ABSTRACT

Objective:Kallmann syndrome(KS) is a complex genetic disease characterized by congenital hypogonadotropic hypogonadism and anosmia. More than 20 genes have been reported to be associated with KS. Herein, we explore potential genetic aberration in 3 KS patients using the whole-exome sequencing. The potentially pathogenic variants filtered were validated by Sanger sequencing. Methods:Genomic DNA was extracted from the peripheral blood of 3 patients with KS and their family members. Sanger sequencing and pedigree verification were performed on the pathogenic variants identified using whole-exome sequencing. The function of the mutation sites were analyzed with bioinformatics software. Results:The proband 1 was a 25 years old male, characterized by lower gonadotropin gonad hypofunction, early grey hair and bilateral sensorineural hearing loss. A heterozygous mutation c. 475C>T(p.R159W) of SOX10 gene was detected in the proband 1. His mother, sister and cousin who had KS phenotype were also found carrying this mutation, showing an autosomal dominant inheritance. The proband 2 was a 15-year-old male with hypogonadotropic hypogonadism and unilateral renal agenesis. The proband was hemizygous for c. 844delC(p.R282Vfs*28) of ANOS1 gene, his mother was heterozygous for the mutation, which was consistent with the X-linked recessive inheritance. The proband 3 was a 21 years old female, characterized by hypogonadotropic hypogonadism and anosmia. A heterozygous missense mutation c. 149G>A(p.R50Q) was detected in FGF17 gene. The mutation p. R50Q was predicted to be pathogenic by the SIFT and PolyPhen2 programs, and has not been reported in HGDM database yet, which considered to be a novel mutation. Conclusion:KS is a clinically and genetically heterogeneous disease. In this study, ANOS1 c. 844delC, SOX10 c. 475C>T and FGF17 c. 149G>A mutations were found in 3 patients with KS by whole exome sequencing, which would expand the genotypic and phenotype spectrum of KS.

ABSTRACT

This review made a systematic textual research on the historical evolution and changes of the origin, producing areas and quality, harvest time and processing methods, and cultivation technique of Poria in famous classical formulas from the Catalogue of Ancient Famous Classical Formulas (the First Batch) by referring to the literature of ancient materia medica and medical books combining with the modern literature in CNKI, Wanfang Data and other databases, which could provide reference for the development and research of the famous classical formulas containing Poria. According to the research, the origin of Poria in ancient times was the dried sclerotia of Poria cocos. The producing area of Poria is widely distributed in China, mainly produced in Anhui, Hubei, Henan and Yunnan provinces. The quality of Poria from Yunnan is better, and the yield is larger in Anhui and Hubei provinces. The quality evaluation of Poria is basically the same from ancient to modern, which has the characteristics of large block, heavy weight, thin and wrinkled skin, and white and delicate cross section. The harvesting period of Poria is usually in August of the lunar calendar. The initial processing has undergone the evolution from drying in shade in the ancient time to drying in shade after sweating in the modern time. From ancient times to the present, Poria has been processed by cutting to prepare the Fuling lumps and Fuling slices. The cultivation technique has experienced the evolution process of "wild Poria-inoculation of live pine root-basswood cultured". At present, Poria is mainly cultivated by artificial basswood with mature technical methods and abundant yields, which can meet the research and development needs of the famous classical formulas.

ABSTRACT

Objective:To study the material basis and potential molecular mechanism of Epimedii Folium against osteoporosis. Method:The chemical components in 14 batches of Epimedii Folium were analyzed by ultra-performance liquid chromatography-quadrupole-time of flight-tandem mass spectrometry (UPLC-Q-TOF-MS/MS). With the activity of alkaline phosphatase (ALP) as the pharmacodynamic index, the partial least squares regression analysis (PLSR) was conducted to establish a model uncovering the spectrum-effect relationship between UPLC-Q-TOF-MS/MS spectral peak and ALP activity and screen the active components against osteoporosis. Online databases such as the Traditional Chinese Medicine Systems Pharmacology Database and Analysis Platform (TCMSP),Comparative Toxicogenomics Database (CTD),Database for Annotation,Visualization, and Integrated Discovery (DAVID) and Cytoscape 3.6.1 were employed to predict the possible mechanism of action of Epimedii Folium against osteoporosis. Result:A total of 61 peaks and 56 compounds were identified by UPLC-Q-TOF-MS/MS. PLSR showed that icariin, baohuoside Ⅰ,epimedin A,sagittatoside A,and baohuoside Ⅱ might be the active components for Epimedii Folium to inhibit osteoporosis considering their strong correlation with ALP activity. As revealed by the network pharmacological analysis of the five components mentioned above,Epimedii Folium mainly regulated seven targets such as tumor necrosis factor (TNF),androgen receptor (AR),and estrogen receptor 1 (ESR1) and eight key pathways like endocrine and other factor-regulated calcium reabsorption, vascular endothelial growth factor (VEGF) signaling pathway, and transient receptor potential (TRP) channels to exert its anti-osteoporosis effect. Conclusion:The exploration of material basis and potential molecular mechanism of Epimedii Folium against osteoporosis based on UPLC-Q-TOF-MS/MS,spectrum-effect relationship, and network pharmacology has provided an experimental basis for the scientific explanation and clinical application of Epimedii Folium in treating osteoporosis.

ABSTRACT

Objective:To explore active components and mechanism of Dipsaci Radix in treating rheumatoid arthritis (RA) based on ultra-performance liquid chromatography coupled with quadrupole time-of-flight tandem mass spectrometry (UPLC-QTOF-MS/MS) and Integrative Pharmacology-based Research Platform of Traditional Chinese Medicine (TCMIP). Method:UPLC-QTOF-MS/MS with electrospray ionization (ESI) was used to qualitatively analyze the chemical components in methanol extract of Dipsaci Radix under positive and negative ion scanning modes. The mobile phase consisted of 0.1% formic acid aqueous solution (A)-acetonitrile (B) for gradient elution (0-10 min, 0.2%-20%B; 10-20 min, 20%-40%B; 20-25 min, 40%-50%B; 25-33 min, 50%-98%B; 33-35 min, 98%-0.2%B), and the scanning range was m/z 50-2 000. Based on TCMIP, candidate target groups of Dipsaci Radix, RA and syndrome with Yin deficiency of liver and kidney were obtained, and correlation analysis on "disease-syndrome-prescription" was used to enrich the main active components and key targets. Cytoscape 3.8.0 and STRING 11.0 database were used to construct protein-protein interaction (PPI) network diagram. Metascape platform was used to analysis gene ontology biological progress and Kyoto Encyclopedia of Genes and Genomes (KEGG) signaling pathways. Result:A total of 81 ingredients were identified by UPLC-QTOF-MS/MS. Based on TCMIP, 283 candidate targets corresponding to 81 ingredients, 7 RA related targets and 215 genes corresponding to syndrome with Yin deficiency of liver and kidney were collected. With further correlation analysis on "disease-syndrome-prescription", 17 key active ingredients were predicted, mainly including saponins and fatty acids of Dipsaci Radix. It mainly involved 7 hub targets, namely tumor necrosis factor (TNF), nuclear factor-κB subunit 1 (NF-κB1), hepatocyte nuclear factor 4 alpha (HNF4A), nuclear receptor subfamily 3 group C member 1 (NR3C1), peroxisome proliferator activated receptor gamma (PPARG), nuclear receptor subfamily 1 group H member 4 (NR1H4) and nuclear receptor coactivator 1 (NCOA1). All of them were related to inflammation, and two of them were related to bile acid pathway. The 7 hub targets and 7 pathways played an important role in RA were screen out, including 4 bile acid related pathways and 3 inflammatory related pathways. Conclusion:UPLC-QTOF-MS/MS combined with TCMIP preliminarily elucidates the regulatory effect of multi-components in Dipsaci Radix on several pathways related to the inflammatory response and bile acid synthesis and metabolism, which lays a foundation for further study on the mechanism of Dipsaci Radix against RA.

ABSTRACT

Objective:To investigate the effect of Elian granule on autophagy and the phosphatidylinositol -3 kinase (PI3K)/protein kinase B (PKB/Akt)/mammalian target of rapamycin (mTOR) signaling pathway in gastric tissue of rats with gastric cancer. Method:SPF SD rats were randomly divided into the normal, model, Elian granule, and Weifuchun groups. In addition to the routine feeding in the normal group, the model, Elian granule, and Weifuchun groups received N-methyl-N'-nitro-N-nitrosoguanidine (MNNG) to induce gastric cancer in rats, and they were respectively given normal saline, Elian granule aqueous solution (3.240 g·kg-1) and Weifuchun aqueous solution (0.390 g·kg-1) by gavage (ig) for 48 weeks. The gross changes of the stomach taken by laparotomy were observed by naked eyes. Hematoxylin-eosin (HE) staining was performed to observe the histopathological changes of the gastric tissue in rats. Real-time quantitative polymerase chain reaction (Real-time PCR) and Western blot (WB) were used to detect the mRNA and protein expression of microtubule-associated protein 1 light chain 3 beta (LC3B), Beclin1, p62, PI3K, Akt, mTOR in rat gastric tissue. Result:Compared with the normal group, the model group showed gastric distension, thinner gastric wall, pale gastric mucosa, atrophied and flat folds, disordered course, and visible nodules and vegetations. Compared with the model group, the Elian granule group demonstrated alleviated gastric distension, dark gastric mucosa, reduced folds, and regular course, with the thinned gastric wall improved and granular nodules observed occasionally. According to HE staining, compared with the normal group, the model group showed crowded and disordered rat gastric glands, diverse in shape, varied cell morphology, basophilic cytoplasm, large irregular hyperchromatic nuclei, visible mitosis, and infiltrated and destroyed muscularis mucosae. While compared with the model group, the arrangement of gastric glands was regular, and a few mildly atypical cells could be observed in rats of the Elian granule group. Compared with the normal group, the model group exhibited decreased expression of LC3B and Beclin1 mRNA and protein in gastric tissue (P<0.05), and increased expression of PI3K, p62, Akt, and mTOR mRNA and protein (P<0.05). Compared with the model group, the Elian granule group showed increased expression of LC3B and Beclin1 mRNA and protein in gastric tissue (P<0.05), and decreased expression of PI3K mRNA and p62, Akt, and mTOR mRNA and protein (P<0.05). Conclusion:Elian granule can improve the cell atypia of gastric tissue in rats with gastric cancer, and the mechanism may be related to the inhibition of the PI3K/Akt/mTOR signaling pathway to promote autophagy.

ABSTRACT

OBJECTIVE:To prepare supersaturated system of lip ophilic aci clovir(ACV)prodrug, and to increase the cutaneous bioavailability of ACV. METHODS :Three prodrugs of ACV were synthesized by anhydride acylation ,i.e. aciclovir acetate (ACV-Ace),butyrate(ACV-But)and hexanoate (ACV-Hex). The structures of ACV and three ACV prodrugs were confirmed by 1H-NMR and HRESI-MS ;the concentrations of ACV and three ACV prodrugs were determined by UPLC-triple quadrupole tandem mass spectrometry ,and saturated solubility of them in different volume fractions of propylene glycol-water solution was calculated. The compound with the greatest potential of form supersaturated system was screened out. The supersaturated system of that compound was prepared by co-solvent method. The effect of hydroxypropyl methylcellulose E 3 (HPMC E 3) on its physical stability was observed by light microscope. Vertical Franz diffusion cells were used to study the effects of degree of supersaturation (DS)and HPMC E 3 on the deposited amount of drug in the excised porcine skin after using the supersaturated system for 1 h. The distribution of ACV in the excised porcine skin was determined by frozen slicing stratified quantitative method after using the supersaturated system and marketed aciclovir cream for 1 h. RESULTS :Three ACV prodrugs were successfully synthesized. The established quantification methods met the requirements of biological sample analysis. Among all of the three ACV prodrugs , ACV-Hex showed the lowest saturated solubility in water [ (0.5±0.0)mmol/L] a nd the highest saturated solubility in propylene glycol [(53.4 ± 14.2)mmol/L],which made it potentially feasible to form supersaturated system with high DS. In 10%propylene glycol-water system ,the addition of HPMC E 3 163.com enabled ACV-Hex supersaturated systems ,with DS no morethan 4,to maintain physical stability within 1 h. The total deposited amount (ACV + ACV-Hex ) in skin after the application of ACV-Hex supersaturated system with DS of 4 for 1 h was higher than that after the application of ACV-Hex supersaturated system with DS less than 4 or without HPMC E 3. In addition ,the concentration of ACV in the basal epidermis (skin thickness was 100-160 mm)by supersaturated system was significantly higher than that of the marketed aciclovir cream (P<0.05). CONCLUSIONS:ACV-Hex,the lipophilic prodrug of ACV ,can form stable supersaturated system with DS of 4 in 10% propylene glycol-water system in the presence of HPMC E 3. High concentration of ACV could be accumulated in the basal epidermis after the skin was exposed to supersaturated system for 1 h,which may be valuable for local treatment skin infection of herpes simplex virus .

ABSTRACT

Type 1 diabetes mellitus and autoimmune thyroid disorders are the most common combination of autoimmune polyendocrine syndrome type Ⅲ(APS Ⅲ). However, APS Ⅲ combined with myasthenia gravis is rare. We described a male patient with myasthenia gravis, type 1 diabetes mellitus, and Hashimoto thyroiditis, who was diagnosed as APS Ⅲ. The human leukocyte antigen (HLA)type was analyzed in this patient. We subsequently reviewed 11 cases of APS Ⅲ combined with myasthenia gravis. This review revealed that HLA-DR9/DQ9 might be a specific HLA subtype associated with APS Ⅲ and complicated with myasthenia gravis .

ABSTRACT

@#Objective    To investigate the value of chest high-resolution computed tomography (HRCT) score in evaluating the severity of hip fracture-induced early acute lung injury (ALI) in the elderly patients. Methods    The clinical data of 289 elderly hip fracture patients in Chongqing Traditional Chinese Medicine Hospital from July 2014 to April 2020 were retrospectively analyzed. All patients were divided into two groups, including an ALI group (n=114, 36 males and 78 females at age of 82.94±6.85 years) and a non-ALI group (n=175, 51 males and 124 females at age of 84.42±6.31 years). General information, chest HRCT scores and PaO2/FiO2 were compared between the two groups. Correlation analysis was used to compare the relationship between chest HRCT scores and PaO2/FiO2. Multiple linear stepwise regression analysis was applied to evaluate the effective extent of the diffuse ground glass opacity (DGGO), intense parenchymal opacification (IPO), and reticulation HRCT scores to the overall HRCT scores. Results    The DGGO scores, IPO scores, reticulation scores, overall HRCT scores and PaO2/FiO2 were higher in the ALI group than those in the non-ALI group (P<0.001). In the ALI group, correlation analysis showed that DGGO, overall HRCT scores were in significantly negative correlation with PaO2/FiO2 (P<0.001). In addition, the correlation among PaO2/FiO2 and overall HRCT scores was more significant than that of DGGO scores. Multiple stepwise regression analysis indicated that DGGO, IPO, and reticulation scores were independent influencing factors for overall HRCT scores. Among the influencing factors, DGGO scores had the greatest impact, then IPO scores and reticulation scores. The HRCT signs of DGGO, IPO, and reticulation appeared simultaneously had the greatest effects on the overall HRCT scores. Conclusion    The chest HRCT score, which is associated with PaO2/FiO2, also can be used in the severity assessment of elderly patients with early ALI caused by hip fracture.

ABSTRACT

OBJECTIVE:To st udy the effects of Anchang decoction on TLR 4/NF-κB signaling pathway and the expression of fecal calprotectin (FC)in TNBS-induced ulcerative colitis (UC)model rats . METHODS :SD rats were randomly divided into blank group ,model group ,positive control group [Live Bifidobacterium capsules ,5 mL(containing Bifidobacterium 0.35 g)], Anchang decoction low -dose,medium-dose and high-dose groups (1,5,10 mL,each milliliter is approximately equivalent to 0.11 g of total crude drug ),with 15 rats in each group. Other groups were given TNBS combined with ethanol enema to establish UC model rat ,except blank group was given normal saline. Two days after successful modeling ,blank group and model group were given normal saline 5 mL,administration groups were given relevant medicine intragastrically ,once a day ,for consecutive 14 d. After last medication ,HE staining was used to observe the pathological change of colon tissue in rats. Western blotting assay was used to detect the protein expression of TLR 4,TRAF6 and NF-κB in colon tissues of rats;Real-time fluorescent quantitative PCR was used to detect mRNA expression of TLR 4,TRAF6,TNF-α and NF-κB;ELISA assay was adopted to detect serum level of TNF-α,IL-6 and FC in stool samples. RESULTS :Compared with blank group ,the colonic mucosa of model group was severely damaged, and the protein expression of TLR 4,TRAF6 and NF-κB,mRNA expression of TLR 4,TRAF6,TNF-α and NF-κb as well as serum levels of TNF-α,IL-6 and FC level in stool samples were increased significantly (P<0.05). Compared with model group, the pathological changes of colon tissue in rats were improved in different administration groups to different extents ,and above indexes were all decreased significantly (P<0.05). CONCLUSIONS :Anchang decoction may relieve the inflammation of UC model rats by regulating the TLR 4/NF-κB signaling pathway and the expression of FC.

ABSTRACT

To evaluate outcomes between extraperitoneal robotic single-port radical prostatectomy (epR-spRP) and extraperitoneal robotic multiport radical prostatectomy (epR-mpRP) performed with the da Vinci Si Surgical System, comparison was performed between 30 single-port (SP group) and 26 multiport (MP group) cases. Comparisons included operative time, estimated blood loss (EBL), hospital stay, peritoneal violation, pain scores, scar satisfaction, continence, and erectile function. The median operation time and EBL were not different between the two groups. In the SP group, the median operation time of the first 10 patients was obviously longer than that of the latter 20 patients (P < 0.001). The median postoperative hospital stay in the SP group was shorter than that in the MP group (P < 0.001). The rate of peritoneal damage in the SP group was less than that in the MP group (P = 0.017). The pain score and overall need for pain medications in the SP group were lower than those in the MP group (P < 0.001 and P = 0.015, respectively). Patients in the SP group were more satisfied with their scars than those in the MP group 3 months postoperatively (P = 0.007). At 3 months, the cancer control, recovery of erectile function, and urinary continence rates were similar between the two groups. It is safe and feasible to perform epR-spRP using the da Vinci Si surgical system. Therefore, epR-spRP can be a treatment option for localized prostate cancer. Although epR-spRP still has a learning curve, it has advantages for postoperative pain and self-assessed cosmesis. In the absence of the single-port robotic surgery platform, we can still provide minimally invasive surgery for patients.

ABSTRACT

OBJECTIVE@#To investigate the clinical features, treatment and prognosis of patients with hematological diseases complicated with mucor infection.@*METHODS@#The risk factors, clinical features, treatment regimen and prognosis of 18 hematological disease patients with mucor infection diagnosed by histopathology in our center from April 2014 to June 2020 were retrospectively analyzed.@*RESULTS@#Thirteen males and five females, with an average age of 30 (13-54) years old, were diagnosed as mucor infection by histopathological examination at the site of infection, including 16 cases of mucor infection alone and 2 cases of mucor + aspergillus mixed infection. There were 12 cases with malignant hematological disease and 6 cases with severe aplastic anemia, all of whom with long-term agranulocytosis, and their clinical manifestations and imaging findings were not specific. The common sites of infection were sinuses and lungs, and some patients showed multiple systemic manifestations. The remission status of hematological diseases and recovery of immune function showed an impact on the prognosis. All the patients were treated with amphotericin B liposome combined with posaconazole, and 15 patients were treated with surgery combined with antifungal drugs, 9 of whom were effective and 6 were ineffective, while intravenous administration in 3 cases was ineffective.@*CONCLUSION@#It is difficult to diagnose hematological disease complicated with mucor infection. After early diagnosis, prognosis can be improved by amelioration of primary state and combination of drugs and surgery.
