ABSTRACT

Silk fibroin is a natural polymer with certain water solubility, structural modification, good biocompatibility and biodegradability, which can be used as a drug delivery carrier material. As a promising drug delivery system, drug-loaded silk fibroin nanoparticles can control drug release, reduce toxicity and improve therapeutic effects. In this paper, the basic characteristics of silk fibroin, the preparation methods of drug-loaded silk fibroin nanoparticles and the application of silk fibroin in nanoparticulate drug delivery systems are reviewed, and on this basis, the further development of drug-loaded silk fibroin nanoparticles is prospected.

ABSTRACT

OBJECTIVE@#To explore the effect of electro-acupuncture (EA) on glucose and lipid metabolism in unmarried patients with polycystic ovary syndrome (PCOS).@*METHODS@#Fifty-four PCOS patients were equally randomized into true acupuncture group and sham acupuncture group (control) for totally 16 weeks of treatment by random method with a computerized randomization program. Patients in true acupuncture group accepted traditional acupuncture methods with EA and two sets of acupoint groups were used alternatively. The first set consisted of Zhongji (CV 3), Qihai (CV 6), Guilai (ST 29), Sanyinjiao (SP 6), Yinlingquan (SP 9), Hegu (LI4) and Baihui (GV 20), and the second set consisted of Tianshu (ST 25), ST 29, CV 3, CV 6, SP 6, Taichong (LR 3), Neiguan (PC) 6 and GV 20. Patients in the sham acupuncture group accepted shallow acupuncture methods through EA without electricity at 4 non-meridian points in each shoulder and upper arm. Outcome measures included body mass index (BMI), waist-hip-ratio (WHR), oral glucose tolerance test (OGTT), insulin release test, glucose and lipid metabolism indicators such as total cholesterol (TC), triglycerides, high-density lipoprotein cholesterol (HDL-C), low-density lipoprotein cholesterol, adiponectin, leptin, visfatin, resistin, and interleukin (IL-6).@*RESULTS@#Twenty-six subjects in the true acupuncture group and 20 subjects in the sham group completed the clinical trial. After 16 weeks of treatment, no significant difference in the outcome measures were observed between the two groups (P>0.05). However, as compared with baseline data, a reduction in weight, BMI, hipline, WHR, fasting glucose, homeostatic model assessment of insulin sensitivity, visfatin and HDL-C, and an increase in resistin and IL-6 were observed in the true acupuncture group (P<0.05). In addition, a reduction in visfatin and an increase in TC were also observed in the sham group (P<0.05).@*CONCLUSIONS@#Acupuncture may have a beneficial effect in the treatment of PCOS by improving glucose and lipid metabolism. Moreover, the sham acupuncture may be not completely ineffective. Sham acupuncture may improve some of the aspects of the glucose and lipid metabolism of PCOS patients through a placebo effect. ABSTRACT

The components of traditional Chinese medicine(TCMCs) are the basic unit of raw materials for Chinese medicines, and their physical and chemical properties directly affect the choice of dosage forms and the optimization of prescriptions. However, most of TCMCs are multi-component complex systems, and the characterization of their overall properties is still in the exploration stage. On the basis of biological activity, the representative components are determined, and then the individual characteristics are fitted with the weight coefficient of efficacy contribution rate, which may provide reference for characterizing the overall properties of TCMCs. In this study, with the pharmacological effects of isoproterenol(ISO)-induced myocardial ischemia in rats as the indicators, the pharmacodynamic contribution rates of three representative components of chishao terpene glucoside components(CSTGCs) were evaluated by the normalization weighting method. The contribution rates of paeoniflorin, paeoniflorin and benzoylpaeoniflorin were 54.87%, 32.46% and 12.67%, respectively. The oil-water partition coefficients of paeoniflorin, albiflorin, benzoylpaeoniflorin in water and buffer solutions with different pH values were measured, and the oil-water partition coefficients of CSTGCs were characterized by the weight of their pharmacodynamics contribution rate. The results showed that the apparent oil-water partition coefficient(log P) of CSTGCs in the phosphate buffer system such as n-octanol-water(pH 2.0, 2.5, 5.0, 5.8, 6.8) were 0.18-0.22, indicating that CSTGCs have common absorption and low permeability, providing basis for the preparation of CSTGCs.

OBJECTIVE@#To examine the clinical effects of Yisui Shengxue Granules () in the treatment of β-thalassemia and explore its mechanism on DNA methylation levels.@*METHODS@#A randomized placebo-controlled double-blinded trial was conducted. Forty patients with β-thalassemia were recruited and distributed randomly by envelope method into an experimental group and a control group, 20 patients in each group. The patients were given Yisui Shengxue Granules in the experimental group and placebo in the control group (12 g/bag three times a day) during a 3-month intervention. Before and after 1, 2, and 3 months of treatment, peripheral intravenous blood was sampled, and blood parameters such as hemoglobin (Hb), red blood cells (RBCs), reticulocytes (Ret), and fetal hemoglobin (HbF) were analyzed. Mononuclear cells from 5 patients, who showed an obvious treatment effect, were isolated by density gradient centrifugation. DNA methylation was analyzed using an Affymetrix USA GeneChip Human Promoter 1.0 Array and Input-promoter 1.0.@*RESULTS@#Compared with pre-treatment, there was an obvious increase in Hb and RBCs counts after 1, 2, and 3 months in the experiment group (P<0.01 or P<0.05). Meanwhile, HbF increased from the 2nd to the 3rd month (P<0.05). In the control group, Hb and RBCs showed no obvioas change. After 3-month treatment, DNA methylation results from 5 patients revealed that there were 24 hypomethylated genes and 3,685 hypermethylated genes compared with pre-treatment. Genes of insulin-like growth factor 1 receptor (IGF1R) and Janus kinase 3 (JAK3) revealed the most relations with other genes (degree: 21) and genes of 1-phosphatidylinositol-4, 5-bisphosphate phosphodiesterase gamma 2 (PLCG2) and mitogen-activated protein kinase 10 (MAPK10) showed a stronger intermediary role (betweenness centrality=0.04).@*CONCLUSIONS@#JAK3 and MAPK10 are two key genes in bone marrow and the lymphatic system, and JAK3 is likely to be related to hematopoietic cytokines in the process of early hematopoiesis. ABSTRACT

Objective: To discuss the myocardial protection effect of Qishen Yiqi dropping pills on patients with Qi deficiency and blood stasis syndrome after PCI and the control effect and mechanism on cardiac adverse events. Method: One hundred and twenty-five patients were randomly divided into control group (61 cases) and observation group (64 cases) by random number table after PCI. Patients in control group got Tigrillo tablets, 90 mg/time, 2 times/days, aspirin enteric-coated tablets, 100 mg/time, 1 time/day, and bisoprolol fumarate tablets, 5 mg/time, 1 time/day. In addition to the therapy of control group, patients in observation group were also given Qishen Yiqi dropping pills, 0.5 g/time, 3 times/days, 3 days before operation. A course of treatment was 6 months, and follow-up were carried out for 6 months. Before treatment, and at the 24th, 72th hour after treatment, cardiac troponin I (cTnI) and creatine kinase isoenzyme (CK-MB) were detected. During the first week after operation, and at the 6th month after operation, left ventricular ejection fraction (LVEF), settling velocity (SV), wall motion score index (WMSI), percentage of normal myocardium and myocardial perfusion score index (PS) were recorded. After 12 months of the operation, recurrence of angina pectoris, severe arrhythmia, heart failure, revascularization, non-fatal myocardial infarction, cardiogenic death and in-stent restenosis (ISR) were all recorded. During the first week after the operation, and at the 6th month after operation, six-minute walking test (6 MWT), Seattle angina scale (SAQ) and Qi deficiency and blood stasis syndrome were all scored. Before the operation, and at 48 hours and 6 months after operation, levels of tumor necrosis factor (TNF-α), interleukin-6 (IL-6), plasma soluble intercellular adhesion molecule-1 (sICAM-1), high-sensitivity C-reactive protein (hs-CRP), superoxide dismutase (SOD), malondialdehyde (MDA) and endothelin-1 (ET-1) were all detected. Result: At the 24th, 72th hour after treatment, levels of cTnI and CK-MB in control group were lower than those in observation group (Pth hour after operation and the 6th month after operation, levels of TNF-α, IL-6, s ICAM-1 hs-CRP, MDA and ET-1 were lower than those in control group (PPth months after operation, LVEF, SV and percentage of normal myocardium were higher than those in control group (PPPPth month after operation, score of six-minute walking test was higher than that in control group (PPPConclusion: Qishen Yiqi dropping pills has anti-inflammatory and antioxidant effects, and can ameliorate intradermal function, prevent and treat myocardial injury, reduce rate of MACE, relieve symptoms, and improve activity ability and quality of life of patients.

ABSTRACT

Objective: To discuss the clinical effect of modified Yuyetang to type 2 diabetes mellitus (T2DM) combined with coronary heart disease (deficiency of Qi and Yin or phlegm syndrome and stasis syndrome) and the mechanisms of anti-inflammation and anti-oxidation. Method: One hundred and eighteen patients were randomly divided into control group (58 cases) and observation group (60 cases) by random number table. Patients in control group was orally given insulin or special blood glucose for hypoglycemic agents, aspirin enteric-coated tablets, 100 mg/time, 1 time/day. Patients with dyslipidemia got atorvastatin, 10 mg/time, 1 time/day. And patients with hypertension got captopril tablets, 12.5-25.0 mg/time, 1 time/day. In addition to the therapy of control group, patients in observation group were also given modified Yuyetang, 1 dose/day. A course of treatment was 3 months. Before and after treatment, levels of glycated hemoglobin (HbA1c), fasting blood glucose (FPG), 2-hours postprandial glucose (2 hPG), blood lipids triglyceride (TG), total cholesterol (TC), high density lipoprotein cholesterol (HLD-C), low-density lipoprotein (LDL-C), apolipoprotein A (ApoA), apolipoprotein B (ApoB), procalcitonin (PCT), homocysteine (Hcy), interleukin-6 (IL-6), tumor necrosis factor-alpha (TNF-alpha) and hypersensitive C-reactive protein (hs-CRP), superoxide dismutase (SOD), malondialdehyde (MDA), glutathione peroxidase (GSH-Px) and 8-Isoprostaglandin F2a (8-ios-PGF2a) were detected, Gensini and traditional Chinese medicine (TCM) syndrome were scored, and electrocardiogram was detected. Result: After treatment, levels of FPG, HbA1c, LDL-C, ApoB, scores of Gensini and TCM syndrome, PCT, Hcy, IL-6, TNF-α and hs-CRP in observation group were all lower than those in control group (PPPPPPConclusion: In addition to hypoglycemic, lipid-lowering and antiplatelet therapies, Modified Yuye decoction can regulate glucose and lipid metabolism of patients with T2DM and coronary heart disease, relieve degree of coronary artery stenosis, with anti-inflammatory and anti-oxidative stress effect.

ABSTRACT

Objective To compare the clinical efficacy and safety of electroacupuncture and hormone replacement therapy (HRT) on the treatment of perimenopausal syndrome (PMS). Methods Clinical randomized controlled trial literature about electroacupuncture and HRT for the treatment of PMS in CNKI, Wanfang database, CBM, Chongqing Weipu, PubMed and Cochrane Library from establishment of the databases to 28thof January 2017 was retrieved by computers. Two researchers screened out data and extracted materials quality. Qualities of the literature were assessed by relevant standards in the Cochrane Handbook 5.1.0. Meta-analysis was conducted with RevMan5.3 software. Results 6 articles with 457 participants met the inclusion criteria and were included in this study. Meta-analysis showed that during the treatment or the follow-up period of 6 months, there was no significant difference in the improvement of serum E2and Kupperman scores between the electroacupuncture and HRT. With the prolongation of treatment time, electroacupuncture showed superiority over HRT in depressing the level of serum FSH [MD=-5.93, 95%CI (-9.90, -1.96), P=0.003], with statistical significance. During the treatment period, the electroacupuncture group had fewer adverse reactions. Conclusion Electroacupuncture can effectively treat PMS, with similar efficacy as HRT, and with good safety, which can provide references for clinic. However, the quality of included studies is generally low, yet more large sample, multicenter, high-quality clinical RCTs are needed for further validation.

ABSTRACT

<p><b>Background</b>Although much attention has been paid to the pharmacokinetics (PKs) of different factor VIII (FVIII) concentrates in persons with hemophilia A (HA), limited information is available in young boys with severe HA. In this study, we aimed to assess the PK parameters of FVIII products in boys with severe HA in China.</p><p><b>Methods</b>A total of 36 boys (plasma-derived [pd]-FVIII, n = 15; recombinant [r] FVIII, n = 21) were enrolled between January 2015 and May 2016 in Beijing Children's Hospital. PK characteristics of FVIII products were studied according to a reduced 4-sampling time point design (1 h, 9 h, 24 h, and 48 h postinfusion).</p><p><b>Results</b>The mean FVIII half-life (t) was 10.99 ± 3.45 h (range 5.52-20.02 h), the mean in vivo recovery (IVR) was 2.01 ± 0.42 IU/dl per IU/kg (range 1.24-3.02 IU/dl per IU/kg) and mean clearance (CL) of FVIII is 4.34 ± 1.58 ml·kg·h (range 2.29-7.90 ml·kg·h). We also analyzed the influence of several parameters that potentially modulate FVIII PK. The age was closely associated with FVIII half-life (R = 0.32, P < 0.01). The tof FVIII increased by 0.59 h per year. Besides age, von Willebrand factor antigen (VWF:Ag) also was associated with FVIII half-life (R = 0.52, P < 0.01). Patients with blood Group O had a shorter FVIII half-life than patients with non-O blood group (9.40 ± 0.68 h vs. 12.3 ± 0.79 h, t = 2.70, P = 0.01). The FVIII IVR correlated with age (R = 0.21, P < 0.01) and VWF:Ag level (R = 0.28, P < 0.01). CL rates were faster in young patients and in those with low-VWF:Ag levels. CL rates of FVIII are higher in blood Group O versus non-blood Group O persons (5.02 ± 0.38 vs. 4.00 ± 0.32 ml·kg·h, t = 2.53, P = 0.02).</p><p><b>Conclusions</b>Chinese boys with severe HA have similar PK values to other ethnic groups and large differences in FVIII PK between individual patients. Age, blood group, and VWF:Ag levels are important determining factors for FVIII CL.</p>

The present study was designed to improve storage stability and oral bioavailability of Ganneng dropping pills (GNDP) by transforming lignans of Herpetospermum caudigerum (HL) composed of herpetrione (HPE) and herpetin (HPN) into nanosuspension (HL-NS), the main active ingredient of GNDP, HL-NS was prepared by high pressure homogenization and lyophilized to transform into solid nanoparticles (HL nanoparticles), and then the formulated HL nanoparticles were perfused into matrix to obtain NS-GNDP by melting method. For a period of 3 months, the content uniformity, storage stability and pharmacokinetics test in vivo of NS-GNDP were evaluated and compared with regular GNDP at room temperature. The results demonstrated that uniformity of dosage units of NS-GNDP was acceptable according to the criteria of Chinese Pharmacopoeia 2015J. Physical stability of NS-GNDP was investigated systemically using photon correlation spectroscopy (PCS), zeta potential measurement, and scanning electron microscopy (SEM). There was a slight increase in particles and PI of HL-NS re-dispersed from NS-GNDP after storage for 3 months, compared with new formulated NS-GNDP, which indicated a good redispersibility of the NS-GNDP containing HL-NS after storage. Besides, chemical stability of NS-GNDP was studied and the results revealed that HPE and HPN degradation was less when compared with that of GNDP, providing more than 99% of drug residue after storage for 3 months. In the dissolution test in vitro, NS-GNDP remarkably exhibited an increased dissolution velocity compared with GNDP and no distinct dissolution difference existed within 3 months. The pharmacokinetic study showed that HPE and HPN in NS-GNDP exhibited a significant increase in AUC, C and decrease in T when compared with regular GNDP. These results indicated that NS-GNDP possessed superiority with improved storage stability and increased dissolution rate and oral bioavailability.

ABSTRACT

Objective To investigate the display status of ultrasonography imaging check in central nervous system (CNS) in infants of early pregnancy and the diagnostic value of CNS malformation in infants of early pregnancy.Methods Gestational weeks of 2 751 enrolled subjects were divided according to the ultrasonic measurement of the crown rump length (CRL):11-11 +6 weeks group,12-12+6 weeks group,and 13-13 + 6 weeks group,prenatal ultrasound were performed to examine fetal CNS anatomy in infants of early pregnancy,record the display status in each groups of infants and analyze the relationship between the display situation and gestational age.Results Fourteen cases of fetal CNS malformation (20 malformations) in total were found by prenatal ultrasound,and the incidence of CNS malformation was about 5.09% (14/2 571).Wherein,12 cases of early pregnancy were diagnosed,and 2 cases of middle pregnancy were diagnosed.The sensitivity of ultrasound of early pregnancy in the diagnosis of fetal CNS malformation was 85.71%.In the group of research,the ultrasound display ratios of 11-11+6 weeks group,12-12+6 weeks group and 13-13+6 weeks group were 96.73%,97.94%,98.06%,respectively.There was no significant difference in early pregnancy fetal CNS display ratio among groups (x2 =1.56,v =2,x2< x0.05.2 =5.99,P > 0.05).Conclusions The display rate of CNS structure in infants of early pregnancy (11-13+6 weeks)is higher,and is not affected by gestational weeks.Prenatal ultrasound can effectively diagnose CNS severe malformation in infants of early pregnancy.

ABSTRACT

Objective To assess the clinical value of compression-braking pants in preventing postoperative bleeding and pressure sore after transcatheter arterial chemoembolization (TACE).Methods A total of 50 patients with liver cancer,who received interventional treatment via femoral artery route at authors' hospital during the period from May 2015 to May 2016,were enrolled in this study.The patients were randomly divided into observation group and control group with 25 patients in each group.In the observation group,the patients were asked to put on the compression-braking pants after TACE procedure and routine postoperative nursing care was executed as usual.In the control group,routine postoperative nursing care and compression of puncture point with sandbag were carried out.After TACE,the conditions of local congestion,local hematoma,formation of pseudoaneurysm at the puncture site,etc.in both groups were recorded.The local skin conditions,including skin moist feeling,burning sensation,feeling of numbness and rubefaction were determined.Results The postoperative bleeding incidence in the observation group was 4%,which was obviously lower than 32% in the control group.No pressure sore occurred in both groups.In aspect of skin moist feeling,burning sensation,feeling of numbness and rubefaction,the observation group was significantly superior to the control group (P<0.05).Conclusion The use of compression-braking pants after TACE can effectively prevent the occurrence of postoperative bleeding and pressure sore.The compression-braking pants have reasonable design and simple structure,and it is easy to put on and take off the pants.The use of the pants can improve patient's comfort and satisfaction.Therefore,it is worth promoting its use in clinical practice.

ABSTRACT

Objective To evaluate the application of nursing quality indicators in gastrointestinal surgery department. Methods The indicator system was established by use of the Delphi method, applied in nursing quality control in gastrointestinal surgery department, and was used to analyze and improve nursing quality continuously according to the routine data collection. Results After the application of quality indicators for 6 months, the complications′ observation and prevention awareness rate of the patients increased from 75.00% to 92.00%(x2=4.200, P<0.05), and most of the process indicators had improved. Conclusions The nursing quality indicators provide basis for nursing quality management,and provide standards and guidance for clinical works.

ABSTRACT

BACKGROUND: Vertebroplasty (PVP) and kyphoplasty (PKP) are important methods for the treatment of osteoporotic vertebral compression fractures in the elderly. Although bone cement has certain liquidity and vertebral fractures are often in different situations, bone cement leakage rate is still high. OBJECTIVE: Based on the theoretical discussion and clinical analysis, to study the theoretical causes of bone cement leakage and effective prevention methods. METHODS: A total of 162 cases (186 vertebrae) were treated with three methods of vertebroplasty. Group A: 64 cases with 78 vertebrae were treated with conventional cemented vertebroplasty; Group B: 57 cases with 65 vertebrae were treated with cemented vertebroplasty using cement pump; Group C: 41 cases with 43 vertebrae were treated with balloon kyphoplasty. The leakage of bone cement was observed in three groups. RESULTS AND CONCLUSION: Of the 186 vertebrae, postoperative bone cement leakage occurred in 25 vertebrae, with the leakage rate of 13% (25/186). Group A had bone cement leakage in 11 vertebrae, and the leakage rate was 14% (11/78). Group B had bone cement leakage in 8 vertebrae, and the leakage rate was 12% (8/65). Group C had bone cement leakage in 6 vertebrae, and the leakage rate was 14% (6/43). There was no significant difference among the three groups in the leakage rate of bone cement (P > 0.05). That is to say, the causes of bone cement leakage are not completely controllable, and the leakage position has some randomness. Strict and careful imaging monitoring is an intuitive method to prevent bone cement leakage.

ABSTRACT

Objective To develop an instrument for measuring self-management in Chinese cancer patients and to test its reliability and validity.Methods Based on Bandura's social cognitive theory,self-efficacy theory and self-determination theory,with three tasks and six skills of self-management as framework,combining interview results of 20 cancer patients,item pool of Cancer Patient Self-management Assessment Scale was established.The items were selected by Delphi technique and pilot survey,and the provisional version scale was formed.Totally 300 cancer patients were selected for investigation to test the reliability and validity.Results Cancer Patient Self-management Assessment Scale consisted of 44 items.The total scale's Cronbach's α coefficient was 0.959,split-half reliability coefficient was 0.932,2-week test-retest reliability was 0.461.Exploratory factor analysis extracted six common factors,and the cumulative variance contribution rate was 61.090%;the content validity index(S-CVI) of the total scale was 0.900.Conclusion The developed Cancer Patient Self-management Assessment Scale has good reliability and validity,which can be used as a tool to measure self-management in Chinese cancer patients.

ABSTRACT

BACKGROUND: Vertebroplasty (PVP) and kyphoplasty (PKP) are important methods for the treatment of osteoporotic vertebral compression fractures in the elderly. Although bone cement has certain liquidity and vertebral fractures are often in different situations, bone cement leakage rate is still high. OBJECTIVE: Based on the theoretical discussion and clinical analysis, to study the theoretical causes of bone cement leakage and effective prevention methods. METHODS: A total of 162 cases (186 vertebrae) were treated with three methods of vertebroplasty. Group A: 64 cases with 78 vertebrae were treated with conventional cemented vertebroplasty; Group B: 57 cases with 65 vertebrae were treated with cemented vertebroplasty using cement pump; Group C: 41 cases with 43 vertebrae were treated with balloon kyphoplasty. The leakage of bone cement was observed in three groups. RESULTS AND CONCLUSION: Of the 186 vertebrae, postoperative bone cement leakage occurred in 25 vertebrae, with the leakage rate of 13% (25/186). Group A had bone cement leakage in 11 vertebrae, and the leakage rate was 14% (11/78). Group B had bone cement leakage in 8 vertebrae, and the leakage rate was 12% (8/65). Group C had bone cement leakage in 6 vertebrae, and the leakage rate was 14% (6/43). There was no significant difference among the three groups in the leakage rate of bone cement (P > 0.05). ABSTRACT

<p><b>OBJECTIVE</b>To explore the effects and related mechanisms of total flavone of epimedium treatment(TFE)on primary callus for mation in ovariectomized rats.</p><p><b>METHODS</b>Forty male SD rats weighted from 209 to 246 g and aged 6 to 8 weeks were selected. Six weeks after ovariectomy a femur fracture model with middiaphyseal segment fracture was established, estimated and randomly divided into TFE group (150 mg·kg⁻¹·d⁻¹) and control group(received saline). HE staining was used to evaluate the morphologic difference of primary callus during the bone callus healing between these two groups. The relative expression of Runt-related transcription factor 2(Runx2) mRNA in the callus was identified by real-time polymerase chain reaction. Immunohistochemical technique was used to observe the Casein kinase 2-interacting protein 1(CKIP-1) protein level in the callus of the two groups. Maximum fracture load was tested by three point bend test.</p><p><b>RESULTS</b>The BMD, primary callus volume, trabecular member(Tb.N) and trabecular thickness(Tb.Th) were higher in TFE group than that in control group(<0.001). The Tb.N and Tb.Th of primary callus were higher in TFE group than control group (=0.001). The volume and bone volume/tissue volume of primary callus were in TFE group than control group(<0.01). The trabecular separation(Tb.Sp) of primary callus were in control group higher than TFE group(<0.01). The HE staining of the 6 week slices showed that the degree of cartilage ossification in callus of the TFE group was significantly higher than that in control group under high magnification. Real-time PCR revealed that the comparative expression of Runx2 mRNA in control group was higher than that in TFE group(<0.001); the positive number of CKIP-1 was less in TFE group than that in control group (<0.001). TFE could increase the maximum load of the primary callus (<0.001).</p><p><b>CONCLUSIONS</b>TFE can promote the cartiage ossification of callus in ovariectomized rats, enhancing the bone strength and bone quality in the process of fracture healing via the CKIP-1/Runx2 pathway.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the serum protein targets of Qianggu Decoction (, QGD) on treating osteoporosis by the proteomics analysis using tandem mass tag (TMT) and liquid chromatographytandem mass spectrometry (LC-MS/MS).</p><p><b>METHODS</b>Twenty serum protein samples were recruited (10 patients with primary type I osteoporosis before and after QGD treatment) and the high abundance ratios protein was removed, two serum samples were extracted and labeled with TMT reagent. Then, mass spectrometric detection, identification of differentially expressed proteins and bioinformatics analysis of differentially expressed proteins were carried out.</p><p><b>RESULTS</b>A total of 60 proteins were identified, within a 99% confidence interval, to be differentially regulated of which, 34 proteins were up-regulated and 26 proteins were down-regulated. Differentially expressed proteins analyzed by Gene Ontology (GO) annotation mainly get involved in 12 different biological processes, 7 types of cellular components, and 6 kinds of molecular functions. Angiotensinogen (AGT), stromelysin-1 (MMP3), heparanase (HPSE) and glyceraldehyde-3-phosphate dehydrogenase (GAPDH) were screened as candidate protein targets of QGD treatment, which were related to metabolic mechanism of bone remodeling and/or bone collagen of osteoporosis. By the utilization of the protein-protein interaction network analysis tool named STRING10.0, it showed that AGT, MMP3, HPSE and GAPDH were located in the key node of the protein-protein interactions network. Furthermore, AGT, MMP3, HPSE and GAPDH were found to be directly related to BMP, MAPK, Wnt, SMAD and tumor necrosis factor ligand superfamily member 11 (TNFSF11) families.</p><p><b>CONCLUSIONS</b>The proteomics analysis by using TMT combined with LC-MS/MS was a feasible method for screening the potential therapeutic targets associated with QGD treatment. It suggests that AGT, MMP3, HPSE and GAPDH may be candidate protein targets of QGD treatment which can be used as therapeutic effect monitor and early diagnosis of primary type I osteoporosis.</p>

ABSTRACT

Objective:To assess the effects of direct moxibustion on 24-hour ambulatory blood pressure (ABP) and clinical symptoms of traditional Chinese medicine (TCM) in elderly patients with essential hypertension, and to explore the antihypertensive effect and influencing factors of moxibustion. Methods:A total of 101 elderly hypertension patients who met the inclusion criteria were randomly assigned to a direct moxibustion I group (n=33), a direct moxibustion II group (n=34), and a control group (n=34). The treatment of calcium antagonist (CCB) orangiotensin II receptor antagonist (ARB) was adopted in the control group. The treatment of direct moxibustion I plus the same medicine as the control group were adopted in the direct moxibustion I group, five cones per acupoint and three times per week, for 5 weeks in total. The treatment of direct moxibustion II plus the same medicine as the control groupwere adopted in the direct moxibustion II group, five cones per acupoint and three times per week, for 5 weeks in total. The changes of 24-hour ABP and clinical symptoms of TCM after treatment were compared in the three groups. Results: The mean 24-hour ambulatory systolic blood pressure (mean 24 h ASBP), night ASBP, percentage of mean 24-hour ambulatory diastolic blood pressure (mean 24 h ADBP)>90 mmHg, and percentage of day ADBP>90 mmHg in the control group were elevated after treatment (P80 mmHg in the direct moxibustion I group was reduced by treatment (P140 mmHg, percentage of day ASBP>140 mmHg, percentage of night ASBP>120 mmHg , mean 24 h ADBP, day ADBP, night ADBP, 24 h ambulatory pulse pressure (APP), after treatment in all groups (P>0.05). The degree of improvement of the clinical symptoms of TCM showed significant differences among the three groups of patients (P<0.01). The total effective rate in the direct moxibustion I group was 73.3%, which was superior to those in the direct moxibustion II group and control group (13.3% and 10.0%, respectively). Conclusion:The direct moxibustion has benign regulative effect on blood pressure of elderly patients with essential hypertension, and improves their clinical symptoms. The direct moxibustion method I (burning the next moxa cone after the previous one had totally burnt out) was superior to method II (burning the next moxa cone when the previous one had not totally burnt out ) in lowering blood pressure and improving symptoms of elderly patients with essential hypertension.
