABSTRACT

Objective:To study the preventive and therapeutic effects of Jiechangyan Qixiao granule ( JQX) in the rats with ulcer-ative colitis ( UC) induced by dextran sulfate sodium .Methods:The UC model was induced by drinking dextran sulfate sodium ( DSS, 4%) freely in Wistar rats weighting 180-220g.Guben Yichang tablets and sulfasalazine was used as the standard drugs for the compari -son.After 7-day intragastric administration of Jiechangyan Qixiao granules at the dose of 2.70, 1.35 and 0.68 g· kg-1 · d-1 , the se-rum levels of malondialdehyde (MDA), superoxide dismutase (SOD), nitric oxide (NO), interleukin -6(IL-6)and tumor necrosis factor-α(TNF-α), and the protein expression of myeloperoxidase (MPO) and intercellular adhesion molecule (ICAM-1)in colon and nuclear factor(NF-kBp65) were detected.Results:Compared with the model group, the granule at high and medium dose could sig-nificantly increase the activity of SOD in blood and decrease the contents of MDA , TNF-αand IL-6 (P<0.05 or P<0.01).The granule could also notably decrease the MOP activity in colonic mucous of UC rats (P<0.05 or P<0.01), and the contents of NF-kBp65 and ICAM-1 in the inflammation reaction(P<0.01).Conclusion: JQX shows promising efficacy in the treatment of UC rats induced by dextran sulfate sodium .

ABSTRACT

Objective To investigate the variation of stress distributions on proximal femur after hip resurfacing arthroplasty (HRA) by using three-dimensional (3D) finite element method. Methods The 3D finite element model of proximal femur was reconstructed based on 64-slice spiral CT scan image data. Both the stress distributions on proximal femur after metal-on-metal HRA and normal proximal femur were studied, so as to analyze the biomechanical environment changes after HRA. Results After HRA, the superior, anterior, rear and inferior area of the proximal femoral head showed significant stress shielding, with peak stress of 0.60, 0.57, 0.66, 0.79 MPa, respectively, and stress shielding rate of 99.80%, 99.16%, 98.92%, 96.66%, respectively. Increased stress occurred in most regions of the distal femoral head, while stress shielding appeared only in rear area of the distal femoral head, with stress shielding rate of 4.92%. Increased stress occurred in anterior region of the proximal femoral neck, while stress shielding appeared in the superior, inferior and rear area of the proximal femoral neck, with shielding rate of 16.48%, 22.75% and 7.83%, respectively. Increased stress also occurred in inferior area of the distal femoral neck, while the remaining area showed stress shielding. The stress in greater trochanter increased by 9.22%, and the stress shielding rate for lesser trochanter area and basal area of femoral neck were 2.49% and 14.44%, respectively. Conclusions Stress distributions on most regions of proximal femur after HRA were similar to that on normal femur, and the stress transfer was close to physiological status, which could effectively avoid obvious stress shielding in proximal femur and preserve bone mass, which could contribute to normal physiological activity of patients.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the early clinical results of two stage hip replacement after failed internal fixation for femoral neck fractures in young patients.</p><p><b>METHODS</b>From June 2008 to June 2010,24 patients with femoral head necrosis caused by failed internal fixation were treated with hip arthroplasty. Among them, 12 patients were male and 12 patients were female, with an average age of 42.9 years old (ranged, 18 to 58). According to Harris score and X-ray examination, the clinical result was evaluated.</p><p><b>RESULTS</b>Twenty-three cases were followed up with an average age of 34.4 months (ranged, 25 to 48). After operation, 1 case complicated with the dislocation of hip joint. No deep infection of hip joint, prosthetic loosing or peripheral fracture was found. The mean Harris score was 90.9 +/- 4.3, and 18 obtained excellent results, 4 good and 1 fair.</p><p><b>CONCLUSION</b>Although treatment of femoral head necrosis with two stage hip replacement after failed internal fixation is difficult during operation, its early result is satisfactory.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study short-term results of hip resurfacing arthroplasty (HRA) in the treatment of patients with avascular necrosis of femoral head (ANFH), and to explore indication and strategy in this surgery.</p><p><b>METHODS</b>From December 2006 to December 2009, 37 patients (43 hips) with avascular necrosis of femoral head were treated with total hip resurfacing arthroplasty. Among the patients, 25 patients were male and 12 patients were female, with an average age of 44.5 years (ranged, 21 to 67 years). According to ARCO classification, 3 hips were 3A stage, 6 hips were 3B stage, 16 hips were 3C stage and 18 hips were 4 stage. X-ray evaluation of the patients were conducted. The clinical results were evaluated by the Harris hip score system including pain,range of motion, correction of deformity and total function.</p><p><b>RESULTS</b>Thirty-four patients (40 hips) were followed up with an average period of 32.4 months (ranged, 16 to 53 months), and 3 patients were lost. Thirty-seven hips got complete relief of joint pain and 3 patients feel aching pain after walking. There was 1 heterotopic ossification, no femoral neck fracture no dislocation, no infection and no revision in all patients. From pre-operation to present, the average Harris hip score improved significantly from (51.5 +/- 1.7) to (94.3 +/- 1.4). Thirty-seven hips got an excellent result, 3 hips good and no poor.</p><p><b>CONCLUSION</b>The total hip resurfacing arthroplasty is an effective solution for the problems of the younger and active patients with ANFH, and the short-term results are satisfying.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the incidence, predisposing factors and therapeutic modalities of acute colonic pseudo-obstruction (ACPO) in patients after total hip arthroplasties (THA) and total knee arthroplasties (TKA).</p><p><b>METHODS</b>From January 2006 to December 2009, 12 patients with ACPO after THA and TKA operation were investigated retrospectively,who were viewed as the ACPO group. There were 10 males and 2 females with an average age of (78 +/- 12) years in the ACPO group. Other 853 patients without ACPO after THA and TKA operation were viewed as the control group treated at the same period. The incidence of ACPO was calculated. The clinical data were collected and compared between the two groups including patient age, gender, procedure, anesthetic class, clinical presentation, radiographic findings, duration from index surgery to diagnosis of ACPO, treatment, postoperative mobilization time, and length of hospital stay.</p><p><b>RESULTS</b>The incidence of ACPO was 1.4%. The incidence of primary THA (1.3%) was higher than that of primary TKA (0.4%); the incidence of hip and knee revisions (5.0%) was higher than that of primary THA and TKA (1.0%); there was no difference in incidence between hip revisions (5.5%) and knee revisions (4.0%). The mean age was (78 +/- 12) years old in ACPO group and (71 +/- 13) in the control group. The male/female ratio was 5:1 in ACPO group and 2:3 in control group. There were statistical differences in mean age and gender ratio between the two groups. No association was found with respect to anesthetic class. On average, ACPO occurred at 2.5 days after index surgery. The abdominal distention occurred in all 12 cases, nausea or vomiting in 8 cases and abdominal pain in 3 cases. Radiographically cecal dilation occurred in all cases and intestinal dilation in 3 cases. All patients initially were treated conservatively with immediate cessation of oral intake,a nasogastric tube and oral mineral oil. Three patients received a rectal tube. Only 1 patient required endoscopic decompression. There were no deaths after ACPO in the series. Mean mobilization time after surgery averaged (5.0 +/- 2.2) days in ACPO group compared with (2.5 +/- 1.1) days in the control group. Mean hospital stay averaged (16.5 +/- 6.4) days in ACPO group compared with (10.5 +/- 4.5) days in the control group. There were statistical differences in mean mobilization time after surgery and mean hospital stay between two groups.</p><p><b>CONCLUSION</b>ACPO mainly happened in old male patients. The majority cases response to conservative treatment and their prognoses are good. But ACPO will delay mobilization time after surgery and increase hospital stay.</p>

ABSTRACT

<p><b>OBJECTIVES</b>To compare the indication, technical keys during surgery and early results of humeral head replacement and internal fixation for the 3 parts and 4 parts fractures of proximal humerus in the elderly.</p><p><b>METHODS</b>From July. 2004 to July. 2006,58 patients with 3 parts and 4 parts fractures of proximal humerus were randomly treated with AO locked compressive plates (LCP) or humeral head replacement. In LCP group, there were 12 males and 16 females with an average age of (66.5 +/- 1.4) years, including 24 cases of 3 parts fractures, 1 case of 3 parts fractures with dislocation, 2 cases of 4 parts fractures and 1 case of 4 parts fractures with dislocation. In the humeral head replacement group,there were 12 males and 18 females with an average age of (68.9 +/- 3.5) years, including 9 cases of 3 parts fractures, 2 cases of 3 parts fractures with dislocation, 12 cases of 4 parts fractures, 4 cases of 4 parts fractures with dislocation and 3 cases of humeral head split fractures. Trauma series X-rays of shoulder were taken after operation, 2-week, 6-week, 10-week, 16-week, 6-month and the latest followed-up. VAS, SST (simple shoulder test) questionnaire, ASES (American shoulder & elbow surgeon)score, Constant-Murley score and UCLA score had been adopted for evaluation at the latest followed-up.</p><p><b>RESULTS</b>The mean followed-up period of LCP group was 29.8 months and that of humeral head replacement group was 28.2 months. VAS of LCP group and humeral head replacement group were (2.2 +/- 1.5) and (2.6 +/- 1.9), respectively. There was a statisticaly difference between the two groups (P = 0.002). The degrees of forward elevation of shoulder in LCP group and humeral head replacement group were 110.2 degrees (81 degrees to 130 degrees) and 120.2 degrees (89 degrees to 140 degrees), respectively. There was a statistical difference between the two groups (P = 0.031). ASES score, Constant-Murley score, UCLA score and response "yes" in SST questionnaire in LCP group were 87.7 +/- 2.2, 83.9 +/- 6.8, 30.3 +/- 2.2, 8.0 +/- 0.9, and in humeral head replacement group were 86.4 +/- 4.5, 85.5 +/- 5.6, 31.2 +/- 2.1, 9.0 +/- 0.7, respectively. There was no significant difference between the two groups in ASES score, Constant-Murley score, UCLA score and SST questionnaire.</p><p><b>CONCLUSION</b>Both humeral head replacement and internal fixation are effective treatment for the 3 parts and 4 parts fracture of proximal humerus in the elderly. Humeral head replacement is more technically demanding and has a strict indications. The satisfactory results of humeral head replacement are based on the suitable placement of prostheses and the correct reconstruction of rotator cuff insertion.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the short-term results of metal on metal hip resurfacing arthroplasty (HRA) for the treatment of patients with ankylosing spondylitis (AS), to explore the indications and technology of this surgery.</p><p><b>METHODS</b>From February 2006 to April 2008, 11 patients (15 hips) with ankylosing spondylitis were treated with metal on metal total hip resurfacing arthroplasty. Among them, 9 patients were male, and 2 patients were female, with an average age of 32.5 years (ranged from 16 to 53 years). The comparative study on preoperative and postoperative pain, range of motion, correction of deformity and function evaluation were performed. The preoperative and postoperative Harris scores were compared and the feasibility and technical difficulty were analyzed.</p><p><b>RESULTS</b>Ten patients were followed up for an average period of 16.2 months (ranged from 8 to 34 months) and 1 patient was lost. Pain disappeared in 10 patients. There were no heterotopic ossification, no femoral neck fracture, no dislocation, no infection and no revision in all patients. From preoperation to present, the mean flexion angle of hip was improved from preoperative 0 degrees to 75 degrees to postoperative 35 degrees to 105 degrees; the mean abducting angle of hip was improved from preoperative 0 degrees to 30 degrees to postoperative 15 degrees to 55 degrees; and the average Harris hip score improved significantly from (30.9 +/- 3.4) (2-47) to (85.1 +/- 3.1) (46-94). According to evaluation criteria, 10 hips got an excellent result, 3 good and 1 poor.</p><p><b>CONCLUSION</b>The total hip resurfacing arthroplasty is an effective solution for the treatment of the younger and active patients with AS and it shows satisfactory short-term results. It is very significant to analysis the different conditions of patients so as to choose proper strategy.</p>

<p><b>OBJECTIVES</b>To investigate the role of glycyrrhizin on TGFbeta1 stimulated signaling transduction in rat hepatic stellate cells (HSCs).</p><p><b>METHODS</b>The mice HSCs were isolated and cultured with or without glycyrrhizin (1 micromol/L-1000 micromol/L) in vitro after TGFbeta1 stimulation. The mRNA level of Smad2, 3, 7 were measured with RT-PCR; protein expression level of Smad2, 3, 7 and collagen I, III were analyzed with Western blot.</p><p><b>RESULTS</b>TGFbeta1 increased the mRNA level and protein expression of Smad2, 3, 7 in HSC; it also increased protein expression of collagen I and III. 1 micromol/L-1000 micromol/L glycyrrhizin decreased the mRNA level and protein expression of Smad2, 3, 7; it also inhibited protein expression of collagen I and III gradually.</p><p><b>CONCLUSION</b>Interventing the TGFbeta signaling pathway and decreasing the synthesis of collagen, might be involved in the anti-fibrosis mechanism of glycyrrhizin.</p>

ABSTRACT

Objective To design,synthesize and screen high efficient small interfering RNA(siRNA) targeting to cyclooxygenase-2(COX-2)on rheumatoid arthritis synovial fibroblasts(RASF).To further study the effect of specific COX-2 siRNA interfering on mediators of inflammatory cytokines.Methods Four pairs of siRNA for human COX-2 mRNA were synthesized by utilizing RNA design software,while another random sequence was designed as control.They were divided into group A to H.Among them,group A was used as the negative control(CTL),and group B to F were transfected as random siRNA(NC),1#~4#siRNA in order. These siRNAs were transferred into RASF by LipofectAMINE2000 package and PMA(phorbol-12-myristate- 13-acetate)was added into each culture and with a final concentration of 100 nmol/l.RASF was collected 48 hours after transfection.The expression of hCOX-2 at mRNA level was determined by reverse transcription- polymerase chain reaction(RT-PCR)and hCOX-2 protein level by Western Blot.The supernatant levels of PGE_2,IL-1?,IL-6,TNF-?and vascular endothelial growth factor(VEGF)of the above groups were detected by ELISA.Results The levels of hCOX mRNA and protein in RASF treated with 4-#siRNA were significantly lower than those of the negative control and other groups.The level of PGE_2 and cytokines like IL-1?,IL-6, TNF-?and VEGF in the supernatant were lower in the 4#siRNA group than in other groups.Conclusion 4#siRNA can effectively inhibit the expression of COX-2 mRNA and the synthesis of the COX-2 protein in human synovial fibroblasts.The level of PGE_2,IL-1?,IL-6,TNF-?and VEGF is the lowest in the super- natant.Thus 4#siRNA has been confirmed to specifically block the COX-2 in human synovial fibroblasts.
