ABSTRACT
Objective:To deepen understanding of IgG 4-related diseases (RDs), we analyzed the associated lymphocyte subtypes, and explored the pathogenesis and potential immunotherapeutic targets. Methods:Eighty-six patients with IgG 4-RDs were enrolled, and their clinical characteristics, peripheral lymphocyte subtypes, and disease course were analyzed. Results:The mean age of the participants was 36-87(62±11) years; 51 were male (59.3%) and 35 were women (40.7%); and 34.9% had a history of allergy. Follow-up lasted 4.8 (0.4, 14.1) months. The most common symptoms were abdominal pain, and submandibular gland and lacrimal gland swelling (each 20.9%). Sixty-five (75.6%) participants had multiple organ involvement, and the most frequently affected organs were the pancreas (52.3%), submandibular gland (51.2%), and lacrimal gland (34.9%). A high eosinophil count; high IgE, IgG, IgG 1, and IgG 4 concentrations; and low complement C3 and C4 concentrations were present in 18.8% (16/85), 30.0% (24/80), 72.9% (62/85), 58.3% (28/48), 89.5% (77/86), 61.2% (52/85), and 50.0% (42/84), respectively, of the participants. In addition, 64.7% (55/85) were positive for autoantibodies, and the most frequent was anti-nuclear antibody (63.5%). The proportion of CD4 +T lymphocytes increased in 25.7% (9/35) of the participants, which was accompanied by an increase in the ratio of CD4 +/CD8 +T lymphocytes (22.9%, 8/35). Importantly, most participants (90.0%, 18/20) had a high proportion of regulatory T (Treg) cells. High interleukin (IL)-2, IL-6, and IL-10 concentrations were present in 50.0% (11/22), 33.3% (10/30), and 16.7% (5/30), respectively, of the participants. Substantial lymphoplasmacytic infiltration, fibrosis, IgG 4-positive plasma cell infiltration, and lymphoid follicle hyperplasia or ectopic formation were present in 79.2% (42/53), 67.9%(36/53), 35.8%(19/53) and 30.2% (16/53), respectively, of the participants. Fifty-three participants with detailed pathologic data were also further evaluated, of whom 24.5% (13/53), 3.8% (2/53), and 67.9% (36/53) had definite, probable, and possible diagnoses; and 3.8% (2/53) could not be diagnosed. Compared with baseline, the percentage of eosinophils and the IgE, IgG, and IgG 4 concentrations decreased significantly; and the complement C3 and C4 concentrations had increased significantly after 6 months of treatment (all P<0.05). The IgG 4 concentration after 6 months of treatment negatively correlated with that of C4, and positively correlated with the baseline concentration of IgE and the IgG 4/IgG ratio. Conclusion:IgG 4-RDs are a group of diseases characterized by male predisposition; multiple organ involvement; a high eosinophil count; high IgE, IgG, IgG 1, and IgG 4 concentrations; and a low C3 concentration. Peripheral CD4 +T cells and Treg cells are also more abundant. The diseases can be controlled with glucocorticoids and immunosuppressive drugs in the majority of instances. The IgG 4 concentration after 6 months of treatment negatively correlates with the baseline complement C4 concentration and positively correlates with the IgE concentration and IgG 4/IgG ratio, which suggests that IgG 4/IgG, IgE, and complement should be closely monitored to evaluate disease activity and the efficacy of treatment in such patients.
ABSTRACT
A case of spinal cord abscess caused by Nocardia cyriacigeorgica is reported. The patient is an elderly man with a history of nephritic syndrome who presented with aggravating lower back pain and then gradually developed urinary retention, weakness and numbness in both lower extremities. Operative intervention was performed, and postoperative pathological findings suggested spinal cord abscess formation. Metagenomic next-generation sequencing of the cerebrospinal fluid identified Nocardia cyriacigeorgica as the causative pathogen. Although appropriate antibiotics were prescribed, the patient died 3 months later.

【Objective】 To investigate the effect of isoliquiritigenin on inflammatory response of vascular endothelial cells and whether the regulatory effect of isoliquiritigenin on inflammation is mediated by histone deacetylase 3 (HDAC3). 【Methods】 Human umbilical vein endothelial cells (HUVECs) were cultured in vitro and treated with LPS, different concentrations of isoliquiritigenin and HDAC3 specific inhibitor, respectively. Real-time PCR and Western blotting were used to detect the mRNA and protein expressions of inflammatory cytokines and HDAC3. Male C57BL/6J mice were randomly divided into vehicle group and isoliquiritigenin treatment group. The vascular inflammation model of C57BL/6J mice was established by ligation of the left carotid arteries. The mRNA expressions of inflammatory cytokines and HDAC3 in the carotid arteries of mice were detected by Real-time PCR. A molecular docking study was performed to investigate the interaction between isoliquiritigenin and HDAC3. 【Results】 Compared with the vehicle group, isoliquiritigenin reduced the mRNA expressions of inflammatory cytokines NLRP3, IL-1β, IL-18, MCP-1 and ICAM-1 and decreased the expression of HDAC3 mRNA and protein in HUVECs stimulated with LPS. In addition, isoliquiritigenin also decreased the mRNA expressions of NLRP3, IL-1β and HDAC3 in carotid arteries of ligated C57BL/6J mice. The docking of isoliquiritigenin in the active site of HDAC3 showed that isoliquiritigenin might act through HDAC3. Furthermore, HDAC3 specific inhibitor RGFP966 further promoted the inhibitory effect of isoliquiritigenin on the expression of inflammatory cytokines in vascular endothelial cells. 【Conclusion】 These results suggest that isoliquiritigenin suppresses the inflammatory response of vascular endothelial cells via HDAC3.

Histone deacetylases (HDACs) are a class of epigenetic modification enzymes and closely related to chromatin structure and gene transcriptional regulation. HDAC3 belongs to class I HDACs. It is reported that HDAC3 plays a key role in heart development. Recent studies find that HDAC3 plays an important regulatory role in cardiovascular diseases. This paper reviews the class I HDAC family HDAC3, focuses on its localization, enzyme activity and the research progress of HDAC3 in congenital heart disease, coronary atherosclerotic heart disease, cardiomyopathies, heart failure, and arrhythmias. This review may provide new drug target for the clinical treatment of cardiovascular diseases.
ABSTRACT
Objective To research of Huangong decoction combined with routine treatment of western medicine for the postpartum hemorrhage and fundus reduced height of the postpartum hemorrhage (PPH). Methods A total of 242 patients with postpartum hemorrhage were enrolled and divided into two group by random number table: the treatment group (121 patients) and the control group (121 patients). Patients in the control group were treated with the western medicine routine treatment, while the treatment group were treated with Huangong decoction plus routine treatment. Both groups were treated for 3 days with a follow-up of 42 d. The postpartum haemorrhage amount, fundus height reduction were compared. The clinical effect rate was evaluated with postpartum haemorrhage amount, palace at the uterine bottom of the height reduction. Results The total clinical effectiveness rate of the treatment group was 97.5% (118/121), and the control group was 90.1%(109/121). The difference of two groups was statistically significant (χ2=4.478, P=0.034). After production, the uterine bottom height reduction at the first day (9.3 ± 1.4 mm vs. 8.0 ± 1.3 mm, t=7.501), the second day (10.2 ± 1.2 mm vs. 9.1 ± 1.1 mm, t=7.447) and the third day (12.2 ± 1.5 mm vs. 10.1±1.4 mm, t=11.283) in the treatment group were significantly greater than the control group (P<0.01). The postpartum hemorrhage at the first day (732.1 ± 84.3 ml vs. 766.4 ± 90.0 ml, t=-3.066), the second day (480.1 ± 62.0 ml vs. 590.0 ± 74.3 ml, t=-12.513) and the third day (240.4 ± 36.3 ml vs. 340.3 ± 52.0 ml, t=-17.351) in the treatment group were significantly less than the control group (P<0.01). After postpartumt, the uterine volume (632.1 ± 101.2 mm3 vs. 701.4 ± 135.7 mm3, t=-5.095) at third day and (142.3 ± 16.5 mm3 vs. 160.6 ± 19.7 mm3, t=-7847) at the follow-up, in the treatment group were significantly less than the control group (P<0.01). Conclusions The Huangong decoction combined with routine treatment of western medicine can improve the clinical symptoms in patients with PPH, and contribute to the recovery of disease.

Purpose:It has been shown that the heart is a chaotic oscillator. So it is appropriiate to use the Lyapunov exponent, an important parameter to identify the nature of non-linear dynamical systems, for identifying the state of human heart. Methods:Preliminary results are obtained in this paper using Wolf's algorithm for 8 normal and 107 abnormal ECG recordings. Results:Significant differences are found between the Lyapunov exponents of normal ECG and ECG with obvious coronary stenosis (OCS), but there is no significant difference between the Lyapunov exponents of normal ECG and ECG with mild coronary stenosis (MCS);Significant differences are also found between the Lyapunov exponents of R-R interval series of normal ECG、ECG with MCS and ECG with OCS. Conclusions:It is apparent that the R-R interval series can give us more messages about human heart, and the Lyapunov exponents of ECG and R-R interval series are the appropriate parameters for the identification of the physiological states of human heart. It is possible to use Lyapunov exponent for early diagnosis of Coronary Heart Disease.
