ABSTRACT
Bladder cancer is a common malignant tumor of the urinary system. The prognosis of patients with positive lymph nodes is worse than that of patients with negative lymph nodes. An accurate assessment of preoperative lymph node statushelps to make treatmentdecisions, such as the extent of pelvic lymphadenectomy and the use of neoadjuvant chemotherapy.Imaging examination and pathological examination are the primary methods used to assess the lymph node status of bladder cancer patients before surgery. However, these methods have low sensitivity and may lead to inaccuate staging of patients. We reviewed the research progress and made an outlook on the application of clinical diagnosis, imaging techniques, radiomics, and genomics in the preoperative evaluation of lymph node metastasis in bladder cancer patients at different stages.

Dysmenorrhea is a common gynecological disease in clinic, with primary dysmenorrhea and secondary dysmenorrhea. Primary dysmenorrhea is usually not accompanied by organic lesions in genital organs, which is mainly related to the increase of prostaglandin content in endometrium during menstruation. Secondary dysmenorrhea is accompanied by organic lesions of reproductive organs, often associated with local lesions of reproductive organs, but also with patients' mental factors and neuroendocrine factors. The incidence of dysmenorrhea is as high as 73.8%, and there is no radical cure method, which has a great impact on the life, work and learning of patients. Chinese medicine essential oil widely exists in aromatic Chinese medicine, with antibacterial, antioxidant and anticancer activities. It can regulate neuroendocrine function, anti-inflammatory and analgesic effects, and improve mood by regulating the levels of prostaglandins, oxytocin and other hormones in the body and regulating the emotions of patients, there by alleviating dysmenorrhea to a certain extent. In recent years, many scholars have made more in-depth research on Chinese medicine essential oil in alleviating dysmenorrhea, but there is a lack of comprehensive collation of such studies. In this regard, the author has systematically sorted out the generation and classification of dysmenorrhea, the mechanism of action of essential oil of traditional Chinese medicine to alleviate dysmenorrhea and the application of essential oil of traditional Chinese medicine in the field of dysmenorrhea by consulting relevant literature in Chinese and foreign languages in recent years, so as to provide reference for the treatment of dysmenorrhea.
ABSTRACT
OBJECTIVE@#To observe the effect of acupuncture on the balance of T helper (Th) 1/Th2 cells in peripheral blood, inflammatory reaction and intracerebral neuroinflammation in vascular dementia (VD) rats, and to explore the mechanism of acupuncture for improving cognitive function in VD.@*METHODS@#A total of 60 SPF Wistar rats were randomized into a normal group (n=12), a sham operation group (n=12) and an operation group (n=36). Bilateral common carotid artery occlusion was adopted to establish the VD model in rats of the operation group. The rats of successful modeling were randomized into a model group and an acupuncture group, 12 rats in each one. In the acupuncture group, Sanjiao acupuncture was applied at "Danzhong" (CV 17), "Zhongwan" (CV 12), "Qihai" (CV 6), "Xuehai" (SP 10) and "Zusanli" (ST 36), the needles were manipulated for 30 s at each acupoint, without retaining. The intervention was given once a day for 15 days, and there was 1-day rest on day 8. Morris water maze test was adopted to observe the ethology, flow cytometry was used to detect the ratio of Th1/Th2 in peripheral blood, and Luminex liquid chip technology was used to detect the levels of interleukin (IL)-4, IL-10, interferon-γ (IFN-γ) and tumor necrosis factor-α (TNF-α) in serum and hippocampus.@*RESULTS@#There were no significant differences in various indexes between the normal group and the sham operation group (P>0.05). Compared with the sham operation group, in the model group, the escape latency of hidden platform test and reversal platform test was prolonged (P<0.01), the residence time of the original platform quadrant was shortened and the number of crossing the original platform was reduced in probe test (P<0.01, P<0.05), the proportion of Th1 cells was increased, the proportion of Th2 cells was decreased and the ratio of Th1/Th2 cells was increased in peripheral blood (P<0.01), the levels of TNF-α and IFN-γ were increased, the levels of IL-4 and IL-10 were decreased in serum and hippocampus (P<0.05, P<0.01). Compared with the model group, in the acupuncture group, the escape latency of hidden platform test and reversal platform test was shortened (P<0.01), the residence time of the original platform quadrant of the probe test was prolonged (P<0.05), the proportion of Th1 cells was decreased, the proportion of Th2 cells was increased and the ratio of Th1 / Th2 cells was decreased in peripheral blood (P<0.05), the levels of TNF-α and IFN-γ were decreased, the levels of IL-4 and IL-10 were increased in serum and hippocampus (P<0.05, P<0.01).@*CONCLUSION@#Acupuncture can improve the cognitive dysfunction and reduce the intracerebral neuroinflammation in VD rats, its mechanism may relate to the regulation of Th1/Th2 cells balance and reduce the inflammatory reaction in peripheral blood.

Objective:To explore the differences in imaging characteristics and risk factors for mortality between the young and middle-aged and elderly patients with aortic dissection. Methods:The data from 1 220 patients with aortic dissection from January 2007 to December 2015 were successively collected. They were divided into young and middle-aged and elderly groups according to their ages. The basic information, computer comography angiography and echocardiogram results, hospital admissions and surgical details were collected and compared between the two groups using single variable regression analysis. Results:The ratio of female patients in the middle-aged and elderly group (>45 years old) was significantly higher than that of the young group (≤45 years old) (24.9% vs 18.7%, P=0.014), the ratio of hypertension patients was also significantly higher than that in the young group (77.5% vs 68.1%, P<0.001). Echocardiographic parameters showed that the prevalences of left ventricular enlargement (39.9% vs 26.9%) and left ventricular dysfunction (22.3% vs 14.1%) in the young group were significantly higher than in the middle-aged and elderly group (all P<0.001). The prevalences of ascending aorta dilation [(43.04±9.12)mm vs (41.69±10.99)mm] and thrombosis (39.1% vs 28.5%) were higher ( P<0.05). Although there was no statistically significant difference in mortality within 60 days after admission between the two groups (12.0% vs 15.1%, P=0.164), univariate regression analysis showed that aortic branch involvement was the main independent risk factor for mortality in young patients. Left ventricular remodeling (left atrial enlargement, ascending aortic dilation, aortic regurgitation) and decreased heart function were the main independent risk factors for death in the elderly patients. Conclusions:The mortality rate of young and middle-aged and elderly patients with aortic dissection are comparable, but the influencing factors are different. Screening out the relevant risk factors that affect the survival rate of the two groups, and conducting targeted prevention and intervention on them areof great significance to improve the survival rate of patients.
ABSTRACT
Objective: To analyze the features of degenerating cystic thyroid nodules (DCTN) on conventional ultrasound and contrast-enhanced ultrasound (CEUS), and to explore the differentiation between DCTN and papillary thyroid carcinomas (PTC). Methods: A total of 46 DCTN (39 cases, including 12 males and 27 females, with an age range of 25 to 76 years) and 36 PTC (32 cases, including 8 males and 24 females, with an age range of 23 to 68 years) diagnosed via fine- needle aspiration (FNA) or surgery from February 2019 to January 2020 in the First Affiliated Hospital of Nanchang University were enrolled. The size, shape, margin, echogenicity, presence of shadowing, calcification and vascularity of DCTN and PTC were retrospectively evaluated, and 28 DCTN and 30 PTC underwent CEUS were separately analyzed and compared. The t test, χ² test or Fisher's exact test were implemented to compare the features of ultrasound among the two groups. The binary Logistic regression test was performed to determine whether the feature whose difference was statistically significant was an independent predictive risk factor. Results: A univariate analysis indicated that DCTN more frequently showed wider-than-tall shapes, marked hypoechogenicity, well-defined margin and no or dot-lined enhancement (wider-than-tall shapes: 36 vs. 17, χ2=8.511; well-defined margin: 30 vs. 15, χ2=4.523; marked hypoechogenicity: 27 vs. 9, χ2=9.310; no or dot-lined enhancement: 24 vs. 3, χ2=33.369; all P<0.05). A multivariate analysis demonstrated that wider-than-tall shapes, well-defined margin and marked hypoechogenicity were independent predictors for DCTN (OR values were 5.204, 3.134 and 5.042, P values were 0.003, 0.031, and 0.003, respectively). Among 28 DCTN, 15 showed a decrease in mean maximum diameter (24.3±11.4 mm) with a mean time span of (18.6±10.5) months between the presence and absence of suspicious ultrasound features. Conclusions: Compared with PTC, DCTN shows the ultrasound characteristics of wider-than-tall shapes, well-defined margin, marked hypoechogenicity and no or dot-lined enhancement pattern. Ultrasound follow-up can help to identify spontaneous DCTN.

ABSTRACT
Volatile oil is widely distributed in Chinese medicinal materials with complex chemical components. The main components are terpenes, aromatics, aliphatics, and nitrogen and sulfur containing. It has a variety of pharmacological activities. Such as antibacterial, anti-inflammatory, analgesic, anti-tumor, anti-oxidant, anti-aging and so on. It is widely used in medical and health care, agricultural efficiency enhancement, and daily products. In recent years, there have been a large number of studies on the pain relief of traditional Chinese medicine (TCM) essential oils, but there is no systematic generalization. The author found that the mechanism of TCM essential oils to exert analgesic effects mainly includes regulation of the central nervous system, anti-inflammatory and analgesic, antispasmodic and analgesic effects by consulting Chinese and foreign literatures in recent years, but the exact mechanism needs to be further verified. This article reviews the research progress of TCM essential oil pain relief from the aspects of pain classification, generation, analgesic mechanism and combination of other technologies, in order to provide reference for related research in the future.
ABSTRACT
With advanced brewing technology and contemporary table culture, alcohol drinking, which can be traced back to Dukang wine in the Xia dynasty, is very common in China. However, excessive alcohol intake can easily cause alcohol liver damage, ranging from abdominal pain and venous thrombosis to severe hypoglycemia and fat embolism, coma shock and even life-threatening cases. Puerariae Lobatae Radix has a cool property and sweet taste, with functions of antipyretic, promoting the secretion of saliva or body fluid, rash and hangover alleviation, and so on. It was first recorded in Shen Nong's Materia Medica and has been listed as a special anti-alcoholic medicine in traditional Chinese medicine since ancient times. For example, the ancient medical book Compendium of Materia Medica and other records claim that Puerariae Lobatae Radix has the effect of relieving alcohol and protecting the liver. At the same time, Puerariae Lobatae Radix has a long history in both medicine and food. It was listed in the List of Articles That Both Serve as Food and Medicine published by the National Health Commission. Therefore, there are many products containing pueraria for hangover and liver protection. Prior to this, many scholars have carried out relevant researches on the anti-alcoholism efficacy of Puerariae Lobatae Radix, but there is a lack of systematic summaries. The author has consulted relevant domestic and foreign literatures in recent years. The related products were summarized and it was found that the anti-alcoholic effect of pueraria root mainly came from puerarin, pueraria flavonoids and pueraria polysaccharide, puerarin polypeptide, pueraria daidzein and its derivatives, including the main mechanisms such as inhibiting alcohol absorption, accelerating metabolism, anti-oxidation, protection of liver and cardiomyocytes, and neuroprotection. Related products are abundant and well evaluated, but research on related genes needs to be deepened. This article reviews the main anti-alcoholic components, mechanism of action and related products of pueraria, and puts forward suggestions for future research directions, hoping to provide reference for further related research.
ABSTRACT
Objective:To explore the morphological parameters of tricuspid in patients with moderate or above functional tricuspid regurgitation (FTR ) by real-time three-dimensional (3D) transthoracic echocardiography. Methods:A total of 31 moderate or above FTR(FTR group) and 36 healthy adults(control group) were included in the study from May to July 2020 in the First Affiliated Hospital of Air Force Medical University (Xijing Hospital). Two-dimensional (2D) and 3D echoardiography were performed on both FTR group and control group. The 2D parameters included tricuspid regurgitation (TR), transverse diameter of right atrium/ ventricle, transverse diameter of tricuspid annulus(TVD), left ventricular ejection fraction (LVEF) and left ventricular fractional shortening (LVFS). The tricuspid 3D parameters were analyzed by TomTec software, including annular parameters[annular anterior-posterior diameter (AP), annular left-right diameter (AL-PM), sphericity index (SI=AP/AL-PM), non-planar angle (NPA), annulus circumference (AC), annulus area (AA), annulus height (AH)], leaflet parameters[tenting volume(TV), tenting area(TA), tenting heigh(TH), commissural diameter(CD)], and dynamic parameters[annular displacement max(ADmax), annulus area fraction(AAF)].Results:Compared with the control group, AP, AL-PM, SI, AC, AA, TV, TA parameters of FTR group were larger, ADmax was smaller (all P<0.05). Multiple linear regression model was used to evaluate the effects of RA, RV, AP, AL-PM, SI, TV, TA on FTR, and the results showed that FTR was associated with TV and TA( t=4.253, -2.14; all P<0.05). Conclusions:RT-3DE can be used to assess the morphological parameters of tricuspid valve with moderate or above FTR patients effectively. TV and TA are the two most important factors for moderate or above FTR.
ABSTRACT
In this study, gas chromatography coupled with mass spectrometry(GC-MS) was used to analyze the changes of 12 kinds of cancer cells treated by curcumin. The related differential metabolites were screened and the metabolic pathways were analyzed to explore the anti-tumor mechanism of curcumin. Methyl thiazol tetrazolium(MTT) assay was used to detect the 50% inhibiting concentration(IC_(50)) of curcumin on 12 human tumor cells. After treatment with curcumin for 48 h, the cells were collected and analyzed by GC-MS, followed by pathway analysis and multivariate data analysis including principal component analysis(PCA), orthogonal partial least squares discriminant analysis(OPLS-DA) and One-way analysis of variance(ANOVA),etc. Overall, 34 metabolites showed significant concentration changes after intervention for 48 h, mainly involving multiple metabolic pathways, including lysine degradation, glycine, serine and threonine metabolism, arginine and proline metabolism, cysteine and methionine metabolism, aminoacyl-tRNA biosynthesis, primary bile acid biosynthesis, lysine biosynthesis. In this study, the anti-tumor mechanisms of curcumin interfering with energy metabolism, amino acid metabolism, microtubule system, protein synthesis and oxidative stress response of tumor cells were analyzed from the perspective of metabolism, providing a new reference for further tumor pharmacology study.

Objective The aim of this experiment was to study the improving effects of a ginsenoside hydrolysis product DS-1227 on scopolamine-induced learning and memory impairment in mice. Methods Sixty healthy 5-6-week old male ICR mice (body weight 22 ±2 g) were randomly divided into control group, model group, three DS-1227 groups (25 mg/kg, 50 mg/kg, 100 mg/kg), and positive control group (0.3 mg/kg).Fourteen days after oral administration of DS-1227, an open-field test was conduct to determine the mouse locomotor activity.Fifteen days after oral administration of DS-1227, all experimental animals were intraperitoneally administered scopolamine (0.75 mg/kg) and the mice of control group received the same volume of saline. In addition to scopolamine, the mice of positive control group received intraperitoneal injection of physostigmine in a dose of 0.3 mg/kg.Twenty minutes after completion of all the drug administration, object recognition test and Morris water maze test were conducted to evaluate the learning and memory abilities of the mice. Results DS-1227 had no significant effect on locomotor activity of the mice.Scopolamine obviously decreased the discrimination indexes in object recognition test, and prolonged the escape latency of water maze place navigation test. While 25 mg/kg, 50 mg/kg, 100 mg/kg of DS-1227 increased the discrimination indexes and decreased the escape latency of place navigation in the mice. Conclusion DS-1227 can improve the learning and memory impairment induced by scopolamine in mice.
ABSTRACT
Objective To compare the efficacy of intralesional glucocorticoid injection with a needle-free injector versus an ordinary injector for the treatment of keloid. Methods A total of 60 patients with keloid were enrolled and randomly divided into two groups by using a random number table to receive intralesional injection of compound betamethasone with a needle-free injector(n=31)or an ordinary injector(n=29). The injection was given at a dose of 0.2 ml/cm3 once every 3 weeks for 3 sessions. Parameters for therapeutic efficacy were assessed, adverse reactions were recorded, and clinical pictures were taken before and after each treatment. Statistical analysis was carried out by the Mann-Whitney U test and chi-square test with the SPSS 19.0 software. Results Compared with the ordinary injector group, the needle-free injector group showed significantly different injection time during the first and second treatment (U=299.000, 773.500, respectively, both P=0.000), as well as duration of pain after the first injection(U=730.000, P=0.003). After three sessions of treatment, there was a significant difference in the volume, height, hardness of keloid, scores for pain, itching and appearance, and number of injection points between the needle-free injector group and ordinary injector group (U=295.000, 336.500, 264.000, 464.000, 451.500, 308.000, 233.500, P=0.001, 0.007, 0.000, 0.041, 0.043, 0.003, 0.001, respectively). No significant differences were observed in the incidence of adverse reactions between the two groups(all P>0.05). Moreover, the interval for lesion recurrence was significantly shorter in the needle-free injector group(11.8 days, 95%CI:10.96-12.6 days)than in the ordinary injector group(21.2 days, 95%CI:13.96-28.45 days). Conclusion Compared with the ordinary injector, the needle-free injector shows better efficiency for the treatment of keloid with decreased difficulty in injection and improved compliance in patients.
ABSTRACT
This study was aimed to search anti-platelet aggregation effectors from Gardenia jasminoides extract with the employment of platelet affinity extraction method coupled with HPLC, in order to provide pharmacological experi-mental evidences of the selected effectors to verify its feasibility. Under physiological conditions, washed rat platelets were added into G. jasminoides extract and then a mixture was gained. Consequently, some components from G. jas-minoides extract were combined to the platelets in the mixture while some were not owing to their special chemical structures and properties. Firstly, the uncombined components were washed off from the mixture. Secondly, the com-bined components in the leftover was washed down and collected, respectively, right after destroying the occupied platelets' structures. Thirdly, different collected eluents were analyzed, respectively, by HPLC established in the pre-vious work to search the effectors. Fourthly, pharmacological experiments were implemented for confirmation. The re-sults showed that dominant effective components from G. jasminoides extract acting on anti-platelet aggregation were identified as geniposide. Further evident was provided as well by pharmacological experiment that geniposide exhibit-ed significant inhibitory effect on anti-platelet aggregation in rats induced by ADP, rat tail collagen and thrombin(P< 0.01). It was concluded that the platelet affinity extraction-HPLC method proposed in this paper can be utilized to analyze the correlation of effectors from G. jasminoides extract and its pharmacological effects. Moreover, there are some correlations between screened chemical substances and their pharmacological effects.
