ABSTRACT

Objective To analyze the clinical outcomes of early invasive fungal disease(IFD)in patients after allogenetic hematopoietic stem cell transplantation(allo-HCST)with metagenomic next-generation sequencing(mNGS).Methods A retrospective analysis was conducted on patients undergoing allo-HCST in our Bone Marrow Transplantation Center between July 2021 and October 2022.These patients experienced one of the following conditions within 100 d after transplantation:① Patients with persistent fever and negative blood culture after empiric antimicrobial therapy for 72 h or longer;② Hyperpyrexia of unknown origin occurred again after effective anti-infection in the past;③ Symptoms in lower respiratory tract associated with lung lesions on CT scan,and empiric anti-infective therapy was ineffective.Peripheral blood or bronchoscopic alveolar lavage fluid were tested with mNGS,and overall survival(OS)and non-relapse mortality(NRM)were analyzed.Results There were 60 patients enrolled in this study.For the peripheral blood samples of 47 cases and bronchoalveolar lavage fluid samples of 13 cases,mNGS found that 19 cases were negative to pathogens,30 cases were non-fungal positive,and 11 case were fungal positive,including 3 cases of aspergillus,5 cases of mucor,2 cases of Candida tropicalis,and 1 case of Trichosporon asahii.Of the 11 patients with fungal positive,8 achieved complete remission after antifungal therapy according to the mNGS results.The 1-year OS and NRM of the 60 patients were 70.0%(95%CI:64.1%~75.9%)and 20.0%(95%CI:11.9%~32.5%),respectively,while those of the fungal infection patients were 54.5%(95%CI:49.5%~69.5%)and 36.4%(95% CI:15.5%~70.3%),respectively.No significant differences were seen in 1-year OS(P=0.487)and 1-year NRM(P=0.358)among the negative,fungal infection and non-fungal infection patients,neither OS(P=0.238)and NRM(P=0.154)between the fungal infection and the non-fungal infection patients.Conclusion mNGS can rapidly diagnose the early IFD after allo-HSCT,which is helpful for timely and effective treatment and improves the prognosis of patients.

ABSTRACT

Objective@# To analyze the influencing factors for bladder cancer in Taizhou,and to provide evidence for strengthening the prevention and treatment of bladder cancer in Taizhou.@*Methods @#A total of 500 cases of bladder cancer diagnosed in Taizhou Hospital from 2012 to 2017 were selected as a case group,and 504 patients without tumor or urinary system diseases during the same period were selected as a control group. A structured questionnaire was used to retrospectively investigate the demographic information,occupational exposure(whether they were exposed to aromatic amine,polycyclic aromatic hydrocarbons,tobacco,tobacco smoke or heavy metals at work),healthy behaviors and diets of the two groups one year before admission. A Logistic regression model was used to analyze the influencing factors for bladder cancer.@*Results @#There was no significant differences in sex,age,ethnicity,education and marital status between the case group and the control group(P > 0.05). Patients with occupational exposure history accounting for 31.60% in the case group and 24.60% in the control group,for overweight/obesity were 37.60% and 31.74%,for smoking were 55.80% and 46.23%,for high vegetable intake frequency were 43.80% and 52.58%,for high fruit intake frequency were 55.40% and 62.70%,for physical activity were 24.60% and 31.75%,respectively. The results of multivariate logistic regression analysis showed that occupational exposure(OR=1.861,95%CI:1.229-2.836),overweight/obesity(OR=1.374,95%CI:1.021-1.863),current smoking(OR=1.664,95%CI:1.101-2.503)and previous smoking(OR=1.454,95%CI:1.016-2.066)were the risk factors for bladder cancer. High vegetable intake frequency(OR=0.731,95%CI:0.566-0.947),high fruit intake frequency(OR=0.659,95%CI:0.463-0.927)and vigorous physical activity(OR=0.566,95%CI:0.403-0.798)were the protective factors for bladder cancer.@*Conclusion @#Occupational exposure,overweight/obesity,current smoking,previous smoking were the risk factors for bladder cancer. High vegetables intake frequency,high fruit intake frequency and vigorous physical activity were the protective factors for bladder cancer.

ABSTRACT

Objective To explore the writing quality and rational use of drugs for outpatient prescription of western medicine in Yanqing District Hospital of Beijing.Methods From January 2011 to December 2015, 12 000 prescriptions of western medicine were selected randomly.The prescriptions were analyzed by Excel and the Pareto diagram analysis was used to find out the main reasons of irrational prescription.Results From 2011 to 2015, the basic indicators of prescription of western medicine were reasonable:the average number of drugs on each pre-scription was 2.31,the utilization rate of injections was 15.91%,the utilization rate of essential drugs was 38.00%, the utilization rate of antibacterial was 14.56%.The unreasonable rate of prescription was 6.32%,and the main irra-tional reason were dosage inappropriate,indications inappropriate,inappropriate selection of drugs and combination therapy was not suitable.Conclusion By prescription review,we have listed the problems,and provide the basis for rational drug use and improvement measures.

ABSTRACT

Transplantation of mesenchymal stem cells [MSCs] can promote functional recovery of the brain after hypoxic-ischemic brain damage [HIBD]. However, the mechanism regulating MSC migration to a hypoxic-ischemic lesion is poorly understood. Interaction between stromal cell-derived factor-1 alpha [SDF-1 alpha] and its cognate receptor CXC chemokine receptor 4 [CXCR4] is crucial for homing and migration of multiple stem cell types. In this study, we investigate the potential role of SDF-1 alpha /CXCR4 axis in mediating MSC migration in an HIBD model. In this experimental study, we first established the animal model of HIBD using the neonatal rat. Bone marrow MSCs were cultured and labeled with 5-bromo-21-deoxyuridine [BrdU] after which 6×10[6] cells were intravenously injected into the rat. BrdU positive MSCs in the hippocampus were detected by immunohistochemical analyses. The expression of hypoxia-inducible factor-1 alpha [HIF-1 alpha] and SDF-1 alpha in the hippocampus of hypoxic-ischemic rats was detected by Western blotting. To investigate the role of hypoxia and SDF-1 alpha on migration of MSCs in vitro, MSCs isolated from normal rats were cultured in a hypoxic environment [PO[2]=1%]. Migration of MSCs was detected by the transwell assay. The expression of CXCR4 was tested using Western blotting and flow cytometry. BrdU-labeled MSCs were found in the rat brain, which suggested that transplanted MSCs migrated to the site of the hypoxic-ischemic brain tissue. HIF-1 alpha and SDF- 1 alpha significantly increased in the hippocampal formations of HIBD rats in a time-dependent manner. They peaked on day 7 and were stably expressed until day 21. Migration of MSCs in vitro was promoted by SDF-1 alpha under hypoxia and inhibited by the CXCR4 inhibitor AMD3100. The expression of CXCR4 on MSCs was elevated by hypoxia stimulation as well as microdosage treatment of SDF-1 alpha . This observation illustrates that SDF-1 alpha /CXCR4 axis mediate the migration of MSCs to a hypoxic-ischemic brain lesion in a rat model

ABSTRACT

Objective To explore the role of HIF-1 and its downstream SDF-1α/CXCR4 and VEGF/VEGFR pathway in mediating MSC mobilization with DMOG .Methods Male SD rats were randomly divided into five groups:Normal saline control group , DMOG group, YC-1 group, AMD3100 group, SU5416 group.We used CFU-F assay and flow cytometry to determine the number of MSCs in rat bone marrow ( BM ) and peripheral blood ( PB ) in each group , respectively.The concentrations of SDF-1αand VEGF both in BM and PB serum in each group were detected by ELISA . Western blotting was used to test protein levels of HIF-1α, SDF-1αand VEGF in BM.Results Compared with NS group, the number of CFU-Fs as well as the percentage of CD 45 -CD90 +cells increased in DMOG group ( P <0.05);Compared with DMOG group, the number of CFU-Fs as well as the percentage of CD 45 -CD90 +cells decreased in YC-1 group, AMD3100 group and SU5416 group (P <0.05).Compared with DMOG group, the concentration and protein expression of HIF-1αdecreased significantly in YC-1 group ( P <0.05 ) , the concentration and protein expression of SDF-1αdecreased significantly in AMD 3100 group ( P <0.05 ) , the concentration and protein expression of VEGF decreased significantly in SU5416 group ( P <0.05 ).Conclusion DMOG can induce MSCs mobilization possibly via up-regulating the expression of HIF-1αand activating its downstream SDF-1α/CXCR4 and VEGF/VEGFR pathway .

ABSTRACT

Objective To analyze the clinical value of 24 h dynamic electrocardiogram (Holter) for cardiogenic syncope caused by arrhythmia and myocardial ischemia.Methods A total of 134 patients from February 2010 to October 2012 diagnosed syncope who received Holter and coronary arteriography (CAG) or electrophysiological examination were enrolled in this study.The daily record and the time of syncope were recorded exactly.Results Seventy-eight patients had final diagnosis by CAG and electrophysiological examination,and 59 patients were diagnosed by Holter.The true positive was 42 cases,the true negative was 39 cases,the false positive was 17 cases and the false negative was 36 cases.Thus,the sensitivity,specificity,positive predictive value and negative predictive value were 53.85% (42/78),69.64% (39/56),71.19% (42/59) and 52.00%(39/75).Conclusions Holter is a very important diagnostic method for cardiogenic syncope caused by arrhythmia and myocardial ischemia.Repeated and multiple examination can improve the diagnostic yield.

ABSTRACT

Objective To study the effects ofgranulocyte colony-stimulating factor(G-CSF) and AMD3100 on the proliferation,migration and adhesion of mesenchymal stem cells (MSCs).Methods The proliferation,migration and adhesion of MSCs were detected by MTT chromometry,transwell and adhesion test.Results When the concentration of G-CSF was 200 μg/L and AMD3100 was 0.5 mg/L,the proliferation and migration of MSCs were the strongest.When the concentration of G-CSF was 200 μg/L and AMD3100 was 0 mg/L,the adhesion of MSCs was the strongest.Conclusion The proliferation,migration,adhesion of MSCs are promoted by G-CSF,and inhibited by AMD3100.

ABSTRACT

BACKGROUND: Diabetic cardiomyopathy is closely related to the high incident rate of diabetic heart failure. It is extremely significant to set up the successful dietetic cardiomyopathy model for the study on improving cardiac function.OBJECTIVE: To understand the influence of high blood sugar on myocardium by setting up the model of type 2 diabetes and diabetic cardiomyopathy.DESIGN: Completely randomized design, randomized and controlled study,in which, the experimental animals were taken as the objects.SETTING: Department of endocrinology of a university.METHODS: In the experiment, 99 newly born Wistar male rats were employed, randomized into experiment group (75 rats) and the control group(24 rats). In the control group, abdominal injection was done with sodium citrate buffer and the single abdominal injection was done with strptozocin(STZ)90 mg/kg in the experiment group. Ten weeks later, the glucose tolerance was assayed to determine the set-up of diabetes model. The ultrastructure of myocardium of rats was tested under electronic microscope after being sacrificed.MAIN OUTCOME MEASURES: To observer whether the myofibrillae and mitochondria of myocardiocytes alter or not in diabetic rats.RESULTS: In the experiment group, the fasting blood sugar[(8.87± 0.76)mmol/L] was slightly higher than that[ (5.88 ± 0. 45)mmol/L] in the control group, the blood sugar 2 hours after meal was remarkably higher than that in the control group and the glucose tolerance was abnormal. Under the electronic microscope, the myofibrillae presented focal degeneration and disarranged order of myofilament; mitochondria of myocardiocytes presented swelling and focal proliferation.CONCLUSION: This model is the satisfactory carrier for the study on diabetes Ⅱ and its myocardiopathy, which lays an experimental foundation for the study on cardiac function of type 2 diabetes.

ABSTRACT

20?10 9/L) were 12 (8-20), 17(10-28) and 21 days (7-78), respectively. The average follow-up period was 7 3 years(ranged from 1 month to 14 years). The overall survival (OS) and death rates after transplantation were 70%(18/26) and 30%(8/26) respectively. Transplant-related mortality was 7 7%(2/26). Conclusion The combination of chemotherapy and G-CSF is an effective and low toxicity protocol for the mobilization of peripheral blood stem cells. APBSCT can be safely performed in children with malignant tumors.
