ABSTRACT

Leptin receptor overlapping transcript (LepROT) plays multiple roles in the regulation of immune systems. However, very little information is available about the anti-infectious mechanisms of amphibians LepROT. In this study, the cDNA sequence of the Rana dybowskii LepROT gene was determined by using RT-PCR and bioinformatics analysis. Then, the Aeromonas hydrophila (Ah) and lipopolysaccharides (LPS) infected models of R. dybowskii was constructed to obtain histopathological characteristics. Constitutive expression of LepROT mRNA and NF-κB signaling pathway were detected by real-time quantitative PCR. The full-length cDNA of LepROT gene was 396 bp and encoded 131 amino acids. Amino acid sequence analysis revealed LepROT shares 93.74% and 86.39% identity with homologues from other amphibians and mammals respectively, and the LepROT gene was quite conserved among different species. After infection, the relative expression levels of LepROT, NF-κB, IKKα and IKKβ mRNA were all significantly upregulated (P < 0.01), but showed a diverse temporal pattern of up-regulation in different tissues. Therefore, it was proposed that the LepROT gene of R. dybowskii might activate the NF-κB signaling pathway to exert anti-infectious effects, thus providing evidence for further extending the biological function of LepROT.

ABSTRACT

Inflammatory bowel disease (IBD) is a non-specific inflammatory disease of the gastrointestinal (GI) tract that is generally accepted to be closely related to intestinal dysbiosis in the host. GI infections contribute a key role in the pathogenesis of IBD; however, although the results of recent clinical studies have revealed an inverse correlation between Helicobacter pylori (H. pylori) infection and IBD, the exact mechanism underlying the development of IBD remains unclear. H. pylori, as a star microorganism, has been a focus for decades, and recent preclinical and real-world studies have demonstrated that H. pylori not only affects the changes in the gastric microbiota and microenvironment but also influences the intestinal microbiota, indicating a potential correlation with IBD. Detailed analysis revealed that H. pylori infection increased the diversity of the intestinal microbiota, reduced the abundance of Bacteroidetes, augmented the abundance of Firmicutes, and produced short-chain fatty acid-producing bacteria such as Akkermansia. All these factors may decrease vulnerability to IBD. Further studies investigating the H. pylori-intestinal microbiota metabolite axis should be performed to understand the mechanism underlying the development of IBD.

ABSTRACT

Objective:To obtain the inter-fractional set-up errors of intensity-modulated radiotherapy (IMRT) of cervical cancer by cone-beam CT (CBCT), and to analyze the variations of the set-up errors on the cumulative dose deviation of the target volume. Methods:A total of 48 patients with cervical cancer who underwent IMRT were enrolled in this study, and the set-up errors of 696 CBCTs were obtained. The set-up errors were input into the treatment planning system, and the cumulative set-up error dose was obtained by superposing the set-up errors dose. The deviation percentage was calculated by the deviation formula and the standard planning dose. Results:The set-up errors caused the offset of isocenter distance by 0.58(0.36, 0.80) cm. Different statistical differences were noted between the cumulative set-up error dose and the standard planning dose by WilCoxon test. All the dose deviations in the target volume were reduced, and the differential dose volume histogram (DVH) appeared negatively skewed, and the peak value was declined. The DVH diagram shifted to the left with an inverse S-curve and the slope was increased. The HI deviation of the target volume from small to large were: CTV 1, CTV 2, GTV/CTV, CTV 3, CTV n, CTV all, and GTV nd; The HI deviation of the target volume were increased. Conclusions:The effect of set-up errors in IMRT of cervical cancer upon the cumulative doses of the target volume significantly differs. The cumulative dose of the target volume is reduced, and the uniformity of the target volume dose becomes worse. The uncertainty of the inter-fractional position leads to an increase or decrease in the the fractional doses of the target volume. The biological effect on tumor cells and the tumor recurrence remains to be investigated. In IMRT of cervical cancer, the CBCT position calibration is required before each treatment to ensure the dose accuracy of each structure in the target volume.

ABSTRACT

Objective:To investigate effects of metformin and rosiglitazone in non-obese polycystic ovary syndrome (PCOS) women with insulin resistance. Methods:Totally 200 non-obese PCOS women with insulin resistance in West China Second Hospital of Sichuan University were enrolled into this study from Sep. 2013 to Jun. 2016, and were randomly divided into two treatment groups: metformin group (1 500 mg/d) and rosiglitazone group (4 mg/d). The treatment lasted for 6 months. Their clinical and biochemical parameters were collected and compared. Results:In both groups, menstrual cycles [metformin group (37±4) days, rosiglitazone group (35±4) days] were shorter after treatment for 6 months (both P<0.01). After treatment for 6 months, body mass index [metformin group (21.6±1.6) kg/m 2, rosiglitazone group (21.7±1.7) kg/m 2] decreased in both groups (both P<0.01); decreased LH/FSH ratio (metformin group 1.67±0.80, rosiglitazone group 1.70±0.83) was also observed (both P<0.05). After treatment for 6 months, fasting insulin level [metformin group (13.5±5.1) mU/L, rosiglitazone group (12.7±5.6) mU/L] and homeostasis model assessment-insulin resistance index (metformin group 3.0±1.2, rosiglitazone group 2.8±1.2) were decreased in both groups (all P<0.01). Conclusions:For non-obese PCOS insulin resistance patients, screening of anthropometric and metabolic parameters is necessary. For PCOS with insulin resistance, lifestyle plus insulin sensitizers such as metformin could improve their clinical symptoms, correct the biochemical and metabolic dysfunction.

ABSTRACT

Objective:To analyze the feasibility and efficacy of ultrasound-guided percutaneous biopsy and radiofrequency ablation of gallbladder polyps. Methods:From April 2019 to January 2021, 25 patients with gallbladder polyps underwent ultrasound-guided biopsy and radiofrequency ablation in the ultrasound department of the First Affiliated Hospital of Zhengzhou University were collected, the maximum diameter of the lesion was 11.00(10.00, 12.50)mm. Under general anesthesia, ultrasound-guided injection of water into the gallbladder serosa layer was performed to make the gallbladder edema thicker than or equal to 10 mm. Percutaneous biopsy and radiofrequency ablation of gallbladder polyps were performed to ablate the gallbladder mucosa layer of polyps and polyp attachment.Intraoperative contrast-enhanced ultrasound was used to evaluate the need for supplementary ablation. The operation time, intraoperative and postoperative conditions were recorded. The complete ablation rate of gallbladder polyps, the reduction rate of lesion volume after ablation, the incidence of complications and the effect of operation on gallbladder wall thickness and gallbladder contraction rate were evaluated. Results:Biopsy and radiofrequency ablation of gallbladder polyps were successfully performed in 25 patients. There were 14 cases of cholesterol polyp, 5 cases of adenoma, 5 cases of inflammatory polyp and 1 case of gallbladder adenomyosis. Twenty-nine gallbladder polyps showed changes after ablation, and the lesion volume was reduced to varying degrees. In the first, third and sixth months, the volume reduction rates of ablation focus were 70.74%(58.55%, 77.56%), 89.47%(85.04%, 96.87%) and 100%(95.68%, 100%) respectively, and the differences were statistically significant ( P<0.05). There were no significant differences in gallbladder wall thickness and gallbladder contraction rate before and 1 month after operation ( P>0.05). The operation time was 14-39(23.32±6.68)min. During the operation, 3 patients(12.0%) had a decrease in heart rate, 2 patients(8.0%) had mild abdominal pain and 1 patient(4.0%) had obvious abdominal pain, which was relieved after treatment. No bleeding, gallbladder perforation, abdominal infection and other complications occurred. All patients were followed up for 1 to 22 months, with a median of 6 (3, 7) months. No patients were lost or follow-up, polyp recurrence, or new polyps, or secondary gallstone. Conclusions:Ultrasound-guided biopsy and radiofrequency ablation of gallbladder polyps is a feasible choice for gallbladder preserving treatment of gallbladder polyps with low complication rate.

ABSTRACT

Objective:To explore the influencing factors of massive blood transfusion during liver transplantation and establish a prediction model for massive blood transfusion. Methods:Clinical data were analyzed retrospectively for 103 patients undergoing liver transplantation performed by the same surgical team of our hospital in 2018 from Department of Blood Transfusion Affiliated Drum Tower Hospital Nanjing University Medical School. According to the intraoperative volume of red blood cells, they were divided into two groups of massive transfusion (≥12U, n=40) and non-massive transfusion (<12U, n=63). General patient data and preoperative laboratory parameters were compared between two groups. Logistic regression analysis was utilized for constructing a prediction model for massive transfusion during liver transplantation. Results:No significant inter-group difference existed in gender, age or blood group ( P>0.05) while preoperative diagnosis, hemoglobin (Hb), hematocrit (HCT), platelet count (PLT), international normalized ratio (INR), prothrombin time (PT), activated partial thromboplastin time (APTT), thrombin time (TT), total bilirubin (TBIL), direct bilirubin (DBIL) and end-stage liver disease (MELD) score were correlated with intraoperative massive blood transfusion ( P<0.05). By Logistic regression analysis, the predictive model of massive blood transfusion in liver transplantation was Y = 3.545-0.112 × HCT-0.009 ×P LT + 0.005×DBIL. The area under ROC curve was 0.813 and the sensitivity and specificity 80.0% and 71.4% respectively. Youden's index of 0.514 indicated that if Y value was greater than or equal to 0.514, massive blood transfusion might occur. Conclusions:Preoperative HCT, PLT and DBIL may be employed as independent predictors of intraoperative massive transfusion during liver transplantation. The prognostic model has demonstrated an excellent sensitivity and specificity.

ABSTRACT

Hyperglycemia is considered to be a predictor of death, poor functional outcome, symptomatic intracerebral hemorrhage, and increased infarct volume in patients with acute ischemic stroke. Many studies have used parameters such as standard deviation, coefficient of variation, blood glucose level, stress hyperglycemia ratio, and glucose/glycated hemoglobin ratio to express hyperglycemia, and then study the relationship between hyperglycemia and stroke outcome. In patients with stroke receiving intravenous thrombolysis, hyperglycemia is closely associated with symptomatic intracerebral hemorrhage, poor neurological outcome, and lower vascular recanalization rate. In patients with stroke undergoing endovascular mechanical thrombectomy, hyperglycemia is closely associated with poor long-term outcome, increased mortality, and increased infarct volume. However, whether hyperglycemia will affect the effect of reperfusion therapy in acute ischemic stroke remains controversial. The possible mechanisms of hyperglycemia leading to poor outcomes after reperfusion therapy in patients with ischemic stroke include inhibition of vasodilation, reduction of plasma fibrinolytic activity, destruction of the blood-brain barrier, promotion of oxidative stress, and reduction of ischemic penumbra volume. Studies have shown that for stroke patients with hyperglycemia, intensive blood glucose control does not bring significant benefits. Therefore, the blood glucose control program of patients with acute ischemic stroke and hyperglycemia needs further study and discussion.

ABSTRACT

Objective:To understand the knowledge and behavior changes of pregnant women on iodine deficiency disorders (IDD) prevention and treatment in iodine deficiency areas in Anhui Province before and after the implementation of the intervention measures, and to provide a scientific basis for pregnant women's iodine nutrition improvement. Methods:From March to December 2018, from Lujiang County, an iodine deficiency area in Anhui Province, Lucheng and Nihe towns were selected as the survey sites. Relying on the township health centers, pregnant women in early pregnancy (≤12 weeks) were selected as the survey subjects, and long-term follow-up was conducted. The edible salt samples of pregnant women in early pregnancy were collected and salt iodine content was detected by direct titration method. The urine samples of pregnant women in the morning in early, middle (13 - 28 weeks) and late pregnancies (≥29 weeks) were collected, urinary iodine content was determined by arsenic-cerium catalytic spectrophotometry. Baseline questionnaire survey was conducted for pregnant women in early pregnancy, mainly including basic information, IDD prevention and treatment knowledge (pregnant women prone to iodine deficiency, the harm of iodine deficiency in pregnant women, suitable iodine supplement methods for pregnant women and foods with high iodine content), and the consumption frequency of iodine-rich foods. After the baseline survey, the knowledge publicity on IDD prevention and treatment was carried out in townships, and iodine-rich foods such as kelp and laver were recommended to supplement iodine. The intervention activities lasted for 6 months, and retrospective questionnaire survey was conducted on pregnant women in late pregnancy. Results:A total of 128 edible salt samples were collected from the families of pregnant women in early pregnancy, and the median salt iodine was 21.5 mg/kg. The iodized salt coverage rate was 99.2% (127/128), the qualified rate of iodized salt was 98.4% (125/127), and the consumption rate of qualified iodized salt was 97.7% (125/128). A total of 129, 95 and 70 urine samples were collected from pregnant women in early, middle and late pregnancies, the medians urinary iodine were 179.0, 185.5 and 189.7 μg/L, respectively, all of which were at the appropriate iodine level. The total awareness rates of IDD prevention and treatment before and after intervention were 22.4% (28/125) and 64.6% (82/127), respectively, and the difference was statistically significant (χ 2 = 45.538, P < 0.01). Compared with the awareness rates before the intervention, the awareness rates of the harm of iodine deficiency in pregnant women, suitable iodine supplement methods for pregnant women and foods with high iodine content were all higher after the intervention ( P < 0.01). There were statistically significant differences in the frequency of eating kelp, laver and other iodine-rich foods among pregnant women in early, middle and late pregnancies (χ 2 = 163.170, 102.373, P < 0.01). Before the intervention, 57 (45.2%) pregnant women had not eaten kelp, which decreased to 1 (0.8%) pregnant woman after the intervention. Before the intervention, 72 (57.1%) pregnant women had not eaten laver and other iodine-rich foods, which decreased to 7 (5.5%) pregnant women after the intervention. Conclusions:After the intervention, the awareness rate of IDD prevention and treatment knowledge and the frequency and proportion of iodine-rich foods consumption among pregnant women in iodine deficiency areas in Anhui Province have increased significantly. It is recommended to carry out publicity and education on IDD prevention and treatment knowledge in early pregnancy.

Objective:To investigate the safety and efficacy of ultrasound-guided percutaneous radiofrequency ablation for gestational trophoblast tumor. Methods:The clinical data of 5 patients with gestational trophoblast tumor treated by ultrasound-guided percutaneous radiofrequency ablation in the First Affiliated Hospital of Zhengzhou University from January 2018 to December 2019 were retrospectively analyzed, the postoperative complications, changes in blood human chorionic gonadotropin (hCG), and the volume reduction rate of ablation lesion at 1, 3, and 6 months after surgery were observed. Results:None of the 5 patients with gestational trophoblast tumor had postoperative complications.Blood hCG decreased to normal level on 7 d, 8 d, 8 d, 12 d and 26 d, respectively. The volume reduction rates of ablation lesions 1, 3 and 6 months after operation in cases 1-5 were respectively 34.7%, 62.5%, 87.5%; 45.3%, 60.4%, 84%; 85.6%, 89.9%, 92.2%; 38.3%, 72.6%, 85.5%; 40.8%, 57.3%, 87%.Conclusions:Percutaneous radiofrequency ablation is minimally invasive, low risk, safe and effective treatment for gestational trophoblast tumors.
