ABSTRACT

Objective@#To understand the prevalence of overweight and obesity, dietary habits and main food intake frequency among primary school students in Shenyang, so as to provide a reference for exploring the effect of diet related factors on the development of overweight and obesity in children.@*Methods@#A total of 2 041 students from two primary schools in a certain district of Shenyang were selected by convenience sampling in May 2017, with height and weight measured, meanwhile the questionnaire survey regarding dietary habits and the frequency of main food intake were administered.@*Results@#The rates of overweight and obesity were 18.4% and 22.1% respectively, and the rate of overweight and obesity in boys (21.0%,27.8%) were significantly higher than that in girls (15.8%,16.2%)(χ 2=22.45,53.40,P<0.01). The results of univariate analysis showed that frequency of eating breakfast, eating speed, appetite, picky eaters or not, and the frequency of fruit, seafood and canned food intake were associated with overweight and obesity in primary school students (χ 2=7.67,97.92,229.70,95.88,6.40,6.58,7.96,P<0.05). Multivariate Logistic regression analysis showed that slow eating speed (OR=0.46, 95%CI=0.29-0.69) and normal eating speed (OR=0.47, 95%CI=0.32-0.69) were associated with lower rates of overweight and obesity; good appetite (OR=43.73, 95%CI=5.88-325.36) was associated with higher rates of overweight and obesity in primary school students (P<0.01).@*Conclusion@#The detection rate of overweight and obesity is relatively high among primary school students in Shenyang, and the rate of obesity is already higher than that of overweight; The frequency of common food intake has little impact on the development of overweight and obesity in primary school students, but fast eating speed and good appetite are the risks of overweight and obesity among them.

ABSTRACT

Bioremediation is regarded as one of the most promising methods for the remediation of the soil and groundwater contaminated with organic compounds, mainly due to its cost-effectiveness and environmental friendliness. Using VOSviewer and CiteSpace, we analyzed all publications in this area in core database of Web of Science from 1990 to 2020. The number of publications in this area showed an increasing trend worldwide. The country with the largest total number of publications was USA, followed by China and India. Since 2012, the number of annual publications of China exceeded USA and ranked the first. The top three journals which published papers in this area were Chemosphere, Environmental Science & Technology, and Science of the Total Environment. The Chinese Academy of Sciences published the largest number of papers among the research institutions globally, but the University of California in USA had the highest total citations and h-index. Bibliometric analysis indicated that it is important to develop biotechnologies of bioremediation coupled with chemical/physical remediation to overcome the challenge of low efficiency and long remediation duration associated with bioremediation. In addition, the bioremediation on the mixed contaminants, such as organic contaminants and heavy metals, or mixtures of different organic contaminants, is an important direction. Finally, the rapid development of microbiome technologies will further promote the exploration of mechanisms involved in bioremediation on the genetic and molecular level.

ABSTRACT

Objective: To observe the effect of Anrou-pressing and kneading Hegu (LI 4) and Sanyinjiao (SP 6) on uterine inertia during painless parturition. Methods: A total of 100 cases of patients with uterine inertia during painless parturition were randomized into an acupoint group and a medicine group by the SPSS programming, with 50 cases in each group. Patients in the acupoint group received the treatment of Anrou-pressing and kneading bilateral Hegu (LI 4) and Sanyinjiao (SP 6), while patients in the medicine group received intravenous oxytocin. The labor time and maternal-child safety were evaluated, and the labor stage was compared. Results: The latent and active phases in the 1st labor stage and the 2nd labor stage in the acupoint group were substantially shorter than those in the medicine group, and the between-group comparisons showed statistical significance (all P<0.05). The between-group comparison of the 3rd labor stage showed no statistical significance (P>0.05). The postpartum hemorrhage amount in both groups was within the safe range, and the between-group comparison showed no statistical significance (P>0.05). The newborn 1 min Apgar score was ranged 8-10 points, and the between-group comparison showed no statistical significance (P>0.05). The between-group comparison of the labor stage efficacy showed statistical significance (P<0.01), with a better progression in the acupoint group. Conclusion: Anrou-pressing and kneading Hegu (LI 4) and Sanyinjiao (SP 6) is both effective and safe for uterine inertia during painless parturition.

ABSTRACT

Objective@#To investigate the affecting factors of type 1 diabetes mellitus(T1DM) in children, and to provide reference for the prevention and treatment measures in the future.@*Methods@#A case-control study was carried out among 106 children diagnosed with T1DM in a hospital in Shenyang. At the same time, 106 healthy children were selected as the control group by 1∶1 matching of age, gender and living area. The early life-related factors that may affect the development of T1DM in two groups were investigated by questionnaire.@*Results@#The average age of diagnosis in 106 cases with T1DM was(8.02±3.49) years, girls accounted for 60.4% and boys 39.6%. Multi-factor Logistic regression analysis showed that high family socioeconomic status and higher education of fathers were associated with lower odds of development of T1DM. Family member smoking(OR=2.51, 95%CI=1.19-5.29), family history of diabetes(OR=2.56, 95%CI=1.18-5.55), maternal obesity(OR=5.42, 95%CI=2.49-11.83) and infection during pregnancy(OR=3.45, 95%CI=1.04-11.46) were associated with higher risk of T1DM(P<0.05).@*Conclusion@#The occurrence of T1DM in children is affected by many factors including heredity, maternal health during pregnancy and family economy,highlighting the importance of early life prevention of T1DM.

ABSTRACT

Objective@#To investigate the prevalence and associated factors of eczema in children in order provide reference for the prevention and treatment of eczema.@*Methods@#Two primary schools in a district of Shenyang were randomly selected to conduct a questionnaire survey on 2 240 students in May 2017. Children with eczema need to be diagnosed by a doctor.@*Results@#There were 838 cases of eczema with a reported rate of 37.4%, 38.7% in boys and 36.1% in girls. The age of 6 to 12 months was the peak period of first eczema in children, and the proportion of eczema within 1 year was up to about 70%. Children who have had eczema in the past were now more likely to catch a cold, and the reported rates of allergic rhinitis, asthma and food allergies were also significantly higher. The Logistic regression analysis showed that the mother was unhappy during pregnancy(OR=1.32,95%CI=1.03-1.69), the parents were more educated (OR=1.60, 95%CI=1.19-2.16), and the parents were allergic (OR=1.58,95%CI=1.07-2.33;OR=1.41, 95%CI=1.02-1.94), family history of diabetes(OR=1.95, 95%CI=1.41-2.68), and cesarean(OR=1.33, 95%CI=1.07-1.65) were risk factors of eczema in children.@*Conclusion@#The incidence of children eczema is affected by maternal mood during pregnancy, family factors and delivery mode. It is of practical significance to prevent children eczema in early development for reducing the incidence of other allergic diseases in the future.

ABSTRACT

The aim of the study was to determine the feasibility of the Vitellaria paradoxa nutshell as a new medicinal resource for treating diabetes. A total of forty-one compounds were identified by HPLC-DAD-Q-TOF-MS and phytochemical methods in V. paradoxa nutshell methanol extract. Based on HPLC fingerprints, four characteristic constituents were quantified and the origin of twenty-eight V. paradoxa nutshells from seven sub-Saharan countries was compared, which were classified into three groups with chemometric method. Twenty-eight samples contained high total phenolic content, and exhibited moderate-higher antioxidant activity and strong α-glucosidase inhibitory activity. Furthermore, all fractions and isolated compounds were evaluated for their antioxidant and α-glucosidase inhibitory activities, and α-glucosidase inhibitory action mechanism of four characteristic constituents including protocatechuic acid, 3, 5, 7-trihydroxycoumarin, (2R, 3R)-(+)-taxifolin and quercetin was investigated via molecular docking method, which were all stabilized by hydrogen bonds with α-glucosidase. The study provided an effective approach to waste utilization of V. paradoxa nutshell, which would help to resolve waste environmental pollution and provide a basis for developing potential herbal resource for treating diabetes.

ABSTRACT

OBJECTIVE@#To explore the clinical efficacy of exercise acupuncture and osteopathy in the treatment of traumatic knee arthritis.@*METHODS@#Fifty-one patients with traumatic knee arthritis were divided into three groups:routine group, exercise acupuncture group and osteopathy group. In the routine group, there were 17 cases, 12 males and 5 females, with a mean age of (49.4±8.9) years old; the averaged course of disease was (4.89±1.52) years; total WOMAC score before treatment was 100.77±15.48. Seventeen patients (11 males and 6 females) in the exercise acupuncture group were aged (48.6±10.1) years old; the course of disease was (4.21±1.37) years; and the total WOMAC score before treatment was 106.16±14.95. In the osteopathy group, there were 17 cases, 8 males and 9 females, with a mean age of (52.3±8.4) years old; the average course of disease was (4.79±1.50) years; total WOMAC score before treatment was 103.87±10.14. The patients in the routine group were treated with routine rehabilitation; the patients in the exercise acupuncture group were treated with routine rehabilitation combined with exercise acupuncture, and the patients in the osteopathy group were treated with osteopathy combined with routine rehabilitation. The changes of WOMAC score and ROM in three groups were compared before treatment and 4 weeks after treatment.@*RESULTS@#The overall effect of osteopathy group was better than that of routine group (=3.151, =0.005). The scores of WOMAC before and after treatment:pain of 23.84±4.66, 11.98±2.66, stiffness of 10.44±1.71, 6.42±0.74, daily life of 66.49±11.85, 35.80±4.44 in the routine group; pain of 22.64±3.22, 8.90±2.19, stiffness of 11.82±2.57 , 6.03±1.06, daily life of 71.72±13.59, 32.94±4.73 in the exercise acupuncture group; pain of 22.38±3.68, 10.66±2.75, stiffness of 11.81±2.08, 5.63±1.69, daily life of 69.69±8.96, 28.84±5.76 in the osteopathy group. Compared with the other two groups after treatment, the improvement of pain score in the exercise acupuncture group was better than those in the other two groups. There were no significant differences in stiffness score among the three groups. The degree of difficulty in daily life in the osteopathy group was better than that in the other two groups. The total score of WOMAC in the exercise acupuncture group and the osteopathy group were better than that in the conventional group, but there was no significant difference between the two groups.@*CONCLUSIONS@#On the basis of routine rehabilitation treatment, exercise needling and osteopathy have positive significance for the improvement of joint pain and dysfunction of joint movement in patients with traumatic knee arthritis, with certain popularlized value in the treatment of traumatic knee arthritis.

ABSTRACT

<p><b>Background</b>The introduction of individualized abdominoperineal excision (APE) may minimize operative trauma and reduce the rate of complications. The purpose of this study was to evaluate the safety and efficacy of individualized APE for low rectal cancer.</p><p><b>Methods</b>Fifty-six patients who underwent individualized APE from June 2011 to June 2015 were evaluated retrospectively in Beijing Chaoyang Hospital, Capital Medical University. The main outcome measures were circumferential resection margin (CRM) involvement, intraoperative perforation, postoperative complications, and local recurrence. Statistical analysis was performed using SPSS version 16.0.</p><p><b>Results</b>Fifty (89%) patients received preoperative chemoradiotherapy: 51 (91%) patients were treated with the sacrococcyx preserved; 27 (48%) patients with the levator ani muscle partially preserved bilaterally; 20 (36%) patients with the levator ani muscle partially preserved unilaterally and the muscle on the opposite side totally preserved; 7 (13%) patients with intact levator ani muscle and part of the ischioanal fat bilaterally dissected; and 2 (4%) patients with part of the ischioanal fat and intact lavator ani muscle dissected unilaterally and the muscle on the opposite side partially preserved. The most common complications included sexual dysfunction (12%), perineal wound complications (13%), urinary retention (7%), and chronic perineal pain (5%). A positive CRM was demonstrated in 3 (5%) patients, and intraoperative perforations occurred in 2 (4%) patients. On multiple logistic regression analysis, longer operative time (P = 0.032) and more intraoperative blood loss (P = 0.006) were significantly associated with perineal procedure-related complications. The local recurrence was 4% at a median follow-up of 53 months (range: 30-74 months).</p><p><b>Conclusion</b>With preoperative chemoradiotherapy, individualized APE may be a relatively safe and feasible approach for low rectal cancer with acceptable oncological outcomes.</p>

ABSTRACT

Objective@#Reconstruction of defects after wide resection of tumor in perineal region is challenging because the defects are extensive and complex. In this report, we retrospectively analyzed the clinical experience of perineal reconstruction in last 6 years, to evaluate the value of clinical application of perforator flaps in defects of perineal region.@*Methods@#From January 2011 to December 2016, 32 cases of perineal cancer were treated by radical extensive excision, with defects repaired with perforator flaps. The size of perineal defects was between 8 cm×4 cm-20 cm× 20 cm, 52 perforator flaps were used for perineum reconstruction.@*Results@#All of the defects achieved tension-free closure. All the flaps were survived and healed with first intention except two infection and minor wound dehiscence cases. Both cases healing after dressing and secondary suture. There were no donor-site complications. During an average follow-up of 11.1 months (range, 10-12 months), the reconstructed areas achieved good functional and cosmetic outcomes.@*Conclusions@#Perineum reconstruction with perforator flaps following total radical extensive excision leads to good result in patients with perineal cancer. It can achieve tension-free closure and produces almost normal appearance and function of the perineum.

ABSTRACT

This study aimed at investigating the effects of Guttiferone K (GUTK),a polycyclic polyprenylated acylphloroglucinols (PPAPs) compound isolated from Garcinia yunnanensis,on the cell proliferation of human prostate cancer LNCaP cell line.Human prostate cancer LNCaP cells in the period of logarithmic phase were treated with GUTK.MTI assay was used to detect cell proliferation.Flow cytometry of propidium iodide (PI) staining,BrdU assay and immunocytochemistry were adopted to analyze the cell cycle phase distribution.Protein levels of Cyclin A,p27 and SKP2 were detected by western blotting.The results showed that GUTK inhibited the proliferation and induced G0/1 arrest of LNCaP cells in a time and dose dependent manner.The protein levels of Cyclin A and SKP2 were decreased,while p27 was increased by GUTK in human prostate cancer LNCaP cells.It was concluded that GUTK,a compound isolated from G.yunnanensis,presented the effect of inhibiting the cell proliferation by inducing G0/1 arrest of human prostate cancer LNCaP cells,with potential anti-prostate cancer action.

ABSTRACT

AIM:To compare the short-term and long-term curative effect between two implantation methods for treating refractory glaucoma.METHODS:A total of 150 patients with refractory glaucoma (150 eyes) were divided into Group A (treated by implantation with Ex-press glaucoma drainage device) and Group B (treated by implantation with Ahmed glaucoma valve) according to different implantation methods.Changes of intraocular pressure and anti-glaucoma drugs at different time points, the success rate of surgery, improvement of visual acuity and complications were compared between the two groups.RESULTS:The intraocular pressure of two groups at 1d, 1wk, 3, 6 and 12mo after surgery was significantly lower than that before surgery (P0.05).CONCLUSION:The effects of implantation with Ex-press glaucoma drainage device are similar to those of implantation with Ahmed glaucoma valve in the treatment of refractory glaucoma.However, the intraocular pressure control effect of the former is better than the latter, and used anti-glaucoma drugs are fewer.

ABSTRACT

Twenty-two items of national criteria of acupuncture-moxibustion operation standardization have been issued by the People’s Republic of China. To understand the situation of use of these criteria, the authors analyzed and studied the situation of application of acupuncture-moxibustion criteria in related papers, textbooks and monographs to provide a basis for further establishing and revising the criteria. The results show that people are building awareness of use of acupuncture-moxibustion criteria, and the criteria have gradually been used in more and more papers, textbooks and works.

It is found that making use of the standard is being realized in various types of articles and it is described in almost all the textbooks. Implementing the standard intensively promotes the standardization of acupuncture-moxibustion education and clinical manipulation as well as the academic exchange domestically and overseas. In turn,its scientificalness and authority are further strengthened.

ABSTRACT

<p><b>OBJECTIVE</b>To explore the correlation between propagated sensation along meridian (PSM) and TCM constitution at different age stages.</p><p><b>METHODS</b>According to age, 840 participants were divided into a youth group (326 cases), a middle aged group (243 cases) and an elderly group (271 cases). The TCM constitution of all the participants was evaluated, and the PSM test was performed. The distribution of TCM constitution, the occurring rate and transmission of PSM in each group were observed and compared; the correlation between PSM and the TCM constitution was preliminary investigated by Logistic regression analysis.</p><p><b>RESULTS</b>The distribution of nine types of TCM constitution in three groups:the proportion of normal constitution and partial constitution were significantly different (all<0.05); the occurring rate and transmission of PSM in three groups were not significantly different (all>0.05); the proportion of occurring rate for nine types of TCM constitutions in the whole population, from high to low, presented special intrinsic quality, neutral quality,-deficiency quality,-deficiency quality,-deficiency quality, damp-heat quality, phlegm-dampness quality,-stagnation quality and blood-stasis quality; besides, the proportion of occurring rate for different TCM constitutions in the youth group, middle aged group and elderly group was similar to that of whole population. The Logistic regression analysis results indicated the neutral quality (=0.025) and special intrinsic quality (=0.018) were positively while blood-stasis quality (=0.043) was negatively related with PSM in all subjects; the-deficiency quality (=0.025), phlegm-dampness quality (=0.019), blood-stasis quality (=0.012) and-stagnation quality (=0.035) were negatively related with PSM in youth group; the neutral quality (=0.001) was positively related with PSM inthe middle aged group; the neutral quality (=0.006) anddeficiency quality (=0.004) were positively related with PSM in the elderly group.</p><p><b>CONCLUSIONS</b>The occurrence of PSM in different age stages is related with TCM constitution, which could be increased in clinical treatment to improve acupuncture efficacy.</p>

ABSTRACT

<p><b>OBJECTIVE</b>To explore clinical effect of vacuum sealing drainage (VSD) combined with discontinuous windowing technique for repairing large area exposed wounds of Achilles tendon.</p><p><b>METHODS</b>From July 2009 to May 2014, 11 patients with large exposed wounds of Achilles tendon were treated, including 5 males and 6 females with an average age of 43 years old (aged from 7 to 65 years old). Among them, 4 cases were skin necrosis caused by heavy objects abrasion and contusion; 3 cases were caused by distal tibiofibula fractures; 3 cases were caused by bicycle-spoke injuries; 1 case was caused by diabetes. Areas of exposed Achilles tendon were from 6 cmx3 cm to 14 cmx5 cm without tendon rupture or bone exposed. After debridement, discontinuous fenestration on Achilles tendon was made by knife blade parallel with longitudinal axis of Achilles tendon, combined with Vacuum Sealing Drainage (VSD) treatment.</p><p><b>RESULTS</b>After drainage treatment with one VSD cycle (5 to 7 days), abundant fresh granulation tissues were growing on all wounds and survived well after the second phase dermatoplasty. All patients were followed up for 12 to 24 months, the color of skin flap was good, the texture was soft without burst. At 3 to 4 months after operation, subcutaneous fat was appeared under the flap, the skin was sliding, movement of ankle joints was good. No delayed Achilles tendon rupture were occurred.</p><p><b>CONCLUSION</b>Vacuum sealing drainage (VSD) combined with discontinuous fenestration is a simple, safe and effective method for repairing large area exposed wounds of Achilles tendon,which could minimize the secondary damage caused by wounds of skin flap grafting.</p>

Objective To analyze the correlation between the expression of phosphoinositide-3 kinase,catalytic subunit beta (PIK3CB) and the clinicopathologic characteristics in gastric adenocarcinoma tissues.Methods Immunohistochemistry was used to determine PIK3CB expression in human gastric adenocarcinoma tissues and matched adjacent tissues.The correlation between PIK3CB expression and the patients' clinicopathological characteristics and prognosis was statistically analyzed.Results PIK3CB is highly expressed in gastric adenocarcinoma tissues (positive rate 63.8%) compared with matched adjacent tissues (positive rate 45.7%) (x2 =9.139,P =0.003).PIK3CB expression is significantly correlated with tumor size (x2 =8.677,P =0.003),infiltration (x2 =9.306,P =0.025),and clinical stage (x2 =8.242,P =0.041) ; Level of PIK3CB expression has a significant impact on prognosis (x2 =25.644,P =0.000).Multivariate analyses showed that high PIK3CB expression is an independent poor prognostic factor for overall survival (HR =15.785,95% CI:1.943-7.805,P =0.000).Conclusions Upregulated PIK3CB is correlated with poor postoperative prognosis of gastric carcinoma.

The common evaluation tools that are publicly and extensively used for the evaluation of clinical trial literature quality in the world are analyzed, including Jadad scale, Consolidated Standards of Reporting Trials (CONSORT) statement and Grades of Recommendations Assessment, Development and Evaluation (GRADE) system and the others. Additionally, the present development, updates and applications of these tools are involved in analysis.

ABSTRACT

<p><b>OBJECTIVE</b>To investigate the impact of tumor size on the prognosis of patients with colon cancer.</p><p><b>METHODS</b>The clinicopathologic factors of 345 cases who underwent curative surgery for colon cancer from January 2004 to June 2008 in the Peking University Cancer Hospital were analyzed retrospectively. The optimal cut-off value was determined by receiver-operator characteristic (ROC) analysis.</p><p><b>RESULTS</b>The overall 3-year survival rate was 85.1%. The optimal cut-off value was found to be 5.0 cm. The survival rate of patients with tumor size ≥5 cm and tumor size <5 cm was 79.5% and 90.5% respectively, and the difference was statistically significant(P=0.005). Multivariate analysis showed that preoperative CEA level (P=0.004), tumor size(P=0.020), regional lymph node metastasis(P=0.000), and lymphovascular invasion(P=0.002) were independent prognostic factors. Tumor size was significantly associated with overall survival in univariate and multivariate analyses in stage III( disease (P=0.011), but not in stage II( disease(P=0.827) using 5 cm as a cut-off.</p><p><b>CONCLUSIONS</b>Tumor size of 5 cm is an independent prognostic parameter for patients with stage III( colon cancer but not for stage II(. Determination of the association between tumor size and survival should be base on TNM staging.</p>

ABSTRACT

<p><b>BACKGROUND</b>Although the first leading cause of death in China was malignant neoplasms (mortality, 374.1 per 100,000 person-years), the full impact of primary brain tumors (PBT) on the healthcare system is not completely described because there are a few well documented reports about the epidemiologic features of brain tumors. This study aimed to report a comprehensive assessment on the prevalence of PBT.</p><p><b>METHODS</b>A multicenter cross-sectional study on brain tumor (MCSBT) in China was initiated in five regional centers: Daqing (northeast), Puyang (north of China), Shiyan (center of China), Ma'anshan (center of China) and Shanghai (southeast). Prevalence rate was calculated by counting the number of people living with a PBT between October 1, 2005 and September 30, 2006 and dividing by the total population of the five communities at January 1, 2006. Estimates of prevalence were expressed as percentages and grouped according to gender and to age in fifteen-year categories. Within these strata, the rates were estimated with 95% confidence intervals (CI) using the accurate calculation of CI for Poisson distribution. A chi-square test was used to compare the various frequencies with α < 0.05. Age-standardized prevalence with the direct method was calculated with the ten-year age-specific prevalence and the age distribution of the Chinese population in 2010, obtained from World population prospects: the 2008 revision.</p><p><b>RESULTS</b>We estimated that the overall prevalence of PBT was 24.56 per 100,000 (95%CI, 14.85 to 34.27), and the overall prevalence of PBT in female population (30.57 per 100,000 and its 95%CI ranged from 19.73 to 41.41) was higher than that in male population (18.84 per 100,000 and its 95%CI ranged from 10.33 to 27.35). However, the discrepancy between genders was not statistically significant because the 95%CI overlapped. Of 272 cases of newly diagnosed PBT, the proportion of histological subtypes by age groups, gender was statistically different (χ(2) = 52.6510, P < 0.0001). More than half of all reported tumors (52.57%) were either gliomas or meningiomas. For the youngest (aged from 0 - 19) strata of the population, glioma appeared to occur more than other subtypes, accounting for 55.56% of all of cases. The majority of brain tumors presented in those aged from 20 to 59 years was pituitary adenomas (45.12%) and gliomas (31.10%). Opposed to brain tumors in adults and teenage, gliomas only accounted for 22.22%. Meanwhile, the median ages at diagnosis of the patients with PBT were similar between males and females except for pituitary adenomas (male: 59 years old; female: 45 years old).</p><p><b>CONCLUSIONS</b>Age standardized prevalence of PBT is 22.52 per 100,000 (95%CI, 13.22 to 31.82) for all populations, 17.64 per 100,000 (95%CI, 9.41 to 25.87) for men, and 27.94 per 100,000 (95%CI, 17.58 to 38.30) for women. Age standardization to China's 2010 population yielded an estimated population of 304 954 cases with PBT. Our prevalence estimates provide a conservative basis on which to plan health care services and to develop programmatic strategies for surviving. In the future, it would be helpful to have long-term observed survival rates that would make the assumptions and the resulting imprecision in the current estimates unnecessary.</p>
