ABSTRACT

Objective To investigate the predictive value of body mass index (BMI), waist circumference (WC), waist to height ratio (WHtR) and visceral obesity index (VAI) in the prevalence of hypertension in adult residents of Anlu City.  Methods A multi-stage stratified random sampling method was used to investigate adult residents in Anlu. T test, χ2 test, Pearson correlation analysis and logistic regression model were used to analyze relationship between the obesity indicators and the prevalence of hypertension. The receiver operating curve (ROC) was used to analyze the predictive power of different obesity indexes for the prevalence of hypertension.  Results A total of 2 518 subjects were included in this study, including 705 patients with hypertension, with a prevalence rate of 28.00%. Occupation, marital status, educational level, abnormal blood glucose, dyslipidemia and obesity were the risk factors of hypertension. The correlation coefficients of BMI, WC, WHtR and VAI with systolic blood pressure were 0.32, 0.30, 0.34 and 0.10, respectively, and with diastolic blood pressure were 0.27, 0.26, 0.29 and 0.08, respectively. Logistic regression analysis showed that overweight OR=1.79 (CI: 1.43 - 2.25), obesity 2.94 (CI: 2.04 - 4.24), waist circumference OR=1.86 (CI: 1.43-2.40), waist height ratio OR=2.52 (CI: 1.97-3.20) and higher VAI OR=1.08 (CI: 1.02-1.15) were associated with a higher risk than those with normal weight. The AUCs of the four obesity indicators for predicting the prevalence of hypertension were WHtR (0.74) >BMI (0.70) >WC (0.69) >VAI (0.56), and the difference was statistically significant (P<0.001). Conclusion BMI, WC, WHtR and VAI are positively correlated with hypertension, and WHtR has a higher predictive value for hypertension.

ABSTRACT

Objective:To examine the long-term survival outcomes and identify mortality risk factors for coronary artery bypass grafting patients who received venoarterial extracorporeal membrane oxygenation(VA-ECMO) for postcardiotomy cardiogenic shock.Methods:Data from 121 consecutive venoarterial extracorporeal membrane oxygenation-treated coronary artery bypass grafting(CABG) patients at Beijing Anzhen Hospital between June 2012 and December 2016 were analyzed. There were 84 males and 24 females. The median age was 62(55, 67) years, and the median body mass index was 25(23, 27)kg/m 2. Thirty-seven patients(34%) had left main artery disease, and 56 patients(52%) underwent off-pump CABG. According to the 3 year survival outcome after ECMO, patients were divided into survival group(35 cases) and non-survival group(73 cases). The differences of clinical indicators between the two groups were compared and analyzed. Multivariable Cox regression modeling was used to identify factors independently associated with 36 month mortality. Results:Sixty-five patients(60%) could be weaned from VA-ECMO, 49 patients(45%) survived to hospital discharge, and 35 patients(32%) survived to 3 years. The median( IQR) time on VA-ECMO support was 4(3, 5) days. The median( IQR) length of ICU stay and hospital stay duration were 8(5, 12) and 20(13, 29) days, respectively. Older age( HR=1.06, 95% CI: 1.03-1.10, P<0.001), left main coronary artery disease( HR=1.62, 95% CI: 1.00-2.60, P=0.048), and vasoactive inotropic score( HR=1.09, 95% CI: 1.03-1.17, P=0.007)were independent risk factors associated with 3-year mortality. The area under the receiver operating characteristic curve for the model, which was constructed with age≥60years, left main coronary artery disease, and vasoactive inotropic score>60, was 0.88(95% CI: 0.80-0.95). Conclusion:Long-term survival of patients who survive to discharge appears favorable. Older age, left main coronary artery disease, and vasoactive inotropic score were associated with 3-year mortality in coronary artery bypass grafting patients who received VA-ECMO.

ABSTRACT

Objective:To summarize the hotspots and developmental status of non-suicidal self-injury research by clustering and co-occurrence to the literature on non-suicidal self-injury on the basis of Cite Space, and in order to provide references for future research and intervention.Methods:Non-suicidal self-injury literature included in the Web of science core collection from January 1975 to August 2020 was searched, and the included literature was visualized and analyzed using Cite Space 5.5.R2 knowledge mapping software.Results:A total of 974 articles were retrieved, and the number of articles published showed an increasing trend year by year, mostly in developed countries. The country with the highest cumulative number of articles was the United States, with a total of 412 articles, and the first organization was Katholieke Univ Leuven, with a total of 42 articles. Key words co-occurrence and clustering showed that the current research focus was on adolescents, suicidal behavior, dialectical behavior therapy, and borderline personality disorder. The most cited literature was by Muehlenkamp.Conclusions:Non-suicidal self-injury research has developed rapidly in recent years. At present, non-suicidal self-injury population, related intervention measures, screening and evaluation tools, Meta-analysis and risk factor analysis are its research frontiers and hot spots.

ABSTRACT

OBJECTIVE To quantitatively evaluate the existing payment policies of diagnosis-related groups (DRGs)in China , so as to provide reference for the formulation and improvement of policies. METHODS Totally 58 documents related to DRGs payment issued by the national and provincial medical security bureaus from 2017 to 2022 were processed by text mining method. PMC index evaluation model of DRGs payment policy was established. Nine typical DRGs payment policies were quantitatively evaluated and analyzed by 10 primary variables and 40 secondary variables. RESULTS Among the 9 policies,5 were excellent and 4 were acceptable. The average score of PMC index was 6.882. Generally ,there was still room for improvement because of the acceptable level. By comparing the two representative policies ,it was found that the main reasons was a lack of consideration in terms of supervision and management ,incentives and constraints when facing policy changes ,reason of the lower level of urban development. CONCLUSIONS Although DRGs payment policy in China is basically perfect ,there is still a lot of room for improvement in terms of extending the time limit of the policy ,summarizing and spreading the successful experience of pilot cities.

ABSTRACT

@#Objective To demonstrate the changes in flexor digitorum and extensor digitorum tension in the affected hands with shear-wave elastography (SWE) before and after manual digitorum sensory stimulation (MDSS) in hemiplegic patients with stroke. Methods A total of 51 hemiplegic post-stroke inpatients in the Department of Rehabilitation Medicine in Second Hospital of Anhui Medical University from April to June, 2020, underwent MDSS completed by a researcher who used a bare thumb and index finger to squeeze each nail bed as well as the sides of each fingertip in the affected hand. The stimulation intensity was the minimum that could cause finger extension without obvious pain, and the interval between two stimulations was five to ten seconds. Muscular tension of the flexor digitorum superficialis, flexor digitorum profundus, flexor pollicis longus and extensor digitorum were assessed with modified Ashworth Scale (MAS) and shear-wave velocity (SWV) of SWE on the affected side before and immediately after MDSS. MAS score was -1 as low muscular tension. Results The MAS scores of all the muscles significantly reduced after MDSS (|Z| > 2.843, P < 0.001); while the changes of SWV were not significantly in all the muscles with initially MAS score of 0 or -1 (t < 1.052, P > 0.05), and it reduced in those muscles with initial MAS scores of one to three (t > 2.672, P < 0.05). The SWV were positively correlated with the MAS scores both before and after MDSS (r > 0.334, P < 0.05). Conclusion MDSS can effectively, immediately, and safely relieves muscle spasms of the flexor digitorum and facilitate active finger extension in the affected hand for hemiplegic patients with stroke. SWE is useful for quantitatively and objectively evaluating muscular tension in the affected hand for hemiplegic patients with stroke.

ABSTRACT

OBJECTIVE To stu dy the effects of different processing me thods on the contents of the pharmacodynamic index components in Citrus aurantium and their antioxidant activity. METHODS According to the general principles of 2020 edition of Chinese Pharmacopoeia (volume Ⅳ) and the relevant processing methods in 2015 edition of Processing Specifications of Traditional Chinese Medicine in Zhejiang Province ,the samples of C. aurantium were prepared by steaming with water ,boiling with water ,stir-frying with vinegar ,stir-frying with wine ,stir-frying with bran ,processing with bran and processing with honey. The contents of moisture and ash in different products of C. aurantium were detected. The contents of naringin and neohesperidin in different products of C. aurantium were determined by high performance liquid chromatography. The antioxidant activity of different products was investigated by DPPH and ABTS + radical scavenging experiments and the total reducing power test. RESULTS The contents of moisture ,ash,naringin and neohesperidin were in line with the relevant requirements in 2015 edition of Processing Specifications of Traditional Chinese Medicine in Zhejiang Province . The content of naringin in descending order was as follow : unprocessed sample >sample processed with honey >sample processed with bran >sample boiled with water >sample stir-fried with vinegar>sample stir-fried with wine >sample stir-fried with bran >sample steamed with water. The content of neohesperidin in descending order was as follow :unprocessed sample >sample boiled with water >sample processed with bran >sample processed with honey >sample stir-fried with vinegar >sample steamed with water >sample stir-fried with wine >sample stir-fried with bran. The samples after boiling with water ,processing with bran ,and stir-fried with bran had better DPPH radicals scavenging ability (IC50 were 7.49,8.37 and 10.22 mg/mL,respectively). The samples after boiling with water ,steaming with water ,and processed with bran had better ABTS + radicals scavenging ability (IC50 were 1.76,2.03 and 2.72 mg/mL,respectively). In addition , compared with sample stir-fried with wine and processed with 发。E-mail:wanglu1286@163.com honey,unprocessed sample and other processed products of C.aurantium had bet ter total reducing ability. CONCLUSIONS After processing ,the contents of the main pharmacodynamic index components in C. aurantium have been reduced ,but they were also in line with the relevant requirements in 2015 edition of Processing Specifications of Traditional Chinese Medicine in Zhejiang Province . The antioxidant ability of some processed products has been enhanced.

ABSTRACT

OBJECTIVE To establis h and validate a population pharmacokinetic model of docetaxel in malignant tumor patients. METHODS The clinical data of malignant tumor patients treated with chemotherapy regimen containing docetaxel in our hospital from June 2019 to December 2021 were retrospectively collected . According to the results of blood concentration detection , based on the three -compartment model the nonlinear mixed effect model (NONMEM)was used ;covariates(age,weight,height, body surface area ,Karnofsky performance scale ,total protein ,albumin,total bilirubin ,aspartate aminotransferase ,alanine aminotransferase and serum creatinine )affecting clearance (CL)were screened by “forward inclusion and backward exclusion ”; the population pharmacokinetic model of docetaxel was established . The model was tested for goodness -of-fit diagnosis and internal validation by Bootstrap . RESULTS A total of 264 measured blood concentrations of 132 patients with malignant tumors during chemotherapy were included . The covariates that had significant effect on CL of docetaxel were serum creatinine and total bilirubin (P<0.01). The results of Bootstrap analysis (parameter median values and 95% confidence intervals )were close to predict results of the established model ;the final model estimated that the population typical value of docetaxel CL was 37.82 L/h. CONCLUSIONS The population pharmacokinetic model of docetaxel in malignant tumor patients is established successfully , which can be used for the formulation and optimization of clinical individualized regimen .

ABSTRACT

Objective:To investigate the feasibility, effectiveness and safety of indocyanine green (ICG) navigation in the surgical resection of abdominal wall endometriosis (AWE).Methods:Seven women undergoing surgery for AWE in First Affiliated Hospital of Sun Yat-sen University (from July 1, 2021 to October 1, 2021) were collected. After exposure of the focus, ICG were used intravenously (0.25 mg/kg) as fluorescent dye for the intraoperative evaluation of AWE vascularization. Resection of the AWE was guided by direct visualization of the focus under standard laparoscopy with a near-infrared (NIR) camera head. Surgical margin around the AWE (3, 6, 9 and 12 point) and the margin under the focus were obtained for postoperative pathological examination of endometriosis. Time from injection to fluorescence visualization, the proportion of fluorescence visualization, time of fully resection of AWE, side effects related to the use of ICG, perioperative complications as well as the pathological result of the surgical margins were recorded.Results:ICG fluorescence of the AWE were seen in 5 patients (5/7). The mean time from injection to fluorescence visualization was (46.7±9.8) s. The mean time of fully resection of AWE was (16.4±7.0) minutes. There were no side effects related to the use of ICG. The rate of class-A wound healing was 7/7. All of the surgical margins were confirmed endometriosis-negative by postoperative pathological examination.Conclusion:ICG fluorescence visualization could conduct accurate resection of AWE, which is clinically safe and effective.

ABSTRACT

Objective:To investigate the effects of sentinel lymph node biopsy utilization on operative time, intraoperative blood loss and extubation time.Methods:Sixty-two patients with breast cancer who received treatment in the First Hospital of China Medical University from January to December 2019 were included in this study. They were randomly assigned to receive either conventional breast cancer surgery (control group, n = 31) or sentinel lymph node biopsy combined with breast cancer surgery (study group, n = 31). Extubation time, operative time, intraoperative blood loss, drainage volume, breast cosmetic effect, upper limb function and complications were compared between the two groups. Results:Extubation time and operative time in the study group were (16.3 ± 1.1) hours and (61.6 ± 11.3) minutes, respectively, and they were (28.2 ± 6.4) hours and (124.2 ± 28.5) minutes, respectively in the control group. There were significant differences in extubation time and operative time between the two groups ( t = 14.922 and 16.479, both P < 0.05). Intraoperative blood loss and postoperative drainage volume in the study group were (68.7 ± 17.9) mL and (105.9 ± 19.5) mL respectively, and they were (122.4 ± 23.1) mL and (257.2 ± 36.4) mL respectively in the control group. There were significant different differences in intraoperative blood loss and postoperative drainage volume between the two groups ( t = 15.928 and 18.797, both P < 0.05). The excellent and good rate of breast cosmetic effect in the study group was significantly higher than that in the control group (93.6% vs. 83.8% , χ2 = 5.584, P < 0.05). After treatment, the difference in the upper arm circumference between the healthy and affected sides, and shoulder abduction and shoulder function score in the study group were (0.5 ± 0.1) cm, (123.7 ± 6.6) ° and (75.9 ± 4.9) points respectively, and they were (0.7 ± 0.1) cm, (120.1 ± 6.1) °, (73.5 ± 4.4) points, respectively in the control group. There were significant differences in these indices between the two groups ( t = 11.432, 12.450, 12.647, all P < 0.05). The incidence of complications in the study group was significantly lower than that in the control group (12.9% vs. 38.8%, χ2 = 6.309, P < 0.05). Conclusion:Sentinel lymph node biopsy can help improve therapeutic effects, shorten the operative time, reduce the amount of intraoperative bleeding, shorten the extubation time, and reduce the incidence of complications in breast cancer patients.

ABSTRACT

Objective:To investigate the effects of Dexmedetomidine on postoperative pain and stress response in elderly patients undergoing laparoscopic colorectal cancer surgery.Methods:A total of 150 elderly patients who had undergone laparoscopic radical resection of colorectal cancer in our hospital from June 2019 to December 2020 were divided into two groups using the random number table method, the control group(n=75)and the study group(n=75). In the study group, Dexmedetomidine 0.5 μg/kg was injected intravenously over 10 min before induction of anesthesia and maintained at 0.5 μg/kg/h until 30 min before the end of surgery.Changes in heart rate, mean arterial pressure, postoperative pain and stress response index scores were compared between the two groups.Results:The MAP and HR in the study group at T1(10 min after administration), T2(immediately after endotracheal intubation), T3(30 min after pneumoperitoneum)and T4(leaving the operating room)were significantly lower than in the control group( P<0.05); The concentrations of norepinephrine and epinephrine in the study group were lower than those in the control group at T2, T3 and T4(all P<0.05); Pain scores of patients in the study group were lower than those in the control group at T4 and T5( P<0.05); Compared with the control group, the study group used less intraoperative Sufentanil[(65.5±7.0)μg vs.(76.2±7.6)μg, t=8.968, P=0.000], needed a lower number of PCIA compressions 24 h after surgery[(6.3±2.3) vs.(9.5±3.0), t=7.331, P=0.000]and had a shorter length of PACU stay[(23.4±4.0)min vs.(30.2±5.7)min, t=8.457, P=0.000]. Conclusions:Dexmedetomidine used in laparoscopic colorectal cancer surgery in elderly patients can reduce the use of sufentanil and the length of PACU stay time, improve postoperative pain scores and reduce stress response, and therefore should be recommended.

Objective@#To summarize the effective of high intensity interval training on cognitive executive function among adolescents through Meta analysis,and to provide reference for medical rehabilitation and physical education practice.@*Methods@#Literature search was conducted for Chinese and English keywords "High Intensity Interval Training", "Cognition", "Cognition Function","Executive Function" and "Executive Controls" regarding the effect of high intensity interval training on cognitive executive function among adolescents published prior to September 20, 2020 in PubMed, Cochrane library, Web of science, Embase, the China National Knowledge Infrastructure (CNKI), the Wanfang database and the VIP database. Stata 14 software and Revman 5.3 software were used for Meta analysis, fixed effect or random effect model was used to combine the results based on the heterogeneity.@*Results@#The response time of Stroop test immediately after acute high intensity interval training significantly decreased ( SMD=0.70,95%CI =0.28-1.11, z=3.29,P <0.01); there was no significant change in response time of Stroop test 30 min after training ( SMD=0.23,95%CI=-0.14-0.60,z=1.23,P >0.05); the correct rate of Stroop test immediately after acute high intensity interval training increased significantly ( SMD=0.26,95%CI=0.03-0.50,z=2.21,P <0.05); there was no significant change in correct rate of Stroop test 30 min after training ( SMD=-1.38,95%CI=-4.28-1.52,z=0.93,P >0.05). After long term high intensity interval training, the response time of Stroop and TMT test were significantly shortened ( SMD=0.38, 95%CI=0.07-0.70, z=2.41, P < 0.05 ).@*Conclusion@#Acute and long term high intensity interval training can effectively improve cognitive executive function among adolescents, but the long term effect of acute training is unclear.

ABSTRACT

OBJECTIVES@#Cardiac hypertrophy and fibrosis are major pathological manifestations observed in left ventricular remodeling induced by angiotensin II (AngII). Low-intensity pulsed ultrasound (LIPUS) has been reported to ameliorate cardiac dysfunction and myocardial fibrosis in myocardial infarction (MI) through mechano-transduction and its downstream pathways. In this study, we aimed to investigate whether LIPUS could exert a protective effect by ameliorating AngII-induced cardiac hypertrophy and fibrosis and if so, to further elucidate the underlying molecular mechanisms.@*METHODS@#We used AngII to mimic animal and cell culture models of cardiac hypertrophy and fibrosis. LIPUS irradiation was applied in vivo for 20 min every 2 d from one week before mini-pump implantation to four weeks after mini-pump implantation, and in vitro for 20 min on each of two occasions 6 h apart. Cardiac hypertrophy and fibrosis levels were then evaluated by echocardiographic, histopathological, and molecular biological methods.@*RESULTS@#Our results showed that LIPUS could ameliorate left ventricular remodeling in vivo and cardiac fibrosis in vitro by reducing AngII-induced release of inflammatory cytokines, but the protective effects on cardiac hypertrophy were limited in vitro. Given that LIPUS increased the expression of caveolin-1 in response to mechanical stimulation, we inhibited caveolin-1 activity with pyrazolopyrimidine 2 (pp2) in vivo and in vitro. LIPUS-induced downregulation of inflammation was reversed and the anti-fibrotic effects of LIPUS were absent.@*CONCLUSIONS@#These results indicated that LIPUS could ameliorate AngII-induced cardiac fibrosis by alleviating inflammation via a caveolin-1-dependent pathway, providing new insights for the development of novel therapeutic apparatus in clinical practice.

ABSTRACT

Objective:To explore the protective effect of tetramethylpyrazine on newborn rats with necrotizing enterocolitis (NEC) caused by hypoxic cold stress combined with formula milk invasive feeding.Methods:According to the random number table methods, 50 newborn SD rats were randomly divided into five groups, including blank group, model group, low dose group, middle dose group and high dose group. Except for the blank group, newborn rats in all other groups were treated with hypoxic cold stress combined with formula milk invasive feeding to establish NEC models. The rats in the low, medium and high dose group were given tetramethylpyrazine by 2.09, 5.18 and 10.36 mg/kg respectively for 3 consecutive days. The rats in the model group was given equal volume of sodium. In the blank group, no treatment was given, and the lactating mothers were used to substitute milk. Twelve hours after the last artificial administration, the rats in each group were sacrificed by cervical dislocation method, the small intestine ileocecal tissue was taken, and the levels of malondialdehyde, catalase (CAT), glutathione peroxidase (GSH-Px) and superoxide dismutase (SOD) in ileocecal intestinal homogenate was detected. Hematoxylin eosin staining was used to observe the pathological changes of ileocecal intestinal tissue, and Khailova standard was used to evaluate ileocecal intestinal tissue injury. The expression of cysteine-containing aspartate-specific proteases-3 (caspase-3) was detected by immunohistochemical staining.Results:Compared with the model group, the levels of malondialdehyde in the treatment groups were significantly lower, and the levels of CAT, GSH-Px and SOD were significantly higher (all P<0.05). The newborn rats in the low, medium and high dose groups were accompanied by mild and moderate intestinal villi hyperemia and edema and the ileocecal mucosa lamina propria swelling were separated, but the degree was not as high as that in the model group. Compared with the model group (3.21±0.43), the ileocecal tissue injury scores of the low, medium and high dose groups (1.91±0.33, 1.56±0.31 and 1.35±0.26) were significantly lower, and the differences were statistically significant (all P<0.05). Compared with the model group (9.34±0.98), the positive expression of caspase-3 was significantly relieved in the low, medium and high dose groups (5.51±0.57, 4.46±0.45 and 2.15±0.21), and the differences were statistically significant (all P<0.05). Conclusions:Tetramethylpyrazine has good protective effects in the hypoxic cold stress combined with formula milk invasive feeding induced necrotizing enterocolitis in newborn rats.

ABSTRACT

Objective@#To describe physical activity (PA) level of college students, and to examine the associations between PA and physical health and depression, so as to provide evidence for health promotion of college students.@*Methods@#During June to July 2018, a cross-sectional survey was conducted among 4 624 college students from 2 universities in Anhui and Jiangxi provinces. The basic information was investigated by the questionnaire. PA was assessed by the International Physical Activity Questionnaire(IPAQ). Physical and mental health were evaluated by the 36-item Short Form Health Survey Questionnaire (SF-36) and the Center for Epidemiologic Studies Depression Scale(CES-D), respectively.Chi-square test was used to examine the differences between physical health and depression among college students with different demographic characteristics. The generalized linear model and binary logistic regression model were used to analyze the relationships between PA and physical health and depression.@*Results@#The proportion of good, medium and poor physical health were 64.5%, 31.6% and 3.9%, respectively. The detection rate of depression was 34.1%. Compared to low PA group, physical health was negatively correlated with medium PA (OR=0.78, 95%CI=0.66-0.91) and high PA(OR=0.46, 95%CI=0.38-0.56). Binary logistic regression results showed that, depression was negatively associated with medium PA (OR=0.69, 95%CI=0.59-0.82) and high PA(OR=0.66, 95%CI=0.55-0.79)(P<0.05). @*Conclusion@#There is a significant association between PA and physical health and depression. Further longitudinal studies are needed to clarify the causal relationship between PA and physical and mental health.

ABSTRACT

OBJECTIVE:To investigate th e stabilities of antitumor candid ate Gedatolisib in plasma in vitro and simulated gastric/intestinal fluids ,and to analyze the possible catabolites in plasma. METHODS :HPLC method was adopted. Using indometacin as internal standard ,the contents of Gedatolisib incubated in plasma of SD rats (male)for 0,0.5,1.0,1.5,2.0,3.0 h and blank simulated gastric fluid (pH 1.3,no enzyme ),blank simulated intestinal fluid (pH 6.8,no enzyme ),simulated gastric fluid(pH 1.3,containing pepsin )and simulated intestinal fluid (pH 6.8,containing trypsin )for 0,0.5,1.0,2.0,3.0,4.0,6.0 h were determined. The remaining percentage of Gedatolisib was calculated. UPLC-Q-TOF/MS was used to analyze the TIC of blank plasma and incubated samples. The differential peaks were compared, and catabolites were inferred by MS 1Z073).gzwjkj2019-1- chromatograms. RESULTS : The remaining percentage in plasma of rats for 2.0 h was about 63%,and there was nosignificant change after continued incubation. The remaining percentage of Gedatolisib incubated in blank simulated intestinal fluid for different time ranged (99.18 ± 2.15)% -(103.20 ± 3.41)% . The remaining percentage in simulated com intestinal fluids for 2.0 h ranged (88.76 ± 1.53)% . The remaining percentage in blank simulated gastric fluids for 2.0 h was ranged (85.63±1.55)%,and there was no significant change after continued incubation. The remaining percentage in simulated gastric fluid was from (94.94±3.52)%(0 h)to(16.19±1.17)% (6.0 h). TIC of UPLC-Q-TOF/MS showed that the differential peaks of incubated samples and blank plasma was 6.42 min under positive mode scanning ,molecular ion peak of m/z 616.335 1,simulated 632.327 7,630.317 0,602.278 6 [M+H]+ could be found in scanning channel. It was speculated that Gedatolisib could generate 1-(4-(3-(4,6-dimorpholino-1,3,5-triazin-2-yl)phenyl)urea) benzoyl)-N,N-dimethylpiperidin-4-amine oxide ,1-(4-(4-(dimethylamino)piperidine-1-carbonyl)phenyl)-3-(4-(4-morpholino-6- (3-oxomorpholino)-1,3,5-triazin-2-yl)phenyl)urea and 1-(4-(3-(4-(4,6-dimorpholino-1,3,5-triazin-2-yl)phenyl)urea) benzoyl)-N-methylpiperidine. CONCLUSIONS :Gedatolisib is not stable in rat plasma ,and it may undergo terminal N oxidation, morpholine ring oxidation and terminal N demethylation. Gedatolisib is stable in artificial intestinal fluid and blank artificial gastric/ intestinal fluid ,and degrades obviously in the presence of pepsin.

ABSTRACT

Objective:To explore an effective method of academic integrity commitment to reflect the management of hospital scientific research papers to avoid the occurrence of academic misconduct.Methods:The retrospective analysis method was adopted to compare the effectiveness of paper signed academic integrity commitment (paper sign method) and hospital network academic integrity commitment (the net sign method), main indicators including the medical personnel participation, commitment submission time spots, extraction/find commitment of time and paper into the achievements management system management system were taken into consideration for difference identification.Results:The comparison of the two groups' commitment to integrity and active participation was P<0.01, indicating that the online signature method could stimulate the agency and consciousness of the first author to fulfill the commitment to academic integrity. The time of submitting the commitment letter and the time of extracting/finding the commitment letter in both groups were all P<0.01, indicating that the network signature method could save a lot of time for waiting or shelving in the docking process of each link and improve the efficiency; The inclusion time of the two groups of papers into the outcome management system was P<0.05, which was positively correlated with the increase of the active participation rate of medical personnel by the online signature method. Conclusions:The net sign method can timely discover the risk factors of academic misconduct and prevent different forms of academic misconduct in the bud, which is of positive significance to the management of hospital scientific research results and even to boost the development of discipline construction. The method has the advantages of high participation, rapid approval and obtaining evidence according to law.

ABSTRACT

Objective:To study whether simvastatin could inhibit viral replication during human metapneumovirus (hMPV) infection.Methods:Human bronchial epithelial cells (16HBE) were infected with hMPV and then treated with or without simvastatin. Real-time quantitative PCR (qPCR) and Western blot were used to detect virus titers and the activation of autophagy and related pathways. BALB/c mice were infected with hMPV and then treated with simvastatin through intragastric administration. Pathological changes in lung tissues were observed. Changes in viral loads and the activation of autophagy and related pathways in proteins and RNA extracted from lung tissues were detected.Results:The in vitro experiment showed that the hMPV+ simvastatin group had decreased virus titer and enhanced autophagy than the hMPV group. The AKT/mTOR pathway in the hMPV+ simvastatin group was inhibited, which was verified by a further experiment using rapamycin, a specific inhibitor of AKT/mTOR pathway. The in vivo experiment showed that the virus titer in the hMPV+ simvastatin group was lower than that in the hMPV group, but there was no significant difference in the activation of autophagy. The AKT/mTOR pathway was down-regulated in the hMPV+ simvastatin group. HE staining revealed that obvious pathological changes were observed in the hMPV group, but the condition was improved after simvastatin intervention. Conclusions:Simvastatin can inhibit the replication of hMPV, which is associated with the activation of autophagy induced by AKT/mTOR pathway.

ABSTRACT

Objective@#To analyze the clinical characteristics of hand, foot and mouth disease (HFMD) caused by coxsackievirus A6 (CV-A6) in different age groups.@*Methods@#From January 2015 to December 2017, throat swabs were collected from children with or suspected of having HFMD then quantitative real-time PCR was performed to detect enterovirus nucleic acid. HFMD cases caused by CV-A6 were divided into different groups according to age for comparison.@*Results@#In total, there were 467 cases of HFMD caused by CV-A6 with the age ranging from 3 months to 16 years. There were 273 cases in the infants and young children group (< 3 years old), 131 cases in the pre-school group (3-6 years old), and 63 cases in the school-age group (> 6 years old). The peak incidence was found between May and November.Fever was the common symptom, and the rate of fever in infant group was the highest (220/273, 80.5%); The proportion of cases with leucocyte elevation in the infant group was the highest (127/273, 46.5%) than that in the school-age group (17/63, 27.0%) with a statistical significance. The skin erythra of the HFMD caused by CV-A6 were diverse in forms. Over two forms of skin erythra accounted for 53.9% (257/476) of all cases, and the cases in the infant group showing more forms of skin erythra (163/273, 59.7%). The oral herpes were mainly distributed in the upper palate and pharyngeal isthmus, but the school age group had the least number of distribution sites (0.89±0.86). The cases in the infant group showed higher incidence of skin rash at the elbow joint (109/273, 39.9%), knee (88/273, 32.6%), thigh (112/273, 41%), buttock (122/273, 44.7%) than the other two groups, However, the school age group showed lower incidence of skin rash in the lower leg (0/63, 0%) and thigh (6/63, 9.5%) than the other two groups. The differences between groups were statistically significant. All cases were cured clinically, no severe cases occurred. Among the 288 cases followed up for 6 months, 33 (33/288, 11.5%) suffered from nail exfoliation.@*Conclusions@#Different age groups of HFMD caused by CV-A6 had different clinical manifestations. In the infant group, more cases had fever and the erythra were more diverse in forms and wider in distribution. In addition, the increased leukocytes in routine blood test was also more common in the infant group.

Objective@#To study the effects of altitude exposure and altitude exposure on mental health of Tibetan cadres.@*Methods@#105 cadres in Tibet were selected as the research objects. Among them, 74 cadres in Shannan and Lhasa of Tibet (average altitude 3 680 m), 31 in Shigaze and Ali (above 3 800 m) and 14 in Ali (above 4 300 m) were selected. Using Symptom Checklist 90 (SCL-90), Self-Rating Anxiety Scale (SAS), Self-Rating Depression Scale (SDS) and Ascension Insomnia Scale (AIS), 105 Tibetan aid cadres were tested by SAS, SDS, AIS and SCL-90 one week after entering Tibet and one week before leaving the plateau. The scores were collected and the mental health and sleep status of Tibetan aid workers were measured.@*Results@#The number of positive items of SCL-90 of 105 Tibetan cadres increased from (13.21±9.05) one week after entering Tibet to (38.35±18.84) one week before leaving Tibet. SAS, SDS and AIS also increased from (25.49±5.19), (26.41±5.15), (5.16±3.54) points one week after entering Tibet to (36.78±7.53), (41.42±9.15), (8.71±4.64) points one week before leaving Tibet. The difference was significant in the last week (Z = 8.420, 8.689, 6.277, P<0.01). The correlation analysis between SCL-90 and SAS, SDS and AIS showed that SCL-90 was positively correlated with SAS, SDS and AIS scores (r = 0.651, 0.616, 0.356, P<0.01). Different altitudes have an effect on the mental health of Tibetan aid cadres. The paranoid item scores of Tibetan aid cadres with altitude>4 300 m in SCL-90 were (1.68 ± 0.50) points, which were higher than those of Tibetan aid cadres with altitude<4 300 m (1.24±0.28) points and (1.40± 0.38) points. The difference was statistically significant (F=5.255, P=0.007); The SDS scores of Tibetan aid cadres with altitude>4 300 m were (46.64 ± 7.24) points, which were higher than those of Tibetan aid cadres with altitude<4 300 m (40.76 ± 8.99) points and (40.58 ± 8.28) points. The difference was statistically significant (F=3.223, P=0.044).@*Conclusion@#Exposure to high altitude affects the mental health of cadres assisting Tibet to varying degrees. The mental health of cadres assisting Tibet deserves attention.
