ABSTRACT

Chlamydia, a gram-negative obligate intracellular pathogen, is a major cause of human reproductive tract, eye and respiratory tract infections. It replicates in a special membrane-binding chamber called inclusion and survives in the host′s hostile intracellular environment through secreting effectors, but requires host-derived lipids to grow and develop in the cells. Emerging evidences suggest that Chlamydia has evolved a variety of strategies to meet its lipid needs by interacting with host cell compartments and redirecting the transport pathway to its intracellular niche. This paper briefly described the pathway for obtaining host lipids and the mechanism of lipid metabolic during Chlamydia infection.

ABSTRACT

Attention is an essential cognitive function which was use to perceive the external world, and it is the basis of all cognitive activities.Other cognitive functions such as working memory, executive function, information processing speed and so on, are also adversely affected in hypoprosexia or hyperfocusing state.Patients with schizophrenia have severe attention disorder, which makes patients unable to successfully complete the task of work or study, thus reduce the quality of life.Attention disorder exists throughout the whole course of this disease, and there is no specific drug for it.In recent years, antipsychotic medications have widespread application, but it has not shown up significantly improvement of attention disorder, worse still, its side effects will exacerbate the disease, which results in the impairment of the patients' ability to learn and explore the novelties.Neuroimaging markers of attention disorder in schizophrenia are the focus of current researches.White matter fibers are important pathways that connect attention networks and maintain the three-dimensional structure of the brain.Previous studies have suggested that attention disorder may be related to abnormalities of white matter fibers connecting various encephalic regions.In particular, abnormalities in the integrity of white matter, such as corpus callosum, cingulum bundle, superior longitudinal fasciculus, inferior fronto-occipital fasciculus, have been reported to be significantly associated with attention disorder in patients with schizophrenia.In this study, the relationship between attention disorder and white matter structure in patients with schizophrenia is reviewed through diffusion tensor imaging (DTI) technique.

Our aim is to find out the consistent abnormal brain regions of the patients with schizophrenia by using regional homogeneity (ReHo), and indirectly understand the degree of brain damage of the patients with drug-naive first episode schizophrenia (Dn-FES) and chronic schizophrenia. @*Methods@#We performed the experiment by activation likelihood estimation (ALE) software to analysis the differences between people with schizophrenia group (all schizophrenia group and chronic schizophrenia group) and healthy controls. @*Results@#Thirteen functional imaging studies were included in quantitative meta-analysis. All schizophrenia group showed decreased ReHo in bilateral precentral gyrus (PreCG) and left middle occipital gyrus (MOG), and increased ReHo in bilateral superior frontal gyrus (SFG) and right insula. Chronic schizophrenia group showed decreased ReHo in bilateral MOG, right fusiform gyrus, left PreCG, left cerebellum, right precuneus, left medial frontal gyrus and left anterior cingulate cortex (ACC). No significant increased brain areas were found in patients with chronic schizophrenia. @*Conclusion@#Our findings suggest that patients with chronic schizophrenia have more extensive brain damage than FES, which may contribute to our understanding of the progressive pathophysiology of schizophrenia.

ABSTRACT

Cutaneous metastasis of renal cell carcinoma (RCC) is rare. Particularly, it is even more rare that multiple subcutaneous nodules become the first symptom of RCC. On January 19, 2018, a RCC patient with first symptom of multiple subcutaneous nodules was admitted to Wuxi No.2 People’s Hospital. The patient's right renal cancer stage was cT 4N 2M 1, stage Ⅳ. He has no surgical indication and medical treatment was the main strategy. Sunitinib was administered orally five days after the biopsy. He died of multiple brain metastases seven months after the operation. The overall survival was 7 months.

ABSTRACT

Objective:To compare CT with MRI on the accuracy in TNM staging of pancreatic ductal adenocarcinoma (PDAC) based on the 8th American Joint Committee on Cancer System and Pathological Staging.Methods:From October 2013 to October 2019, 105 patients who had pathologically confirmed PDAC treated at the Affiliated Wuxi No.2 People’s Hospital of Nanjing Medical University were retrospectively studied. Of 52 patients who met the inclusion criteria, there were 32 males and 20 females, with age ranging from 45 to 84 years (mean 66.1 years). Based on the 8th American Joint Committee on Cancer System and Pathological Staging, CT was compared with MRI in their accuracies in categorizing TNM and staging of PDAC.Results:Forty-three of 52 patients who underwent curative resection had a mean tumor size of (3.4±1.2) cm, compared with tumor sizes of (3.3±1.2) cm and (3.3±1.4) cm on CT and MRI, respectively. No significant differences were observed between gross pathological examination and CT ( P>0.05) or MRI ( P>0.05). The accuracy of T, N, and M categories on CT versus (vs) MRI was 97.7% (42/43) vs 97.7% (42/43), 79.1% (34/43) vs 76.7% (33/43), and 100% (9/9) vs 88.9% (8/9), respectively. No significant differences were observed between the two modalities ( P>0.05). The overall accuracy of PDAC staging on CT vs MRI was 82.7% (43/52) vs 76.9% (40/52), respectively. There was also no significant difference between the two modalities ( P>0.05). Conclusion:Both CT and MRI had similar accuracies in categorizing TNM and staging of PDAC. However, the accuracies of T and M stages were higher than that of the N stage for these two imaging modalities.

ABSTRACT

The mucosal surfaces of the human body are typically colonized by polymicrobial communities, they exist in the gastrointestinal tract, skin, oral cavity, and genitourinary tract, as well as in anatomical sites previously considered sterile, including the lung. These communities interact with the mucosal immune system to maintain homeostasis in health, emphasizing the important role of " second genome" , which is very important to human health. The human lower respiratory tract also harbor many microbes, the imbalance of microbiome composition maybe critically related with many lung disease. Up to date, researches on microbiome of lung caner is still on the first step. Our review mainly describes the characteristic composition of lung microbiome, and the relationship with diseases, especially with lung cancer. We attempts to provide the basis information for further researches about the relationship between human lung microbiome and lung cancer.

ABSTRACT

OBJECTIVE:To compare the che mical composition differences of raw and fried processed Glycyrrhiza uralensis , and provide reference for clarifying the regularity of chemical composition change before and after processing and material basis of differential use of raw and fried G. uralensis . METHODS :UPLC-Q-TOF-MS technology was used to detect the composition of raw and fried G. uralensis . By comparing the retention time ,relative molecular weight and other information of the compounds with the databases such as METLIN ,Agilent MassHunter PCDL Manager off-line component identification workstation and references ,the chemical compositions of the compounds were preliminarily identified. Principal component analysis (PCA)method was used to observe the overall distribution trend of samples. Orthogonal partial least square (OPLS-DA)method was used to screen differential compounds [VIP >1.0 and | P(corr)|≥0.5 as criteria] and analyze the content changes of differential compounds. RESULTS :A total of 31 common compounds were preliminarily identified from the raw and fried G. uralensis . PCA analysis showed that raw and fried G. uralensis could be separated well. OPLS-DA analysis result showed that 15 characteristic differential compounds were screened out from raw and fried G. uralensis ,including 13 flavonoids and 2 coumarins;the contents of 8 flavonoids compounds such as licoflavone A ,glycyrrhizin and glabridin and so on in G. uralensis were significantly increased (P<0.05 or P<0.01), while the contents of 5 flavonoids components such as glycyrrhizinol ,glycyrin A and 2 coumarins components such as 2′-4′-trihydroxy-5-methoxy-3-coumarim and hedysarimcoumestan B were significantly decreased (P<0.05) after honey-fried processing. CONCLUSIONS :Before and after processing ,there are obvious differences between some flavonoids and coumarins , which may be the main material basis for differential use of raw and honey-fried G. uralensis .

ABSTRACT

OBJECTIVE:To optimi ze the ratio of four comp onents of Compound renshen jianti formulation (Panax ginseng , Dioscorea oppositifolia ,Lycium barbarum fruit,Alpinia oxyphylla ),and to investigate its anti-fatigue activity and acute toxicity. METHODS:The water extract of Compound renshen jianti formulation was prepared by water extraction ,concentration and decompression drying. By single factor tests ,using weight-bearing swimming time as index ,the effects of four factors were investigated,such as the amount of P. ginseng ,D. oppositifolia ,L. barbarum fruit,A. oxyphylla . On the basis of single factor tests,using comprehensive score of weight-bearing swimming time ,serum urea nitrogen content ,liver glycogen content and AUC of blood lactate after exercise as index ,the formulation was optimized by Box-Behnken response surface method. The mice was divided into blank control group (water),positive control group (Renshen hongjingtian capsules ,0.135 g/kg)and compound low-dose,medium-dose and high-dose groups [the optimal ratio of Compound renshen jianti formulation extract (called“optimal compound formulation ”for short )4.08,8.16,12.24 g/kg,by crude drug] ,intragastric administration of drug or distilled water 20 mL/kg,once a day ,for consecutive 30 d. The weight-bearing swimming time ,the contents of serum urea nitrogen ,liver glycogen and blood lactate AUC after exercise were used to optimize its anti-fatigue activity of optimal compound formulation. The comprehensive score was calculated based on the measured data of mice in the compound formulation middle-dose group , and the difference between it and the theoretical prediction value was compared. The mice were given optimal compound formulation intragastrically (total dose 16.00 g/kg, by extract). The general state , body mass change , toxic characteristics and death of mice were observed and recorded for 14 days. Median lethal dose (LD50)and maximum tolerated dose (MTD)were measured. RESULTS :The optimal formulation ratio of Compound renshen jianti formulation included that P. ginseng 1.5 g,D. oppositifolia 10 g,L. barbarum fruit 10 g,A. oxyphylla 3 g. Results of anti-fatigue activity validation test showed that the optimal compound formulation could significantly prolonged weight-bearing swimming time ,reduced serum content of urea nitrogen ,blood lactate content and its AUC (except for low-dose group ),while significantly increased the content of liver glycogen (P<0.05 or P<0.01). Average comprehensive score of medium-dose group was 96.95,which was only 0.06% different from the theoretical prediction value of 97.01. The results of acute toxicity test showed that there was no death in mice. The oral MTD of the optimal compound formulation was more than 15 g/kg,which was non-toxic. CONCLUSIONS :The optimal Compound renshen jianti formulation has effective anti-fatigue activity of mice ,and has no significant toxic effect.

OBJECTIVE@#To evaluate the effect of standardized health education on the sputum specimen collection rate for nucleic acid detection of coronavirus disease 2019 (COVID-19).@*METHODS@#Two hundred and twenty-seven patients in fever clinics and isolation wards of Sir Run Run Shaw Hospital of Zhejiang University and 307 migrant workers returning to 5 enterprises in Shanghai from February 3 to March 14, 2020 were enrolled in the study. Through clarifying the procedures of collecting sputum specimens, making graphic/video health education materials, standardizing the contents and methods of health education, we conducted education to the subjects. The subject expectorated spontaneously or with medical assistance. For patients, the number of sampling attempts and sputum acquisition times were documented before and after the implementation of the standardized expectoration method; for the returning migrant employees in the enterprises, only the number of collected samples after the implementation of the standardized expectoration method were recorded.@*RESULTS@#A total of 378 sputum samples were collected from 227 patients. The sputum sampling rates before and after the implementation of health education were 40.9%and 58.4%, respectively (@*CONCLUSIONS@#The education for standardized sputum sample collection method can effectively increase the sputum collection rate.

ABSTRACT

OBJECTIVE:To optimi ze the optimal composition proportion of 4 ingredients (Panax ginseng ,Astragalus membranaceus,Polygonatum sibiricum ,Lycium chinensis )in Compound ginseng immune-enhancing formula (CGIF),and to study immune activity and acute toxicity of the extracts with the optimal ratio. METHODS :The cell activity test was used to screen the crude drug concentration range of 4 ingredients. After treated with different crude drug concentrations of each medicinal material,using the contents of NO ,IL-6 and TNF-α as indexes,uniform design was used to determine the optimal ratio of each ingredient in CGIF. Totally 240 mice were taken and randomly divided into 4 batches,with 60 mice in each batch. Each batch of mice was randomly divided into blank group (normal saline ),model group (normal saline ),positive drug group [levamisole ,4 mg/(kg·d)],and optimal proportion extract of CGIF low-dose ,medium-dose and high-dose groups [ 0.952 8,1.905 6,3.811 2 g/(kg·d)],with 10 mice in each group ;they were given medicine intragastrically ,qd,for consecutive 30 d. Except for blank group,mice in the other groups were intraperitoneally injected with cyclophosphamide [ 40 mg/(kg·d)] on the 24th day after first administration,qd,for consecutive 3 d to induce immunocompromised model. The immune activity of the optimal proportion extract was evaluated by determining visceral coefficients ,spleen lymphocyte transformation capacity ,serum contents of hemolysin,IL-2,IgM,IgG and IgA ,phagocytosis function of peritoneal macrophages. Another 20 mice were collected and given the optimal proportion extract 20 mL/kg intragastrically ,twice;acute toxicity of the formula was investigated with oral maximum tolerated dose (MTD). RESULTS :The optimal ratio of CGIF was that crude drug mass ratio of P. ginseng , membranaceus,P. sibiricum ,L. chinensis was 1 ∶ 2 ∶ 2 ∶ 4. The immunological activity experiment showed that theoptimal proportion extract can significantly improve visceral indexes of mice , spleen lymphocyte proliferation ability serum contents of hemolysin ,IL-2,IgM,IgG and IgA as well as macrophage phagocy tosis ability (P<0.05 or P< 0.01). The acute toxicity test indicated that oral MTD was over 15 g/kg,which was non-toxic. CONCLUSIONS :The optimal proportion extract of CGIF can significantly enhance the immune function of mice and are non-toxic.

ABSTRACT

Objective To explore the effects of curcumin on pro-inflammatory factors in the lung microvascular endothelial cells (LMVEC) model stimulated by thrombus. Methods The LMVECs were divided into six groups according to the random number table method. No treatment was given to the blank control group ; the model group was cultured for 7 hours in normal medium; the curcumin group was treated with 40 μmol/L curcumin for 72 hours ; the shRNA group was infected with shRNA adenovirus for 72 hours; the irregular chemokines (CX3CL1) overexpression group was infected with CX3CL1 overexpressing adenovirus for 72 hours; the shRNA+curcumin group infected with shRNA adenovirus and treated with 40 μmol/L curcumin together for 72 hours; CX3CL1 overexpression +curcumin group infected with CX3CL1 overexpressing adenovirus and treated with 40 μmol/L curcumin together for 72 hours. After each group was given the corresponding pretreatment, the thrombus natural precipitation was added each group for 12 hours. The contents of interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α), the mRNA expression levels of CX3CL1, CX3CL1 receptor (CX3CR1), IL-6, TNF-α and the protein expression levels of CX3CL1/CX3CR1, CX3CR1/NF-κB in various groups were observed, repeat 3 times in each group. Results The contents and mRNA expression of IL-6, TNF-αand protein expression of CX3CR1, NF-κB in the LMVEC group were significantly higher than those in blank control group [IL-6 (ng/L): 207.90±16.69 vs. 85.93±20.32, TNF-α (ng/L): 239.60±15.27 vs. 101.23±11.92; IL-6 mRNA: 0.66±0.05 vs. 0.11±0.02, TNF-α mRNA: 1.06±0.04 vs. 0.02±0.01; CX3CR1 protein:3.94±0.58 vs. 1.00±0.31, NF-κB protein: 1.20±0.07 vs. 1.00±0.10; all P < 0.05]; the contents of IL-6 in shRNA group, CX3CL1 overexpression group, shRNA + curcumin group, CX3CL1 overexpression + curcumin group were all obviously lower than those in LMVEC group (ng/L: 183.60±11.52, 159.27±15.02, 117.03±7.91, 119.97±11.43 vs. 207.90±16.69, all P < 0.01); the content of TNF-α was markedly increased in shRNA group compared with that of LMVEC group (ng/L: 282.00±5.63 vs. 239.6±15.27), while the contents of TNF-α in CX3CL1 overexpression group, shRNA+ curcumin group, CX3CL1 overexpression + curcumin group were all lower than those in LMVEC group (ng/L: 216.97±9.20, 203.97±19.03, 191.97±17.50 vs. 239.6±15.27, all P < 0.05). The mRNA expression levels in CX3CL1 overexpression group and CX3CL1 overexpression + curcumin group were significantly higher than those in the blank control group and the LMVEC group (CX3CL1 mRNA: 55 210.3±1 209.2, 165 296.3±8 082.4 vs. 3.3±0.6, 2.0±0.0, all P < 0.01). The mRNA expression level of IL-6 in shRNA group was higher than that in LMVEC group (0.82±0.17 vs. 0.66±0.05), the mRNA expression level of IL-6 in CX3CL1 overexpression was lower than that in LMVEC group (0.29±0.03 vs. 0.66±0.05), the changes after pretreatment with curcumin were more significant (1.06±0.03 vs. 0.66±0.05 and 0.15±0.01 vs. 0.66±0.05); the mRNA expressions of TNF-α in shRNA group, CX3CL1 overexpression group, shRNA+ curcumin group were significantly lower than those in LMVEC group (0.41±0.04, 0.88±0.07, 1.01±0.02 vs. 1.06±0.04), the mRNA expression level of TNF-α in CX3CL1 overexpression + curcumin group was significantly higher than that in LMVEC group (1.36±0.01 vs. 1.06±0.04). The protein expression of CX3CL1, CX3CR1, NF-κB in shRNA group, CX3CL1 overexpression group, shRNA + curcumin group, CX3CL1 overexpressing + curcumin group were significantly higher than those in the LMVEC group (CX3CL1 protein: 0.41±0.07, 0.59±0.09, 0.69±0.61, 1.02±0.23 vs. 1.33±0.33, CX3CR1 protein: 0.85±0.18, 1.10±0.16, 1.32±0.18, 1.54±0.08 vs. 3.94±0.58, NF-κB protein: 0.33±0.07, 0.41±0.08, 0.41±0.07, 0.63±0.08 vs. 1.20±0.07). Conclusion Curcumin can inhibit the secretion of IL-6, TNF-α, CX3CR1 and NF-κB in thrombus-stimulated LMVEC model.

ABSTRACT

Objective To investigate the expression of DDX46 protein in colorectal cancer and its correlation with expressions of C-erbB-2, Ki-67, p53 and nm23 proteins. Methods A total of 149 cases of colorectal cancer tissues and 45 adjacent mucosa tissues after operation in the Wuxi No.2 Hospital Affiliated to Nanjing Medical University from January 2010 to December 2012 were collected. The expressions of DDX46 protein and C-erbB-2, Ki-67, p53 and nm23 proteins in tissues of colorectal cancer were detected by using immunohistochemistry. The relations between the expression level of DDX46 protein and the clinicopathological features including the age, gender, tumor differentiation, tumor infiltration, lymph node metastasis, TNM stage were analyzed, and the relations between DDX46 protein expression and expressions of C-erbB-2, Ki-67, p53 and nm23 proteins were also analyzed. Results Of 149 colorectal cancer tissues, 50 (33.6%) showed low expressions of DDX46 protein and 99 (66.4%) showed high expressions. Of 45 adjacent mucosa tissues, 37 (82.2%) showed low expressions of DDX46 protein, and 8 (17.8%) showed high expressions. There were significant differences between the high expression rate of DDX46 protein in cancer tissues and adjacent mucosa tissues (χ2=33.09, P<0.01). There were statistical differences in the expression level of DDX46 protein and tumor differentiation, tumor infiltration, lymph node metastasis, TNM staging (all P<0.05). The ratio of high-expressed DDX46 protein was increased with the low differentiation, high invasion degree, lymph node metastasis and late TNM staging. DDX46 protein expression was correlated with expressions of Ki-67, p53 and nm23 proteins (all P< 0.05), and the expression of DDX46 protein had a positive correlation with expressions of Ki-67 and p53 proteins (r values were 0.161 and 0.347), and a negative correlation with expression of nm23 protein (r= -0.561). Conclusions The high expression of DDX46 protein in colorectal cancer tissues is related with malignant biological behavior of the tumors and the key oncogenes. DDX46 protein could be regarded as the potential marker for the diagnosis and molecular targeting therapy of colorectal cancer.

Objective To investigate the role of COOH-terminus tensin like molecule (CTEN) in epithelial-mesenchymal transformation of gastric cancer and its prognostic factors.Methods The expressions of CTEN,E-cadherin and Vimentin in 220 patients with gastric cancer were detected by immunohistochemical staining.Results The positive expression rate of CTEN was 61.8% (136/220) in gastric cancer specimens,higher than 15% (3/20) in adjacent tissues (x2 =16.488,P <0.01);the positive expression rate of E-cadherin in gastric cancer specimens was 25.5% (56/220),lower than 85% (17/20) in adjacent tissues (x2 =30.713,P <0.01);and the positive expression rate of Vimentin in gastric cancer specimens was 35.0% (77/220),higher than 10% (2/20) in adjacent tissues.There was no significant difference in the expression of CTEN between different genders,ages,size of tumors and histopathological types (P > 0.05).In different infiltration depth,the CTEN expression difference were statistically significant (x2 =54.058,P <0.01),CTEN expression rate in lymph node metastasis group was obviously higher than without lymph node metastasis group (x2 =15.989,P <0.01),and CTEN expression rate in distant metastasis group was obviously higher than without distant metastasis group (x2 =4.143,P <0.01).CTEN and E-cadherin expression in gastric cancer tissue showed a negative correlation (r =0.357,P < 0.01),while CTEN and Vimentin expression were positively correlated (r =0.498,P <0.01).220 cases of patients with gastric cancer received follow-up.Median survival time of CTEN negative express group was 48.2 months (95% confidence interal,44.0-52.5 months),69.0% for the 5-year cumulative survival rate.The survival analysis showed median survival time of CTEN positive expression group was 41.8 months (95% confidence interal,38.3-45.3 months),48.5 % for the 5-year cumulative survival rate,which were lower than that in negative group (x2 =4.884,P < 0.05).Multifactor analysis showed CTEN expression,lymph node metastasis and distant metastasis were independent risk factors for the prognosis of gastric cancer patients (P < 0.05).Conclusions CTEN was positively correlated with gastric cancer invasion and metastasis,and played an important role in gastric cancer epithelial mesenchymal transformation.CTEN is independent risk factor for prognosis of gastric cancer,detection of CTEN gene mutation has important significance to evaluate the treatment and prognosis of gastric cancer.

ABSTRACT

Objective To compare the effects of ticagrelor and clopidogrel on platelet aggregation rate, myocardial perfusion and prognosis in patients with acute myocardial infarction. Methods One hundred and sixty-nine patients with acute ST segment elevation myocardial infarction (STEMI) and emergency percutaneous coronary intervention (PCI) were recruited and randomly divided into ticagrelor group (85 cases) and clopidogrel group (84 cases). The TIMI blood flow before and after PCI was recorded, and the ST segment fall rate of 1 h ECG after PCI was calculated. The platelet aggregation rate was measured. After 12 months' follow-up, the incidence of major adverse cardiac events (MACE) was recorded. The Logistic regression analysis was used to discover the factors of MACE. Results One hundred and sixty-nine patients with acute STEMI were recruited including 85 cases treated with ticagrelor and 84 cases in clopidogrel group. The ECG ST segment fall rate after PCI in ticagrelor group was significantly higher than that in clopidogrel group: (61.3 ± 30.7)% vs. (47.8 ± 26.6)%, P<0.05. The platelet aggregation rate 2 h, 24 h and 7 d after PCI in ticagrelor group was significantly lower than that in clopidogrel group (P<0.05). MACE occurred in 19 cases (22.4%) in ticagrelor group and in 21 cases (25.0%) in clopidogrel group, and there was no significant difference (P > 0.05). Multiple Logistic regression analysis revealed that age and ECG ST segment fall rate were independent risk factors for MACE (P<0.05). Conclusions Compared with clopidogrel, ticagrelor can effectively reduce platelet aggregation rate and accelerate ST segment fall in STEMI patients, but their long-term prognosis is similar.

OBJECTIVE: To provide reference for standardization and improvement of teaching model for endocrinology speciality in community clinical pharmacists training base. METHODS: Based on the requirments of community clinical pharmacists training in Shanghai,  the characteristics and contents of community clinical pharmacists training were summarized, and teaching characteriatiostics, effect evaluation and problems werer analyzed on the basis of the acutual situation in our hospital. RESULTS & CONCLUSIONS: Community clinical pharmacists training has the characteristics of training for chronic diseases, short training cycle, and support from the medical consortium platform. Its contents are theoretical learning and professional knowledge training, pharmaceutical care and medication education, case discussion, homework guidance and comment. Compared with other specility, endocrinology speciality has the characteristics of strengthening comprehensive pharmaceutical care (for example, pharmaceutical care of diabetic patients should pay attention not only to hypoglycemia, but also to hypobaric, hypolipidemic and complications), emphasizing medication treatment management (MTM) teaching (provide a series of professional services such as medication education, counseling and guidance, and medical reorganization for patients, so as to improve the safety, effectiveness and compliance of medication, and then control blood glucose, delay or avoid complications), emphasizing the cultivation of health education ability and communication ability. The evaluation of training effect included the evaluation of trainees and teachers. The former includes objective evaluation (theoretical knowledge test, bedside medication education assessment and homework assessment) and subjective evaluation (attendance, attitude, clinical practice performance). The latter is mainly scored by trainees on teachers (including professional level, teaching attitude, teaching ability and content). Through the training, it is found that the trainees’ literature retrieval ability is poor, the trainees have not clear idea to diabetic patients, the homework writing and typesetting are not standardized; the teaching teachers have less experience and teachers could not teach the students in accordance with their aptitude and provide individualized training. The training and teaching mode needed to be further improved.

ABSTRACT

Objective To investigate the effect of DDX46 gene on the radiosensitivity of colorectal cancer cells and underlying mechanism.Methods SW480 cells transfected with DDX46 RNAi or its empty plasmid by lentivirus were set as experimental group and control group,respectively.After 72 h of transfection,the cells were irradiated with 4 Gy X rays.The cell viabilities of these two groups were detected by CCK-8 assay.The number of γ-H2AX foci and the expressions of some key proteins related to DNA damage repair were detected by immunofluorescence technique and Western blot at 24 h after irradiation.Results At 24 h after 4 Gy irradiation,the cell vitality of experimental group was decreased to (15.02±3.92)%(t=-4.696,P < 0.05) of control and(17.43 ±1.83)%(t=4.844,P<0.01) of nonirradiated cells,but there was no significant difference between 4 Gy irradiated control cells and nonirradiated cells.Meanwhile,compared with the control group,the ATM protein expression level (t =7.530,P <0.01) and the number of γ-H2AX foci (t =-3.108,P <0.05) were significantly increased in the experimental SW480 cells,while the expression levels of p-ATM and Rad50 were significantly reduced (t =4.260,4.260,P < 0.05),and the protein levels of DNA-PK in these two groups had tiny difference.Conclusions DDX46 RNA silence increases the radiation sensitivity of SW480 cells by inhibiting ATM activation and DNA repair.

ABSTRACT

OBJECTIVE:To observe the clinical efficacy and safety of intravitreal injection of ranibizumab combined with argon laser therapy in the treatment of ischemic neovascular glaucoma. METHODS:A total of 166 patients with neovascular glaucoma selected from our hospital during Mar. 2014-Mar. 2016 were divided into control group and observation group according to random number table,with 88 cases in each group(there were 117 and 125 eyes respectively). Both groups received argon laser photocoagulation therapy twice a week for 1 month. Observation group was additionally given intravitreal injection of Ranibizumab injection(0.5 mg)once a week,for 1 month. Clinical efficacies of 2 groups were observed,and the intraocular pressure and visual acuity of 2 groups were observed before treatment,1 week and 1 month after treatment. The occurrence of ADR was recorded. RESULTS:After 1 month of treatment,total response rate was 95.20% in observation group,which was significantly higher than 76.92% in control group,with statistical significance (P<0.05). After 1 week of treatment,intraocular pressures of observation group were lower than before treatment and control group at the same time,while visual acuity was better than before treatment and control group at the same time,with statistical significance (P<0.05). After 1 month of treatment,intraocular pressures of 2 groups were decreased significantly and visual acuity was significantly improved;intraocular pressure was decreased and visual acuity was improved more significantly in observation group than control group,with statistical significance(P<0.05 or P<0.01). The incidences of ADR of 2 groups were 3.41% and 2.27%,without statistical significance (P>0.05). CONCLUSIONS:The ischemic intravitreal injection of ranibizumab combined with argon laser therapy is better than argon laser therapy alone in the treatment of ischemic neovascular glaucoma,and can significantly reduce intraocular pressure and improve visual acuity with good safety.

ABSTRACT

OBJECTIVE:To evaluate the effectiveness of Liuwei wuling tablets combined with entecavir in the treatment of chronic hepatitis B (CHB),and to provide evidence-based reference in clinic.METHODS:Retrieved from Cochrane library,PubMed,Web of Science,CJFD,Wanfang database and VIP,randomized controlled trials (RCTs) about Liuwei wuling tablets combined with entecavir (trial group) vs.entecavir alone (control group) in the treatment of CHB were collected.Meta-analysis was conducted by using Rev Man 5.3 statistical software after data extraction and quality evaluation with Cochrane systematic evaluator manual 5.1.0.RESULTS:A total of 8 RCTs were included,involving 728 patients.Results of Meta-analysis showed that the levels of ALT [MD=-16.67,95 % CI (-19.83,-13.51),P< 0.001],AST [MD =-20.52,95% CI (-25.65,-15.39),P<0.001] and TBIL [MD=-5.85,95%CI(-11.31,-0.38),P=0.04] in trial group were significantly lower than control group;clinical total response rate [RR=1.23,95%CI(1.03,1.46),P=0.02] and HBeAg negative conversion rate [RR=1.26,95%CI(1.03,1.54),P=0.02] were significantly higher than control group,with statistical significance.There was no statistical significance in the rate of serum HBV DNA negative conversion [RR=1.22,95% CI(0.99,1.52),P=0.07] between 2 groups.CONCLUSIONS:Compared with entecavir alone,Liuwei wuling tablets combined with entecavir in the treatment of CHB have better efficacy,can significantly improve the biochemical indexes of liver function in patients.

ABSTRACT

<p><b>OBJECTIVE</b>To establish rabbit VX2 colorectal cancer(CRC) model and to compare CT images with gross pathology in order to offer help for TNM staging in patients with CRC.</p><p><b>METHODS</b>VX2 tumor pieces were implanted into colonic wall in 9 New Zealand white rabbits and rectal wall in 2 New Zealand white rabbits. Four weeks after inoculation, Ultravist(370 mg/ml) was injected through ear marginal vein with high pressure injector for stage 3 scanning of chest, abdomen and pelvis, and enhanced CT (collimation 0.5 mm mm × 320, pitch factor 0.828, bulb rotation speed 0.5 s/cycle, 120 kV, automatic ma, range 80 to 100 mAs) was performed to determine the presence of CRC or metastasis once a week for 4-6 weeks. Once inoculated CRC or metastases occurred or 6 weeks after implantation, the rabbits were sacrificed regardless of the presence or absence of CRC or metastasis on the CT images. One rabbit was used for gross anatomy observation. Others were placed in wood boxes with -80centi-degree for 24 hours, then samples of 3 mm thickness were cut using a motorized saw to make macropathology. Each cutting surface of the specimens was photographed in serial number. If certain or suspected lesions were found on the slices, such part was labeled and then placed in 10% phosphate-buffered formaldehyde numbered box for subsequent pathological examination. CT image postprocessing was performed referring to the gross slice specimens and all findings were compared with the pathological reports.</p><p><b>RESULTS</b>Among 11 rabbits, tumor was successfully established in 8 rabbits. Pathology showed that single lung metastasis (7 to 10 mm) was found in 2 rabbits and liver metastasis (9 mm) in 1 rabbit. Number of lymph node located around the inoculated tumor was 22 and that around mesenteric vessels was 13 with diameter of 2 to 16 mm. Among these 35 lymph nodes from 8 successful rabbits, 9 nodes were positive, including 7 around inoculated tumor and 2 around mesenteric vessels. CT identified above 8 primary inoculated tumors, 2 lung metastatic lesions and 1 liver metastatic lesion, with detection rate of 100%. For the detection of lymph node in CT, 27 nodes were identified in the pericolorectal region (17 nodes) and perimesenteric vessels (10 nodes), in which 6 were positive metastasis (ring-shaped enrichment and central low density necrosis), resulting in a detection rate of 77.1%(27/35 nodes), and positive detection rate of 66.7% (6/9 nodes), respectively.</p><p><b>CONCLUSION</b>Living rabbit CT-gross pathological slice(3 mm-cut) of VX2 CRC model can be applied in image evaluation of small metastatic lesion.</p>
