ABSTRACT

OBJECTIVE:To provide ideas for the development of electronic prescription (e-prescription)service mode under the background of “Internet+”. METHODS :Retrieving official websites and related online resources of the national and provincial governments,health committees ,medical products administration ,and medical security bureaus (up to Jun. 2020),relevant policies,supporting documents and implementation status of e-prescription service mode were sorted out and analyzed ,and the suggestions were put forward. RESULTS :Current e-prescription service modes included four modes ,such as pharmaceutical retail enterprise e-prescription service mode ,medical institution e-prescription information sharing service mode ,internet hospital e-prescription service mode ,and medical e-commerce e-prescription service mode. The pilot of e-prescription service mode of drug retail enterprises covered the widest range of provinces and cities ,but the participation of health departments and medical institutions was insufficient ,and the pattern of comprehensive power had not been formed. The medical institutions e-prescription information sharing service mode was mainly composed of three typical examples ,namely“Wuzhou mode ”,“Fuzhou mode ”and “Gansu mode ”. However ,it had not been unified in terms of technical standards ,access conditions ,exit mechanism and execution process,etc. At present ,this model was still in a small scope of exploration and pilot stage. The internet hospital e-prescription service mode was limited by the popularization of internet hospital medical insurance payment and the improvement of corresponding rules and regulations ,which was still at the stage of cautious pilot exploration. Medical e-commerce e-prescription service model was limited by the types of drugs within the legal service scope. Meanwhile ,the industry standards and regulations of e-prescription service were in the absence at present. CONCLUSIONS :As an innovative service mode ,the e-prescription service mode meets the real needs of the pharmaceutical retail industries ,medical institutions and patients. Each pilot mode conforms to the development trend of “Internet+”and shows a strong development momentum ,but overall it is still in the early stage of active exploration. It is urgent to further explore and research around the leading departments ,stakeholders and prescription transfer procedures.

ABSTRACT

OBJECTIVE:To evaluate the current situation and implementation effect of the pilot project of electronic prescription and remote pharmaceutical care in social pharmacy from the perspective of consumers ,and to provide reference for improving and optimizating the electronic prescription and remote pharmaceutical care in social pharmacy. METHODS :Totally 264 consumers in 6 counties and 36 drugstores of Chengdu were randomly selected for the questionnaire survey. Descriptive statistics were conducted by using SPSS 23.0 software. Compare the distribution and differences of various. The influential factors in consumer satisfaction based on χ2 test and ordinal multiple Logistic regression analysis. Consumers were randomly selected for semi-structured interviews. Text analysis was conducted by Nvivo 12.0 software to refine the problem of this service . RESULTS :A total of 271 questionnaires were distributed to consumers in various districts of Chengdu ,and 264 valid questionnaires were collected,with an effective recovery rate of 97.4%. Among 264 respondents,178(67.9%)knew about electronic prescription and remote pharmaceutical care ;197(74.6%)received electronic prescription and remote pharmaceutical care service in pharmacies ; 202(76.5%)said they needed this service ,and 63(23.9%)expressed that they would like to pay for it. Of the 197 respondents who had received the service ,163(82.2%)were satisfied or very satisfied with the service ,and only one (0.5%)was dissatisfied with the service. Waiting time ,satisfaction of drug demand ,staff attitude ,service platform configuration ,the qualifications of tele-practice pharmacists and service quality were the main influential factors of satisfaction ;while the service also suffered from difficulties in electronic prescription circulation ,inadequate service monitoring measures ,insufficient certification of physicians and pharmacists,cultural lag ,and irregular service processes. CONCLUSIONS :Electronic prescription and remote pharmaceutical care service in social pharmacy is inadequate in terms of technical level ,institutional design and social acceptance. It is recommended that relevant departments implement incentive policies ,optimize technology ;follow up supporting policies in time ,strengthen supervision;and increase social publicity and supervision ,so as to promote the continuous improvement and long-term development of electronic prescription and remote pharmaceutical care in social pharmacy.

ABSTRACT

Rickettsia felis is a Gram-negative prokaryotic organism that is obligatorically parasitic in cells. Cat fleas are the main vector of Rickettsia felis. Clinical symptoms of human infection with Rickettsia felis include fever, fatigue, headache, macular papules, and eschar. There are few reports of serious complications or deaths due to infection of Rickettsia felis. A confirmed case of severe encephalitis caused by Rickettsia felis infection was admitted to neurology department of Affiliated Hospital of Jining Medical University on January 29, 2020. After comprehensive treatment of antiviral by acyclovir, mannitol dehydration to reduce intracranial pressure, human immunoglobulin to regulateimmunity, minocycline hydrochloride capsule, levofloxacin mesylate and sodium chloride against Rickettsia felis infection, control of seizure and pulmonary infection, the patient's condition was improved and then discharged. By summarizing the experience and lessons in the treatment of this patient, we hope to remind everyone to strengthen the understanding of Rickettsia felis infection.

ABSTRACT

OBJECTIVE:To provide reference for chronic disease man agement service develophed in social pharmacy. METHODS:Questionnaire about the Status Quo of Chronic Disease Management and Service in Social Pharmacy of Chengdu was designed,using the quota sampling method ,social pharmacies in five main urban areas of Chengdu were selected from May to July,2018 to conduct a questionnaire survey (one questionnaire by each social pharmacy ) on the basic situation of social pharmacies,the development of chronic disease management services ,the cognition of chronic disease management services ,and the challenges faced by chronic disease management services ,and suggestions were proposed. RESULTS & CONCLUSIONS :A total of 272 questionnaires were sent out ,and 252 valid questionnaire were actually collected (effective recovery rate of 92.65%). Totally 189 sample pharmacies (75.00%) had carried out chronic disease management services ,of which 112 (59.26%) pharmacies had been launched for 1-3 years;87(46.03%)had set up service areas ;68(35.98%)had full-time staff ,and 54 (28.57%)had part-time staff ,most of which were licensed pharmactists. 116(61.37%)had trained related staff for 1-2 times per year. 176(93.12%)pharmacies could provide services such as basic indicator testing (176,93.12%),establishing health records (142,75.13.%),and rational medication guidance for patients (163,86.24%). According to the survey ,the substantial benefits of chronic disease management services included changing the health status of patients (163,86.24%),improving patients ’trust in the pharmacy and staff (141,74.60%),improving patients ’quality of life (129,68.25%),etc. More than 50% of pharmacies faced the challenges of limited number of licensed pharmacists (102,53.97%),difficulty in establishing professional teams (112, 59.26%),and lack of trust in services (101,53.44%). The current chronic disease management service of social pharmacy in Chengdu is in the initial stage of active exploration ,which can bring many benefits to patients and pharmacies ,and is conducive to the promotion of medical and health policies ,but there are some weak links at the same time. Objective@#To develop the environment scale of unintentional injury in the home for children aged 0-6 years living in urban area of China, and test its validity and reliability.@*Methods@#The content of the environment scale was established through the literature review, expert consultation and pilot study. A total of 1 104 children aged 0-6 years in urban area of Changsha were enrolled in this study by using a multi-stage stratified cluster random sampling method. The questionnaire was used to collect the basic information of children, the incidence of unintentional injury and the status of home environment. The reliability of the scale was tested by using Cronbach′s α coefficient and split-half reliability coefficient. The content validity and construct validity were tested by using Pearson correlation analysis and factor analysis. All children were divided into two groups according to the incidence of unintentional injury in the home and the discrimination validity of the scale was tested by using t-test.@*Results@#The scale had 54 items in 6 dimensions. The number of eligible questionnaires was 1 074, including 554 (51.6%) from boys and 519 (48.3%) from children under 3 years old. The incidence rate of unintentional injury and in-home injury was 18.34% (197 children) and 10.71% (115 children). The internal consistency reliability (Cronbach′s α) coefficient for the scale was 0.87 and the split-efficacy reliability coefficient was 0.82, both meeting the standard of reliability above 0.70. The Pearson correlation coefficient between each dimension and the whole scale ranged from 0.53 to 0.84 (all P values <0.001). The common factor cumulative variance contribution rate of the scale was 58.34%. There were 54 items with factor loadings greater than 0.30. The root mean square error of approximation, comparative fit index and goodness-of-fit index were 0.07, 0.61 and 0.71, respectively. The score of scale in children with injury was significantly higher than that in children without injury (P=0.022).@*Conclusion@#The validity and reliability of the environment scale for unintentional injury in the home for children aged 0-6 years old in the urban area of China are good.

ABSTRACT

Objective::To explore the potential active ingredients and possible anti-breast cancer mechanism of Glycyrrhizae Radix et Rhizome and Aurantii Fructus based on the method of network pharmacology. Method::The main potential targets of Glycyrrhizae Radix et Rhizome and Aurantii Fructus on breast cancer were summarized by comparing the Glycyrrhizae Radix et Rhizome-Aurantii Fructus active ingredients screened from Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform(TCMSP) and breast cancer targets searched in Therapeutic Target Database (TTD). Cytoscape 3.7.1 software was used to establish a Glycyrrhizae Radix et Rhizome-Aurantii Fructus active ingredients-target-disease network and perform topology analysis based on the network. Result::According to related conditions of drug-like (DL) and oral bioavailability (OB), the network of Glycyrrhizae Radix et Rhizome-Aurantii Fructus active ingredients-breast cancer target was obtained, covering a total of 133 nodes, 116 chemical components and 17 breast cancer drug targets, 109 active components of Glycyrrhizae Radix et Rhizome interacting with breast cancer drug target, 6 active ingredients of Aurantii Fructus interacting with breast cancer drug targets, and 1 common active ingredient of Aurantii Fructus and Glycyrrhizae Radix et Rhizome interacting with breast cancer targets. There were 400 breast cancer target-interaction target pairs in the network diagram. Conclusion::The anti-breast cancer effect of Glycyrrhizae Radix et Rhizome and Aurantii Fructus is based on the overall pharmacodynamic effect of multi-component, multi-pathway and multi-target, the investigation of its potential anti-breast cancer mechanism provides theoretical basis for further experimental research.

ABSTRACT

To evaluate the drug susceptibility of multidrug-resistant bacteria in elderly patients(≥65 years)with bloodstream infection, and to explore the influencing factors on generation of multidrug-resistant bacteria, so as to provide scientific basis for the early and accurate administration of antibiotics.Methods:The clinical data of the elderly patients diagnosed as bloodstream infection in Luhe Hospital from January 2016 to December 2018 was retrospective analyzed.Based on blood culture results, the isolated pathogens were divided into the multidrug-resistance group and the control group.Univariate analysis was performed by χ2 test, and the variables with statistical significance were included in the multivariate Logistic regression analysis to evaluate the influencing factors of multidrug-resistance bacteria’ infection. Results:Among 389 elderly patients with bloodstream infection, 221(56.81%)had multidrug-resistant bacteria infection.The top three bacteria detected were coagulase-negative staphylococcus(77.42%, 48/62), escherichia coli(56.80%, 71/125)and acinetobacter baumannii(55.88%, 19/34). Patients who had nosocomial acquired infections( OR=2.000, 95% CI: 1.160-3.447), used penicillins( OR=3.911, 95% CI: 1.835-8.336), used cephalosporins and quinolones within 3 months( OR=3.867, 95% CI: 1.956-7.646; OR=2.257, 95% CI: 1.087-4.688, respectively), or application of more than two antibiotics( OR=14.711, 95% CI: 1.861-116.258)had a significantly increased risk of multidrug-resistant infections. Conclusions:Taking effective preventive measures against nosocomial infections, rational use of penicillins, cephalosporins and quinolones, and careful combination use of antibiotics might be effective measures to reduce bloodstream infections caused by multidrug-resistant bacteria in the elderly.

ABSTRACT

Objective:To report our experience in diagnosis and treatment of posterior atlantoaxial dislocation with odontoid retrolisthesis.Methods:A retrospective study was conducted of the 5 patients who had been treated from July 2012 to August 2018 at Department of Orthopaedics, General Hospital of Southern Theater Command for posterior atlantoaxial dislocation. They were 4 men and one woman, aged from 34 to 67 years (average, 47 years). All of them had a history of trauma. Of them, 4 were complicated with odontoid fracture and one with congenital free os odontoideum. Their posterior atlantoaxial dislocation ranged from 3 to 9 mm (average, 6 mm). By the American Spinal Injury Association (ASIA) grading system, their preoperative spinal injury was rated as grade B in one, as grade C in 3 cases and as grade D in one. All the 5 patients underwent skull traction at 10° flexion. Surgical trans-oralpharyngeal atlantoaxial reduction and internal fixation was performed for the one patient whose reduction had not been achieved by traction while posterior atlantoaxial screw-rod fixation or anterior odontoid screwing was conducted for the 4 patients whose reduction had been achieved by traction. The distance of posterior atlantoaxial dislocation was measured to evaluate their reduction and ASIA grade system was used to assess their spinal function after operation.Results:The postoperative distance of posterior atlantoaxial dislocation was 0 mm, showing a reduction rate of 100%. The 5 patients were followed up for 6 to 36 months (average, 15 months). By the ASIA grade system, the postoperative functional recovery of the spine was grade D in 4 cases and grade C in one. No implant loosening or breakage occurred.Conclusion:As a kind of high-energy hyperextension injury, posterior atlantoaxial dislocation is rare in clinic, but an appropriate treatment can be adopted to deal with its different clinical types to achieve good outcomes.

ABSTRACT

Objective@#To evaluate the validity of psychological care combined with enhanced recovery after surgery (PC+ERAS) management in perioperative nursing care of andrological patients.@*METHODS@#A total of 300 male patients undergoing andrological surgery were included in this study, 150 given PC+ERAS and the other 150 receiving routine nursing care as controls. We evaluated anxiety and depression of all the patients on admission and discharge using Self-Rating Anxiety Scale (SAS) and Self-Rating Depression Scale (SDS), and compared post-operative hospital days, off-bed time, first passage of flatus, Visual Analog Scale (VAS) score and satisfaction with nursing care between the two groups of patients.@*RESULTS@#On discharge, significant improvement was observed in SAS and SDS scores in the PC+ERAS group compared with the baseline, even more significant than in the control group (P 0.05). The patients in the PC+ERAS group also achieved a significantly shorter post-operative hospital stay, earlier post-operative off-bed time and passage of flatus, lower VAS score, and higher satisfaction with nursing care than those in the control group (P < 0.05).@*CONCLUSIONS@#Psychological care combined with ERAS management deserves wide application in the perioperative nursing care of andrological patients, which can significantly improve the patients' anxiety and depression, shorten post-operative hospital stay, reduce VAS score, and increase their satisfaction with nursing care.

ABSTRACT

OBJECTIVE:To eval uate therapeutic effic acy,safety and economical efficiency of Ginkgolide injection versus Butylphthalide injection in the treatment of ischemic stroke. METHODS :Among the GISAA of Ginkgolide injection in the treatment of ischemic stroke of large-artery atherosclerosis ,106 patients who were given Ginkgolide injection+Asprin enteric-coated tablets but did not use butylphthalide in any dosage in previous trial group were selected as ginkgolide group ;56 patients who were given Butylphthalide injection+Ginkgolide injection+Asprin enteric-coated tablets in previous placebo group were selected as control group. The effects ,safety and economical efficiency were compared between 2 groups. Effect indexes included recurrence rate , mortality,NIHSS score ,modified Rankin score (mRS),Barthel index and comprehensive efficacy. The safety indexes included incidence of bleeding event and adverse event during treatment. Cost-minimization analysis was used for economic evaluation. RESULTS:There was no statistical difference in recurrence rate ,mortality,NIHSS score ,the proportion of subjects with mRS 0-2,Barthel index ,comprehensive efficacy and the incidence of adverse event between 2 groups on 28th day after treatment (P> 0.05). NIHSS score of ginkgolide group was better than that of control group on 7th and 14th day (P<0.05). Results of cost-minimization analysis showed that total cost of ginkgdide group was (13 768.19±4 981.54)yuan on 14th day of treatment , which was significantly lower than (22 578.52±7 523.23)yuan of control group (P<0.01). The results of sensivity analysis indicated that the minimum lost analysis was stable. CONCLUSIONS :For the treatment of ischemic stroke ,ginkgolide+aspirin is similar to butylphthalide+aspirin in improving clinical outcome and safety of 28 days,but is better than it in short-term efficacy of improving neurological deficit , and better short-term economical efficiency.

Objective@#To discuss the surgical safeness of the cleft palate children with airway stenosis by means of analyzing characteristics of the shape of the upper airway and comparing clinical data of cleft palate children with airway stenosis and non-airway stenosis.@*Methods@#Tracing back from Apirl 2015 to Apirl 2017, 126 cleft palate children treated in Shanghai Children′s Medical Center, Shanghai Jiao Tong University School of Medicine, were included (46 male and 80 female, age of 7 to 74 months, median age 18 months). According to the spiral CT scan of neck, patients were categorized to airway-stenosis group (65 patients) and non-airway-stenosis group (61 patients). For airway-stenosis group, suspected difficult intubation plan is applied, guiding endotracheal intubation via visible laryngoscope. For non-airway-stenosis group, ordinary plan of endotracheal intubation is applied. Study the statistics of both groups in the measurement of the upper airway, the success rate of tracheal intubation, operation time, hospital day.@*Results@#Based on anatomical location of the airway stenosis, cleft palate children were divided into: nasopharynx, 5 cases; laryngel, 55 cases; initiation part of trachea to arch of aorta, 2 cases; arch of aorta to bronchial bifurcation, 3 cases. Regardless of airway stenosis, the upper airway of cleft palate children in the subglottic area and the cricoid area was elliptical, with the transverse dimension narrow and the anteroposterior dimension wide. Comparing to non-airway-stenosis group, the airway in airway-stenosis group remained narrower in the anteroposterior dimension in the subglottis area [(7.69±1.76) mm]; also remained narrower in the transverse dimension [(5.96±1.27) mm] and the anteroposterior dimension [(8.16±1.31) mm] in the cricoid area (P<0.05). Pre-and post-operative monitor blood oxygen saturation of all patients were normal. Ventilator weaning of all patients was successful. There were no statistical significance in operation time and hospital day between airway-stenosis group and non-airway-stenosis group (P>0.05).@*Conclusions@#The upper airway of the subglottic area and the cricoid area in cleft palate children are elliptical, with the transverse dimension narrow and the anteroposterior dimension wide. Cleft palate children with airway stenosis underwent surgery smoothly by using a portable visible laryngoscope.

ABSTRACT

Objective To investigate a new clinical classification of hepatolithiasis based on biliary anatomy and pathology in order to guide clinical decision-making.Methods The cases of hepatolithiasis admitted to our hospital from Mar 2015 to Aug 2018 were classified into six types and an additional type according to biliary anatomy,stone site and extent,liver pathological changes,and Oddi sphincter function.Results Of the214 cases of hepatolithiasis,5 cases (2.3%) were type Ⅰ;41 cases (19.2%) were type Ⅱ;76 cases (35.5%) were type Ⅲ;17 cases (7.9%) were type Ⅳ;29 cases (13.5%) were type Ⅴ;46 cases (21.5%) of type Ⅵ.The most common complication was incision infection (28 cases,13.1%),liver failure 0,and perioperative death 0.During the follow-up from 3 months to 3 years,there were 14cases of stone recurrence,the recurrence rate was 7.6%.Conclusion This classification scheme and the "individualized precise anatomical combined hepatectomy or segmental hepatectomy" based on this classification is safe and effective while enhancing the cure rate.

ABSTRACT

Objective To evaluate the effect of aerobic exercise combined with resistance exercise on blood sugar and constitution in patients with type 2 diabetes mellitus.Methods Fifty eligible subjects in community type 2 diabetes database were randomly selected and a model intervention group of"1+2+1" was established.The regular exercise intervention was conducted for six months by combining aerobic exercise with resistance exercise.The fasting blood sugar,blood pressure,heart rate,body mass and fat mass before and after intervention were compared by paired t test.Changes in related physical indicators.Results After 6 month of exercise therapy,fasting blood glucose of subjects reduced from (8.58±4.40) mmol/L to (6.29 ±1.72) mmol/L(P=0.032).Body weight also reduced from (62.44±7.35) Kg to (60.70±7.54) Kg(P=0.008). In physical quality related indexes, fat mass decreased significantly, while protein content increased significantly.There were no changes in muscle mass and fat free body weight.In addition, grip strength and flexion range of subjects significantly increased after exercise therapy.Comprehensive score of physical quality of subjects increased from (73.25± 5.65) to (75.48± 5.04) ( P=0.010).Conclusion Exercise therapy of aerobic exercise combined with resistant exercise can effectively reduce blood glucose, body weight and fat mass,increase muscle strength,flexibility and physical quality.This also reflects that the community?led "health and physical integration" mode of work is an effective means to manage chronic diseases and enhance the physical fitness of residents.

Objective@#To explore the safety, feasibility and advantages of Da Vinci robot "3+ 2" mode in radical gastrectomy.@*Methods@#Clinical data of 30 patients received radical gastrectomy at our department under the "3+ 2" mode of Da Vinci robot from Jan. 2018 to Oct. 2018 were retrospectively analyzed. 30 patients who received classical robot mode in the same period were randomly selected as control group.@*Results@#Compared with the control group, the observation group had a shorter operative time (174±16)min vs.(197±19)min, t=-3.275, P<0.01) , a lower operation costs[(22 794±3 660)yuan vs.(29 307±3 220)yuan, t=-3.316, P<0.01; and inhospital costs (70 910±6 399)yuan vs.(78 180±6 636)yuan), t=-7.316, P<0.01].@*Conclusion@#The "3+ 2" mode of Da Vinci robot in radical gastrectomy is beneficial to enhance the exposure of the surgical field, improve the cooperation of assistants, reduce the costs of surgery, shorten the operative time and the learning curves. In addition, it has similar clinical efficacy to radical gastrectomy under the classical robot mode.

ABSTRACT

Objective To investigate the expression and prognostic significance of DEAH-box helicase (DHX16) by pan-cancer analysis. Methods The expression and prognostic significance of DHX16 were analyzed using the UALCAN web-portal. Gene ontology and kyoto encyclopedia of genes and genomes analyses of proteins interacting with DHX16 were performed using DAVID 6.8 functional annotation software. Results DHX16 was highly expressed in bladder urothelial carcinoma, head and neck squamous cell carcinoma, esophageal carcinoma, liver hepatocellular carcinoma, and cholangiocarcinoma (all P < 0.001). Proteins interacting with DHX16 were located mainly in catalytic step 2 spliceosome, nucleoplasm, and cell membrane, and participated in mRNA splicing and processing, binding to poly (A) RNA and nucleic acids, and RNA helicase activity. Spliceosome, mRNA surveillance, RNA degradation, and transport pathways were implicated. Conclusion The high expression of DHX16 is helpful for the prognosis of bladder urothelial carcinoma prognosis, and unfavorable for prognoses of adrenocortical carcinoma, sarcoma, brain lower grade glioma, liver hepatocellular carcinoma, and acute myeloid leukemia. Thus, DHX16 may have value as a prognostic marker.

ABSTRACT

Purpose To describe the clinicopathologic features, diagnosis and differential diagnosis, and prognosis of secretory breast carcinoma (SBC). Methods Clinicopathological and follow-up data of six SBC patients were collected. Histopathologic analysis was performed on hematoxylin and eosinstained (HE) section. Immunohistochemical staining was performed by En Vision two-step method and ETV6 gene detected by fluorescence in situ hybridization (FISH), then relevant literatures were reviewed. Results The ages of the patients ranged from 6 to 76 years with a mean age of 38.7 years, including one male and five female patients. The right breast was involved in 4 cases, and the left, in 2 cases. Five cases showed painless breast mass while one presented with a nipple discharge. The tumor size ranged from 1.0 to 3.1 cm with a mean size of 2.0 cm. Most of the tumors were circumscribed, solid gray white to light brown. Histologically, tumor showed solid nested microcystic, glandular or papillary pattern separating by hyaline fibrous tissue and growed in multiple nodular from. The cytoplasm contains abundant eosinophilic secretions or secretory vesicles. Immunhistochemistry, all cases were positive for CK7, S-100 and CEA, but negative for estrogen and progesterone receptors (ER and PR) and HER-2, and the proliferation index Ki-67 ranged from 10% to 40%. Molecular testing confirmed the presence of the EVT6 gene translocation in one case. Lumpectomy was performed in 2 cases and modified radical mastectomy in 4 cases, two of them had lymph node metastasis (3/15, 1/16). Five cases were followed up for 6 months to 20 years, 1 case had lung metastasis. Conclusion SBC is a rare breast tumor with relatively indolent clinical and good prognosis. It can be diagnosed according to typical pathological morphology and immunohistochemical characteristics. The characteristic EVT6 gene translocation also has important differential diagnostic value.

OBJECTIVE@#To investigate the long-term outcome of laparoscope-assisted transanal total mesorectal excision (taTME) for rectal cancer.@*METHODS@#Clinicopathological data of 29 patients with mid-low rectal cancer undergoing laparoscope-assisted taTME at Department of Gastrointestinal Surgery, the First Affiliated Hospital of Guangzhou Medical University from May 2010 to December 2015 were collected for the retrospective case-series study. All the operations were performed with transabdominal and transanal procedure simultaneously or sequentially. Perioperative presentations, pathological examinations, and oncologic outcomes were retrospectively analyzed. Long-term recurrence, metastasis and survival were assessed during follow-up. Outpatient clinic and telephone survey were used for follow-up. The follow-up time ended in December 2018. The overall survival (OS) rate and disease-free survival (DFS) rate were calculated by the Kaplan-Meier method.@*RESULTS@#The average intra-operative blood loss was (75.9±9.5) ml (range,20 to 200). The average operating time was (223.6±4.1) minutes (range, 165 to 280). The average number of harvested lymph node was 22.3±2.0. The average length of pathological specimen was (13.1±0.6) cm. The average distal resection margin was (2.9±0.2) cm. 89.7% (26/29) of specimens was complete and 10.3% (3/29) near complete. Two cases (6.9%) had positive cutting circumferential margin. Median follow-up was 56 (range, 22 to 91) months. Local recurrence rate, distant metastasis rate, 3-year OS rate, 3-year DFS rate, 5-year OS rate, 5-year DFS rate were 10.3% (3/29), 20.7%(6/29), 96.6%, 83.2%, 87.6% and 79.6%, respectively. No incisional hernia or adhesive intestinal obstruction was found.@*CONCLUSION@#Long-term outcomes of mid-low rectal cancer patients undergoing laparoscope-assisted taTME are satisfactory.

ABSTRACT

OBJECTIVE@#To explore infection rate of different adeno-associated virus (AAV) on knee joint cartilage in mice and to find a good gene editing tool for mice chondrocytes of knee joint.@*METHODS@#Forty-five 4-week-old SPF C57BL/6 weighed(14.3±0.2) g were selected. According to different injections(6 μl) for right knee joint, mice were divided into 9 different groups, 5 mice in each group. The groups were such as following:control group (normal saline), Vigene 2 group (AAV2 from vigene biosciences, titer for 1×10¹³ vg/ml), Vigene 5 group (AAV5 from vigene biosciences, titer for 1×10¹³ vg/ml), Vigene 6 group (AAV6 from vigene biosciences, titer for 1×10¹³ vg/ml), Vigene 7 group (AAV7 from vigene biosciences, titer for 1×10¹³ vg/ml), Vigene 8 group (AAV8 from vigene biosciences, titer for 1×10¹³ vg/ml), Vigene 9 group (AAV9 from vigene biosciences, titer for 1×10¹³ vg/ml), Hanbio DJ group(AAV2-DJ from Hanbio, titer for 1×10¹² vg/ml), Hanbio 5 group (AAV5 from Hanbio, titer for 1×10¹² vg/ml). All AAVs were over-expressed green fluorescent protein(GFP). Knee joint specimens were taken and observed injury of cartilage under stereomicroscope at 30 days after injection, then 10 μm thick frozen sections were prepared. Distribution of green fluorescent protein of meniscus and cartilage of knee joint was observed under fluorescence microscope.@*RESULTS@#Stereomicroscope observation indicated that no obvious lesion was observed in knee joint cartilage of mice after intra-articular injection of AAV. According to frozen sections of knee joints, strong green fluorescence was observed in knee joint cartilage in all AAV experimental groups. Compared with other groups, significantly stronger green fluorescence were observed both in AAV2 and AAV7 groups, whose average fluorescence density was 0.077±0.020 and 0.061±0.022. There were significant differences between two groups and other groups.@*CONCLUSIONS@#AAV could infect chondrocyte of knee joint in vivo by injecting into knee joint cavity. Higher infection efficiency of AAV2 and AAV7 on knee joint cartilage were observed. Local injection of AAV into knee joint cavity could be used as an effective tool for gene editing of knee joint chondrocyte.
