ABSTRACT
OBJECTIVE To explore the efficacy of alfacalcidol combined with conventional antihypertensive and lipid- lowering drugs on liver and kidney function, serum inflammatory cytokines and renin-angiotensin system(RAS) in hypertensive patients with renal impairment. METHODS A total of 200 hypertensive patients with renal impairment who were treated in the department of nephrology in our hospital from December 2017 to December 2020 were selected and randomly divided into control group and observation group, with 100 cases in each group. Both groups of patients were treated with conventional antihypertensive and lipid-lowering drugs for a total of 14 weeks, patients in the observation group were additionally treated with oral alfacalcidol after 2 weeks of treatment (0.25 μg each time, once a day, for a total of 12 weeks). The levels of liver function indexes [aspartate aminotransferase (AST), alanine aminotransferase (ALT)], renal function indexes [blood calcium, blood phosphorus, blood urea nitrogen (BUN), cystatin C (Cys-C), serum creatinine (Scr), urine microalbumin (mAlb), β2-microglobulin (β2-MG), urinary N- acetyl β-D-glucosaminidase (NAG), 24 h urinary protein], inflammatory factors [serum interleukin 6 (IL-6), tumor necrosis factor α (TNF-α), hypersensitive C-reactive protein (hs-CRP)] and RAS activity indexes [renin, angiotensin Ⅰ(Ang Ⅰ), Ang Ⅱ and aldosterone] were observed in 2 groups before and after treatment, and the occurrence of adverse drug reactions was recorded during treatment. RESULTS There was no statistical significance in the levels of detection indexes between 2 groups before treatment (P>0.05). After treatment, the level of blood calcium in the observation group was significantly higher than before treatment (P<0.05), but remained at clinically normal level. Compared with before treatment, the levels of Cys-C, Scr, BUN, urine mAlb, β2-MG, NAG and 24 h urinary protein, hs-CRP, IL-6, TNF-α, renin, Ang Ⅰ, Ang Ⅱ and aldosterone were significantly decreased in the observation group after treatment (P<0.05). After treatment, the level of blood calcium in observation group was significantly higher than control group (P<0.05). Additionally, the levels of Cys-C, Scr, BUN,urine mAlb, β2-MG, NAG, 24 h urinary protein, hs-CRP, IL-6, TNF-α, renin, Ang Ⅰ, Ang Ⅱ and aldosterone were significantly lower than control group (P<0.05). There was no statistical significance in the incidence of adverse drug reactions between 2 groups during treatment (P>0.05). CONCLUSIONS Alfacalcidol combined with routine therapy of antihypertensive and lipid-lowering drugs could effectively improve liver and renal functions, inhibit inflammation and RAS activity in hypertensive patients with renal impairment, with a favorable safety.
ABSTRACT
Objective:To investigate the efficacy of Bailing capsule combined with conventional western medicine on diabetic nephropathy patientswith maintenance peritoneal dialysis. Methods:Eighty-six type 2 diabetic nephropathy patients receiving peritoneal dialysis in our hospital from February of 2016 to February of 2019 were selected and were divided into control group and observation group according to the random number table method with 43 cases in each group. All patients received continuous ambulatory peritoneal dialysis. Patients in the control group received routine treatment. In the observation group, patients took Bailing capsules based on the treatment of the control group. The two groups were treated continuously for 3 months. Insulin resistance (IR), fasting blood glucose (FPG), fasting insulin (FINS), renal function, and serum levels of microinflammatory factors high sensitivity c-reactive protein (hs-CRP), interleukin (IL)-6, IL-17, and tumor necrosis factor (TNF)-α were compared in the two groups. Results:After the treatment, FPG, FINS, HOMA-IR, blood creatinine, urea nitrogen, and 24 h urinary protein in the observation group were significantly less than those of the control group ( t value were 5.204, 8.227, 14.070, 5.022, 5.900, 5.246, respectively, all Ps<0.01). After thetreatment, serum levels of hs-CRP, IL-6, IL-17, TNF-α in the observation group were significantly lower than those of the control group ( t value were 12.871, 7.021, 7.733, 8.165, respectively, all Ps<0.01). Conclusions:Bailing capsule can effectively improve the insulin resistance and renal function of diabetic nephropathy patients during maintenance peritoneal dialysis and relieve the state of micro-inflammation.

To evaluate the relationship between atherosclerosis and hemodynamic of coronary artery in mice detecting by ultrasound bio‐microscopy flow imaging . Methods Double 14 20‐week‐old LDL‐R‐/‐and C57BL/6 male mice were selected ,and randomly divided into two groups in each genotype according to weight . Each two groups were fed to 28 weeks or 36 weeks age respectively with west diet . Coronary artery hemodynamics in these mice were assessed in vivo by Vevo ?2100 ultrasound imaging system ,then the intima‐media thickness( IM T ) of aorta in histopathology were analyzed . T he differences of coronary artery hemodynamic parameters such as maximum velocity ( Vmax ) ,mean velocity ( Vmean) and velocity time integral ( V T I) were compared between mice of different genotypes of the same week and mice of different weeks of the same genotype . And the relationship between coronary artery hemodynamic in ultrasound and aortic IM T in histopathology were analyzed . Results ① All coronary hemodynamic parameters in LDL‐R‐/‐ mice were significantly lower than those of wild‐type mice except the Vmax between two 28‐week‐old genotypes group at the same weeks of age of different genotypes ( all P <0 .05) . But there was no significant difference in coronary artery hemodynamic parameters between mice of the same genotype at different weeks of age( P >0 .05) . ②T he histopathological measurements of aortic IM T in LDL‐R‐/‐mice were significantly higher than those of wild type mice ( all P < 0 .05 ) ,and those of 36‐week‐old mice were significantly higher than those of 28‐week‐old mice ( all P < 0 .05 ) . ③ All coronary hemodynamic parameters such as Vmax ,Vmean and V TI were negatively correlated with pathological measurements of aortic IM T ( r = -0 .532 , -0 .423 , -0 .524 ; all P < 0 .05 ) . Conclusions The parameters of coronary artery hemodynamics obtained by ultrasound bio‐microscopy are well correlated with the pathological results of atherosclerosis . Ultrasound bio‐microscopic flow imaging can be used as a new method to evaluate the degree of atherosclerosis in mice by detecting the hemodynamic parameters of coronary artery .

Objective@#To investigate the impact of blood pressure and age on arterial stiffness in general population.@*Methods@#Participants who took part in 2010, 2012 and 2014 Kailuan health examination were included. Data of brachial ankle pulse wave velocity (baPWV) examination were analyzed. According to the WHO criteria of age, participants were divided into 3 age groups: 18-44 years group (n=11 608), 45-59 years group (n=12 757), above 60 years group (n=5 002). Participants were further divided into hypertension group and non-hypertension group according to the diagnostic criteria for hypertension (2010 Chinese guidelines for the managemengt of hypertension). Multiple linear regression analysis was used to analyze the association between systolic blood pressure (SBP) with baPWV in the total participants and then stratified by age groups. Multivariate logistic regression model was used to analyze the influence of blood pressure on arterial stiffness (baPWV≥1 400 cm/s) of various groups.@*Results@#(1)The baseline characteristics of all participants: 35 350 participants completed 2010, 2012 and 2014 Kailuan examinations and took part in baPWV examination. 2 237 participants without blood pressure measurement values were excluded, 1 569 participants with history of peripheral artery disease were excluded, we also excluded 1 016 participants with history of cardiac-cerebral vascular disease. Data from 29 367 participants were analyzed. The age was (48.0±12.4) years old, 21 305 were males (72.5%). (2) Distribution of baPWV in various age groups: baPWV increased with aging. In non-hypertension population, baPWV in 18-44 years group, 45-59 years group, above 60 years group were as follows: 1 299.3, 1 428.7 and 1 704.6 cm/s, respectively. For hypertension participants, the respective values of baPWV were: 1 498.4, 1 640.7 and 1 921.4 cm/s. BaPWV was significantly higher in hypertension group than non-hypertension group of respective age groups (P<0.05). (3) Multiple linear regression analysis defined risk factors of baPWV: Multivariate linear regression analysis showed that baPWV was positively correlated with SBP(t=39.30, P<0.001), and same results were found in the sub-age groups (t-value was 37.72, 27.30, 9.15, all P<0.001, respectively) after adjustment for other confounding factors, including age, sex, pulse pressure(PP), body mass index (BMI), fasting blood glucose (FBG), total cholesterol (TC), smoking, drinking, physical exercise, antihypertensive medications, lipid-lowering medication. (4) Multivariate logistic regression analysis of baPWV-related factors: After adjustment for other confounding factors, including age, sex, PP, BMI, FBG, TC, smoking, drinking, physical exercise, antihypertensive medication, lipid-lowering medication, multivariate logistic regression analysis showed that risks for increased arterial stiffness in hypertension group were higher than those in non-hypertension group, the OR in participants with hypertension was 2.54 (2.35-2.74) in the total participants, and same results were also found in sub-age groups, the ORs were 3.22(2.86-3.63), 2.48(2.23-2.76), and 1.91(1.42-2.56), respectively, in each sub-age group.@*Conclusion@#SBP is positively related to arterial stiffness in different age groups, and hypertension is a risk factor for increased arterial stiffness in different age groups. Objective@#To explore the gender-specific risk factors of new-onset cerebral hemorrhage.@*Methods@#In this prospective cohort study,a total of 98 961 participants((51.1±12.6)years old), who underwent the 2006 to 2007 physical examination and met the inclusion criteria, were enrolled from the Kailuanstudy cohort. There were 78 908 (79.7%) male,and 20 053 (20.3%) female.The incidence of cerebral hemorrhage was observed once per year until December 31, 2016.The difference on the incidence of cerebral hemorrhage between male and female was compared. Multivariate Cox regression analysis was applied to analyze therisk factors of cerebral hemorrhage events among different genders.@*Results@#The participants were followed up for(10.00±0.73) years,and 860 cerebral hemorrhage events were recorded during follow up. The incidence of cerebral hemorrhage in the population was 86.90/10 million person years (standardized incidence rate of 47.85/10 million person years). The incidence of cerebral hemorrhage was significantly higher in male (49.61/10 million person years) than in female (34.07/10 million person years, P<0.05). Multivariate Cox regression analysis showed that 45-59 years old, ≥ 60 years old, diabetes,and waist-hip ratio were more strongly related to new-onset of cerebral hemorrhage events in female than in male, and the hazard ratios(95%CI) were 2.33 (1.23-4.43) ,2.71 (1.30-5.66) ,2.16 (1.24-3.74) and 8.79 (1.42-54.32) in female versus 1.55 (1.21-1.97) ,2.16 (1.68-2.78) ,1.19 (0.93-1.53) and 3.21 (1.09-9.41) in male, respectively. The risk of male cerebral hemorrhage increased by 29% (HR=1.29, 95%CI 1.19-1.40) in male and 24% (HR=1.24, 95%CI 1.20-1.28) in female,when the systolic blood pressure increased 10 mmHg (1 mmHg=0.133 kPa).@*Conclusions@#The incidence of cerebral hemorrhage is higher in male than in female in this cohort.The association between systolic blood pressure and cerebral hemorrhage is stronger in male than that in female.The associations between age, waist-hip ratio, diabetes and cerebral hemorrhage are stronger in female than in male.@*Trial Registration@#Chinese Clinical Trail Registry, ChiCTR-TNC-11001489.

Objective To observe the early changes of related indexes after high dose of 60Co γ-ray irradiation on rhesus monkey hematopoietic system.Methods A total of 33 rhesus monkeys were randomly divided into normal control and different irradiation control group,rhesus monkeys in irradiation control group were given different doses(4,8,12 Gy) irradiation to establish acute radiation sickness(ARS) models.XE-2100 automatic blood cell analyzer detected the peripheral blood before and after the irradiation of 3,6,9,12,24,48,80 h.The rhesus monkeys were sacrificed to have a observation of sternum pathological changes at 6,48 and 80 h after 4,8,12 Gy 60Co γ-ray irradiation.Results The number of white blood cell in peripheral blood of the rhesus monkeys after 4 and 8 Gy 60Co γ-ray irradiation were lower than that before irradiation at 3 h after irradiation,as was significant increased at 6 h after irradiation,the highest values were 136.04%.and 221.38% after 9 h(with before irradiation values was 100.00%,the same below),become obviously drooped from 12 h after irradiation,show clearly temporary peak.But the number of white blood cell after 12 Gy 60Co γ-ray irradiation was significant increased at 6 h after irradiation,at the highest of 9 h,become obviously drooped from 12 h after irradiation.Peripheral blood neutrophile count was significant increased at 6 h after irradiation,at the highest of 9 h,become obviously drooped from 12 h after irradiation.Peripheral blood lymphocyte count fell sharply after irradiation,3 h detection value was only 12.02%-25.04% of before irradiation.Sternal bone marrow nucleated cell number decreased sharply after irradiation,the more irradiation dose,the less residual hematopoietic cells.Conclusion In the early stage of BM-ARS,temporary peaktime node of the white blood cell and neutrophil count could be regarded as the best delivery time of hematopoietic cytokine therapy.

Objective: To explore the relationship between longitudinal trajectory of systolic blood pressure (SPB) and atrial fibrillation (AF) in Kailuan group population. Methods: Our study cohort consisted of 40727 participants with the specific criteria in Kailuan group, Tangshan. SAS Proc Traj procedure was used to identify longitudinal trajectories of SPB throughout 2006-2007, 2008-2009 and 2010-2011. There were 5 longitudinal trajectories generated: Low-stable group,n=10950, Moderate-stable group, n=19158, Moderate-high stable group,n=3713, High-moderate stable group,n=4702 and High stable group,n=2181. Log-rank test was performed to compare AF incidence throughout 2012-2013 and 2014-2015 by physical examination among different groups; Multi Cox regression analysis was conducted to study the relationship among different SBP longitudinal trajectories and AF occurrence. Results:①The mean age of participants was (51.81±11.54) years including 30693 (75.4%) male.②AF occurrence rates in Low-stable, Moderate-stable, Moderate-high stable, High-moderate stable and High stable groups were 0.1%, 0.2%, 0.5%, 0.5% and 0.6% respectively, allP<0.05.③Multi Cox regression analysis presented that with adjusted confounding factors, compared with the patients in Low-stable group, Moderate-high stable, High-moderate stable and High stable SBP longitudinal trajectories were the risk factors for new AF occurrence (HR=7.58, 95% CI 2.08-27.73), (HR=5.30, 95% CI 1.88-14.95) and (HR=8.52, 95% CI 1.96-37.09) respectively, allP<0.05. With excluded history of myocardial infarction/stroke, the sensitivity study showed the similar result with the major research trend. Conclusion: Elevated long trajectory of SPB was the risk factor for new AF occurrence in Kailuan group population.

Through efforts of several generations over the past half century,great advances have been achieved in the develop?ment of radiation countermeasures for the acute radiation sickness(ARS). Convergent studies have disclosed numerous radioprotec?tants with significant radioprotective efficacy which include granulocyte colony stimulating factor(G-CSF),granulocyte macrophage colony stimulating factor(GM-CSF),thrombopoietin(TPO),interleukin-12,derivatives of the bacterial flagellin,androst-5-ene-3β, 17β-diol(AED),beclomethasone 17,21-dipropionate(BDP),vitamin E(and its)derivatives,and genistein. particularly,the two growth factors G-CSF and TPO show greater radioprotective effects. In this paper,we summarize the radioprotective effects of com?pounds or biological agents on severe ARS(SARS),which have been evaluated in large animal models or assembled into a nuclear accident emergency treatment medicine box,and review their research advances in recent years.
ABSTRACT
A new urine analysis core module based on high performance 32-bit microprocessor and high precision color sensor was presented. A novel optical structure and a specific circuit were applied to improve measurement precision and temperature was used to compensate for results in this core module. The information of urine test peice, such as all original data and color RGB value, reflectivity, semi-quantitative level, etc. can be output. The results showed that the measuring precision was about 95% or above with ideal stability and reliability using this presented core module, which can be conveniently applied in various urine analyzers, and can greatly decrease the cost of urine analyzers in development and production.

Objective To investigate ventricular remodeling and glucose transporter 4(GLUT4) expression in rat with type 2 diabetes mellitus(DM) complicated with myocardial infarction (MI), as well as the effects of ramipril on ventricular remodeling and CLUT4 expression. Method Diabetic animal model (DM) was produced by cholesterol-diet and intraperitoneal injedtion of streptozotocin (STZ,30 mg/kg). The successful model was identified based on the blood gulcose value > 6.7 mmol/L. Left anterior descending coronary artery was ligated to produce the model of MI. 46 wistar rats were randomly(random number) divided into 5 groups: DM + MI + placebo group(placebo group, n = 9), DM + MI + ramipril group(ramipril group, n = 10), DM + sham-operation group (DM group, n = 10),non-DM group + MI (MI group, n = 9), and non-DM + sham-operation group (n = 8). After six weeks, echocardiographic studies was performed ; Left ventricular mass index (LVMI), myocardial infarct size and myocardial collagen volume fraction(CVF) of non-infarction area were also detected. GLUT4 mRNA in the myocardium away from infarction region was measured with real-time fluorescent quantitative RT-PCR and the expression of GLUT4 protein was measured with western blot. Analysis of variance was used to compare the groups. P -values< 0.05 were considered to indicate statistically significant differences. Results Compared with Ml group and DM group,EF[(32 ±3)% vs. (65 ±4)%, P <0.01 and (32 ±3)% vs. (38 ±4)%, P <0.05 respectively] significantly decreased,while E/A ratio (P < 0.01), LVMI (P < 0.05) and CVF (P < 0.05) obviously increased in placebo group. The expression of GLUT4 mRNA and protein were similar between placebo group and DM group, however, much lower than those in MI group(P<0.01).In rampiril group, E/A ratio (P < 0.01), LVMI(3.1±0.2 vs.4.2±0.4, P <0.01) and CVF(P < 0.05) significantly decreased compared with placebo group, while EF[(45±3)% vs.(32±3)%, P <0.01] and the expression of GLUT4 mRNA and protein (P < 0.05) increased. Conclusions Rampiril could improve left ventricular hypertrophy and systolic/di-astolic dysfunction, increase the expression of GLUT4 mRNA and protein in diabetic rats complicated with MI.

OBJECTIVE To carry out "Occupation Exposition Risk Guard" curriculum practice in high rank nursing and discuss the value of built this curriculum in senior professional school.METHODS The skin damage and after-damage processing were investigated by questionaire before and after this curriculum.RESULTS People not accepted this curriculum had skin damage of 96.25%,after-damage processing rate of 36.25%,and reporting rate of 20.00%,vs 70.00%,100.00% and 87.5% in but people who accepted this curriculum.CONCLUSIONS In the process of training the education of occupation exposition protection knowledge has important value.
ABSTRACT
Objective The purpose of this study was to evaluate the effects and mechanism of ACEI(Fosinopril),AT1 receptor antagonism (Irbesartan),and combination of these two drugs on ventricular remodeling in rats with myocardial infarction.Methods Rats were randomly divided into four groups at twenty-four hours after MI,and treated for 6 weeks.Mean blood pressure and left ventricular end diastolic pressure(LVEDP) were evaluated,as well as ventricular weight(VW)/body weight(BW) ratio.The total,type I and type Ⅲcollagen and nonmyocyte cellular proliferation were quantified by histomorphometry.The expression of transforming growth factor-beta 1(TGF-? 1)mRNA within the noninfarction zone was determined by in situ hybridization.Results Irbesartan and combination therapy decreased total collagen more significantly compared with fosinopril.Combination therapy decreased typeⅠcollagen more significantly than fosinopril.Nonmyocyte cellar proliferation in MI-combination group was more significantly suppressed than in MI-fosinopril group.Treatment with irbesartan or combination therapy normalized TGF-? 1 mRNA level within the noninfarction zone.Conclusion Fosinopril or irbesartan alone ,and combination of these two drugs can limit myocardial hypertrophy,attenuate the development of myocardial interstitial fibrosis,and prevent nonmyocyte cellar proliferation in the noninfarcted left ventricle.The use of irbesartan,especially combined with fosinopril was more effective than fosinopril alone in the suppression of histopathologic changes resulting in ventricular remodeling after MI.Irbesartan and combination therapy was more effective than fosinopril alone in suppressing TGF-? 1 mRNA expression.

Objective:Our aim was to To clarify the roles of p16 and p15 proteins in the genesis of acute lymphoblastic leukemia(ALL).Methods:Twenty-three samples of ALL were studied by the method of indirect immunofluorescence.Flow cytometer was used to estimate the cellular fluorescent intensity to determine the levels of p16 and p15 proteins.Results:Negative expression for p16 protein was found in 10 of 23 samples,and 8 of 23 were p15 negative expression.Both kinds of proteins were abscent in 6 samples.2 of 3 cases of T-ALL were negative expression of p16,p15 protein.In non T-ALL,6 of 13 were negative expression for p16 protein,5 of 13 were p15 protein deficient.The expression rates of p16,p15 protein in high leukocyte group were lower than those of non-high leukocyte group(P<0.05).The expression rates of p16,p15 proteins in HR-ALL were lower than those of SR-ALL(P<0.05).Conclusion:The p16 and p15 proteins take part in the genesis of ALL.Negative expression of p16,p15 proteins might imply the poor clinical outcome.

Objective: To explore whether high epidural blockade(HEB) can lower the elevated serum free fatty acids(FFA) level in the patients with acute myocardial infarction(AMI). Method:In 7 patients with AMI,HEB at T 15 was performed by injecting 5 to 7ml of 0.5%-0.75% lidocaine every 2 to 4 hours for 7 successive days;while the other 7 patients with AMI didn't use HEB as control. only morphine or dolantine was used for relieving angina cordis. Result: The elevated FFA level returned to normal range in 12 hours following HEB in the study group, but in one week in the control group. Conclusion:HEB(T1-5) can lower the elevated FFA level in AMI patients to normal within12 hours.
