ABSTRACT

Objective:To investigate the signs and diagnostic value of transabdominal ultrasound of pregnant women with placenta accreta.Methods:The clinical data of 140 pregnant women with placenta previa who received regular prenatal examination between August 2015 and August 2019 in Lishui City People's Hospital were retrospectively analyzed. Clinical and pathological results were used as the gold standard for clinical diagnosis of placenta accreta. The sensitivity, specificity and accuracy of transabdominal ultrasound in the diagnosis of placenta accreta were evaluated. The included 140 women were divided into observation (75 women with placenta previa complicated by placenta accreta) and control (65 women with placenta previa not complicated by placenta accreta) groups. The difference in ultrasonic signs was compared between the two groups.Results:The sensitivity, specificity and accuracy of transabdominal ultrasound in the diagnosis of placenta accreta were 93.33% (70/75), 93.84% (61/65) and 93.57% (131/140), respectively. The incidences of placental thickening, disappearance of retroplacental space, uneven echo in placenta, abundant blood vessels at the interface between uterine serosa and bladder, and focal or extensive intraplacental blood flow in the observation group were 92.00% (69/75), 52.00% (39/75), 82.66% (62/75), 60.00% (45/75) and 50.66% (38/75), respectively, which were significantly higher than those in the control group [32.30% (21/65), 29.23% (19/65), 30.76% (20/65), 35.38% (23/65), 27.69% (18/65), χ2 = 54.041, 7.439, 38.650, 8.446 and 7.658, all P < 0.05]. The score of ultrasonic sign in the observation group was significantly higher than that in the control group ( Z = 10.656, P < 0.05). Conclusion:Transabdominal ultrasound examination can provide information to help screen placenta accreta in patients with placenta previa. Transabdominal ultrasound is of great significance for clinical diagnosis of placenta accreta.

ABSTRACT

Objective:To enhance the comprehensive learning and clinical practice capabilities of intravenous therapy specialized nurses based on the "Internet+" and standardized patient (SP) multidisciplinary linkage teaching.Methods:In this study, 54 nurses who took training of the intravenous therapy specialists in Chaoyang Hospital affiliated to Capital Medical University from July 2019 to December 2019 were selected as research subjects and randomly divided into a control group and a study group, with 27 ones in each group. The control group used traditional training of intravenous therapy specialized nurse, and the research group used the multidisciplinary linkage model of "Internet+" and SP. After the training, the nurses were assessed for theoretical and practical operation and satisfaction surveys, and the incidence of venous catheterization complications during the training of the two groups of nurses was counted. SPSS 22.0 was used for t test and chi-square test. Results:There was no statistically significant difference between the two groups of nurses in the theoretical assessment of basic knowledge and professional knowledge ( P>0.05). The total score of case analysis and theoretical assessment of the study group was higher than that of the control group, and the difference was statistically significant ( P<0.001). The nurses of the study group scored higher than the control group did in such 8 aspects, namely, central venous catheter (CVC) maintenance, peripherally inserted central catheter (PICC) placement, PICC maintenance, infusion port maintenance, peripheral venous catheters, aseptic technique, prevention and treatment of catheterization complications, and health education, with statistically significant differences ( P<0.05). There were statistically significant differences in the average score of nurse satisfaction between the two groups [(85.4±3.7) vs. (95.7±2.6)], and in the incidence of complications between the two groups ( χ2=4.618, P=0.032). Conclusion:"Internet+" and SP multidisciplinary linkage teaching helps to enhance the comprehensive learning and clinical practice ability of intravenous therapy specialized nurses, reduce the incidence of complications, enable patients to receive high-quality intravenous infusion therapy and care, and ensure the safety of patients.

ABSTRACT

Objective:To investigate the expression of estrogen receptor alpha (ERα) protein and BRAF V600E protein in thyroid papillary carcinoma (PTC) and their relationship with clinical factors of PTC. Methods:The expression of ERα and BRAF V600E protein in 1 105 PTC patients was detected by immunohistochemistry. The relationship among ERα, BRAF V600E protein and clinical factors were analyzed. Results:Positive ERα protein was correlated with maleness (χ 2= 6.087, P=0.001), age< 45 years old (χ 2=5.197, P=0.023) and multifocal tumors (χ 2=4.446, P=0.035). Positive BRAF V600E protein was correlated with positive ERα protein (χ 2=6.209, P=0.013), Hashimoto thyroiditis (χ 2=29.388, P<0.001), no lateral lymph node metastasis (χ 2=6.849, P=0.009) and multifocal tumors (χ 2=9.596, P=0.035). Conclusions:ERα expression is more common in male patients, patients younger than 45 years of age, those with multifocal tumors and positive BRAF V600E protein. BRAF V600E protein may inhibit Hashimoto's thyroiditis, tumor growth and the occurrence of lateral lymph node metastasis, and promote the occurrence of multiple focal tumors.

ABSTRACT

Objective:To extract the common factors for evaluating the teaching effect of the integrated curriculum of basic medicine in China, and to analyze and explain the practical significance of these common factors.Methods:A questionnaire survey was conducted among undergraduates of 15 medical colleges in China to evaluate the teaching effect of integrated curriculum of basic medicine, and exploratory factor analysis was conducted for screening and classification. SPSS 22.0 was used to conduct factor analysis.Results:The four major common factors evaluating the teaching effect of integrated curriculum were the effectiveness of teaching process, the rationality of teaching organization, the education of teaching content, and the innovation of teaching methods.Conclusion:The integrated curriculum reform of basic medicine should pay great attention to the simultaneous development of basic theory learning and practical ability training, the effective connection between theoretical teaching and experimental teaching, the educational role in comprehensive quality cultivation of students, and the flexible application of various teaching means and methods.

@#Objective    To compare and analyze the treatment effect of thoracoscopic surgery and traditional open surgery on infants with congenital diaphragmatic eventration, and summarize the experience of thoracoscopic surgery in infants with congenital diaphragmatic eventration. Methods    We retrospectively analyzed the clinical data of 105 children with congenital diaphragmatic eventration who received operation in the Department of Cardiothoracic Surgery of Children’s Hospital of Chongqing Medical University from January 2010 to January 2019. The patients were divided into an open group and a thoracoscopic group according to the operation methods. There were 41 patients in the thoracoscopic group, including 30 males and 11 females, with an average of 13.42±11.08 months (range: 1 d to 3 years). There were 64 patients in the open group, including 44 males and 20 females, with an average age of 8.21±9.33 months (range: 15.0 d to 1.6 years). The operation time, intraoperative bleeding volume, postoperative mechanical ventilation time, hospital stay and other operation indexes as well as the mortality, recurrence rate and complication rate of the two groups were observed. Results    The operation indexes such as operation time, intraoperative bleeding volume, postoperative mechanical ventilation time, thoracic drainage time, CCU stay and hospital stay of the thoracoscopic group were better than those in the open group (P<0.05). There was no statistical difference between two groups in postoperative diaphgram muscles descent, postoperative feeding time or patients needing thoracic drainage (P>0.05). The incidence of postoperative complications in the thoracoscopic group (19.51%) was lower than that in the open group (23.44%, P>0.05), and the difference in mortality and recurrence rate between the two groups was not statistically significant (P>0.05). Conclusion    Both thoracoscopic diaphragmatic plication and traditional open surgery can effectively treat congenital diaphragmatic eventration, but compared with traditional open surgery, thoracoscopic diaphragmatic plication has the advantages of shorter operation time, less trauma, more rapid recovery and fewer complications, so it should be the first choice for children with congenital diaphragmatic eventration.

ABSTRACT

Berberis amurensis (Berberidaceae) is a traditional Chinese medicine, which is often used to treat hypertension, inflammation, dysentery and enteritis. It contains alkaloids, mainly including berberine, berbamine, magnoflorine, jatrorrhizine and palmatine. Berberis amurensis extracts (BAEs) is often orally taken. Oral herbs might be metabolized by intestinal bacteria in the small intestine. However, the interaction between the herb and the gut microbiota is still unknown. In the current study, UPLC/Q-TOF-MS/MS combined with Metabolitepilot and Peakview software was used to identify the metabolites of BAEs in anti-biotic cocktail induced pseudo germ-free rats and normal rats. As a result, a total of 46 metabolites in normal rats were detected and its main metabolic pathways include demethylation, dehydrogenation, methylation, hydroxylation, sulfation and glucuronidation. Only 29 metabolites existed in pseudo germ-free rats. Dehydrogenated metabolites (M29, M30, M34 and M36), methylated metabolites (M33, M41 and M46) and other metabolites were not detected in pseudo germ-free rats. The result implied that the intestinal bacteria have an influence on the metabolism of BAEs. Furthermore, this investigation might contribute to the understanding of the metabolism of BAEs, and further promote its clinical application.

BACKGROUND: Photocrosslinked alginate hydrogel has been a popular bone tissue engineering material because of its excellent biocompatibility and minimally invasive injection, but there are still problems such as insufficient strength and poor cell adhesion. OBJECTIVE: To construct the negatively charged hydrogels by introducing sodium methacrylate into photocrosslinked alginate hydrogels, and to explore the changes in its physical performance and cell adhesion. METHODS: After preparation of methacrylated alginate by reacting sodium alginate with 2-aminoethyl methacrylate, methacrylated alginate, photoinitiator and sodium methacrylate (0, 20, 40, 60 mmol/L) were homogeneously mixed. The negatively charged photocrosslinked alginate hydrogels were prepared under ultraviolet light. The functional groups of the hydrogels were analyzed by fourier transform infrared spectroscopy. The surface morphology of the hydrogels was observed by scanning electron microscopy and the swelling ratio was measured. MC3T3-E1 cells were cultured with each group of hydrogels for 48 hours, and the cytotoxicity of the hydrogels was investigated by cell counting kit-8 assay. MC3T3-E1 cells were seeded on the surface of each group of hydrogels. The early adhesion of the cells was observed by live/dead staining at the 4th hour, and cell spreading was observed on the 3rd day. RESULTS AND CONCLUSION: (1) Fourier transform infrared spectroscopy demonstrated that the introduction of sodium methacrylate could lead to a new peak at wavenumber of about 1 600 cm-1 in the hydrogel infrared wave, which was from the sodium methacrylate. (2) Scanning electron microscope observed that the density of the negatively charged photocrosslinked alginate hydrogels increased and the pore size of the gels decreased with augment of concentrations of sodium methacrylate. (3) The swelling ratio of the hydrogel decreased with the increase of the concentration of sodium methacrylate. (4) The live/dead staining revealed that the cells grew well on the surface of each hydrogel, and the cell viability reached above 95%. The cell counting kit-8 assay results showed that the negatively charged photocrosslinked alginate hydrogels had no cytotoxicity. (5) The early cell adhesion rate increased gradually and the cell extension became better with the increase of concentration of sodium methacrylate. (6) In summary, the introduction of sodium methacryl into photocrosslinked alginate hydrogels can adjust its physical properties and significantly improve its cell adhesion.

ABSTRACT

Suitable animal models with high throughput are required to investigate the integrating and regulating effects of active constituents of traditional Chinese medicine(TCM). The zebrafish model has become an ideal research model for the diseases of various human systems due to its unique features, such as small body size, strong reproductive capacity, in vitro development, and high homology with human beings, and has been increasingly studied and applied in the field of TCM pharmacological effects. Here we reviewed recent published papers of zebrafish-based multimodal studies, focusing on the pharmacological effects of TCM. The applications of zebrafish model in studies of human diseases were systematically summarized, including angiogenesis, inflammation, neurobehavior changes, heart injury, osteoporosis, hyperglycemia, hyperlipidemia, and tumors. The advantages and major findings of zebrafish models in multimodal TCM studies were discussed. In addition, we anticipated the use of immunohistochemistry, fluorescent probes and transgenic fish lines to dynamically observe and quantitatively analyze the endogenous changes of target organs or cells in the zebrafish model, so as to realize the multimodal identification of active compositions of TCM, and further explore the mechanism of active compositions of TCM by considering the detected expression levels of biomarkers.

ABSTRACT

Currently, the epidemic of 2019 coronavirus disease (COVID-19) is still ongoing. The characteristics including high contagiousness, herd susceptibility and clinical phenotype diversity, made a serious influence on people’s daily life and rountine therapy for other diseases. Breast dieases are clinical common diseases. In the central epidemic area of COVID-19, Hubei province, especially Wuhan, the clinical specialists of breast diseases should consider all of the following factors comprehensively: the prevention of COVID-19, the diagnosis and treatment of breast diseases and the accessibility of medical resources. Besides, we should select the appropriate therapy and optimize treatment process so as to prevent the propagation and cross infection of COVID-19 as well as manage the breast diseases without delay. Therefore, we carried out some management proposals of the patients with breast diseases in the central epidemic area during the epidemic of COVID-19 on the basis of conventional treatment guidelines and clinical experiences. The suggestions and corrections from colleagues will be welcomed.

Objective@#To screen the chronic kidney disease (CKD) patients among the high-risk groups in Jing'an district of Shanghai, and provide suggestions for the screening and analysis of CKD.@*Methods@#Retrospective analysis was used to analyze the disease status of high-risk groups of CKD who participated in community screening from July 2016 to November 2018. A total of 25 199 subjects underwent two laboratory examinations at intervals of more than 3 months. The CKD was diagnosed in high-risk groups according to the diagnostic criteria, and the patients with CKD were classified and stratified. The screening population was divided into groups according to gender, age and medical history to compare the difference in the detection rate of CKD.@*Results@#There were 788 CKD patients diagnosed previously in this screening population, and 3 713 CKD patients were confirmed by this district-level hospitals screening. Potential CKD patients were 4.71 times as many as previously known CKD patients. The CKD detection rate was 14.73%. The CKD detection rate of female high-risk group was higher than that of male (16.00% vs 13.00%, χ2=44.213, P<0.001). The CKD detection rate in the elderly group (≥65 years old) was higher than that in the non-elderly group (14.94% vs 13.76%, χ2=4.001, P=0.046). The CKD detection rate in high-risk group with hypertension, hyperuricemia and family history of chronic nephritis was significantly higher than those in the group without such diseases (all P<0.05).@*Conclusions@#The number of patients detected in high-risk groups of CKD is 4.71 times as much as previously known patients, indicating that it is very necessary to carry out CKD screening in community high-risk group. Women, elder, hypertension, hyperuricemia, and a family history of chronic nephritis are independent risk factors for patients at high risk of CKD.

Terminalia chebula is a traditional Chinese herbal medicine, which distributed in Yunnan, Guangdong, Guangxi, Tibet and etc. T. chebula is widely used in the clinical medicine of Chinese medicine and it plays a significant role in the Mongolian medicine and the Tibetan medicine. The chemical composition of T. chebula is rich and diverse, including phenolic acids, tannins, triterpenoids, aliphatics, flavonoids, volatile oils, amino acids, trace elements, carbohydrates and so on. Modern pharmacological studies have shown that T. chebula extract has many pharmacological activities, such as anti-oxidation, anti-cancer, anti-tumor, detoxification, antibacterial, strong heart, anti-inflammation, immunomodulation, anti-microbial, and promoting bronchial smooth muscle contraction. From the aspects of textual research, chemical composition characteristics, pharmacological action and so on, this paper expounds the research progress of T. chebula. According to the core concept of Q-marker, we predicted and analyzed the quality markers of T. chebula from the aspects of chemical composition characteristics, traditional efficacy, medicinal properties, pharmacokinetics, new clinical use and measurable composition. It provides reference for the quality evaluation of T. chebula.

Objective: To explore the combination rule of Corydalis Rhizoma-Toosendan Fructus drug pair through the research in "Pharmaceutical Standard of the Ministry of Health of the People's Republic of China-Traditional Chinese Medicine Formulation", so as to provide theoretical basis for clinical application and new product development of this pair. Methods: The prescriptions containing Corydalis Rhizoma-Toosendan Fructus were collected from Pharmaceutical Standard of Ministry of Public Health of the People's Republic of China-TCM Formulation and input into TCM inheritance support platform software (V2.5) to establish the database. The frequency of attending diseases, compatibility medicinal materials, and meridian of drug were analyzed statistically. The core combination of medicinal materials in the prescriptions containing Corydalis Rhizoma-Toosendan Fructus were analyzed statistically with association rule Apriori algorithm (support degrees were 10%, 20%, 30%, confidence was 0.90). Meridian of drug with higher compatibility frequency were selected and analyzed in respect of prescription rules. Results: A total of 51 prescriptions containing Corydalis Rhizoma-Toosendan Fructus were screened, eight kinds of attending diseases (frequency≥2), such as stomachache, hypochondriac pain, leukorrhagia, dysmenorrhea, pelvic inflammation, menstrual disorder, etc. Nine kinds of attending syndrome (frequency≥2) were included such as liver-stomach disharmony, syndrome of qi stagnation and blood stasis, syndrome of liver depression and qi stagnation, syndrome of liver depression and blood deficiency, syndrome of blood-heat in uterus, etc. There were 17 commonly used compatibility medicinal materials (frequency≥5), including Paeoniae Radix Alba, Aucklandiae Radix, Glycyrrhizae Radix et Rhizoma, Cyperi Rhizoma etc. There were 20 commonly used medicinal materials combinations (frequency≥6), including Corydalis Rhizoma-Paeoniae Radix Alba-Toosendan Fructus, Corydalis Rhizoma-Aucklandiae Radix-Toosendan Fructus, Corydalis Rhizoma-Glycyrrhizae Radix et Rhizoma-Toosendan Fructus, Corydalis Rhizoma-Cyperi Rhizoma-Toosendan Fructus. Meridian of drug with higher compatibility frequency was distributed to liver, spleen and stomach channel tropism. There were five kinds of core medicinal materials commonly acted on the liver meridian with Corydalis Rhizoma-Toosendan Fructus, including Aucklandiae Radix, Paeoniae Radix Alba, Citri Reticulatae Pericarpium, Amomi Fructus, and Aquilariae Lignum Resinatum. There were commonly used core medicinal materials combinations (frequency≥7), including Corydalis Rhizoma-Aucklandiae Radix-Toosendan Fructus, Corydalis Rhizoma-Paeoniae Radix Alba-Toosendan Fructus, Corydalis Rhizoma-Citri Reticulatae Pericarpium-Toosendan Fructus, Corydalis Rhizoma-Citri Reticulatae Pericarpium-Paeoniae Radix Alba-Aucklandiae Radix-Toosendan Fructus. There were three kinds of core medicinal materials commonly acted on the spleen meridian and the stomach meridian with Corydalis Rhizoma-Toosendan Fructus, including Aquilariae Lignum Resinatum, Aucklandiae Radix and Amomi Fructus. There were commonly used core medicinal materials combinations (frequency≥4), including Corydalis Rhizoma-Aquilariae Lignum Resinatum-Toosendan Fructus, Corydalis Rhizoma-Aucklandiae Radix-Toosendan Fructus, Corydalis Rhizoma-Amomi Fructus-Toosendan Fructus. Conclusions: Herbal medicines used frequently in prescriptions containing Corydalis Rhizoma-Toosendan Fructus can regulate qi flowing for relieving pain. The medicine using is relatively concentrated, and the composition is clear. In addition, Prescription rules of three kinds of meridian prescriptions were chosen to make comparative analysis, and the compatibility features of the prescriptions containing Corydalis Rhizoma-Toosendan Fructus was reflected in different meridian of drug and the composing principles. This study also can provide theoretical basis for treating different diseases with same method and the better application of Corydalis Rhizoma-Toosendan Fructus in clinical practice.

ABSTRACT

Objective:To report the clinical, electrophysiological and genetic features in a Chinese family with distal hereditary motor neuropathy type V (dHMN-V) and screen the pathogenic mutant gene.Methods:A family with the history of inherited peripheral neuropathy was recruited in the First Affiliated Hospital of Zhengzhou University in July 2017. The clinical features and electrophysiological data were investigated. Genetic testing on well-established genes associated with hereditary peripheral neuropathy was conducted by targeted high throughput sequencing and the candidate variant was screened in the family and normal controls.Results:There were four affected individuals in the family. The proband, a 25-year-old male, was characterized by weakness and atrophy in the distal extremities primarily affected the upper extremities without sensory impairment. Electrophysiological study showed chronic neurogenic pattern in the upper and lower limb muscles. The motor conduction showed reduced velocity and compound muscle action potential amplitude, while the sensory conduction studies results were normal. The grandfather, a maternal uncle and a cousin of the proband exhibited similar clinical manifestations and electrophysiological abnormality. Genetic testing revealed a heterozygous mutation, c.880G>A(p.G294R), in the GARS gene in the proband. Proband′s mother and two other affected individuals carried the mutation which was confirmed by Sanger sequencing. The mutation site was not found in the unaffected members from the family and 300 unrelated normal controls. The variant is a novel mutation which has not been reported in dbSNP, ExAC and 1000 Genomes Project databases. Conclusion:The results suggest that the novel c.880G>A(p.G294R) mutation of the GARS gene is responsible for the Chinese patients with dHMN-V, and the findings broaden the mutational spectrum of GARS gene.

ABSTRACT

Objective@#To investigate the clinicopathologic features, immunophenotype and prognosis of CIC-rearranged sarcoma (CRS).@*Methods@#The clinical and pathological data of 10 cases of CRS diagnosed between January 2017 and December 2018 at the Department of Pathology,Fudan University Shanghai Cancer Center were analyzed. Immunohistochemical study and fluorescence in situ hybridization (FISH) were performed. The literature was reviewed.@*Results@#There were five males and five females with a mean age of 28 years (range, 5 to 63 years). Eight tumors developed in the somatic soft tissues, including trunk (n=3),head and neck (n=3),and extremities (n=2). One case each arose in the small intestine and the occipital lobe. The average size was 4.9 cm (range,1.5-8.0 cm). Microscopically,all cases were composed of small to medium-sized round, oval to short spindled cells, showing nodular or lobular architecture, or were arranged in sheets. Compared with Ewing sarcoma, tumor cells of CRS usually showed irregular nuclear outline, coarse chromatin with prominent nucleoli and brisk mitotic activity. Necrosis was present in four cases with one showing geographic necrosis. Immunohistochemically, tumor cells usually showed focal or patch staining of CD99 (9/10),diffuse and strong nuclear expression of WT1 in half cases (2/4),and a high Ki-67 index (median 70%). By FISH, nine cases demonstrated convincing break-apart signal of CIC gene. Follow-up data available in seven cases (mean 12.1 months); of these two patients died of disease (2/7), whereas one patient was alive with unresectable recurrent tumor,the remaining four patients were alive with no evidence of disease. Five patients (5/7) experienced local recurrence and two patients (2/7) developed metastasis. The mean and median intervals to recurrence/metastasis were eight months and four months, respectively.@*Conclusions@#CRS is the most common type of EWSR1-negative small round cell sarcoma (SRCS). Although it has clinical and pathological overlapping features with Ewing sarcoma,the prognosis is comparatively poor. Focal or patch staining of CD99 but diffuse staining of WT1 in a case of small RCS should raise the possibility of CRS. FISH assay is required for the final diagnosis.

ABSTRACT

Bilateral medial medullary infarction is a rare type of stroke. Hypertrophic olivary degeneration (HOD) is usually secondary to the lesion involving the Guillain-Mollaret triangle with vacuolar degeneration of inferior olivary nucleus neurons and enlargement of inferior olivary nucleus. The primary lesions involving the Guillain-Mollaret triangle are usually located in midbrain, pons and cerebellum. A case of unilateral HOD secondary to bilateral medial medullary infarction is reported and the clinical characteristics, diagnosis and treatment are analyzed in order to improve the understanding of HOD.
