ABSTRACT
Based on the correlation between Qi and blood in traditional Chinese medicine, the collateral disease theory puts forward that the Qi-collateral go hand in hand with the vessel-collateral of the brain, and to be as close as lips to teeth in structure and function, which is an important basis for the function of brain governing mind. And this theory proposes that deficiency/stagnancy of collateral-Qi, stagnation of collaterals and loss of consciousness are the main pathogenesis of Alzheimer's disease(AD), which is different from the research strategy of modern medicine focusing on neurons. It is suggested that it is necessary to treat AD from two aspects, including neuronal protection(elimination of pathological products such as β-amyloid and phosphorylated tau protein) and cerebral microvascular protection(protection of cerebral microvascular structure and function, promotion of therapeutic angiogenesis and increase of cerebral blood flow. Tongxinluo capsules is a representative drug for dredging collaterals developed under the guidance of the therapeutic principle of collaterals need circulation, it can protect microvessels and play a neuroprotective role mediated by vascular protection. Clinical studies have confirmed that Tongxinluo capsules can effectively treat AD, vascular dementia and cognitive impairment related diseases, which can provide new ideas and effective treatment ways to prevent and treat AD from neurovascular protection in a comprehensive manner.

Objective@#To explore the effect of proprioception combined with core stability training on the static balance ability and dynamic balance ability of college students, and to provide an experimental basis for studying body balance ability and preventing sports injuries.@*Methods@#In August 2019, 112 non sports students from Shandong Institute of Physical Education were recruited, and 52 subjects were selected as the intervention group and 60 as the control group by random number table method. The intervention group was given proprioceptive training combined with core stability training, while the control group was given core stability training. The static and dynamic balance abilities of the two groups of subjects before and after training were compared.@*Results@#The static balance ability of the intervention group was higher than that before the intervention ( F =2.17, P <0.05) at the 5th and 11th weeks, which were 18.31% and 47.37% higher, respectively. The ability to stand on one foot with eyes closed at the 11th week in the intervention group was higher than that in the control group at the 11th week( t =2.25, P <0.05). After training, the equilibrioception of the intervention group was improved, increasingly improved in the 11th week. And compared with that in intervention group in the 5th and 11th week, the ability was also higher than the balance ability of the intervention group before the intervention( F =2.37, P <0.01), 12.01% and 12.99% higher, respectively.@*Conclusion@#Proprioception and core stability training can effectively improve the static and dynamic balance ability of college students, and the training effect of proprioceptive training combined with core stability training is better than that of core stability training.
ABSTRACT
Objective:To explore the evaluation dimensions and indicators of research hospital, provide empirical evidence for the construction of research hospital.Methods:Non-probability sampling was adopted, physicians from 9 hospitals in Shanghai that have a National Clinical Medical Research Center or Shanghai Clinical Medical Research Center were invited as survey participants. The physician participants were divided into senior and junior groups. For senior group, a questionnaire including 5 dimensions and 21 indicators was provided. The survey participants were consulted to determine whether the indicators can be used to measure this dimension, and also invited to propose additional dimensions and indicators for improvement.For junior group, two open questions were proposed to explore their needs of support in both clinical service and research.Results:Based on suggestions from the research participants, the research team, and other expert consultant, this article tried to propose 5 dimensions and 21 indicators for evaluating research hospitals. Among them, 11 were indicators compiled by the research team based on the literature review and agreed by more than 2/3 of senior physicians surveyed, while others were proposed based on the survey results. The research team planned to use the entropy method to determine the weights of different indicators, thus, the participants were not required to make judgments on the weights of dimensions and indicators.Conclusions:The evaluation of research hospitals has not yet formed a systematic consensus. Through the exploration and establishment of evaluation dimensions and indicators, identify hospitals that are closer to the development goals of research hospitals can provide a basis for future next-step practices.
ABSTRACT
Objective:To investigate the effects of theta burst stimulation(TBS)mode repeated transcranial magnetic stimulation (r-TMS) in the left prefrontal on negative symptoms and cognitive function in the elderly chronic schizophrenic patients.Methods:Totally 48 patients with stable chronic senile schizophrenia (24 cases in r-TMS treatment group and 24 cases in r-TMS pseudo stimulation control group) were selected. The treatment group was given the TBS mode r-TMS performed in the left dorsolateral prefrontal cortex. The control group was given pseudo stimulation at the same site. Before and after treatment, the brief psychiatric rating scale(BPRS), scale for assessment of negative symptoms(SANS), and positive and negative syndrome scale(PANSS)were used to assess mental symptoms, while Mattis-dementia rating scale(MDRS-2)and social adaptation functioning evaluation(SAFE)were used to assess cognitive function and social function. SPSS 20.0 was used for statistical analysis.Comparisons of the differences between inter groups and intra groups were conducted by independent sample t test and paired t-test. Results:(1)There were significant differences in the total score of PANSS scale((60.17±3.73), (56.67±3.12)), the negative symptom subscale score of PANSS((20.88±2.94), (17.96±2.33)) and the score of SANS((30.67±1.66), (30.25±1.45)) before and after treatment in the treatment group (all P<0.05). The D-value before and after treatment in the scores of BPRS ( t=3.513, P=0.001), PANSS ( t=6.048, P<0.01), negative symptom subscale ( t=6.610, P<0.01) and SANS ( t=8.239, P<0.01) were significantly different between the two groups. (2)There were significant differences in the scores of MDRS-2 and its sub scales before and after treatment in the treatment group (all P<0.05). The D-value before and after treatment in the scores of MDRS-2 ( t=6.216, P<0.01), attention ( t=4.596, P<0.01), start/maintain ( t=6.424, P<0.01), concept formation ( t=3.974, P<0.01), construction( t=2.194, P=0.033) and memory ( t=3.162, P=0.003) were significantly different between the two groups.(3)There was no significant difference in the SAFE score between the treatment group and the control group before and after treatment ( t=0.138, 0.142, both P>0.05). Conclusion:TBS can improve the negative symptoms and cognitive function in patients with the elderly chronic schizophrenic, but the effect of social function is not clear.

OBJECTIVE:To know about the research status of health utility value obtained by mapping method in pharmacoeconomic evaluation ,and to provide reference for bibliometric study in pharmacoeconomic evaluation . METHODS : Using“Mapping method ”“Health utility value ”“Cost-utility”“Utility point system ”as Chinese and English keywords ,retrieved from CNKI ,Wanfang database ,PubMed,Medline,Ebsco,Ovid and Wiley database ,empirical journal documents published from the inception to Dec. 31st,2018 about using mapping method to obtain health utility value were collected. The bibliometrics was used to statistically analyze basic information of included literature ,the construction and test of the model ,the type of the best model and so on. RESULTS :The 124 included documents were all published in English journal. In the construction and testing of the mapping model ,the most frequently used econometric methods ,performance evaluation indicators and model testing methods were ordinary least squares (OLS),mean absolute error (MAE)and residual normality test ,application frequency of which were 97 times(31.60%),89 times(24.93%)and 62 times(21.09%). There are 117 articles that define the best mapping model ,of which 101 articles(86.32%)have the best direct mapping effect. Most of the non-utility measurement scales adopted specific scales (92 articles,77.97%),and a few literatures adopted the universal scale (26 articles,22.03%). The most utility measurement scales were 3-level European 5-dimensional health scale (79 articles,66.95%). CONCLUSIONS :The domestic empirical researches that use the mapping method to obtain health utility values need to be developed yet. A series of mapping models successfully developed by foreign scholars not only provide the feasibility of using non-utility measurement scales for cost-utility analysis ,but also provide more ideas for China to choose the corresponding econometric methods ,evaluation indicators and mapping methods in the empirical research of the mapping method in the future.
ABSTRACT
Muscle fatigue has widespread application in the field of rehabilitation medicine. The paper studies the muscle fatigue using surface electromyogram (sEMG) in the background of rehabilitation training system. The sEMG and ventilatory threshold of vastus lateralis, rectus femoris and erector spinae are collected synchronously and the electromyogram fatigue threshold (EMG ) of different sEMG was analyzed by increasing load cycling experiments of 10 healthy subjects. This paper also analyzes the effect of isotonic and isometric contraction on EMG . Results showed that the appeared time of EMG was earlier than that of ventilatory threshold in the incremental load cycling. While the differences were subtle and EMG was verified to be effective. EMG has been proven effective for different muscle contraction by comparing the EMG of vastus lateralis and erector spinae. EMG could be used to keep muscle injuries from overtraining in the process of rehabilitation. Therefore, EMG has a great significance for femoral shaft fractures's fatigue monitoring in rehabilitation training.

Objective As one of the important parts of medical scientific research outcomes,the significance of nursing patents is becoming more and more prominent during recent years.However,the nursing department is the blind spot of intellectual property in most hospitals.This article tried to explore strategies to eliminate harriers to the implementation of nursing intellectual property rights in order to promote nursing patents.Methods This paper conducted systematic analysis of the problems and obstacles in the implementation and transformation of nursing patents,and proposed tailored administrative plan.Results Identified obstacle factors include personal reasons of the nursing staff,heavy daily clinical work,as well as the immature patent system and patent market in China.Accordingly,our hospital adopted several corrective measurements,for instance,improving the support for the implementation of nursing patents,strengthening the cultivation and incubation of the intellectual property of nursing staff and standardizing the patent management.Conclusions These measures have effectively im proved the awareness and enthusiasm of nurses' patent knowledge.And we have obtained the ice breaking of the transformation of nursing patents in our hospital.

Objective To explore the appropriate scientific research award mechanism to promote the transformation of scientific and technological achievements.Methods Literature review was adopted to conduct comprehensive analysis of the incentive mechanisms of foreign patent transformation as well as related policies in China.Results Aimed to providing sufficient opportunities and motivation for the patent transformation platform,increasing the proportion of incentives for R&D personnel and setting up performance rewards for scientific research administration,a policy named Provisions on the proportion of rewards for R&D personnel after the transformation of intellectual property rights in Zhongshan Hospital was developed,which effectively facilitated the medical patents to market transformation.Conclusions It is alerted that improve the enthusiasm of relevant personnel for patent transformation plays important role in increasing the transformation of medical patents.

Objective The demand for medical intellectual property transformation has been increasing.However,the current patent transformation system in our country is not yet perfect.The specialization and complexity of patent value evaluation hampered the transformation.In this article,we argued and analyzed the method of value evaluation before the transformation of medical patent in order to promote the transformation of medical patent.Methods Through the analysis and comparison of classic patent valuation models,the advantages and disadvantages of the three types of cost method,market method and income method are summarized,also introduced the methods and experience of Zhongshan Hospital affiliated to Fudan University.Results The three methods including comprehensive cost accounting,market value valuation and negotiation between the two parties to carry out the pre-transformation value evaluation,can help to obtain the transaction valuation value of the medical patent that meets the current actual situation.Conclusions The scientific and operable value evaluation method of medical patents before transformation,organized approval process and clear revenue management are vital strategies for the transformation of medical patents.

Objective To explore the role of National Natural Science Foundation (NSFC) funding on improving medical research of China,based on the SCIE paper output of a teaching hospital in East China,in order to provide evidence-based decision-making basis for hospital management of scientific research and discipline construction.Methods SCIE output of a teaching hospital in East China during 2009-2015 was retrieved based on the web of science (WOS) platform.The papers funded by the NSFC were further analyzed.Results From 2009 to 2015,the output of SCIE of the hospital showed a steady upward trend,with an annual growth rate of 26.10%.Among them,the NSFC funded project output 835 papers SCIE,accounting for 46.62%oo of the total paper;and the absolute value of the output of the paper showed an upward trend year by year,an increase of 49.52%.The main subjects of the NSFC funded by the hospital focus on oncology,liver disease and digestive system diseases and cardiovascular and cardiovascular diseases,and the hospital's clinical status basically.The quality of the journals published by the NSFC SCIE project funded by the hospital was significantly improved.Conclusions NSFC funding plays an important role in improving the output and quality of SCIE papers in clinical medicine,which should be paid attention to in scientific research management and decision-making.
ABSTRACT
PURPOSE: The association of ischemic stroke and metabolic syndrome (MetSyn) with or without diabetes mellitus (DM) is not clear. The present study aimed to identify the impact of diabetes or hyperglycemia on the risk of MetSyn-associated ischemic stroke. MATERIALS AND METHODS: This study comprised an Asian population of 576 patients with acute nonembolic cerebral infarction and 500 controls. MetSyn was defined according to the criteria of the International Diabetes Federation. MetSyn patients were further subgrouped according to their glucose levels: MetSyn with DM, MetSyn with impaired fasting glucose (IFG) and MetSyn with normal glucose tolerance (NGT). The impact of MetSyn on cerebral infarction was then evaluated. RESULTS: At baseline, the prevalence of MetSyn in patients with cerebral infarction was higher than that of the controls (57.29% vs. 10.00%, p<0.01). In the stroke group, the prevalences of MetSyn with DM, IFG, and NGT were 25.69%, 8.85% and 22.74%, respectively, all of which were higher than that of the controls (all p-values <0.05). By multiple logistic regression analysis, we discovered that MetSyn was associated with an increased risk of cerebral infarction (odds ratio: 5.73, p<0.01). After adjustment for all the components of MetSyn, the odds ratios of MetSyn with DM, IFG, and NGT were 5.70, 2.24 and 2.19 (all p-values <0.05), respectively. CONCLUSION: In Asian population, patients with MetSyn accompanied by T2DM are at the greatest risk for acute non-embolic stroke. Additionally, IFG was not observed to be associated with an increased risk for MetSyn-related ischemic stroke.

Along with the development of modern biological technology, the adversity of the transgenic products has aroused increasing attention. The origin of biosafety issues, the safety of genetically modified organisms and its impact on the ecological environment and human health were introduced in this paper. Here, the safety issues of the leakage of laboratory recombinant gene fragments were discussed also, finally, the focuses on biosafety and preventive measures were reviewed.
