ABSTRACT

Objective:To explore the efficacy and safety of endovascular intervention combined with preoperative laparoscopic exploration in the treatment of patients with acute mesenteric artery ischemia.Methods:This was a prospective cohort study (NCT04686981). The study enrolled 31 patients with acute mesenteric artery ischemia from Oct 1, 2020 to Oct 1, 2022. Among them, 26 patients (84%) were male, with a mean age of (67±13) years and a mean time to onset of (21±8) hours. All patients underwent laparoscopic exploration in the hybrid operating room. If the presence of intestinal necrosis or suspected necrosis was clearly determined, the patient would undergo open surgery (mesenteric artery embolization, intestinal resection and intestinal double stoma) as the treatment by gastrointestinal surgeon. If intestinal necrosis or suspected necrosis was not found by laparoscopy, the patient would undergo endovascular intervention by vascular surgeon. The primary observational endpoints of this study were the proportion of patients who were not dependent on total parenteral nutrition and all-cause mortality within 30 days after operation. The secondary observational endpoints were the rate of mesenteric vascular patency within 30 days and the proportion of interventions that were converted to open surgery.Results:Six patients underwent open surgery and 25 patients underwent endovascular intervention, including 13 cases of thrombus reduction alone, 3 cases of stent implantation during the same period after reduction, and 9 cases of stent implantation alone. Twenty-four patients (77%) were completely weaned from the TPN within 30 days after the procedure, and all-cause mortality was observed in 3 cases (9.7%). The patency rate of the mesenteric artery within 30 days after the procedure was 82.1%. The rate of conversion to open surgery after intervention was 16%.Conclusions:Endovascular intervention combined with preoperative laparoscopic exploration can clarify intestinal ischemia in acute mesenteric patients as early as possible, and individualized treatment strategies for each patient by multidisciplinary care team can potentially improve the prognosis of such patients.

ABSTRACT

【Objective】 To develop a mobile software application named " Component Assistant" and test for its performance in practical work, so as to address the difficulties and problems encountered during the management process of blood component preparation, such as communication and coordination in the workflow, personnel scheduling and workload arrangements. 【Methods】 The software was developed based on the daily work requirements and processes using Java language, and foreground-background separation technologies were employed to provide secure and reliable data support. 【Results】 The results of practical work verification showed that through this software, component preparation managers were able to real-time monitor blood collection situations, blood transfusion details, manage inventory levels, and summarize and review the details of the preparation process. Comparison of the usage sequence of this software, the average amount of blood prepared of employees has increased(198 bloodbag, /M), the workload of employees has increased(3.5, /M) and the rest time has been shortened(1 h, /M). 【Conclusion】 The innovation of this software lies in providing effective data support for matching the workload and personnel in component preparation operations, meeting the needs of blood component preparation management, and greatly improving work efficiency in this field.

ABSTRACT

Objective:To analyze the clinical characteristics of pneumatosis cystoides intestinalis in the Tibet Plateau, and to explore the diagnosis and treatment method of this disease.Methods:The medical records of 60 patients with confirmed pneumatosis cystoides intestinalis who were admitted to Tibet Autonomous Region People's Hospital from January 2014 to July 2021 were retrospectively analyzed. A descriptive statistical analysis was made.Results:The male-to-female ratio of 60 patients with pneumatosis cystoides intestinalis was 2.2: 1. The average age of onset was (48.6 ± 14.5) years. The average living altitude was (3 832.3 ± 399.8) meters. All patients were admitted mainly because of gastrointestinal symptoms. Twenty patients had hemorrhoids, chronic hepatitis, and gallstones. Pneumatosis cystoides intestinalis was confirmed by endoscopic examination in 59 patients and by surgery in 1 patient. Among the patients, 43 patients had lesions involving the colon, 2 patients had lesions involving the small intestine, and 1 patient had lesions involving the duodenum. The specific lesion sites were not determined in the remaining 14 patients because of unclear descriptions of the examination results. Two patients were positive for fecal occult blood tests, with a median hemoglobin value of 156.0 g/L. Forty-nine patients were given hyperbaric oxygen therapy, medication, and diet adjustment. Eleven patients were treated surgically. Clinical symptoms were relieved in 43 patients, and 17 patients were lost to follow-up.Conclusion:Pneumatosis cystoides intestinalis is relatively common in Tibet Plateau, and more common in men than in women. It occurs at any age. Clinical manifestations are not specific. The lesions most often involve the colon. The disease is mainly diagnosed by endoscopy. Individualized treatment based on different causes can help improve the prognosis of pneumatosis cystoides intestinalis.

ABSTRACT

The parabrachial nucleus (PBN) integrates interoceptive and exteroceptive information to control various behavioral and physiological processes including breathing, emotion, and sleep/wake regulation through the neural circuits that connect to the forebrain and the brainstem. However, the precise identity and function of distinct PBN subpopulations are still largely unknown. Here, we leveraged molecular characterization, retrograde tracing, optogenetics, chemogenetics, and electrocortical recording approaches to identify a small subpopulation of neurotensin-expressing neurons in the PBN that largely project to the emotional control regions in the forebrain, rather than the medulla. Their activation induces freezing and anxiety-like behaviors, which in turn result in tachypnea. In addition, optogenetic and chemogenetic manipulations of these neurons revealed their function in promoting wakefulness and maintaining sleep architecture. We propose that these neurons comprise a PBN subpopulation with specific gene expression, connectivity, and function, which play essential roles in behavioral and physiological regulation.

ABSTRACT

Objective:To investigate the prevalence and influencing factors of mild visual impairment in type 2 diabetic patients aged ≥50 years living in Funing County, Jiangsu Province.Methods:A population-based cross-sectional study was adopted.Cluster sampling was conducted among diabetic patients aged ≥50 years registered in the Chronic Disease Information Center of Funing County Center for Disease Prevention and Control from March 7th to April 20th, 2019.General information of selected patients was collected.Visual and ocular examinations on site were performed by ophthalmologists and technicians.The International Council of Ophthalmology standard for mild visual impairment, 0.3≤best corrected visual acuity<0.8 was adopted.The influencing factors of mild visual impairment were analyzed by logistic regression.This study protocol was approved by the Ethics Committee of Affiliated Hospital of Nantong University (No.2014-071).Results:Among the included 2 067 patients, 1 909 cases completed the investigation, with a response rate of 92.4%.There were 1 146 patients with mild visual impairment in at least one eye, and the prevalence of mild visual impairment was 60.00%.Among the eyes examined, 1 832 eyes were with mild visual impairment, accounting for 47.98%.Cataract (896/1 632), refractive error (515/1 632) and diabetic retinopathy (166/1 632) were the main causes of mild visual impairment in diabetic patients, accounting for 54.90%, 31.61% and 10.19%, respectively.Aging, female gender, low educational attainment, oral hypoglycemic drugs or subcutaneous insulin injection were risk factors for mild visual impairment in both eyes.Higher educational attainment, more sunshine exposure and exercise were protective factors for mild visual impairment in both eyes.Conclusions:The prevalence of mild visual impairment is relatively high in type 2 diabetic patients.Cataract surgery, correction of refractive error may help to improve mild visual impairment.Strengthening blood glucose control, more physical exercise and outdoor activities may slow the progression of mild visual impairment and prevent it from moderate and severe visual impairment.

ABSTRACT

Objective:To analyze the effect of endovascular therapy (including balloon dilation, debulking and anti-restenosis technique) for below-the-ankle atherosclerotic lesions in chronic limb threatening ischemic patients.Methods:A retrospective analysis was performed on the clinical data of 465 patients (570 limbs) with below-the-ankle atherosclerosis who were consecutively admitted to Beijing Tsinghua Changgung Hospital Affiliated to Tsinghua University from April 2018 to December 2021. All patients received balloon dilatation below the ankle artery as basic treatment, including 67 patients in debulking subgroup, 82 patients in anti-restenosis subgroup and 11 patients in the triple therapy subgroup. The reocclusion rate, target lesion reintervention rate at 1, 3, 6 and 12 months after surgery were compared; The wound healing rate at 3 months, limb salvage rate at 6 and 12 months and mortality at 12 months in the Global Limb Anatomic Staging System (GLASS) grading groups P0, P1 and P2 were compared.Results:The technical success rate of this group was 76.0%(433 limbs/570 limbs). The follow-up rates of 6 and 12 months after operation were 85.4%(370 limbs/433 limbs) and 75.3%(326 limbs/433 limbs), respectively. The length of treated lesions was (3.6±1.3)cm; The reocclusion rates were 5.5%(23 limbs/420 limbs), 8.2%(33 limbs/403 limbs), 14.9%(55 limbs/370 limbs) and 23.6%(77 limbs/326 limbs) at 1, 3, 6 and 12 months after operation; The reintervention rate of target lesions in 6, 12 months was 7.6%(28 limbs/370 limbs) and 12.0%(39 limbs/326 limbs) respectively; The wound healing rate in groups P0, P1 and P2 within 3 months was 87.3%(192 limbs), 70.5%(62 limbs) and 10.5%(2 limbs) respectively ( P<0.001), the limb salvage rate in 6 months was 96.2%(200 limbs), 84.7%(127 limbs) and 33.3%(4 limbs) respectively ( P<0.001), and the limb salvage rate in 12 months was 78.7%(170 limbs), 54.5%(55 limbs) and 22.2%(2 limbs) respectively ( P<0.001). The mortality was 6.9%(32/465) 12 months after operation. Conclusions:Endovascular therapy below-the-ankle is feasible and has high technical success rate. The constitute integrity of pedal-plantar loop in limb- threatening phase is related to 3-month wound healing rate and limb salvage rate.

ABSTRACT

Objective:To evaluate the efficacy and impacts of air tamponadein treating rhegmatogenous retinal detachments (RRD) including inferior breaks after 25G pars plana vitrectomy (PPV).Methods:A prospective non-randomized study. From November 2016 to December 2018, 110 RRD patients receiving PPV in the Ophthalmology Department of Affiliated Hospital of Nantong University were included in the study. All the patients choose the operation mode voluntarily after fully understand the condition and operation mode. There were 90 eyes in the air filling group and 20 eyes in the silicone oil filling group. There was no difference of age ( t=-0.082), sex ratio ( χ2=1.538), left or right eye ( χ2=0.839), average course of disease ( Z=-0.276), intraocular pressure ( t=3.669), axial length ( t=0.765), grade of proliferative vitreous retinopathy ( Z=-2.101), high myopia ( χ2=0.013), lens state ( χ2=1.275), logMAR BCVA ( t=-0.681), the scope of retinal detachment ( t=0.970), the site ( χ2=0.108) and numbers ( t=0.158) of the retinal holes, whether involving macula ( χ2=1.791) between the two groups ( P>0.05). At 1, 3, and 6 months after surgery, the first retinal reduction rate, BCVA increase rate, visual recovery rate, intraocular pressure, fovea retinal thickness (CFT), ellipsoid and cross sectional area, and the occurrence of postoperative complications in the two groups were observed and compared. Independent sample t test and Chi-squared test, Mann-Whitney U test, and ANOVA were used for data comparision. Results:After the first operation, retinal reattachmnents were achieved in 87 eyes (96.6%) in the air group and 19 eyes (95.0%) in the silicone oil group ( χ2=0.130, P>0.05). In the air group and the silicone oil group, there were 7 and 2 eyes with subretinal holes, whose retinal reattachmnents were achieved. At 6 months after surgery, logMAR BCVA of the eyes in the air group and the silicone oil group were 0.23±0.23 and 0.37±0.23, respectively. All of them were higher than that before operation ( t=-2.410, P<0.001). BCVA in the air group was superior to that in the silicone oil group ( P<0.05). The increase rate of BCVA of the eyes in the air group and the silicone oil group were 0.69±0.28 and 0.48±0.30 logMAR units, respectively ( t=-3.225, P<0.05). The CFT of the eyes in the air group and the silicone oil group were 226.87±42.30 μm and 234.83±36.10 μm, respectively ( t=-0.448, P=0.657). In the air group and silicone oil group, posterior cataract was observed in 15 and 4 eyes, subretinal residual fluid in 1 and 2 eyes, and visual object deformation in 3 eyes respectively. In the air group and silicone oil group, posterior cataract was observed in 15 and 4 eyes, subretinal residual fluid in 1 and 2 eyes, and visual object deformation in 3 eyes respectively. Conclusions:Air filling combined with PPV can effectively treat RRD, which is suitable for both superior and inferior holes. The visual acuity recovered well and the complication is less. The time of retinal detachment (especially the time of macular detachment) is an important factor affecting the prognosis.

Objective@#To explore the value of magnetic resonance fat quantification technology (MR mDixon-quant) in evaluating thyroid-associated ophthalmopathy (TAO) activity.@*Methods@#Case control study was performed.Fifty patients (100 eyes) with TAO which met Mourits criterion were enrolled from September 2016 to March 2018 in Ningxia Hui Autonomous Region People's Hospital.The TAO patients were grouped by clinical activity score (CAS). Twenty nine patients (58 eyes) with CAS≥3 served as TAO active group, twenty one patients (42 eyes) with CAS<3 served as TAO non-active group.Twenty three healthy subjects (46 eyes) were collected as normal control group at the same time.All subjects underwent orbital MR mDixon-quant and MR imaging.Orbital fat content (FF values) and exophthalmus was calculated on MR work-station.FF values and degree of exophthalmus were analyzed by analysis of variance.The correlation of FF values and CAS in TAO patients was conducted by Spearman rank correlation analysis.The cut-off values of FF in predicting TAO activity was determined by receiver operating characteristics (ROC) curve.The Kappa consistency test was chosen to assess the consistency of CAS and FF values.Intraclass correlation coefficient (ICC) were used to analyze the intra-observer and inter-observer repeatability of the FF values and the degree of exophthalmus.This study was approved by the Ethics Committee of Ningxia Hui Autonomous Region People's Hospital (No.20160718). All the candidates signed informed consent.@*Results@#The FF values were (85.190±4.346)%, (88.715±5.686)% and (82.345±5.445)% in TAO active group, TAO inactive group and normal control group, respectively, with a significant difference among the 3 groups (F=17.072, P<0.001). The FF values of TAO active group and TAO inactive group were significantly higher than that of control group, and the FF value of TAO active group was significantly lower than that of TAO inactive group (all at P<0.01). The degrees of exophthalmos were (20.221±1.714), (20.855±2.103) and (15.363±1.667)mm in TAO active group, TAO inactive group and normal control group, respectively, with a significant difference among the three groups (F=126.298, P<0.01). The degrees of exophthalmos in TAO active group and TAO inactive group were significantly higher than that in normal control group (both at P<0.01), there was no statistical difference in degrees of exophthalmos between TAO active group and TAO inactive group (P<0.05). Spearman rank correlation revealed that FF value was negative correlated with CAS (rs=-0.443, P<0.01). The cut-off value of FF value was 87.180%, the sensitivity, specificity, positive predictive value, negative predictive value and accuracy were 74.14%(43/58), 66.67%(28/42), 75.44%(43/57), 65.12%(28/43) and 71.00%(71/100), respectively.There were consistencies of FF values with the CAS (Kappa=0.431, P<0.001). The intra-observe and inter-observer reproducibility were fairly good for FF value (ICC=0.953, 0.920) and degree of exophthalmus (ICC=0.935, 0.917).@*Conclusions@#MR mDixon-quant can quantitative measure orbital fat with good measurement repeatability.Fat quantification technology by MR is valuable in evaluating TAO activity.

ABSTRACT

Objective To explore the mediating effect of perfectionism between rumination and par-enting style among undergraduates. Methods The investigation was carried out among 915 college students by random sampling method,using ruminative responses scale(RRS),short-form egma minnenav bardndosna uppforstran(S-EMBU) and college student general perfectionism scale. Results The scores of perfection-ism,rumination and their division dimensions in undergraduates were ( 26. 35 ± 4. 13),( 17. 14 ± 4. 86), (22. 27±4. 64),( 11. 10± 2. 71),( 10. 38± 2. 71) and(43. 75± 8. 37),respectively. Rumination of under-graduates was positively correlated with father rejection,mother rejection(r=0. 23,0. 29,P<0. 01),father overprotection,mother overprotection(r=0. 16,0. 20,P<0. 01) and minding mistakes(r=0. 36,P<0. 01), while negatively correlated with father emotional warmth and mother emotional warmth(r=-0. 07,-0. 09,P<0. 05). Minding mistakes had significantly positive correlations with father rejection,mother rejection(r=0. 12,0. 19,P<0. 01) and father overprotection,mother overprotection ( r=0. 16,0. 12,P<0. 01). Minding mistakes had significantly negative correlations with father emotional warmth and mother emotional warmth (r=-0. 18,-0. 20,P<0. 01). The structural equation model proved that minding mistakes mediated the re-lationship between mother and father rejection and rumination through mediating effect test ( χ2/df=8. 45, RMSEA=0. 091,CFI=0. 96,GFI=0. 97). The mediating effect value was 0. 112(95%CI=0. 048~0. 194), and mediating effect accounted for 28. 95% . Conclusion The mediating effect of minding mistakes is re-markable in father and mother rejection and rumination,and reducing father rejection,mother rejection and perfectionism can reduce rumination among undergraduates.

ABSTRACT

Objective To evaluate the experience and early results of radiofrequency ablation (RFA) therapy for varicose veins.Methods In this study 380 extremities of 356 patients undergoing RFA therapy with RFA catheter for varicose veins from Nov 2015 to Sep 2017 in Beijing Tsinghua Changgung Hospital were retrospectively reviewed.Each patient was scheduled to follow up at 1,6 and 12 months respectively.Results Technical success were achieved in all cases.Obliteration rate was 99.0% in 1 month,97.9% in 6 months and 97.1% in 12 months after the RFA therapy.Conclusions RFA therapy for varicose veins is safe and effective and achieves good early result.Intraoperative ultrasound examination and endovascular technique is important for a successful RFA procedure.

ABSTRACT

Objective To investigate the prevalence and associated risk factors for pterygium among people aged 50 years and above in Funing County,Jiangsu Province.Methods A cluster random sampling method was performed,the subjects aged 50 years or above were randomly selected from 30 survey sites in Funing County,Jiangsu Province.Questionnaires,visual acuity tests,the examinations of eye surface,anterior segment,fundus examinations were conducted.Pterygium was diagnosed and graded clinically by slit lamp examination.The risk factors were acquired from questionnaires and analyzed by the multivariate logistic regression analysis.This study protocol was approved by Ethic Committee of Affiliated Hospital of Nantong University (NO.2010-05).Written informed consent was obtained from each subject prior to entering study cohort.Results A total of 6 145 persons aged 50 years and above were enumerated,and actually 5 947 (96.8%) participants were examined.Among them,1 950 cases were diagnosed as pterygium in either eye and 1 228 cases were diagnosed as pterygium in binoculus,which was equivalent to the 32.79% [95% confidence interval(CI):31.60%-33.98%] of pterygium in either eye and 20.65% (95% CI:19.62%-21.68%) in bilateral pterygium.Among 2467 male subjects,838 were diagnosed as pterygium (33.97%,95% CI:32.10%-35.84%).Among 3480 female subjects,1 112 were diagnosed as pterygium (31.95%,95% CI:30.40%-33.51%).There was no significant difference in the prevalence of pterygium between genders (P =0.135).Multivariate Logistic analysis showed that,older age (50 ~ <60 years:odds ratio [OR] =1.00;60 ~ <70 years:OR=1.54,P<0.001;70 ~ <80 years,OR=1.83,P<0.001;≥80 years:OR=1.99,P<0.001),low educational level (no education:OR =1.00;<primary:OR =0.87,P =0.031;primary education:OR =0.72,P =0.002;≥ secondary education:OR =0.63,P =0.002),farmer occupations (OR =1.34,P =0.020),and long outdoor work time (OR =1.13,P =0.026) were independent risk factors for pterygium.Gender,marriage,income,hypertension,diabetes,smoking and alcohol use history were not associated with pterygium (all at P>0.05).Conclusions The prevalence of pterygium in Funing County is 32.79% in people aged 50 years and above.The high prevalence of pterygium may be associated with older age,low education level and long outdoor work time.

ABSTRACT

The mammalian central nervous system (CNS) is considered an immune privileged system as it is separated from the periphery by the blood brain barrier (BBB). Yet, immune functions have been postulated to heavily influence the functional state of the CNS, especially after injury or during neurodegeneration. There is controversy regarding whether adaptive immune responses are beneficial or detrimental to CNS injury repair. In this study, we utilized immunocompromised SCID mice and subjected them to spinal cord injury (SCI). We analyzed motor function, electrophysiology, histochemistry, and performed unbiased RNA-sequencing. SCID mice displayed improved CNS functional recovery compared to WT mice after SCI. Weighted gene-coexpression network analysis (WGCNA) of spinal cord transcriptomes revealed that SCID mice had reduced expression of immune function-related genes and heightened expression of neural transmission-related genes after SCI, which was confirmed by immunohistochemical analysis and was consistent with better functional recovery. Transcriptomic analyses also indicated heightened expression of neurotransmission-related genes before injury in SCID mice, suggesting that a steady state of immune-deficiency potentially led to CNS hyper-connectivity. Consequently, SCID mice without injury demonstrated worse performance in Morris water maze test. Taken together, not only reduced inflammation after injury but also dampened steady-state immune function without injury heightened the neurotransmission program, resulting in better or worse behavioral outcomes respectively. This study revealed the intricate relationship between immune and nervous systems, raising the possibility for therapeutic manipulation of neural function via immune modulation.

ABSTRACT

Objective To investigate the abnormal region of gray matter and distribution in long-term night shift physicians of the emergency department,and explore the brain structural changes closely related to long-term chronic sleep loss.Methods The study was carried out among two groups of medical staff,with the chronic sleep deprived(CSD)group(n=15)performing a fixed night work and the regular sleep controls (RSC)group (n=15)working regularly during the day at our hospital.Both groups were age-and gender-matched.The data of two groups were analyzed using voxel-based morphometry (VBM)based on SPM8 to generate gray maps.Results Compared to the RSC group without night shifts,increased gray matter volume in the CSD group was observed in the middle temporal gyrus,inferior frontal fyrus,insula,thalamus,posterior cingulate,medial frontal gyrus,inferior parietal lobule and postcentral gyrus (P<0.05).Conclusion These findings suggest that long-term,chronic sleep loss is associated with the change of the gray matter volume in the network,and may have a neuroanatomical basis.The brain structural changes are related with the domains of emotion and language,this contributes to further explore on the neural mechanism of the effects of the long-term night shift on brain function.

Objective To investigate the effects of enteral immunonutrition supplemented with omega-3 polyunsaturated fatty acid (ω-3 PUFA) on inflammatory response,intestinal mucosal barrier function and the prognosis in patients with severe traumatic brain injury (sTBI).Methods 122 patients of sTBI hospitalized between January 2015 and December 2016 were randomly divided into experimental group (ω-3 PUFA,n=61) and control group (n =61).The serum levels of tumor necrosis factor-α (TNF-α),interleukin (IL)-6 and neuron specific enolase (NSE) were tested with enzyme linked immunosorbent assay.Meanwhile,D-lactate acid and intestinal fat acid binding protein (I-FABP) were evaluated by enzymology spectrophotometer method.After 14 days of treatment,the Glasgow Coma Scale (GCS) scores,Acute Physiology and Chronic Health Evaluation (APACHE) Ⅱ scores and prognoses of both groups were compared.Results The serum levels of inflammatory factors (TNF-α and IL-6),intestinal mucosal barrier function indicators (D-lactate acid and I-FABP) and NSE proteins significantly increased after sTBI (P =0.01).Compared with the control group,the experimental group on day 3 had significantly lower serum levels of inflammatory factors [TNF-α:(107.77± 19.79) μg/Lvs.(151.76±21.65) μg/L,P=0.01;IL-6:(76.85±7.15) μg/Lvs.(105.27±10.12) μg/L,P=0.01] and intestinal mucosal barrier function indicators [D-lactate:(69.81 ±6.32) μg/L vs.(89.80± 8.75) μg/L,P=0.03;I-FABP:(40.81±6.73) μg/Lvs.(56.60±8.58) μg/L,P=0.01].On day 7,the experimental group had significantly lower expression of NSE proteins than the control group [(13.63± 2.53) μg/L vs.(19.12±3.00) μg/L,P=0.02].The experimental group received better prognosis compared to the control group on day 14 [GCS scores:(9.74±0.76) vs.(8.44±0.53),P=0.04;APACHE Ⅱ scores:(14.67±1.37) vs.(17.53±1.47),P=0.03].The experimental group also had fewer days in hospitalization [(19.37±2.27) d vs.(25.42±2.61) d,P=0.01].Conclusion Enteral immunonutrition supplemented with ω-3 PUFA can effectively regulate the inflammatory response,and reduce impairment to the intestinal mucosal barrier function and damage to neurons in patients with sTBI.

ABSTRACT

Objective: To explore school counselors' attitude and decision-making towards dual relationship and confidentiality. Methods: Totally 123 school counselors (27 males and 96 females, aged 20 to 60 years old, 41 primary and secondary counselors, 82 college counselors) filled up of self-designed Questionnaire for School Psychological Consultants on Dual Relationship and Confidentiality Rules, and the related factors which could influence the school counselors' attitude and their decision-making in dual relationship and confidentiality in the counseling work. Results:Totally 71.5% (n =88) school counselors chose not to accept other relationships with students after counseling, 14.6% (n = 18) school counselors chose to accept other relationships with students after counseling. The school counselors who had not accepted supervisioneasier to answer as "accept other relationship may raise clients self-esteem or feelings" than those had got supervision (P <0.05). The school counselor believed that the confidentiality agreement should be breached of the top 3(suicide attention, drug use and bad gangs). Counselors easier to breach confidentiality agreements on situation of students had suicide plans than situation of students had plans to run away from home(P <0.001). Conclusion:This study shows that two factors, including supervision and differential clients "issue may have effect on school counselors" attitudes and decision-making towards dual relationship and confidentiality situation in their counseling practice.

ABSTRACT

Objective To compare and explore the optimal pathway of enteral nutrition between nasointestinal tube and nasogastric tube in patients with severe traumatic brain injury (sTBI). Methods One hunderd patients with sTBI, admitted to our hospital from January 2016 to December 2016, were divided into nasointestinal tube group and nasogastric tube group (n=50) according to different enteral nutrition pathways. The serum levels of tumor necrosis factor-α (TNF-α) and interleukin (IL)-6 were tested with enzyme linked immunosorbent assay (ELISA). Meanwhile, intestinal mucosal barrier function indexes D-lactate acid and intestinal fat acid binding protein (I-FABP) were evaluated by enzymology spectrophotometer method. On the admission, and one, 3, 7 and 14 d after treatment, gastrointestina complications, pneumonias and prognoses of the two groups were compared. Results As compared with those in the nasogastric tube group, the serum levels of inflammatory related factors (TNF-α and IL-6) in the nasointestinal tube group were significantly decreased 3 d after treatment (TNF-α: [112.77± 14.52] μg/L vs. [163.16±18.85] μg/L P=0.01; IL-6: [80.11±8.02] μg/L vs. [100.97±9.82], P=0.02). As compared with those in the nasogastric tube group, the intestinal mucosal barrier function indexes (D-lactate acid and I-FABP) in the nasointestinal tube group were significantly decreased 7 d after treatment (D-lactate: [47.51±4.87] μg/L vs. [62.20±6.01] μg/L, P=0.04; I-FABP: [29.12±3.74] μg/L vs. [40.08±5.51] μg/L, P=0.01). The nasointestinal tube group had significantly lower gastric retention incidence (18% vs. 28%, P=0.00) and aspiration incidence (14% vs. 26%, P=0.06). Meanwhile, the nasointestinal tube group had significantly lower incidence of pneumonia (24% vs. 34%, P=0.01), reduced pneumonia severity (clinical pulmonary infection scores [CPIS]: 14.27±1.22 vs. 17.30±1.50, P=0.03), better prognosis (GCS scores: (9.41±0.66 vs. 8.23±0.57, P=0.03; APACHE II scores: 14.27± 1.22 vs. 17.30±1.50, P=0.03) 14 d after treatment as compared with the nasogastric tube group. Enteral nutrition by nasointestinal tube could shorten the hospitalization days of patients (18.52±2.00 vs. 24.82± 2.31, P=0.02). Conclusion Enteral nutrition by nasointestinal tube can effectively reduce the incidences of gastrointestinal and pulmonary complications, and alleviate inflammatory response and intestinal mucosal barrier function damage in patients with sTBI, possibly improving prognosis.

ABSTRACT

Objective:To explore the mediating effect of positive emotion on relationship between cognition reappraisal and meaning of life in freshmen.Methods:Totally 192 freshmen[62 males and 130 females,aged 18 to 22 years,mean age (20 ± 2) years] were assessed with the Chinese version of the Emotion Regulation Questionnaire (ERQ),Positive words of the Positive and Negative Affect Scale (PANAS) and Chinese version of the Meaning of Life Questionnaire (MLQ).Interval of 3 months of longitudinal data was used to investigate the mediating role of positive emotions in cognitive reappraisal sense and meaning of life.Results:The EPQ scores were positively correlated with the scores of PA,MLQ-P,and MLQ-S(r =0.17-0.26,Ps < 0.05).The PA scores were positively correlated with the scores of MLQ-P and MLQ-S (r =0.16-0.35,Ps <0.05).Bootstrap test showed that positive emotions mediated meaning of life through cognitive reappraisal (95% CI:0.065-0.295).The structural equation model proved to exhibit an appropriate data fit (x2/df=1.58,RMSEA =0.06,CFI =0.98,GFI =0.95).Conclusion:It indicates that cognitive reappraisal is related to meaning of life and positive emotions,and positive emotions mediate the relationship between cognitive reappraisal and meaning of life among freshmen.

ABSTRACT

Objective To investigate the value of quantitative dynamic contrast-enhanced MRI (DCE-MRI)in diagnosis of breast adenosis.Methods 54 patients with breast glandular lesions diagnosed as 4th or 5th grade in BI-RADS and proved by pathology un-derwent routine 3.0T quantitative DCE-MRI.Quantitative parameters including volume transfer constant (Ktrans ),plasma volume fraction (Vp )were measured.According to the pathological findings and MRI morphological characteristics,the lesions were divided into breast cancer group,adenosis group and control group.The contralateral healthy side of the adenosis group was regarded as control group.The quantitative parameters of the three groups were statistically analyzed.Results Pharmacokinetic parameters of Ktrans and Vp were (0.289±0.1 63)min-1 and 0.042 8±0.045 respectively in adenosis group (n=21),(0.959±0.45 1)min-1 and 0.057±0.079 in breast cancer group (n=33),and (0.048±0.022)min-1 and 0.01 6 ±0.01 9 in control group (n=21).Ktrans and Vp between adenosis and control group had statistical significance(Z =-5.733,-2.844,all P <0.05),Ktrans between breast cancer group and adenosis group had statistical difference (Z =-5.421,P =0.000 ).Vp between breast cancer and adenosis group had no statistical difference (Z=-0.009,P =0.993).Area under curve (AUC)of the receiver operator curve (ROC)to differentiate benign and malignant lesions of Ktrans were 0.941.When the cutoff were 0.304 min-1 ,the sensitivities and specificities were 93.9%,85.7%. Conclusion Quantitative DCE-MRI shows good diagnostic value in differentiation of breast glandular lesions and cancer.

Objective To evaluate comparability of two different assay system for detecting CRP.Methods Following the profile of Clinical and Laboratory Standard Institute (CLSI)document EP9-A2,50 blood samples with anti-coagulant ED-TA-2K were collected from emergency patients at Changhai Hospital.The test result of samples by the i-CHROMA Reader was compared and evaluated with those by Beckman Immage 800.Results The linear regression equation for plasma CRP was:Y=1.076 5X-3.031 5,R2=0.986.The linear regression equation for whole blood CRP was:Y=0.882 6X-1.180 8, R2=0.931 1.For whole blood samples with low HCT (<30.45%).Used correction equation:CRP (after corrected)=CRP (before corrected)/(1-HCT).The regression equation (after corrected)was:Y=1.006 8X-3.612 2,R2=0.950 9.Con-clusion CRP concentration detected by i-CHROMA showed good correlation and comparability compared to laboratory ref-erence system by using plasma samples.Results form whole blood samples with low HCT should be corrected to improve comparability.

ABSTRACT

Objective To evaluate the nursing effect of orientation of traditional Chinese medicine drug therapy combined with multiple frequency vibration therapeutic instrument in treating pediatric pneumonia with dyspneic cough. Methods A total of 277 children (pneumonia with dyspneic cough syndrome of wind-cold invading lung, wind-heat invading, lungphlegm-heat congesting lung) were enrolled in this study. They were divided into the orientation of traditional Chinese medicine drug therapy combined with multiple frequency vibration therapeutic instrument group (lung therapy and sputum excretion group,100 cases), the orientation of traditional Chinese medicine drug therapy group (lung therapy group, 94 cases) and the multiple frequency vibration therapeutic instrument group (sputum excretion group, 83 cases) by random digits table method.The main symptoms improvements, therapeutic evaluation, the comfortable level of the children, family satisfaction degree of the 3 groups were evaluated before and after 5 days of treatment. Results After 5 days, the main symptoms score was (7.22±2.74) points in the lung therapy and sputum vibration group, and (8.98±3.40), (8.78±2.91) points in the lung therapy group and sputum excretion group, there were significant differences among 3 groups, F=9.756, P0.05. The family satisfaction degree in the lung therapy and sputum vibration group was higher than that in the other two groups, and there were significant differences among 3 groups, χ2=9.102, P<0.05. Conclusions The nursing effect of orientation of traditional Chinese medicine drug therapy combined with multiple frequency vibration therapeutic instrument has more advantages in treating pediatric pneumonia with dyspneic cough.
