ABSTRACT
Objective The objective of this study is to improve the accuracy of automatic identification in complex background herbal slice images.The goal is to achieve accurate recognition of herbal slice images in the presence of complex backgrounds.Methods The experiment was conducted on a collected and organized dataset of Tibetan herbal slice images.The RGB,HOG,and LBP features of the slices were analyzed.An improved HOG algorithm was used to fuse multiple features,and a deep learning network was utilized for image recognition.Results The proposed method of multi-feature fusion combined with deep learning achieved an identification accuracy of 91.68%on 3610 Tibetan herbal slice images with complex backgrounds.Furthermore,the average identification accuracy for 20 common traditional Chinese medicine slices,such as Chuan Beimu,Hawthorn,and Pinellia,reached 98.00%.This method outperformed existing methods for identifying herbal slices in complex backgrounds,indicating its feasibility and wide applicability for the identification of other traditional Chinese herbal medicines.Conclusion The fusion of multiple features effectively captures distinguishing characteristics of herbal slices in complex backgrounds.It exhibits high recognition rates for Tibetan herbal slices with complex and heavily occluded backgrounds,and can be successfully applied to the recognition of natural scene-based traditional Chinese herbal medicines and herbal slices.This approach shows promising prospects for practical applications.

Persistent postural-perceptual dizziness(PPPD) is the most common chronic vestibular disease, the clinical manifestation is dizziness, unstable and non-rotational dizziness for three months or more. And the symptom is exacerbated by upright posture, active or passive movement, and complex visual stimuli. In addition, PPPD is a functional disease, so routine vestibular function tests and imaging tests are often negative. According to the diagnostic criteria established by the Barany Association, the diagnosis of PPPD often relies on history. This article provides a review of PPPD-related questionnaires.

Objective:To summarize the characteristics and nursing experience of patients of transthyretin amyloid polyneuropathy.Methods:A retrospective study was performed in 9 patients with transthyretin amyloid polyneuropathy from different area in china from February 2018 to October 2021 in the Second Affiliated Hospital Zhejiang University School of Medical, summarized nursing points:used list to popularize disease knowledge, improved the nursing skills of rare diseases; did a good job in the safety nursing of sensory motor disorders to prevent pressure sores, scalds, fall, thrombosis and other complications, paid attention to the nursing of autonomic nerve dysfunction, prevented postural hypotension. Observe and prevent heart failure, manage diarrhea and constipation, and applied the guide to personalized urinary tract management;provided genetic counseling services, personalized continuing care and psychological care.Results:During hospitalization, the clinical symptoms of 9 patients were improved to varying degrees, after 3 months of telephone follow-up after discharge, 9 patients had no adverse events such as pressure ulcer and fall.Conclusions:By improving specialized skills, predictive risk management and symptom care, the symptoms of postural hypotension, urinary retention, constipation, diarrhea and other symptoms of transthyretin amyloid polyneuropathy patients can be prevented and improved, and adverse events such as falls and pressure sores can be avoided.

Objective To investigate the effect of micro-ribonucleic acid-21(miR-21) on the malignant biological behavior of human cholangiocarcinoma cell line QBC939 by targeting the protein tyrosine phosphatase (PTEN)/inositol phosphate 3-kinase (PI3K)/protein kinase B (Akt) pathway. Methods The cholangiocarcinoma cell line QBC939 in the logarithmic growth phase was divided into empty vector group, blank control group, overexpression group, and silencing group.An inverted fluorescence microscope was used to observe transfection efficiency; MTT assay, flow cytometry, Transwell assay, and wound healing assay were used to measure cell proliferative activity, apoptosis rate, invasion activity, and migration activity.Quantitative reverse transcription PCR was used to measure the mRNA expression levels of miR-21, PTEN, PI3K, Akt, and mammalian target of rapamycin (mTOR); Western blotting was used to measure the protein expression levels of PTEN, PI3K, Akt, phosphorylated Akt (p-Akt), and mTOR; dual-luciferase reporter assay was used to verify the effect of miR-21 on PTEN.A one-way analysis of variance was used for comparison of continuous data between multiple groups, and the SNK- q test was used for further comparison between two groups. Results Transfection efficiency was 90.27%±18.03% in the overexpression group, 91.43%±18.26% in the silencing group, and 92.16%±18.41% in the empty vector group.Compared with the blank control group and the empty vector group, the overexpression group had a significant increase in the proliferative activity of QBC939 cells (both P < 0.05) and a significant reduction in apoptosis rate (both P < 0.01);compared with the blank control group, the empty vector group, and the overexpression group, the silencing group had a significant reduction in proliferative activity ( P < 0.01) and a significant increase in apoptosis rate ( P < 0.01).Compared with the blank control group and the empty vector group, the overexpression group had significant increases in the migration rate of QBC939 cells and number of cells penetrating the membrane (all P < 0.01);compared with the blank control group, the empty vector group, and the overexpression group, the silencing group had significant reductions in migration rate and number of cells penetrating the membrane (all P < 0.01).Compared with the blank control group and the empty vector group, the overexpression group had significant increases in the mRNA expression levels of miR-21, PI3K, Akt, and mTOR and a significant reduction in the mRNA expression level of PTEN (all P < 0.05);compared with the blank control group, the empty vector group, and the overexpression group, the silencing group had significant reductions in the mRNA expression levels of miR-21, PI3K, Akt, and mTOR and a significant increase in the mRNA expression level of PTEN (all P < 0.05).Compared with the blank control group and the empty vector group, the overexpression group had significant increases in the protein expression levels of PI3K, Akt, p-Akt, and mTOR and a significant reduction in the protein expression level of PTEN (all P < 0.01);compared with the blank control group, the empty vector group, and the overexpression group, the silencing group had significant reductions in the protein expression levels of PI3K, Akt, p-Akt, and mTOR and a significant increase in the protein expression level of PTEN (all P < 0.01).Furthermore, miR-21 showed targeted regulation of PTEN expression. Conclusion MiR-21 silencing may inhibit the malignant biological behavior of human cholangiocarcinoma cell line QBC939 by targeting the PTEN/PI3K/Akt signaling pathway to upregulate the expression of PTEN and downregulate the expression of PI3K, Akt, mTOR, and p-Akt.
ABSTRACT
Objective:To investigate the assisted pregnancy clinical effect of autologous peripheral blood mononuclear cells (PBMCs) combined with low molecular weight heparin (LMWH) on patients with intrauterine adhesion separation.Methods:Nine patients with intrauterine adhesion separation were treated with intrauterine perfusion of PBMCs 48 hours before transplantation. The low molecular weight heparin 3 200 IU injected daily subcutaneously were carried out on the day of transplantation. Measurement of endometrial thickness and observation of pregnant outcome were perfomed.Results:After 9 patients were injected into the uterine cavity with autologous PBMCs. It was found that, compared to the previous, the thickness of the endometrium was increased, and the morphology of the endometrium was improved in 6 cases. After transplantation, 7 cases of the patients were clinically pregnant, 1 of the patients was biochemically pregnant, and 1 of the patients was not pregnant; among the 7 clinical pregnancy cases, 1 case was intrauterine compound pregnancy (operation on the affected side of fallopian tube pregnancy had been performed), another 2 cases had embryo termination at the 7-8th week of gestation by B-scan ultrasonography analysis. However, 3 cases of clinical pregnancy in 4 patients with repeated implantation failure. At present, 2 cases were in live labor, and the other three cases were in normal continuous pregnancy.Conclusions:Intrauterine perfusion of human PBMCs combined with low molecular weight heparin is beneficial to the improvement of endometrial receptivity and the pregnancy outcome of some patients after the separation of intrauterine adhesions.

OBJECTIVE:To study the spectrum-effect relationship of HPLC finger print of different polar parts of Ampelopsis grossedentata with its in vivo anti-inflammatory effect. METHODS :A. grossedentata was reflux extracted with 70% ethanol,then extracted with petroleum ether ,chloroform,ethyl acetate and water saturated n-butanol;or it was directly decocted with water and then concentrated to obtain different polar parts. The xylene-induced mice ear swelling model was established ;using dexamethasone as positive control ,anti-inflammatory activity of different polar parts of A. grossedentata was investigated. Fingerprints of different polar parts of A. grossedentata were established by HPLC. The determination was performed on Poroshell 120 EC-C18 column with mobile phase consisted of acetonitrile- 0.1% phosphoric acid solution (gradient elution )at the flow rate of 1 mL/min. The column temperature was 25 ℃. The detection wavelength was set at 365 nm,and sample size was 5 μL. The grey ralational analysis method was used to analyze the spectrum-effect relationship of HPLC fingerprint common peaks of different polar parts of A. grossedentata with its anti-inflammatory effect. The correlation coefficient and correlation degree were calculated and ranked. RESULTS:Anti-inflammatory experiment showed that the anti-inflammatory effects of 70% ethanol extraction part ,ethyl acetate extraction part and water extraction part were the most significant (inhibitory rates of ear swelling were 54.07%,30.54%, 30.45%). Five common peaks were determined in HPLC fingerprints of different polar parts from A. grossedentata . The spectrum-effect analysis results showed that the correlation of5 common peaks were higher than 0.6;among them ,peak 3 and peak 2 (dihydromyricetin) had the strongest anti- inflammatory effect ,and their correlation degrees were both mail:123745789@qq.com greater than 0.8. CONCLUSIONS : The anti-inflammatory effect of A. grossedentata on xylene-induced ear swelling in mice is the result of multi-comp onent synergy ; unknown substance of peak 3 and dihydromyricetin may be the main active components of A. grossedentata .

Objective To analyze the characteristic of articles, authors and citations in Journal of Clincal Neurology from 2008 to 2015.Methods The data about articles, authors and citations in Journal of Clincal Neurology from 2008 to 2015 were analyzed by bibliometrics method.Results There were 1623 papers published in Journal of Clincal Neurology during the 2008 to 2015 years.The average number of articles per issue was 33.8.The average number of article per paper was 0.424.The papers with funded projects were 432 (26.6%).The first authors were mainly come from Jiangsu, Shandong, Beijing, Shanghai and Hebei, etc.The average cooperation degree was 4.17, and the average co-author rate was 95.81%.The average rate of the papers with citation was 93.96%, and the total number of citations was 18708.The citations were mainly come from foreign language (74.3%) and journals (97.7%).The average Price index was 37.72%.Conclusion Journal of Clincal Neurology is an academic journal of high quality, with strong influence in field of clinical neurology.

Objective To explore the level of knowledge,attitude and practice in preventing sudden deafness and the influencing factors among nursing students in Taiyuan City.Methods By random cluster sampling,1 187 nursing students from 6 universities in Taiyuan City were surveyed about the level of knowledge,attitude and practice and the influencing factors by questionnaires in November 2013.The results underwent analysis.Results Total scoring rate of knowledge,attitude and practice of preventing sudden deafness in the nursing students was 48.36% (574/1 187),among which knowledge,attitude and practice was 28.30% (336/1 187),62.68% (744/1 187) and 54.34% (645/1 187),respectively.Logistic regression analysis showed that whether taking health education or not,different parents' education degree and household capita monthly income had significantly different influence on level of knowledge,attitude and practice of preventing sudden deafness,P<0.01.Significant correlation existed between knowledge and attitude,attitude and practice,r=0.467,0.149,P<0.05.Conclusions The level of knowledge,attitude and practice of preventing sudden deafness of the nursing students is low and has muhifaceted influencing factors.Colleges and universities should strengthen the health propaganda,and community should step up intervention to the families of low income and low education degree,at the same time to improve the compliance of health behavior in order to play a role in prevention and treatment of sudden deafness among nursing students.

Objective To investigate the influence on right-to-left shunt (RLS) of patent foramen ovale with contrast transcranial Dopple (cTCD) in different positions.Methods 276 consecutive patients included ischemic cerebrovascular disease (ICVD),migraine,episodic vertigo and syncope.They underwent cTCD at rest and after Valsalva maneuver in 4 positions:upright sitting,supine,left lateral decubitus and right lateral decubitus,in random order.RLS was graded 0 (no microbubbles detected),1 (1-10microbubbles),2 (> 10 microbubbles but no curtain),and 3 (curtain,shower of microbubbles).Meanwhile,each patients was examined by contrast transthoracic echocardiography(cTTE),contrast transesophageal echocardiography (cTEE) or right heart catheterization.Results RLS detection rate was 48.2% (133/276)through cTCD.They were also confirmed patent foramen ovale by 82 cases in cTTE,28 cases in cTEE,and 23 patients in right heart catheterization.At rest,RLS detection rate and microbubbles of 4 positions had no significant difference (P >0.05).After the Valsalva maneuver,RLS detection rate and microbubbles of each position were significantly higher than at rest (P <0.001),and the RLS detection rate and microbubbles in upright sitting were higher than that at rest(P <0.05).Conclusions RLS is best detected in the upright sitting position with Valsalva maneuver with cTCD.Upright sitting detection helps to improve the detection rate and microbubbles of RLS.

OBJECTIVE@#To investigate the factors influencing sterility in males undergoing routine sperm inspection by masturbation.@*METHODS@#Scales for demographic data, self-compiled infertility questionnaire, Symptom Checklist-90 (SCL-90) , and sexual life subscale of Olson Marital Quality Questionnaire (ENRICH) were assessed in 220 cases of sterility in males who had undergone sperm examination after ejaculation.@*RESULTS@#The total SCL-90 scores and the factor scores of anxiety, phobia, somatization, obsessive compulsive behavior, interpersonal-sensitivity, hostility, and depression were significantly higher than the norm (P<0.05). The total SCL-90 score of 69 males was higher than 160, implying that 31.36% of the sterile males had negative emotions. The total score was related to wife's attitude, semen collecting room, ejaculation situation, and the general state of sexual life. The ejaculation situation was subjected to a multivariate linear regression model.@*CONCLUSION@#About 1/3 of males with sterility problems undergoing routine semen examination by masturbation have negative emotions such as anxiety, phobia, somatization, and interpersonal sensitivities. The defective ejaculation may be the influential factor at the stage.

OBJECTIVE@#To investigate the change of fasting serum adiponectin in obstructive sleep apnea-hypopnea syndrome (OSAHS) patients.@*METHOD@#Forty males with OSAHS and forty age-matched male normal controls were included in the study. Subjects in OSAHS group were divided into two sub-groups according to body mass index (BMI): obese OSAHS group (BMI > or =25, n=26) and non-obese group OSAHS (BMI <25, n=14). All subjects underwent an overnight sleep study. The serum adiponectin levels were measured by ELISA.@*RESULT@#(1) Except for BMI,compared with control subjects, levels of fasting adiponectin level were significantly lower in OSAHS subjects (P < 0.05). (2) In obese OSAHS sub-group, serum adiponectin level was negatively correlated with BMI and AHI. However, serum adiponectin level was positively correlated with the minimum oxygen saturation. There were similar correlations between serum adiponectin level and sleep parameters in non-obese OSAHS sub-group.@*CONCLUSION@#Despite age and BMI, fasting adiponectin level was significantly lower in OSAHS patients than that in control subjects. And fasting adiponectin level was correlated with BMI, AHI and the minimum oxygen saturation. OSAHS is one of the main reasons of the decreased adiponectin.

Objective To construct a new recombinant immunotoxin expression vector by using human VEGF165 and a truncated pseudomonas exotoxin A ramification (PE38) gene, and explore the expression of the VEGF165-PE38 fusion protein in HEK293 cells. Methods VEGF165 was cloned by polymerase chain reaction (PCR). PE38 gene was gained from an vector plasmid pRB391 by restriction endonuclease digestion, and then inserted to the eukaryotic expression vector pIRES2-EGFP. After the eukaryotic recombinant vector pIRES2-VEGF165-PE38-EGFP was identified by restriction endonuclease digestion and sequence analysis, the vector was transfected into HEK293 cells by liposome protocol. RT-PCR and ELISA method was used to confirm the expression of the fusion gene in the HEK293 cells. Results Restriction endonuclease digestion and sequence analysis revealed the VEGF165-PE38 fusion gene was cloned into the eukaryotic expression plasmid vector pIRES2-EGFP successfully. The pIRES2-VEGF165-PE38-EGFP fusion gene could express in the HEK293 cells. Conclusion The result provide the basis for search of the targeted cytotoxic activity to tumor vascular endothelial cells and may have some potential value in clinical application.

OBJECTIVE@#To investigation the clinic long-term result and explore the prognostic factor of patients with laryngectomy.@*METHOD@#Three hundred and fifteen patients with laryngectomy were analysed. The survival rate and the cause of death were collected from this study.@*RESULT@#Five years later, 233 cases were still alive, 60 cases were dead, 22 cases failed to be followed-up. Overall 5-year survival rate was 73.97%, 5-year survival rate for patients of early stage was 82.69. Whereas, for patients of late stage was 62.64%. Five year survival rate for patients of supraglottic carcinoma, glottic carcinoma, subglottic carcinoma and transglottic carcinoma was respectively 73.76%, 82. 55%, 55.56%, 68.75%. Five year survival rate for patients with partial laryngectomy was 79.89%, whereas, for total laryngectomy was 1.03%. The cause of death were local recurrence and cervical glands metastasis.@*CONCLUSION@#Early diagnosis was the key points to both larynx preservation and survival rate. for improving survival rate, we should handle the indications strictly. remain sufficient security cutting edge and follow-up visit.

OBJECTIVE: To investigate the effects of extract of Ginkgo biloba(Egb761) on the expression of matrix metalloproteinase-9(MMP-9) and tissue inhibitor of metalloproteinase-1(TIMP-1) and evaluate its intervention effect on airway remodeling in asthmatic rats model. METHODS: Rats were randomly assigned to normal control, asmatic group, Dexamethasone (DXM) group and Egb761 group. The asmatic model was established in rats. The changes of airway morphologic parameter and the relative content of MMP-9 and TIMP-1 in bronchial epithelium in these groups were determined using immunohistochemical technique and computer assisted image analysis system.
