ABSTRACT

ObjectiveTo investigate the diagnosis and treatment of familial hypokalemic periodic paralysis with acidosis. MethodsThe proband's medical history, clinical manifestations, laboratory examinations and imaging characteristics were retrospectively analyzed, and prevalence situation of family members was investigated in detail. Next generation sequencing technology was used to detect the pathogenic gene loci related to periodic paralysis, and the relevant literatures were summarized. ResultsThe proband was definitely diagnosed as familial hypokalemic periodic paralysis. There was a heterozygous mutation in the SCN4A gene of the proband, which was c.2006G>A, resulting in amino acid changes R669H.The proband's grandfather, father and uncle shared the same variation. ConclusionsFamilial hypokalemic periodic paralysis with paroxysmal acidosis is rare, which is easily misdiagnosed as renal tubular acidosis. c 2006G>A mutation in SCN4A gene is the molecular basis of the disease in this family. The clinical phenotypes of different gene mutations are different, and gene screening is helpful for diagnosis and treatment.

ABSTRACT

Objective To investigate the predictive value of quantitative flow ratio(QFR)on the occurrence of major adverse cardiovascular events(MACE)3 years after percutaneous coronary intervention(PCI)in patients with non-acute myocardial infarction.Methods This study included 139 patients with non-acute myocardial infarction who underwent PCI from January 2020 to June 2020 in the cardiac catheterization room of our hospital,all of them underwent post-PCI target vessel QFR measurements,and the incidence of MACE was followed up 3 years after PCI.The cut-off value of QFR was calculated by receiver operating characteristic(ROC)curve,according to which patients were divided into QFR>0.95 group and QFR≤0.95 group,and patients were divided into MACE group and non-MACE group depending on whether MACE occurs.The independent influencing factors of post-PCI QFR in patients with non-acute myocardial infarction were investigated by univariate and multifactorial linear regression analysis.Univariate and multivariate Cox regression analysis was used to investigate the predictive value of post-PCI QFR in the occurrence of MACE 3 years after PCI in patients with non-acute myocardial infarction.The long-term prognosis of patients with QFR>0.95 and those with QFR≤0.95 was compared by drawing the survival curve of no-MACE event.Results ROC curve analysis showed that post-PCI QFR predicted the occurrence of MACE 3 years after surgery in non-acute myocardial infarction patients with statistical significance(AUC 0.666,95%CI 0.556-0.777,P=0.003),and the cut-off value of MACE was 0.95.The sensitivity and specificity were 75.00%and 51.30%for the diagnosis of MACE with QFR≤0.95.Patients were divided into QFR≤0.95 group(n=74)and QFR>0.95 group(n=65)according to the cut-off value.Multivariate linear regression analysis showed that the maximum area stenosis rate after PCI was an independent factor for post-PCI QFR(P<0.001).Multivariate Cox regression analysis showed that body mass index(BMI),post-PCI QFR,three-vessel lesions and post-PCI QFR groups were independent influencing factors of no-MACE lifetime.The no-MACE survival curve showed that the long-term prognosis of patients in the QFR>0.95 group was significantly better than that in the QFR≤0.95 group(x2=5.272,P=0.022).Conclusions The optimal cut-off value of post-PCI QFR for predicting the occurrence of MACE 3 years after PCI in non-acute myocardial infarction patients was 0.95,and patients with QFR≤0.95 had worse long-term prognosis,and BMI,three-vessel lesions,and post-PCI QFR≤0.95 were independent risk factors for the occurrence of MACE 3 years in non-acute myocardial infarction patients after PCI.

ABSTRACT

Objective To investigate the application value of injectable autogenous tissue homogenate implantation in surgical treatment of secondary hyperparathyroidism(SHPT).Methods A total of 46 patients with SHPT who underwent long-term regular hemodialysis and failed medical treatment in Sichuan Mianyang 404 Hospital from June 2017 to June 2021 were selected.According to random number table method,the patients were divided into homogenate injection group and particle embedding group,with 23 cases in each group.Parathyroidectomy(PTX)plus autogenous forearm injection homogenate implantation was performed in homogenate injection group,and PTX plus autogenous forearm particle implantation was performed in particle embedding group.The changes in blood calcium,blood phosphorus,intact parathyroid hormone(iPTH),and alkaline phosphatase(ALP)of two groups before and after operation,as well as the improvements in vascular calcification,pruritus,bone pain,and sleep disorders after operation were compared.Results One year after surgery,the serum calcium level of two groups were significantly higher than before treatment,and the serum phosphorus,iPTH and ALP levels were significantly lower than before treatment(P<0.05).The serum calcium level in homogenate injection group was significantly higher than that in particle embedding group,and the serum phosphorus,iPTH and ALP levels were significantly lower than those in particle embedding group(P<0.05).The improvement of blood vessel calcification,pruritus,bone pain and sleep disorder in homogenate injection group were better than those in particle embedding group(P<0.05).Conclusion Injectable autologous tissue homogenate implantation for refractory SHPT patients is safe and effective,with high success rate,less trauma,and can improve patient symptoms and quality of life.

Wnt signaling pathway can regulate the proliferation and differentiation of neural stem cells, the growth and develop¬ment of neural cells, which is closely related to the therapeutic mechanism of cerebral ischemia-reperfusion injury (CIRI) after ischemic stroke.Researches have showed that some single Chi¬nese medical herb and Chinese medical herb compounds with the effects of tonifying Shen and nourishing Gan, supplementing Qi and nourishing Yin, promoting Xue and Qi, removing blood sta¬sis and resolving phlegm can activate the Wnt signaling pathway in brains and play the role of treating CIRI, and the essence of the treatment is according to the pathogenesis of CIHI which can be reduced by Yin deficiency of Gan and Shen, Qi and Xue de¬ficiency, blood stasis and phlegm toxin.This paper has summa¬rized the research results of Lycium chinensis, Cornus officinalis, turmeric, Salvia miltiorrhiza and other single Chinese medical herbs and Traditional Chinese Medicine(TCM) compounds such as Naoluoxintong, Danlong Xingnao formula, Yiqi Huoxue For¬mula, Bush en Shengsui formula and Huangjing pill in the treat¬ment of CIHI, regulating Wnt signaling pathway in brains.We also have analyzed that the essence of modern medicine of tonify-ing Shen and producing Sui, supplementing Qi and filling Jing in TCM is closely related to promoting cerebral angiogenesis and re¬modeling and stimulating the proliferation and differentiation of neural stem cells in specific areas of the brain after activating Wnt signaling pathway, and promoting blood circulation and re¬moving blood stasis, resolving phlegm and clearing toxin are re-lated to improving brain blood supply, anti-inflammatory, anti- free radical an ti ox id at ion and other functions.

ABSTRACT

Alzheimer's disease (AD) is a neurodegenerative disease that leads to progressive memory and cognitive impairment and behavioral disorders, which has seriously threatened the health of the majority of middle-aged and elderly people. Traditional Chinese medicine (TCM) believes that the basic pathogenesis of AD is deficiency of kidney-essence, blood stasis and meridian stagnation. In recent years, many studies have shown that TCM has obvious value and advantages in the prevention and treatment of AD by multi-target mechanism. Therefore, it is of great significance to screen out effective anti-AD drugs from TCM compound prescriptions. Huangjingwan, also known as Jiuzhuan Huangjingwan, has the effects in tonifying kidney-essence, activating blood and removing stasis, with a potential effect in preventing AD. In this article, the feasibility of Huangjingwan in the prevention and treatment of AD was analyzed and discussed from the perspective of TCM theory, the study results of Huangjingwan in the prevention and treatment of AD were summarized, and the mechanism of its action was analyzed from the perspective of pharmacological mechanism. Based on TCM theory, Huangjingwan has the effect of anti-AD. According to relevant findings, Huangjingwan has many targets, such as anti-oxidation, anti-inflammatory, decrease of the level of oxidative stress in brain, activation of Wnt/β-catenin signal transduction in brain, regulation of glycogen synthase kinase-3β (GSK-3β), protein phosphatase 2A (PP2A) activity balance, reduction of amyloid β (Aβ) content and tau protein hyperphosphorylation in brain, so as to exert effects in improving neurological symptoms and increasing learning and memory ability, with an anti-AD neuroprotective function. This will provide new ideas for in-depth studies and clinical applications of Huangjingwan against AD.

ABSTRACT

Objective:To treat mice with Alzheimer's disease (AD) with β-catenin RNA interference (RNAi) Huangjingwan (HW), so as to explore the neuroprotective signal mechanism of its prevention and treatment of AD. Method:A total of 81 male Kunming mice were randomly divided into normal control group, sham model control group, AD model group, Donepezil group, HW+scrambled group, HW+RNAi group, HW group, with 8 mice in each of donepezil group and HW group, and 13 mice in each of other groups. The AD models were established through injection with D-galactose and scopolamine in the last 5 groups for 5 consecutive weeks. On the 1st day of the 4th week after modeling, 0.75 μL PEI-LMW/β-catenin siRNAs nano-complex was injected into the right lateral ventricle of each mouse in for one time to treat with β-catenin RNAi in mice brains of the HW+RNAi group. The 0.75 μL complex was injected into the right lateral ventricle of each mouse for one time as for β-catenin interference control of the HW+scrambled group. The 0.75 μL normal saline was injected into the right lateral ventricle of each mouse in one time of the sham control group. Two weeks after intracerebroventricular injection, β-catenin RNAi was confirmed to be successful, and Donepezil (6.5×10-4 g·kg-1) was intragastrically administered to each mouse of donepezil group. HW (2.5 g·kg-1) was intragastrically administered to each mouse of HW group, HW+RNAi group and HW+scrambled group. Normal saline (0.5 mL·d-1) was intragastrically administered to each mouse of the sham control group. All gastric perfusion lasted for 4 weeks. At the end of gavage, the difference in learning and memory ability of mice was evaluated by platform jumping test. Nissl staining was used to count the number of neurons in s1Tr area of cerebral cortex and CA1 and CA3 areas of hippocampus of each mouse in each group. The mRNA expressions of Wnt1, DVL2, GSK-3β, β-catenin and CyclinD1 in mice brain of each group were detected by real-time fluorescence quantitative polymerase chain reaction (Real-time PCR). Western blot was used to detect the expressions of Wnt1, DVL2, GSK-3β, β-catenin and CyclinD1 in mice brain of each group. Result:The expression of β-catenin could be significantly inhibited through the injection with PEI-LMW/β-catenin siRNAs nano-complex into the lateral ventricle of AD mice, and nearly no β-catenin expression could be detected, which successfully achieved gene silencing. Compared with the normal control group, mice in AD model group showed that the learning and memory performance decreased significantly, the number of jumping errors increased (P<0.01), the number of neurons in S1Tr area of cerebral cortex and CA1, CA3 areas of hippocampus decreased significantly (P<0.01), the mRNA and protein expressions of Wnt1, DVL2, β-catenin, CyclinD1 in brain decreased significantly (P<0.01), while the mRNA and protein expressions of GSK-3β increased significantly (P<0.01). Compared with the AD model group, mice in HW group showed that the learning and memory performance increased significantly, the number of jumping errors decreased, the number of neurons in S1Tr area of cerebral cortex and CA1, CA3 areas of hippocampus increased significantly, the mRNA and protein expressions of Wnt1, DVL2, β-catenin, CyclinD1 in brain increased significantly, while the mRNA and protein expression of GSK-3β decreased significantly (P<0.01). Compared with the HW group, mice in HW+RNAi group showed that the learning and memory performance decreased significantly, the number of jumping errors increased significantly (P<0.01), the number of neurons in S1Tr area of cerebral cortex and CA1, CA3 areas of hippocampus decreased significantly (P<0.01), there was no significant change in mRNA and protein expressions of Wnt1, DVL2, GSK-3β in the brain, and the mRNA and protein expressions of β-catenin, CyclinD1 decreased significantly (P<0.01). Conclusion:HW can treat and prevent AD by activating Wnt/β-catenin signal pathway.

ABSTRACT

Objective:To explore the effect of Huangjingwan (HW) on the expressions of Wnt/β-catenin signal pathway-associated proteins in the hippocampus of mice with Alzheimer's disease (AD) induced by D-galactose and okadaic acid with learning and memory disorders, as well as its mechanism. Method:After subcutaneous injection with 1.0% D-galactose (0.14 g·kg-1·d-1) into the back and neck of mice for 4 weeks, the right ventricle of mice was injected with 2 μL(75 ng) of okadaic acid for one time to make AD model, and the successfully modeled AD mice were selected by Morris water maze. Then, the selected AD mice were randomly divided into AD model group, memantine group (1.3×10-3 g·kg-1·d-1) and HW group (2.5 g·kg-1·d-1). In addition, the sham model control group and the normal control group were set up. At the same time, 2 μL normal saline was injected into the right ventricle of mouse in the sham model control group as the modeling control. Two weeks after molding, the mice in the two experimental drug groups were given the corresponding dose of the experimental drug by gavage for 4 weeks. In addition, after 2 weeks of AD modeling, mice in sham model control group and AD model group were intragastrically administrated with the same amount of normal saline daily for 4 weeks. There was no special treatment in the normal control group. At the end of gavage, the shuttle experiment was performed to detect the differences in learning and memory levels of mice in each group. The changes of β-catenin and GSK-3β positive neurons in CA1 area of hippocampus in each group were tested by immunohistochemistry. Quantitative real-time polymerase chain reaction (Real-time PCR) was used to measure the mRNA expressions of GSK-3β, β-catenin and CyclinD1 in hippocampus of mice in each group. The Western blot was used to detect the expressions of total GSK-3β (t-GSK-3β), phosphorylation of GSK-3β at Ser9 (p-Ser9-GSK-3β), phosphorylation of GSK-3β at Tyr216 (p-Tyr216-GSK-3β), total β-catenin (t-β-catenin), phosphorylation of β-catenin (p-β-catenin) and CyclinD1 proteins in hippocampus of mice in each group. Result:Compared with the normal control group, mice in AD model group showed an obvious dementia state, which was characterized by significant declines in learning and memory ability, the number of β-catenin immunoreactive neurons in hippocampal CA1 area, the mRNA and protein expressions of t-β-catenin and CyclinD1, the protein expressions of p-Ser9-GSK-3β, and the ratio of p-Ser9-GSK-3β/t-GSK-3β and p-Tyr216-GSK-3β/t-GSK-3β in hippocampal region (P<0.01), and significant increases in the number of GSK-3β immunoreactive neurons in hippocampal CA1 area, the mRNA and protein expressions of t-GSK-3β, the protein expressions of p-Tyr216-GSK-3β and p-β-catenin, the ratio of p-β-catenin/t-β-catenin in hippocampal region (P<0.01 respectively). Compared with the AD model group, the dementia symptoms of mice in HW group were significantly alleviated, and the number of β-catenin immunoreactive neurons in hippocampal CA1 area, the mRNA and protein expressions of t-β-catenin and CyclinD1, the protein level of p-Ser9-GSK-3β, the ratio of p-Ser9-GSK-3β/t-GSK-3β in hippocampal region were all significantly increased (P<0.01 respectively), whereas the number of GSK-3β immunoreactive neurons in hippocampal CA1 area, the mRNA and protein expressions of t-GSK-3β, the proteins expressions of p-Tyr216-GSK-3β and p-β-catenin, the ratio of p-β-catenin/t-β-catenin in hippocampal region were all significantly decreased (P<0.01 respectively), but the ratio of p-Tyr216-GSK-3β/t-GSK-3β has no significant statistical difference. Conclusion:HW shows the role of AD treatment, which can down-regulate the expression of GSK-3β in the hippocampus of AD mice and reduce its protein activity, and up-regulate the expression of β-catenin as well as increase its protein activity, so as to enhance the expression of downstream CyclinD1 and promote the transcription of the target genes. Its mechanism may be related to the activation of Wnt/β-catenin signal pathway.

ABSTRACT

Objective:To investigate the effect of Huangjingwan (HW) on the activities of glycogen synthase kinase-3β (GSK-3β), protein phosphatase 2A (PP2A) and the mechanism in inhibiting tau protein hyperphosphorylation in the hippocampal neurons of mice with Alzheimer's disease. Method:After subcutaneous injection with 1.0% D-galactose (0.14 g·kg-1·d-1) into the back and neck of mice for 4 weeks, the right ventricle of mice was injected with 2 μL (75 ng) of okadaic acid for one time to make AD model, and the successfully modeled AD mice were selected by Morris water maze. Then, the selected AD mice were randomly divided into AD model group, memantine group (1.3×10-3 g·kg-1·d-1) and HW group (2.5 g·kg-1·d-1). In addition, the sham model control group and the normal control group were set up. At the same time, 2 μL normal saline was injected into the right ventricle of mouse in the sham model control group for modeling control. Two weeks after modeling, the mice in the two experimental drug groups were given the corresponding dose of the experimental drug by gavage for 4 weeks. In addition, after 2 weeks of AD modeling, mice in control group and AD model group were intragastrically administrated with the same amount of normal saline daily for 4 weeks. The mice in normal control group were only given daily feed. At the end of gavage, all the mice were tested by the open field experiment and jumping platform experiment to evaluate the differences in exploratory activity ability, anxiety level and learning and memory ability. The number of neurons in CA1 and CA3 areas of hippocampus in all the mice was detected by Nissl staining. Quantitative real-time polymerase chain reaction (Real-time PCR) was used to detect mRNA expressions of GSK-3β and PP2A in hippocampus of mice in each group. Protein expressions of GSK-3β, PP2A, phosphorylated tau (p-tau) and total tau protein (t-tau) in hippocampus of mice in each group were detected by Western blot. Result:Compared with the normal control group, mice in AD model group showed an obvious dementia state, which was characterized by a lower spontaneous activity, lower exploration behavior ability, higher anxiety level, less movement and easier to stay and hide, longer learning response time, significantly increased number of learning and memory errors, and decreased numbers of hippocampal neuron in CA1 and CA3 areas, and reduced mRNA and protein expressions of PP2A, mRNA and protein expressions of GSK-3β, p-tau protein and the ratio of p-tau/t-tau were all increased significantly (P<0.01), while expression of t-tau protein was decreased, with no significant difference. Compared with the AD model group, mice in the HW group showed a higher spontaneous activity, higher exploration ability, lower anxiety level, higher learning and memory performance, and the numbers of hippocampal neuron in CA1 and CA3 areas increased, while mRNA and protein expressions of PP2A increased, and the mRNA and protein expressions of GSK-3β, the expression of p-tau protein and the ratio of p-tau/t-tau were all decreased significantly (P<0.01), but with no significant difference in the protein expression of t-tau. Conclusion:HW can inhibit tau hyperphosphorylation in hippocampal neurons of AD mice, restore tau protein function, protect hippocampal neurons, and exert an anti-AD effect, which may be related to the regulatory mechanism in the activity balance between GSK-3β and PP2A in hippocampal neurons.

ABSTRACT

Objective::To observe the effect of Huangjingwan (HW) on antioxidant functions and β-amyloid 1-42 (Aβ1-42) and amyloid precursor protein (APP) expressions in the brain of Alzheimer' s disease (AD) rats. Method::SD rats were randomly divided into normal control group, sham model control group, AD model group, and low, medium, high-dose (equivalent raw drug dose 1, 3, 9 g·kg-1·d-1) HW groups.The AD models were established through intraperitoneal injection with 1.25% D-galactose (120 mg·kg-1·d-1, 6 consecutive weeks) and then one-time right ventricular injection with Aβ1-42 (10 μg). Two weeks after modeling, the rats in each HW group received corresponding drugs through intragastric administration, once a day, while the rats in sham model control group, AD model group were given normal saline 1 mL through intragastric administration, once a day.Gastric perfusion lasted for 8 weeks.At the end of the experiment, learning and memory abilities of the rats were assessed by Platform Jumping Test.The changes of physical endurance in rats were tested by 10% weight swimming under load.The activities of superoxide dismutase (SOD), glutathione reductase (GR), glutathione peroxidase (GSH-Px) antioxidant enzymes and the contents of glutathione (GSH) and malondialdehyde (MDA) in rat brain tissue were detected by colorimetry.The changes of interleukin-1β (IL-1β), tumor necrosis factor-α (TNF-α), Aβ1-42 and APP protein in rat brain tissues were determined by enzyme linked immunosorbent assay (ELISA). Western blot analysis was used to measure the expression of APP protein in rat brain. Result::Compared with the normal control group, rats in AD model group showed an obvious dementia state, that is more lying and less movement, longer learning response time, significant increase in the number of learning and memory errors, significant attenuation in physical fitness, significant decrease in the activities of antioxidant enzymes (SOD, GR, GSH-Px) and anti-inflammatory factors GSH in brain, significant rise in the levels of inflammatory factors MDA, IL-1β and TNF-α and the content of Aβ1-42 protein, and significant reduction in the content of APP protein in brain (P<0.01). Low, medium and high-dose HW could ameliorate dementia symptoms in AD rats, improve the achievement of learning and memory, antagonize body weakness and increase physical fitness, promote SOD, GR, GSH-Px activities and anti-inflammatory factor GSH level in the brain, reduce the levels of MDA, IL-1β and TNF-α in the brain, decrease the level of Aβ1-42 and increase the level of APP protein in the brains of AD rats compared with the AD model group (P<0.05, P<0.01), besides, within the dose range of 1-9 g·kg-1·d-1, HW has a more obvious effect with the increase of dose. Conclusion::HW has the effects in preventing and treating AD, which is related to the HW' s mechanisms in enhancing the function of antioxidant system in brain, reducing neuroinflammatory reaction and deposition of Aβ1-42 induced by oxidative stress, and maintaining the expression level of APP protein.

Replacement of the native promoter of theglobal regulator LaeA-like gene of Daldinia eschscholzii by a strong gpdA promoter led to the generation of two novel cyclopentenone metabolites, named dalestones A and B, whose structures were assigned by a combination of spectroscopic analysis, modified Mosher's reaction, and electronic circular dichroism (ECD). Dalestones A and B inhibit the gene expression of TNF-α and IL-6 in LPS-induced RAW264.7 macrophages.

Objective To compare the curative effect of fixation of adolescent tibial intercondylar emi-nence fracture among suture anchor,hollow screw and wire. Methods Forty-six adolescent cases of the tibial intercondylar eminence fracture treated with surgical treatment were selected from January 2010 to June 2016 and divided into three groups refer to intra-operative fixation suture anchor group(Group A),hollow screw group (Group B)and wire group(Group C).Duration of treatment,total operation time,hospital stay and surgery times were recorded. All patient condition was assessed with the Lysholm,Tegner,IKDC and VAS score. Results All patients received an average of 13(11~14)months follow-up visit. No blood-vessel,nerve and osteoepiphysis injured,infection and fracture displacement occurred.Before receiving treatment,difference in Lysholm,Tegner, IKDC and VAS score of group A,B and C showed no statistical difference.When it comes to hospitalization condi-tion,data were as follows.Group A/B/C:operation time(80.67 ± 16.68/114.00 ± 20.28/111.88 ± 20.07)min, hospital stay(8.40 ± 1.12/ 15.47 ± 1.25/ 15.19 ± 1.17)d,surgery times(1/2/2)times. Moreover,compared with those before operation and after operation in both of groups,the Lysholm,Tegner,IKDC and VAS score were improved(P < 0.05). Besides,the Lysholm,Tegner,IKDC and VAS score of group A,B and C did not have statistically significant difference yet after post treatment(P>0.05).Conclusions The curative effect of fixation of adolescent tibial intercondylar eminence fracture among suture anchor,hollow screw and wire was similar. By contrast,the fixation of fracture by use of suture anchor can decrease operation time and hospital stay to some extent. It had advantage of need not to have a second operation to remove the internal fixation and can be used in preference.

Objective To evaluate the safety and efficacy of epicardial ventricular restoration (EVR) using REVIVENT system in patients with antero-septal scar and dilated ischemic cardiomyopathy. Methods Ten ischemic heart patients with antero-septal scar underwent the operation. The scarred lateral left ventricular wall was apposed to the septal scar with serial paired anchors placed through epicardial transmural excluding the non-viable portions of the chamber. Left ventricular hemodynamic assessments as well as left ventricular ejection fraction, left ventricular end-systolic/diastolic volume (LVEDV/LVESV) and their indexes (LVEDVI/LVESVI) were measured by cardiac magnetic resonance (CMR). Results Ten ischemic heart failure patients with antero-septal scar, aged(55.2±13.9)years, received a hybrid epicardial ventricular restoration. Cardiac MR done at one a month after the procedure showed an elevation of LVEF from(27.8±4.6%)to(37.5±11.4)% (+35%, P<0.01). LVESV was significantly reduced from(149.9±61.6) ml to(109.9±58.0)ml (–26.7%, P<0.01), LVESVI was reduced from(84.8±36.7)ml/m2to(63.0±34.2) ml/m2(reduced by 25.7%, P<0.01); LVEDV was reduced from(203.0±64.0)ml to(167.9±58.2)ml (reduced by 17.3%, P<0.01), and LVESV was reduced from(114.5±37.8)ml/m2to(96.2±35.2)ml/m2(reduced by 16.0%, P<0.01). Cardiac output (CO) increased from(4.0±1.5)L/min to(4.8±1.2)L/min(increased by 20.0%, P=0.034) and cardiac index (CI) increased from(2.2±0.7)L/(min ? m2) to(2.7±0.7)L/(min ? m2) (increased by 22.4%, P=0.023). Conclusions Our preliminary experience on EVR using the REVIVENT system demonstrated signifi cant increase in LVEF, CO and CI, with decreases in LVEDV/LVESV at 1 month following the procedure. Its feasibility and safety need further evaluation in the future.

ABSTRACT

Objective To evaluate the preliminary experience and mid-term outcome of transcatheter aortic valve replacement(TAVR)in patients with calcifi ed severe aortic stenosis.Methods From December 2014 to February 2016, 10 TAVR cases were admitted in the Cardiovascular Hospital, Xiamen University. The baseline characteristics, echocardiography parameters and clinical follow-up data were analyzed. Results All cases were complicated with impaired heart function(NYHAⅢ in 4 cases, NYHA Ⅳ in 6 cases). The mean age was (75.1±6.2) years and 4(4/10) of them were men. The mean logistic EuroSCORE was (27.2±23.6) % and the mean society of thoracic surgery (STS) mortality score was (9.1±4.8) %. Five cases had bicuspid aortic valve. TAVR was successfully performed in all 10 patients, and valve-in-valve implantation was done in 1 (10%) case. Immediately after procedure, the peak trans-aortic valve pressure gradient decreases from (85.9±22.7) mmHg to (23.2±5.4) mmHg. One case had marginal moderate periprosthetic leak and one case received stent implantation for femoral artery complication during the procedure. During hospitalization, 1 case had blood transfusion for gastrointestinal bleeding and permanent pacemakers were implanted in 2 (2/10) cases. The survival rate was 10/10 at 30 days after TAVR. One case with end-stage renal disease died for gastrointestinal bleeding 36 days after TAVR. For the other 9 patients, 12 months echocardiography data showed that the peak and mean trans-aortic valve gradient was (20.0±5.2) mmHg and (10.6±3.1) mmHg respectively. The lef t ventricular diastolic diameter(LVDD)decreased[(56.5±9.4)mm vs.(51.8±7.6)mm,P=0.035] and left ventricular ejection fraction(LVEF)increased significantly[(46.9±22.2)% vs.(63.7±9.4)%, P=0.029].To date,median follow-up period was(22.0±4.8)month.Clinical symptoms improved in all the 9 cases. The patient with periprosthetic leak had record of hospitalization for several times due to heart failure. Conclusions From the initial TAVR experience of our hospital, TAVR can be done safely and smoothly after strictly TAVR candidate cases selection.

ABSTRACT

OBJECTIVE:To compare the clinical efficacy and safety of Jianpi shengxue tablets and Iron polysaccharide complex capsules in the treatment of nondialysis renal anemia. METHODS:A total of 60 nondialysis renal anemia patients in our hospital during Mar. 2016 to Mar. 2017 were divided into control group(30 cases)and observation group(30 cases)with random allocation concealment method according to random number and admission order. Both groups received routine treatment as rhEPO injection,Folic acid tablets,Vitamin B12 tablets. Based on it,control group was given Iron polysaccharide complex capsules 0.15 g orally,once a day;observation group was given Jianpi shengxue tablets 1.8 g orally,3 times a day. Both groups were treated for 12 weeks. Clinical efficacies were compared between 2 groups. The levels of Hb,RBC,HCT and Ret% were observed before treatment and 2,4,8,12 weeks after treatment;the levels of SI,SF and TS were also observed before treatment and 8,12 weeks after treatment. The occurrence of ADR was recorded. RESULTS:Both groups completed the treatment. The total effective rate of observation group(86.67%)was significantly higher than control group(63.33%),with statistical significance(P<0.05). Before treatment,there was no statistical significance in the levels of Hb,RBC,HCT,Ret%,SI,SF or TS between 2 groups (P<0.05). After treatment,the levels of above indexes in 2 groups were significantly higher than before treatment,and observation group was significantly higher than control group(except for Ret% at 8th week),with statistical significance(P<0.05). The case number of black stool and rust colored stool in observation group were significantly lower than control group,while the incidence of black-dyed teeth in observation group was significantly higher than control group, with statistical significance (P<0.05). CONCLUSIONS:Therapeutic efficacy of Jianpi shengxue tablets are significantly better than Polysaccharide iron complex capsules in the treatment of nondialysis renal anemia,and can significantly improve iron reserve and anaemia. But Jianpi shengxue tablets causes high incidence of black-dyed teeth.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate whether Lactobacillus rhamnosus GG conditioned medium(LGG-CM)has preventive effect against E. coli K1-induced neuropathogenicity in vitro by inhibiting nuclear factor-κB (NF-κB) signaling pathway.</p><p><b>METHODS</b>An in vitro blood-brain barrier (BBB) model was constructed using human brain microvascular endothelial cells (HBMECs). The effect of LGG-CM on E. coli-actived NF-κB signaling pathway was assayed using Western blotting. Invasion assay and polymorphonuclear leukocyte (PMN) transmigration assay were performed to explore whether LGG-CM could inhibit E. coli invasion and PMN transmigration across the BBB in vitro. The expressions of ZO-1 and CD44 were detected using Western blotting and immunofluorescence. The changes of trans-epithelial electric resistance (TEER) and bacterial translocation were determined to evaluate the BBB permeability.</p><p><b>RESULTS</b>Pre-treament with LGG-CM inhibited E. coli-activated NF-κB signaling pathway in HBMECs and decreased the invasion of E. coli K1 and transmigration of PMN. Western blotting showed that LGG-CM could alleviate E. coli-induced up-regulation of CD44 and down-regulation of ZO-1 expressions in HBMECs. In addition, pre-treatment with LGG-CM alleviated E. coli K1-induced reduction of TEER and suppressed bacterial translocation across the BBB in vitro.</p><p><b>CONCLUSION</b>LGG-CM can block E. coli-induced activation of NF-κB signaling pathway and thereby prevents E. coli K1-induced neuropathogenicity by decreasing E. coli K1 invasion rates and PMN transmigration.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the clinical outcomes of limited open reduction and percutaneous K-wire internal fixation for the treatment of irreducible Gartland type III humerus supracondylar fracture in children.</p><p><b>METHODS</b>From May 2006 to October 2014, 132 patients with irreducible Gartland type III humerus supracondylar fracture were treated with reduction and percutaneous K-wire internal fixation. The reduction was performed with the guiding of surgeon's finger, and the lateral approach with periosteum torn was chosen according to the shift direction of the distal fractures. Among them, there were 82 males and 50 females with an average age of 5.8 years old(ranged from 2 to 14 years old).</p><p><b>RESULTS</b>All the patients were followed up, the duration ranged from 6 to 36 months, with an average of 13.7 months. Ninety-five patients got an excellent result, 27 good, 8 fair, and 2 poor.</p><p><b>CONCLUSIONS</b>Limited open reduction and percutaneous K-wire internal fixation for the treatment of irreducible Gartland type III humerus supracondylar fracture in children has many advantages: simple manipulate, not affected by the elbow swelling, and satisfactory curative effect. It is worth popularizing in clinic.</p>

ABSTRACT

Objective To evaluate the effects of Achilles tendon lengthening on talipes equinus in children with spastic cerebral palsy. Methods From December, 2013 to June, 2014, seventeen spastic cerebral palsy children with talipes equinus (34 feet) received Achilles ten-don lengthening. Ankle dorsiflexion range of motion (ROM) and surface electromyography from tibialis anterior and medial head of gastroc-nemius were measured before and 8 to 12 months after operation, respectively. ROM of passive and active dorsiflexion, root mean square (RMS) of tibia muscle group and co-contraction ratio (CR) when standing were compared. Results The ROM of ankle passive and active dorsiflexion increased (Z>4.867, P0.05), while CR reduced (t=2.38, P<0.05). Conclusion Achilles tendon lengthening can improve the coordination of tibia muscle group to increase the ROM of ankle for chil-dren with talipes equinus after spastic cerebral palsy.

Objective To investigate the current working situation of specialist nurses in China via questionnaire survey among 873 specialist nurses.Methods Eight hundred and seventy-three specialist nurses in 474 hospitals in 30 provinces(cities or autonomous regions) in China were investigated using a self-designed questionnaire on SO JUMP platform.Results One hundred and ninety-seven (22.6%) specialist nurses worked on nursing management positions.One hundred and fifty (17.2%) specialist nurses worked in other places besides their own wards,121 (13.9%) undertook nursing consultation work,58 (6.6%) worked in specialist nursing outpatient clinics,and 528 (60.5%) worked as clinical part-time teachers.One hundred and seventy(19.5%) specialist nurses undertook scientific research tasks,and 129(14.8%) published papers after attending the training courses.Conclusion Specialist nurse position is paying increasing attention to clinical practice;the working sites have been expanded,the working contents have been richened,and specialist working capacity has been improved.

ABSTRACT

<p><b>BACKGROUND</b>Proteasome subunits (PSMB) and transporter associated with antigen processing (TAP) loci are located in the human leukocyte antigen (HLA) Class II region play important roles in immune response and protein degradation in neurodegenerative diseases. This study aimed to explore the association between single nucleotide polymorphisms (SNPs) of PSMB and TAP and Parkinson's disease (PD).</p><p><b>METHODS</b>A case-control study was conducted by genotyping SNPs in PSMB8, PSMB9, TAP1, and TAP2 genes in the Chinese population. Subjects included 542 sporadic patients with PD and 674 healthy controls. Nine identified SNPs in PSMB8, PSMB9, TAP1, and TAP2 were genotyped through SNaPshot testing.</p><p><b>RESULTS</b>The stratified analysis of rs17587 was specially performed on gender. Data revealed that female patients carry a higher frequency of rs17587-G/G versus (A/A + G/A) compared with controls. But there was no significant difference with respect to the genotypic frequencies of the SNPs in PSMB8, TAP1, and TAP2 loci in PD patients.</p><p><b>CONCLUSION</b>Chinese females carrying the rs17587-G/G genotype in PSMB9 may increase a higher risk for PD, but no linkage was found between other SNPs in HLA Class II region and PD.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To study the effect of acupuncture on the immune function of sepsis patients.</p><p><b>METHODS</b>Ninety sepsis patients were assigned to the control group, the thymosin a1 group, and the acupuncture treatment group according to random digit table, 30 patients in each group. Patients in the control group were treated according to the guideline of Surviving Sepsis Campaign (SSC). Patients in the control group received routine treatment. Those in the thymosin alpha1 group additionally received subdermal injection of thymosin alpha1 (1.6 mg), once per day for 6 successive days. Needling at related points such as Zusanli (ST36), Yanglingquan (GB34), Neiguan (PC6), Guanyuan (RN4), and so on, was performed in patients of the acupuncture treatment group, once per day for 6 successive days. T cell subgroups (CD3+, CD4+, CD8+, CD4+ /CD8+) and immunoglobulin levels (IgG, IgA, IgM) were detected. The length of ICU hospital stay, hospital readmission rate, and 28-day mortality were compared among the three groups.</p><p><b>RESULTS</b>After six days of treatment, CD3+, CD4+, CD8+, IgG, IgA, IgM, and CD4+ /CD8+ ratio of three groups were all significantly increased (P < 0.01). Of them, CD3+, CD4+, CD8+, IgG, IgA, and IgM increased more significantly in the thymosin alpha1 group and the acupuncture treatment group (P < 0.01). Compared with the control group, the ICU hospitalization length was significantly shortened, the hospital readmission rate and the 28-day mortality were lower in the thymosin alpha1 group and the acupuncture treatment group (P < 0.05, P < 0.01). There was no statistical difference in each index between the thymosin alpha1 group and the acupuncture treatment group (P > 0.05).</p><p><b>CONCLUSION</b>Acupuncture could adjust the immune function of sepsis patients, improve their immunological indicators and prognoses.</p>
