ABSTRACT

Dermatitis herpetiformis is an autoimmune bullous disease associated with celiac disease, which occurs mostly in Caucasians carrying HLA-DQ2 or HLA-DQ8 alleles. Studies have shown a variety of antibodies against transglutaminase and other antigens in the serum of patients with dermatitis herpetiformis, and epidermal transglutaminase is an autoantigen. The occurrence of dermatitis herpetiformis not only is associated with genetics and gluten sensitivity, but also may be related to the imbalance of intestinal microbiome. Gluten-free diet and dapsone are still the main therapies for dermatitis herpetiformis. This review summarizes epidemiological features, pathogenesis, clinical manifestations, laboratory features, diagnosis and treatment of dermatitis herpetiformis.

ABSTRACT

Objective:To explore the antidepressant mechanism of Yinxing Mihuan oral solution (YMO) by investigating its effect on depression model rats. Method:The depression rats were induced by isolation combined with chronic unpredictable mild stress (CUMS) and then randomly divided into model group, fluoxetine group (10 mg·kg-1) and high-dose (618 mg·kg-1) and low-dose (309 mg·kg-1) YMO groups. A blank control group was also set up and ten rats were included in each group. Modeling lasted for 21 consecutive days, and rats were administered the 8th day after stimulation at a dose of 10 mL·kg-1 for 14 days, except those in the blank control and model groups which were given distilled water. Afterward, the sucrose preference test, open field test, tail suspension test were carried out. The pathological changes of hippocampus in depression rats were observed after hematoxylin-eosin (HE) staining. The content of interleukin-1β (IL-1β), interleukin-6 (IL-6) and tumor necrosis factor-α (TNF-α) in the hippocampus of rats in each group and the expression of NOD-like receptor 3 (NLRP3) and other proteins in its related activation signaling pathways were detected with multi-factor detection (Luminex) and Western blot. Result:After 14 days of continuous administration, compared with the blank control group, the model group witnessed significantly reduced sugar water consumption rate and the times of rearing and significantly prolonged cumulative time of immobility during tail suspension (P<0.05, P<0.01). Compared with the model group, the fluoxetine group and the high-dose YMO group saw increases in the times of rearing, times of crossing and sugar water consumption rate and a significant decrease in the cumulative time of immobility during tail suspension (P<0.05, P<0.01). The results of HE staining showed that the neurons in the hippocampus of rats in the high-dose YMO group were arranged in order and slightly loosened, without obvious microglia infiltration observed. The levels of IL-1β, IL-6 and TNF-α in the hippocampus of the model group increased significantly as compared with the blank control group (P<0.05, P<0.01), and their content in the high-dose YMO group was significantly lowered in the comparison with the model group (P<0.05, P<0.01). Molecular biology experiments demonstrated that compared with the results of blank group, the expression of purinergic receptor P2X7 (P2RX7), NLRP3, apoptosis-associated speck-like protein (ASC), Caspase-1 and IL-1β remarkably increased in the model group (P<0.05, P<0.01). Additionally, the expression of P2RX7, NLRP3, ASC, Caspase-1 and IL-1β was significantly inhibited in the fluoxetine group and the high-dose YMO group compared with the model group (P<0.05, P<0.01). Conclusion:YMO can improve the depression-like behaviors of rats induced by isolation combined with CUMS, and its mechanism of action is related to the regulation of the P2RX7/NLRP3 signaling pathway.

ABSTRACT

Optic neuritis (ON) is one of the symptom of a central nervous system demyelinating, systemic or infectious disease. The pathogenetic mechanism of ON is still not completely clear, and its core is inflammation and immune that occurred in the optic nerve axons, and apoptosis of RGC was induced.Few patients experience recurrent episodes after treatment, presenting a remission-recurrence course of polyphasic disease, named recurrent ON (RON).Two forms of RON have been assigned:recurrent isolated optic neuritis, which is a chronic corticosteroid-dependent optic neuropathy with intermittent acute relapses, and recurrent isolated optic neuritis, which is a non-progressive relapsing ON without steroid dependence.Recurrence of ON causes cumulative damage to the optic nerve lesions and impaired visual signal transmission, thus causing irreversible damage to vision. Therefore,it is very important to have a deep understanding of the pathogenesis of ON and the clinical characteristics of RON,so as to better conduct clinical treatment.

ABSTRACT

PURPOSE: The purpose of this study was to detect the lymphatic drainage pattern of internal mammary area and verify the concept of internal mammary sentinel lymph node (IM-SLN) in breast. MATERIALS AND METHODS: A small particle radiotracer ((99m)Tc-Dextran 40) was prepared and tested. (99m)Tc-Dextran 40 was injected into intraparenchyma at the sound breast by a modified radiotracer injection technique. Subsequently, dynamic single-photon emission computed tomography (SPECT), computed tomography (CT), and SPECT/CT combination images were performed to identify the radioactive lymph vessels and internal mammary lymph nodes (IMLNs). The direction of lymph drainage and the location of the IMLNs were identified in the SPECT/CT imaging. RESULTS: The radiochemical purity of (99m)Tc-Dextran 40 was > 95%. (99m)Tc-Dextran 40 could drainage into first, second, and third lymph node and the radioactive lymph node could be detected by the γ detector in the animal experiment. After (99m)Tc-Dextran 40 injecting into intraparenchyma, 50.0% cases (15/30) were identified the drainage lymphatic vessels and radioactive IMLNs by SPECT. The drainage lymphatic vessel was found from injection point to the first IMLN (IM-SLN) after 10.5±0.35 minutes radiotracer injection, and then (99m)Tc-Dextran 40 was accumulated into the IM-SLN. The combination imaging of SPECT/CT showed the second IMLN received the lymph drainage from the IM-SLN. The lymphatic drainage was step by step in the internal mammary area. CONCLUSION: The lymph was identified to drain from different regions of the breast to IM-SLN, and then outward from IM-SLN to other IMLN consecutively. It demonstrated the concept of the IM-SLN and provided more evidences for the application of internal mammary sentinel lymph node biopsy.

ABSTRACT

Transient receptor potential cation channel 6 (TRPC6) is a member of the transient receptor superfamily encoded by the TRPC6 gene and is widely expressed in tissues and organs of the human body, especially in the glomerular podocytes. TRPC6 interacts with various slit diaphragm (SD) proteins including podocin, nephrin, ACTN4, and CD2AP to maintain the normal structure and function of glomerular podocytes. Foot process fusion caused by podocyte damage due to various factors is the most important morphological change in kidney disease. This article reviews the biological function of TRPC6 and its effect on kidney disease.

ABSTRACT

<p><b>BACKGROUND</b>Accurately, characterizing plaques is critical for selecting the optimal intervention strategy for the left main coronary artery (LMCA) bifurcation. Coronary angiography cannot precisely assess the location or nature of plaques in bifurcation lesions. Few intravascular ultrasound (IVUS) classification scheme has been reported for angiographic imaging of true bifurcation lesions of the unprotected LMCA thus far. In addition, the plaque composition at the bifurcation has not been elucidated. This study aimed to detect plaque composition at LMCA bifurcation lesions by IVUS.</p><p><b>METHODS</b>Fifty-eight patients were recruited. The location, concentricity or eccentricity, site of maximum thickness, and composition of plaques of the distal LMCA, ostial left anterior descending (LAD) coronary artery and, left circumflex (LCX) coronary artery were assessed using IVUS and described using illustrative diagrams.</p><p><b>RESULTS</b>True bifurcation lesions of the unprotected LMCA were classified into four types: Type A, with continuous involvement from the distal LMCA to the ostial LAD and the ostial LCX with eccentric plaques; Type B, with concentric plaques at the distal LMCA, eccentric plaques at the ostial LAD, and no plaques at the LCX; Type C, with continuous involvement from the distal LMCA to the ostial LCX, with eccentric plaques, and to the ostial LAD, with eccentric plaques; and Type D, with continuous involvement from the distal LMCA to the ostial LAD, with eccentric plaques, and to the ostial LCX, with concentric plaques. The carina was involved in only 3.5% of the plaques. A total of 51.7% of the plaques at the ostium of the LAD were soft, while 44.8% and 44.6% were fibrous in the distal LMCA and in the ostial LCX, respectively.</p><p><b>CONCLUSIONS</b>We classified LMCA true bifurcation lesions into four types. The carina was always free from disease. Plaques at the ostial LAD tended to be soft, whereas those at the ostial LCX and the distal LMCA tended to be fibrous.</p>

ABSTRACT

This study was aimed to further clarify the pathogenesis of acute myeloid leukemia(AML), and to forecast new somatic mutations related with leukemia. The peripheral blood samples on initial diagnosis and remission stage of a patient with partial differentiated AML(AML-M2) were sequenced by high throughput transcriptome sequencing technology. The single nucleotide variation (SNV) which possibly related with pathogenesis of leukemia was screened through comparison of the expressed genes on initial diagnosis and after remission. The results showed that the Reads distributed uniformly in genome and covered completely, detecting most expression genes. by screening the SNV, a total of 29881 mutations were discovered, including 28113 germline mutations and 752 individual mutations. Among them, 11 acquired mutations (P < 0.05) in coding regions were got, including ZRSR1, MLXIP, TLN1, LAP3, HK3. It is concluded that the high throughput sequencing as an unbiased new method can find new tumor-related mutations. MLXIP may be a new molecular marker of AML-M2.

ABSTRACT

<p><b>OBJECTIVE</b>To determine the expressions of miR-155, miR-34a and miR-30a in diffuse large B-cell lymphoma (DLBCL) and to explore their potential correlation with clinicopathological characteristics.</p><p><b>METHODS</b>The expression level of miR-155, miR-34a and miR-30a in 46 DLBCL samples were determined with TaqMan real-time polymerase chain reaction. Interphase fluorescence in situ hybridization (I-FISH) was performed to detect MYC and p53 genes' status, and immunohistochemistry (Envision method) was used to evaluate the expression of CD3, CD10, CD20, BCL-6 and MUM-1 in DLBCL. The DLBCLs were classified into germinal center B cell-like (GCB) and non germinal center B cell-like (non-GCB) subtypes according to Hans' criteria.</p><p><b>RESULTS</b>Compared with normal controls, miR-155 expression level was significantly higher in DLBCL. The expression level of miR-155 in non-GCB type was higher than that in GCB type. It was shown that the patients with MYC rearrangement had lower expression level of miR-155 than the negative controls. Compared with p53 normal group, the expression level of miR-34a was significantly lower in p53 deletion group. It was also shown that the patients with BCL-6 protein expression had lower expression of miR-30a compared with the negative group.</p><p><b>CONCLUSION</b>miR-155 expression level is different in normal controls, DLBCL and patients with subtype DLBCL. It therefore has a diagnosis value for DLBCL. miR-34a is of great prognostic significance. miR-155, miR-34a and miR-30a may be potential therapy targets for DLBCL.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the influence of veneer application on failure behavior and reliability of lithium disilicate glass-ceramic (LDG) crowns of maxillary first molar, and thus to reveal the failure mechanism of bilayered LDG crowns.</p><p><b>METHODS</b>Twenty-six LDG maxillary first molar crowns were fabricated in a dental laboratory using IPS e. max Press or IPS e. max Press/Ceram. The crowns were randomly assigned into two groups (with or without veneer application) with thirteen in each group. The crowns were cemented on composite resin dies. After storage in water for one week, the sliding-contact fatigue test was performed by sliding the steatite ceramic ball indenter (6 mm in diameter) from central fossa up to the lingual surface of disto-buccal cusp, cyclic loaded 1 200 000 times with a weight of 100 N at 2 Hz with a fatigue chewing simulator. Survived specimens were subjected to single-load-to-fracture testing using a steatite ceramic ball of 6 mm in diameter at a cross-head speed of 0.5 mm/min in a universal testing machine. Fracture load values were recorded and analyzed with t test. Weibull modulus was calculated to evaluate structure reliability. Fractographic analysis was carried out to determine fracture modes of the failed specimens by a stereomicroscope and a scanning electron microscope (SEM).</p><p><b>RESULTS</b>Statistical analysis results indicated a significant difference of the fracture load values between monolithic group [(2071.23 ± 397.05) N] and bilayered group [(1483.41 ± 327.87) N] (P < 0.001). Monolithic and bilayered groups present similar Weibull modulus (95% confidence interval) as 6.15 (5.15 ∼ 7.15) and 5.54 (4.01 ∼ 7.08) respectively, with no significant difference (the confidence bounds overlapped with each other). Bulk fracture initiating from the middle of oblique ridge of the first maxilla molar was the primary failure mode of monolithic/bilayered LDG crowns. Crack propagation initiated from core-veneer interfacial defects was another major failure mode of bilayered all-ceramic crowns.</p><p><b>CONCLUSIONS</b>Veneer application has some influence on fatigue failure of LDG crowns, but shows no effect on structure reliability. Accumulated damage combined with tensile stress concentration on the surface of veneer layer and defects within core-veneer interface lead to initiating of cracks. The mechanical property of veneering materials should be increased, and procedure of veneer application should be standardized and improved in order to reduce the failure rate of LDG molar crowns.</p>

ABSTRACT

Objective: To investigate the interindividual variation of nuclear factor erythroid 2-related factor 2 (Nrf2) mRNA expression level in the peripheral blood nucleated cells and its correlation with the severity of chemotherapy-induced myelosuppression. Methods: The expression level of Nrf2 mRNA in the peripheral blood nucleated cells from 30 cases of cancer was detected by semi-quantitative RT-PCR before chemotherapy, and the association between the Nrf2 mRNA expression and the chemotherapyinduced myelosuppression was analyzed. Results: The Nrf2 mRNA expression level in the peripheral blood mononuclear cells exhibited significant interindividual differences. The Nrf2 mRNA expression level was significantly higher in the patients with grades 0-1 leukopenia or neutropenia than that in the patients with grades 2-4 (P <0.05), but it was not associated with the age, gender, chemotherapeutic regimen and the type of tumor. The association analysis showed that the Nrf2 mRNA expression level in the peripheral blood mononuclear cells was negatively associated with leukopenia and neutropenia (r = -0.448, P = 0.013; r = -0.493, P = 0.006). Conclusion: The expression level of Nrf2 mRNA in the peripheral blood mononuclear cells is significantly correlated with chemotherapy-induced myelosuppression. It may be employed as an ideal marker to predict the severity of chemotherapy-induced myelosuppression and helpful for clinical decision-making and early prevention and treatment of myelosuppression. ABSTRACT

Objective: To examine the expression of Nrf2 (nuclear factor erythroid 2-related factor 2) protein in patients with NSCLC (advanced non-small cell lung cancer), and to elucidate the correlation of the expression level of Nrf2 with response to platinum-based first-line chemotherapy and the prognosis of NSCLC patients. Methods: This retrospective study enrolled 50 patients with chemotherapynaïve advanced NSCLC. All patients received platinum-based regimens as first-line chemotherapy. The expression level of Nrf2 protein in tumor specimens was detected by immunohistochemical analysis. The associations between Nrf2 expression with response to chemotherapy and the prognosis were analyzed by SPSS 17.0 statistical software. Results: The Nrf2 protein expression level in NSCLC patients exhibited interindividual differences and the positive rate was 34% (17/50). The positive-expression rate of Nrf2 was higher in patients with age ≥ 70 years (58.3%, 7/12) than that in patients with age < 70 years (26.3%, 10/38) (P = 0.041), which was also higher in patients with distant metastasis (46.7%, 14/30) than that in patients without metastasis (15.0%, 3/20) (P = 0.021). The expression of Nrf2 was correlated with age (r = 0.370, P = 0.020), distant metastasis (r = 0.289, P = 0.042), response to platinum-based chemotherapy (r = 0.315 P = 0.026), progression-free survival (r = -0.393, P = 0.006) and overall survival (r = -0.309, P = 0.029). Furthermore, the expression of Nrf2 was an independent predictive factor for response to platinum-based first-line chemotherapy and progression-free survival (P < 0.05). Conclusion: Nrf2 expression is an independent predictor of response to platinum-based first-line chemotherapy and the prognosis of patients with advanced NSCLC, and it may be helpful to select the appropriate chemotherapy regimen for advanced NSCLC. ABSTRACT

<p><b>OBJECTIVE</b>To evaluate the efficacy and safety of thrombopoietin (TPO) on platelet engraftment in hematological malignancies patients after allogeneic haematopoietic stem cell transplantation (allo-HSCT).</p><p><b>METHODS</b>One hundred and twenty patients were enrolled in a multicenter, open-label, randomized, controlled clinical trial, and were randomized into 4 treatment groups following allo-HSCT. Group A was the control arm without TPO, while group B, C and D were trial arms with received 300 U×kg(-1)×d(-1) of TPO starting from day +1, +4 and +7, respectively. A total of 89 cases were evaluated, of which 22 cases in group A, 23 in group B, 20 in group C and 24 in group D. Efficacy evaluation (the time of platelet engraftment, the number of platelet transfusion) and safety evaluation \[adverse events, routine blood tests, liver and renal function, coagulation function and occurrence of graft-versus-host disease (GVHD)\] were observed.</p><p><b>RESULTS</b>The median platelet engraftment time in experimental groups (groups B, C and D) were on day (13.17 ± 2.89), day (12.15 ± 2.08), day (12.33 ± 1.76), respectively, and that in control group was on day (14.82 ± 5.05). There was statistically significant difference between two groups (P = 0.029), There were no statistically significant difference in the average amount of platelet transfusion, platelet engraftment time, and platelet nadir value among the 3 experimental groups. No significant adverse events were observed in experimental groups.</p><p><b>CONCLUSIONS</b>TPO administration following allo-HSCT for patients with hematologic malignancies appears to shorten platelet engraftment time. TPO given starting from day +7 is effective and safe.</p>

ABSTRACT

To investigate the effects of 3-hydroxy-3-methylglutaryl coenzyme A reductase inhibitor simvastatin (SV) on proliferation, apoptosis and the PI3K/AKT signaling pathway in human acute monocytic leukemia cell line SHI-1. SHI-1 cells were incubated with different concentrations of SV (5, 10, 15 µmol/L). Otherwise, SHI-1 cells without any treatment were used as control. Cells in different groups were collected at 24, 48 and 72 h after incubation for further detection. MTT method was used to assay the growth inhibition rate and flow cytometry was used to detect the early stage apoptosis ratio. The human PI3K-AKT Signaling Pathway RT(2) Profiler(TM) PCR Array was used to detect the expression of 84 genes involved in PI3K-AKT signaling. The results indicated that the SV inhibited the proliferation and inducted the apoptosis of SHI-1 cells in time- and dose-dependent manners significantly. The growth inhibition rates of SHI-1 cells treated with 15 µmol/L SV for 24, 48 and 72 h were 26.82, 47.09 and 63.92, respectively; and their early stage apoptosis ratios were 5.75, 13.25 and 15.59, respectively. Compared with the control group, expression levels of 39 genes were changed in the group of 15 µmol/L SV at 48 h, among them 26 genes were down-regulated and 13 genes were up-regulated. It is concluded that the SV can inhibit proliferation and induce apoptosis of SHI-1 cells, and the mechanism may be associated with the changes of gene expression level in PI3K-AKT signaling pathway regulated by SV.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate BCL-6, MYC and p53 genes abnormalities in diffuse large B-cell lymphoma (DLBCL) and correlate the result with immunosubtypes and prognosis.</p><p><b>METHODS</b>Interphase fluorescence in situ hybridization (I-FISH) was performed to detect the BCL-6, MYC and p53 genes. Immunohistochemistry (Envision method) was used to measure the expressions of CD3, CD10, CD20, BCL-6, MUM -1, BCL-2 and Ki-67 genes in DLBCL. The patients were classified into germinal center B cell-like (GCB) and non-GCB subtypes according to Hans' algorithm.</p><p><b>RESULTS</b>BCL-6 rearrangement was detected in 10 of 46 DLBCL cases. The presence of gene rearrangement had no correlation with BCL-6 protein expression (P= 0.245). Overall survival (OS, P= 0.138) and progression-free survival (PFS, P= 0.095) were not influenced by BCL-6 rearrangement. All MYC rearrangements were detected in GCB type DLBCL. Deletion of p53 gene was detected in 14 cases and was significantly associated with shorter OS (P= 0.046) and PFS (P= 0.043).</p><p><b>CONCLUSION</b>I-FISH is a rapid, accurate and sensitive method for detecting BCL-6, MYC and p53 abnormalities. No correlation was found between BCL-6 gene rearrangement and BCL-6 protein expression. MYC translocation was more common in GCB type DLBCL compared with non-GCB type ones. Patients with p53 deletion had a poorer prognosis. The p53 gene may provide a useful indicator for the prognosis of DLBCL.</p>
