ABSTRACT

Objective:To study the effects of Fangji Huangqitang(FJHQT) on migration, adhesion,invasion and tube formation of human umbilical vein endothelial cells (HUVECs) induced by vascular endothelial growth factor (VEGF). Method:HUVECs were induced by VEGF (20 μg·L<sup>-1</sup>) <italic>in vitro</italic>. The effects of FJHQT (0.25,0.5,1 g·L<sup>-1</sup>) on HUVECs were detected by methyl thiazolyl tetrazolium(MTT), scratch repair, transwell migration, adhesion, invasion and tube formation. Protein in HUVECs was extracted and protein expression levels of phosphorylated Janus kinase 1 (p-JAK1) were detected by Western blot. Result:Compared with control group, VEGF (20 μg·L<sup>-1</sup>) can increase the proliferation, scratch repair, transwell migration, adhesion, invasion and tube formation of HUVECs cells (<italic>P</italic><0.01), compared with VEGF group, FJHQT (0.25,0.5,1 g·L<sup>-1</sup>) ,there is no significant effect on the proliferation of HUVECs induced by VEGF for 24 hours, but it can significantly reduce the scratch repair, migration, adhesion, invasion and tube formation of HUVECs induced by VEGF within 24 hours (<italic>P</italic><0.05). Compared with blank group, VEGF could induce abnormal elevation of p-JAK1 in HUVECs (<italic>P</italic><0.01), while FJHQT (0.25,0.5,1 g·L<sup>-1</sup>) could significantly reduce the expression levels of p-JAK1 (<italic>P</italic><0.05,<italic>P</italic><0.01). Conclusion:FJHQT can inhibit the migration, adhesion and invasion of HUVECs, the mechanism may be related to JAK1.

ABSTRACT

In traditional Chinese medicine, it is believed that the spleen is the foundation of acquired nature and the source of Qi and blood. All life activities of a person since birth depend on the water and grain essence transported by spleen and stomach. The liver helps the spleen to strengthen the movement, the liver and spleen cooperate with each other. The liver and the spleen are invigorated, so that the Qi and blood are sufficient. The external energy can nourish the limbs, muscles and fur. The Qi and blood can be supplied to the internal organs, meridians and bones, and the body can be nourished both inside and outside to strengthen the acquired foundation. Emotional dissatisfaction can lead to stagnation of liver Qi, loss of spleen Qi, failure to dredge Qi, and deficiency of spleen Qi, forming the syndrome of liver depression and spleen deficiency. Its clinical manifestations include the symptoms of liver Qi stagnation such as depression, stamina, and chest fullness, as well as symptoms of spleen deficiency such as anorexia, abdominal distension, loose stools. Xiaoyaowan is an effective classic prescription for the treatment of liver stagnation and spleen deficiency syndrome, which is based on the dosage form of Xiaoyaosan in Prescriptions of the Bureau of Taiping People's Welfare Pharmacy. It has the effect of relieving depression, nourishing blood and invigorating spleen. In modern research, it has been found that Xiaoyaowan has good curative effect in the treatment of endocrine diseases, liver diseases, immune diseases, and neurological diseases, etc. It was praised by the famous medical scientist YE Tian-shi in the Qing Dynasty as "the holy medicine for women", with a wide range of significant curative effects gynecology. Progress has been also made in pharmacological research. In this article, we have searched and consulted the relevant literature reports of Xiaoyaowan in recent years, summarized the key directions of the pharmacological research literature, and proposed deficiencies to provide relevant basis for the in-depth study of Xiaoyao pill in the future.

ABSTRACT

OBJECTIVE: To establishment the Sense of Security Scale for Medical Staff and test its reliability and validity. METHODS: The pre-test version scale was formulated through methods of literature review, semi-structured interviews with clinical medical staff, discussion with members of research groups, and consultation with medical experts. A total of 350 and 403 medical staff who worked in a tertiary A hospital in Guangdong Province were selected as the prediction and the verification samples, respectively, by convenience sampling method. The reliability and validity of the final version scale were tested by item analysis, exploratory factor analysis and confirmatory factory analysis. RESULTS: Through item analysis, it was determined that the scale was mainly composed of 24 items. Exploratory factory analysis extracted five characteristics: environmental factors, patient factors, self factors, organizational management, and social support, that explained 72.8% of the total variation. The results of confirmatory factory analysis showed that the ratio of chi-square to degree of freedom was 2.851, the mean square error of approximation was 0.068, the standardized mean square residual was 0.078, the goodness of fit index was 0.883, the normative fit index was 0.904, the comparative fit index was 0.935, the non-standard fitting index was 0.924, and the incremental fitting index was 0.936. The results of reliability and validity test showed that the total Cronbach alpha coefficient, Spearman-Brown coefficient and retest reliability were 0.939, 0.967 and 0.808, respectively. The item reliability of each item was 0.420-0.814, the composite reliability was 0.775-0.920, the content validity ratio was 0.904, the convergent validity was 0.514-0.741, and the discriminative validity was 0.717-0.861. CONCLUSION: The reliability and validity of each dimension of the Sense of Security Scale for Medical Staff is good, and it can be used as an evaluation tool to measure the sense of security in medical staff.

BACKGROUND@#Twin pregnancies continue to increase worldwide; however, the current clinical prenatal evaluation for the intrauterine growth of twins still relies on the growth standards of singletons. We attempted to establish a set of fetal biometric references for Chinese twin pregnancies, stratified by chorionicity and conception mode as spontaneously conceived monochorionic diamniotic (SC-MCDA), spontaneously conceived dichorionic diamniotic (SC-DCDA), and assisted reproductive technology dichorionic diamniotic (ART-DCDA) twins.@*METHODS@#From 2016 to 2019, the ultrasonographic fetal biometric measurements were longitudinally collected in pregnant women, including fetal weight, biparietal diameter, head circumference, abdominal circumference, femur length, and humerus length. The linear mixed models were used to test the difference of growth patterns between groups, and the growth curve of each biometric parameter was modeled by a generalized additive model for location scale and shape.@*RESULTS@#A total of 929 twin pregnant women and 2019 singleton pregnant women, met the inclusion criteria. Among twin pregnancies, 148 were SC-MCDA, 215 were SC-DCDA, and 566 were ART-DCDA twins. Overall, SC-DCDA twins grew faster than SC-MCDA twins, while slower than ART-DCDA twins (all P < 0.05), and all of the three groups showed significant differences comparing with singletons, especially during the third trimester. Hence, the customized fetal growth charts of each fetal biometric parameter were, respectively, constructed for SC-MCDA, SC-DCDA, and ART-DCDA twins.@*CONCLUSIONS@#The fetal biometric trajectories demonstrated characteristic patterns according to chorionicity and conception mode. To fill the gap, we modeled fetal biometric parameters for Chinese SC-MCDA, SC-DCDA, and ART-DCDA twin pregnancies, hoping to provide a reference for the further establishment of fetal growth reference values for Chinese twin fetuses.

ABSTRACT

Fluid resuscitation is an important support for the early treatment of acute pancreatitis. However, the timing, volume and type of fluid, the goal of fluid resuscitation and other related issues remain unanswered. A dilemma on under- and over-resuscitation is often encountered in clinical practice. This paper summarized the recent advances on early fluid resuscitation in acute pancreatitis and proposed an early fluid resuscitation strategy of " three stages and two steps" for acute pancreatitis from a perspective of the relationship and the corresponding changes between Qi and Xue based on the Qi-Xue-Jin-Ye theory of traditional Chinese medicine by integrating the Chinese and Western medicine thoughts, which could provide some insights into optimizing the current strategy.

ABSTRACT

BACKGROUND@#Albuvirtide is a once-weekly injectable human immunodeficiency virus (HIV)-1 fusion inhibitor. We present interim data for a phase 3 trial assessing the safety and efficacy of albuvirtide plus lopinavir-ritonavir in HIV-1-infected adults already treated with antiretroviral drugs.@*METHODS@#We carried out a 48-week, randomized, controlled, open-label non-inferiority trial at 12 sites in China. Adults on the World Health Organization (WHO)-recommended first-line treatment for >6 months with a plasma viral load >1000 copies/mL were enrolled and randomly assigned (1:1) to receive albuvirtide (once weekly) plus ritonavir-boosted lopinavir (ABT group) or the WHO-recommended second-line treatment (NRTI group). The primary endpoint was the proportion of patients with a plasma viral load below 50 copies/mL at 48 weeks. Non-inferiority was prespecified with a margin of 12%.@*RESULTS@#At the time of analysis, week 24 data were available for 83 and 92 patients, and week 48 data were available for 46 and 50 patients in the albuvirtide and NRTI groups, respectively. At 48 weeks, 80.4% of patients in the ABT group and 66.0% of those in the NRTI group had HIV-1 RNA levels below 50 copies/mL, meeting the criteria for non-inferiority. For the per-protocol population, the superiority of albuvirtide over NRTI was demonstrated. The frequency of grade 3 to 4 adverse events was similar in the two groups; the most common adverse events were diarrhea, upper respiratory tract infections, and grade 3 to 4 increases in triglyceride concentration. Renal function was significantly more impaired at 12 weeks in the patients of the NRTI group who received tenofovir disoproxil fumarate than in those of the ABT group.@*CONCLUSIONS@#The TALENT study is the first phase 3 trial of an injectable long-acting HIV drug. Objective:The phase separation of Baihutang was carried out. The content of mangiferin,new mangiferin,calciumion,glycyrrhizin and ammonium glycyrrhizinate in the solution phase,the nano phase and the precipitated phase of Baihutang were measured,so as to define the effect of nanometer particles of Baihutang on the growth of active components,and explain the mechanisms of Baihutang in potent detoxification and heat removal. Method:The phase separation of Baihutang was performed by high-speed centrifugation and dialysis. The contents of mangiferin,new mangiferin,glycyrrhizin and ammonium glycyrrhizinate in Baihutang were determined by HPLC. Chromatographic column Diamonsil C18 (4.6 mm×250 mm,5 μm) was adopted, with acetonitrile-25 mmol·L-1 potassium dihydrogen phosphate solution as the mobile phase, and eluted in a gradient mode. The detection wavelength was 257 nm,the column temperature was 30 ℃,and the flow rate was 1.0 mL·min-1. EDTA-2Na solution was used to calibrate the calcium concentration in different phase states of Baihutang. Result:1 mL Baihutang nanoparticles contained 483.00 μg new mangiferin,1 068.88 μg mangiferin,219.93 μg glycyrrhizin and 187.10 μg ammonium glycyrrhizin,and the content of new mangiferin and mangiferin accounted for 89.4% and 89.9% respectively in 1 mL Baihutang. The new mangiferin and mangiferin in the nano phase were 230.0 and 23.3 times the true solution,and 8.5 and 14.4 times of the precipitation,respectively. The content of calcium ions in Baihutang in the nano phase was higher,accounting for 86.9% of Baihutang,and the content of calcium ions in Baihutang and Baihutang in the nano phase was higher than that in gypsum group. Conclusion:The content of main components in Baihutang in nanometer phase is significantly higher than that in other phases. The nanoparticles of Baihutang have a solubilizing effect on the main antipyretic components, such as mangiferin,mangiferin and calcium ions as well as the antitoxic components glycyrrhizic acid and glycyrrhizic acid. The mechanism of action of Baihutang is related to the formation of nanoparticles.

Objective:To explore the effect of Ermiaosan(EMS) on the polarization of M1 by lipopolysaccharide(LPS)+interferon(IFN)-γ and M2 induced by interleukin(IL)-4+IL-13 in rat bone marrow-derived macrophages. Method:Macrophages from rat bone marrow were extracted in vitro, stimulated by macrophage colony stimulating factor(M-CSF), induced to macrophages (marked by F4/80), stimulated by LPS+IFN-γ and induced to polarize to M1,while stimulated by IL-4+IL-13 and induced to polarize to M2. After adding different concentrations of EMS (0.2,0.4,0.8 g·L-1), the phenotypes of M1 and M2 were detected by immunofluorescence, and the effect of EMS on M1(marked by CD68 and iNOS)/M2(marked by CD206 and Arginase) polarization of macrophages from rat bone marrow was detected. Result:Compared with control group, LPS + IFN-γ could increase the polarization of M1 (P<0.01),while IL-4+IL-13 could increase the polarization of M2 (P<0.01); compared with LPS+IFN-γ/IL-4+IL-13 group, EMS (0.2,0.4,0.8 g·L-1) could inhibit the polarization of M1 induced by LPS+IFN -γ for 24 hours (P<0.05), but had no significant effect on polarization of M2 induced by IL-4+IL-13. Conclusion:EMS can inhibit M1 polarization induced by LPS+IFN - γ, but has no effect on M2 polarization induced by IL-4+IL-13.

ABSTRACT

Objective:To investigate the spectrum-effect relationship of effect of material components in Shaoyao Gancaotang on the levels of malondialdehyde (MDA), superoxide dismutase (SOD) and adenosine triphosphatase (ATPase) in brain tissue of epileptic mice, and to reveal the material basis of the antiepileptic effect of Shaoyao Gancaotang. Method:HPLC was used to establish the fingerprint of 15 batches of Shaoyao Gancaotang, the mobile phase was consisted of acetonitrile (A)-0.1% phosphoric acid aqueous solution (B) for gradient elution (0-15 min, 19%A; 15-45 min, 19%A-50%A; 45-46 min, 50%A-90%A; 46-48 min, 90%A), the flow rate was 1.0 mL·min-1, and the detection wavelength was 237 nm. The mouse epilepsy model was induced by penicillin, the protective effect of 15 batches of Shaoyao Gancaotang on MDA, SOD and ATPase levels in brain tissue of epileptic mice was investigated. Grey correlation analysis was used to analyze the correlation between the peak areas of characteristic peaks in the fingerprint of 15 batches of Shaoyao Gancaotang and three pharmacodynamic indexes (the activities of MDA, SOD and ATPase), and the mathematical model of spectrum-effect relationship between different material components and pharmacodynamic indexes was established. Result:Shaoyao Gancaotang could increase the contents of ATPase and SOD and reduce the content of MDA in the brain tissue of epileptic mice, and most of the differences were significant (P<0.05, P<0.01). There were 28 common peaks in the fingerprint of 15 batches of Shaoyao Gancaotang, and 15 of them were characteristic peaks. Grey correlation analysis found that the characteristic peaks contributed to the activities of MDA, SOD and ATPase included peak 2 (albiflorin), peak 3 (paeoniflorin), peak 5 (liquiritin), and so on, the specific ranking was peak 3>peak 6>peak 12>peak 8>peak 2>peak 5>peak 9>peak 4>peak 10>peak 7>peak 13>peak 11>peak 15>peak 1>peak 14. Conclusion:Shaoyao Gancaotang can affect the activities of SOD, MDA and ATPase in brain tissue of epileptic mice by multi-component synergy. Simultaneously, introducing the grey correlation analysis into the correlation evaluation of the spectrum-effect relationship between components and efficacy of Chinese herbal compounds, it can objectively reflect the essence of the synergistic action of multiple components in traditional Chinese medicine (TCM), and it is an effective analysis method for screening and predicting the pharmacodynamic components of TCM.

Objective::To study the effects of Ermiaosan on migration, adhesion and invasion of human fibroblast-liked synovial cells(FLS) and explore its mechanism. Method::Using the human FLS as the research object, the nontoxic concentration of FLS.FLS was determined by methyl thiazolyl tetrazolium (MTT) colorimetric assay for the follow-up experiment. The transwell migration, adhesion and transwell invasion test were used to detect the migration and adhesion of the different concentration of Ermiaosan on FLS, respectively. The expression of interleukin (IL)-1 beta of FLS supernatant was detected by enzyme linked immunosorbent assay (ELISA). Protein in FLS was extracted and protein expression levels of phosphorylated Janus kinase 1 (p-JAK1), p-signal transducer and activator of transcription (STAT1) and p-STAT6 were detected by Western blot. Result::Compared with control group, tumor necrosis factor(TNF)-α (20 μg·L-1) increased the proliferation, migration, adhesion, invasion and the secretion of IL-1β of FLS (P<0.01). Ermiaosan(0.2, 0.4, 0.8 mg·L-1) had no significant effect on the proliferation of FLS induced by TNF-α for 24 h. Within 24 h, the migration, adhesion, invasion, invasion, and secretion of IL-1β of FLS cells induced by TNF-α were also decreased significantly(P<0.05, P<0.01). Compared with the blank group, TNF-α could induce abnormal elevation of p-JAK1, p-STAT1 and p-STAT6 in FLS (P<0.01), while Ermiaosan of 0.2, 0.4, 0.8 g·L-1 could significantly reduce the expression levels of p-JAK1, p-STAT1 and p-STAT6 (P<0.05, P<0.01). Conclusion::Ermiaosan can inhibit the migration, adhesion and invasion of FLS, and its mechanism may be related to the inhibition of the secretion of IL-1β, the mechanism may be related to JAK/STAT pathway.

Objective:To investigate the mechanism of Sinisan in treating type 2 diabetes mellitus(T2DM) based on network pharmacology. Method:Based on electronic databases, active ingredients of Sinisan and target genes of ingredients as well as type 2 diabetes were screened out. Cytoscape software was applied to construct "herb-active ingredient-target-disease" interaction network diagram,respectively. The common genes of ingredients and disease were uploaded to the STRING database, and the protein interaction network map (PPI) was constructed. CytoNCA and Cluster Marker were used to analyze PPI,respectively. Finally,ClueGO was used to analyze gene ontology (GO) enrichment and Kyoto Encyclopedia of genes and genomes (KEGG) pathway enrichment. Result:A total of 137 active components and 110 effective genes of Sinisan in the treatment of type 2 diabetes were screened out. Based on PPI analysis,these effective genes were divided into 4 different clusters, and 12 genes were considered as the most effective targets. GO enrichment analysis showed that cluster 1 mainly involved acute inflammatory response and acetylcholine receptor signaling pathway, cluster 2 mainly involved cellular response to steroid hormone stimulu and xenobiotic stimulus,and cluster 3 was mainly engaged in the metabolism process of protein,fatty and glucose and the response to hypoxia. KEGG analysis was highly correlated with advanced glycation end products-receptor for AGE (AGE-RAGE) pathway,apoptosis pathway,inflammatory pathway,cell cycles and various metabolic pathways. Conclusion:Sinisan has a regulatory effect in pathogenesis and prognosis of T2DM,such as inflammation,cell apoptosis and nutrition metabolism. Moreover,its potential mechanisms on diabetic angiopathies,diabetic cognitive impairment and cancers were reveal,so as to define a direction for the fundamental research of Sinisan.

Breast cancer is currently among the most frequently occurring malignant tumors in female patients, with more than one million female patients suffering from breast cancer worldwide each year. Postoperative radiotherapy for breast cancer treatment is an important treatment option for patients with early breast cancer, which could significantly reduce the mortality and recurrence rate of breast cancer patients. Intramammary lymph node irradiation after radical mastectomy could improve the survival of patients with early breast cancer is currently be of controversy. This study mainly reviews the research progress of internal mammary lymph node radiotherapy, including the techniques, dose selection, survival, and adverse reaction-related reports.

ABSTRACT

Traditional Chinese medicine is the product of clinical medication practice of the Chinese nation for thousands of years. Its material basis is the key to reveal the essence of the roles of traditional Chinese medicine, and the fundamental guarantee to solve the difficulties in the quality control of traditional Chinese medicine. However, the material basis of traditional Chinese medicine is to exert the overall pharmacodynamic effect through multi-targets, multi-approaches and mutual cooperation, resulting in unclear quality control index. In recent years, the quality control standards of traditional Chinese medicine have experienced great changes by shifting the focus from the appearance characteristics to the internal material basis, which however is limited to the control of a single com-ponent or multiple components. In other words, the intrinsic effectiveness and safety could not be guaranteed without the characteristics of the integrity of traditional Chinese medicine. With Moutan Cortex as an example, this paper analyzed the evolution of Moutan Cortex quality standards based on Chinese Pharmacopoeia, and comprehensively summarized the material basis of Moutan Cortex. Based on the theory of "component structure", this study analyzed current quality control of the material basis of Moutan Cortex and its preparations, and expounded the development trend of multi-dimensional quality control, so as to lay a foundation for establishing a more rational quality control system for traditional Chinese medicine in the future.

ABSTRACT

Guided by the theory of "component structure", we analyzed the structural characteristics of pharmacodynamical components in genuine Moutan Cortex. The compositions of organic small molecules were determined by high performance liquid chromatography(HPLC) for 20 batches of genuine Moutan Cortex and 12 batches of non-genuine Moutan Cortex. By means of similarity analysis, clustering analysis(CA), principal component analysis(PCA) and orthogonal partial least-squares discriminant analysis(OPLS-DA), the elements in structural characteristics of the pharmacodynamical components were extracted as follows: terpene glycosides components(oxidized paeoniflorin, paeoniflorin,galloyl paeoniflorin, benzoyloxy paeoniflorinand benzoyl paeoniflorin), tannin components(1,3,6-tri-O-galloside acyl glucose, pentagalloyl glucose), and phenolic acid components(methyl gallate, paeonol). The contents and quantity ratios of terpene glycoside component, tannin component and phenolic acid components in genuine Moutan Cortex were determined as 14.1, 12.5, 21.7 mg·g~(-1), 1.00∶0.89∶1.54. The contents and quantity ratios of the oxidized paeoniflorin, paeoniflorin and benzoylpaeoniflorin in the terpene glycoside components were characteristic and determined as 2.05, 7.05, 3.30 mg·g~(-1), 1.00∶3.44∶1.61. The unique structural characteristics of genuine Moutan Cortex provide scientific basis for the formulation of quality standards.

ABSTRACT

Based on the theory of "component structure", the component structure characteristics of 17 inorganic elements in 20 batches of genuine Moutan Cortex and 12 batches of non-genuine Moutan Cortex were analyzed. The analytical method of inductively coupled plasma mass spectrometry(ICP-MS) for inorganic elements in Moutan Cortex was established, and the fingerprint of inorganic elements was drawn. Through correlation analysis and principal component analysis, the constituent elements of inorganic elements in Moutan Cortex were excavated, namely characteristic elements As, Cr, Pb, Se, K, Cu and Cd. The amount and the quantity relative ratio between the components of genuine and non-genuine Moutan Cortex samples were analyzed. It was found that the amount of characteristic elements in the bulk genuine Moutan Cortex samples was 0.33, 1.42, 1.33, 0.11, 4 385.78, 4.87, 0.49 mg·kg~(-1), with the quantity relative ratio of 1.00∶4.30∶4.03∶0.33∶13 290.24∶14.76∶1.48. The amount of characteristic elements in sub-packaged genuine Moutan Cortex was 0.24, 1.96, 0.36, 0.05, 5 122.01, 4.81, 0.05 mg·kg~(-1), with the quantity relative ratio of 1.00∶8.17∶1.50∶0.21∶21 341.71∶20.04∶0.21. The unique structural characteristics of inorganic elements in genuine Moutan Cortex are studied to provide a basis for the quality control and safety evaluation of Moutan Cortex.

ABSTRACT

In this paper, we aim to control and evaluate the quality of Liuwei Dihuang Concentrated Pill by using the model of fingerprint technique and "component structure" theory. Agilent 5 TC-C_(18) column(4.6 mm×250 mm,5 μm) was used, with 0.1% formic acid solution-acetonitrile as mobile phase at a flow rate of 1.0 mL·min~(-1). The column temperature was 30 ℃, with detection wavelength of 242 nm and the sample volume of 10 μL. The characteristic fingerprint of Liuwei Dihuang Concentrated Pill was established by high performance liquid chromatography(HPLC) for its quality control. Seventeen common peaks were identified, and the similarity was 0.550-0.997 in 29 batches of samples, indicating that the quality difference among batches of Liuwei Dihuang Concentrated Pills was significant. The structural characteristics of the Moutan Cortex components in Liuwei Dihuang Concentrated Pills were characterized. On this basis, combined with the structural characteristics of genuine components of Moutan Cortex, the structural characteristics of components in Liuwei Dihuang Concentrated Pills were further analyzed. The results showed that there were significant differences in the contents and quantity ratios of 9 representative components(components) of Moutan Cortex in Liuwei Dihuang Concentrated Pills from different manufacturers, indicating internal quality differences among different batches of products. The fingerprint technique and the "component structure" theory established by the above research provide an analytical method and a research foundation for the quality evaluation of Liuwei Dihuang Concentrated Pills.

ABSTRACT

To evaluate the quality of Moutan Cortex Formula Granules from different manufacturers based on the structural characteristics of the genuine components of Moutan Cortex. High performance liquid chromatography(HPLC) fingerprint analysis method of Moutan Cortex Formula Granules was established to characterize the elements of the structural characteristics for genuine Moutan Cortex. Nineteen common peaks were determined and the similarity between the Moutan Cortex Formula Granules of each batch and the control fingerprint was 0.856-0.981. The results showed that there were differences in the internal quality of Moutan Cortex Formula Granules from three manufacturers. The contents of components as well as inter-component and intra-component quantity ratio for Moutan Cortex Formula Granules from different manufacturers were analyzed. It was found that the inter-component quantity ratio of G1 and G2 produced by manufacturer A was close to that of G4 produced by manufacturer B; the inter-component quantity ratio of G3 from manufacturer B was close to that of G7 from manufacturer C; and the inter-component ratios of samples G5, G6, G8-G11 produced by manufacturer C were closer to each other. The results of the study guided by the theory of "component structure" provide a new analytical method and basis for the quality evaluation of Moutan Cortex Formula Granules.

ABSTRACT

Objective To investigate the application effect of metoprolol in the treatment of atrial fibrillation complicated with chronic heart failure (CHF).Methods From February 2015 to February 2019,398 elderly patients with atrial fibrillation and CHF in Wangjiangshan Branch of the People's Hospital of Zhejiang Province were selected in the study.The patients were divided into two groups according to the digital table ,with 199 case in each group.The control group was treated with rest ,digitalis inhalation,diuretics,angiotensin converting enzyme inhibitors ,nitrate and enteric-coated aspirin.The observation group was treated with metoprolol on this basis.The effective rate of treatment, heart rate changes before and after treatment , left ventricular ejection fraction and 6 -minute walking distance in the two groups were observed and analyzed.Results The effective rate of the observation group was 88.94%(177/199),which was significantly higher than 71.86%(143/199) in the control group,the difference was statistically significant (χ2 =18.433,P=0.000).The highest heart rate,the lowest heart rate and the resting heart rate in the observation group after treatment were significantly better than those in the control group [( 115.12 ± 0.35)times/min vs.(122.12 ±16.22) times/min,(90.21 ±18.12) times/min vs.(94.12 ±11.11) times/min, (97.00 ±9.64)times/min vs.(102.03 ±11.20)times/min],the differences were statistically significant (t=4.246, 4.783,4.662,all P<0.05).The LVEF and 6 -minute walking distance of the observation group after treatment were significantly better than those of the control group [( 45.0 ±2.0 )% vs.( 41.0 ±3.0 )%, ( 340.09 ± 31.45)m vs.(302.43 ±29.45)m],the differences were statistically significant ( t=4.246,7.662,all P<0.05). Conclusion Digitalis combined with metoprolol can effectively control the heart rate of atrial fibrillation combined with CHF,improve ventricular function and increase cardiac endurance.The combined therapy is worthy of promoting in this disease.

Objective@#To investigate the effects of long-term exposure to silica dust on serum CC16 and KL-6 levels.@*Methods@#The patients with stage I silicosis who were hospitalized in our hospital from April 2016 to April 2017 were treated as silicosis group. The silica dust exposed workers without silicosis who were taken the physical examination in our hospital were taken as a dust-exposed group. The healthy control group comes from in the same period of community physical examination did not touch the dust. The levels of CC16 and KL-6 in serum of all subjects were determined by enzyme-linked immunosorbent assay (ELISA) , and the levels of CC16 and KL-6 in serum were compared in three groups.@*Results@#Compared with the control group, the serum levels of CC16 in the silicosis group (P<0.01) and the dust-exposed group (P<0.01) were significantly lower. Compared with the control group, the level of serum KL-6 in the silicosis group was significantly decreased (P<0.01) compared with the control group, while the level of KL-6 in the serum of the dust-exposed group was significantly increased (P<0.01) . The ROC area of CC16 for diagnosis of silicosis was 0.92 (P<0.01) , with a sensitivity of 81.37%, specificity of 92.63% and Kappa value of 0.74.@*Conclusion@#Long-term exposure to silica dust may lead to a decrease in serum CC16 levels. Reduced serum CC16 levels may be useful in identifying the diagnosis of silicosis.
